An investigation into the source mechanism of mining-induced seismic activity plays a vital role in understanding the various modes of failure observed in the underground deep-level South African mining environment. The seismic events data for this study was captured by the mine-wide seismic system operating at Kloof Gold Mine. This system comprises 40 4.5 Hz tri-axial geophones sampling at 6 kHz installed at locations that surround the stopes mining the Ventersdorp Contact Reef (VCR), Main Reef (MR) and Kloof Reef (KR). A set of 103,753 mining-related seismic events (-3.0≤ ML ≤2.7) that occurred during 2017 and 2018 was selected for this study. The mining depth of these seismic events ranges from 1600 – 4200 m; most located close to stopes mining the VCR ore body. The main criteria used to select a subset of events for detailed analysis were the number of stations that recorded the seismic event and the azimuthal distribution of the sensors.

Shotcrete is a key initial rock support component used in underground mining and civil projects. The strength of shotcrete plays a vital role in providing and deciding the support adequacy. The strength of shotcrete is evaluated by its uniaxial compressive strength (UCS) test that is usually conducted on extracted core samples. Shotcrete cores are commonly extracted from the test panels along the direction of its application; thus, its strength perpendicular to the direction of application is often neglected. The true bearing strength of shotcrete is thereby ignored despite the fact that higher stresses act normal to the direction of its application. Although the anisotropic behavior of shotcrete is well reported, no study has reported extensive testing of this scale and nature. This study reports extensive testing of shotcrete cores extracted from tunnels of a mega hydroelectric project in Pakistan. The study reports 7- and 28-days strength tests of shotcrete cores. The research also investigates the anisotropic behavior of steel fiber reinforced shotcrete and suggests that shotcrete exhibits higher strength perpendicular to the direction of its application.

During the initial process of mining a longwall working face, it is often found that the roof will not collapse readily, which increases the risk of later roof collapse disasters. Explosive and hydraulic fracturing are common roof weakening techniques; however, they have certain limitations. In the present study, CO2 fracturing was used for roof weakening during the initial mining phase in the Changcun coal mine, China, which is a novel strategy. First, by analyzing the lithological characteristics of the working face and applying combined beam theory, the breaking spans of the top coal, immediate roof, and main roof were determined before weakening. An F-35 CO2 fracturing device with a rated pressure of 270 MPa was used, which provides fracturing energy equivalent to 0.968 kg of TNT. The fracture radius of the main roof was studied by ANSYS/ LS-DYNA combined with the hanging roof-breaking theory to determine the optimal layout of CO2 fracturing drills. The results show that the breaking spans of the top coal, immediate roof, and main roof were reduced by 3.99 m, 7.66 m, and 22.03 m, respectively. The main roof collapsed as the working face advanced, eliminating the chance of later roof collapse disasters. Compared with hydraulic fracturing and explosives, CO2 fracturing is safer and more practical, giving it significant application potential.

INTRODUCTION The inactive uranium mill tailings investigated in this study are located in two mining districts, Elliot Lake and Bancroft, Ontario, Canada. The sites exhibit a mixture of surface features consisting of dry areas, with or without vegetation and areas covered with water. On the edges of the water bodies, indigenous vegetation has invaded the tailings beaches [ Typha] spp. (Tourn.) L. a dominant plant on these tailings beaches, has been studied for the uptake of radium-226 and lead-210 (Kalin and Sharma, 1981b) from the tailings. It was found that most of the radium-226 remains in the roots of the plants, and that the solubility of radium-226 in control soil differs from that in tailings. The uptake of radium226 by vegetation and other biota is related to the solubility of the element in water. Factors controlling the solubility of radium-226 in uranium mill tailings are of interest in assessing the environments effects of these wastes. Rusanova (1962) found that the soluble or extractable amount of radium from the soil is inversely related to the total concentration of calcium and magnesium in the soil. Experimental work has clarified some aspects of the leachability of radium from uranium mill tailings (Levins, et al., 1978; Wiles, 1978, and others). Halvik, et al., (1967) studied the effects of pH and chemical composition of surface water on the liberation of radium from uranium mill tailings and uranium ore. He found that an increase in pH, up to a value of pH 9, decreases the amount of radium released from the tailings and the ore. A positive effect of calcium chloride was noted on the leachability of radium. Benes (1981) reviewed the physicochemical forms of radium and its migration in water. Based on experimental work, he identifies primary factors which determine leaching of radium from uranium mill tailings. The ratio of the volume of the leaching solution to the weight of the leached sample; the composition of the leached solids and the leaching solution, and finally the pH of the leaching mixture are of importance. He emphasized that a scarcity of field data exists, which would relate experimental work to the actual situation in the tailings ponds. Inactive tailings ponds in Ontario are 16 to 23 years old, and processes which are of importance in evaluating the long term effects of uranium mill tailings in the environment can be studied. The objective of this work was to investigate the leachability of radium-226 from the tailings under field conditions. MATERIALS AND METHODS Sample Collection A description of inactive tailings sites in Ontario, where the tailings and the water for this study were collected, can be found in a report by Kalin (1981). [Typha] spp. specimens were excavated from 16 different locations along with the tailings attached to the roots of the plants. A tailings sample from the surface area, around the plant, was secured before the excavation. Tailings samples were also collected from dry areas free of vegetation in the vicinity of the wetland stand. Surface samples (depth 0-5 cm) and samples at a depth of 20-25 cm were secured. Water was collected from the shallow tailings beaches around the vegetation stands. Sample Preparation The roots of the plants were washed free of tailings with distilled water. The resulting thick slurry of tailings was allowed to settle in the wash basin for five minutes and the supernatant water was decanted. The remaining saturated tailings were brought to 400 ml volume with distilled water in a beaker. The slurry was mixed with a magnetic stirrer for two 24-hour periods. After the first 24-hours, the slurry was allowed to settle for 20 minutes and the supernatant water was removed. The remaining sediments were suspended again in distilled water (400 ml total mixture) and leached for a second 24hour period. The tailings slurries from the different locations had a solid to liquid ratio, which ranged from 0.5 to 1.4 grams of dry tailings material per millilitre. The variations in the ratios are the result of differing fractions of coarse and fine tailings on the sites. In the final leachates, the solid to liquid ratios of the samples were 0.17 (±0.1) g/ml. The pH of the surface water and the leachate was determined with an I.L. Portomatic pH meter. All the tailings samples were homogenized in a mortar and brought to dryness at 75 to 85°C. INTRODUCTION Uranium mining operations release radioactive materials into both air and water and generate large quantities of solid wastes containing low levels of radioactive materials. Solid wastes produced by mining operations remain on the surface at many inactive mining sites in the Western United States. These mining effluents may present a potential health and environmental hazard. Therefore, Congress, in Section 114(c) of the Uranium Mill Tailings Radiation Control Act of 1978, instructed the Administrator of EPA to prepare a report identifying the location and potential health, safety, and environmental hazards of uranium mine wastes and to recommend a program to eliminate these hazards. Several facts and limitations helped shape the method and approach of the EPA study. Little information on uranium mines was available; measurement information that was available on uranium mine wastes was frequently influenced (biased) by nearby uranium mills; time and resources did not permit comprehensive field studies to provide additional data; and there are inherent variations between uranium mines and sites that complicate generic assessments of mine wastes. To accommodate these facts, the EPA developed conceptual models of uranium mines and made health and environmental projections from them. The models were based upon available data from the literature, supplemented with information from discussions with persons inside and outside the EPA, and by doing several short-term field studies in Texas, New Mexico, and Wyoming. When necessary, conservative (maximizing) assumptions were employed. This paper presents a brief account of a part of the EPA study dealing with the potential health and environmental effects caused by active surface and underground uranium mines. Airborne contaminants are emphasized, although solid and liquid effluents are also included. Due to the limited space, only the methods and parameters used and the results of the assessments will be presented here. Anyone interested in the source of the data used and the development of the parameters should refer to the EPA report (Blanchard et al., 1981). The occurrence and emissions of stable elements were included in the EPA report, however, due to space limitations and their apparent small impact, except for possibly at some specific mines, only radioactive sources will be included in this presentation. MODEL URANIUM MINES The model surface mine was located in the South Powder River Basin of Wyoming and the model underground mine was located in the Ambrosia Lake area of New Mexico. These are the prevalent type mines in those areas. The model mines were based on the average production parameters of the 63 open pit mines and the 256 underground mines that were operating in the United States in 1978 (Department of Energy, 1979) and on a report of an extensive study of open pit mines in Wyoming (Nielson et al., 1979). Information contained in environmental impact statements and in reports from federal and state agencies was also used. Parameters for the model mines are listed in Table 1. The surface mining scenario is that 7 pits are opened in the 17-year mine life with overburden from each successively mined pit used to backfill a previously completed pit, resulting in an equivalent of one pit of overburden (2.4 year production) stored on the surface. No backfilling is assumed at the underground mine. Overburden or waste rock, ore, and sub-ore are separated into separate piles that are either rectangular in shape with length twice the width or in the shape of a frustum of a regular cone. Both shapes have 45 degree sloping sides. To account for bulking, the volume of the material comprising the piles was considered to be 25% greater than the volume of material removed from the ground. It was assumed that dewatering was required at both mine sites. Wastewater discharge rates at the surface and underground mines were assumed to be 3.0 and 2.0 cu m per min, respectively. SOURCE TERMS The following radioactive contaminants at active uranium mines were assessed in the EPA report: 1. Radioactive particulates in a) wind suspended dust from waste rock (overburden) pile, sub-ore pile, ore stockpile, b) suspended dust from mining activities (rock breakage, loading and unloading ore and wastes), and c) vehicular dust, 2. Rn-222 emanation from waste rock (overburden) pile, sub-ore pile, ore stockpile, and mining activities, 3. Rn-222 emanation from mine surface areas, and 4. Radionuclides in wastewater discharged to land surface. Estimated average annual dust emissions (item 1 above) from the model mines are listed in Table 2. Emission factors and the assumptions used to estimate these dust emissions are described in detail in the EPA report. Radioactive source terms were computed for each of the sources; dust emissions were multiplied by the concentrations listed in Table 1

The Lupin mine consists of a gold mining, milling, and refining complex located 400 km (250 miles) northeast of Yellowknife and 100 km (60 miles) south of the Arctic Circle on the southwest shore of Contwoyto Lake in Canada's Northwest Territories. (Fig.1). The mine was commissioned in 1982 and is presently producing 6 t/a (193,000 oz per year) of gold from 612 kt (675,000 st) of ore. The success of this operation is due to such factors as the continuity and grade of the ore body, the competency of the host rock, low mining costs, efficient milling, the transportation of people and materials to and from the mine site, and the unique work schedule resulting in a stable workforce. Echo Bay Mines operated a silver mine at Port Radium on Great Bear Lake, Northwest Territories, from 1965 to 1982. The company first obtained an option on the Lupin property from Inco in February 1979 and completed an underground exploration program in 1979-1980. Topographic relief is low and vegetation is sparse in the continental subarctic climatic zone, consisting mainly of moss and lichens. Temperature extremes are from 24°C to - 45°C (+75°F to -49°F) with an annual mean of -12°C (+54°F). Permafrost extends from near surface to 500 m (1640 ft) in the ore zone. Its remote location and harsh climate presented some challenging design and logistical problems. Construction began at the Lupin mine site in 1980. Before construction, a 1.5-km (5000-ft) gravel landing strip was prepared, suitable for landing a C 130 Hercules. The entire Lupin project took 20 months to build. All of the men and materials were transported to the site with some 1100 Hercules flights and several hundred Convair 640 flights. The Convairs transported construction crews, which numbered 400 at their peak, and also carried 3.2 kt (3535 st) of supplies during construction. The facilities were constructed and commissioned for a total cost of C$135 million. The operation was originally designed to throughput 860 t/d (950 stpd). Expansion in 1983, circuit refinements, and some capital projects brought the daily throughput to 1.7 kt/d (1850 stpd). The underground mine delivers 612 kt/a (675,000 stpy) of ore to the mill at an average head grade of 8.46 g/t (0.3 oz per st). The ore is nonrefractory and is processed in a conventional cyanide leach using the Merrill Crowe process. Gold recovery is about 95.0%. The average production cost is $US5.85/g ($US 182 per oz) based on 1987 figures. Mining at Lupin – Geology The Lupin deposit occurs in amphibolite grade iron formation overlain by mudstones (phyllites) and underlain by graywacke (quartzites). Contacts between the wallrock units and the iron formation are well defined. It has been folded and tilted into a megascopic antiform-synform-antiform structure (Fig. 2). Gold occurs primarily within the sulfide rich iron formation, with some minor occurrences in sulfide poor iron formation. The distinction between sulfide rich and sulfide poor iron formation is based on a visual cutoff of 5% total sulfide content. Mining widths are determined by an assay cutoff of 4.2 g/t (0.15 oz per st) gold. There are few tons between 1.7 and 4.2 g/t (0.06 and 0.15 oz per st). The amphibolitic iron formation at the mine ranges from 1.5 to 20 m (5 to 65 ft) wide and has been followed over a strike length exceeding 1.7 km (5600 ft). The wider portions of the ore body tend to occur at its north extent and south nose. The gold-bearing iron formation appears on plan as a Z-shaped structure made up of three zones: the West, Center, and East. The West and Center zones dip steeply to the East (75° to 90°). Each of the ore zones plunge at an angle of about 65°. Total strike length of the three zones is more than 610 m (2000 ft). The zones are confirmed at a depth of 650 m (2130 ft) below surface. The Center zone is the widest and varies from 4.5 to 20 m (15 to 65 ft) while the West is the narrowest, averaging 1.5 m ( 5 ft). The footwall is comprised of quartzites that are strongly jointed and locally grades into phyllite, which comprises the hanging wall. The hanging wall and footwall are reversed in the West zone. Mineralogy of the ore at Lupin consists of amphibole minerals (hornblende, cummingtonite, and grunerite), feldspars, quartz, occasionally garnet, pyrrhotite, arsenopyrite, minor pyrite, and trace chalcopyrite. Also found in minor amounts are scheelite, apatite, epidote, calcite, tourmaline, and some arsenides (notable loellingite). Quartz

Since 1978, the Colorado Department of Health has become involved in specific investigations of possible radiation hazards among nonminers in Colorado communities. In each instance, the improper disposal of mill tailings has precipitated concerns and allegations of radiation hazards. This presentation is a brief summary of the findings, to date, of 4 such investigations involving tailings disposal problems in Canon City, Denver, Durango and Grand Junction, Colorado. Canon City In the summer of 1979, allegations of excessive cancer incidence were made at public hearings concerning an application for a Radioactive Materials License submitted to the Colorado Department of Health by the Cotter Corporation for a uranium mill at Canon City. Canon City is the site of a uranium mill operated since 1958 by Commonwealth Edison. Local residents accumulated figures and calculated rates suggesting a 2-fold excess in total cancer mortality. These rates were not adjusted or standardized for demographic varibles. A review of cancer mortality data and computation of age standardized rates for years 1950 through 1977 showed that Canon City's cancer mortality rates were within expected limits, actually lower than rates for the State of Colorado or the United States. The first slide shows rates for every fifth year, 1950 through 1975. Further, an analysis of Canon City cancer incidence data from the Colorado Central Cancer Registry revealed that 1979 incidence rates were not significantly different from Colorado rates, with the exception of prostate cancer. The next slide shows age standardized incidence rates for lung, colon, breast and prostate cancer. Data were also reviewed for leukemia, myeloma, lymphoma and cancers of the thyroid stomach, uterine certix, ovary, kidney, bladder-and brain, however, small numbers of cases prevented meaningful rate calculation. 1980 data are presently being analyzed. None of the data yet examined support allegations of radiation-associated cancer in Canon City. Denver In February of 1979, the Department of Health became aware of a number of radium mill tailing deposits in the Denver metropolitan area, remnants of the radium milling industry of the early 1900's. Several of the deposits were situated so as to possibly contribute significantly to radiation exposure of a small number of people over a period of several decades. The Department developed protocols for radiation surveys, dosage estimates and, for a small number of persons, body burdens determinations and peripheral blood lymphocyte cytogenetic studies. The results of this investigation suggested no measurable biomedical impact as a result of the radium deposits. Durango In October of 1979, a physician residing in Durango, Colorado released information from a preliminary analysis of lung cancer data suggesting an incidence rate several times expected in that city. The data were presented at a meeting of a citizen's group concerned about the possible health hazards of 2 uranium mill tailing deposits located in the south end of the city. Since cancer incidence data generally do not exist in most of western Colorado, a team of epidemiologists was dispatched to Durango to work with local physicians and hospitals in conducting an epidemiologic study of selected cancers for the period 1969 through 1978. After case-finding and record abstracting were completed, it was determined that sufficient data were available for study of only 3 sites: lung cancer, breast cancer and leukemia. The next slide shows age standardized incidence rates for these 3 sites for 1969-1978 for Durango and the State of Colorado. There are no significant differences in these rates. The data did show a geographic peculiarity with regards to the relative proportion of tumor types near the tailings deposits as opposed to away from the deposits, however, this finding is based upon very small numbers of cases and may represent only the random excursion of rates based upon small observations. This investigation suggests that if a carcinogenic hazard is present, it is too small to be detected by the study method employed. Grand Junction The Department of Health has been involved in the Grand Junction mill tailings problem for several years. In 1966, the Department issued an order terminating the practice of free public access to a 55-acre pile of uranium mill tailings. Prior to this order, an estimated 300,000 tons of material was removed. Of this amount, an estimated 50,000 tons was presumably used in residential and commercial construction. Despite many allegations of cancer and birth defects excesses in Grant Junction since the mid 1960s, the first epidemiologic study of cancer was conducted in the spring of 1977. Data from the Colorado Central Cancer Registry were analyzed and reported to the Executive Director of the Department of Health in June of that year. The findings of the first and preliminary study were an unexplained excess of acute leukemia and chronic myelocytic leukemia. The excess was based upon small numbers but was present across all age groups. No increase in chronic lymphocytic leukemia was evident.

Researchers at the National Institute for Occupational Safety and Health have tested the finite element code, UT3PC, with LaModel to evaluate their results using know case studies. The purpose of the testing was to demonstrate the usefulness and limitations of these programs in evaluating mine layout design. Both of these software packages are simple to use and free for public download. Their use by mine engineers, along with other tools, such as empirical methods, can enhance the assessment of a mine layout design. LaModel can model mining layout on a mine scale or, at least, on a multiple-panel scale. However, this software has limitations in calibrating to conditions at many coal mines in the western United States. The UT3PC software models smaller parts of a mine and is used to evaluate the design of pillars, shafts, or drifts; but the software calculates all components of the stress tensor and provides stresses, displacements, and safety factors throughout the model volume. The two codes are used to evaluate design of one known case, and UT3PC is used to evaluate another case. The results and approaches demonstrated in the testing are intended to aid mine engineers in evaluating site-specific problems and to show that these tools can provide useful information to the mine engineer in evaluating mine layout design.

The Waste Isolation Pilot Plant (WIPP) is a U.S. Department of Energy research and development facility designed to demonstrate the permanent, safe disposal of U.S. defense-generated transuranic waste. The waste storage horizon is 655 m (2150 ft) below surface in bedded salt. To date the WIPP project has not emplaced any waste. There are three intake shafts used to supply air to the underground. All air is exhausted though a single return shaft. The total design airflow during normal operations is 200 m3/s (424,000 cfm). The ventilation system is designed to provide separate air splits to construction, experimental, and storage activities. Separation is achieved by isolating the storage circuit from the construction or experimental circuits with bulkheads. Any air leakage must be towards the storage area of the facility. Field studies have shown that the pressure differential necessary to maintain the correct leakage direction is susceptible to the effects of natural ventilation; therefore, extensive studies and analyses have been conducted to quantity the natural ventilation effects on the WIPP underground airflow system. A component of this work is a monitoring system designed to measure the air properties necessary for calculation of the natural ventilation pressure (NVP). This monitoring system consists of measuring dry bulb temperature, relative humidity, and barometric pressure at strategic locations on surface and underground. The psychrometric parameters of the air are measured every fifteen minutes. From these data, trends can be determined showing the impact of NVP on the ventilation system during diurnal variations in surface climate. Both summer and winter conditions have been studied. To the author's knowledge this is the first reported instance of automatic and continuous production of time and temperature variant NVPs. This paper describes the results of the initial monitoring study. INTRODUCTION The ventilation system at the Waste Isolation Pilot Plant (WIPP) in Carlsbad, New Mexico, is designed to perform two distinct functions. First, it supports normal mine ventilation requirements complying with all state and federal mine regulations. Second, the system is designed to prevent an uncontrolled release of radioactive contaminants from the storage and transportation areas of the facility. Although a nuclear radiation release in the facility is considered unlikely, many special features are implemented in the ventilation system to prevent the possible spread of contamination. The facility is constructed with the waste transportation and storage areas separated from the mining and non-radioactive experimental areas. The ventilation system is designed such that air leakage is from the mining and experimental areas to the storage areas. Furthermore, radiation detectors are located throughout the storage and waste transportation areas underground and an exhaust filtration building is installed on surface to prevent the possible release of radiation to the environment. For over two years the underground ventilation system has been rigorously tested and balanced. It was during this period that the adverse effects of NVP were noticed and subsequently quantified. From extensive field studies and computer models, several mitigating features were designed and constructed and special operational procedures were implemented to control the impacts of NVP. To quantify more accurately the NVP at the WIPP, a continuous monitoring system was installed. This monitoring system consists of measuring dry bulb temperature, relative humidity, and barometric pressure every fifteen minutes at strategic locations on surface and underground. From this psychrometric data, the NVP is calculated. Fan operating pressures and flows and strategic differential pressures are recorded from the site Continuous Monitoring System (CMS). The monitoring system provides a means of evaluating how the ventilation system behaves in regard to climatic conditions and to judge the efficacy of the mitigating features and operational procedures. To the author's knowledge, continuous calculation of NVP as a function of time and surface temperature has not been previously reported. Overview of the Waste Isolation Pilot Plant The U.S. Department of Energy determined that the plastic nature of bedded salt may provide the best solution to isolate transuranic (TRU) waste from the biosphere. Initial evaluations at the WIPP site began in 1974. In 1979, the United States Congress enacted Public Law 96-164 for the construction and development of the WIPP project. The mission of the WIPP is to demonstrate the safe, long-term disposal of TRU waste generated by the national defense programs of the United States. TRU waste is classified as a low to medium level waste. The waste is stored in drums and does not produce significant heat (not greater than 1 W per drum). The WIPP site is located approximately 47 km (29 miles) east of Carlsbad, New Mexico in the Chihuahuan Desert. The repository is located in the 630 m (2000 ft) thick Salado Formation. This Permian Basin salt deposit is about 225 million years old and appears to have been minimally disturbed by earthquake, faulting, and ground water activity since it was deposited. There has been an increasing trend in recent months by regulatory agencies to require that estimates be made of the hydraulic head acting, under operational conditions, on the surface of primary leach pad liners. This move towards inferred performance standards, rather than specifying a prescriptive standard for all sites regardless of economics, is to be generally welcomed. The theory of flow on a sloped beach pad, which was developed by East et al, 1987 (Reference 1 and 3) has been refined and extended. Unitized curves have been developed by graphically displaying the seepage losses and resulting dissolved gold lost as seepage through the primary liner as an aid to pad liner selection and design. Very little information is available of actual hydraulic head profiles acting on the liners beneath heap leached ores. Attempts have been made to measure these by drilling from the top of the heap and installing piezometers at the base. With this procedure it is difficult to obtain accurate data due to the fact that the drilling operation itself creates a highly permeable vertical flow path which may not exist to the same degree elsewhere in the heap. Combined with the fact that it is almost impossible to ensure that the piezometer tip is sitting directly above the liner without the risk of causing some damage to the liner itself. The best time to install hydraulic head monitoring devices on the pad liner is during construction and when piezometers can be carefully placed directly above the liner and suitably protected hydraulic lines laid to ensure full hydrostatic head measurement with simple manometer devices.

The content of the paper by V. Rajaram does not fulfill the expectations of the title. But Rajaram submitted the article in October 1983, and he could not have foreseen the numerous developments that would occur before his paper was published more than two years later. Nevertheless, Rajaram failed to mention the interest in oil shale development in southern Indiana beginning in late 1979 and continuing through the present as a result of special financial encouragement of three oil shale projects in May and August 1984 by the Indiana Energy Development Board and the Indiana Corp. for Science and Technology. A session at the 1985 Eastern Oil Shale Symposium in Lexington, KY, Nov. 18-20, 1985, gave the current status of oil shale developments in the eastern United States. Speakers at this session reported on the three Indiana projects. First, Gary D. Aho, Cliffs Engineering Inc., spoke on a feasibility study by Cliffs Engineering and Allis Chalmers for a site-specific pilot plant using the Allis Chalmers process. The plant site is in Clark County, IN, on property of the Midwest Energy Resources Co. The project is funded by $240,908 each from the Indiana Energy Development Board/Corp, for Science and Technology (EDB/CST) and the US Department of Energy (DOE) and $120,454 from the corporate sponsors (total $602,270). Completion is scheduled for mid-1986. Next, Edwin M. Piper, Stone and Webster Engineering Corp., discussed the American Syn-Crude/Indiana Oil Shale Project. This effort followed the completion of two smaller projects funded by the Indiana EDB/ CST: "Testing of Indiana Oil Shale in a Petrosix Pilot Plant" (total funding by EDB/CST at $50,000) and "Assessment of the Petrosix Process for an Indiana Shale Oil Plant" (total funding by EDB/CST at $100,000). The status at the time of the report at the Oil Shale Symposium was that a request for an extension of time for securing nonfederal support had been submitted to the US Synfuels Corp. The proposal included building an 11-m (36-ft) diam retort in south-eastern Indiana to process the New Albany Shale and to produce 366 m3 (2300 bbl) of shale oil per day by the PETROSIX process. The Indiana EDB/CST had contracted with Stone and Webster at $401,100 from EDB/CST and $245,835 from Stone and Webster for activities to advance the project in the negotiations with Synfuels Corp. At the time of the report, this project was the only eastern oil shale proposal that was still on the agenda of the Synfuels Corp. As a result of Congressional action in late December 1985, federal support from the Synfuels Corp. is no longer possible. Finally, Victor H. Carr, Eastern Shale Research Corp., described his firm's project, which is jointly supported by DOE ($227,749) and the Indiana EDB/ CST ($73,850) and is entitled "Feasibility Study to Determine Suitability of an In-Situ Process to Recover Hydrocarbons from Eastern Shale." An area in Scott County, IN, had been chosen, but not a specific 9 x 15-m (30 x 50-ft) site. One burn of a small in situ retort is contemplated as part of the project. Besides these three projects, several reports on shale research were presented. Joseph Damukaitis, American Syn-Crude Corp., reported that a pilot plant using the hydrogenation-extraction (H-E) process (described at the 1984 Eastern Oil Shale Symposium) was 94% built and would go through shakedown with oil shale but would then shift mechanical devices to process coal mine waste. Current status of research on the HYTORT process was then presented by Raymond C. Rex, Jr. Oil shale beneficiation research at the University of Alabama/Minerals Research Institute was reported by R. Bruce Tippin. Scott D. Carter discussed continued research on fluidized-bed retorting of shale at the Kentucky Center for Energy Research Laboratory. Carl E. Roosmagi of DOE, Morgantown, reported on the oil shale research at the Morgantown Energy Technology Center (METC) laboratory. Henry J. Gomberg, Ann Arbor Nuclear Inc., discussed "Radiation Combined with Donor Solvents for Extraction and Up-Grading of Kerogen." Aurora M. Rubel and coauthor Eileen Davis presented results of research at the Kentucky Center for Energy Research Laboratory under the title "The Effect of Shale Particle Size on the Products from the Bench Scale Fixed Bed Steam Pyrolysis of Kentucky Sunbury Shale." Lastly, Maria Rockwell, Technical University of Nova Scotia, presented "Processing and Up-Grading of Low Grade Nova Scotia Oil Shale for Potential Use." A review of shale oil prospects by Gerald Parkinson in Chemical Engineering for Feb. 3, 1986, covers both western and eastern projects and includes a report that the US DOE is funding a few relatively small projects; most of the fiscal 1986 budget of $12 million for shale oil is for fundamental research. Projects include a $3.2 million three-year contract with Hycrude Corp. (Chicago) for development of the HYTORT process and a $1.2 million three-year contract with the University of Alabama's Mineral Resources Institute on beneficiation of eastern oil shale by froth flotation. The report incorrectly states that current projects include a total investment of $466 million in a pair of 18-month technical and economic feasibility studies for proposed projects in southern Indiana. Beginning in late 1985 and continuing through 1987, Bureau of Mines personnel investigated longwall chain pillar and entry design in two-and three-entry gate-road systems at a deep underground coal mine located in central Utah. To evaluate their respective stability characteristics, four chain pillars and two longwall panels within the two separate entry systems were instrumented with Bureau of Mines hydraulic borehole pressure cells to continuously monitor vertical and horizontal pillar and panel stresses through adjacent panel retreat. In addition, supplemental entry closure information was obtained from sites located in the vicinity of the instrumented pillars. The findings presented in this report demonstrate the practicality of evaluating mine pillar stability using in situ methods and support three significant conclusions regarding ground control aspects of the investigated two-entry system: (1) Total areal entry loading was considerably less for the two-entry system, as opposed to the three-entry system, (2) a smaller percentage of opened ground in the two-entry system contributed to a marked reduction in roof falls, and (3) lower areal loading in the two-entry system suggests improved ground conditions when mining the underlying seam.

"Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are at a “plateau” and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas Refinery disaster, organizations that have “exemplary” safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters, the BP’s Deepwater Horizon Rig disaster in the Gulf of Mexico. Looking into the causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset, or its culture? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there features that can be delineated? The research and review presented in this paper covers a period since1995, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions of near zero organizations (NZO) that suffered unexpected disasters, based on extensive research by the author. More recently, the massive West Fertilizer plant explosion in Texas, and the Train Disaster in Lac-Mégantic, Quebec, Canada underlined the same trends. Organizations may experience calamities not because they are bad, or unsafe, but because they have become “cult-like”. INTRODUCTION The train disaster of 6 July 2013, in the small town of Lac-Mégantic in Quebec, Canada, in which 47 people were killed, had many strange pre-cursorsso-called “black swan” events that have occurred in many different industries. Black swan events are so rare, that there is almost zero chance that they will ever occur again, but they are so calamitous that the responses to these are emotional and irrational, (but necessary) and in those responses, the seeds for other, different black swan events are put into place. It is not what we don’t do, or do wrong, that causes modern disasters, it is what we do. The same things in the culture that make us successful, may also cause the disasters. We slowly, incrementally and insidiously ‘float’ into disaster. One such Black Swan event: The Piper Alpha Oil Rig disaster, killing 167 men in the North Sea on the 6 September 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry."

The U.S. Bureau of Mines (USBM) is investigating alternative methods to strip metal-loaded solvent extractants. Hydrolytic, precipitation, and hydrogen pressure stripping all recover metal or metal salts directly from the organic solvent extractant. Pure metals, metallic oxides, or metal salts can be formed, allowing direct production of value-added products. Aspects affecting commercial application are emphasized, including metal recovery from actual ore leach solutions, particle size control, organic modifier and diluent effects on stripping and metal recovery efficiencies, metal grade, seed addition, mixing intensity, recycling efficiency, and reactor plating. Metal separations from copper ore leach solutions and complex waste leach solutions were examined.

A team of researchers at the University of Kentucky, working under a USBM contract, developed over 65 simulation exercises for teaching and testing problem-solving and decision-making skills to miners and mining engineers. These skills involve both improving judgment and decision making in mine emergencies and enhancing the learning of real-world engineering design principles. The exercises are ideally suited for either latent image format or for computer-aided instruction. Exercise features are summarized, the structure and field testing of typical exercises described, and sample frames of individual exercises presented. Empirical data are summarized that describe the effectiveness of using these short, time-efficient performance tasks that simulate real problems and that provide immediate corrective feedback for teaching and testing critical skills. The process of magnetic separation is based on the differences among magnetic susceptibilities of various mineral species. Performed either wet or dry, the separation has application in both the recovery and concentration of value minerals and in removing deleterious mineral constituents from a product stream. There are two major catagories to be considered when addressing magnetic separation of minerals. The first catagory refers to particle characterization and relative differences in magnetic susceptibility. The second catagory refers to design parameters and the magnetic field generated by the separator.

Of the variables affecting the electrical properties of coal measure rocks, water content is by far the most important, This paper describes specially developed apparatus and methods of measuring electrical conductivity and permittivity at constant humidity conditions. Electrical properties variations were measured for Applachian and Illinois, U.S.A. coal 2 basin samples in the frequency range 2 X 10 to 2 X 10 Hz. Coal measure rock electrical behavior, when correlated with mechanical behavior, is potentially useful in a number of mining applications.

A custom-designed shotcrete system, consisting of an 8-m3 (10-yd3) transit mixer and a shotcrete spray boom carrier, is used bya 2-man crew to transport and place up to 25 m3 (32 yd3) of steel-fiber reinforced, dry, silica fume shotcrete per II-hour shift. Shotcrete is custom blended by a local supplier and delivered predampened. Mix design and quality control is performed with a computer program that develops and graphs a properly balanced mix based upon the materials sieve analyses.

In terms of resources of amphibole asbestos only South Africa can be considered. There are only two asbestos producing fields, one in the Northern Cape Province producing only crocidolite and one in the Eastern Transvaal producing only amosite. The banded ironstone sequence in which amphibole asbestos occurs is discussed, and the genesis of the fibre is briefly reviewed. The properties and characteristics of amphibole asbestos are discussed and compared to those of chrysotile. Approximately 300,000 tonnes of amosite and crocidolite are produced annually. Crocidolite has a potential for a fivefold increase of production, which could be sustained beyond the year 2000. An estimate of the resources of the various producing fields in South Africa is made. Mining methods are reviewed and evaluation techniques discussed.
