Estimating the probability of failure for embankments is not intuitive. It requires not only an appreciation of the strength of the embankment, but an understanding of the variability and uncertainty in the strength parameters as well. These additional aspects are, in themselves, relatively new to most practicing engineers, and therefore require some discussion.This paper discusses the concepts of probability and uncertainty, and presents a reliability-based numerical experiment, together with a literary study and collation of some of the author’s historical slope stability analyses, to investigate whether a direct relationship between safety factors and probabilities of failure exists for certain levels of uncertainty in strength parameters. In doing so, a useful tool is presented whereby knowing the safety factor of a simple slope or embankment with a consistent level of uncertainty, one can infer an approximate probability of failure.CITATION:Gover, S M, 2018. Os aços inoxidáveis superduplex possuem em sua constituição as fases austenita e ferrita em proporções semelhantes, esses aços apresentam propriedades mecânicas oriundas da combinação dessas duas fases. As fases sigma (s) e austenita secundária (g2) surgem nesses aços, de uma possível reação de decomposição eutetóide da ferrita (a ? s + g2). Este trabalho estuda as transformações de fases, a caracterização das microestruturas do aço inoxidável superduplex UNS S32750, através de microscopia óÓptica, ferritoscopia, quantificação de fases, evolução da dureza Vickers do material, e a precipitação da fase s após tratamento térmico de recozimento a 800ºC em tempos variados. Essa fase é prejudicial ao aço, pois diminui a resistência à corrosão do mesmo e o fragiliza, sua formação ocorre entre 600ºC e 1.000ºC. Foi estudada a evolução da fase s através do método do caminho microestrutural proposto por Vandermeer, geralmente aplicado a recristalização e não em estudos de precipitação da fase s. Os resultados indicam que a nucleação da fase s ocorreu por saturação de sítios.

"Canopy air curtains on roof bolting machines have been proven to protect miners from respirable dust, preventing their overexposure to dust. Another desired application for canopy air curtains is in the compartments of shuttle cars. The challenges faced in developing the design of canopy air curtains for shuttle cars include mine ventilation rates in tandem with the shuttle car tram speeds. The resulting cab airspeeds may exceed 182 m/min (600 fpm), as found in the present study conducted in a central Appalachian underground coal mine by U.S. National Institute for Occupational Safety and Health (NIOSH) researchers. Prior research and laboratory testing had indicated that successfully protecting a miner in high air velocities is difficult, because the clean air from the canopy air curtain is unable to penetrate through the high-velocity mine air. In this study, the dust concentrations to which a shuttle car operator was exposed were measured, and air velocities experienced by the operator were measured as well using a recording vane anemometer. The results indicate that the highest exposure to respirable dust, 2.22 mg/m3, occurred when the shuttle car was loading at the continuous miner, where the average airspeed was 48 m/min (157 fpm). While tramming, the operator was exposed to 0.77 mg/m3 of respirable dust with an average airspeed of 62 m/min (203 fpm). This study indicates that a canopy air curtain system can be designed to greatly reduce an operator’s exposure to respirable dust by providing clean air to the operator, as the majority of the operator’s dust exposure occurs in air velocities slower than 61 m/min (200 fpm). IntroductionThe U.S. National Institute for Occupational Safety and Health (NIOSH) issued contract #200-2015-63485 to develop a canopy air curtain for coal mine shuttle cars with Marshall University and J.H. Fletcher & Co. (Salem, Begley and Ross, 2016). The proposed design maintains a design similar to that of a roof bolter canopy air curtain by providing filtered air through a blower over the operator. The plenum, which will provide uniform airflow over the operator, is anticipated to be built into the shuttle car canopy. One of the main interferences with a canopy air curtain is ventilation airflow perpendicular to the plenum airflow (Engel, Johnson and Raether, 1987). This ventilation airflow can shear the downward flow from the plenum, and this shear can reduce the effectiveness of the canopy air curtain by either disrupting the downward flow or allowing contaminated mine air into the canopy air curtain zone of protection."

"The Mine Improvement and New Emergency Response Act (MINER Act) of 2006 (PL 109-236, 2006) established the Office of Mine Safety and Health, functioning as the Office of Mine Safety and Health Research (OMSHR) within the National Institute for Occupational Safety and Health (NIOSH) and provided authority for awarding contracts related to the development and implementation of new mine technology and equipment. The OMSHR external contracts program was formed in accordance with the MINER Act and has since issued more than 150 contracts focused on worker health and safety. Initial funding for this program was provided by the United States Congress through two emergency supplemental appropriations and was focused on emergency oxygen supply, communications and tracking, and refuge chambers (also referred to as refuge alternatives). Since the MINER Act, Congress has continued to fund the program through the annual NIOSH mining appropriation. The program was supplemented starting in 2009 with an academic capacity building program directed toward developing university tenure-track faculty and facilities and supporting graduate students in the area of mine ventilation. Ground control was added as an additional focus area in 2011. A short synopsis of each contract is published on the NIOSH Mining website (www.cdc.gov/niosh/ mining/researchprogram/contracts/index.html) and final reports are available in digital form on request. A summary of the progress OMSHR has made over the 10-year period since the Act was published previously (Snyder et al., 2016).External contracts are broadly classified by the following areas:• Communications and tracking.• Emergency response.o Self-escape and breathing air supply.o Refuge alternatives.o Escape and rescue.• Equipment safety.• Fires and explosions.• Ground control.• Mine environmental systems (mine ventilation and atmospheric monitoring).• Worker/workplace health and safety.Contracts are issued under a request for proposal (RFP), through a broad agency announcement (BAA) or under an interagency agreement (IAA). An RFP solicitation includes a specific problem description and a well-defined scope of work to guide the offeror in preparing a bid to conduct the research. Under the BAA several problem areas are described and the offeror develops the scope of work and deliverables for the proposed research. An IAA is very similar to an RFP except it is a contract between two federal government agencies."

"Much of the copper being produced today uses heap leaching, where pure metal is extracted from the ore with a chemical process rather than smelting. This technique is deployed extensively in major copper producing areas including Chile, Peru and the southwestern United States.Copper ore is excavated from the ground, crushed and then spread in uniform layers on a pad. A slow trickle of sulfuric acid is distributed over the pile, where it percolates to the bottom and a collection point. The acid leaches out the copper, along with other minerals, which can be extracted chemically. This process can be used at large sites producing 100 kt/a (110,000 stpy) and greater.Production on a massive scaleProduction at heap-leach sites is continuous and can operate on an enormous scale. The ore is fed into a crusher to break down the large pieces, and then it moves into an agglomerator to capture the fines and create more uniform particles. While in the agglomerator, the ore is mixed with acid for the first time, helping it to bind the fines into larger particles and starting the leaching process. The treated ore moves by conveyors to a stacker designed to spread it uniformly on the pad.The size of the operation comes into perspective by considering a typical pad measures 1.6-km (1-mile) long and 0.8-km (0.5-miles) wide. Working day after day, the stacker creates individual piles or modules on the pad, each 122 x 61 m (400 by 200 ft) and 3 m (10 ft) deep. When one module is filled, it moves across the pad to the next, filling the area in one continuous pile. A completed row of modules spanning the width of the pad is a cell. Once the entire pad is covered, the process begins again laying down another 3-m (10-ft) layer, and it continues until the entire pad reaches its height limit which can be several hundred feet.Once a new module is completed, technicians lay a network of plastic pipes over the surface in a pattern similar to agricultural drip irrigation installations. The diluted sulfuric acid, called raffinate, is pumped through the pipes so it can be distributed over the module, allowing the acid to soak through the ore and leach out copper as it percolates to the bottom. At the same time, approximately 35 to 45 percent of the acid is consumed by dissolving the calcite carrying the metal in the process. The raffinate reaching the bottom is now characterized as pregnant-leach solution (PLS) since it carries the copper the company is seeking to recover."

"Mining is an inherently risky business. Over the years, major mine disasters have resulted in the deaths and injuries of thousands of miners in the United States. These disasters also resulted in innovations in approaches to mine design, operations and the training of miners and mine rescue teams. The U.S. Congress responded to these disasters with new laws designed to create mandatory safety and health standards as a means to eliminate fatal accidents; to reduce the frequency and severity of nonfatal accidents; to minimize health hazards and to promote improved safety and health conditions in the nation’s mines.Following a mine emergency preparedness and response holistic gap analysis in 2012, the U.S. Mine Safety and Health Administration (MSHA) identified the need for the development of risk and readiness assessment models for MSHA and the mining industry. MSHA’s objective was to evolve existing theoretical concepts for risk and readiness assessment into simplified tools that can be applied by the mining industry for use at operational levels. In 2012, MSHA hired ABS Group, a recognized industry leader in safety risk management and a wholly owned subsidiary of the American Bureau of Shipping, to develop self-assessment models to help meet this objective.In September 2012, MSHA chartered a project to supply the coal mining industry with a pro-active toolset for mine operators to:1. Self-assess the risks associated with underground coal mining operations in order to prevent major mine emergencies.2. Assess the preparedness of mine operators to respond to an emergency.3. Measure the readiness of mine rescue teams and responsible persons to execute emergency plans.MSHA’s objective was to evolve existing theoretical concepts for risk and readiness assessment into simplified tools that can be applied by the coal mining industry for use at operational levels."

"The City of Atlanta’s Water Supply Program Tunnel project includes a 7,315-m (24,000-ft) long, 4-m (13-ft) diameter, hard rock tunnel. A complex aspect of the project involves connecting five, blind bore shafts to the tunnel in a location close to current drinking water reservoirs. During the supplemental geotechnical investigation following the initial contract award, results from additional borehole geophysics were reviewed. Unfavorably oriented fracture sets forced a change in the original pre-excavation grouting program designed for that site. Real-time grout monitoring and geophysical data were compared to provide assurance that the program, as designed, correlated with the in situ ground conditions. Following grouting, results were modeled, and all grouting data were reviewed to determine if grouting was complete.Project backgroundThe current water supply system operated by the city of Atlanta’s Department of Watershed Management (DWM) consists of four, aged, raw-water pipelines, one of which dates back to 1893. Based on previous assessments completed by the DWM, the entire water supply system is at, or will soon reach, its recommended useful life. As such, the city acquired the Bellwood Quarry in 2006 with the intention to create a water storage facility with a volume of approximately 9.1 billion L (2.4 billion gal) to serve approximately 1.2 million people.The project location is shown in Fig. 1, which is generally in the northwest part of downtown Atlanta, GA. The overall project has been divided into two phases. The Phase 1 project connects the quarry and the Hemphill Water Treatment Plant (HWTP), and the Phase 1 Extension project connects the HWTP to the Chattahoochee Water Treatment Plant (CWTP) and the Chattahoochee River. A 7,315- m (24,000-ft) tunnel with a finished diameter of 3 m (10 ft) connects all three elements. The HWTP location is also where the city’s two most proximate drinking water reservoirs are located."

"After industrial sand has been mined and processed, the finished product is typically loaded into small bags of 45 kg (100 lb) or less, large bulk bags of 454 to 1,361 kg (1,000 to 3,000 lb), or vehicles such as trucks or trains for transport to end users. As the sand is being transferred and loaded, dust can be released into the work environment, potentially exposing workers to respirable crystalline silica. A number of control technologies have been developed and utilized in an effort to reduce dust liberation during loading operations. For bulk loading into trucks or trains, the U.S. National Institute for Occupational Safety and Health (NIOSH) evaluated one of these technologies, the Dust Suppression Hopper (DSH), at two industrial sand processing plants. Results from these case studies show that the DSH reduced airborne respirable dust levels by 39 to 88 percent, depending upon the product size being loaded.IntroductionInhalation of respirable crystalline silica dust can lead to the development of silicosis, a disabling and potentially fatal lung disease. Once contracted, there is no cure for silicosis, so the goal is to prevent the development of the disease by limiting exposure to respirable dust containing crystalline silica at or below the exposure limit of 50 µg/ m3 recommended by the U.S. National Institute for Occupational Safety and Health (NIOSH, 2002). At industrial silica sand operations, the product contains high levels of crystalline silica, so effective control technologies are needed to minimize the respirable dust exposure of workers.NIOSH, in partnership with the Industrial Minerals Association–North America, published a dust control handbook (NIOSH, 2012) focused upon control technologies for industrial- mineral mining and processing operations. One chapter in this handbook discussed technologies for controlling dust liberated during bulk loading, including telescoping loading spouts, dust collection systems for loading spouts, enclosures for load-out areas and a newer technology known as Dust Suppression Hopper (DSH)."

It was a hardwearing slurry pump that used replaceable liners and a replaceable impeller, something that had never been done before.“Charles Warman was a visionary and an innovator. The use of rubber lining in the pump was groundbreaking as it reduced total ownership costs, was safer to handle and offered extended wear life in many abrasive slurry applications. It is a great example of solving customer problems with material science, and is still taught in universities today,” said Marcus Lane, global product manager for centrifugal pumps at Weir Minerals.It was quickly adopted throughout the goldfields in Western Australia and by the 1950s, as customer demand rose, Warman expanded his business by opening offices in Sydney and Perth.During the 1960s the rapid growth of the business continued with manufacturing and sales outlets established around the world. "The majority of tunnels for civil engineering applications are now being constructed using some form of mechanical excavation. Beginning in the 1960s with rock tunnel boring machines (TBM), the tunneling industry has introduced both earth pressure balance (EPB) and slurry pressure balance (SPB) soil machines; mixed soil and rock machines and a variety of different mechanical devices for the construction of small diameter tunnels. Over time, these machines have become more powerful and more adaptable to a wider variety of ground conditions; so much so that tunnels are now being constructed in ground conditions and in the vicinity of third-party impacts that would have been considered beyond the state of the art just 10 years ago. Figure 1 is an example of a mixed ground crossover TBM used successfully at Mexico City’s Túnel Emisor Poniente II.All of the above is highly advantageous for the tunneling industry, but it has also placed a much higher level of risk on the performance characteristics of the tunneling machines, on the contractors operating those TBMs and on the manufacturers of those machines. Most of the risks for a tunneling project are associated with creating the space inside of which the finished facility will be constructed. In order to create that space, the tunnel contractor must make many decisions about the best way to excavate the ground, the best way to control the ground at the face of excavation and the preferred method for supporting the ground around the tunnel in a manner that is safe for the workers and stable with respect to all of the overlying and adjacent existing structures. If it is proposed to use some form of TBM in order to build the tunnel, then the TBM becomes central to all three of these activities and becomes an integral part of managing the risks associated with these activities."

"The last few decades have seen some remarkable advancement in information technology, but recently various digital technologies have resulted in several synergies to deliver innovation in diverse applications. The mining industry has been relatively slow in adopting these technologies compared to the other industries, but this scenario is changing fast.Challenges. Mining companies globally are going through a series of challenges such as declining ore grades with complex metallurgy, increasing depth of ore bodies, high capital and operating costs, lower productivity, shortage of technical resources, significant pressure from stake-holders to deliver, along with growing environmental and corporate social responsibility issues. Improvement in profitability would need significant improvements in operational performance.Consequently, over the last few years, there has been a major focus among some mining companies to imbibe operational excellence. This has been only possible because of the replacement of a silos-based approach by an integrated view of the organization. Such an integrated view is now possible via the evolving digital technologies. Even so, such a transformation calls for a change in culture, which, for most organizations is not trivial. Attempts in the past to integrate functions, such as through mine-to-mill initiatives in the mining industry, have demonstrated significant benefits to many operations (McKee, 2013). Still, sustaining these benefits has clearly been a challenge due to manpower turnover and lack of proper systems and structures that integrate information between business functions within an operation and between operations and the corporate level.Opportunities. The advent of digital technology has opened up opportunities to integrate interdepartmental information for a unified understanding across an entire organization. The advances in instrumentation, equipment control systems, supervisory control systems (SCADA/ Historian) and industrial networks have helped in the direction of enterprise- wide integration. The availability of real time information across an enterprise has given rise to numerous possibilities to improve organization performance. "What is the primary responsibility of geo-engineers in mining? Early in my career, I was asked this question by an experienced engineer. After explaining some various tasks, I was informed that the main responsibility was to monitor. Although this proved true for many cases, I came to understand that monitoring is merely one means to achieve the primary responsibility of a geo-engineer — ensure the safety of all personnel. The potential for groundwater to impact stability allows this responsibility to be shared with hydrogeologists as well. Inherently, we monitor to identify risks; we analyze designs and develop models to mitigate risks; and we are provided opportunities to explain to supervisors the geo-risks to personnel and mine plans that will result from particular courses of action. A couple years ago, I took an opportunity to share experiences from the first several years of my career as a geotechnical engineer with junior geoengineering staff within my company. The intent was to share situations that were a part of my development, so they could learn and understand their responsibility to the safety of all personnel. Similarly, the purpose of this article will be to share these thoughts with new geo-engineers in the industry in order to help prepare them for the challenges they will face.What is our purpose?A senior hydrogeologist shared an experience that occurred in their career. The experienced hydrogeologist had spent some time with the hydrogeology staff, most of them relatively new to the mining industry, to understand how they viewed, and carried out, what they perceived as their responsibility at an active mining operation. After a couple months of observing and mentoring, the senior hydrogeologist met with everyone and posed the question “What is our purpose?” I am sure that most junior geo-engineers would start explaining different tasks or projects that they must complete. The list would include monitor ground water levels, track slope movement, evaluate designs, build models, oversee drilling programs, etc. These individuals did as well their supervisor. After going around the room allowing each individual to answer, the experienced hydrogeologist provided them the real answer to the question: Safety. If you examine the root intent of nearly every responsibility geotechnical engineers and hydrogeologists have, it is to apply our professional skills to ensure the safety of our workforce. When the objective of safety from the geotechnical and hydrogeology disciplines is achieved, inherently accomplishing the mine plan should follow suit, or it should significantly minimize the impact by gravity or ground water."

"Mineworkers are routinely tasked with making critically important decisions about whether or not a hazard presents an imminent danger. Researchers from the U.S. National Institute for Occupational Safety and Health collected formative data to investigate mine safety professional perspectives on workplace examinations, which revealed a potential gap in how mineworkers are assessing risk. During interviews, participants revealed not having a systematic methodology for mineworkers to use to determine if a hazard is considered imminent danger. In this paper, we identify and describe three distinct categories of imminent danger complexity and discuss potential steps that could lead to improved identification of imminent danger situations. Finally, we identify potential practices to incorporate into risk management efforts, including feedback, communication and specialized training, to increase awareness of imminent danger situations. IntroductionIn recent years, the metal and nonmetal mining sector, as categorized by the U.S. Mine Safety and Health Administration (MSHA), experienced an increase in the number of fatalities occurring at mine sites. From 2013 to 2015, 69 mineworkers were fatally injured, twice the number of fatalities in each of the previous two years (MSHA, 2015a). As a way to address this increase, MSHA issued its P15- IV-01 program policy letter, suggesting that “miners would benefit from rigorous workplace examinations conducted by experienced and trained examiners” (MSHA, 2015b). The goal of these examinations is to find hazards in the field and mitigate them before they cause injury or death to mineworkers. While it is important to identify all hazards in the workplace, it is critically important to identify those hazards that are considered imminent danger because they require an immediate response to avoid a severe or even fatal injury."

"The National Mining Hall of Fame and Museum (NMHFM) announced the 2018 National Mining Hall of Fame inductees. This year’s inductees, selected by the National Mining Hall of Fame’s Board of Governors, represent exploration, extraction, management, education and research and development in the U.S. mining industry. Silver will join 240 other mining industry luminaries when formally inducted into the National Mining Hall of Fame on Sept. 29, 2018. Guilbert was a renowned geologist and professor of economic geology at the University of Arizona; he had a distinguished 50-plus year career in academia and business. Guilbert, together with J. David Lowell, worked on the most important breakthrough in mineral exploration in the last 100 years, the Porphyry Copper Model. This work, known as the Lowell and Guilbert Model, was utilized by exploration geologists worldwide over the next four decades to discover hundreds of millions of tons of copper metal and numerous profitable mines including the prosperous Bajo de la Alumbrera copper mine in Argentina. Guilbert was a founder of The World Museum of Mining in Butte, MT. He was the recipient of countless awards including the Society of Economic Geologists (SEG) R.A.F. Penrose Medal as well as the D.C. Jackling Award from the SME.Dr. Leo J. Miller was a maverick in the mining industry and no armchair geologist. He believed that active fieldwork, on foot with a rock pick, was indispensable for exploration. His basic tenet was to absorb as much information about an exploration target up front, formulate ideas – sometimes novel ones — and then get out into the field to prove those ideas. Miller was known to diverge from the standard interpretation of the geological processes, which formed the mineral deposits, and to apply innovative applications of high-tech tools to more usual exploration programs. This approach resulted in a 50-plus year career that included the discovery of the Kidd Creek massive sulfide deposit and the development of the Lee Creek phosphate deposit; which other explorers had previously given up on, believing whatever phosphate might be present would be too difficult and expensive to mine. The recipient of many honors and awards, Miller received the SME Robert M. Dreyer award in recognition of outstanding achievement in ore discoveries."

"It has been nearly eight years since rare-earth elements (REE) became the subject of frontpage headlines. It was when the controversial Chinese export policy for these critical commodities was epitomized in a maritime border dispute with Japan in September 2010 that resulted in the big REE crisis and price spike of 2011 (Barakos et al., 2016c; Mancheri, 2015).The world was dismayed, especially in REE-importing countries such as the United States that was, and still is, totally dependent on Chinese production and exports (Barakos et al, 2016c; Kennedy, 2015). The short-lived alarm initiated a treasure hunt by way of exploration for REE deposits all over the world. The continuously growing demand on the one hand, and the Chinese sovereignty of the REEmarket on the other, led the rest of the world to explore their own REE resources. In just a few years, more than 400 projects were initiated to explore REE deposits outside of China (ERECON, 2015; Goodenough, 2016).The prices of REE, however, dropped rapidly in late 2017 and were in six-year lows, before the 2011 crisis (Barakos, 2017). The viability of even major established producers is being challenged, not to mention the development prospects of potential REEmarket entrants (Barakos et al., 2016b; Rollat et al., 2016). Production cutbacks and industry consolidation in China have impelled REE market experts to forecast that prices have already reached their lowest point and have nowhere to go but up (Adamas, 2015; Roskill 2015). Yet, expectations for increased REE prices are limited and drastic changes are unlikely to happen in the near future, as there is still excess supply in the overall REE market (Barakos, 2017). Nevertheless, the low prices of rare earths are not the only reason that potential producers are struggling to enter the global REE market. Among other parameters, the separation process of the individual elements seems to be the most important factor, as it is technically challenging and expensive to build and to operate."

"Diesel particulate matter (DPM) is an occupational health hazard in underground mines. It generally occurs in the submicrometer size range and is often present in the mine atmosphere with significant concentrations of dust particles that tend to occur in the supramicrometer size range. As dust can interfere with analytical methods to measure DPM, it is often removed from a sample stream using an impactor-type size selector such as the DPM impactor (DPMI). Because the DPMI physically removes oversized particles from the stream, its performance may be gradually reduced with aging. Sharp-cut cyclones (SCCs) are an alternative size selector for DPM sampling applications, with a major advantage that, by design, they should not be susceptible to rapid aging. This paper presents the results of a field study designed to compare the performance of aged versus new/clean DPMIs and SCCs in an underground mine. DPMI aging resulted in clogging of the device and, eventually, a reduction of its effective particle cut size, but when sample flow rate was maintained, DPM sample mass collection was not affected until significant aging had occurred. Under the conditions of this study, the effects of SCC aging were observed to be minimal at the end of the study period. IntroductionSubmicrometer-sized particles are increasingly recognized as significant respiratory health hazards (Cantrell and Watts, 1997; Kenny, Gussman and Meyer, 2000; Ristovski et al., 2012). Diesel particulate matter (DPM) represents a major source of submicrometer particles in both public and occupational environments (Kittelson, 1998; Abdul-Khalek et al., 1998). Relative to other occupational groups, underground miners can have some of the highest DPM exposures due to their work in confined spaces where large diesel equipment is used (U.S. Environmental Protection Agency, 2002; Grau et al., 2004). DPM largely consists of solid elemental carbon (EC), commonly known as soot, and organic carbon (OC) that may be sorbed to EC particles (Kittleson, 1998; Abdul-Khalek et al., 1998). EC and OC can be measured by thermal-optical methods such as the U.S. National Institute for Occupational Safety and Health (NIOSH) 5040 standard method (Birch, 2016), and their sum is referred to as total carbon (TC). While the complex nature of DPM does not allow for its direct measurement, both TC and EC have been accepted as suitable surrogates for monitoring occupational exposures to DPM (Noll et al., 2007; Birch, 2016)."
