To allow routine measuring of the exposure of underground workers to N02 from diesel engines and residual gases from blasting. The Approach: A miniature sampler absorbs NO2 which can be measured using a simple method adaptable to both multiple automatic analyses or individual on-site analysis. How It Works: The NO2 sampler is encased in a plastic tube approximately half the length of an ordinary ball point pen and clipped to an underground worker's hard hat. Removal of the end cap activates the personal sampler. Air from the working place enters the open end and diffuses at a controlled rate down the tube interior. The diffusion rate is dependent on tube dimensions (ratio of cross-sectional area to length). This report describes a process for quickly developing information that is useful for skills training. The process is called job training analysis (JTA). Its main use is to structure skills training at the jobsite. JTA supports structured skills training by identifying the job duties, tasks, and steps and the reasons why those job components are important. The outcome of the JTA process is a worksheet—a training outline based on the knowledge and expertise of experienced workers. Time invested in developing a JTA will result in significantly more time saved when it is used appropriately by skilled trainers. Like a roadmap, JTA saves time and energy. Experimental work at several mine sites helped document, refine, and validate the JTA process. JTAs for two mining jobs—roof bolting and lift truck operation—at two different mine sites are used to illustrate some of the concepts and the specific JTA process described in this report. The work at these two sites was influenced by cooperative work between the Mine Safety and Health Administration (MSHA) and the National Institute for Occupational Safety and Health (NIOSH) at several other mine sites and prior collaborative work with the U.S. Navy. The mining JTA process is grounded within the military research on the design of instructional systems. MSHA and NIOSH worked to simplify and apply the military training development model for mining industry use through collaborative work at several mine sites. The JTA process involves three activities: (1) planning, (2) a 1- to 3-day workshop where the JTA is developed using facilitated work with subject-matter experts, and (3) followup. Guidelines and considerations for each of these activities are included. Results indicate that the JTA process has been well received and valued by mining industry participants. It blends aspects of health and safety, production, maintenance, and crew coordination.

The aerospace and automotive industries are facing a significant risk for cumulative trauma disorders from high-repetition, long-duration tasks. Additional risk factors such as shocks, vibrations and sustained uncomfortable postures oftentimes contribute to musculoskeletal, neurological injuries associated with the hand-arm system. The power tools used for the operation reduce the comfort and working efficiency of the operators, thus lowering their health and safety and the quality of operation. This paper investigates the fastening operation on the assembly line of a major aerospace company for quantifying hand-arm vibrations with the objective of developing a dynamic model of the hand-arm system. The model will be used to ascertain the effect of the various risk factors on the quality of the operation. Experiment Setup A system of three tri-axial accelerometers is used to collect vibrations entering the hand-arm system and their positioning follows the ISO-5349 standard. Reaction force during the operation is measured using a force sensor. The posture of the hand-arm system is measured using two goniometers and one torsiometer. The pilot study conducted has two subjects and one pistol-grip power tool. The experiment setup and positions of various sensors is depicted in Figure 1. The U.S. Bureau of Mines has developed ultrasonic measurement instrumentation capable of measuring strains and elongations on tensioned and untensioned mine roof bolting systems. This report evaluates the pulsed-phase-lock-loop and pulse-echo systems. The pulse-echo systems measure time of flight of a signal through the bolt. The ultrasonic instrument converts the measurement into elongations or stress. The most recent Bureau instrument development converts travel-time measurements directly into elongation, load, stress, and torque by simple data manipulation from the keyboard. Bolt preparation and instrument calibration are described in detail.

During the past few years, the Pittsburgh Research Laboratory of the National Institute for Occupational Safety and Health (NIOSH) examined and characterized conditions at a majority of the underground stone mines in the United States. Observations at these mines revealed a limited degree of roof monitoring beyond visual inspection and sounding. When monitors are used, they typically require the miner to measure movement at the roof. If conditions are unstable, the mine rmay be in a hazardous situation while recording data. Based on this scenario, researchers surmised that a simple, inexpensive monitor with the capability of recording data at a distance from the mine roof would be a safer way to gain the information. Additionally, more widespread use of monitors could potentially lead to better understanding of roof movement in general. A monitor to meet this need was designed, tested, and subsequently improved as experience was gained in its’ use at a number of underground stone mines. The Roof Monitoring Safety System (RMSS) can provide an initial indication of movement in roof beams. By understanding and measuring roof movement in underground mines, the potential for injuries and fatalities to mine workers from falls of ground can be reduced. Also, officials at a mine with a history of data are better prepared to make a decision on remedial actions in the event of ground falls. This paper will outline the evolution of the RMSS and how it can be used in a comprehensive proactive ground control safety program. Also included is a case history describing how the RMSS was used in an evaluation of the effectiveness of a mechanical impact scaling machine at an operating limestone mine.

This paper describes mining and milling methods of the American Smelting & Refining Co. at the Morning mine in the Mullan area of the Coeur d'Alene mining region in Idaho and is one of a series of Bureau of Mines reports dealing with methods and costs of mining throughout the United States. The Morning mine has been described previously in a Bureau of Mines report published in 1930,2/ and the milling methods and costs at the Morning concentrator were presented in a Bureau of Mines report published in 1932. 3/ These reports are supplemented by this publication, which presents data to show the effect of changing times and conditions on a typical medium-size mining operation during the latter part of its history, when costs were rising and production was from increasingly greater depths. The report contrasts two periods in the history of the mine-periods not too far separated in years but considerably different in economic conditions. During the first period (1939) costs of labor and materials were moderate, mining operations were just being transferred from the main shaft to deeper workings opened from the offset shaft, and the operation prospered. During the second period (12 years later) production costs had increased following the national trend in all industry, mining was from greater depths near the bottom of the ore body, and the operation was becoming unprofitable. Mining on the lower levels of the property ceased in 1953.

This paper briefly describes the progress in mine safety in the United States, with emphasis on recent events that have followed since the explosion at the Sago Mine in January 2006. Legislation following the mine accidents last year and areas of work carried out by the National Institute for Occupational Safety and Health (NIOSH) related to the accidents are discussed. American coal production in 2006 was 1,150 million short tons. Most of this coal is consumed domestically, with annual exports of coal about 50 million tons. One third of annual production is produced at surface mines and the rest from underground. This coal is produced by about 82,000 miners working at 1,400 coal mines. Major improvements in coal mine safety have been made in the US since the Federal Mine Safety and Health Act of 1977. The fatal injury rate in underground coal mines between 1977 and 2004 was reduced by 47.8% to 0.036. The annual number of fatalities dropped from 112 to 14 during that period. Likewise, the non-fatal days-lost injury rate was reduced by 42.6% from 10.87 to 6.24. The improvements in mine safety during this period can be seen in the graph of underground coal mine explosion fatalities and injuries from 1980 to present, which is shown in Figure 1. Between 1984 and 1993 (9 year period) there were 5 explosions and one major mine fire, for a total of 55 fatalities. Over the seven year period from 1993 to 1999 there were no fatalities from coal mine fires and explosions. However, in the six years since 2000 there have been 5 explosions and one fire, which have caused 37 fatalities. [ ]

Research by the National Institute for Occupational Safety and Health (NIOSH) on multiple-seam mining has resulted in two new ground control products. Information Circular (IC) 9495 In some U.S. coalfields, particularly in central Appalachia and the West, most mines are operating above and/or beneath previously mined seams. The effects of multiple-seam interactions can include roof falls, rib spalling, and floor heave. These can seriously threaten the safety of miners and disrupt mining operations. Information Circular (IC) 9495 is a proceedings document containing a variety of papers that summarize recent knowledge about multiple-seam mining. A NIOSH study that analyzed case histories collected from 44 U.S. coal mines is described in detail. The study confirmed that overmining is much more difficult than undermining, and isolated remnant pillars cause more problems than gob-solid boundaries. The study's results were the basis for the Analysis of Multiple-Seam Stability (AMSS) software package. The proceedings also contain two papers describing the LaModel family of software. The original three-dimensional version of LaModel is essential for detailed analyses of complex multiple-seam scenarios. LaM2D implements a simplified, two-dimensional model suitable for quick approximations of the multiple-seam stresses and strains. Three papers in the proceedings focus on case histories. One describes the extensive multiple-seam experience at the Harris No. 1 Mine in West Virginia, a second contains examples of "extreme" multiple-seam mining from the central Appalachian coalfields, and a third describes interactions with regard to longwall mine experiences in Pennsylvania, West Virginia, and Utah. The final paper reports on a numerical modeling study that provided some insight into the mechanics of multiple-seam mining. [ ] AMSS Software Package NIOSH has developed a simple, user-friendly computer program called Analysis of Multiple-Seam Stability (AMSS). AMSS can help mine planners to evaluate each potential multiple-seam interaction and take steps to reduce the risk of ground control failure. AMSS begins by evaluating the pillar design. The purposes of the research were to assess the applicability of new accident investigation methods to mining accidents, and to adapt those found to be applicable in the development of a new methodology for use in mining. The development included field testing, evaluation and modification of the new methodology. The report is in two volumes. This, volume one, discusses primarily the Phase I work: background research, design of a methodology for field evaluation, and preliminary field tests in Utah mines. Phase I was completed in June, 1980. Also discussed are some activities to prepare for a field evaluation during the period May 1980 -October 1981. Volume 2 discusses the subsequent field tests, modifications of the initial methodology and the results of use of the methodology in two mining companies for a period of approximately eighteen months, and in other companies for selected loss incidents during that period.

The National Institute for Occupational Safety and Health (NIOSH), in partnership with the Mine Safety and Health Administration (MSHA), is conducting research to test, evaluate, improve or modify coal mine fire-fighting strategies and methodologies through large-scale tests. Since wood is the second most abundant fuel available during a coal mine fire, it was decided that series of large-scale wood crib fire tests were needed to measure the products of combustion and to observe the capability of commonly available direct application fire suppression equipment, namely, fire extinguishers, water and gas-enhanced foam. This paper discusses the wood crib fire tests, provides insight into the products of combustion and describes observations made during the application of the fire suppression agents.

THE TREND towards larger quantities of waste in materials handled at metal and nonmetal mines (excluding fuels) continued. A gain of 10 percent from 1962 in the tonnage of waste removed in mineral operations reflects the kind of technological problems being encountered. Technology has a major role in the mineral industry in the task of supplying raw materials obtained from resources occurring beneath the surface of the earth and used in manufacturing commodities essential to modern living. Changes in technology are often brought about by such factors as variations in economic conditions, differences in physical properties of raw material and changes in mineralogical compositions of ore. Mining is usually begun on the richest and most accessible of known deposits, and when these are exhausted, is continued with leaner ores or with less accessible deposits that are either lying at greater depths or are more remote from markets. The increasing difficulties of extraction and depletion of reserves are offset by improvements in extraction technology, improved transportation, and exploration for new deposits. Although depletion has prevailed at many individual mines and in many districts, technology has been the means of providing for increasing demands of an expanding economy with adequate supplies of minerals despite either the changing character or decreasing grade of certain ores. This has been accomplished by various methods applicable to minerals. Among the implements of technology are utilization of lower grade or more complex refractory ores by mass mining, concentration, processing, recovery of byproducts, and mechanization. Better methods to find concealed deposits or to penetrate to greater depths also have been helpful in maintaining a supply of particular minerals.

This report, sponsored by the U. S. Bureau of Mines, presents the results of an examination of the safety of the impactor shaft sinking system in comparison to conventional shaft sinking technology. Detailed study of the impactor system was also performed to identify and recommend design changes for optimum safety conditions. A comparison of the impactor machine and its support systems with conventional shaft sinking was made on the basis of relative risk levels. Risk incorporates measures of both the frequency and the potential consequence of accidents. A safety evaluation of the com¬ponent operations of the impactor shaft sinking system was performed using a Failure Modes and Effects Analysis (FMEA) methodology. The results of this analysis show generally that the impactor shaft sinking system is safer than conventional shaft sinking. The potential risk of conventional shaft sinking operations could be reduced by an average of over 35 percent with use of the impactor shaft sinking system. Design change recommendations to further improve the safety of the impactor system include a revised fire suppression system, improved access to and from the operator cabs, reliable noise attenuation measures and improved communication systems. An operational safety manual was also developed for the impactor shaft sinking system.

Because the cutting and polishing of stones are parts of the work of the Bureau of Mines in its study of rocks and minerals and in the determination of specimens sent in for identification, it was decided that a circular covering the subject in a general way would be of practical interest. As several books and periodicals give details of the problems in cutting and polishing, this report will not duplicate their information more than necessary. To initiate this project a critical evaluation of the pre-sent phosphate mining industry in the Western phosphate field was conducted. Mines were visited, mine operators were interviewed and pertinent literature studied. Three phosphate mining enterprises were reviewed and mining cost estimates prepared. The report discusses the present state of phosphate processing, rock alteration and location. New ideas and concepts are proposed to improve the recovery of phosphate rock from the Phosphoria formation. The report evaluates two variations to the present phosphate open pit mining methods, which were developed to improve the economics of mining phosphate and to better accomplish necessary reclamation and accelerate the time for environmental recovery. A material handling system using conveyor haulage will eliminate the need for trucks. Terrace haulback utilizes safety benches to haul and dispose of waste in the mined out pit. Ore will be removed from the pit via an inclined, cable-supported mobile conveyor.

THIS PUBLICATION is the third to be prepared as a result of a cooperative project by the Bureau of Ships and the Bureau of Mines for accumulating literature published on zirconium and hafnium. The undertaking also resulted in the establishment of an information center on zirconium and hafnium at the Bureau of Mines station at Albany, Oreg., for the use of research organizations and others. The present bibliography consists of references (with abstracts) to pertinent literature on zirconium. The first publication in this series was Information Circular 7771, Bibliography of Zirconium, published in April 1957. It consists of 969 abstracted reports and 645 U.S. and foreign patents. The second publication, called a supplement to Information Circular 7771, is numbered Information Circular 7830 and consists of 844 abstracted reports and 62 U.S. and foreign patents. It was published in October 1958.

Introduction This paper is a descriptive analysis of injuries and fatalities at small surface bituminous coalmining companies in Pennsylvania. The purpose of analyzing a specific industry sector and location is to identify issues that might be missed in a wider analysis. Other studies of small mine safety (e.g., Peters and Fotta, 1994, Randolph and Boldt, 1997) have shown that the relationship between safety and mine size is not a simple one. Some injury types occur at higher rates at small mines, while others occur at lower rates. Consequently, an analysis limited to a specific type of mining can help identify hazards and issues specific to that sector. The results of this analysis will be useful for directing training, intervention, and research efforts towards the most prevalent causal factors and incident types. Procedures and Definitions Data sources The primary sources of data for this analysis were files provided by MSHA containing information collected under Title 30 Code of Federal Regulations, Part 50. These files, commonly referred to as “Part 50 data”, are provided though MSHA’s Program Evaluation and Information Resources directorate. Two primary types of injuries were selected from the Part 50data: Fatalities (MSHA “degree of injury” code 1) and lost-time injuries, otherwise known as nonfatal with days lost (MSHA “degree of injury” codes 2 through 5). Lost-time injuries involve either actual days away from work or days of restricted activity or both. Fatality and lost-time injury incidence rates were calculated using the standard MSHA formula: Number of injuries per 200,000 employee-hours. Additional information on company sizes and mine ownership was provided by the Pennsylvania State University College of Earth and Mineral Sciences. Time span A five-year time span was selected for this study. More than a single year of safety experience is usually needed to capture a broad range of incident types. Many types of injuries, especially fatal injuries, can be extremely rare, and may be missed if too short a time span is selected. However, a time span that extends too far back may begin to suffer historical threats to validity. That is, incidents that occurred several years ago may be caused by hazards that have since been remedied or may be related to mining practices and conditions that no longer exist. A five-year span offers a practical compromise between these two concerns. Consequently, the MSHA Part 50 data files from 1993 through 1997 were used. The 1997 file was in “preliminary” form at the time of the study, so small variations from the final 1997 “close out” file can be expected. Based on past experience, we do not expect the close-out variations to have a significant impact on the findings.

To help insure the continued viability of the minerals and materials sector of the U.S. economy, the Bureau of Mines is conducting research and disseminating technology that will result in more efficient minerals extraction. As part of this effort, a process is being investigated that involves an aqueous chlorine-oxygen leaching procedure to treat complex sulfide concentrates to produce ZnCl2. Subsequent fused-salt electrolysis of the ZnCI2 produces high-purity zinc metal. Previous reports described preliminary, small-scale research on the chlorine-oxygen leaching system and fused-salt electrolysis of ZnCl2. This report discusses the details of design, construction, and longterm, continuous operation of both monopolar and bipolar fused-salt cells. The effects of temperature, electrode gap, current density, electrolyte composition, and electrode configuration on current efficiency and energy consumption were determined for a ZnCl2-KC1-NaCl electrolyte system. Zinc metal was produced at the rate of about 50 lb/day in a monopolar cell which was operated continuously for 60 days with current efficiencies from 85.8 to 98.6 pct and an average power consumption of 2.1 kwhr/lb. Bipolar cells with several different electrode configurations were operated for 90 days. A current efficiency of about 70 pct and power consumption of about 1.7 kwhr/lb of zinc were achieved using a three-electrode-pair, bipolar assembly with a chlorine gas lift pump to circulate electrolyte.

THE ANTHRACITE region of Pennsylvania contains over nine-tenths of the total anthracite deposits of North America and produces all but an insignificant proportion of the total output of anthracite in the Nation. The whole economy of the northeastern section of the United States depends, to a large extent, on the mining of Pennsylvania anthracite, which is shipped to New Jersey, New York, and New England and is a principal source of energy and heat for that entire region. The responsibility of the Federal Government for developing an engineering method that will prevent inundation of the Pennsylvania anthracite mines and keep them in operation has long been recognized. To obtain a reliable picture of the anthracite-mine-water problem and to solve that problem, it is necessary to have factual data on the volume and the quality and character of the mine water handled by the pumping plants in the anthracite region, as well as the kinds of pumps utilized and problems concerning their operation.

Background: Frictional ignitions continue to be a problem facing the U.S. underground coal mining industry. Many methane igni-tions are associated with the impact of mining machine cutter bits on quartzitic material (e.g., sandstone) during the coal-cutting process. Most ignitions result in small methane-air fireballs often only a few feet in size and limited to the cutter head location. However, under certain conditions, frictional ignitions can lead to larger methane explosions and/or fires with the potential for causing serious injury or death to the mining machine operator and other nearby miners. The requirements for a frictional ignition are a flammable volume of methane-air and an ignition source. This source is usually the hot streak caused by the frictional impact of the mining bit on hard rock, and the frequency of occurrence increases with increased bit wear. Another possible ignition source is the fric-tional impact of rock on rock during a roof fall.
