This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. On the basis of field observation, four principal styles of hydraulic breccia are recognised, and their characteristic features noted. In practice, hybrid styles are most commonly encountered. Numerous examples will be illustrated during the oral presentation, and the potential for definition of a formative environment is emphasised. In one example presented, it is suggested that more than 50 episodes of hydraulic fracturing can be recognised. In all cases at least sane useful conclusions can be drawn in the context of modeling for drill targets.

Vibration levels on a variety of equipment in a South African opencast mine were measured in this study. The well-established ISO 2631-1 (1997) standard methodology was used for the computation of weighted root mean square (WRMS) and vibration dose value (VDV). This method was used to evaluate the whole-body vibration (WBV) on a wide range of equipment in the opencast mine using the two main parameters, namely the daily exposure A(8) which is based on the WRMS concept and the VDV which measures shocks. The results obtained show that while the majority of the equipment rendered exposure levels that are below the exposure limit value (ELV), exposure levels on about half of the equipment do exceed the exposure action levels. The study also highlights the importance of shock in the observed responses.

Two basic cases involved in the design of an opening in bedded rock are: 1) where the beds deflect from each other so as to be separated; and 2) where the beds deflect onto their lower neighbor, loading the latter while reducing their own load. Analysis of case 1 is relatively simple, while the interference of the original deflection curves in case 2 complicates analysis of the latter. Figures and formulae are presented for determination of interference loading in this situation. The design of an opening in bedded rock involves two basic alternatives.1'2 In one case the beds deflect away from each other, so as to be separated from their neighbors; in the other case the beds deflect onto their lower neighbor, loading it, while reducing their own load. This is shown in Fig. 1. The first case lends itself to a simple analysis, since the upper and lower boundaries of each beam remain a free surface. In the latter case, however, due to the interference of original deflection curves, the analysis is more complex. Earlier investigators of this interference problem solved it by making the tacit assumption that this type of loading was uniformly distributed along the beam's length. While there is no intuitive justification for this assumption, its use does lead to an approximate solution. Still, a more exact solution does not require this apparently unrealistic restriction. To achieve initial simplicity, the analysis will at first be limited to a two-bedded sequence, as in Fig. 2. Since both beams have identical end conditions and spans, the original deflections of their center lines are members of the same family of curves, as shown in Fig. 3. Therefore, if interference occurs at one point along their center lines, it will occur at every point. For convenience, the midspan point is that usually examined. Should it occur, the additional loads along the lower boundary of the upper bed and the upper boundary of the lower bed must be determined. From the principle of action and reaction, these added loads are known to be equal and opposite, as indicated in Fig. 4. Arbitration is a legally recognized alternate litigation for the resolution of civil disputes. The parties agree, either before or after a dispute arises, to submit that dispute to an independent third party for resolution. The decision of that third party is binding on the parties. A decision of an arbitral tribunal, typically referred to as an "award," cannot be reviewed by a court except on very limited grounds, such as fraud. See, e.g., Va. Code 8.01-581.010. Indeed, an arbitral award is typically less subject to judicial review than the decision of a trial court, whose decisions are almost always subject to appellate review as a matter of right. Arbitration is typically the result of a contractual agreement between the parties to submit disputes to arbitration. On occasion, arbitration can be required by statute. For example, the California Department of Transportation requires the arbitration of all disputes arising under its contracts. Public Contract Code, Article 7.1 (§§10240-10240.13). There may be a question whether statutorily required arbitration is truly arbitration or an administrative law disputes resolution procedure. The significance of the difference is that the decisions of administrative law bodies are typically reviewable by a court of law as a matter of right while, as set forth above, arbitral awards are almost never reviewable.

The key aspects of process metallurgy that distinguish platinum group metal (PGM) concentrate smelting from that of other base metal sulphide concentrates are presented. These differences include considerably higher input chrome and magnesia contents that directly raise the slag liquidus temperature and have the potential to increase accumulations of refractory spinels. Most importantly, the higher processing temperature required for PGM smelting and the resulting very high matte superheat lead to considerably more onerous smelting conditions than those typical of other smelting operations. This has presented challenges to furnace design and integrity, especially when coupled with the progressive intensification of smelting, involving doubling, and then redoubling, of furnace power inputs over the past 20 years. These power increases have been enabled by increasingly more advanced furnace cooling and structural technologies. Key technologies include strong constant-force spring-loaded bindings acting in three dimensions to minimize infiltration of superheated matte into brick joints, and robust well-cooled tapholes for reliably tapping the superheated matte. The result has been substantially improved productivity, and reduced smelting capital cost outlay per unit of production. A significant challenge, which was not anticipated, presented itself in the form of insidious corrosion of the furnace lining, and especially high-intensity copper cooling elements. Investigation of corrosion in related industries eventually identified chloride-accelerated sulphidation', and this term has been retained as it generically describes the most pertinent aspects of the accelerated low-temperature wear of copper coolers observed in PGM smelting. In addition to discussing the corrosion mechanism, this paper describes a number of solutions that were developed jointly by Anglo American Platinum and Hatch to address the copper corrosion problem. First, new monitoring technologies allowed furnaces to be operated more safely for a longer period of time. Second, a system for replacing corroded coolers from outside the furnace during a fast 'hot' shutdown minimized the impact on furnace operating factor and hearth life. Finally, a corrosion-resistant graphite-protected cooler design significantly improved furnace campaign life, and heralds a more lasting solution to cooler corrosion in PGM furnaces.

One of the main targets in the automotive mass industry and supplying industry is the weight reduction of the vehicle for decrease of the fuel consumption. For that reason combinations of different kinds of materials and connections of, for example, steel and aluminium are developed or are already successfully field-tested. A determining factor concerning the quality assessment of the welded joint during hybrid welding is the characterization of the intermetallic Fe-Al layer regarding strength, hardness, morphology and mainly the different phases in which Fe occurs together with the Aluminum (FeAl, Fe2Al5, FeAl3.). A combination of conventional light microscopy, SEM - EDX - analysis, the usage of an atomic force microscope including a Nanoindenter and theoretical thermodynamic calculations turned out to be highly effective. Based on the attained results, the constitution of the intermetallic FeAl layer, influenced by silicon, manganese and zinc, can be predicted.

Two small coral islands in the Pacific Ocean, situated close under the equator, and formerly classed among the Line Islands, have, during the last three or four years risen into prominence owing to the valuable deposits of phosphate of lime discovered on them.The deposits of Ocean and Pleasant Islands are under the control of the Pacific Islands Company (1902) Limited, of London and, Sydney, and the high grade of the phosphate, together with the exceptionally favourable facilities for mining this mineral, of which there are several millions of tons on these islands, give this company advantages which enable them to command the Australian and New Zealand markets, and also to make large contracts with Japan and other countries. Both islands, as far as surface indications are concerned, are entirely constructed of coral, with phosphate on the top from the highest points to the present sea level, there being no sign of volcanic rocks, with the exception of a few pieces of drift pumice on the coast.Banaba (Paanopa according to. the Admiralty charts), also known as Ocean Island, was discovered in 1804. It is situated in lat. 0° 52" S. and long. 169P 3[)" E.

A laboratory size solvent extraction apparatus has been constructed, which incorp- orates three stages of countercurrent extraction, followed by two stages of counter- current stripping. The model uses a gravity flow system from one stage to another and air lift pumps to recirculate organic and aqueous phases. A novel pump/mixer system is used to transfer solutions from the mixer compartments to the settler compartments. The model has proven valuable in the teaching of solvent extraction principles.

This paper discusses the Installation of a Vibwall for the Cardiff Bay Barrage Project Wales, UK, by Kvaerner Cementation Foundations. Only the second use of the technique in the UK the wall was successfully placed to a depth in excess of 20 metres, in unique and challenging conditions. The installation of the Vibwall was a technical success, but not a financial one. The use of the technique was at its "limits" and this paper recommends some of the controls essential in the carrying out of such work in the future.

A mining system consists of many sub-systems such as drilling, blasting, loading, hauling, ventilation, hoisting and supporting. During mining operation, these sub-systems may experience various problems that stop the operation because of possible environmental equipment and safety issues. In order to ensure delivery contracts in the required quality and safe mining medium, the operation should be, at least, performed in the specified reliability level of the system. If the system reliability decreases below the specified level, there will be safety and financial losses for the mining company. Therefore, the mine should be maintained by a reconstruction procedure to guarantee the operation continuity. Given that each sub-system has a different reliability function and maintenance cost, the determination of reconstruction time will be a complicated decision making problem. In this paper, the determination of reconstruction time is formulated as a nonlinear optimization problem and solved by genetic algorithms (GA). A case study was conducted to demonstrate the performance of the approach for an underground operation. The results showed that the approach could be used to determine the best action time. A hypothetical economic model has been developed for diamond projects in the Canadian Arctic using data from recent exploration and mining projects. The model reflects economic conditions applicable to the diamond mining industry in the Canadian Arctic at the present time. The economic model captures pre-production expenditures, production data, and post-production costs. Before- and after-tax cash flows are derived from the data along with the relevant economic criteria. From the corporate perspective, the model provides guidance concerning the minimum size and/or in situ value per tonne requirements of targets being sought in order to ensure economic success and predict exploration and development expenditures. From the government perspective, the model provides an indication of income and mining tax revenues that can be expected from a particular diamond project. The model shows, on average, diamond projects in the Canadian Arctic are expensive to explore for and develop, but are economically robust under the present conditions. They generate great economic return for mining companies and significant amounts of tax revenue for governments. The fact that diamonds can be transported directly from the minesite by air makes them particularly attractive targets for mining companies that operate in the Canadian Arctic.

As the mining landscape continues to transform, the challenge of accessing deeper orebodies quickly and safely is becoming a frequent topic of conversation amongst those in the mining industry. The rapid development of underground mine entries to gain earlier access to deeper orebodies is an ever-present challenge to mine owners and contractors. Overcoming this and providing economical solutions to unlock the value in these deeper deposits is a challenge that should be confronted.In 2013, Redpath Australia Pty Ltd (Redpath) commenced construction of what would be the first of two drifts excavated through the use of a tunnel boring machine (TBM) in Queensland’s Bowen Basin.The excavation and development of the two drifts (declines) was completed as part of a contract for underground mining services between Redpath and Anglo American at the Grosvenor Coal Mine Project in Central Queensland, located approximately 5 km north of the town of Moranbah. Having recently completed the drift development, this paper will extend on the early writings of Donnelly, Ramage and Donghi (2014) and provide detail on the operational achievements, advance rates, threats, opportunities and learnings during the excavation of the two declines: the conveyor (CV) drift and the transport (M&M) drift.CITATION:Ramage, G, Donghi, M and Barnes, A, 2016. This paper is in two parts. The first part provides background to coal in New Zealand, information on Solid Energy the Company and details of the resources, mines and products of the company. This part of the paper is intended for people who have limited knowledge of the New Zealand coal scene. The second part of the paper focuses on developments underway in New Zealand and discusses planned or possible changes and opportunities. This paper was first presented at the Coaltrans Asia Conference in June 2000.

The Mildred Lake Settling Basin (MLSB) is the largest disposal site for mature fine tailings (MFT) at the Syncrude Canada Ltd oil sands plant. Over the past years (since 1997), there has been a marked change in the consolidation behaviour of MFT in the MLSB, which is attributed to microbial activity. Methane-producing microorganisms, known as methanogens, have become very active, and large amounts of biogas (mainly methane) have been produced. In certain regions within the MLSB, gas bubbles are released to the water surface of the tailings pond. Continued field monitoring of the MLSB has provided convincing evidence of the rapid densification process (rapid water drainage from the tailings). This phenomenon contradicts the consolidation models for MFT developed over the past 20 years. The above phenomenon has also been reported at the oil sands tailings ponds operated by Suncor Energy Ltd. in Fort McMurray. This rapid densification has caused pumping challenges in the transfer of fine tailings from the Mildred Lake Settling Basin for the creation of composite tailings. The rapid densification of the MFT may also have potential positive effects in accelerating the reclamation of the oil sands fine tailings. A field and laboratory research program is underway to study the mechanism leading to the rapid densification phenomenon. Systematic field investigations were performed to determine the distribution and characteristics of the rapidly densified MFT. Gas bubble distribution on the water surface was mapped to determine the zones of different biological activities in the pond. These field investigations provided solid ground for further experimental and theoretical research. A number of small-scale column tests were carried out to observe the gas evolution and to measure the changes of some geotechnical parameters under different microbial activity. Also, a series of gassy MFT densification tests are being conducted to study the mechanism of the rapid densification of the MFT under microbiological activity. A review and discussion of the research program is given and some preliminary research results are presented in the paper.

The foundations for the new South Viaduct Bridge in San Francisco were extremely challenging due to the requirement for very large and deep piles installed adjacent to historic structures. Drilled shafts up to 12? foot in diameter were constructed to a maximum depth of 200? feet in alluvial soils and Franciscan Bedrock using an oscillator system for the installation of the permanent steel casing. This paper provides an overview of the construction techniques, the equipment, and the challenges associated with constructing 12? foot diameter shafts on this very complex project in one of the nation?s nicest landscape setting.

As part of the New York City’s redevelopment of the far west side of mid-town Manhattan, this project will add 1.5 miles of twin bored tunnel to No. 7 Subway Line. The proposed alignment extending from the Times Square Station west beneath 41stStreet turning south under Eleventh Avenue and terminating at 25th Street will include two new stations. The tunnels and station caverns will be primarily constructed in the Manhattan Schist, and will pass in close proximity to several major overly ingstructures. This paper discusses the lithology and structural discontinuities of rockmass along the project alignment, tunnel and cavern rock loading, groundwater hydrology, groundwater quality, groundwater treatment and sustainable design.

A new loading apparatus was fabricated and used to conduct true-triaxial compression tests on KTB amphibolite, a crystalline basement rock. Tests in dry jacketed samples reveal that the intermediate principal stress has a significant effect on the onset of dilatancy and on rock strength, contrary to the implicit assumption made by Mohr-type failure criteria. Failure is induced through a shear fracture that dips steeply in the direction of the least principal stress. Experimental results lead to a new generalized strength criterion in terms of all three principal stresses. Unjacketed KTB amphibolite specimens, exposed to confining fluid, are considerably weaker than the jacketed ones, despite the negligible porosity and permeability. Failure is facilitated by several closely spaced subparallel tensile cracks normal to the least stress direction. SEM analysis suggests that microcracks developed upon dilatancy onset are penetrated and extended by the confining fluid, bringing about the unexpected rock failure mechanism at reduced stress levels. Un nouveau dispositif à chargement fut fabriqué et utilisé afin de réaliser un vrai essai de compression tri-axial sur KTB amphibolite, une roche cristalline du sous-sol. Les essais effectués sur échantillons enveloppés et secs révèlent que les contraintes principales intermédiaires affectent considérablement le début de la dilatance et la résistance de la roche, contrairement à la supposition fondée sur le critère de rupture de Mohr. La rupture est produite grâce à une cassure de cisaillement très incliner en direction où la contrainte principale est moindre. Ces résultats expérimentaux indiquent un nouveau critère de rupture général en fonction des trois contraintes principales. Les échantillons de KTB amphibolites sans enveloppe, exposés à un liquide confiné, sont considérablement plus faibles que ceux enveloppés, malgré la porosité et la perméabilité négligeable. La rupture est facilitée par plusieurs fissures de traction adjacentes et sub-parallèles, normales à la plus petite contrainte principale. L’analyse SEM suggère que les micro-fissures développent suivant le début de la dilatance, elles sont pénétrées et agrandies par le liquide confiné, produisant un mécanisme de rupture de roche inattendu, ceci à un niveau de contrainte réduit. Messungen in trockenen, bedeckten Proben zeigen that die mittlere Hauptspannung eine bedeutende Wirkung auf das Einsetzen von Dilatanz und auf die Stärke des Gesteins hat, im Gegensatz zu der impliciten Annahme auf Grund von Mohr-ähnlichen Versagungskriterien. Versagen ist induziert durch eine Scherfraktur die sich steil in die Richtung der geringsten Haupspannung nach unten senkt. Messergebnisse führen zu einem neuen, verallgemeinerten Stärke Kriterium dass auf alle drei Hauptspannungen basiert ist. Unbedeckte KTB Amphibolit Proben, die unmittelbar den umgebenden Fluiden ausgesetzt sind, sind wesentlich schwächer als bedeckte, trotz geringfügiger Permeabilität und Porosität. Versagen wird gefördert durch mehrere, nebeneinander liegende, sub-parallele Spannungsbrüche, normal zu der geringsten Spannungsrichtung. SEM Analyse lässt vermuten dass Mikrobrüche, die sich beim Überschreiten der Dilatanzschwelle entwickeln, durch die umgebenden Fluiden fortgepflanzt und weiter verbreitet werden, und unerwartet, auf Grund der reduzierten Spannungswerte, den Versagungsmechanismus des Gesteins einleiten

This bulletin is one of a series of Bureau of Mines reports dealing with mining methods, practices, and costs. A study of this subject was undertaken by the Bureau in cooperation with a large number of mining companies in 1928. This study continued for 5 years, during which engineers of the Mining Division of the Bureau visited most of the larger mines and many smaller ones in the United States, as well as several mines in Canada and Mexico. As a result of this study, 75 reports dealing with underground mining methods and costs at different mines were published as information circulars. Circulars also were issued on 6 open-pit mines and 2 gold-dredging operations. These circulars were written by the managers, superintendents, and engineers in charge of the operations, in some instances in collaboration with Bureau engineers. They were prepared according to a standard outline drawn up by the Bureau to obtain uniformity in textual treatment and in the arrangement and basis of setting up cost and operating data.
