Near misses recorded and reported by workers can provide awarenessto the potential causes of injury and prompt safety management initiatives. Although most companies require near-miss reporting, it is unclear what the value of these reports are, if any, and how they influence subsequent actions or controls to reduce on-the-job risks. Researchers at the National Institute for Occupational Safety and Health (NIOSH) conducted a case study with an aggregates company in which near-miss reports were analyzed at each of their locations over an entire quarter during the summer of 2018. Within that quarter, workers recorded 249 near misses. Of those, 167 were valid near misses that occurred at work. Researchers coded the reports using a qualitative 5 × 5 risk matrix. Of the 167 near misses, 19% were deemed low risk, 25% moderate risk, 30% high risk, and 26% critical risk. Several patterns in the near-miss incidents were documented, including classification of incidents and common corrective actions referenced (i.e., elimination/substitution, engineering control or redesign, work process/procedures, and personal protective equipment). The analysis provides insight into ways that risk communication and management programs can be improved to reengage workers and their situational awareness on the job.

In 1991, LTV Steel Mining Company decided to close an open pit taconite mine in northeastern Minnesota, using a passive treatment approach consisting of limiting infiltration into the stockpiles and wetland treatment to remove metals. Over 50 million metric tons of sulfide-containing waste had been stockpiled adjacent to the mine during its 30 years of operation. Drainage from the stockpiles contained elevated levels of copper, nickel, cobalt and zinc. Nickel is the major trace metal in the drainages, and prior to closure annual median concentrations ranged from 1.5-50 mg/L. Copper, cobalt and zinc are also present but are generally less than 5% of the nickel values. Median pH ranges from 5.0 to 7.5, but most of the stockpile drainages have pH greater than 6.5. Based on the chemical composition of each stockpile, a cover material was selected. The higher the potential a stockpile had to produce acid drainage, the lower the permeability of the capping material required. Covers ranged from overburden soil removed at the mine to a flexible plastic liner. Predictions of the reduction in infiltration ranged from 40% for the native soil to over 90% for the plastic liner. Five constructed wetlands have been installed since 1992, and have removed 60-90% of the nickel in the drainages. Total capital costs for all the infiltration reduction and wetlands exceeded $6.5 million, but maintenance costs are less than 1% of those for an active treatment plant. Since mine drainage problems can continue for over a hundred years, the lower annual operating costs should pay for the construction of the wetland treatment systems within seven years.

INTRODUCTION, The transalpine rail routes in Switzerland are well over one hundred years old. As the established routes no longer meet the demands of the continually increasing volumes of rail traffic between north and south, two new routes through the Alps were planed. The old Gotthard rail line is in fact a mountain railway. The northern and the southern access ramps?with a maximum speed of 80 km/h and a maximum slope of 2.2%?reach the old Gotthard rail tunnel at an elevation of approximately 1,100 m above sea level. This is approximately 900 m higher than the city of Milan. In 1992, the Swiss electorate people voted with an overwhelming majority in favor of the project to build the new base routes through the Alps. The Swiss voted again in 1998 in favor of the financing proposal for the new infrastructure. Swiss Federal Railways together with the BLS Lötschbergbahn were commissioned for the realization and management of the Gotthard and Lötschberg routes. AlpTransit Gotthard Ltd., founded by Swiss Federal Railways, was given the task of managing the design and realization of the Gotthard route until the start of regular service. Laboratory experiments were conducted to determine the responses of a prototype smoke detector and a commercially available photoelectric smoke detector to smoke particles generated from various combustion sources. The prototype smoke detector combines optical scattering measurements with ionization chamber measurements in order to reduce/eliminate nuisance alarms due to the presence of airborne dusts or diesel exhaust particles. The commercially available smoke detector is designed for use in harsh environments where airborne dust represents a major problem due both to nuisance alarms and detector contamination. In the experiments, the responses of the two detectors were measured when exposed to smoke particles from the exhaust of a diesel engine and from a variety of fire sources, including wood, coal, styrene butadiene rubber, and No. 2 diesel fuel. For the solid fuels, data were obtained for both smoldering and flaming combustion. This report describes the experiments, their results, and the use of these results as they apply to early-warning fire sensors capable of the rapid and reliable detection of fires in atmospheres that may or may not be contaminated by either airborne dust or the products produced from diesel engines. Subsequent to these laboratory experiments, six smoke detectors were installed in an underground limestone mine for further testing and evaluation. Deep-sea hydrothermal vents (hereafter chimneys) are commonly found on the new crust forming along divergent plate boundaries. Related to the chimney cycle from birth to death, it is hard to define the growth stage of the chimney due to a brief life span of the venting hydrothermal plume. Here we try to categorize the mineral magnetic properties of the chimneys according to their venting fluid-temperatures. Hydrothermal sulfide deposits in the Southwest Pacific were explored in 2012 and chimneys were collected using remotely operated vehicle (ROV) and Grab with AV camera (GTV). In the present study, mineral magnetic properties of three Cu-rich chimneys were measured: a high-temperature chimney showing a venting fluid-temperature of about 200°C (ROV01); a lowtemperature chimney of about 80°C (GTV01); and an inactive chimney (ROV02). The chimneys were sliced to ~1 cm intervals from core to outer margin. Magnetic properties of ROV01 are dominated by pyrrhotite, except for the most outer part that experienced severe oxidation. Concentration and grain-size of pyrrhotite are relatively low and fine respectively. In the GTV01, pyrrhotite, dominant in the core, is gradually mixed with hematite toward the outer margin of the chimney due to secondary oxidation. Both the concentration and grain-size of the magnetic minerals are greater compared to the GTV01. High concentration and large grain-size of magnetic minerals characterize ROV02. Dominant magnetic phases are pyrrhotite, hematite, and goethite. In particular, the most outer margin shows the presence of magnetite produced possibly by magnetotactic bacterial activity. Such distinctive contrast in magnetic concentration, grain-size, and mineralogy among three different types of chimneys enables the life stage of the Cu-rich chimneys to be determined through a mineral magnetic study.

Cable shovel requires continuous monitoring through excavation to achieve reliability, longevity and economic usage. The operating environment and the stress and deformation fields on the front-end assembly must continually be monitored to avoid random fatigue failure. In this paper, the authors advance frontiers of cable shovel reliability through real-time stress and deformation monitoring during excavation. Cable shovel boom stress and deformation analysis are developed using a combination of flexible and rigid body approach. A virtual prototype is developed to simulate the cable shovel excavation in oil sands and to examine the motion, stress and local deformation of the boom. A numerical example is used examine cable shovel boom durability using stress and deformation fields simulation. The results show that stress and deformation are large around the joint between the upper body and the boom. The simulation of the effect of material properties on boom deformation shows that the deformation is larger in hard formation than in soft formation. The study provides a solid foundation for further study of failure life analysis of the cable shovel components.

Because shape is an assessment of the three-dimensional form of a particle, it may be described in terms of sphericity (?), which is a measure of how closely a particle approaches a spherical configuration. In this study, Darcy?s law and the Kozeny-Carman model for fluid flow through porous media were applied to packed beds to determine the sphericity (?) of apatite particles. The beds were composed of glass spheres or particles of apatite (igneous from Brazil and sedimentary from the United States) of three classes of size (Class 1: -297 +210 µm; Class 2: -210 +149 µm; Class 3: -149 +105 µm). Glass spheres were used to validate the model because of its known sphericity (? = 1.00). Apatite particles, either igneous or sedimentary, showed very close values for particle sphericity (? ? 0.6). Observations on particle images conducted by scanning electron microscopy illustrated that igneous (? = 0.623) and sedimentary (? = 0.644) particles of apatite of Class 2 predominantly exhibit elongated shape. The close value of particle sphericity (? ? 0.6) showed by either igneous or sedimentary apatite may be justified by the similarity in particle shape.

Attending the Prospectors and Developers Association of Canada (PDAC) meeting and exhibit has become an annual tradition for many in the mining industry. Similar to last year’s synopsis, this report is an overview of the 2003 conference from the eyes of a consulting mineralogist. It is supplemented by conversations with other participants. And it examines the conference handouts. Certain segments of the mining industry have seen improvement throughout the year in terms of metal prices, stronger demand and renewed interest by the investment community. Other segments continue their decline due to depressed commodity prices, higher oil prices, a continued weakness in the global economy and war uncertainties. SARS was known to the world, but it was not a Toronto epidemic during PDAC. In short, this year’s PDAC was permeated by an air of anxiety about the Middle East.

Floor heave events in underground mining often happen suddenly and cause risk to workers and equipment. Reducing the risk of floor heave must be addressed in mining layout design. To improve that design one must understand the significant parameters that are correlated to the occurrence of floor heave. To accomplish that goal, researchers from the National Institute for Occupational Safety and Health (NIOSH) completed a numerical study using FLAC3D, taking advantage of its strain softening and ubiquitous joint capabilities to better simulate the floor heave and caving mechanisms involved at an actual mine site.

In the past two years recent increases in silver prices over levels that prevailed for many years have changed supply/demand patterns. On the supply side, a silver mining boom is in progress in the United States and elsewhere. On the demand side, traditional uses of silver by individuals are dropping while industry is conserving. These changes will alter the supply/demand balance and bring about more moderate rises in silver prices, which in turn will impact the mining industry.

The Hong Gai coal field in northeast Vietnam has been the main coal mining area of Vietnam for more than 150 years. Anthracite coal is produced from several open pit and underground mines situated close to the UNESCO world natural heritage site Ha Long Bay. The mining-induced environmental impacts such as dust emissions, acid mine drainage, and visual impairments due to waste dumps resulted in severe conflicts of interests regarding future land use and the protection of the world natural heritage site Ha Long Bay. Within the RAME project (Research Association Mining and Environment in Vietnam), Brenk Systemplanung is contributing to an improved environmental and sustainable planning in Vietnam's mining industry and the reduction of environmental impacts on Ha Long Bay by developing suitable and sustainable technology and planning measures (e.g. for dump construction/rehabilitation, predictive modelling, and/or monitoring) for Vietnam's leading coal producer.

The 5 km (16,194 lf) of 6.1m (20 ft) diameter TBM driven tunnel built by the J.F.Shea Co. will serve as sewage overflow storage. The TBM tunnel and four connecting tunnels excavated by drill and blast methods in hard granite will remain unlined. Based on the Geotechnical Baseline Report (GBR), the Contract documents provided a design with no required pattern of rock bolts, no probing, no pre-excavation or consolidation grouting, and lastly, no final lining of the tunnels was deemed necessary. This paper will provide a general overview of these topics with the emphasis on the actual ground conditions encountered versus the projected conditions provided in the GBR and how the GBR reflected the design of the project. Proposed modifications to the temporary and permanent ground support will be presented and the necessity of probing and pre-excavation grouting will be evaluated.

The mining industry in northern Ontario is known mostly for its huge nickel deposits. Inco’s and Falconbridge’s nickel mines in the Sudbury Basin are the best known. And with nickel prices being what they are, interest in the region continues. And how. Inco and Falconbridge want to merge. Teck Cominco wants Inco, but without Falconbridge. Xstrata made a hostile bid for Falconbridge, which Falconbridge rejected. And, in June, Phelps Dodge trumped them all when it made an approximately US$40-billion friendly offer for Inco and Falconbridge in a two-part deal.

The empirical rock failure criteria play an important role in predicting the mechanical behavior of rocks, since affecting parameters on the behavioral properties of rocks are complex and numerous. Usually the empirical failure criteria are obtained by statistical methods, namely by means of fitting the mathematical function onto the experimental data. For evaluation and verification of the results, however, the quality and accuracy of the fit must be considered. This paper studies the efficiency and accuracy of conventional empirical rock failure criteria using a new approach, which is based on the analysis of uncertainty amplitude on the constant parameters of each criterion. The approach adopted here, is based on the combination of some parameters such as Mean Squared Error (MSE), Root Mean Squared Error (RMSE), type of function, degree of freedom and the confidence level of constants. Correlation coefficient (R2) is then a comprehensive test and can be used effectively for comparison and accuracy of empirical rock failure criteria.

The opportunity to make a close study of the available geologic data existing along the line of the California Aqueduct crossing of the Garlock fault in the Tehachapi Mountains was brought about first by my involvement in the prebid examinations, and later with contractors' claim cases. The California Aqueduct crosses through the western end of the Tehachapi Mountains and cuts across the Garlock fault zone at a location about 9 miles east from its junction with the San Andreas fault zone. This is about 65 miles northwest of Los Angeles and 38 miles southeast of Bakersfield (Fig. 1) (1). This map was published after the tunnels were started and shows the corrected geology. At the crossing the fault has north and south branches which extend northeastward for about 22 miles before converging. In the vicinity immediately east of the tunnel line the north branch of the fault starts to converge toward the west. After crossing the line it passes under the upper plate of the apparent easterly extension of the Pastoria thrust fault, so the actual junction with the south branch is obscured. The aqueduct design called for the expected active slip zone of the north Garlock fault branch to be crossed by the 2,800 foot long Pastoria siphon, which connects with the north portal of Tehachapi Tunnel No. 3. The Arrowhead Tunnels Project represents the final portion of a 70 km (44-mile) long water conveyance facility that will bring up to 28 m3/sec (1,000 ft 3/sec) of water into Southern California. The 13 km (8-mile) tunnelling project is well underway and consists of two, 5.8 m (19-ft) diameter TBM bores through extremely variable geological conditions. An extensive and essential pre-excavation drilling and grouting program was performed in all reaches of the Arrowhead Tunnels Project, located north of San Bernardino, CA. This program supported the advance of two hybrid hard rock TBMs though extremely challenging ground conditions under substantial ground cover and up to 27 bar (900 ft) of ground water pressure. The well known San Andreas Fault lies nearby and has greatly influenced the local geological conditions. This paper will describe the means, methods and materials used throughout the numerous phases of the pre-excavation grouting program. It will also describe many versatile, flexible and innovative drilling and grouting methods and equipment systems utilized to improve performance, save time and costs in constantly changing geological conditions. Considerable lessons learned have been learned while overcoming some extremely formidable ground conditions and safely advancing the tunnels.

With both unit costs per meter and average depth of drilling constantly increasing, there is a strong incentive to maximize the information acquired from drill holes, and then to make this information accessible to the geological staff. The larger the drilling project, the more advantageous computerization of data is over manual methods. The conversion to computer-readable form and the verification of large amounts of drill-hole data can be a substantial data processing task, but once accomplished, the benefits in increased accuracy and speed of interpretation will compensate for the time and effort spent in the work. For an exploration project progressing towards a preliminary feasibility study, about half of both the time and costs for all computer work of the entire feasibility study are devoted to drill-hole data base organization, and maintenance. Once drill-hole results are quantified and verified to be correct, they are immediately accessible and are not subject to further transcription or reinterpretation of errors. To obtain maximum benefits from computer utilization in this area, the drill-hole data base should be set up along with availability of the first data. For some companies, this may initially comprise a large number of purely speculative drill holes on a spacing of perhaps 1 or 2 km. Although few of these drill holes may encounter economic material, thought should be given, at this time, to the variables which will be logged when such material is found. In this way, it should be possible to set up a consistent format into which all future drill-hole information will readily be accommodated. This format may even be carried over into the data base requirements for an operating property.

Climax Molybdenum Company’s Henderson Mine is a large panel caving molybdenum mine located about 43 miles west of Denver, Colorado. Three Joy M120-72-880 fans are installed in parallel on the No. 1 exhaust shaft. Throughout the mine’s history, ventilation changes such as the completion of the No. 5 exhaust shaft have decreased the utilization of these fans. To optimize fan performance they have been run with the full complement of 16 blades, down to as few as four blades. Since the completion of ventilation upgrades associated with the Henderson 2000 project, one surface fan has been operated under normal conditions with 10 or 12 blades. With the completion of a new network model of the Henderson ventilation system, a tool was built to develop fan curves for possible future fan configurations and predict future fan performance. This paper describes the tool and its use at Henderson.

Building indicators that respond to all stakeholders' needs is a time-consuming and complex exercise, with certain questions still unsolved. The aim of the current three-year research project, carried out in partnership between BRGM’s Mineral Resources Division and C3ED, is to propose a top-down / bottom-up indicators building methodology that combines conception and coordination methods and a participative approach. The tool-design phase is based partly on a comparative analysis of existing international initiatives according to different criteria (objective, observation scale, framework, etc.). The study of two African mining projects will provide the opportunity to confront the conceptual aspects with reality in the field. Careful consideration of the participants’ suggestions will result in a set of indicators that can be easily understood and adopted by all stakeholders. The final objective is to measure the impact of a mining site on a local, regional and national sustainable development frame, throughout the full life cycle of a mine.

Many techniques that are used to characterize and monitor microbial populations associated with sulfide mineral bioleaching require the cultivation of the organisms on solid or liquid media. Chemolithotrophic species, such as Acidithiobacillus ferrooxidans and Leptospirillum ferrooxidans, or thermophilic chemolithotrophs, such as Acidianus brierleyi and Sulfolobus solfataricus can grow quite slowly, requiring weeks to complete efforts to identify and quantify these microbes associated with bioleach samples. Real-time PCR (polymerase chain reaction) assays in which DNA targets are amplified in the presence of fluorescent oligonucleotide primers, allowing the monitoring and quantification of the amplification reactions as they progress, provide a means of rapidly detecting the presence of microbial species of interest, and their relative abundance in a sample. This presentation will describe the design and use of such assays to monitor acidophilic microbes in the environment and in bioleaching operations. These assays provide results within 2-3 hours, and can detect less than 100 individual microbial cells.
