This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. SUSTAINED growth characterized the U.S. economy in 1964. Many new peaks were achieved in production, sales, employment, employee earnings, and business profits. At the end of 1964, the economy had experienced 4 years of uninterrupted expansion, and the greatest net gains in output for all postwar cyclical advances had been registered. There were no indications of the various weaknesses which have previously characterized advanced stages of business expansion. Both gross national product (GNP) and national income were 6.7 percent higher than in 1963. In 1964 GNP totaled $628.7 billion, a gain of $39.2 billion over the 1963 figure. On an overall basis, prices rose 1.75 percent in 1964. The real volume of output of goods and services (GNP in constant dollars) increased by 5.0 percent ill 1964; this was more than the 3.8-percent gain in 1963 but less than the 6.6 percent gain in 1962. The 1964 output gain also exceeded the average 3.7 percent annual increase for the postwar period. Congress authorized a two-stage tax reduction designed to further stimulate economic growth and employment. The tax cut favorably affected the 1964 economy in the following ways: By helping to boost personal income, corporate profits, and industrial production; by accelerating the pace of consumer spending and business capital expansion; and by increasing employment, thereby reducing unemployment to 5 percent. This paper describing the concentrator practice of the Hurley Plant of the Nevada Consolidated Copper Co. at Hurley, N. Mex., is one of a series of papers on milling methods and costs being prepared by the United States Bureau of Mines. The Hurley concentrator has a normal daily capacity of 13,500 tons of ore, and for the first four months of 1930 the heads of the concentrator averaged 1.32 per cent of copper and 3.14 per cent of iron. The recovery for this period was 83.98 per cent and the concentrates produced averaged 27.54 per cent of copper.

First in a projected series on the use of flexible liners for support of underground mine openings, this report presents information obtained from an extensive review of the literature on this subject. It also includes results of some preliminary test work by the Bureau of Mines. Investigations thus far indicate that flexible liners offer numerous advantages over rigid-type sup-ports, By deforming in the direction of the applied stress, the stresses are redistributed from the liner to the surrounding material. The corresponding buildup of passive resistance in the direction of diametric extension causes an equalization of stresses around the liner, thereby enabling the liner to carry loads in a more efficient manner. However, additional experimental data are needed to establish design criteria that will permit maximum utilization of the flexible liner concept in mining. Testing of small-scale models, accompanied by measurement of free-field stresses in material surrounding the model, is recommended as the most suit-able approach to the problem. The experimental procedures necessary to such an approach are outlined in detail and a statistical design for the experiments is proposed. The report also describes a device developed by the Bureau for measuring free-field soil pressures to 1,000 psi and a technique for evaluating the response of the device.

Tae purpose of this paper is to present types of charts adapted to aid in the graphic solution of fan-performance problems of selection and operation, and to urge the adoption of similar charts by the National Association of Fan Manufacturers as standard forms for presenting fan-performance data. As matters now stand, it is difficult for the average engineer to solve such problems because a laree number of variables are involved, and performance data are not presented in a manner that permits their direct application to many problems encountered in practice. The performance of any centrifugal machine is determined not only by the machine itself but also by the fluid-flow system on which it operates. Actual performance of centrifugal fans is therefore e matter of the type, size, and speed of fan, on the one hand, and of the relation of pressure to quantity of flow, as determined by the physical condition of the airway system, on the other.

This publication presents abstracts of articles and investigations pertaining to the utilization of fly ash. The terms "fly ash" and "flue. ash," as' used in most of the papers, refer to the ash from pulverized-coal firing collected by electrostatic precipitators or mechanical collectors. Several investigators do not define the material on which they report beyond calling it fly ash, but they probably refer to the fine material resulting from burning pulverized coal. A few of the papers report on investigations involving not only the use of fly ash but boiler slag as well. These can be identified by the title of the articles. Bibliographical information and the abstracts are arranged according to the date the papers were published, with the most recent listed first. With the continually increasing use of pulverized-coal firing, profitable disposal of the fly ash is becoming a problem of greater importance to many utility and industrial companies. Also, in recent years, the public demand for cleaner air, and the more highly efficient fly-ash collectors available have resulted in the collection of a greater percentage of the fly ash produced. The solution to better utilization of fly ash has, therefore, been the object of research for some years.

Fresh coal refuse was sampled at 21 US mines. Five of the sites were drilled, sampled and tested more extensively. Field tests included Standard Penetration tests, and in-situ density determinations. Laboratory testing included specific gravity, gradation, maximum, and minimum density tests, as well as 9-inch) drained triaxial tests, 9 and 10-inch, permeability tests, slake-durability tests, and accelerated slaking tests. Some refuse sites had significantly lower shear strengths and permeabilities in old, slaked or weathered material than in fresh refuse. Testing did not show useful correlations between strength and permeability changes and other material properties which could serve as "predictors". Testing of refuse under "accelerated slaking" conditions showed promise as a comparative tool and as a potential means of preparing artificially aged samples for evaluations of long-term behavior. The state-of-the-art in slaking research, and the implications of slaking on coal refuse disposal, and on practical methods of managing slaking, were also studied.

"The electrolytic manganese process4/ developed by the Bureau of Mines involves the electrolysis of a solution of manganese sulfate and ammonium sulfate in a diaphragm-type cell. Metallic manganese is deposited on the cathodes, and sulfuric acid is produced at the anodes. The spent electrolyte contains, in addition to the sulfuric acid, approximately 40 percent of the manganese sulfate and most of the ammonium sulfate originally present in the feed solution. This spent solution is used to leach fresh ore that has previously been subjected to a reducing roast. The resulting leach liquor is purified and returned to the cell.Manganese of very high purity is produced by the electrolytic process. The properties of this metal are so different from those of the best grade of manganese that can be made by any other commercially feasible process that it is virtually a new material. Electrolytic manganese has been used in the preparation of new high-manganese alloys with valuable properties.3/ and further developments are to be expected. The electrolytic manganese process not only results in a product of superior qualities, but is applicable to large tonnages of domestic ores that are unsuitable for direct use in producing ferromanganese.The future industrial use of electrolytic manganese is of course, related to its cost. Decreases in the cost of production will lead to wider use of the metal; conversely, increases in the volume of production made possible by greater demand will bring further .decreases in cost."

In recent years, thermodynamic data have been used to a greater degree than ever by the chemical and metallurgical engineer as a means of predicting and controlling chemical reactions. To satisfy the spiraling demand for basic physical data needed for the calculations, the Bureau of Mines is studying the heat capacities of a variety of chemical compounds at high and low temperatures. This paper is one of a series presenting such data. The heat capacities of two alkaline earth compounds, strontium bromide and strontium nitrate, were measured in a temperature range of 60° to 300° K. in an adiabatic calorimeter and at temperatures of 300° to about 1,200° K. in a Bunsen ice calorimeter. The construction and operation of the calorimeters are discussed in other reports.3 4 The heat-capacity data presented in this report were extrapolated to 0° K., and smooth values of heat capacity, entropy, enthalpy function, and free-energy function were calculated and tabulated at even 10-degree intervals. The enthalpy data were fitted with algebraic equations by applying the method of least squares; calculations a£ heat-capacity values, entropy, enthalpy function, and free-energy function were made at even 50-degree intervals. The heat and entropy of fusion of strontium bromide were calculated from the enthalpy measurements.

The Bureau of Mines is conducting a series of investigations with the common aim of minimizing the losses that occur in the mining and treatment of mineral substances. The results of the investi- gations are being published in reports of the bureau. This report treats of a method of preventing some of the waste of the natural gas incidental to oil mining. This method, the condensation of gasoline from natural gas, offers to the oil operator and others a profitable means of utilizing some of the oil-well gas now being wasted. The most desired constituent of crude oil is obtained, the production of oil is not hindered, and the gas, after the extraction of gasoline, can be returned to the leased area to drive pumps or into pipe lines for uses to which natural gas is ordinarily put, usually with its fuel value lessened only in slight degree. Publications already issued briefly discuss the subject. In this report the work is treated in greater detail, and the results of many additional tests are shown.

Composite samples of nonmagnetic taconite from the western Mesabi range were evaluated by reductive roasting, fine grinding, and magnetic separation to determine potential sources of concentratable material. Although large tonnages of nonmagnetic taconite are known to occur in the western Mesabi district, these materials are not as yet a commercial source of iron ore. The technique employed permits the evaluation of individual composites and blocks of composites in terms of responsiveness to a process of potential commercial significance. The 159 composites, from open-pit mines located between Grand Rapids and Chisholm, Minn., were evaluated in the following sequence: 1. Minus 8-mesh material was roasted at 500° C. in hydrogen. 2. The roasted are was ground to one or more sizes in the minus 100-to minus 400-mesh range. 3. The roasted ground ore was magnetically separated in a Davis tube. 4. Composites were classified according to degree of grinding required to produce minimal concentrates. (Minimal concentrates are herein defined as those containing less than 10 percent silica, and responsive composites as those yielding minimal concentrates.)

The first section of this circular has been prepared with a view to answering inquiries frequently received by the Bureau regarding laws governing the staking of claims and regulations covering requirements for holding them and obtaining title in fee to placer ground. These laws and regulations were summarized briefly in Information Circular 6796, "Placer Mining in the Western United States." These laws, however, are administered by the General Land Office, Department of the Interior, and it has been considered advisable to combine an authoritative résumé thereof by the Commissioner of that Bureau with information on small-scale placer mining previously published by the Bureau of Mines. The second section of this circular is largely a revised reprint of Information Circular 6611, "Small-Scale Placer Mining Methods", first published in April 1932 for the purpose of answering the many daily inquiries then being received on this subject. This circular has been reprinted repeatedly, and about 30,000 copies have been distributed, most of them in response to applications therefor. At present (June 1937) requests for this publication continue to be received daily.

"DEFINITION OF VISCOSITYViscosity has been defined as the resistance experienced by one portion of a liquid moving over another portion, such as one molecular plane moving over an adjacent plane. A measure of viscosity is the force required to keep one plane in motion at Unit velocity, the other plane being considered stationary. Figure 1 illustrates the general idea that the surface friction between two adjacent areas will be greater the larger the molecules. Therefore it may be assumed that viscosity will be increased by molecular association or agglomeration, and likewise dissociation rill decrease viscosity. According to Maxwell (1)6 the unit of viscosity, the poise, is the tangential force per square centimeter of area of either of two horizontal planes 1 centimeter apart, one of which is fixed while the other moves with unit velocity, the space between being filled with viscous liquid of unit viscosity.The need for a satisfactory apparatus and experimental procedure for the determination of viscosity of high-melting materials at temperatures corresponding to those in pig-iron and steel making has been recognized during the past few decades. The condition of the slag during the smelting and refining periods in the iron- and steel-making processes exerts an important influence on the metal product obtained, this slag condition really meaning the state of viscosity or, vice versa, fluidity, which the slag attains under certain conditions."

The investigation of mine shafts and underground workings at Leadville, Colo., was started in May 1946 and recessed in November 3, 1946. Thirty-eight shafts and connecting accessible underground workings, comprising about a third of the accessible shafts in the district, were investigated. The Leadville district, a former important producer of lead and zinc, was considered by the Bureau of Mines to be capable of bolstering 'the waning domestic output of .these important metal. Accordingly, the investigation was initiated for the purpose of. .obtaining more definite data on sulfide and oxide reserves, evidence to justify completion of the Leadville drainage tunnel, representative large samples of the complex oxidized and mixed ores for testing to determine methods, beneficiation factual data to be supplied to property owners in order to stimulate production, and to determine the more favorable areas for development by the Bureau.

This is the final, report submitted for the Contract No. H0220002 with the U. S. Bureau of Mines for the design and development of a Permissible Automatic Data Acquisition System (PADAS) to measure and record roof stresses in underground mines Basically the PADAS is a portable, battery powered, 24-channel data acquisition system designed for safe unattended operation of up to 30 days duration in explosive environments. The unit will accommodate twenty-four pressure transducers or eight borehole deformation gages (or any combination) at distances up to 300 feet from the PADAS to measure pressures up to 10, 000 psig. All 24 channels as a selected group are scanned sequentially at one of several selectable rates.

Coal carbonization, the basis of a major industry, consists in heating coal in the absence of air at various temperatures (ranging from 500° to 1,000° C.) that are selected according to the kind of products desired. During this treatment, the complex organic substances that comprise the major part of the coal are converted into several useful products that are quite different from the original material. The principal products of coal carbonization or distillation are coke and gas. These products, as well as others of which lower yields are obtained (light oil, tar, and ammonia), occupy an important place in present-day commerce and civilization. This paper is concerned with the composition of the tar and light oil, which, as complex mixtures of organic compounds, are the source of many important dyes, inter-mediates, medicinals, resins, etc. The volatile products obtained when bituminous coals are destructively distilled or carbonized are tar, aqueous liquor, and gas. These products vary in composition and properties, depending on the nature of the coal and the temperature and conditions of carbonization. The tar is a dark, rather thick and viscous liquid at ordinary temperatures, becoming semisolid when cooled to the freezing point of water. The aqueous liquor (chiefly water) contains several inorganic substances and certain organic compounds, which, because of their solubility characteristics, have been extracted from the tar. The carbonization gases consist chiefly of carbon dioxide, carbon monoxide, hydrogen, hydrogen sulphide, ammonia, methane, ethane, ethylene, and several hydrocarbons of higher molecular weight. The hydrocarbons of higher molecular weight (benzene, toluene, xylene, etc.) may be re-moved as liquids from the gas by suitable means. This liquid and the first fractions obtained on distilling the tar are known as light oil.

This report is intended as a reference for personnel interested in the computer anlaysis of coal-mine-electrical-power systems under either normal operating conditions or fault situations. The procedures developed for this purpose are described and implementation considerations are discussed with respect to the mine-power system. Considerations for choosing a solution technique and a number of examples are included. Procedures described include the Newton-Raphson iterative technique for the analysis of normal operation as well as a non-iterative Z-bus technique for analysis of unbalanced or balanced faults. During program development, strict attention has been given to the inclusion of core and/or time saving techniques such that these algorithms may be implemented on small as well as large computers. Program languages include FORTRAN IV for batch processing applications and APL or BASIC for interactive applications.

A few studies on the relationship of the fusion temperature of the ash on coke to that of the corresponding coal have been made on commercial high-temperature cokes. H. J. Rose3/ in 1921 compared the fusion temperatures of the ashes of 10 coals and the cokes produced from them and concluded that they were in such hose agreement that either figure could be used. More recently, Quarfort4/ found that the ash of coke fused at a somewhat lower temperature than that of the original coal. As the ash of a coal and that off tie coke produced therefrom have the same origin their chemical composition should be identical, provided that carbonization has not altered the inorganic, or ash-forming, constituents of the coal, and such alteration seems highly improbable. However, it has been shown5/ that coke is liable to contamination by iron during crushing and grinding preparatory to sampling. Pieters6/ found that coke ash was contaminated in this manner up to 0.4 percent. Extraneous iron would lower the ash-fusion temperature because such iron may form ferrous silicate, which is readily fusible.7/ These results provide a basis for the prediction that the ash of coke will fuse at a lower temperature than that of the corresponding coal, especially if the fine grinding of the coke was done by rubbing -surfaces of iron, as in a disk pulverizer or bucking plate.
