Fortescue Metals Group flagship operation, Cloudbreak, is located in the Pilbara region of Western Australia. The mine is capable of delivering an annualised production rate of 40 Mt/a. Positioned 50 km to the east of Cloudbreak is Fortescue’s Christmas Creek operation, which has a combined processing capacity in excess of 50 Mt/a. Following on from these successes, Fortescue advanced development of its western Pilbara mines at the 60 Mt/a Solomon Hub. These processing installations place Fortescue in a strong position to deliver on a guidance of between 155–160 Mt/a for financial year 2014/2015.As Fortescue enters a phase of asset consolidation and debt repayment over expansion, the business is commanding stronger performance through enhanced plant operating practices and mine site cost reduction initiatives. To this end, the strategic placement of instrumentation is an area being given greater consideration. Access to real-time information pertaining to conveyed materials is of major interest to operators for making timely process control decisions. Fortescue has recognised the importance of accurate and up-to-date process information about their ores and hence, has adopted the use of Scantech’s Geoscan mineral analysers at two of their mines.There are two Geoscans at Cloudbreak; one on the ROM feed conveyor and another on the final product conveyor. These allow the in-feed material grade characteristics to be assessed. Moreover, the final product stream can be monitored to ensure product quality is maintained within acceptable quality tolerances. A third Geoscan is installed at Christmas Creek mine on a conveyor that transfers final products to Stockyards.This paper discusses the use of the Geoscans in these installations, including benefits to Fortescue’s operations, as well as functional considerations such as the impact of maintenance and calibration. "Early on Jan. 9, 2014, approximately 37,800 L (10,000 gal) of crude 4-methylcyclohexanemethanol (MCHM) leaked from a storage tank sitting on the bank of the Elk River near Charleston, WV (WVDEP, 2014; CSB, 2014). MCHM is a chemical commonly used in preparation of fine coal and is considered nonhazardous in terms of transportation, storage regulations and also toxicity (CDC, 2014a). Freedom Industries Inc., a producer and vendor of specialty chemical products, had acquired the storage facility from Etowah River Terminal, LLC just a few weeks prior in December 2013 (WVDEP, 2014; Freedom, 2005). The direct cause of the spill was ultimately determined to be two small holes in the base of the 76-year-old tank, which were created due to corrosion damage (Bauerlein and McWhirter, 2014).The spill site was located a little more than 1.6 km (1 mile) upstream from the intake of a municipal water treatment facility operated by West Virginia American Water (WVAW), which serves a population of 300,000 across nine counties in central West Virginia (CDC, 2014a). In a matter of hours, the leaked MCHM made its way through the treatment facility and into the water of nearby homes and businesses, where its strong smell was noticed almost immediately. On the evening of the spill, WVAW concluded that its conventional treatment processes, which include carbon filters to remove a variety of contaminants from water, were not capable of fully removing the MCHM and a "Do Not Use" order was issued for all customers (WVAW, 2014a).With little information readily available regarding the potential human health effects of MCHM, the U.S. Centers for Disease Control and Prevention (CDC) quickly issued an advisory ruling that set a maximum screening level of 1 ppm for safe drinking water (CDC, 2014a). Based on this recommendation, the "Do Not Use" order remained in effect for a total of eight days, until Jan. 17, when the WVAW distribution water was confirmed to have MCHM concentrations below the approved level (WVAW, 2014a). However, due to the pungent odor of the chemical, its presence was still recognizable in the WVAW facility and in consumers’ tap water after the order was lifted (Manuel, 2014). Recent studies have shown that the chemical can be detected in water at sub-ppb concentrations, a fact that undoubtedly added to growing public concern (Rosen et al., 2014; Gallagher et al., 2014). In response to numerous complaints and demands, WVAW continued flushing the treatment facility and pipes until Feb. 25 when MCHM levels were measured below 2 ppb (WVAW, 2014b), and eventually replaced the affected filter media (WVAW, 2014c)."

NYC has always been a high-profile target for both foreign and domestic terrorists. Explosives have always been the go-to tool for terrorists to bring about serious physical injury, death, and property damage in our society. Today with the anti-police movement gaining traction in the United States and anarchists groups using homemade explosives against law enforcement and municipalities, it is the responsibility of the explosives industry to ensure that every explosive product on a blasting site is secure. We must prevent the theft of these energetic materials used on projects and sold by vendors so they are not used by these terrorists. We have learned many lessons here in NYC which began with the theft of explosives used for blasting projects in NYC to supply the IRA (Irish Republican Army) 50 years ago. The liability is too significant to take short cuts in securing energetic materials. Over the years through cooperation with all agencies and the industry we in NYC have developed certain proven protocol and procedures we would like to share with the industry. An efficient and effective security plan for the logistic movement of explosives, as well as for the blast site is imperative to keeping the good name of the industry from negative repercussions and possible additional bureaucratic oversight of an industry that is highly regulated and still recovering from the pandemic. Communication, security assessments and transparency with all stakeholders and the Authority Having Jurisdiction (AHJ) will greatly improve the overall security for the blasting project. This SOP (standard operating procedure) will allow the AHJ to become a proactive partner in the security of the blasting project. If the AHJ is not a law enforcement agency, the law enforcement agency specialty unit must be notified and included in all phases of the project. The security for a blasting project with AHJ and law enforcement input starts at the inception of the project when the memorandum of understanding (MOU) is obtained, and involves the AHJ through all steps of the approval process. This includes threat assessments on new and existing structures. SME’s (Subject Matter Experts) must be present at all stages. The blasting industry and regulators must work collaboratively to improve safety, security, and create a dialogue with law enforcement and the AHJ. Relationships are built through trust, teamwork, transparency, and communication and greatly improve the safety of our great country.

"Failures of coal ribs are major hazards in underground coal mines. For the period of 2010 to 2014, rib falls were the leading cause of groundfall fatalities and accounted for 35% of all groundfall-related fatalities. Currently, the National Institute for Occupational Safety and Health (NIOSH) is conducting a research project to develop an engineering-based design methodology for rib control in underground coal mines to reduce rib fall accidents and fatalities. Numerical models can be used as part of the design methodology provided the models are appropriately calibrated and verified.This paper presents a coal rib model to simulate the mechanism of coal deformation and fracturing that can lead to stress-driven rib falls. The proposed coal rib model was calibrated against published data from an instrumented rib site at the West Cliff Mine in Australia. Fracturing of the coal rib was defined by critical VALUES (of plastic shear strain of the coal material and rib displacement. A critical plastic shear strain of 2.8% and a sudden increase of rib displacement of 30 to 45 mm were sufficient to develop a fracture plane in the rib model. Accordingly, a discontinuous rib deformation was achieved and it compared favorably against field monitoring data.INTRODUCTIONCoal ribs are the vertical walls of coal formed when entries are developed in a coal bed. Failures of coal ribs are a major hazard in underground coal mines. Furthermore, failing ribs can contribute indirectly to roof and floor instability by increasing opening widths across intersections and entryways. Over the last 18 years, there has been a continual occurrence of injuries and fatalities in underground coal mines due to rib falls. For the period of 2010 to 2014, rib falls (excluding longwall face failure and bursts) were the leading cause of ground fall fatalities and accounted for 35% of all groundfall-related fatalities (MSHA, 2015)."

BHP’s Western Australian Iron Ore Operations (WAIO) move approximately 250 million tons of ore per annum. The magnitude of the constant changes to the terrain models along with the diverse output and presentation requirements of numerous stakeholders presents significant challenges for the geospatial team. This paper describes how the WAIO Survey team use a combination of technology and optimised workflows to capture, ingest, publish and access large data sets for diverse stakeholder requirements. Machine learning for processing of data and the use of web based systems to track and prioritise task allocation of resources will be discussed in detail, in addition to the changes to the traditional roles of surveyors from the use of automation and mobile technology.

Shale gas wells can sometimes drill through coal reserves. Hypothetically, the caving of the gob from longwall coal mines may deform the shale gas well casing and potentially result in a breach. NIOSH researchers studied the permeability of the surrounding ground strata that may affect the potential for an inflow of gas into an underground mine environment. An overview and comparison of measured permeability values under differing cover depths (147 m and 353 m to the Pittsburgh coal seam) and mining cycle phase will be presented and discussed in this report. The findings highlight data-driven experimental evidence that affect the interaction between gas wells extraction and mining operations that may lead to potentially hazardous environments.

The purpose of this paper is to highlight the environmental benefits of properly managing slurry waste as it is generated on site. The following projects will be used to illustrate the importance of proper onsite waste management. Project: Jet Grout Backflow Dewatering The goal was to dewater the backflow directly onsite to produce a dry, solid waste and reusable water. The backflow spoil was pumped directly to the dewatering system as it was generated. Utilizing mechanical separation equipment and flocculation polymer the waste volume was reduced by 44%. Due to the site footprint and high volumes, slurry waste removal and onsite processing were the only two feasible options. Onsite dewatering proved to be the environmentally beneficial option as it eliminated the need for slurry waste hauling and disposal. The benefits of utilizing a dewatering system are listed below: Project: Soil Mixing Spoil Recycling for Slurry Wall Construction In this application the spoils generated contain bentonite mud, cement mixture and organic ground soils. The goal of this project was to process the backflow mixture to recover reusable bentonite mud and separate the soil as a dry, stackable waste product. The treatment system replaced the traditional method of vacuum truck extraction and facility disposal. The environmental benefits of properly handling the backflow is a decrease in fresh water usage, bentonite additive reduction, and the reduction of CO2 emissions. The paper will consist of a project scope, technical equipment and process description, environmental impact reduction calculations and lessons learned for each project.

This paper describes some of the results of previously unpublished research studies conducted in underground operations in Chile and Australia, where cross-hole seismic measurement profiles were interpreted in terms of dynamic modulus. This work dates from a period between 1987 and 1990 and the Author suggests that 30 years on, applying modern day technology, instrumentation and modelling methods, more meaningful, practical and applicable results may be obtained.

The most striking innovations on diaphragm-wall construction plant are those associated with continuous real-time trajectory monitoring of the excavating tool (either a grab or a hydro-cutter). The recent equipments are fitted with clinometers and gyrometers to control deflection, rotation or deviation from the vertical, thus allowing to install diaphragm-walls with a verticality accuracy of less than 0.3%. One of the greatest advantages of extensive use of on-board IT systems on the Hydrocutters and hydraulic grab machines is that it aids quality control by automatically producing complete real-time reports of all excavating data (verticality, depth, type of ground, rate of progress, stoppages, etc.). These reports are available to the Engineer, providing the total transparency required for Works Supervision and the Quality Assurance Plan. They also provide the Contractor with input data for databases used for optimising efficiency and progress criteria on future jobs.

In recent years, the mineral applications dealing with more complex data in distributed environment have grown rapidly. Many mineral and mining organizations have to organize internally and globally distributed manufacturing and experiment sites, which share the roles of databases and other multi-services like software programs. Thus, there are commercial and technical pressures pushing the mineral software vendors to move their current service toward Web service. XML has gained wide acceptance for representing, storing and transferring structured data and documents of many kinds, especially in scientific communities. However, so far there is no such a Markup Language in mineral industry. A XML-based uniform encoding, which is called MineML(Mineral Markup Language), is proposed and developed in this project. The framework of the MineML specification is created. MineML will play the role as shared protocols within mineral domain for archiving, transferring the multi-nature data between the software packages on the desktop over the Internet and sharing the services between users at different sites. Inspired by MLA’s (Mineral Liberation Analyser, JKMRC, University of Queensland, Australia) Mineralogical Data Presentation Program, a proof-of-concept prototype of Web Service is developed here to validate and demonstrate the MineML specification. Certain strategies, specific technologies and tools are adopted to create such an environment. To accomplish that, a set of experimental MineML documents including correlated XML, XSD (schemas) and XSL(stylesheets) documents have been elaborated for archiving, presenting and transferring the mineralogical data information, with the persistence on flexibility and extensibility for generic mineral and mining data handling.

Capacity data represent announced installed and planned capacities and do not necessarily reflect a U.S. Bureau of Mines judgment of actual mine operating levels or timing of planned expansions. "During the previous decade, the role of spray-on liners within the mining industry was limited to providing a corrosion resistant coating to screen and bolts; however, increasingly, liners are being used as ground support products. Expanding use and testing of liners have demonstrated their ground support function, yet the definitive support mechanism is still under debate. The most promising theory is the retain/hold/reinforcement concept (Kaiser et al.,1992). This conceptual theory has been the basis for the recent underground trials at INCO where, specifically, shotcrete and MIROC Mineguard were tested in two trials to further define limitations and applications of each liner.In a cursory overview, shotcrete is becoming accepted as a support product, while Mineguard is only slowly gaining acceptance.This discrepancy is the result of the corresponding life-spans within the industry, whereby shotcrete has had wide-spread use for decades, yet Mineguard has only recently been introduced. Currently at INCO, shotcrete is primarily used: to provide support in poor ground conditions, to add stability when developing through backfill, and to provide damage control in seismically active mines. It is also used to protect production headings and secondary-blasting chambers from fly-rock damage during blasting. Despite an obvious support function of the liner, shotcrete has always been used in addition to bolts and screen. Comparatively, Mineguard is still in the testing stage, where the greatest potential is increased productivity for applications in multi-tasking development headings. In this scenario, Mineguard provides instant support to replace screen, and bolts temporarily. When the heading has advanced, the drill and support crews work in tandem, drilling off the next round and installing bolts for long-term radial support, respectively. Other potential applications of Mineguard are within production headings to replace screen in the back, and screen and bolts in walls. The potential for both liners to replace conventional support is clearly war-ranted, such that defining the support role was the primary goal of the two underground trials."

"The Selmon Expressway includes an elevated structure extending across several miles of weathered karstic limestone geology, and represents a challenge for design because of the high variability of the material and the difficulty in characterizing the strength. The nature of the epikarst in this region of Tampa is such that variability can be extreme within distances of only a few feet. The weathered rock includes extremely soft soil infill within solution features on and within the formation. The limestone is so weathered that coring typically does not yield useful samples for strength testing, and in-situ Standard Penetration Testing (SPT) tests often encounter refusal blow counts. Thus the challenge for foundation design includes two components of reliability risks: the uncertainty associated with the ground conditions at a given foundation location plus the uncertainty associated with the foundation performance at a given location even when the stratigraphy is determined.This paper describes the design approach that has been implemented successfully for two projects at the Lee Roy Selmon Crosstown Expressway (LRS) in Tampa, Florida: the assessment and remediation of the reversible elevated lane (REL) foundations constructed in 2004, and the elevated expressways widening project constructed in 2012. Included in this design approach is a method developed for computing axial resistance in this local geology. Design for axial loading relies primarily on the development of side resistance, which was considered less subject to performance risks than base resistance. The calculation method incorporates a statistical analysis of SPT data, tempered by engineering judgment, to characterize stratigraphy at a given location into either soil, intermediate geomaterial (IGM) or rock.IntroductionThe subsurface stratigraphy of sites underlain by weathered limestone formations can be highly variable with respect to material layer thicknesses and strengths, even over short horizontal distances. A mathematical model was developed to assess the costs for reclamation of surface mining using the shovel overcasting operation, the front end loader operation, and a backfilling operation. A literature review was made of the present laws in Kentucky, Pennsylvania, Virginia, and West Virginia pertaining to surface mining and of the various parameters that influence the input data for a mathematical model. It was determined that the working mathematical model would have to be applied on an area to area basis, preferably on a permit by permit basis in the area where surface mining was being done. To this end, a flexible model was developed with input data obtained from the area of study. The results of the study show that the cost per ton of coal for reclamation can be estimated for various reclamation levels. Validation of the model was done for selected permits from Virginia. It was determined that the permits could easily be used for input data to the model.

The appropriate design of liners continues to be an issue on many mills worldwide. Loss of milling capacity especially at liner change, uneven wear lives along the mill, substantial scrap on worn liners, excess mill down-time, undesirably short and unreliable wear lives are all common issues. This paper presents a designed approach to liner optimisation that follows a logical path of initial design and step-wise improvements based on liner wear monitoring. The target is to have a good liner design from start-up, then tune the profiles, heights and thicknesses based on wear monitoring. This is conducted as a team of liner specialist, site personnel and liner supplier. The application to the Pustynnoye SAG mill of JSC Altynalmas in Kazakhstan is presented. This job was done in Morristown, NJ, with mono fluid system jet grouting for the construction of a new bank building. After the demolition of the existing two story structures, an excavation in loose sandy fill materials was necessary in order to reach the new structure basement level. Due to this jet grouting was selected as a medium to provide both excavation support and permanent underpinning, minimizing the adjacent building settlements. These two adjacent buildings were partially underpinned with a "gravity wall" made with 2 and 3 rows of secant vertical and inclined jet grout columns. Besides the classical jet grouting control methods, during the jet grouting execution, all the drilling and grouting parameters were recorded by a proprietary system called "Slogger". Additionally, after the jet grouting execution, an innovative result control technique was used by means of the application of the same parameter recording system. Some of the production columns were re-drilled and a comparison was made between the obtained drilling parameters and those obtained for the same age test columns. Under the referred existing conditions and in comparison with the conventional underpinning techniques, this project demonstrated that jet grouting, with proper execution and result control, can be faster than them and can maintain the building structural stability and act, at the same time, as a retention system minimizing the existing building settlements.

Precise and data driven methods of predicting and modelling blast induced vibration intensity are created and applied under field conditions on a routine basis today. These models are fed with controlled field measurements of vibration pulses generated by controlled charges under known geometric conditions. With such models blasting and geomechanical engineers can estimate vibration intensity at established critical points in mine bench geometry (surface mining) as well as within semi-permanent crown and rib pillars (in underground mining).Damage to bench walls and destabilization of large-scale geomechanical structures can be partially attributed to high intensity blast induced stresses on the rock mass and structures that define important features such as safety catch berms and haul roads. In the underground environment, weakening of critical pillars in block caving or Long Hole Open Stoping(LHOS) operations may lead to ore sterilisation. Blast vibration intensity can be linked with physical alteration in critical rock mass volumes within themine. Often the potential for vibration damage is estimated by the calculation of a critical value of vibration intensity, derived from a consideration of laboratory measured rock matrix properties. Efforts to confirm the accuracy of these estimates of critical vibration level include post blast excavation photography as well as the visual examination of rock mass, pre-and post-blast, using video camera observation of the walls of purpose drilled observation holes. These methods clearly indicate a change in fracture state due to the blast induced vibration, however further modelling of geomechanical stability will benefit if seismic measurement data can be interpreted in terms of geomechanically relevant strength parameters.
