The Beverley uranium mine û now in production in the remote north of South Australia at1000 tonnes per annum û was established to meet a projected, but little-recognised shortfallin primary supply of uranium for the nuclear industry in the early part of the 21 st century. Inthe context of AustraliaÆs lack of bi-partisan support for uranium mining and the nuclearindustry, a four to five year window of opportunity existed from 1996 to establish the mine.During this period, Beverley achieved a number of firsts including: the first ISL mine inAustralia; the largest ISL mine in the world and the first to produce more than 500 tonnes ofuranium in its first year; the first to be established under a new Native Title regime in SouthAustralia; and the first to be established under a revised South Australian Mining Act.Beverley was brought into production against the background of: hostile opposition fromanti-nuclear activists; a suspicious and at-times hostile media; a regulatory and politicalcommunity that was unfamiliar with ISL mining; commercially unsophisticated Aboriginalgroups; an inexperienced engineering industry; and the vagaries of development in a remote,semi-arid location with neither population nor infrastructure. The lessons that can be learnedfrom Beverley showed that û like much in life û flexibility and timing are crucial.

"‘Gas hydrate’ is the common term for a solid material formed from the combination of various gases as ‘guest’ and water as ‘host’. A series of large-scale scientific drilling and coring programmes conducted since 2000 revealed that natural gas hydrate exists in a wide variety of accumulations in the sediments. The scaled-up onshore and offshore field production tests during the last decade show that gas production technology will be realized to make gas production from gas hydrates viable in the future, recognizing its potentials as one of the future sustainable energy resources.The total produced gas was approximately 470 m3 by the hot water circulation method over 6 days of operation in the permafrost-associated Mallik 2002 project in the Mackenzie Delta of Canada. In the Mallik 2007-2008 project, the gas was successfully produced stably by the depressurization method for 6 days, up to 13,000 m3 cumulatively. The depressurization method applied in the Mallik test was shown to be an effective way to produce gas from gas hydrates. The Alaska field trial to inject mixed gas of CO2 and N2 to exchange CH4 in 2012 was successfully done for the first time to produce maximum 1,270 m3 per day. A remarkable achievement was made by Japan when it completed its first offshore production test in the Eastern Nankai Trough, producing approximately 120,000 m3 of methane by the depressurization method over 6 days in March 2013. The technical challenges and scientific uncertainties obtained from Nankai Trough production test provide Korea with more considerations as to the aspects of well completion, reservoir formation and seafloor stability, sand control, flow assurance, environmental monitoring, and etc., due to the different geological setting and geomechanical properties of the Ulleung Basin in Korea."

The Vale Ferro Carajßs S11D project in northern Brazil is based on the movement of unprecedented volumes of waste and ore for mobile in-pit crushing and conveying or ætrucklessÆ systems. In peak years, the system is intended to handle a combined 160 Mt of material.The planning and implementation of the first such system for Vale, in a remote greenfield site, utilising the services of a range of international study, design and supply partners has provided a unique set of technical and project delivery challenges. The lessons from these are of value to the industry as the trend to remote and high-volume operations continues.This paper describes the key features of the mining system and documents the challenges and mitigations for important technical, cultural and project delivery issues that have arisen. On June 14, 2023 Jay Dee Contractors’ Tunnel Boring Machine (TBM) “Phoenix” holed through and completed mining the second drive of the I-75 Segment 3 Storage and Drainage Tunnel. Completion of the tunnel came only after overcoming several challenges, including a groundwater intrusion event which deposited thousands of cubic yards of wet ground material into the tunnel. This paper will discuss the events which delayed completion of the tunnel by over nine months, the risk management and construction approaches assumed by the Design-Build team during bid, several challenges faced by Design-Build teams when building stormwater resilience tunnels using traditional Department of Transportation standards, and recommendations for improving risk allocation and proposal documents for future similar Design-Build tunnel projects.

Microtunneling is rarely a straight forward “business as usual” endeavor. More often than not, these projects take twists and turns that require on the fly, out of the box solutions to get the project completed. This paper is a case history of a 63-inch diameter, Permalok, microtunneling project, extending nearly 700 linear feet under a hill forming the left-hand abutment to an earth embankment tailings dam. The job had three major and unexpected challenges that required unorthodox solutions. Challenge #1— Face Configuration: The Santiago Formation, a cemented sand formation with widely varying strengths, presented a challenge to configuring the face with regards to the number and length of pics and cutters. Subtle differences in type and size of the tooling had a big impact on the performance of the machine. Challenge #2—Eccentric Loading: Issues at the launch shaft with the Permalok pipe adaptors caused eccentric loading on the casing pipe. This issue resulted in overstressing and breaking of several pipe joints that ultimately led to seizure of the tunnel. Challenge #3—Back-tunnel Excavation and MTBM Recovery: There were many detailed considerations into excavating a recovery tunnel to retrieve the MTBM, especially being under a high hazard earth embankment dam.

This case history for three major additions to a tire manufacturing plant in north Georgia discusses the impact of highly variable Karst (sinkhole) geology on both foundation design and construction. For Phase 3, alternatives were considered for a heavily loaded mixing tower with up to 2,000 kip column loads. A non-traditional approach of using auger cast piles was adopted. To satisfy a risk-averse Owner, a pile load testing program including strain gauges was conducted. One test pile was cored full depth to demonstrate effective bearing on steeply inclined rock. For Phase 4, similar methodology was used at another critical mixing tower structure associated with that phase and the installation procedure was refined to reduce grout take. In Phase 5 construction, another mixing tower and the equipment pits were supported on auger cast piles. After pile installation, a sinkhole opened up between two equipment pits. Remedial grouting was performed. Development also required construction of a R&D building over a 96 inch diameter RCP storm sewer. Auger cast piles were used to support the building to avoid overstressing the existing pipe and risk separating joints. This case history outlines lessons learned while constructing auger cast piles in Karst and proving quality was built into the foundations.

It is always necessary to expand existing buildings, so that they could meet the growing societal needs. Compared with demolishing them and then constructing new ones, the expansion of existing buildings could not only reduce project cost, but also save the use of energy and raw materials in projects. Therefore, it is widely adopted in most under-developed countries. However, the expansion of existing buildings is not an easy task. It has been involved with a lot of issues in defining the scope of work, planning landscape, managing construction activities, etc. This paper investigates the potential challenges in expanding existing buildings. The project of expanding the Al-Agoza Hospital, one of the biggest and oldest hospitals in Cairo, Egypt, is selected as a case study. In the case study, the project documents have been carefully reviewed. The project engineers and the health-care professionals working in the hospital have been interviewed. This way, the point of views from the owner, the architect/engineer, and the contractor have been taken into account. Several lessons are learned through the discussion with these people. The results from this paper are expected to provide pieces of useful advice and suggestion for the expansion of existing buildings in the future.

From many years practical industrial experience the author has drawn together a list of the key points to consider when embarking on a large-scale geometallurgical project. Fundamental aspects which must be considered include (but are not limited to):understanding the variability of the orebody both laterally and with depth;comminution properties and variability;flotation properties, requirements and variability – differing ore zones may have substantially different requirements for optimal performance;the role of texture – the consequences of both grain size and distribution and how the circuit will handle these;realistic expectation in terms of the financial requirements and time frame results are required within; andintegration of, and communication between key personnel within the various departments of the site (mining, geology and processing) is a fundamental aspect of any geometallurgically based processing system. An example of a highly complex nickel sulfide ore will be used as the basis of discussion.

The Waterview Tunnel comprises twin 2.4 km long 13.1 m internal diameter three lane road tunnels in Auckland, New Zealand. The City Rail Link (CRL) is a 3.45 km long, 6.24 m internal diameter twin rail tunnel underneath Auckland’s city center. This paper presents an overview of the tunnel geometry, segmental lining congurations, geometric constraints, performance criteria and functional reuirements for the large diameter Waterview Tunnels and smaller diameter CRL tunnels. Other key aspects, which include sustainability, innovations, construction performance and practical aspects are discussed and compared. Lessons learnt from these mega tunnelling projects are also presented.

The Sobek humidity cell has been in use with minor modification for more than 30 years to determine bulk primary-mineral reaction rates. As a result, it has become a key tool for predicting the bulk rate of sulphide oxidation, the Carbonate Mole Ratio (CMR) of neutralization-potential (NP) consumption to sulphide oxidation, and the rates of metal leaching. Most humidity cells are operated for less than a year, with the resulting rates extrapolated into the future for years or decades. The validity of these extrapolations was tested by examining longer trends from cells that have operated for three to seven years, as found in the International Kinetic Database (IKD). Additionally, large batches of cells for two minesites in the IKD (45 and 46 cells each) were examined to determine if regular, predictable trends were obtained across ranges of geochemistry at a minesite. The long-term cells show that the rates of sulphide oxidation and acid generation, based on sulphate production, have a 50% chance of stabilizing within one year, with the remainder fluctuating significantly throughout their test periods. Despite the fluctuations, the CMR, which is critical for defining site-specific or rock-unit-specific ABA criteria, remains relatively steady in many cases between 1.0 and 2.0 as expected from principles of aqueous geochemistry. However, if the sulphate-production rate falls below 10-30 mg/kg/wk, the CMR value can increase significantly. At and below these low rates, physical factors like flow rates of surface waters and groundwaters will determine the rate of NP consumption, rather than geochemical factors. Also, if weekly rinse pH falls to acidic values, the CMR will decrease below 1 .O, reflecting the exhaustion of fast- neutralizing minerals. The two batches of multiple cells also confirm these trends across arange of geochemistry. The long-term cells show that copper leaching often stabilizes within a narrow range as long as rinse pH remains relatively constant. Also, there is no strong dependence of leaching on the sulphate rate. However, the rate of copper production often increases sharply if rinse pH falls to acidic values. This suggests the measured copper-production rate in some cases is not a true primary rate, but a lower secondary rate after secondary- mineral precipitation within Sobek cells. As with the long-term cells, the batches of multiple cells show that copper production often increases gradually with decreasing rinse pH, with no clear dependence on sulphate production. However, high rates can also occur at near-neutral pH, so secondary-mineral precipitation of copper is not necessarily common.

Accidents involving powered haulage and mobile equipment such as haul trucks often account for the greatest number of fatalities in the mining industry each year. Despite previous analyses that have identified root causes and other contributing factors, there is still a need to better understand the events leading up to these types of accidents, what lessons may be learned, and what strategies can be employed to prevent fatal accidents from occurring. This study examines Naturalistic Decision Making (NDM) using the critical decision method (CDM). The CDM is a retrospective interview approach used to explore timelimited, high-stakes, decision-making. In this study the CDM is used to obtain more information about what happens prior to, during, and after a potentially fatal situation such as a near-miss event, loss of control, or minor accident involving equipment damage. Researchers captured firsthand accounts from 21 haul truck operators involved in near-miss events from mine sites of various sizes and commodities throughout the United States. These accounts provide rich and detailed narratives from the perspective of haul truck operators themselves and reveal insights into what decisions haul truck operators make, what sensory cues they perceive, and what strategies they employ during challenging and non-routine situations. Decision-related themes that emerged from the data are presented and discussed. These results, along with potential solutions offered by study participants, can help to inform future research, raise awareness about hidden hazards, and build more creative interventions and realistic training scenarios for use by the industry to address haul truck safety issues.

High Speed 2 (HS2) mandated that Building Information Modelling (BIM) processes be implemented during design and construction. Gall Zeidler Consultants (GZ) was responsible for developing the BIM Models for the portals, tunnels, and cross passages of the of the 16 km-long Chiltern twin-bore Tunnel. In addition, GZ developed the GBR for the Chiltern Tunnels and incorporated the relevant information into the Project’s BIM framework. This paper presents the implemented BIM framework, describes the modelling approach, and discusses the advantages and challenges of implementing BIM, particularly how BIM improved interface coordination and how BIM was used in feasibility assessments of different construction options.

During the 1980s and early 1990s, the South African coal industry experienced a series of methane explosions in which well over 200 coal miners lost their lives. In addition, a fire and an explosion in the gold mining sector claimed a further 240 lives. These tragic incidents resulted in the establishment of a research facility at Kloppersbos dedicated to explosion and fires research and the funding of an extensive research program, and almost certainly influenced the government of the day in its decision to establish, in 1993, the Leon Commission of Enquiry into Mine Health and Safety. The results of this focused research program, the wide-ranging recommendations of the Leon Commission and the determination of the coal mining industry to improve its safety record all combined to result in the past 20 years without a major explosion.This paper lists some of the research outcomes and, through three case studies, illustrates what can be learnt from investigations following explosions. The paper concludes with an evaluation of some of the findings of the Royal Commission on the Pike River coal tragedy. Many of the Commission’s conclusions are pertinent and relevant to the coal mining industry worldwide and their implementation could significantly improve coalmine safety; however, a careful study of the available facts indicates there could well be an alternative explanation of the source of the methane, the location of the ignition and the size of the explosion at the Pike River coalmine.CITATION:Phillips, H R, 2015. Upstream embankment raises often represent the lowest cost option for increasing the capacity of tailings storage facilities (TSF), but also pose the highest stability risk due to constructing on top of previously deposited tailings.This paper presents an overview of key aspects in the geotechnical investigation and design of upstream raises and some recent lessons learnt from experience across a number of TSFs in Australia. A summary of recommended field investigation and laboratory testing methods is provided with a focus on collecting quality information for design purposes. A strong focus is given in developing an accurate assessment of pore water pressures, which, if not correctly understood can have disastrous consequences on tailings strength parameter interpretation and wall stability assessment in static and post-seismic conditions.The assessment of the contractive or dilative shearing behaviour of the tailings material and rigorous interpretation of design characteristic values are presented. The public relations problems of users of explosives have greatly increased within the past few years. One reason is that explosives and blasting agents are being consumed in increasing quantities. The principal reason, however, is that more people have become physically affected by blasting. Mines, quarries, and construction sites that were formerly in relatively remote locations now find themselves near congested areas. New express traffic ways being constructed to connect the suburbs with downtown areas are being built through the middle of long-established business and residential neighborhoods. The public becomes aware of blasting by two physical sensations-vibration and noise. Vibrations are usually so slight as to be negligible, and in most cases the only sensation noted is noise. The reduction of vibrations to this low level has been primarily achieved by the use of blasting practices and explosive products recommended by vibration engineers based on seismograph recordings. Unfortunately, very little scientific attention was given to the accompanying physical sensation-noise.

Bateman Engineering managed the establishment of a second diamond treatment plant at the Letseng Mine in the Mokhotlong District of northern Lesotho, about 100 km from the town of Buthe Buthe. Letseng Diamonds Proprietary Limited, is owned 70 % by Gem Diamonds and 30 % by the Government of the Kingdom of Lesotho. The second plant, which commenced construction in late 2006, doubled Letseng Mines' hard rock processing capacity from 2.6 million t/yr to 5.2 million t/yr, making it the worlds' seventh largest diamond mine by throughput. Bateman Engineering provided the engineering design, procurement and construction management of the new plant.
