Bow-tie analysis is a commonly used tool in the Major Hazard Facility sector to identify, display and control risks that have the potential to pose serious harm to personnel. It is a simple but powerful risk management tool that displays traditional risk assessment outputs as easily interpreted ‘bow-tie diagrams’. These are an effective and versatile communication format that can be applied to any type of risk (eg safety, environmental, assets etc) and as such show great benefit in managing operational hazards in mining.

Seeking to reduce the likelihood of uncontrolled personnel entry into the operating footprint of a dragline, Coronado Global Resources initiated a review of proximity detection systems currently available and equipment interactions in and around dragline operational areas. At the time of the assessment, there were no established personnel proximity detection systems in compliance with functional safety standards for large machinery in Australia. Our primary objective was to guarantee a functionally safe outcome, particularly by implementing a performance-level-rated system in accordance with AS4024 (Safety of Machinery series of Australian Standards). Applying appropriate detection technologies, the goal was to reduce the residual operating risk for machine-to-machine and machine-to-personnel interactions. A comprehensive analysis of available or in-development technologies was conducted, and a layered approach (demonstrated in Figure 1) was adopted to achieve the desired risk reduction by segregating the sensing zones into distinct areas using diverse and disparate technologies consisting of radar (short); light detection and ranging (LiDAR) (medium); and optical (long). To address the potential limitations of each technology, overlapping layered sensing zones were designed. With LIDAR and optical using two distinct artificial intelligence (AI) systems to classify detections, these were specifically trained to handle the unique environments of a mining operation.

This paper discusses the implementation and potential limitations of the method for estimating sampling and analytical errors of multiple source mine output blended into a shipping product, described by Everett and Ziegelaar (2010). The practice of quality measurement varies between iron ore miners. The common objective is to monitor effective resource utilisation and control of product quality to customers. A flow diagram is given presenting the various options used by iron ore mine operators and the merits of the different options are discussed.

Thank you for the opportunity to speak at this important conference attended by aspiring mining professionals, some of whom will become the leaders of the mineral resources industry that forms the solid foundation of AustraliaÆs past and that will fuel the exciting growth of AustraliaÆs future.I want to briefly touch on the history of our business, then look at the opportunities that exist in the contract mining industry and finally comment on what the educators need to consider in their curriculum to meet the needs of our business. Byrnecut started out as an idea in early-1987 developed by a group of four mining people. At the time the industry was on the high end of a mining boom, the era of the Bonds, Connels, Skases and so on. Riding on this wave of euphoria we borrowed $600 000 from the National Australia Bank and, without having enough knowledge to lose sleep over the potential risks, started business in mid-1987. The euphoria quickly evaporated with the 1987 October stock market crash. The bank pulled our overdraft, equipment finance rates increased to over 20 per cent per annum and my house was mortgagedseveral times over. The business learning curve was extreme for a young mining engineer who had developed his trade in the relatively sheltered environment of a large public mining house, namely WMC. We were somewhat bitter that the banks had done a runner. We were a private company, backing the business with personal assets with our shoulders to the wheel, whilst the Bonds of the world had theirnoses in the trough ripping off mum and dad shareholders. All of this had a sobering effect, namely keep your feet on the ground, stick to the knitting, work hard and worship cash flow. Since that stressful but adrenaline pumping beginning the company now employs over 600 people and is now working for major mining houses at over a dozen locations in Australia, east Africa and WesternEurope and the National bank is again our banker. On this basis I consider myself qualified for the subject of this speech.

Mining is currently undergoing a progressive expansion into novel environments (such as the deep ocean), commodities (such as tantalum, rare earths, and other ‘green economy minerals’), and new methods (such as the autonomous underwater cutting machines required to operate in the deep ocean environment, or the use of expanding grouts to deliver a blast-free mining method). The advent of novel mining, environments and commodities heralds an era that will undoubtedly start with much discomfort on the part of regulators and stakeholders, due to a feeling of overwhelming uncertainty. These proposed mining scenarios have few precedents, few examples of successful implementation, short baseline monitoring periods, and few examples of legacy sites that may assist with understanding long term risk and exposure. The evolution of the environmental and social impact assessment (ESIA) process in many regulatory environments recognises stakeholder engagement in a formal and facilitated manner. This is a positive step, and allows stakeholders to be much more meaningfully involved in the permitting process than they historically were. However, with this advance comes the challenge of identifying and communicating with stakeholders; sharing information in a meaningful way, fostering relationships with what can be an extremely large group, and delivering on commitments that inspire confidence to permit operations that have never been permitted before. In order to meet this challenge, it is suggested that changes to collection of data, communication of data, the nature of commitments, and the reporting of performance need to be made in order to effectively communicate management of new technologies, and the impacts that mining may have in new environments (such as the deep ocean). The communication challenge is necessarily greater when describing technology that has never been used, or environments that have never previously been accessed by humans on an industrial scale. The selection and implementation of the right technology for a project is critical to the future development of the resource industry. It is essential for companies to ensure ongoing competitive viability that where practical new or alternative technology be adopted. Analysis of historical data relating to development of resource-based process plants provides an enlightening insight into the key factors which influence their success or failure. The adoption of new technology does not guarantee the success of a project. The outcomes of this analysis have been used to develop some general guidelines and principles the author believes are useful in the selection and implementation of new or alternative technology in resource projects. Many organisations struggle to yield meaningful outcomes from incident investigations. This paper will demonstrate that we can discover systemic issues by rethinking the ‘Why’ and ‘How’ of investigations, fostering genuine risk reduction. Key points: 1. Shift from ‘prevent recurrence’ to ‘learn and reduce risk’: conventional investigations focus narrowly on recurrence prevention, resulting in narrowly focused investigations with limited ability to identify and reduce organisational risk. Redirecting our objective to learning and improvement opens avenues for broader risk mitigation. 2. Understanding daily work for risk insights: recognise that work is not always completed in full compliance with procedures. Workers will adapt how they complete tasks influenced by the conditions and management systems that exist every day. Understanding the conditions and systems affecting everyday work is vital for uncovering systemic risks. 3. Unearth systemic issues through in-depth exploration: many investigations address surface concerns such as lack of training, incorrect procedures, equipment not being maintained, and missing overarching organisational risks. Systemic, organisation-wide risks are not identified. To discover these systemic issues and create shared learnings, investigators must question management systems, including resourcing, planning and risk management. 4. Design actions for safe failure: common actions like discipline and training fall short and do not reduce risk. Resilience emerges from well-designed controls. Incorporate planning, better resources, interlocks, and guarding to bridge the gap between task execution and risk. 5. Investigation Reports as a Business Case: Investigation reports should not simply state historical information regarding an incident. They are to facilitate decision-making and be presented as a business case to drive change. Organisations can move beyond localised risk and ineffective administrative actions by changing investigation objectives and methods. They can facilitate learning, understand daily work intricacies, identify systemic issues, and enhance the capacity for safety, driving lasting risk reduction.

"An EXTENDED ABSTRACT is available for download. A full-length paper was not prepared for this presentation. Mineral exploration and mining are challenging businesses. Remote and at times harsh conditions have always made it difficult for exploration, but modern explorers also have to deal with cover, numerous technologies, an explosion of data and critical early community engagement. Mining deals with the reality of moving and breaking thousands of tonnes of rock each day to extract what can be a very minor component of economic value, and, furthermore, society expects that this will be done in a clean and sustainable manner. The exploration and mining challenges appear disparate, but they are intimately related.Although not widely appreciated, both exploration and mining have a long tradition of innovation. Over the last 50 years, major step changes in exploration have resulted from radical advances in our understanding of ore forming processes, new geophysical, geochemical and remote sensing technologies, and new ways to integrate and interrogate data. Over a more extended period, close to 100 years, mining has developed bulk surface and underground methods, selective extraction through flotation and leaching and global positioning system–geographic information system control systems that collectively facilitate mining at a massive scale. And yet, in spite of these successes and the boom that we have enjoyed in the last ten years, the industry is under pressure and the need for innovation and change is paramount.The major drivers for innovation are the need for increased quality in discoveries and mines and the need to interact effectively with communities and jurisdictions such that the benefits of quality mines are recognised and enjoyed widely.We are still making new discoveries around the Pacific Rim and elsewhere, but many argue that the quality – grade and accessibility – of these discoveries is decreasing. The net result is that explorers are delivering resources and reserves for development at lower grades and with higher capital and operating costs than current operations. For mining, the increasing age of many existing operations contributes to increasing costs, which, when combined with decreasing cut-offs and lower grades, also results in declining quality. The combined picture does not look good.Just as the pressure to improve performance and margins in the face of declining inputs increases, we are simultaneously faced with increasing societal demands for improved environmental performance and sustainable practices. This is most apparent at the local, community level, where opposition ultimately leads to delays or even cancellation of new projects. Anglo American Metallurgical Coal (AAMC) manages five open cut coal mines in Queensland and New South Wales, producing approximately 30 million tonnes per annum. The size and complexity of these mines require efforts by a large and diverse number of personnel to minimise adverse geotechnical events from occurring. To achieve this, the AAMC coal mines in Australia are developing a concept of managing the geotechnical risks associated with open cut coal mines. The overall system utilises a combination of internal training, a management system (operational management systems) and standards, numerical modelling, monitoring, inspections and audits. The development of the standards as well as training and mentoring of exploration personnel, geotechnical engineers, engineering geologists and mining engineers with an interest in the geotechnical aspects of mining, training of the production workforce, mentoring and training of the technical staff create the basis for achieving a pro-active rather than a reactive system. Audits are used to assure that personnel are maintaining the standards and to facilitate cross-mine leanings so that best practices are developed across all sites.

Australia as a leading producer of mineral sands has also occupied a leading position in many areas of research'in the processing and applications of these minerals. Research in upgrading ilmenite to produce synthetic rutile has resulted in the develop- ment of many processes, e.g,., Becher, Murso, Chlorine Technology, and ICI processes. Studies on the reducibility of ilmenites, aqueous leaching of reduced ilmenites, Fe-Ti-O system, leachability of natural and treated ilmenites in hydrochloric acid and selective chlorination of iron from ilmenites have been carried out in order to develop and improve the efficiency of the above processes. While research on the processing of zircon sand to produce high purity (99.99% ZrO2 zirconia powder and zirconium chemicals started only recently, research by CSIRO in the early 1970s led to the discovery of the phenomenon of transformation toughening in partially stabilized zirconia (PSZ) ceramics. Australia, with very significant resources of lanthanide-containing ores, pro- duces substantial quantities of monazite each year but research in processing and appli- cations of rare earths was neglected in recent years in this country. However, research to improve processes for the recovery of rare earths is now under way and studies on the production and testing of Nd-B-Fe magnets which are 1.5-2 times stronger than the strongest magnets (Sm-Co) currently available, have also commenced.' An overview of the research in the pro- cessing and applications of mineral sands during the last two decades in Australia and its implications for the industry are pre- sented in the paper.

In mineral processing, the management of tailings may become increasingly more challenging with a change of ore type and the quality of water used in the plant. This, in turn, may affect the overall costs of the operation and water usage and, in the long-term, pose challenges for mine closure and rehabilitation. Depending on the applied treatment technique, the short and long-term properties of tailings are largely determined by the fine particles. Most clay materials and ultrafine particles exist in tailings from oil sands, coalmines and base metals, bringing about challenges in dewatering performance. Moreover, given the growing concern for water scarcity, the minerals industry is increasingly turning towards the use of saline water, which poses yet another challenge for tailings management. The high salt content alters the behaviour of clay particles, making dewatering a more complex process than in freshwater systems (Boshrouyeh Ghandashtani et al, 2022; Costine et al, 2018). High-density techniques for tailings management have been used for several years in the mineral industry. In this regard, one of the novel and promising dewatering methods is in-pipe or inline flocculation, where the polymer is added at high dosages to thickener underflows, giving additional (likely denser) aggregation, releasing more water on deposition compared to low solids thickening. Previous research in the field of flocculation has primarily focused on the process of flocculation itself, specifically the settling rate and size of aggregates, rather than the long-term impacts of the produced tailings materials (Liu et al, 2020; Jeldres et al, 2017). This is particularly true for the novel method of this technology, where polymer is added to the thickener underflow. Furthermore, these studies have used small-scale set-ups using buckets/beakers, plungers and mechanical stirrers for the flocculation process. Unfortunately, these methods lack the necessary control over the properties of the treated tailings, which can lead to irreproducible mixing and dispersion of viscous polymer solutions (Fawell, Costine and Grabsch, 2015). Consequently, a system that can work with a wide range of operational conditions for high-solid tailings material is essential for reliable and reproducible results. The objectives of this study were to determine the effect of different type of polymers in the presence of monovalent and divalent salts on the inline flocculation of artificial tailings. The next step was to evaluate the impact of high-dosage polymer addition on geotechnical properties such as settling rate, consolidated density, rate of consolidation and the hydraulic conductivity of tailings. In addition, this research aimed to fill a knowledge gap and investigate the feasibility of using polymer-treated tailing in removing the copper ions and then quantify the performance of produced aggregates under time-dependent processes.

The traditional direct-heated drying process has been replaced by the steam drying process in non-ferrous metal smelters, where steam is available from waste heat boilers. Many recent projects have selected Kumera Steam Dryers in equipping a new production line or modifying an old production line. The industrial performance of the steam dryers has shown significant energy saving and emission reduction. The steam dryers have been operated at high availability and long maintenance interval. As the process principle of Kumera Steam Dryer is different from other equipment, the set-up for steam dryer plants is configured to fit the smelter site and operation conditions to achieve optimal performance. This article discusses the equipment involved in the steam drying process and their combination options.

Anglo Coal Australia (ACA) takes a proactive position with regards to attracting and retaining employees at a graduate level. Currently, ACA has graduates in its program across all disciplines: Mining Engineering, Mechanical Engineering, Electrical Engineering, Commercial, Occupational Health and Safety, Environmental Engineering, Human Resources, Safety Engineering and Processing Engineering. The current program is three years in duration and involves two (generally) equal periods at different ACA mine sites or Brisbane Corporate Office. ACA takes an active role in promoting its status at the university level by maintaining a scholarship program. Six undergraduate scholarships are offered each year across all disciplines. Associated Vacation Employment is offered to each scholarship holder during the period of his or her university study. This commitment to education is regarded highly by future graduates. ACAÆs active role in sponsoring student bodies is also known and appreciated amongst the university population. ACAÆs graduate program has been a successful training program for future leaders of the coal mining industry. The company is committed to making the program more suited to the needs of the graduates across all disciplines and as a result the program is not stagnant but is constantly changing and evolving. Recently the æGraduate PassportÆ has been drafted with an aim to keep each graduateÆs needs in relation to experience and training firmly in focus for the duration of the three-year program. Appropriate training for each graduateÆs position is supported by the company; for instance, support through funding and time for a commercial graduateÆs CPA qualifications is provided.

There has been a reduction in overall workforce in the mining industry during the current downturn; a corresponding reduction in fatalities and high potential incidents (HPIs) has not been observed. The same contributing factors can be seen in the vast majority of these incidents. These include inadequate supervision and lack of adequate risk assessment. How management structures, trains and leads an operation can have a great impact on eliminating these factors and reducing the risk of fatalities and HPIs.Contractors now account for 41 per cent of full-time equivalent employees in the Queensland mining workforce, and are disproportionately represented in the majority of fatalities and HPIs. Having a clear strategy for how contractors are integrated into the workforce is crucial. The risk and consequences of community rejection and loss of a social licence to operate is well articulated (Moffat et al 2018), however, the means for systematically managing this risk are less established. At the local scale, a mining operation is said to have a social licence to operate (SLO) when it achieves ongoing acceptance or approval from the local community and other stakeholders who can affect its profitability (Graafland, 2002). Without this social acceptance, it is very difficult for a business to operate effectively or profitably. It is well understood that social (or community) acceptance of a mine and its associated operations is a reflection of the quality of the relationship a company has with their host community (Thomson & Boutilier, 2011; Lacey & Lamont, 2014; Parsons et al., 2014; Moffat et al., 2015). Community relations are an integral part of successful mining operations and where these interactions are effective, they tend to foster mutual understanding, trust and support between a company and the host community (Kemp et al., 2006; Holley & Mitcham, 2016). Research further demonstrates that where such interactions are perceived to be procedurally fair, the increased trust created in these company-community interactions tends to lead to higher levels of acceptance of mining operations (Moffat & Zhang, 2014; Lacey et al., 2017).The challenge, however, is how to listen systematically to communities that mining and associated operations; communities that are often located across vast geographic areas and with diverse demographics. To date, traditional methods deployed to both guide and evaluate community engagement and social investment activities have been challenged to reflect the nature of the phenomenon they seek to observe, the voice of community. For example, detailed baseline surveys of community attitudes every three years by companies, as is the requirement within the policy frameworks of most large miners, provide rich insights but do not reflect the dynamic, temporal nature of the relationships that underpins their SLO. These processes typically are largely opaque to communities and distinct from the company’s engagement activities. The CSIRO Local Voices project was developed with Rio Tinto to go some way towards meeting this challenge of how to listen to the voice of community systematically. The relative infrequency of fatalities and other serious events can give them an appearance of being random, of being beyond any reasonable degree of anticipation and prevention. In fact, the vast majority of these events result from exposures that are identifiable, measurable, and manageable. The lessons of prominent incidents, such as the Space Shuttle Columbia, OxyÆs Piper Alpha, Esso Longford, BP Texas City, Massey Mine, as well as lessons from single fatality events, is that alongside the proximate causes of each incident, there also coexisted an underlying fabric of systems, mechanisms, and culture that allowed risk in the workplace to persist and often to become normalised. All of these ingredients of the organisationÆs safety fabric lend themselves to the influence and intervention of senior leaders. The difficulty is that traditional injury prevention paradigms are not sufficient for addressing the precursors that lead up to catastrophic and fatal events. A recent study of such events, led by Dr Thomas Krause concluded that the descriptive aspect of HeinrichÆs safety triangle is useful, but that the predictive aspect is not. The study surfaced reasons why a reduction in less serious injuries does not necessarily correspond to a proportionate reduction in catastrophic and fatal risks. This presentation expands on the findings to include the concept of exposure to the event potential linked to precursor events and the framework for leading an organisation to be both cognisant and effective in eliminating such events.CITATION:Russell, R, 2012. Rio Tinto Iron Ore is currently working towards finalising comprehensive Native Title land access agreements, which cover all of the current eleven mines and infrastructure owned and operated by the company in Western Australia's Pilbara region. Critically, the agreements represent a mutual commitment to a long-term relationship and address many areas of concern by promoting indigenous enterprise, well-being and regional development opportunities. Our objectives in the process are to obtain the informed consent of the traditional owners for our Pilbara expansions while providing benefits to them in a way which provides lasting and substantial improvements to health, education, housing and real jobs in the mainstream economy.   To facilitate the agreement negotiations, Rio Tinto Iron Ore (RTIO) proceeded in 2005 to research and design a specific strategy around Aboriginal relations and local community engagement in the Pilbara. Wherever we operate, Rio Tinto follows a fairly simple formula in working with communities, which is comprehensively aligned to local context.   This presentation provides an overview of Rio Tinto Iron Ore's community engagement strategy in Western Australia's Pilbara region. It will also explore the establishment of regional standards on employment, business development and contracting that have been developed to ensure Indigenous people are engaged in the development of their region and are able to contribute to supporting the growth and development of mining in the area with the expectation that there is prosperity for all. HukkiÆs law is an empirical law, which does not take into account several events of energy loss during the fragmentation processes. Since experimental results are very difficult to obtain, for a large range of fragment sizes, the verification of the law is very difficult. The relation between fractures and fragmentation processes with fractal geometry was proposed some decades ago. Empirical laws known in this context show basic features of fractal geometry, mainly self-affinity and power law behaviour. Thus, in this work a model was developed to simulate the fragmentation process and to check the relation between energy consumption and fragment sizes. The model is represented on a regular lattice. Links can represent pathways for fracture processes. The events of fragmentation have energy given by a probability distribution function. In the model there is no mass loss. Propagation of fractures occurs as self-avoiding random walks on the regular lattice.

Despite increased expenditure, the global discovery rate of world-class ore deposits has been in decline for over a decade (Schodde, 2017). Mineral exploration has traditionally followed an empirical approach, searching for evidence of mineralisation upon the surface of the planet, then drilling to define the extent of an ore body beneath the surface. As outcropping deposits in well-defined areas are depleted, there is a need for exploration to extend to less well-explored search spaces. One popular new frontier is exploration at greater depths beneath the Earth's surface. However, exploring for deep deposits with limited surficial evidence requires the implementation of predictive targeting methodologies (Hronsky, 2011; McCuaig & Hronsky, 2014). The Mineral Systems Concept (Hronsky & Groves, 2008; McCuaig et al., 2010) provides a framework for predictive exploration targeting. Ore deposit formation is recognised to be a focused mineralising event. These events represent self-organising critical systems and are underpinned by the interaction of complex, non-linear processes. Through the collection and analysis of datasets that map mineralising processes, it may be possible to predict the size and location of resultant ore bodies. Mt Keith concentrator treats low grade disseminated nickel sulfide ore supplied from the adjoining open pit mine. It is a traditional crushing, two-staged closed circuit grinding circuit (SAG & Ball Mills), followed by a complex split-size rougher, scavenger and cleaner flotation circuit. In 2016, 10.5 Mt was milled, at an average head grade of 0.54% Ni, yielding 35 000 nickel metal tonnes in concentrate.Historical analysis of the plant data indicated that of all the nickel lost in the fines rougher circuit tails stream, 25.6% was recoverable in an eight minute laboratory scale flotation test. These results implied that this fine nickel was readily available for recovery in the plant, either through operational improvements or additional flotation capacity. With operation optimisation ongoing, and capital within the business focused elsewhere, plant personnel were challenged to think differently about improving recovery.In 2012, a separate project to improve plant recovery when treating high talc ore was completed. Amongst other vessels, this project saw the installation of two 150 m3 Outotec Cells to the scavenger section of the fines rougher, middlings and scavengers (RMS 1) circuit. Since installation, the operation of these cells resulted in difficulties immediately upstream with instability in the scavenger cells. It was therefore decided that these cells would be better suited to treating the fines rougher circuit tails.The alterations required to divert these flows were minimal, with the capital project totalling $220 000.In the two years since installation, the fines rougher tailings nickel grade has dropped by 9.9%. The scavenger circuit has also seen a 2.8% reduction in tailings nickel grade, and instability issues were immediately resolved. This project serves as a lesson in how existing plant infrastructure may be better utilised to maximise recovery.
