Safety is a key value in most leading mining companies. Despite years of effort, and significant expense in ‘programs’, operating sites seem to have been left with no real improvement in either the elimination of fatalities or sustained reduction in injuries.Safety systems and processes have grown and evolved, often to the point where they have become overly complicated and marginally useful.MMG Limited, following the consolidation of its Australian operations under a single management structure, undertook a program to effect real and sustained change in its approach to managing safety. The program aimed at improving safety by better engaging its workforce and by making safety processes simple, useful and effective. It defined the role of the supervisor in creating safe work, the role of the people doing work and the role of safe work methods.A safety diagnostic tool was developed to assess site against safety-critical mindsets, behaviours, cultures and systems. The outcome of the diagnostic is considered by the organisation to be the true lead indicator of safety performance.While still a work-in-progress, the program at the Australian Operations of MMG Limited shows that challenging common paradigms in safety management can create real change in safety performance.CITATION:Webb, M E, 2016. of the multitude of physiographic problems of immediate interest in volcanic terranes, it is the origin of the magma forming the magnificent volcanoes that is perhaps the most mysterious. Unlike anywhere else on earth, the possible locations of magma production beneath the convergent margins are strongly limited in depth and areal extent. Yet the exact process by which magma is produced still stands elusive, even in light of tremendous gains in analytical and physical insight. The principal aim here is to discuss some of what has been revealed about the physics and chemistry of the origin and evolution of island are magmas.

High-nugget effect vein-gold systems are generally considered to be one of the most challenging of deposit types to evaluate and exploit. To potential investors and mining companies, they are viewed as high risk because of the associated uncertainties in the grade estimate and general paucity of Ore Reserves at production start-up. However, despite the well-known risks, these deposits often have a grade upside that makes them potentially attractive. The reporting of resource grade is of importance and perhaps even more so is the reporting of the associated uncertainty attached to the grade estimate. The use of the grade estimate supported by a grade range is recommended to achieve more complete and useful disclosure. The definition of the grade range can be somewhat subjective, ranging from opinion based estimates through to data-based estimates and geostatistical conditional simulation. Whichever method used, the over-riding issue must be the provision of enough data of appropriate quality and full disclosure to the public of relevant uncertainties.CITATION:Dominy, S C and Edgar, W B, 2012. A description of general mining proced- ures and tyre maintenance practice is given with particular reference to tyre mainten- ance facilities and record keeping at Groote Eylandt. Experience with tyre wear on Wabco 85C rear dump trucks is then outlined, concen- trating on wheel camber, wheel alignment, truck speed, tyre pressure, loading and haul road design. T T.1TUnilhTPTTnAT The Groote Eylandt Mining Company Pty. Limited mines and beneficiates sedimentary manganese ores from a series of orebodies occurring in the western part of the island. The island is one of a group on the western side of the Gulf of Carpentaria, about 50 kilometres from the Arnhem Land coast and about 650 kilometres from Darwin. Whilst capacity of the mine is presently 2.3 mil- lion tonnes of product per annum, current production has been reduced to just more than half this figure, due to a general mar- ket recession.

Hydraulic Monitor mining at Solid EnergyÆs Spring Creek Mine has proven to be a safe and effective method of extracting steeply dipping thick seams at production capabilities approaching one million tonnes per annum.   This paper traces the development of Hydraulic Monitor mining from its initial introduction into old workings at Strongman Mine, its refinement at Strongman 2 in shallow, thick and steep workings, through to its introduction into the deep, very thick seams at Spring Creek Mine. This last step of the progression came with all the added complications that come with depth, particularly flammable gas.   This paper will describe the various changes in mining conditions that dictated the performance of the monitor, how the mine layouts were changed to adapt, and the various operational systems that were put in place, to allow a safe and productive operation.

The twin iron ore islands of Cockatoo and Koolan are located in the spectacular setting of Buccaneer Archipelago off the Kimberley coastline of WA.Cessation of ore shipments from Koolan in 1993 ended 42 years of continuous mining activity which began with the first shipment of ore from Cockatoo in 1951.

In early June, 1986, a fire of unknown origin erupted in one of several main entries of a coal mine located in southwestern Colorado. The fire was aggressively fought with high-expansion foam. As a result of several strategic decisions, the fire appeared to be held in-check. Unfortunately, sufficient quantities of foam were unavailable and as soon as the foam was depleted, control of the fire was lost. To prevent further damage and the possibility of an explosion, all entries to the mine were sealed.

Incidents that trap miners in underground mines have serious consequences for the miners and the mine operators involved. It is of utmost importance that mine operators utilise all available tools when trying to locate and rescue trapped miners. From testing conducted at underground hard rock mines, Cumming-Potvin et al (2013) found that pre-existing microseismic monitoring systems, which are routinely used for seismic risk management, have the potential to be used for detecting and locating trapped miners.This paper reflects on the Anzac Day seismic event that trapped two miners at Tasmania Mine, Beaconsfield, on 25 April 2006. The Anzac Day event occurred at 2123 on a Tuesday evening. Rescuers were unable to confirm that the trapped miners had survived until 1930 on Sunday evening. It is not possible to speculate on how the rescue operation may have been different had the technology to use the microseismic monitoring system for the purpose of detecting the trapped miners been available. However, it almost certainly would have been different. The recent research project has demonstrated that knowing the fate of the Beaconsfield miners might have been achievable up to 100 hours earlier than was the case in 2006. That knowledge would have been invaluable to the rescue effort.By examining the seismic array at the Tasmania Mine, the reliability of detection over the working levels of the mine could be quantified. Had the methodology been available, adding six additional sensors (at a cost of $40 000), would have been sufficient to increase the likelihood of detection to greater than 90 per cent in the working levels of the mine. This was true both at the time of closure of the mine in 2012, and just prior to the Anzac day event.CITATION:Cumming-Potvin, D and Hills, P B, 2014. Poor ground conditions associated with historical workings have presented significant challenges to the drilling of production downholes at the Sons of Gwalia Mine. The combination of foam and polymer drilling, the use of Aquacrete to drill through voids and the practice of lining completed holes has allowed downholes to be effectively drilled and blasted in delaminated ground.

Segregation studies in ternary systems of glass ballotini, iron ore and coke particles (with size range of 1-8 mm in each) in filling and emptying a laboratory size Paul Wurth hopper are described. In-bin segregation is shown to be a function of the position in the hopper, the nature of the material and the feed composition. The in-bin results are correlated by the equation C = C0k, where C and C0 are the size compositions (wt per cent) in the hopper and feed respectively, and k is an experimentally obtained segregation constant. The results of now pallern measurements show that material discharges from the hopper according to the sequence; core now - wall now - scaffold now, where the latter represents stagnant material at the hopper wall that discharges last. Size segregation in the discharge stream from the hopper is shown to be a function of in-bin segregation and hopper now pallern. A method of predicting discharge segregation from these two sets of data is suggested. The results of the model studies are also compared with industrial data and these show satisfactory agreement. Discussion of the hitherto ignored effect of size segregation on hopper now rates, stoppages and other operating problems is also included.

Upper Cretaceous and Eocene coal measures of the Ohai Coalfield are stratigraphically complex, and coal seam geometry and quality are influenced by washouts and partings resulting from accumulation in a fluvially active environment. Mining efficiency is influenced by these seam discontinuities, which are difficult to predict in the absence of refined paleoenvironmental models of peat accumulation. Petrographic variations between the Upper Cretaceous and Eocene coals indicate differences in the environments of peat accumulation. Seams in the Upper Cretaceous Morley Coal Measures accumulated autochthonously (in situ) in predominantly forest swamps with a high water table. These swamps were occasionally subject to brief floods. No significant changes in swamp vegetation, as indicated by petrography, have been detected between seams. In contrast, the greater content of mineral matter and maceral detritus within the Eocene Beaumont Coal Measure seams indicates hypautochthonous (locally transported) peat accumulation under more frequently flooded conditions. A precursor forest-moor is again indicated but the greater resin content of these coals compared with the Upper Cretaceous examples may suggest a greater contribution from conifers. Integration of coal petrography with investigation of other seam characteristics and coal measure sedimentology is expected to result in a detailed understanding of original depositional controls on peat accumulation. Paleoenvironmental models derived in this way should facilitate prediction of seam geometry, roof and floor conditions, and variations in coal quality, with obvious benefits to exploration and mine development.

Norwich Park Mine commenced production of export coking coal in November 1979. Attention to consumables control was limited during the early running of the plant. Once higher than anticipated magnetite consumption levels were confirmed, efforts were directed at determining the causes. Extensive plant sampling, monitor- ing of operational procedures and plant modif- ications have led to a significant reduction in magnetite usage.

The long continuing controversy over the Origin of fusain, whether it is a product of primeval forest fires, or of special conditions of weathering (including bacterial decay) of the vegetal debris concerned prior to, burial, has now been extended, to include the possibility that fusain is a product of the coalifaction process subsequent to deposition, so that doubt may reach even to its time of formation.The conflicting views on this point are summarised in the following quotations from two recent publications (my italics in each quotation):1. "Recent observations made in peat and brown coal deposits in Australia, England and Germany strongly suggest that practically all fusain is formed by coalifaction after deposition and 'preliminary compression. In deposits of advanced brown coal rank, it normally takes the shape of pieces of wood compressed parallel to the bedding plane; and some fragments consisting partly of fusain and partly of vitrain exhibit the same amount of compression throughout. This would seem to indicate fusinisation after burial, as hard brittle charcoal would be crushed, or else retain its original irregular shape if strong enough to withstand pressure.In some immature deposits, however, where cover is of a hydrostatic nature rather than a vertical compressionar force, occasional pieces of fusinised wood showing very little flattening are present, but such immature deposits seldom contain the proportion of fusain found in higher rank coals. In view of this, and the possibility of introduction of charcoal into peat deposits, it is felt that a small amount of the fusain in coal may represent wood charred before burial, but most of it almost certaInly originated as a product of fusinisation after burial" (Dulhunty, 1948, p.14).

As a prelude to studying the sorption desorption and permeability behaviour of coal cores in a triaxial stressing rig using sorb- able gases, the air permeability of typical cores was measured. Lump coal samples were obtained from three gas-troubled collieries in Queensland and New South Wales. These were cored in the laboratory and a simple laboratory permeameter employed which accepted 38 x 40 mm cores in a thick rubber sheath. Air pressure of 500 kPa was applied outside the sheath to provide suitable sealing between the rubber and the coal. Differential driving pressures up to 100 kPa were employed and the air flow measured by displacement of water columns in inverted graduated cylinders. Flow measurements accurate to + 2% could thereby be achieved. Coal lumps were cored both parallel and perpen- dicular to the bedding plane, and were observed prior to measurement for any notable features, in particular for the presence of fissures. The presence of coal in the Sydney area was confirmed in 1891 when Cremorne Bore No. I intersected cindered coal at 1 854 m. The decision to develop Sydney Harbour Colliery was based on the results of Cremorne Bore No. 2 (drilled in 1892) in which a 3.12 m. seam of unaltered coal was encountered at 889 m. Geologists from the New South Wales Department of Mineral Resources consider that this seam is probably equivalent to an upper split of the Bulli seam. Long Cove at Balmain was selected by Sydney Harbour Collieries Ltd. as the site for mine development in 1896. This was due to opposition to the proposed establishment of a colliery on the north shore of Port Jackson near Cremorne. Construction of the main access and ventilation shafts (termed Birth- day and Jubilee), other mine infrastructure and the quay wall occurred between 1897 and 1902. A fatal accident in which five men were killed occurred during the construction of the Birthday Shaft in 1900. This acci- dent resulted in amendments to the Mining Act providing for guide rails in shafts.

The performance of a hydrocyclone treating fine coal has been examined over a wide range of feed and operating conditions.The results were expressed in terms of partition curves from which the water split to the oversize flowstream,the corrected separation size and reduced partition curves were determined-.The following conclusions were obtained. (a) The water split to the oversize flowstream was found to be dependent upon the vortex finder and spigot diameters of the cyclone and the flowrate and solids content of the feed slurry. (b) The corrected separation size was dependent on the solids content of the feed,and the vortex finder and spigot diameters.The hydraulic pressure (or flowrate) of the feed slurry did not have a significant effect.This phenomenon could be explained in terms of crowding of the particles at the Spigot.
