Risk management is an established loss-control methodology that has been applied successfully in many industries. Recently, interest in this structured approach has grown in the mining industry. The main objective of this research was to develop a risk-assessment process, which is a part of risk management, that can be used by the U.S. mining industry to more thoroughly characterize risks associated with haul truck-related fatalities. The assessment is based on historical data obtained from the U.S. Mine Safety and Health Administration (MSHA) investigation reports, which includes 113 fatal incidents that occurred from 1995 through 2006. The risk-assessment process used in this research involves the following basic steps: identification of the risks, risk analysis and risk evaluation. The preliminary hazard assessment (PHA) method is used in identifying and quantifying risks. Risk levels are then developed using a pre-established risk matrix that ranks them according to probability and severity. The resulting assigned risk value can then be used to prioritize control strategies. This paper is a part of a detailed study on risk assessment for equipment-related fatalities in mining sponsored by the Western U.S. Mining Safety and Health Training and Translation Center.

The concept of sustainable development is not new to the mining industry. Many people involved with mining have argued that the industry throughout the world has always supported the communities where they operated. They say that the mining industry was employing sustainable development practices before the phrase became fashionable. The mines employed residents and supported the local economy through tax payments and the purchase of goods and services from local businesses. In many cases, mining companies have built medical facilities and contributed money, equipment and services to local schools. In modern times, mining companies have donated reclaimed land to the local communities for use as parks and recreational facilities.

The aim of this study is to synthesize hydrated compounds of calcium silicate (tobermorite) using as siliceous source perlite milling process wastes, containing more than 70% of SiO2. Calcium silicate hydrates are mainly used for construction of lightweight building materials, plasters, tiles and filters. The effect of temperature, time and particle size of perlite on the quality and composition of the solid product was studied. The solid products were characterized by X-ray Diffractometry, Differential Thermal Analysis and the amount of Acid Insoluble Residue. The synthesis of tobermorite was per-formed by the hydrothermal treatment of perlite waste with a saturated calcium hydroxide solution at a molar ratio CaO/SiO2 of 1, liquid to solid ratio 10 mL/g and 1% w.t. NaOH addition. Three experimental sets were performed. In the first series of experiments the synthesis temperature was kept constant at 205°C while the synthesis time varied from 0.5 h to 24 h. In the second experimental series the synthesis time was kept constant at 2 h while the temperature varied between 150°C and 245°C. In the third experimental set the temperature was 205°C and the reaction time was 2 hours and two perlite waste size fractions were used, a coarse (d50=18 µm) and a fine grained one (d50=8 µm). Reaction products consisted of semicrystalline calcium silicate hydrate (I) and crystalline Al-substituted tobermorite or a mixture of both. An isomorphic Si4+ ions substitution with Al3+ ions in the hydrated calcium silicate lattice was detected by thermal analysis. The experimental results show that good quality crystalline Al-tobermorite can be produced after 6 hours of autoclaving at 205°C or after 2 hours of hydro-thermal treatment at 225°C. Generally, the in-crease of synthesis temperature or time favors the crystallization of calcium silicate phases and enriches them in aluminum. The use of fine grain size perlite is more efficient for the hydrothermal synthesis of calcium silicate hydrates.

In German hardcoal mining approximately 1 or 2 cases of spontaneous combustion are recorded per 10 million tonnes of saleable output. In approximately every 20th spontaneous combustion case, CH, is ignited. By the example of a fire caused by spontaneous combustion on Friedrich Heinrich colliery, an overview on the measures taken for firefighting and explosion prevention is given. These measures comprise (in sequential order) - early fire detection by CO-monitoring, - gob-sealing measures against ventilation air leaks, - full-space inertization of the coalface subsequent to a CH,- deflagration, - installation of a target inertization of the caved-in gob under the protection of the full-space inertization, - re-opening of the fire zone and reestablishment of the ventilation-air current, - completion of the salvaging work with continued target inertization of the caved-in gob and further sealing measures (monolithic packing), - sealing of the coalface after salvaging. The measures described allowed gain of control of a safety- critical (because close to the face) gob fire caused by spontaneous combustion, in spite of a CH, deflagration, within few days only, and salvaging of equipment from that coalface and start-up of the follow-up coal-face on schedule. The measures taken as well as the experiences gained allow deduction of general safety criteria for fighiing of gobfires caused by spontaneous combustion, and for controlling the explosion risk. e.g. - warning and alert thresholds for early detection of fires, - criteria for spotting of the zone of fire, - rules for tightening caved-in gobs, - thresholds for the retreat of firefighting crews from the vicinity of the zone of fire, and for starting ineiization, - considerations for positioning of and strength requirements for stoppings with/without inertization, - flow-by-volume adjustment of inertization, - safety rules for work with respiratory apparatus in the inertized zone, - safety rules for the re-opening of zones of fire. The considerations end up in the conclusions that the duration in terms of months of the classic cycles of sealing, waiting, and reopening can nowadays be reduced to a few days only by purposeful combination of full-space inertization and locally confined tar- get inertization of the gob area. The earlier high risks incured by the mine resuce squads en- trusted with sealing of zones of mine fires under explosion hazards can be reduced by the method described, provided the safety regulations are conformed with. In this way, intertizing contributes substantially to reduction of down-time caused by mine fires, to preservation of recoverable parts of the deposit, and to improved safety of the fire-fighting crews.

Recently, G&S Engineering Services used an Enerpac synchronous hydraulic lifting system to lift a 3.2-t(3,500-st) dragline at the Curragh Mine in Queensland, Australia’s Bowen Basin.

Silver paste including nano sized particles maintains conductivity in high temperatures without forming oxide layers. In the industry, printing process produces wasted silver paste inside the cylinder and on silk-screen. Pyrometallugical silver recycling methods have environmental problems and do not extract pure silver after removing polymer and organic materials during the extraction process. In this study a simple wet process, of recycling the waste silver paste, using ammonia, was conducted. Silver ion extraction yield was increased using oxidant in the ammonia solution. Possible extraction yields, such as pH, concentration, temperature of ammonia, oxidant concentration and the extraction time. The particles that were obtained after the extraction process will be used as porous hydrophobic catalytic and applied to AI mask and hydrophobic ink. Recent NIOSH published information has shown an increase of rapidly progressive coal workers? pneumoconiosis (CWP) in the southern Appalachian coal region (SAR) of the U.S., despite the fact that compliance data indicates that most coal miners have been exposed to coal mine dust concentrations below the statutory limit of 2.0 mg/m³. While the exact cause of these elevated CWP levels in the SAR has not been established, several factors may be contributing to the increase in occupational lung disease among coal miners. The mining of high rank coal is known to lead to higher CWP rates, and this type of coal is mined in a portion of this region. Also, a high percentage of the mines in the region are on reduced dust standards because of the high silica content of the airborne dust resulting in miners possibly being exposed to excessive amounts of respirable silica dust. Exposure to excessive amounts of respirable silica dust can lead to silicosis, a disabling and potentially fatal lung disease. NIOSH's Respiratory Hazards Control Branch has been investigating the possible causes that would account for the observed higher trends in disease progression through literature review, data analysis, and in-mine surveying. The investigation to date has revealed that underground mines are faced with cutting large amounts of rock in order to maintain haulage clearances. Cutting rock not only increases the potential for silica exposure, it increases the requirement for machine maintenance which was a concern observed during dust surveys conducted by NIOSH. Over half of the mines operating in the SAR are on reduced standards due to high silica content. Adequate face ventilation of the continuous miner and roof bolter and limited down-wind operations from the miner are also issues of concern and items which require the constant attention of miners operating in these conditions. The United States continued to depend on imported bauxite during 1995. Domestic bauxite mine production remained at about the same level as in 1994. It amounted to less than 1% of total world production. All of it was used for nonmetallurgical products, such as abrasives, chemicals, proppants and refractories. Metallurgical grade bauxite was imported from 12 countries. Principal suppliers for this 10 Mt (11.1 million st) of ore were Guinea (39%), Jamaica (35%), Brazil (17%) and Guyana (8%). The average value of imports, c.i.f. US ports, was $31/t ($28/st). Calcined refractory-grade bauxite imports totaled235 kt (259,000 st). It had an average value of $112/t($102/st) c.i.f. US ports. China supplied 61%, Guyana30%, Hong Kong 4% and France 3%. Imports of other grades of calcined bauxite, primarily for the abrasives market, totaled 259 kt (286,000 st).They were supplied by China (59%), Greece (20%), Brazil (14%) and Australia (6%). The average value was$74/t ($67/st) c.i.f. US ports.

Perhaps the most significant development in China's base metal sector is the discovery of major copper porphyries in northern Jiangxi Province and the adjacent southern Anhui Province. At the center of the new exciting district is the proposed Dexing copper complex, located some 350 km east of Nanchang, the provincial capital of Jiangxi. This project represents China's first attempt to put together a major large-scale open-pit operation, comparable in magnitude to Arizona porphyry operations. With the aid of Fluor Mining and Metals, the Chinese plan to develop a 135-175 kt/d open-pit mine, a matching large-scale mill, and the infrastructure needed to accommodate the big operations. An operation of this magnitude would significantly reduce China's import requirements by some 75%. China imports of the red metal are running around 200 kt/a. Under the direction of the Ministry of Metallurgical Industry, the Chinese are currently operating a 10-kt/d mine and mill with equipment primarily of Chinese origin. Dexing engineers told ME that they are considering a possible scale-up to 20 kt/d. How this effort will ultimately jell with the major expansion program is largely a matter of conjecture. Fluor Mining and Metals received a $10-million contract in December 1978 to develop a preliminary engineering and design program. While the original plans called for a 175-kt/d capacity complex, the Chinese subsequently decided to scale it down somewhat. The current proposal is around a 135 kt/d, which would in- elude the existing operation, according to Dexing engineers. Completion of the engineering and design phase is expected by March 1980 and a go-ahead decision will likely be made at that time. The value of the engineering and construction contract is estimated between $800 million and $1 billion. At right, mined ore is dumped 150 m down the mountain to a holding pocket below for rail haulage to the processing plant. Below, a Chinese miner tackles the universal problem of hard toes in the pit with a jack leg. Reserves and Early History The Dexing mineralization consists of three principal ore bodies, including the Chushahung on the north mountain, and the Tungchang and Fuchiawu on the south mountain. Combined ore reserves are estimated by mine geologists at 1.7 Gt of 0.4-0.5% copper. As is characteristic of porphyries, the ore contains important associated values of molybdenum, rhenium, pyrite, gold, and silver. There is also some secondary calcocite and bornite with an oxidized capping. Historically, copper was recovered with iron from the pregnant solutions emanating from the Tungchang deposit as early as the Tang Dynasty (700 AD). The implementation of advance control systems brings with it the expectation of improvements in process performance and controllability. Specifically, expectations include a shift in the mean value of the gross measure of process performance, and/or a decrease in process variability. When determining statistically-defensible measures of these benefits care must be taken to preclude process parameters from biasing the determination. Specifically, the effects of ore type, shift personnel, water availability, etc., must all be separated from the effect on performance of the advanced control system. Methods are presented for separating the effects of process control from other process parameters. Examples of the required data analyses are also presented.

A novel concept involving the deployment of chemical stabilization has the potential to eliminate the need of slurry impoundments and significantly reduce the release of trace elements from the high surface area fine coal waste. The process involves mixing the fine waste generated from the underflow of a thickener operation with chemical stabilization agents to tie up the water and to create a solid waste material that can be discharged directly. Portland cement, sodium silicate, F type flyash, C type flyash and hydraulic lime were evaluated as stabilizing agents. A detailed experimental study has been conducted to identify the conditions that provide the maximum compressive strength within a short curing time. Results to date show the ability to achieve compressive strengths adequate for equipment to maneuver on using a Portland cement concentration of around 10% by weight. Quick-setting concrete was not found to enhance stabilization time and compressive strength.

While the Kansas River Tunnel is a relatively short, small diameter tunnel, the shaft excavation methods implemented were long and large on innovation and use of reverse circulation drilling equipment. ATS Drilling, L.P. used mechanical excavation techniques for both soil and rock segments of the 42.7 m (140 feet) and 38.1 m (125 feet) deep shafts that were 2.9 m (9.5 feet) and 4.1m (13.5 feet) diameter, respectively.

The flow behavior of bubble-particle aggregates in separation tanks is important for many flotation systems, particularly for wastewater treatment. These aggregates can be considered as the dispersed phase in a continuous liquid phase of water and the flow characteristics determine the efficiency of the bubble separation process. In this work, a general methodology for the computational fluid dynamics (CFD) simulation of a bubble separation tank is described. This system is analyzed by Eulerian-Lagrangian approach. The momentum transfer from the dispersed phase to the continuous phase is quantified using the Particle-Source-In Cell (PSI-Cell) method. With CFD simulations, the effectiveness of different tank design parameters are evaluated in terms of the extent of bubble separation.

A Method to Calculate Hydrodynamic Loads on Underground Bulkheads Bulkheads are structures built in openings in underground mines to impound water, preventing the water from entering the active mine workings. These structures not only have to be designed to withstand the static, hydraulic pressure, but also the dynamic or seismic pressure, which would be generated by an earthquake. Bulkheads can be considered underground dams. Unlike dams on the surface, bulkheads are often subjected to hydraulic heads that exceed the heights of the bulkheads. Theodor von Karman in 1933, and Westergaard earlier, developed the equation by which the hydrodynamic pressures on surface dams could be calculated. Civil engineering handbooks of today contain this equation. In this paper, using the methodology developed by von Karman and Westergaard, the authors have developed an equation by which the hydrodynamic forces on bulkheads could be calculated when the hydraulic head exceeds the height of the bulkhead.

Through the process of Riverbank Filtration (RBF), the Louisville Water Company plans to obtain a higher quality source of water for their B.E. Payne Water Treatment Plant (WTP) by means of four collector wells located in the alluvial aquifer along the bank of the Ohio River. The collector wells are connected to a 2,850 L/s (65 mgd) pumping station by a 2,362 m (7,750 ft) long, 3 m (10 ft) finished diameter tunnel located in limestone and shale bedrock, which is approximately 45 m (150 ft) below the surface. High tunnel boring machine (TBM) production rates were achieved in this moderately weak bedrock; most of the tunnel only required support in the crown. An unanticipated water-bearing fault and unanticipated ground behavior proved challenging and required quick thinking to minimize impacts to construction. Tunneling conditions are discussed in detail, together with the methods used for overcoming the groundwater and ground behavior challenges. The tunnel is expected to be completed in April 2009.

On Friday, May 27, 1983 at about 6 p.m., an avalanche occurred at the Revenue-Virginius Mine. Six mine personnel and two contract diamond drillers were trapped when the avalanche covered and blocked the portal, the only access into the mine. The San Juan Mountain Range in southwestern Colorado is an old volcanic caldera. It has dikes and other intrusions, many of which are mineralized with sulfide ores rich in silver, gold, lead, zinc and copper. Several peaks rise above 4,260 m (14,000 ft) and the district has been the location of avalanches dating back to prehistoric times. This is evidenced by the numerous snow-slide chutes that are infamous enough to be named on topographic maps.

The National Institute for Occupational Safety and Health (NIOSH) has been conducting research at a Pittsburgh Coalbed longwall mine to evaluate and optimize bleeder ventilation and gob gas venthole longwall methane control systems. Gas flow into these two methane control systems was investigated using a combination of Sulphur Hexafluoride (SF6)tracer gas studies and gob venthole pressure monitoring experiments. The insights gained from this research has facilitated the development of a longwall subsided strata methane control system conceptual model. This model provides the basis for design suggestions to increase the efficiency of the gob gas venthole system to permit the bleeder ventilation system to become a "safety net" for methane control.

The Colorado portion of the Rio Grande rift is clearly defined physiographically from the south in the San Luis valley northward to the latitude of Leadville where sediments of the Dry Union Formation (Tweto, 1961) accumulated within the subsiding rift zone. To the north of Leadville is a system of faults active in the late Tertiary, including the Mosquito fault that offset a portion of the Climax mineralization (Wallace et al., 1968). Tweto (197B) describes remnants of Dry Union Formation against the Mosquito fault north of Dillon in the Blue River valley, so it is apparent that the rift and associated sediments extended well to the north of Leadville. Kelley (1952, 1954, 1956) and Chapin (1971) have noted in New Mexico that the Rio Grande rift is a series of en echelon basins, offset to the east, with constrictions connecting them (Fig. 1). In Colorado the structural basins are offset to the west. The net effect of all the rift zone basins in New Mexico and Colorado is an arcuate pattern (Fig. 1) abutting the Colorado Plateau and Sari Juan Volcanic Province on the west. The width of the rift system gradually decreases northward to about Leadville where rift definition is not apparent.

The use of Tunnel Boring Machines (TBMs) for tunnel construction of long tunnels in the Andes of South America has been of mixed success in the past due to a variety of reasons and key lessons can be recognized. A review of past TBM tunnel projects in the Andes has unveiled some interesting findings for different TBM tunnel approaches with both world records of progress as well as nearly in-completed projects with machines being abandoned. Key considerations for the use of TBMs for tunnel construction in the Andes include geological issues; (faults, rock type, rock strength, rock abrasivity, durability, groundwater inflows),depth of cover and the potential for overstressing/rockbursts, site access and terrain, portal locations, intermediate access possibilities, minimum tunnel size, support requirements, contractor and labour experience, and project schedule demands. TBMs offer highly recognized schedule benefits(high progress, no concrete lining) for a project if intermediate access adits are limited as well as hydraulic benefits for water conveyance projects. Minimum specifications for TBMs can be easily developed to suit the unique requirements of any project so as long as all factors are carefully considered. The overall conditions and factors associated with some projects may suggest that it is not appropriate to use TBMs due to a perceived high risk. Finally, access for geotechnical investigations by drilling in the commonly steep terrain of the Andes is typically difficult and often not impossible. Alternative approaches for geotechnical investigations for TBM tunnels including comprehensive mapping, high-resolution airborne electric-magnetics for delineation of possible fault zones, and rock block sampling is presented and discussed.
