Organisations’ incident classification processes are plagued by the overclassification of events, often leading to excessive workloads and suboptimal outcomes. This presentation addresses the pressing need to rethink how incidents are classified to maximise organisational learning and minimise systemic risk. A major issue in the current incident classification landscape is the tendency to classify events at high levels, overwhelming organisations with investigations they cannot efficiently handle. This results in inadequate risk reduction and learning opportunities. The link between injury classification and investigation methods further compounds these problems, creating unnecessary administrative clutter. Challenging the misconception that every event, no matter how minor, requires a comprehensive investigation, this presentation advocates for a shift in perspective. The key recommendation is to decouple injury classification and risk matrices from the decision-making process regarding what and how to investigate. Instead, the emphasis should be placed on identifying opportunities for organisational learning and risk reduction. Organisations can streamline their incident management processes by adopting a more pragmatic approach that aligns incident classification with the organisation’s investigation capacity. This transformation focuses on achieving meaningful outcomes, enhancing safety and improving performance. This presentation provides an innovative process for organisations to revamp their incident classification practices, enabling them to harness the full potential of incident data for learning and risk mitigation. It highlights the importance of aligning incident classification with the opportunity for organisational learning, ultimately leading to more effective incident management and enhanced safety outcomes.

The Shamrock Gold Mine has been brought from exploration to production within three years. Mining of the openpit is scheduled for June 1990 with commissioning in April of the 180 000 TPA carbon-in-pulp mill on old tailings and rock dumps. The development of a small openpit containing 344 000 tonnes at 3.6 g/t Au in a complex sulphide ore has required extensive metallurgical testwork to optimise recovery, and ensure profitability. The project has been undertaken in a changing economic climate, with increasing restrictive legislation. The decision to mine was further complicated by the escalating cost of installing a heap leach operation in South East Queensland. Decreasing costs of consumables, in particular sodium cyanide, and the opportunity to purchase a suitable transportable mill at a reasonable price made the project robustly viable as a CIP operation.

With some notable exceptions, marine placers located just below, or just above, present sea level, are responsible for much of today's production of detrital ilmenite, rutile, zircon, and other minerals of value. The purpose of this paper is to discuss the paleogeographic and paleo-oceanographic factors which may have determined the location of presently submerged marine placers. These factors-include changes in sea level, burial by recent marine sediments, stream capture and other i changes in drainage patterns, and recent changes in climate. Selected geologic situations in the Yellow Sea off the coast of Korea, in the Atlantic Ocean off the coast of Portugal, in the Bering Sea off the coast of Alaska, and in the Arabian Sea off the coast of India, will be used to illustrate the author's point of view.

In today’s mining environment the use of heavy mechanical devices and other high-tech electronic devices like two-way radios, cellular phones and even automated mining systems, form a vital part of the daily operations of a mining site. These human-made devices and some of the naturally occurring events, like lightning and gamma radiation, can have negative effects on electronic detonators and the associated control equipment.On larger mining operations the necessity to move the point of safety for blasting further away from the blast area, introduced remote firing via radio frequencies for electronic detonator systems. Other influences on electronic detonators can be abuse or ignorance. To ensure the safety of the end-user, an electronic detonator must be resistant to some common mistakes that users can make. One of main concerns here is resistance to static discharge or incorrect wiring. Despite the daily use of electronic detonator solutions, scepticism and concern still exists amongst those who believe the system raises the risk of a premature initiation of the blast through some form of interference trigger.This paper will help users understand how electronic detonators and the associated control equipment protects itself from external influences. Focus will be on protection schemes to counter static and over-voltage discharge and methods to ensure protection of secure wireless radio links and detonator communications. With well engineered communication systems and solid static and over-voltage protection, an electronic detonator system can be more reliable and safer than other detonator systems. The following main points will be discussed:what makes a detonator an electronic detonator,static electricity and over-voltage protection,the general influence of electromagnetic energy on the detonator,the influence of lightning on detonators,the protection of radio communication’s links as used by remote firing equipment, andthe safety protection features built into remote firing equipment.

Bow-tie analysis is a commonly used tool in the Major Hazard Facility sector to identify, display and control risks that have the potential to pose serious harm to personnel. It is a simple but powerful risk management tool that displays traditional risk assessment outputs as easily interpreted ‘bow-tie diagrams’. These are an effective and versatile communication format that can be applied to any type of risk (eg safety, environmental, assets etc) and as such show great benefit in managing operational hazards in mining.

In this report,the results of the large diameter drilling tests in coal seams assisted with water jet are described. The stress relief in coal seams by this new method is as effective as by the conventional method and the penetration speed by this method is 1.5 times as fast as by the other method. 1 Introduction The average mining depth of Japanese coal mines has already attained about 700m from the surface. Almost all coal mines are suffered from occurrence of coal and gas outbursts. To prevent outbursts, some countermeasures are taken generally. In particular, large diameter stress relief borings have been carried out quite often at Akabira coal mine located in the northern part of the Ishikari coal field in Hokkaido,JAPAN. In the course of an investigation of the influence of bismuth on copper carried out some years ago, (1) the author found that phosphorus exerted a beneficial effect on bismuth-copper alloys of low bismuth and oxygen content by neutralizing the embrittling effect of the bismuth. In the intervening years, a further examination of bismuth-phosphorus-copper alloys has been canied out and the influence of other elements and combinations of elements on copper containing bismuth has been studied.The results recorded in the present paper are part of this investigation.MATERIALSAll the alloys were made from high-grade copper (KS.A. wire-bar) of the following composition: copper 99·95%, oxygen 0,040'%, iron 0·0015%, sulphur less than 0-001%, bismuth less than (j>'OOl%, other elements each less than O· 0010/<-. The bar was hot rolled and cold drawn to strip which was cut into short lengths and cleaned in nitric acid ready for melting.A piece of this 'strip was drawn to wire 0·08 in. diam., annealed at 5000 C, and measured for electrical conductivity as a check on purity. The result was 101·5% of the I.E'.C. standard.In making the alloys, phosphorus was added as 14% phosphor-copper, bismuth as 17% bismuth-copper, and the other constituents either as copper alloys or as elements of a high degree of purity.PREPARATIQN OF STRIPSThe charges were melted in plumbago crucibles (Morgan salamander brand) in an oil-fired furnace, Le;, in the manner commonly adopted in industrial practice. The copper was first melted under charcoal, well stirred with a chaned stick, and the necessary alloying elements added and stirred in with the stick. The weight of a charge varied according to requirements but generally was about 4 lb. The charges were poured into slightly tapered brass moulds heated to about 1000 C and dressed with a mixture of mineral oil and tallow. They were not only poured under charcoal through the burning mould dressing but also- through a ring of flame produced by burning kerosene on asbestos cord wrapped round the top of the mould. Experiments carried out with plain copper proved that by this means oxygen could be excluded from the stream of metal. The billets were 7·5 in. long x O·83 in. diam. x 1·3 lb. weight.The billets were cropped about 1 in. from the top, cleaned in nitric acid, and measured for density by the usual method of weighing in air and -in water. As these measurements were required only as a check on casting, technique a high degree of accuracy was not essential (accuracy = -+ 0·2%).

The Mutooroo copper mine is one of a number of deposits that have been evaluated by Adelaide and Wallaroo Fertilizers Ltd as a source of feed for the production of high- purity copper oxide at their Burra plant. For this deposit, the technique of solution mining or in-situ leaching appeared ideal. The Mutooroo production circuit and the results of the process optimization and environmental monitoring programmes are outlined. At the end of the experimental period described here, the production liquor typically contained I g/litre copper, yield- ing in excess of 5 t/month of cement product containing about 80 per cent copper. The environmental monitoring programme provided clear evidence that there had been no un- acceptable alteration to the properties of the groundwater in the country rock surround- ing the mineralized zone. It is anticipated that the experience gained at Mutooroo will be of considerable help in the development of other solution mining operations.

The enrichment of magnetite to haematite and haematite-goethite leads to a coincident magnetic low and gravity high within the banded iron formation. Palaeochannel deposits of pisolitic limonite can be expected to provide a relative magnetic high and gravity low in a sinuous geometry. Detrital iron deposits of conglomerate, scree or ‘canga’ might have indistinct magnetic response but will usually have a relative gravity high.These signatures provide clear discriminators detectable by a joint gravity and magnetic survey.The combination of airborne gravity gradiometry and magnetometry is particularly powerful in detecting and discriminating targets, as well as mapping lithologies and structure, across a wide variety of iron ore deposit styles. It provides detection capabilities in a platform capable of rapid, complete coverage with minimal access issues. This approach has been used in FALCON™ airborne surveys for iron ore exploration in the Great Lakes, USA; the Middleback Ranges, South Australia; and the Hamersley Basin, Western Australia.The complex magnetic properties of iron formations (particularly remanence, anisotropy and depth of weathering) prevent this approach from being simple. The use of vector residual magnetic intensity (VRMI), calculated from the measured total magnetic intensity (TMI), assists by reducing the complexity due to the different directions of the remanent and induced magnetic fields.Estimations of the density to magnetisation ratio via the pseudo-lithology calculated from the VRMI and gravity gradients provide a useful method of capturing the discriminatory power of joint magnetic and gravity gradient data.

Batu Hijau is a calc-alkaline, island arc porphyry copper-gold deposit located in the southwest corner of Sumbawa Island in the Banda archipelago of southern Indonesia. Proven and probable reserves are 913 million tonnes at an average grade of 0.53 per cent copper (10.631 billion pounds Cu) and 0.41 g/t gold (389 tonnes Au). Construction of the deposit infrastructure began in October 1996, and formal approval for the project mobilisation was granted by the Indonesian government in May 1997. Preoperational testing and start-up of the first grinding line is scheduled for mid-year 1999.Copper and gold mineralisation are genetically related to at least three tonalite porphyry dykes and small plugs which intrude a quartz diorite stock that was emplaced into andesitic to dacitic pyroclastic rocks and lava flows. Faults are uncommon, and where they occur they do not significantly displace hydrothermal alteration and mineralisation zones.

Airblast overpressure legislative limits can be extremely difficult to meet throughout Australia, which has by far the most stringent limits of any nation in the world. As the difficulty in meeting the limits is strongly dependent on the scale of mining, it is not surprising that the Australian coal mining industry is experiencing particular difficulties in meeting these limits. Eight full scale overburden blasts have been modelled with respect to overpressure impact for a strip coal mine, involving both dragline (cast) blasting and pre-strip (non-cast) blasting. Overpressure levels in the range 100 to 125 dBL have been measured at distances varying from 2.4 to 5.7 km, and modelling has successfully estimated both the amplitude and shape of overpressure waveforms to within an average of 1.2 dBL. Typical errors of prediction are around 0.6 dBL. This accuracy of prediction has been achieved by considering the impact of only the face holes, and the accuracy of prediction of both shape and amplitude suggests that it is these holes which dominate the environmental overpressure impact at distances in the range two to six kilometres, for cast and pre-strip blasting operations. Modelling has been based upon the Elemental Overpressure Wavelet proposed by Richards and Moore (2002), incorporating actual hole coordinates, actual delay timing, and assumed delay scatter statistics. The incorporation of delay scatter statistics has been particularly significant, and allows the determination of the range over which the peak overpressure levels are expected to vary. To date, modelling has considered only the use of conventional pyrotechnic delays, though simple adjustment to the scatter definition would allow estimation of overpressure levels, and their variability, with electronic delays.

The in situ testing of mine ventilation fan systems has been a contentious issue for many years. Site testing was generally undertaken by adapting standards that were developed for the laboratory testing of fans using standardised airways. The deficiencies inherent in this practice have been widely recognised by informed practitioners, and this led to the first stand-alone document specifically intended for the in situ testing of large fans (ISO 5802:2001 Industrial fans - Performance testing in situ).This paper describes the structure of the International Standards organisation and the processes that are followed in the course of preparing such a standard.The application of ISO 5802:2001 is briefly outlined together with how test data is assessed for compliance with the suppliers guarantee. The Gold Fields Cerro Corona copper-gold deposit in Peru has heterogeneous rock characteristics in terms of its mineralogical, geological and geotechnical properties. Blast design variations in these conditions directly impact the run-of-mine (ROM) fragmentation, which in turn influences downstream process efficiencies. In particular, when harder ore is processed, throughput and circuit performance may be limited.To evaluate the potential to increase throughput and improve overall plant performance by optimising blasting practices and comminution circuit operation, Gold Fields La Cima engaged Metso Process Technology and Innovation (PTI) to conduct a full process integration and optimisation project at Cerro Corona. Blasting, crushing and grinding processes were reviewed to identify the potential to achieve substantial improvements in productivity when treating the most competent ore types.Site-specific models of the blasting, crushing and milling processes were developed using SmartTagTM ore tracking technology for calibration and validation. Ore tracking permitted locations in the blast volume to be queried against the geotechnical block model to determine the hardness and structural parameters of the feed to the comminution circuit. They also ensured surveys were conducted when ore from the audit blast was flowing through the crushing and grinding circuits.The predictive models developed enabled PTI to demonstrate how changes in blast design influenced both ROM fragmentation and the performance of downstream comminution processes. Outcomes of the project suggested changes to blast designs and comminution practices, and these were partially implemented for a validation trial. The trial demonstrated a 14.8 per cent increase in mill throughput for the specific ore selected for the study, and an average increase of 5.7 per cent for all ore types. There is potential to further increase throughput and process performance if all recommendations are implemented. Sample preparation for gold samples, where the distribution and size of gold particles is often irregular, usually demands that a relatively large sample weight is taken to produce a representative sample for analysis. Traditional sample preparation of gold samples incorporated multiple stages of coarse particle size reduction followed by subdivision of the sample prior to final fine pulverising. This technique developed because of the limited equipment options available to laboratories. As a result significant errors are introduced and the final sample size being pulverised is a small fraction of the original sample weight. Three significant developments have occurred in the last 25 years to significantly improve sample preparation of gold samples: 1.  large capacity fine pulverising,2.  fine jaw crushing, and3.  whole sample coarse pulverising. In the mid-1980s an Australian manufacturer developed large capacity pulverising bowls which had the ability to reduce samples with a mass of 1.5 - 3.5 kg to a nominal 0.075 mm product in approximately three to five minutes. Laboratories were able to produce a large homogeneous pulverised sample which significantly reduced the error in sampling. However, exploration and mine samples are usually 7 to 10 kg and the sample requires a subdivision stage following crushing to produce the 3.5 kg for the pulverising step, thus introducing potential for significant error. The development of fine jaw crushing allowed particle size reduction to -2 mm which reduced the subdivision error of the bulk sample prior to fine pulverising. Subdivision of the crushed sample at -2 mm still results in potential sampling errors, where the gold particles are not sufficiently liberated to be representatively sampled. Clearly the best option would be to reduce the particle size small enough to ensure a representative sample prior to final pulverising. Continuous mills offer a partial solution but are relatively slow and subject the sample to differential pulverising prior to subdivision. Clearly this does not provide the best solution for routine laboratory applications where high volumes of samples are to be processed. The recent development of a large capacity coarse pulverising system allows samples of up to 10 kg to be reduced to -0.25 mm prior to subdivision, and fine pulverised to -0.075 mm. Laboratories now have the necessary equipment to perform reliable large capacity sample preparation of gold samples. The exact choice of a sample preparation scheme may still depend on particular ore types but the need for multiple crushing and subdivision stages is redundant.

Queensland Pacific Metals (QPM) is progressing its Townsville Energy Chemicals Hub (TECH) project to produce nickel and cobalt sulfates for advanced battery materials. Apart from the nickel and cobalt sulfates, the process also generates a hematite by-product (~640 ktpa). The hematite byproduct precipitated from the process, hereafter ‘hematite precipitate’, is high in Fe and particularly low in impurities. However, it is a fine <20 μm crystalline, hydrometallurgical precipitate and is produced as a filter cake containing a high moisture content of about 15 per cent. This is considerably different from natural hematite ore and is expected to change its performance during sintering. In the present study, laboratory scale sintering tests were undertaken to evaluate the potential impacts of substitution of the hematite precipitate for traditional magnetite concentrates and further for Australian hematite-goethite ores in a simulated industry base blend. Substitution of hematite precipitate for magnetite concentrates was found to slightly increase the sintering temperature required to achieve the strength of laboratory sinters. However, further substitution of hematite precipitate for Australian hematite-goethite ores slightly reduced the sintering temperature required to achieve the laboratory sinter strength. These results reflect the reduction in Al2O3 contents of the blends and are consistent with the trend observed in industry and pilot scale facilities. The reduction in sintering temperature generally correlates to a reduction in fuel rate in the larger scale sintering process. The pore structure of selected laboratory sinters was further examined and discussed in conjunction with sinter strength. The pore structure images highlighted the relative difference between the blends with the most obvious difference being seen at 1290°C which correlates well with the laboratory tumble index (TI). The laboratory sinter samples showed an increase in porosity and irregularity of pore shape, as hematite substitution increased demonstrating the correlation between a more porous structure and a lower TI.

In November 1992, the Internal Revenue Commission (IRC) was formed from the rperggr of the Papua New Guinea Taxation Of- fice (PNGTO) and the Bureau of Customs, and now has the re- sponsibility for the administration of direct and indirect taxes on a national basis in Papua New Guinea (PNG). Since that date we have undertaken many initiatives to make the new organisation more efficient and responsive to clients&apos; needs. Income Tax The mining industry is catered for, in respect of income tax, un- der a separate taxation regime within the Income Tax Act (ITA), Division 10, as well as some general provisions within that Act as may be applicable. Division 10 is itself broken up into subdi- visions to cover the different types of mining leases, namely Spe- cial Mining Leases (SMLs) and Mining Leases (MLs), and peri- ods of time during which those sub-divisions apply. Ring Fencing Income tax assessed on the mining industry, with the exception of Mining Leases (these will be explained later), has a project basis of assessment, that is, each project is assessed separately as if the taxable income of the taxpayer from operations attributable to the project was the only assessable income derived by that taxpayer. This is often referred to as "ring fencing". This is an important point to consider because losses from unsuccessful projects cannot be set off against income from those that are suc- cessful, except in very limited specific circumstances, such as where an unsuccessful exploration licence is cancelled or surren- dered. The project basis of assessment applies to both resident and non-resident companies. A relaxation in the &apos;ring fence&apos; provisions was applied from 1st January 1995, where ten percent of exploration expenditure, in- curred in areas outside of Special Mining Leases and Petroleum Development Licences in a particular year of income, can be claimed as an outright deduction which is limited to ten percent of the tax otherwise payable in that year. This was introduced to encourage exploration in PNG.

Since the 1970s we have seen a marked corporate response in terms of the environment. In part this has come from regulation but it has largely come from stakeholders and the community. We now see a similar trend towards sustainability. The Brundtland (1987) definition of sustainable development provides a direction for advocates: the advance of human prosperity in a way that does not compromise the potential prosperity and quality of life for future generations. In my view its three essential components are: economic, environmental and community. The Global Mining Initiative and the contributions to the Rio World Summit and the Johannesburg World Summit on Sustainable Development indicate that sustainability is being taken very seriously by the mining industry. But does the minerals industry really embrace sustainability as they claim? There is a view that in reality sustainability is a non-event and that the triple-bottom line approach is only a convenient catch-cry for companies who are actually focussed very strongly on the economics part of the equation. Chemical engineers are showing a strong trend to more sustainable practices and the chemical industry has shown real commitment to research into developing sustainable processing systems. This requires a framework for thinking and design and the chemical industry is demonstrating true innovation as it moves towards æno wasteÆ technologies. Whilst there are limits to the rates of change that can occur the minerals industry should also be moving in a much more determined manner towards the notion of processing of products designed for whole-of-life cycle û with minimum/no waste. But is the minerals industry just paying lip service to the importance of research to sustainability?Several common themes influence the science and innovation agenda in Australia, which will affect the development and adoption of innovations by AustraliaÆs minerals sector. First is the well-proven link between peer-reviewed scientific excellence and commercial application. Second is the rise of multi-disciplinary Mode 2 research, which has developed in response to the complexity of problems faced by contemporary researchers, which are best analysed by combining expertise from two or more disciplines. Successful innovation outcomes, in any field, including minerals processing, is promoted by Mode 2 research which benefits from greater levels of networking and collaboration. Kofi Annan in his Message to the World Social Forum puts it very succinctly æThe way forward lies in finding constructive solutions togetherÆ.

The direct smelting of zinc sulfide con- centrates has been investigated by an extensive series of computer-based thermodynamic equili- brium calculations. The study covers both pure 7.n-S-O-C-N systems and practical concentrate- coal-air-flux systems. It is shown that for a zinc concentrate containing iron and lead sul- tide impurities, a 97% yield of zinc vapour with autogenous operation can be obtained under carefully optimized conditions. The possible integration of this process with existing zinc condensation and recovery processes is Discussed.

Rock bolting provides a supplement or alternative to conventional timber and steel supports. The tensile strength of the bolts lies within a range established by manufacturing quality control. Rock bolt testing determines the ability of the rock, anchor and individual bolt as an assembly to accept load. The.test is performed to the yield strength of the bolt or anchor and deflection is observed. The anchorage index, inches deflection per 1000 lb load increment, is a measure of anchorage quality. This allows comparison with anchorages under ideal conditions. It also permits comparative assessment of roof bolt anchors in a particular environment.
