The dramatic rescue of nine Pennsylvania coal miners trapped nearly 90 m (300 ft) underground for77 hours probably would not have happened without the high technology equipment and improved mine rescue training techniques available today. By now, most everyone has heard the story of the accident that occurred at the Quecreek Mine near Somerset, PA. At about 9 pm on Wednesday, July 24, the miners were drilling coal in the room-and-pillar mine. Their drilling accidentally punched through to the adjacent, abandoned Saxman Mine. Old mine maps of the Saxman Mine led Black Wolf Coal, Quecreek’s owners, to believe they were between 60 to 90 m (200 to 300 m) away from the abandoned mine. Those maps proved to be inaccurate.

During the last 20 years, more than 80 km (50 miles) of tunnels have been constructed in the Los Angeles, CA area. Recent projects include The East Central Interceptor Sewer (ECIS), completed in 2004. It spans more than 18 km (11 miles). The Northeast Interceptor Sewer (NEIS), completed in 2005, spans 11 km (7 miles). And the 2-km (1.3-mile) Metropolitan Transportation Authority?s Gold Line Eastside Extension (MCGLEE) was completed in 2006. All of the tunnel routes lay beneath heavily populated urban environments so minimizing ground surface settlement was critical to the success of the projects. Previous tunneling efforts in the Los Angeles area using open shield methods resulted in significant settlement that damaged roads, utilities, buildings and other structures. Earth pressure balance (EPB), or slurry machines, were specified for these projects to reduce subsidence risk. The contractors chose EPB tunnel boring machines (TBM) for the work. The closed and pressured face of the EPB TBM reduces loss of ground at the tunnel face and, when the machines are used in combination with gasketed pre-cast tunnel liners and backfill grout behind the installed segments, losses from over-cut of the excavated surface are also reduced.

Corrosion of steel roof support systems can cause potential premature failure of the support, thus adversely affecting the excavation stability and rock-related safety. An in-house corrosion test system was designed and developed to test full-length roof bolts for material performance based on tensile load capacity and strength under different corrosive conditions found underground. The roof bolts tested are also used to identify the possible corrosion mechanisms that influenced the bolts’ performance over time. Initially, the most commonly used bolt material in the USA, the ASTM A615 grade 60 steel roof bolts, was tested with and without stresses in a simulated underground coal mining environment for approximately 6 months, and the results are discussed. Localized corrosion forms such as crevice and pitting were mainly observed after testing. The stressed and unstressed roof bolts with and without threads that were tested in alkaline coal mine corrosive environment for 6 months passed their load-bearing capacity as per ASTM F432 test standards. From the results, it is concluded that this test protocol could be applicable to test the material performance of different roof bolt metallurgies and coatings with desired modifications to the proposed methodology for the specific underground mine environment. This paper mainly discusses the test system and protocol and shortcomings of the system and makes recommendations based on the observations and results.

The seepage forces generated around pressurized water tunnels in rock are investigated using the analysis tools of geomechanics. The theoretical background of the two-phase material behavior is explained and the equations of motion and continuity are presented. Applications on two case studies typically occurring in practice deal with the required minimum rock cover for a pressurized tunnel and the separation between two tunnels carrying different heads. The rock mass behavior as described by the Mohr-Coulomb and Hoek-Brown material models is investigated by numerical analyses and the results are compared. The effect of key material parameters on the safety factors and the associated modes of failure is discussed.

In spite of a global economy that continues to seesaw and a U.S. Congress that is at odds with the president and themselves, cities around the world are still spending billions on their infrastructure. And that bodes well for the underground construction and tunneling industry and the professionals who work in it. In June, 1,405 of those professionals gathered in San Francisco, CA for the Rapid Excavation and Tunneling Conference (RETC). In addition to the first rate technical programming, attendees were able to see first-hand the latest in tunneling technology from 127 exhibitors who occupied 152 booths. Two short courses preceded the conference and two field trips were offered following the conference.

Introduction Hydrometallurgical solvent extraction (SX) processes – featuring large quantities of combustible or flammable organic solvents - are common in the mining industry for recovering non-ferrous metals such as copper, nickel, cobalt, uranium, tungsten, iodine and lithium. Recent fire loss incidents and the widespread use of sub-grade tank farms and piping trenches, plastic piping, poor drainage, and closely spaced buildings demonstrate opportunities to improve the use of Inherent Safety (IS) in this industry. The mining industry has designed and constructed facilities with an emphasis on personnel safety, production efficiency and cost effectiveness. The use of IS to eliminate or minimize property fire exposures has not seen widespread practice. Lessons learned from the mainstream CPI have not been communicated or applied. As an example, copper SX plants have consistently used design of gravity flow as a cost and production-effective solution for transferring liquids and minerals through the process. In fact, Jergensen1 recommends to “utilize gravity to the fullest extent possible” in design of SX plants. This philosophy has been achieved by laying out SX plants with process equipment at different grade levels so that solutions can flow “downhill”. Because of the use of large quantities of flammable liquids and the potential for these liquids - if released – to flow unimpeded into other areas – gravity SX processes represent severe inherent fire consequences unless costly barriers, drainage and channeling are also provided. It appears that little thought has been given to the fire protection consequences and challenges of this design feature during the evolution of SX plants and most copper SX plants constructed today use some form of a sub-grade processing area (tank farm) and one-way gravity flow. While this layout is used to assure a cost effective operation it has resulted in significant exposures to high value production. The use of sub-grade production units is an inherently unsafe layout where flammable liquids are used and is a layout that would rarely if ever be used today by the mainstream CPI. Gravity-assist layout is only one example where copper SX plants have been designed and constructed without IS in mind. The use of combustible and frangible corrosion-resistant materials – such as glass, wood and plastic - for flammable liquid storage, processing and piping systems has significantly increased the hazards of these facilities. These materials of construction are not rigorous and allow for rapid failure and release of flammable contents during fire exposure. The use of combustible or frangible systems to handle or transport flammable liquids is not a practice seen today in the CPI.

Surface subsidence induced by underground coal mining activities, especially longwall mining operations, can cause various problems to interstate highways ranging from structural integrity, drivability to safety. Accurate subsidence prediction, correct influence assessments, and effective mitigation measures are the keys to ensure the safety of highway traffic. The surface subsidence prediction program, called comprehensive and integrated subsidence prediction model (CISPM), was developed based on the principle of influence function method and has been validated with a large number of subsidence measurements. In this paper, the techniques to use the predicted final surface movements and deformations for assessing their influence on the integrity and functionality of highway structures (e.g., road surface pavements, concrete slabs, transverse joints, etc.), vehicle dynamics, comfort level, and drivability are presented. A number of mitigation measures to protect the highway structures and to ensure traffic safety are proposed along with an application case.

The identification of the major subsidence areas in the Eastern Kentucky Coal Field has been made to distinguish the subsidence phenomena on the basis of geographical location. Further, this identification process is based upon selected critical geological, topographical, and mining features in the coal fields. The proposed identification of the regions is derived from a comparison of the subsidence potential in a particular area. The most likely severe subsidence areas are compared to those with the probable low subsidence severity. The combination of certain geological and mining parameters involved allows for the derivation of a classification of the subsidence potential.

The Docklands Light Railway (DLR) Extension to Woolwich Arsenal provides a much-needed public transport link to this area of London. The project includes construction of 5.3mID running tunnels under the River Thames, in ground conditions that proved challenging for the Earth Pressure Balance Tunnel Boring Machine (TBM). The project also includes cut and cover tunnel approaches, an underground station, and an emergency shaft on the bank of the river. Major challenges have included tunneling under sensitive infrastructure, and working adjacent to existing operational railways at Woolwich Arsenal. This paper will describe the design and construction of the project and how these challenges were overcome.

Between 1989 and 2004 the Massachusetts Water Resources Authority (MWRA)constructed several major wastewater and water tunnels associated with three major projects in the Boston area of eastern Massachusetts. There were many similarities and differences in the bedrock conditions encountered and among the contractual approaches for these independent projects. Through a clarification of the approach similarities and analysis of the contract differences, this paper addresses and presents what are considered to be valuable lessons learned. In doing so, the paper follows the evolution of such disparate issues as contract conditions, baseline reports and dispute review boards. The authors believe that the lessons learned even represent what, in certain cases, could be referred to as “universal truths” that are intended to have application to the next generation of tunneling contracts.

Portland, Oregon’s $370 million West Side Combined Sewer Overflow Tunnel Project has completed the final phase of construction and commissioning. The construction contract, which began in September 2002, was a reimbursable cost plus fixed fee contract that followed a qualifications-based contractor procurement process and apre-construction planning phase. This paper addresses the final results of the contracting method and provides a lessons-learned case history. Specific items addressed include the benefits realized from the contracting method, areas of improvement, final costs compared to the original contract estimates, design changes and schedule impacts, and subcontract management issues.

The City of Portland’s Westside CSO Project is constructing a deep tunnel system to reduce combined sewer overflows (CSO) from entering the Willamette River. Several kilometers of microtunneled pipelines connect the existing system to the new tunnel. The alignments encountered a variety of ground conditions, including fill, manmade obstructions (wood, steel piles, debris), fine-grained to dense coarse-grained materials, cobbles and boulders, all below groundwater. In recent years, tunnelling activities in Singapore have increased considerably with the construction of extensions to the existing sub-surface transport system, a deep tunnel sewerage disposal system, and tunnels for the laying power cables and other utilities. These tunnels are excavated through urban areas, adjacent to high-rise buildings, depressed road structures, beneath other tunnels, and buried services. Earth pressure balance (EPB) tunnel boring machines (TBM) have been used extensively in Singapore with great success. The risk associated with soft ground tunnelling, including tunnelling through Old Alluvium, can be minimized with an understanding of the geology, geotechnical parameters, and the tunnelling-induced behaviour of Old Alluvium (OA The purpose of this paper is to present some design and construction aspects of EPB TBM tunnelling through Old Alluvium.

In order to address the difficulty of accurate segmentation of froth images of different sizes, a method of froth image segmentation based on highlight correction and parameter adaptation is proposed. First, a machine vision system on a single-cell flotation machine is built to collect froth images. Homomorphic filtering is used to improve the uneven brightness and shadow of the images. Fuzzy c-means (FCM) clustering is then utilized to classify similar highlights that belong to the same froth. After Otsu threshold segmentation, a parameter-adaptive morphological operation is used to extract the marker points and edge bands and correct the froth edges in the original image. Finally, the modified image is filtered by morphological reconstruction, and the highlight mark is used as the local minimum point for watershed segmentation. Three sizes of froth images are segmented in comparative experiments. The results show that the proposed method is suitable for the segmentation of froth images of different sizes. The position of the extracted segmentation line is close to reality, with average over-segmentation and under-segmentation rates for froth images of 2.6% and 6.8%, respectively. The froth image segmentation performance is stronger than that of the other methods examined.

Amphiboles are hydrated mineral silicates five of which occur in asbestiform habits as asbestos grunerite (amosite) asbestos, riebeckite (crocidolite) asbestos, anthophyllite asbestos, tremolite asbestos and actinolite asbestos] and non-asbestiform habits (grunerite, riebeckite, anthophyllite, tremolite and actinolite). The asbestiform varieties are characterized by long, thin fibers while non-asbestiform varieties such as cleavage fragments form short fibers with larger widths. The U.S. regulatory method for counting asbestos fibers (aspect ratio [>]3:1, length [>]5 µm) does not distinguish between asbestos and cleavage fragments. The method biases toward increased counts of non-asbestiform cleavage fragments compared to long, thin asbestos fibers. One consequence of this regulatory approach is that workers can be erroneously classified as exposed to concentrations of asbestos (asbestiform amphiboles) above the U.S. 0.1 f/mL exposure standard when in fact they are not exposed to asbestos at all but non-asbestiform amphibole cleavage fragments. Another consequence is that the known carcinogenic effects of asbestos may be falsely attributed to non-asbestiform amphibole cleavage fragments of the same mineral. The purpose of this review is to assess whether amphibole cleavage fragments pose the same risk of lung cancer and mesothelioma characteristic of amphibole asbestos fibers. We identified three groups of workers exposed to non-asbestiform amphiboles: two groups exposed to grunerite (Homestake gold miners and taconite miners) and one group exposed to industrial talc containing non-asbestiform tremolite and anthophyllite in St. Lawrence County, NY. In addition to assessing strength of association and exposure?response trends in the non-asbestiform amphibole cohorts, comparisons were also made with cohorts exposed to the asbestiform counterpart (positive control) and cohorts exposed to the mineral (e.g. talc) that does not contain amphiboles (negative controls). The cohorts exposed to non-asbestiform amphiboles had no excesses of lung cancer or mesothelioma. Similar results were observed in the negative control groups, in stark contrast to the excess risks of asbestos-related disease found in the asbestos cohorts. The only possible exception is the twofold increased risk of lung cancer where exposure was to industrial talc containing cleavage fragments of tremolite and anthophyllite. However, this risk is not considered attributable to the talc or amphibole cleavage fragments for several reasons. A similar increased risk of lung cancer was found in Vermont talc workers, studied in the same time period. Their exposure was to relatively pure talc. There was no relationship between lung cancer mortality and exposure measured as mg/m3 years and years worked. A case?control study reported that all the lung cancer cases were smokers (or former smokers) and attributed the excess to smoking. There were two mesothelioma cases among the NY State talc workers exposed to cleavage fragments of tremolite and anthophyllite, but talc is not a plausible cause because of too short latency and potential for previous asbestos exposure. The positive controls of tremolite asbestos and anthophyllite asbestos exposed workers showed excess risks of both lung cancer and mesothelioma and positive exposure? response trends. St. Lawrence, NY talc does not produce mesotheliomas in animals while amphibole asbestos does. In sum, the weight of evidence fully supports a conclusion that non-asbestiform amphiboles do not increase the risk of lung cancer or mesothelioma. ©2008 Published by Elsevier Inc. The National Institute for Occupational Safety and Health (NIOSH), Spokane Research Laboratory (SRL), investigated a means to evaluate the potential impact of surface conditions above underground mines on emergency response activities. Recent events have highlighted the need to conduct critical emergency operations on the surface above underground work areas. Accessibility and usability of the surface, such as the ability to drill holes or deploy seismic monitoring or through the earth communications systems, are an important consideration for emergency response planning. This paper identifies critical surface conditions such as: surface waters, valley fill, highways, railroads and significant structures that must be considered in emergency response planning and in the conduct of emergency operations as they relate specifically to the seismic detection of trapped underground miners. Results of case studies indicate a need to consider the impact surface conditions may have on emergency response capabilities.

Heavy earth-moving machineries used in open-pit mines intensively expose the drill operators to whole-body vibration and shocks and thus to musculoskeletal disorders. The roles of an individual, machine, and rock-related factors in their exposure remain poorly understood. This cross-sectional study investigated their role among 39 drill operators from Indian iron ore mines. It shows that 70% of the operators were exposed to high levels of vibration, above the limit values recommended by ISO 2631-1. Multiple linear regression models showed that the whole-body vibration and shock exposures were strongly associated with operator’s age, drill machine model, rock’s hardness, uniaxial compressive strength, and density. The role of body mass index was close to significance (p = 0.08). Univariate analyses found that drill’s age, seat pad thickness, and seat backrest height were also strong predictors. These results help in identifying risky operators, materials and occupational situations, and in implementing appropriate prevention and intervention to reduce and monitor the exposures and health risk.
