Safety is a key value in most leading mining companies. Despite years of effort, and significant expense in ‘programs’, operating sites seem to have been left with no real improvement in either the elimination of fatalities or sustained reduction in injuries.Safety systems and processes have grown and evolved, often to the point where they have become overly complicated and marginally useful.MMG Limited, following the consolidation of its Australian operations under a single management structure, undertook a program to effect real and sustained change in its approach to managing safety. The program aimed at improving safety by better engaging its workforce and by making safety processes simple, useful and effective. It defined the role of the supervisor in creating safe work, the role of the people doing work and the role of safe work methods.A safety diagnostic tool was developed to assess site against safety-critical mindsets, behaviours, cultures and systems. The outcome of the diagnostic is considered by the organisation to be the true lead indicator of safety performance.While still a work-in-progress, the program at the Australian Operations of MMG Limited shows that challenging common paradigms in safety management can create real change in safety performance.CITATION:Webb, M E, 2016. With the iron ore industry in Australia producing hundreds of millions of tonnes of ore per annum, the associated costs of these operations extend into the billions. It is no surprise in the current economic climate that more and more companies are looking into different ways of optimizing their costs to keep their sites operational. One way of improving operations is the use of electronic initiating systems like i-kon and Uni tronic detonators. These systems have been used around the world to improve current practices and optimize drill and blast costs. However, these systems have yet to make an impact upon the iron ore sector in Australia. Some of the potential improvements include:pattern expansion and a subsequent reduction in drilling and explosive costs;improved fragmentation and hence increased dig rates, thereby driving $/tonne down;embracing the 'mine-to-mill' concept by reducing the downstream costs of crushing and milling;improving the overall condition of pit walls and floors, leading to a safer working environment in which optimal haulage performance can be achieved; and improvements in vibration management around heritage sites and critical infrastructure, thereby saving the cost of relocation works. This paper introduces some of these concepts and the process of making the changes leading to potential savings from the improvements, using various examples as proof of the concepts.

Ridgeway Deeps is a block cave (BC) located down dip of the operating Ridgeway sublevel cave (SLC) mine. The BC is located approximately 210 m vertically below the lowest SLC level and approximately 1100 m below surface. Due to the mine layout containing a number of common pieces of key infrastructure the activities of both were interlinked.When investigating project construction options for the BC, a number of potential risks were identified including a conflict of priorities between operational and project deliverables and the varying safety and work methods between an owner mining approach in the SLC and a potential contractor workforce in the BC. This led to a review of the construction philosophy and the opportunity to utilise the existing SLC workforce as the basis of all activities in the mine. It was identified that a number of synergies existed with the current SLC operators and mine supervision including common work practices and the opportunity to build on the unique SLC skills of the operators and supervisors which could then be used at other Newcrest caving mines.With the commitment to this philosophy of building on the existing SLC crews and supervision the number of employees reporting through the superintendent underground operations grew from 95 to 245. This increase led to a restructuring of the reporting and work requirements of the superintendent, foremen and shift bosses. In order for the foreman and shift bosses to focus on running the larger crew numbers for the full 12 hour shift the mine superintendent became a pseudo human resources manager taking over all non-mine focused activities including overtime allocation and work and behavioural performance management. In simple terms, ensuring there were competent bums on seats when and where required. As the additional workers were recruited to top up the existing employee numbers, their employment history was used to place them in the most suitable roles. For example, those with building industry experience were placed on the civil construction crews and those with a farming background were used as truck drivers, nippers or on the service crews. To cope with the increased number of new starters a number of key personnel were seconded from the crews into the training group to act as on shift trainers. This initially caused a reduction in mine performance as the main operators were reassigned but it proved a valid move as the time for new operators to reach acceptable levels was significantly reduced with safe operating standards being maintained. This paper discusses how the Ridgeway Deeps Project was delivered on time and under budget whilst at the same time retaining its position globally as an operation to be benchmarked against.

A common problem in the mining industry is that company geologists can operate in relative isolation to the rest of the company (Jones and Moorhead, 2011). This may result in poor knowledge transfer and often priority conflicts with geology, operations and project departments. This problem is amplified by the acquisition of mineralogy capabilities and other metallurgical (met) test work undertaken by many companies in order to solve for complex orebody problems. Met data is often not located and viewed in 3D space so that geologists and engineers can create and work with geometallurgical (geomet) models (Bye, 2006). Visualisation of data in an interactive graphical 3D model provides a bridge enabling met professionals to collaborate with engineers and geologists to provide a common understanding of the relevance of geomet data.In recognition of this problem, Newcrest has undertaken an effective collaboration strategy to improve internal and external engagement through the use of large visualisation displays (Jones, 2009). The displays are underpinned by IT systems and information management practices that recognise the needs of underlying disciplines. Today it is generally understood within Newcrest that this is required to improve decision-making, deliver faster cycle times and improve governance.Collaboration in companies has been generally associated with the introduction of new IT functions (IBM, 2008). However, the science and engineering process that are used to structure modern IT-based collaboration systems and practices are now known to be simplistic compared to the long-standing practices associated with collaboration and knowledge management in disciplines such as geology and mining.CITATION:Jones, J and Moorhead, C, 2013. The formation of magmatic oxide ore deposits hosted in chemically-evolved layered gabbroic intrusions is attributed to a variety of petrological processes, but there is little to no consensus on the general applicability of each model. Complicating the matter further is that many layered gabbroic intrusions are spatially and temporally associated with alkaline A-type granites, suggesting that there is a petrogenetic relationship between the two types of chemically different rocks. Our experimental results show that petrological modelling using a starting composition of high-Ti basalt from the Emeishan large igneous province can produce evolved silicic liquids that resemble the compositions of microgranular enclaves from the syenitic unit of the bimodal, oxide ore-bearing Baima igneous complex. The experimental results confirm that bimodal gabbro-granitoid complexes can form from a common parental magma and that early crystallisation of Ti-rich magnetite can explain the formation of the oxide ore deposits in the lower parts of the layered gabbroic intrusions. We conclude that crystallisation of a typical high-Ti basaltic parental magma can produce world-class giant magmatic oxide ore deposits and that the occurrence of some alkaline granites may be an indicator of their presence.CITATION:Shellnutt, J G, Hsia, T W-Y, Liu, T-C and Iizuka, Y, 2015. Most new engineers in the mining industry do not realise the importance of legal obligations. During investigations the first question you are asked is: Do you have any proof or evidence? The smallest detail in an underground notebook or a diary entry is able to be used as your proof and/or evidence. The importance of a paper trail is often undervalued and overlooked when you first enter the industry. As a new engineer, all too often your legal obligations, including your duty of care are not given enough consideration. Whose job is it to inform you of what your legal obligations are and when do they end? Everyone has legal obligations at every level of the industry, from graduate to mine manager. The level of these responsibilities varies depending on the position that you hold. When you first start in the industry you need to understand what your responsibilities are and how you are expected to discharge them, whether they are memos, databases or senior sign-offs, etc. In order to fulfil your obligations you must first discharge your own obligation, which in turn discharges those of your department and ultimately those of the mine. As a graduate engineer in the mining industry what I have learnt about my own legal obligations has come directly from my own experience, which will form the basis of this paper.

Mining in Western Australia places significant economic and environmental responsibility on Government and the mining industry. Groundwater abstraction is regulated by the Department of Water. The Department has recently issued a guideline for Pilbara mines, and this paper focuses on the Pilbara for its examples and studies. The guideline is intended to form the basis for developing a statewide policy, consistent with reporting and assessment policy.With rapid expansion of the mining industry between 2005 and 2008, the groundwater resources in the Pilbara region of the State are under increasing pressure. Significant new management and regulatory issues arising recently include: cumulative impact assessment; disposal of dewatering excess by discharge, sharing or reinjection; life-of-mine approvals and closure planning; closure water quality including metals concentration in pit lakes; and in situ leaching mining methods and their impacts on regional water resources (and change of State Government Policy with respect to uranium mining).

"It is vitally important in projects where the cut-off grade is low that there is no contamination during the sampling and sample preparation process, both in the ore and the waste zones. Failure to identify contamination could have devastating financial consequences for a project. The use of a variety of blank types is essential for ensuring that contamination is identified and eliminated at every stage of the sampling and sample preparation process. This paper examines an instance where an effective assay quality assurance and quality control (QA/QC) program was able to identify and eliminate a source of significant multi-element contamination in the prefeasibility stages of an open pit project. This examination includes the: evidence for contamination findings of the investigation process of resolving the problem and correcting the data evidence for both addition of metal and dilution through contamination potential impact on the project had this issue not been dealt with. It was through the use of a combination of blank types that the QA/QC process at Mount Isa Mines (MIM) identified a contamination issue at the first crushing phase of sample preparation. Identification of the contamination allowed MIM to stop assaying activities and work with the laboratory to correct the problem. The sampling program could then be continued with no further doubts about the sample preparation process. Contaminated samples were resampled from retained core. With the continuous monitoring provided by the blanks program, it was possible to ensure that only clean results went into the resource modelling data set. CITATION:Webb, J, 2014. In some parts of Australia, mine air cooling may not be needed until a depth of 1300 m below surface is reached. In other locations, it may be essential at a depth of only 300 m below surface. The reasons for these differences are multifaceted. This paper discusses the main factors and describes a simple method to assess whether or not mine air cooling will be required. The method uses a graphical relationship that considers depth below surface, geothermal gradient, surface climatic conditions and directness of the mine intake routes. This relationship has been developed, tested and refined over a period of more than ten years using data collected from most of Australia’s deeper metalliferous mines. The method provides a ‘first-pass’ indication of whether more detailed cooling studies may be needed. Perhaps more importantly, the method’s underpinning by benchmarking against other mines improves confidence in the assessment results. Mine site rehabilitation is the repair of the disturbance caused by mining on the immediate mining area and also on the surrounding environment. The long-term objectives of rehabilitation can vary from simply returning an area to a safe and stable condition, to restoring the mined land to a state as close as possible to its pre-mining condition, capable of being self-supporting and having a beneficial final land use. This paper investigates the importance and benefits of environmental planning, in particular for rehabilitation, and how input into daily operations can allow mining to progress with limited impact on the surrounding environment. It will also identify some of the key mistakes currently made during mining which could be averted by implementing prior planning and incorporating environmental consideration into daily operations. A number of rehabilitation opportunities will also be detailed for the different stages of the life cycle of a coal mine.

"Automation offers the mining industry great potential for improvements in productivity and safety. However, the experience of introducing automation in other industries has been that the full potential of new technology is not always realised. Designers are surprised, for example, to find that automation does not eliminate human errors. Unwanted and unexpected consequences also arise if the introduction of automation fails to consider how people will adapt to the new technology. A focus on the technical aspects of automation is necessary but not sufficient for success. The potential for improvements in productivity and safety promised by automation will only be achieved if the joint cognitive system that emerges from the combination of humans and automation is designed to perform the functions required for system success. Leaving the relatively simple life on a uni student and venturing off in to the outside working world is a big step. There are two paths to follow, the æContikiÆ style graduate program where you pick up your new lease vehicle and set forth on two to three years of planned activities, or can pack everything into one bag, stick a copy of æthe rough guideÆ in your pocket, and set off on your own graduate adventure. I took the path less travelled.The less followed method of developing yourself isnÆt for everybody. Parts of the journey are difficult and sometimes it feels hopeless, but itÆs all a learning experience. Along the way you might not get to all the graduate ætourist stopsÆ like dragline, truck-shovel, or drill and blast. But youÆll see most of them and pick up a lot more of the local culture such as people management, operational knowledge, quality control, and the black arts of project financial management.

Pumped co-disposal of coal processing waste was promoted in the 1990-2000s as a low-cost highperformance alternative to tailings dams. In particular it would avoid high up-front capital for rejects trucks and dam embankments, and would produce a combined waste deposit with better geotechnical properties than either coarse rejects or tailings. Overall, the promise was not delivered; in particular, upon discharge the waste segregated by size, and operators responded by moving the coarser waste to contain the finer waste. In consequence, not only did design analysis lag behind construction, but some of the initial expectations persisted with the result that analyses did not fully represent what was occurring on the ground. After more than twenty years inspecting, designing and tending Co-Disposal Areas (CDAs), it is apparent that they are essentially upstream constructed tailings dams with some more favourable aspects but also a few unique quirks. Although waste management technology has moved on somewhat, the high cost of changing CHPP waste circuits means that most mines with pumped codisposal will continue to discharge mixed waste slurry into CDAs. It is therefore worthwhile to share some lessons in the hope of promoting sounder design, operation and rehabilitation. These include: • Discharged waste does not form a homogeneous wholly competent deposit but instead comprises materials ranging from sandy gravel to silty clay with the distribution determined by movement of the discharge and decant structures; • CDAs circulate and contain a lot of water; • As a result of the first two points, some areas of the CDA comprise saturated fine waste that is no different from tailings and, like tailings, often behaves in an undrained manner; • Seepage is inevitable and desirable because strength improves as water is removed. However, stability is strongly impacted by pore pressures and in design these need to be rationally rather than optimistically considered; • As CDAs are built to about 40m or higher, strength-deformation characteristics of the foundation become more important; • Even if geomechanically stable, CDAs will need covers to prevent salts and acids from reaching the surface; and • Erosional stability of the cover material(s) will probably dictate maximum slopes for rehabilitation.

The fire safety design for road tunnels includes the following aspects: active systems, passive provisions and operational procedures. The active systems include the smoke control system, fire suppression system, detection system and fire fighter’s assistance systems. Passive provisions include escape passages, fire separation and fire resistance systems. The operational procedures refer to fire fighter access, reporting and alerts, evacuation processes and crowd control.Currently, these systems are typically designed by different designers or teams. These systems are intended to work together in a coordinated manner to provide the desired safety outcome. However, in practice, this is not always happening. The fire engineering process is supposed to bring together all these aspects and arrive at an integrated safety system. The fi re engineering process in practice is not integrating the design with the practice or reality and is thus arriving at less than desirably efficient safety systems.The example of design inputs shows how disconnected each system design basis is. The smoke control system design is based on the design fire size. The performance of the smoke control system is to achieve a smoke free environment for a defined space for a particular fire size – the design fire. The fire suppression system – most commonly sprinklers or deluge systems, is designed according to codes and to achieve a certain water discharge rate. The fire detection system is designed to detect temperature rise/rate of rise and perhaps density of smoke. Fire separation is determined to provide a certain period of service – mostly for structural elements. Escape route size and location is determined mostly by code compliance. Evacuation procedures are determined by Authority operational requirements rather than human behaviour.To arrive at an integrated fire safety system, the design basis and performances must be coordinated. An approach using fire scenarios would ensure all systems are designed to a common performance requirement. This paper will discuss how the current design approach outcomes compared with a fire scenario approach. More effective and efficient fire safety systems would require the stake holders to understand the implication of this approach and to embrace this through the fire engineering process.

Paste fill has been used successfully at Henty Gold Mine for the past five years. It has allowed 100 per cent extraction of the orebody, stabilised poor ground and enabled wide sections and offsets to be mined safely. However, there are a number of inefficiencies in the current paste fill system. These include the low output from the drum filter, the addition of aggregate to the paste to compensate and the trucking of paste from the plant to the portal. These inefficiencies have increased the operating cost of the paste fill system and caused a number of operational challenges such as blockages and increased pipe wear. These challenges have been partly overcome by taking ownership of the paste fill process, improved quality control and better communication. Henty is currently developing two new orebodies, the Mt Julia and the Darwin and has an opportunity to build an efficient paste fill operation. This is likely to be a high density paste system without aggregate, either located above the new orebodies or located at the plant and pumped to the new site.

The relationships between Aboriginal Traditional Owners and mining companies have often been mired in dispute, particularly since the High Court Mabo decision and the Native Title Act of 1993. In the Pilbara region, Rio Tinto Iron Ore (RTIO) impacts the traditional country of 12 Aboriginal Traditional Owner/language groups. Over the past 12 years the company and the Traditional Owners have negotiated and settled several significant land use agreements including the Yandi Land Use Agreement over the Yandicoogina mine in 1997, between Hamersley Iron (now Rio Tinto Iron Ore) and three language groups û the Bunjima, Nyapaili and Innawonga people and the Eastern Guruma Indigenous Land Use Agreement (ILUA) of 2006. These substantive agreements are now being followed by a new generation of region wide ILUAs which are close to conclusion. Arising from these regional agreements is a new framework for engagement which has been informed by the implementation of the previous agreements in the Pilbara and across the Rio Tinto group of companies. They are driven by both a company desire to have a consistent approach to engaging with Traditional Owners and the Traditional Owners wanting to have a similarly transparent and consistent standard of performance by the company in implementing the agreements which can deliver improved employment, cultural heritage engagement and direct and indirect benefits for the life of mining by the company. During the past three years the growth in the Pilbara, Western Australia brought new opportunities with people flocking to the region for employment flowing from the mining boom. Housing was at a premium and FIFO (fly-in, fly-out employment arrangements) was developed as a method to manage the general growth and demand for skills. Despite the recent down turn in the world economy since September 2008, which has reduced demand for minerals, the Pilbara continues to enjoy high levels of employment and as a result there is still significant pressure on housing and services provision. Although there has been vast economic growth in the Pilbara not only by mining companies but also oil and gas companies, the 2006 census data indicated that the direct benefits of improved employment, business development and engagement in the mainstream economy continued to exclude Aboriginal people. Indigenous people constitute a significant proportion of the population in the Pilbara region, some 6000 people û currently 14 per cent of the population rising to 18 per cent by 2015 û yet they remain at the bottom of the socioeconomic scale. More than 40 years of mining has returned relatively little to this group of people who are the permanent residents of the region and are connected through their traditional culture to this country (Taylor and Scambary, 2005). Ensuring local Aboriginal participation in the mainstream economy has been a challenge for resource companies, Traditional Owners and local residents in the Pilbara. The case study explores the relationship between a mining company û Rio Tinto Iron Ore and the Traditional Owners of the Pilbara. It examines two projects that have been developed to engage Indigenous people in the development of their region and through regional representation and business development to build a sustainable future for themselves and their future generations and at the same time ensure social cohesion to support social licence to operate. The case study also examines the process that was followed by Rio Tinto Iron Ore and its operations in undertaking a regional agreement for land access, participation and the integration of local Indigenous people into their business and the local economy.

For suitable production planning in an open pit mine, and also to carry out the optimisation process during the mining production cycle, prediction of fragment size distribution is required prior to carrying out the blasting operation. For this purpose, many prediction models, such as empirical models, were presented. The proficiency of these models differs in various conditions and mines. Therefore, in this paper the most important models, including SveDeFo, Kuz-Ram, CZM, Modified Kuz-Ram in 2005, Kuznestsov-Cunningham-Ouchterlony (KCO) and also Gheibie et al, were examined by the information of 20 blasts in Gol-Gohar iron ore mine. Fourteen images were taken on average for each blast. For image processing analysis Split-Desktop software was used. After examination, it was understood that none of the available empirical fragmentation models had presented acceptable performance, except the Kuz-Ram model for magnetite ore, which had somewhat acceptable results. However, this model for blasting patterns in hematite presented unsuitable results. Examination and comparison of two important distribution functions, Rosin-Rammler and Swebrec, were carried out and it was observed that the performance of Swebrec for fines was far better than Rosin-Rammler. This result for the upper part of fragment size distribution curve, coarse, is the reverse.CITATION:Jahani, M and Taji, M, 2015. For Australian mining projects, sustainable groundwater use is typically defined as a groundwater extraction regime, measured over a specified planning time frame, which allows acceptable levels of stress and protects dependent economic, social and environmental values (DEH, 2004). This principle is compatible with most stakeholdersÆ understanding of our national environmental objectives, yet the application of this policy on a project-specific scale is not always straightforward. Three examples of sustainability issues in the management of groundwater in mining are presented. These examples highlight each of the three dependent values: environmental, economic and social.

Project delivery in our current market is not as efficient as it could be. Industry at large (both client and consultant), is diligently following the engineer, procure, construct, manage (EPCM) or engineer, procure, construct (EPC) delivery models and not necessarily appreciating the underlying principles that make them the effective tools they were to start with. We often misinterpret project management with EPCM and end up with a literal and superficial interpretation instead of the disciplined risk reduction and business value capture exercise it is intended to be. One of these underlying principles is the project team as resource or commodity depending on which perspective we follow. Some of us understand this principle only too well and succumb to the temptation to take advantage of the imbalance in the understanding of EPCM by selling bodies and hours. Value and return on investment do not always come into the equation. The latecomers and aspirants are struggling with this as a barrier of entry into this lucrative market, since as we all know - people as commodity was becoming and still is a scarce resource. Clients are also waking up and asking why 1000 engineers on the floor are better than 100, when engineering design in all aspects of industry, new technology aside, is mature. Why is it that we start with a clean sheet of paper with every project or that neighbours in the same industry segment and location just happen to have completely unique conditions and circumstances that require totally original design (Local conditions aside)? The answers to this question appear when we explore the role of the project team and how it is structured. We will need to investigate team culture and levers for success. Then we will formulate a business case for a team to be able to follow traditional project management practices with EPCM delivery models but yet end up with an alternative look, feel and result. An outcome where more input is not necessarily better and the client and consultant both are aligned with success measured in dollars and a win-win outcome for both parties.

Safety management, both personal and that associated with catastrophic hazard, helps to define the success of mining companies. Personal safety management, often categorised by slips, trips and falls, has been widely addressed, with numerous approaches successfully adopted. Catastrophic hazard can lead to a single accident (eg ground collapse, underground fires etc) causing multiple fatalities. Catastrophic hazard management in mining is an emerging discipline, and as such there are various pitfalls to demonstrate its effectiveness and success; it is much more than simply meeting regulatory requirements. Various standards, guidelines, and leading practices are available for reporting key performance indicators. While measuring and reporting safety related key performance indicators (KPIs) such as Total Recordable Injury Frequency Rate (TRIFR), Lost Time Injury Frequency Rate (LTIFR), are important, they often lack detail, and give little insight into the effectiveness of catastrophic hazard management strategies and controls. Catastrophic hazards are low likelihood, high consequence events, and as such demand a different management approach. Companies cannot use the suite of personal safety metrics to help demonstrate management of catastrophic hazards. In-field verifications have revealed that senior management commitment to safety is often clear, well organised, and resourced. However, there are often some inconsistencies between what management promotes and plan, to what the operational teams understand and implement. Communication and clear understanding of expectations were the common themes identified to help empower supervisors to reinforce requirements of catastrophic hazard management. A lack of specific, measurable, and time-sensitive goals and objectives for the short- or medium-term, making it challenging to track progress was another key observation. Development of an integrated, multi-disciplinary auditing process executed by an experienced team of technical subject matter experts, provided an in-depth understanding of the requirements of catastrophic hazard management. Areas such as underground fire prevention, explosive storage, electrical safety, mobile vehicle interactions, ground control, inrush, and shafts and winders management have substantial overlap and intertwined processes, and similar strategies for emergency responses. Auditing of catastrophic hazards and safety management systems compliance is only the first step in safety assurance. Completing operational performance audits in technical areas and benchmarking the site verification of safety management documents and verifications forms a critical part to clearly demonstrate industry leading safety practices. No longer is it the external inspection or regular hazard management plan review requirements that dominate, but rather the culture of continual hazard identification, management, controls verification, underpinned by improvement and follow-up verifications of the corrective actions.
