The appraisal and evaluation of accidents and injuries is required to determine the costs of loss producing practices and conditions. This involves all of an organizations disciplines that concern themselves with a more efficient and cost effective organization. To come to know the cost of your safety program or how much is expended yearly for each employee on safety; to know what your safety costs are per man-hour or per ton-produced; to know what the health, medical and compensation costs are for any given incident are just a few of the important facts and figures that the safety management of a given organization needs to know to operate in the mining industry today.

The mining industry has had three difficult years. Commodity prices have remained low and investor confidence in the industry has been shaken. And even though the Asian economy appears to be improving, it will be some time before this improvement will translate into better commodity price conditions for mining. In addition, increased environmental and social restrictions that have been placed on the industry in developed and developing countries have made mining a more difficult proposition. And, if that is not enough, the global mining industry has found itself in fierce competition with Internet businesses for investor capital. Despite that gloomy picture, more than 10,500 mining industry professionals from about 70 countries gathered in Toronto, Ontario March 5-10, 2000 for the Mining Millennium 2000 conference. This meeting was a joint effort between the Canadian Institute of Mining, Metallurgy and Petroleum(CIM) and the Prospectors and Developers Association of Canada (PDAC).The joint meeting also featured two trade exhibitions and more than 150speakers. PDAC’s trade show attracted about 400 exhibitors and an additional 200 in the concurrent Investors’ Exchange. CIM’s exhibition attracted about 600 equipment and service companies.

INTRODUCTION Gwinnett County, Georgia, is a part of the rapidly expanding metropolitan Atlanta Georgia area. Located approximately 48 km (30 miles) northeast of Atlanta, Gwinnett County has a current population in excess of 600,000 and is projected to reach a population of 1,200,000 by 2025. As a part of the County’s master plan to supply drinking water for the expanding population and to increase system reliability, the Gwinnett County Public Utilities Department undertook the Shoal Creek Raw Water Intake and Pump Station Project. Figure 1 shows a vicinity map for the project. Prior to construction of the Shoal Creek Raw Water Intake and Pump Station Project the County had a single raw water intake to feed water from Lake Sidney Lanier (Lake Lanier) into a filter plant for processing into potable water. This project together with a newly constructed water filter plant provides Gwinnett County additional capacity and system redundancy to prevent interruptions to the drinking water supply. Lake Lanier is a man made reservoir with over 800 km (500 miles) of shoreline and was constructed by the Army Corps of Engineers in the 1950s for hydro electric power generation, flood control, and navigation benefits for the Chattahoochee River. Over time, Lake Lanier has become an increasingly important source of drinking water for the metropolitan Atlanta area. This paper focuses on the primary underground construction and lake tap components of the project including a 42.7 m (140 ft) deep pump station shaft 24.4 m(80 ft) in finished diameter transitioning to a 6.1 m (20 ft) finished diameter shaft, a3.05 m (10 ft) finished diameter by 19.8 m (65 ft) deep intake shaft constructed in an average of 21.3 m (70 ft) of water, and a 190.5 m (625 ft) by 3.05 m (10 ft) finished diameter modified horseshoe tunnel connecting the two shafts. This raw water intake system, in conjunction with the incorporated pump station with 8 pumps and dual1,829 mm (72 inch) pipelines, provides a system capacity of 1,091,062 m3/day(240 MGD) to two water filter plants and is expandable by the addition of pumps to1,500,210 m3/day (330 MGD). Parsons provided design and construction management services for the project. The $29.6 million contract to construct the project was awarded to W.L. Hailey &Company. Notice to proceed was given August 6, 2001 and substantial completion was achieved on January 20, 2004. Value engineering changes adopted for the project contributed to completing the project at a final cost of $28.8 million.

Asking the question, “What do the world’s leading corporations and cutting-edge practitioners of sustainability put forth to guide a given mine site for planning and action toward ‘sustainability’?”, this paper explores the spectrum of methodologies and mindsets in available literature, then constructs an example for the large near-closure facility at Kennecott Utah Copper. Values, opportunities and constraints spring from physical, unutilized byproducts, from critical ecosystems and wildlife populations, from location relative to those ecosystems and proximate communities, and from the political will to perform adequate feasibility investigations. Patience, determination and thoroughness are imperative. Identifying most productive and, simultaneously, most protective and restorative, practical methods to approach ‘sustainability’ has become a central mission of industry and business, worldwide. A survey of recent international initiatives finds no single approach that establishes either a readily adaptable methodology or an appropriate set of ‘attention’ points, or agenda, to guide individual mining operations at specific locations toward the elusive but essential goal of ‘sustainable’ mining. Untying this convoluted knot of contending factors requires willingness to recognize and work within ecological constraints for community long-term needs. Occasionally, the task requires the sword of Alexander the Great. Through integration that is unflinchingly honest ethically, culturally, economically, ecologically and scientifically, fully cognizant of place and community at whatever scales are of greatest significance, and integrative of both problem-response and opportunity, we may envision a mineral extraction and beneficiation future that achieves the best, most nearly sustainable, results possible. Industrial ecology (IE), with its clear emphasis on community, ecosystems, and sustainable economic stimulation, presents a sufficiently broad and adaptable discipline to facilitate planning and management toward sustainable mining. The creation of sustainable jobs encapsulates IE’s analogy to the niche survival strategies of organisms in nature. Open to common-sense recognition of faults and opportunities at appropriate scales, IE is less a discipline or prescribed methodology than a mindset, willing to be rigorously scientific when science is imperative, but also willing to exercise “art” when creativity or community engagement are needed. IE is also capable of future-looking compassion for peoples and nature. Looking beyond what has occurred to date, the case of Kennecott Utah Copper provides ample illustration of possibilities for community economic development, environmental management, and significant initiatives beneficial at regional and planetary scales, to become recognizable as deserving of the label, ‘sustainable.’ Through what this researcher terms “whole mine, whole region, whole planet” thinking, strategies become visible and subject to serious consideration, presenting a path to best possible accomplishment in mining. Constituting a maximal broadening of mining’s “traditional” scope, the integrative approach proposed here is the ‘paradigm shift’ being made by many other industries around the world. In aggregate, these steps are the only hope for avoidance of environmental, social and economic risks that threaten to curtail resource development and impair mining companies’ ‘social license to operate.’

Kenny/Shea JV is constructing the water intake tunnel for the new Elm Road Generating Station, a supercritical coal-fired power plant in Oak Creek, Wisconsin, being built by Bechtel Power Corporation. The intake tunnel system consists of a temporary mining shaft, pump house and intake channel riser shafts, and four lakebed intake shafts. An 8.33 m diameter, 2,800 m long TBM mined rock tunnel is being constructed to connect the shafts. The intakes are located 2.4 kilometers offshore in Lake Michigan. The shafts and tunnel are being constructed in glacial tills and Silurian Dolomite. The estimated tunnel completion is December 2007.

Acid rock drainage (ARD) is produced when sulfides are exposed to oxygen and water. Oxidation reactions typically result in a depression of pH and leaching of solutes and metals. Many oxidation models have been developed based on oxygen diffusion, but in environments with high permeability, other transport processes (i.e., convection and advection) can play an important role. Multiple sulfide mineral phases with differing oxidation rates should also be incorporated. WROC, a finite-difference model, was developed to account for these factors in predicting ARD. The program includes sulfide mineral oxidation using a shrinking core mechanism, heat transfer by conduction, heat of reaction, and oxygen transport by diffusion and advection. The model was validated using data from the literature. After validation, the WROC model was used to simulate several different waste rock scenarios including different forms of sulfide mineralization and the effect of implementing a waste rock cover. The results demonstrate that including specific sulfide mineral phases can have a significant effect on oxidation and that implementation of a cover can also aid in reducing waste rock oxidation. A new comprehensive insitu instrumentation technique named this RCC Method has been developed for rapid assessment and improvement of the structural stability of underground openings. When applied to tunnel construction, the degree of present and prospective safety of underground openings can be determined in quantitative terms within 10 to 20 minutes of microcreep observation. When applied to mining, roofs and floors can be stabilized by controlling the stress conditions. This stress control method has eliminated the need for intensive roof bolting and extensive floor renewal in all cases where it was applied. Use of this technique is illustrated by field examples from the Seikan Undersea Tunnel of Japan and deep potash mining in Canada. NEW INSTRUMENTATION TECHNIQUE: THE RCC METHOD Failure problems in underground excavation usually arise in weak non-elastic grounds where conventional rock mechanics theories of elasticity are not only useless but also misleading. Presently, discontinuity planes in the ground are being considered a factor in assessing these failure problems, and while this development is interesting academically, it is not of much use in actual excavation work, where it is nearly impossible to identify and quantize all existing planes of discontinuity. Even if such an extraordinary task could be accomplished at an underground excavation site, effective use of the data would be difficult and impractical for rapid excavation work, with the possible exception of rare cases of ideally elastic ground.

The Office of Surface Mining Reclamation and Enforcement (OSM) is preparing an Abandoned Mined Lands Reclamation Control Technology Hand- book. (l) GAI Consultants, Inc. , recently prepared Chapter 2 on Mine Subsidence Control which contains a summary of subsidence abatement measures plus two new decision matrices for evaluating the subsidence abatement options that might be considered for a particular problem. These decision matrices are presented for information. Review of the subsidence abatement methods resulted in identification of items where research may impact the current state of technology in mine subsidence control. These research needs are presented to stimulate discussion of their importance in subsidence abatement and to encourage funding agencies, researchers, and practitioners to work on them.

The Powder River Basin (PRB) of northeastern Wyoming and south-central Montana is the largest strippable reserve of compliant coal (0.6 or less lb of sulfur per million Btu) in the United States. In mining parlance, it is the Mother Lode! Wyoming’s portion of the Basin provides one fifth of the coal produced in the United States annually, and the state has been the nation’s top coal producer for the past six years. Based on 1993 production statistics and estimated strippable compliance coal reserves, Wyoming may remain the nation’s top producer for the next 76 plus years. Past articles on the PRB have focused mainly on production. After living and working in the Basin for the past five years, I have developed an appreciation for the area and its major industry that transcends size and production data. In the fall of 1993, MINING ENGINEERING’s publisher and I decided to present a series of articles that would cover many other aspects of the Basin. Ten topics were outlined, and specific individuals, selected for their knowledge and experience, were asked to contribute articles about their specialties. Although Montana has four large mines producing coal from the PRB, this series of articles will concentrate on the Wyoming portion of the Basin.

HIGH SPEED RAILWAY NETWORK Spain is a very adequate country to develop the high speed railway network thank to its geographical feature which has a 600km long radius, from Madrid- the capital that covers mostly the whole country. Therefore, the railway is a major competitor of the airline. In Spain, the new access to the north and northwest is nowadays the most important railway project which will allow a significant saving of time in the near future to the central plateau and the northern territories. Time savings as well as strict environmental considerations have been the main objective of various studies during the last decade leading to Guadarrama and Pajares tunnels as ones of the optimum solutions. Cordoba-Malaga high speed railway, the extension of the present Madrid—Sevilleline, through the Abdalajis range and the rail project to cross oriental Pyrenees between Spain and France are another two samples of large tunnels currently underway. In these four projects, the ground already encountered and to be soon encountered is highly variable, from very hard granite and gneiss in Guadarrama and Pertus to very poor argillite in Abdalajis, shale in Pajares and schist in Pertus. A number of faults are also expected. Hence, double shield hard rock TBM has been chosen. Figure 1 shows a general scheme in four projects. These four projects represent a total of 2 × 68 km of which 87 km are under DRAGADOS contracts. PROJECTS UNDERTAKEN BY DRAGADOS Guadarrama Tunnels This project is the longest twin tunnel with 28.377km, in Spain and considered one of the longest in Europe. The operating speed will be 300 km/h. This tunnel will link Madrid and the north territories through Guadarrama range. (Figure 2). The south portal is placed at el. 997.8m above the sea level. The tunnel starts with an inclination of 3‰, increasing until 15‰. After 6,620m length, the tunnel passes under the Morcuera ridge with an overburden of 960m. After 13,400m length, the elevation is 1,024m which is the highest point, and then the tunnel declines with a slope of 9.5‰, 2‰ and 19‰ until the north portal in Segovia province. Rehabilitation challenges were met nearly two months ahead of contract schedule for the Design-Build, Fast Track Scituate Aqueduct & Tunnel project in Cranston, Rhode Island. The aqueduct and tunnel were built in the 1920s and consisted of three sections—a 2.3 m diameter reinforced concrete section that extends approximately305 m before it transitions to a horseshoe-shaped 2.3 m wide by 2.1 m high cast-in-place concrete tunnel that continues for approximately 5.3 km until it transitions back to a 2.3 m circular section that extends approximately 1.9 km to the end of this project. Major repairs were required to assure sustained operation of this water supply tunnel.

"Pareto analysis was used to investigate the failures database of a Hitachi EX1900 hydraulic shovel over one year of continuous operation. The hydraulic system and hydraulic links, hoses and piping were identified as the most critical system and subsystem, respectively. A three-parameter gamma distribution function was identified as the best fit for a probabilistic modeling of the time between failures (TBFs). This distribution was used in a Monte Carlo reliability analysis of the hydraulic system. Autoregressive integrated moving average (ARIMA), within the context of a time series analysis of the TBFs, was proposed as a viable alternative to the traditional technique of distribution fitting. The mean absolute percentage error (MAPE) metric was used to compare the two models. The MAPE value that resulted from the ARIMA was 66 percent smaller than that from the traditional technique in predicting the TBFs. Se identificó al sistema hidráulico y a los enlaces hidráulicos, mangueras y tuberías como el sistema y subsistema más importantes, respectivamente. Se identificó una función de distribución de tres parámetros gamma como el más apto para un modelado de probabilidades del tiempo transcurrido entre las fallas (Tabes por sus siglas en inglés). Esta distribución fue usada en un análisis de confiabilidad Monte Carlo para un sistema hidráulico. El promedio de movimiento integrado auto-regresivo (ARIMA por sus siglas en inglés), dentro del contexto de análisis de serie temporal de los TBFs, fue propuesto como una alternativa viable a la tradicional técnica de ajuste de distribución. Se usó el porcentaje de la media absoluta de error (MAPE) para comparar los dos modelos. This paper will discuss how the construction management team and the contractor working together on the City of Los Angeles, North East Interceptor Sewer project, managed information on a daily basis. Data was synthesized from various sources including: inspection, survey, instrumentation, laboratory and the tunnel boring machine data logger systems to produce state-of-the-art reports and graphics which will become industry standard. Special emphasis is placed on how the data is managed and interpreted in a clear and objective manner, permitting the tunneling processes to be carefully monitored and suggesting appropriate action to be taken by the construction management team and contractor.

The National Institute for Occupational Safety and Health (NIOSH) uses light-scattering dust monitors (nephelometers) in its research to develop dust controls to reduce respirable silica dust overexposures in miners. Recently, the model 1500-pDR, an active nephelometer, was introduced by Thermo Scientific. Testing was completed to ensure compatibility with the metal/nonmetal mine dust personal sampling unit (M/NMDPSU). Results of testing demonstrated that gravimetric concentrations measured from the 1500-pDR are comparable to the gravimetric concentrations measured using the M/NMDPSU, without need for correction, under commonly observed dust loading conditions.

The two-dimensional predominance area (Kellogg) diagram (PAD) surfaces bounding the three-dimensional Cu-As-S-O system were constructed at 900 K. The computations employed gases As4O6(g), O2(g), and SO2(g) as the independent coordinate species to generate diagrams of As4O6(g) − O2(g),O2(g) − SO2(g), and As4O6(g) − SO2(g), while the third gas (As4O6, O2, or SO2) was fixed. The results indicate that there is no shared boundary between enargite (Cu3AsS4) and copper sulfate (CuSO4); however, at PO2 < 10−6 atm, PAs4O6 < 10−10 atm, and PSO2 > 10−4 atm, CuSO4 is the stable phase at 900 K. The chemical driving force for oxidation of intermediate phases between enargite and copper sulfate is substantial except for basic sulfate (CuO · CuSO4).

"Uma Oya Multipurpose Development Project is under construction in Sri Lanka. This project involves 2 Rolled Concrete Core (RCC) Dams connected through a tunnel currently under construction by Drill and Blast method. The water is transferred from first diversion dam to the 2nd regulatory dam and from which, it will be transferred through a 14 km headrace tunnel to the top of a 650 m deep drop shaft that feeds the high pressure water to an underground powerhouse and turbine chamber for generation of 120 MW of electricity. The Headrace and Tailrace tunnels are being excavated by two 4.4 m diameter double shield TBMs. The headrace tunnel has already passed 5km milestone and is mined in hard rock, while the tailrace tunnel has been completed in rather weak and fractured ground. This paper will review the history of the project along with the geology of the site. The operation of TBM in headrace tunnel will be discussed in more detail, focusing on the challenges of excavating through very strong and abrasive quartzite and gneiss on this site, as well as reaches of tunnel in high inflow of water reaching 400 L/Sec. INTRODUCTION This paper aims to introduce the Uma Oya Multipurpose Development Project (UOMDP) as a hydro mechanical project in Sri Lanka, targeted generating hydro-electric power, transferring water for irrigation purposes and controlling seasonal devastating flood. This project lies in the south-eastern part of the central highland region of Sri Lanka (Figure 1). The main part of the project is located in the south-western part of the Badulla district in the province of Uva. The south-eastern end of the project site is in the central west of the Moneragala district (province of Uva). Bandarawela is the largest urban setting within the project area, located halfway between the upstream and the downstream ends of the planned hydropower plant and combination of dams, tunnels, and downstream facilities. The Multi-Purpose Project Of Uma Oya is planned to convey water from Uma Oya to Kirindi Oya Basin for hydroelectric energy generation includes Puhulpola RCC Dam on the Uma Oya river, Dyraaba RCC Dam on the Mahatotilla Oya river, a conveyance tunnel from Uma Oya to Mahatotilla Oya (L=3750m), a long feeder tunnel from Mahatotilla Oya to Kirindi Oya. The feeder (power) tunnel Lot consists of a headrace tunnel (L=15554m), the surge shaft (L=200m) and tank on the upstream of the vertical shaft, a vertical shaft with steel lining (L=650m), an underground powerhouse and a tailrace tunnel (L=3650m)."

Additional field evaluations of a modified continuous miner using a new spray system and modified scrubbers have been conducted by the authors. In this demonstration, a comprehensive package for dust control on a continuous miner which included previously developed modifications to the scrubber, scrubber intakes and the innovative chassis spray system was demonstrated. Side-by-side comparisons of the modified miner and regular miner indicated that the miner operator with the modified miner was exposed to 62% less respirable dust. Ram car operators with the modified miner had 38% less respirable dust exposure and the return air on the modified miner had 19% less respirable dust. The mining conditions during side-by-side testing were very comparable. These results support the findings of the modified spray system studies conducted earlier at different mines in Illinois on four occasions and the modified scrubber system field evaluation studies.

The 30th annual short course ?Grouting Fundamentals and Current Practice? was held at the Colorado School of Mines, June 22-26, 2009. The field demonstration portion of the course was conducted June 25, at Hayward Baker, Inc.?s yard in Broomfield, CO. The full-scale field demonstration annually presents many different types of drilling equipment being operated as well as various grouting methods being performed under different field conditions. As part of the field demonstration, the class is shown the proportioning, mixing, testing and injection of various cement grout mixes into sand columns under controlled and measured conditions. These sand column demonstrations have been conducted under controlled and measured conditions each year since 1999 as part of the short course. The sand column demonstrations were conducted prior to 1999, but with less quality control and minimal record keeping of the proportioning, mixing, testing, injection pressures and the final permeation results. The goal of the sand column demonstration is to show the students the effect of the water cement ratio and the use of admixture as well as the fineness of the cement, portland versus ultrafine, used has on the engineering properties of the grout and the grout?s vertical permeation through the sand. Test results for the demonstrations conducted in 1999 and for two separate demonstrations, one at the grout course and one at Geo Denver, conducted in 2000 were published in the 2001 proceedings of the Rapid Excavation and Tunneling Conference (RETC) (Henn et al. 2001). The test results for the 2002 and the 2003 demonstrations were published in the 2005 proceeding of the RETC (Henn et al. 2005).

This paper describes a commercial installation of a sophisticated instrumentation and control system designed to maximize the performance of heavy medium cyclones. Samples of plant feed and its products were collected and analyzed for ash content so that measures of efficiency could be determined for plant operation under (1) manual control and (2) automatic (computer) control. The heavy medium cyclone control strategy and its digital computer implementation are discussed in detail. The sample results are analyzed and the economic benefits of automatic control are discussed. Results from the initial data sets quantify the economic benefits of automatic closed-loop control; viz, increased recovery and reduced clean coal variability have been clearly demonstrated.
