Although researchers have struggled to replicate Heinrich’s safety triangle findings (Heinrich 1931) for various reasons (e.g., no access to his original database and it is unclear if the database was of a single establishment/operation or for an entire industry), the occupational safety community has continued to adopt and expand the applications of this theory (e.g., McSween 2003). Within the mining industry, the potential exists for operations and companies to use concepts from the safety triangle as a way to inform or challenge the practices employed within their health and safety management systems (HSMS) to prevent incidents (Backlund 2016). This paper considers data obtained from the Mine Safety and Health Administration’s databases and demonstrates the validity of applying the safety triangle theory at the mine level. The results support the use of this theory and demonstrate that lower severity incidents can predict fatalities in a subsequent year where significant nuances and caveats to applying the theory are identified and discussed.

Ambient electromagnetic (EM) noise (natural or manmade) is a major limiting factor for the design and operation of many electronic devices, including through-the-earth (TTE) communications and tracking systems. Extensive studies on EM noise were conducted by the US Bureau of Mines in the 1970s and 1980s. Changes in the design and operation of electrical equipment since then, however, have resulted in changes in the EM noise environment. This paper reviews past research on surveying EM noise in the extremely low-frequency (ELF) and very-low-frequency (VLF) bands, which are the operating bands for TTE systems, and introduces a new battery-powered, rugged, portable measurement system for surveying EM noise in mining environments. Examples of surveyed EM noise measurement results in and above an active underground coal mine, using the new system, are presented in terms of absolute magnetic field strength levels. A new metric, average noise level (ANL), is introduced to characterize the noise level relative to a particular frequency band for a specific location. It is found that EM noise in mining environments varies significantly with time and location. EM noise levels at frequencies of 30 Hz that shifted from power harmonics (e.g., 90, 150, 210, 270 Hz) are relatively low and their variation with time can be statistically modeled by a Gaussian distribution. The conclusions and findings presented in this paper can help better design and operate TTE communications and tracking systems in underground mines.

In 2003, a high-temperature pressure leaching circuit to treat copper sulfide concentrates was commissioned at the copper/molybdenum operation at Bagdad, AZ. In late 2007, a medium-temperature pressure leaching circuit was commissioned at the Morenci mine in Arizona. The Morenci circuit was shut down in mid-2008, following a fire in the autoclave circuit. The onset of the global financial crisis in late 2008 resulted in significant changes to the operating profile in the Southwestern United States with respect to concentrates production, concentrates treatment, acid production and acid consumption. As a result of these changes, the Morenci concentrator was shut down, and the pressure leaching circuit remained offline. In late 2014, the increasing acid requirements in the Southwestern United States prompted a reevaluation of the Morenci concentrate leaching facilities. As a result, the pressure leaching circuit was restarted in high-temperature (200 °C) mode in May 2015 and has operated successfully for three years since recommissioning. This paper describes the current pressure leaching circuit configuration and the factors leading to the successful application of copper sulfide concentrate leaching at this location.

ABSTRACT Health and safety indicators help mine sites predict the likelihood of an event, advance initiatives to control risks, and track progress. Although useful to encourage organizations to work together, executing such risk assessments comes with challenges. This paper focuses on one trona mine’s experience in the development and implementation of a field level risk assessment program to help its entire organization understand and manage risk to an acceptable level. Through a transformational process of ongoing support and communication, Solvay Green River fostered a culture of trust and safety grounded in risk assessment, safety interactions, and hazard correction. The application of consistent risk assessment (RA) tools was critical to create a participatory workforce that not only talks about safety, but actively identifies energies that contribute to hazards and potential incidents. In this paper, reflecting on the mine’s process of RA implementation provides examples of likely barriers that sites may encounter when trying to document and manage risks. As discussed, consistent efforts to establish a participatory risk management program are worth the initial struggle to ultimately reduce incidents. INTRODUCTION Work-related health and safety (H&S) incidents often account for lost days on the job, contributing to organizational/financial and personal/social burdens [1-2]. Accompanying research demonstrates that risk and ambiguity around risk contribute to almost every decision that individuals make throughout the day [3-4]. In response, understanding individual attitudes toward risk has been unquestionably linked to predicting H&S behavior [5]. Although an obvious need exists to identify more comprehensive methods to assess and mitigate potential hazards, some argue that risk management is not given adequate attention in occupational H&S [6]. Additionally, research suggests that a current lack of knowledge, skills, and motivation are primary barriers to mitigating workplace risks [3-6]. Therefore, enhancing knowledge and awareness around risk-based decisions, including individuals’ abilities to understand, measure, and assign levels of risk to determine an appropriate response, is increasingly important in hazardous environments to predict and prevent incidents. This paper focuses on tailored approaches to field level risk assessment (FLRA), measurement, and common barriers to participating in such activities. The authors use a trona mine in Green River, Wyoming, as a case study to illustrate how the organization overcame widespread and site-specific challenges to implement a proactive risk management program. By discussing the mine’s tailored FLRA program, this case study contributes to the literature by first, providing examples of critical components that need to be in place to ensure successful risk management and second, context for continuous quality improvement that can foster a higher level of trust and empowerment on site."

The performance of a rotating fixed bed reactor in recovering copper from industrial waste solutions by cementation is studied under various conditions. The rotating fixed bed is made of stacked woven steel screens to take advantage of two features of screens: the ability to present large area per unit volume and their ability to promote high turbulence. Variables studied are initial CuSO4 concentration, bed rotation speed, mesh number of the screens and bed thickness. The presence of baffles was found to increase the rate of cementation. The data were correlated by a dimensionless mass transfer correlation suitable for the design and operation of large scale cementation reactors.

The primary grinding stage in the Vuonos concentrator comprises a rod mill in open circuit and a semi-autogeneous mill in closed, circuit with a hydrocyclone classifier. The instrumentation of the grinding circuit is displayed in Fig. 1. All the measurements have computer interfaces and all the control circuits have been implemented by the process control computer. The crushed ore feed to the rod mill is measured by a belt-weigher and controlled by a belt-feeder. The pulp density in the rod mill is stabilized by proportioning the waterfeed at the mill inlet to the crushed ore feed. The slurry level in the pump sump is measured by a pressure transducer and it is controlled by charging the speed of the direct-current drive pump.

Working with junior mining companies as a valuer, my experience with regulators is that simplicity works and the sales comparison method is the way to go. What I like about Mr. Ellis?s approach (as in his Table 7) is the integration of commodity prices with the timing of acquisitions and the implication of price changes in gold on the adjustment of individual project valuations. However, the sample gold-related appraisals in Ellis?s paper are pre-2008 transactions and readers should be made aware of the current market for such projects. Before moving on, to be noted is that the effective date of the Las Brisas appraisal (Ellis?s sample Appraisal 3) is Feb. 10, 2006; given that the subject value of Transaction 9, Choco 10 in Ellis?s Table 6 was listed as $353 million for a 95% interest in the con-cession, I am uncomfortable accepting that the acquisition was suddenly worth the adjusted value of $1,205 million (as given in Ellis?s Table 7). In some cases, illiquidity discounts can apply, and commodity-wise, in the one-month period between the acquisition in January 2006 and the adjustment to the Las Brisas conditions as of February 10, 2006, the indicated gold price change was only 41 cents/g ($12.60/oz); i.e., from $17.50/g to $17.91/g ($544.40/oz to $557/oz).

Phelps Dodge’s Candelaria copper-gold mine is located 20 km (12miles) south of Copiapó in northern Chile’s Atacama province (Region III) (Fig. 1). The Candelaria Mine is about 800 km (500miles) north of Santiago and 75 km (47 miles) east of the Pacific Ocean. Its elevation is 600 m (1,970 ft) above sea level. The Copiapó River Valley is 6 km (4 miles) northeast of the mine site. It is the only major source of water in this arid region. The average rainfall is 14mm/a (0.5 in./year). During 1995, the mine produced 521.4 kt (574,800 st) of copper concentrate containing 150 kt (165,000 st) of salable copper and 3 t (98,800 oz) of gold. The 1995 unit cash cost was less than 50 cents/lb of copper. This makes Candelaria one the world’s lowest cost copper producers.

Introduction The mining industry is perhaps best characterized by the numerous and abnormally high risks associated with the business of mining and by its most basic assets - ore deposits, people and technology. At the most fundamental level, it is the quality and effective utilization of these basic assets that determine a mining company's ability to compete efficiently in the international market place. The gold industry has received much attention in recent years and especially in recent months, for good reason. This commodity serves as an excellent platform for the subsequent discussion of these three basic assets and their role in mining's future. Facets of the gold industry The fate of this noble metal in recent times illustrates some of the various risks associated with the mining industry and the importance of high-quality assets, whether they be ore deposits, people or technologies. The following comments pertaining to the gold industry help place proper focus on subsequent discussions. All monetary amounts given are in US dollars. Throughout 1997, falling gold prices broke record after record: first hitting three-year lows, then a 12-year low in July and finally an 18-year low at year's end. From Jan. 1 through Dec. 31, 1997, the price of gold fell by more than $2.57/g ($80/oz), or some 22%. The decline in gold-mining profitability and stock prices was even more dramatic. At the beginning of 1997, the 20 largest gold producers in North America had a market capitalization of approximately $44.5 billion. At year's end, this market cap had fallen to $26.2 billion, a reduction of 41%. The total market cap of all the world's gold companies fell from $71 billion to $46 billion, a 35% drop (Serwer, 1998). Analyses suggest that, at a spot gold price of $9.65/g ($300/oz), only one, or possibly two, North American gold-producing companies can make money when break-even or "total costs of production" are considered. While strong hedge positions mitigate this situation in the short term, clearly, this is an industry in desperate straits. For example, write-downs for North American producers in 1997 were approximately $2.5 billion. Speculative short positions in 1997 were near historical record highs and generally exceeded long positions by factors of 9:1 or 10:1. Much of this is driven by some US commercial banks, hedge fund managers and commodity speculators who can significantly move gold prices sharply over short periods of time by selling gold they do not own in the expectation they can buy it back at a lower price before they have to deliver. These signifcant short positions are likely to continue as long as central banks are willing to sell or loan gold to financial institutions that in turn make the gold available to speculators who hope to capitalize on price fluctuations. The Far East accounts for approximately 26% of the annual demand for gold. Including India, the region accounts for 43% of total demand. More than 85% of the physical demand for gold is for investment jewelry in Asia and the Middle East. This demand is extraordinarily price sensitive. The fairly recent turmoil in Asian markets and economies, combined with currency devaluations throughout the region, have made gold more expensive (30% to 50% in local currencies) for those peoples most likely to treat gold as a safe investment haven. Higher interest rates also have reduced the amount of money they have to spend on consumer goods such as jewelry. All of these recent Asian events are deflationary in nature. FIPR's program seeks to improve surface mining and pipeline transportation while reducing the energy cost for pumping phosphate matrix. The development of a reliable on-line analyzer and a modern process control strategy will result in improved processing efficiency. Significant improvement in overall efficiency will also be realized as the industry adopts column flotation technology. Economic acidulation of high-Insol rock will allow the industry to adopt simplified flotation flowsheets thereby reducing operating costs. Various applications of phosphogypsum would save the industry millions of dollars in constructing and closing gypsum stacks.

The integrated grouting method, as developed in the USSR, can be applied to multi-purpose operations that in¬clude the construction of shafts, drifts, and tunnels. In the USSR it has been used extensively for primary and final grouting of underground excavations in mining engineering and civil engineering projects. It has been applied in projects where the primary objective was waste contain¬ment. It has also been used to control subsidence beneath buildings located over mined out openings and to eliminate the flow of ground water beneath and around dams. This chapter considers several industrial applications in the form of case histories. The word integrated means that grouting activities were integrated with other activities. 9.1 CASE HISTORIES OF GROUTING FRACTURED ROCK WHEN SINKING VERTICAL SHAFTS The integrated grouting method has been applied most widely in the sinking of vertical shafts in fractured, satu¬rated rock. As explained previously herein, grouting oper¬ations preferably are carried out from the ground surface where they are integrated into the schedule of the setting up of the shaft excavation and construction equipment. Such integration reduces significantly the length of time required for preparation of shaft construction and the time required to sink the shaft. The increased efficiency is achieved by the elimination of cementing operations from the working face upon the penetration of each aquifer. Concomitantly, labor and energy consumption during shaft sinking are minimized because the more complicated and labor-intensive work is carried out at the surface section. Table 9.1 presents the essential parameters for the pri¬mary grouting of saturated fractured rock carried out by the integrated method when sinking vertical shafts. 9.1.1 NAGOL'CHANSK MINE NO. 1-2, VENTILATION SHAFT NO. 1 The integrated method of grouting saturated fractured rock through holes drilled from the surface was employed for the first time during the sinking of vertical shaft No. 1 at the Nagol'chansk mine No. 1-2 in the Don Basin. The shaft had an inside diameter of 6 m. The grouting operations commenced by using cement grout with a density of 1.7 to 1.8 g/cm3. Clay-based grout with cement and other addi¬tives was used only for grouting the aquifers where cement grout proved to be ineffective. By the time of the grouting of ventilation shaft No. 1, hydrodynamic analytical methods were well in hand and a preliminary method for designing isolation curtains had been developed. A method for designing and drilling ori¬ented directional drillholes whose natural curvature re¬flected the characteristics of the fracturing had been devel¬oped. The majority of the necessary equipment had been developed. Clay-based grouts were considered ready for the first industrial application. The hydrogeologic environment at the Nagol'chansk mine No. 1-2 contained many prolific aquifers. According to monitored drillhole data, the total expected inflow into ventilation shaft No. 1 without grouting was 425 m3/hr. This large ground water inflow rate was supported by in¬formation obtained during the sinking of the main shaft and from the auxiliary shaft of the "Nagol'chansk" mine No. 1-2. The construction contractor at the site began sinking ventilation shaft No. 1 to a depth of 217 m by cement grouting from the working face of the shaft. As a result of the cement grouting operations, the inflow of water was reduced from 105.4 to 40 m3/hr over this 217 m interval. However the residual inflow rate caused the shaft to become functional at the depth of 217 m. At this point all grouting operations were transferred to SPETSTAMPONAZHGEOLOGIA (STG) in an effort to improve the operation. In order to carry out preliminary grouting from the sur¬face, seven drillholes were installed and designed for re¬ceiving grout. Drilling was implemented by the ZIF-1200A rig to the design depth of the shaft. The geometry of the principal fracture system that would be encountered by the shaft was estimated while breaking ground using informa¬tion from other shafts. The drillholes were arranged so that they cut through the aquifers uniformly around the shaft. Some preference was given to intercepting the aquifers that were located along the rock dip above the shaft bottom because it was established that the ground water in the aqui¬fers flowed downward along the dip into the cone of de¬pression of the shaft. Investigations in the drillholes using the DAU-3M flowmeter described in Chapter 3 showed that all of the nine aquifers were penetrated by the time a depth of 690 m had been reached. The flowmetric information showed that aquifer numbers [1 and 2], [4, 5, and 6], and [7 and 8] had practically identical hydrogeologic characteristics; so it was decided to connect them into combined stopes and inject the grout using five stopes instead of nine. Table 9.2 shows the results of the grout injection into the aquifers. The volume of grout pumped into the separate intervals through single holes varied from 204 to 15 m3. Although surface chemistry fundamentals of semi-soluble minerals have been studied by many researchers, detailed understanding of these systems is still incomplete. In-situ FT-IR/IRS techniques have recently been used to successfully describe the adsorption of carboxylates at semi-soluble mineral surfaces. However, the wetting characteristics of these adsorbed films require further consideration. In this regard, the hydrophobicity and stability of transferred Langmuir-Blodgett (LB) monolayers of fatty acids at fluorite and calcite surfaces has been studied by contact angle measurements with polar and apolar liquids. Generally, it was found that the transferred LB monolayers of fatty acids at a calcite surface are unstable whereas such monolayers transferred at a fluorite surface are stable, as revealed from advancing and receding contact angle measurements. These results are believed to be due to the incomplete reaction of fatty acid monolayer at the calcite surface. In addition it was found that a closely packed well-ordered stearate monolayer with good hydrophobicity, equivalent to that of a transferred LB monolayer, can be formed at fluorite surfaces by spontaneous adsorption and self-assembly from aqueous solutions.

Relax: the world today is not running short of many mineral commodities, even though prices are spiraling upward. Einstein?s observation still holds true that everything is still relative, as long as one can pass on the incremental costs of production to the consumer. However, a strong consensus exists among many of the companies comprising the minerals processing industry that it is becoming increasingly difficult to find the easy-to-mine, high-quality resources that make juicy profit margins, which shareholders like. For the most part, the resource cherries have all been picked, and established mineral processing companies that are exhausting their reserves, or in the process of looking to add reserves so as to create longevity for their company, have no recourse but to look long and hard at what were once considered the dregs of the earth. Even end users, such as steel companies, are now looking to become ?miners,? to establish captive iron ore resources. Owning a captive resource facilitates a dependable supply of feed materials, which translates into effective cost control and a guarantee for a thriving sustainable business in the future.

"Rock dust is applied in underground coal mines to prevent the propagation of coal dust explosions. The application is conducted either dry or wet. Both methods have advantages and disadvantages however the formation of caked rock dust particles is cited as a significant concern for wet applications. A study was conducted to evaluate the effect of modifying the rock dust particle surfaces to repel water and each other by the addition of oleic acid (OA) and sodium oleate (NaOL). At a NaOL dosage of 2 lbs/ton, a contact angle of 112o was measured indicating strong surface hydrophobicity and ability to repel water. FTIR studies indicated a chemisorption mechanism which is necessary for long term stability of the surface modification. The dispersion rate of the modified rock dust samples was elevated by the application of both OA and NaOL thereby indicating a significant reduction or elimination of the detrimental effect of caking. Tests in an explosion chamber showed a 79% reduction in the dust explosion potential relative to untreated rock test applied by the wet technique. INTRODUCTION Mine explosions have been a long-time threat to underground coal miners attracting significant attention to the accumulations of combustible dusts and gasses. In the presence of sufficient oxygen and heat, the accumulations of combustible dust and gass can lead to large-scale underground coal mine explosions [Harris et al., 2010]. To alleviate the problem, rock dust such as limestone can serve both as a thermal inhibitor and a heat sink to inert the coal dust explosibility thereby prohibiting the occurrence of dust explosions [Richmond et al., 1975; Amyotte et al., 1995; Dastidar et al., 1997; Man et al., 2009]. The dispersibility of the rock dust is an important characteristic that determines the degree of explosion prevention [Greenwald et al., 1938; Mozumdar et al., 1974; Amyotte et al., 1995]. Unfortunately, the environmental conditions of an underground mine has the potential to cause caking of the rock dust which reduces the dispersive characteristics needed. The dispersive property of rock dust can be enhanced by modification of the particle surface properties by the application of chemical additives [Greenwald et al., 1938; Mozumdar et al., 1974; Spence, 2004; Vogt, 2011; Doidge et al. 2014]. The major mechanism for rock dust caking is the adsorption of water which forms liquid bridges at the particle-particle contact points where saturated solutions of soluble powder ingredients are present. Changes in temperature or relative humidity result in solvent (water) evaporation and the formation of dry bridges [Rumpf, 1958; Specht, 2006]. One method to prevent caking is creating hydrophobic rock dust to repel water from the solid surface thereby preventing the formation of liquid and solid bridges."

Surface dependent processes, such as froth flotation and flocculation, are important or potentially important techniques for combating some of the current major problems in coal preparation. In order to correctly apply or imporve a surface dependent process, it is of paramount importance to understand the interfacial phenomenon, especially the double layer properties, exhibited by the solid. The specific objective of this research was to determine the properties of the bituminous coal/liquid interface by an electrokinetic method, streaming potential, and to relate the findings, wherever possible, to the existing unit operations of froth flotation and flocculation. The electrokinetic properties of both the whole coal and its lithotypes were investigated. As part of the total investigation, the role played in the double layer by the reagents commonly employed in the surface dependent process was also established (1). These data will be presented at a latter date.

This is a brief state of the art paper describing current (1975) Uranium recovery plant flowsheets, some notes on system selection, design philosophies, and materials of construction. The in-situ, or borehole solution mining, of Uranium is a process which has been developed over recent years to better match Uranium mining and milling technology with the size, shape, grade and territorial distribution of the remaining identified Uranium deposits of western United States. Solution mining recovery plants are pure hydrometallurgical units ideally suited to automation, modular design, and mobility. Similar to conventional Uranium mills, two basic flowsheets, one acid and one carbonate, are in use, but the predominance is reversed so that the majority of plants employ the alkaline carbonate flowsheet, All plants incorporate resin bead ion exchange Uranium recovery for both acid and alkaline circuits.

Coal mining companies in Australia are managing the high-consequence risks across their operations more effectively by using an interactive online risk management system known as Riskgate Developed by the Minerals Industry Safety and Health Centre (MISHC) at the University of Queensland, Australia, Riskgate contains detailed information about 11 high-consequence risk areas (called topics in the system) for opencut and underground coal mines (Table 1). The existing topics are: fires; strata control for underground mining; ground control for opencut mining; tires; isolation; collisions; explosions and explosives; manual tasks; and trips, slips and falls. Every Riskgate topic is focused on coal industry activities (mining, processing, transport and storage). The scope includes mine sites, lease areas and mine infrastructure (mobile, fixed plant, field equipment, buildings, road and rail transport), and all aspects of the mine life cycle from design through to decommissioning. The topics uniformly recognize that a loss of control can result in injury or fatality, equipment damage, production loss, reputation loss and environmental damage. However, the priority focus for Riskgate is the safety of mine personnel.

This research focuses on the strata control issues in a Bord and Pillar depillaring working under the influence of dead load condition of voluminous and fragmented softcover of overburden dump material. It reports the modeling of a mine working through the FLAC 2D software under the Indian geo-mining conditions. The modeling results of an actual mine working under softcover were compared with that under the intact overburden condition to assess the influence of the soft overburden over the depillaring working. The modeling work involved progressive caving of the strata and the cyclic goaf filling following the modelsimulated occurrence of main fall and periodic caving. Mohr-Coulomb Failure criteria along with a simplified strain-softening (MCSS) model was used to study the failure and caving mechanism. Salamon’s compaction model was used to simulate compaction and resultant stress recovery in the goaf material. Comparison of the model findings was done in terms of load on supports, abutment stress, and convergence, apart from the stress redistribution and failure mechanism. The load transfer mechanism of the soft overburden was also studied for an overall understanding of the strata behavior in such workings.

Bougainville Copper Limited operates a porphyry copper mine in Papua New Guinea. The waste dumps will contain 400 to 500 million tomes of rock at the end of nine life. The paper describes investigations carried out over a period of en years and highlights the problems associated with waste lump leaching in a tropical and seismic active region. Aspects discussed include mineralogy, chemical reaction, leaching rates, microbiological activity, dump stability, dump structures, effects of tropical conditions, seismic activity and topography.

Floating "fines" differs from floating "sands" because the fines have a much larger surface area compared to their mass. The forces that arise from this large specific surface the key to understanding the behavior and treatment of fine particles. Though the nature of these forces is not thoroughly understood, enough work has been done to give a qualitative picture of what is happening in a flotation cell. It is the purpose of this chapter to re-examine some of the more, recent theoretical work and-to indicate its importance.
