Permanent rock bolts used to support rock tunnels are required to perform for the design life of the structure as a minimum. Quality of installation is critical to the performance over the life of the rock bolt.This paper will present the findings from an in investigation into the integrity of rock bolts installed in an underground hydroelectric power station cavern and access tunnel. The 25?mm diameter rock bolts were installed 50 years ago, tensioned and post cement grouted. Due to a major machine upgrade the whole facility was subject to an asset survey. The owners are responsible for managing the risks associated with the failure of these rock bolts and ensuring the structural integrity of the tunnels and caverns is maintained to continue operations, protect machinery, assure production and more importantly maintain a safe working environment for all employees and visitors.To help manage risk a proactive approach is desired. To this end a test schedule using a non-destructive testing (NDT) system was carried out by Independent Integrity Testing Pty Ltd (i2t). The system used in this application was RokTel™. The system operates by applying a small axial impact to the rock bolt end and measuring the resultant acceleration response. The rock bolt characteristics affect the acceleration response.The data output from the system is analysed principally using Fourier Analysis (FFT) and sinusoidal waves. The energy generated in the bar, the rate of damping and changes in frequency are all assessed. Testing was carried out on 70 rock bolts within the tunnel and cavern. A selection were then over cored, photographed and documented. The data gathered from the testing is then correlated with physical evidence to establish a calibration for the remainder of the rock bolts and assess if this is a viable method for identifying defective elements, and what degree of accuracy can be achieved. The development of the Australasian Code for Reporting of Identified Mineral Resources and Ore Reserves is a significan step towards informing the financial community and the public about the future profitability and risks of investing in resource companies. The Code is also important from the accountants' point of view because it provides them with a well-defined bash for assigning costs or attributing value to the assets of resource companies and hence improving their financial reports. Although reporting to the public is ultimately the responsibility of management, accountants, like geologists, are frequently required to provide information for inclusion in these reports. Jr the last ten years, there has been a considerable refinement in the reporting of mineral resources which can be attributed mainly tc the progress made towards developing the Code. It seems tha the classification and terminology that the geologists have developed are widely used in the annual reports. However, little progress has been made by accountants in improving the financia information. The purpose of this paper is twofold: (a) to outline briefl) some of the major problems in accounting and reporting fo; mining companies and; (b) to examine the potential contribution of the Code in solving those problems.

The feasibility of increasing the deportment of titanium species and the stability of the pseudobrookite solid solution (M3O5) in a titaniferous slag was investigated, to facilitate the valourisation of titaniferous slags. Thermochemical simulations using FactSage™ software, ver 8.3 (by Thermofact/CRTC and GTT-Technologies), and equilibration-quench experiments were conducted using an appropriate synthetic titaniferous slag composition in an oxygen partial pressure (pO2) ranging from air to 10-16 atm under non-isothermal conditions. The temperature-pressure phase stability diagram for the synthetic slag containing 37.19 per cent TiO2, 19.69 per cent SiO2, 13.12 per cent Al2O3, 16.00 per cent CaO and 14.00 per cent MgO showed that the spinel solid solution (M3O4) was the primary phase until a pO2 of 10-10.5 atm, thereafter M3O5 was the primary phase until a pO2 10-13.2 atm. M3O4 again was the primary phase between a pO2 10-13.2 and 10-16 atm. The changeover of the primary phase coincided with an increase in the liquidus surface. When cooling the slag in a pO2 of 10-16 atm, M3O5 crystallised as a ternary phase that showed a rapid increase in mass while that of the primary M3O4 phase decreased and that of the secondary perovskite (CaTiO3) phase increased at a slower rate. At 1300°C under sub-liquidus conditions, the contents of M3O5, M3O4 and CaTiO3 were about 20 per cent, 15 per cent, and 11 per cent, respectively. Experimental results showed that the Ti3+/Ti4+ mass ratio in the M3O5 phase, ie a measure of the reduction of titanium species, is not considerably sensitive to equilibration temperature in the reviewed range. Slow cooling and minimum/no oxidation during cooling of titaniferous slag would increase the deportment of titanium species to, and the growth and stability of the M3O5 phase.

"Ventilation risks in an underground mine have essentially remained the same for centuries, yet theevidence suggests that there are many coal mines that have failed to adequately control these risks.The conduit for the knowledge learned by previous generations exists in the mining legislation. In an ideal world, mines would not need legislation to tell them what to do; they would resolve the risks to an acceptable level so as to protect the safety of persons in the mines based on the acquired knowledge. There will be some who have been personally aff ected by a mining disaster but for the majority they will be remembered as a vague recollection of an event from long ago. The mining legislation is the repository for the knowledge and outcomes from inquiries of past disasters. The challenge for the current generation, and the generations to come is to appreciate the legislation that exists, so as to not ælearn the hard wayÆ. In reviewing most disasters there is comment or discussion on the adequacy of the legislation, which ultimately results in some amendment to the law. For each person, there is an increasing responsibility to ensure that the same mistakes are not repeated. Understanding the legislation, its history and its purpose is the fi rst step in ensuring thatventilation risks are eff ectively managed. Failing to do this will be disastrous. This paper discussesthe relevance, importance and potential pitfalls of coal mine ventilation regulation."

Since the early 1900s the geotechnical community has investigated every significant failure of a tailings dam, improved our knowledge of the behaviour of the tailings storage facilities, developed analysis methods, compiled lessons learned, and committed to zero failures. The rate of occurrence of tailings dam failures has reduced since the 1980s, however, failures continue to occur and continue to surprise us. At BHP, we have decided to take an approach to the tailings management that is similar to the approach to safety. We are looking at ways to eliminate, substitute, separate or engineer around the hazard (tailings dam failure). As such, our ultimate long term goal would be to eliminate the need for new tailings facilities. Unfortunately, we have not developed yet technologies for extracting many of the common metals and minerals without producing tailings. Our shorter term goal is to substitute the current conventional slurry tailings facilities with less risky dryer facilities. We are working with vendors and research organizations to develop technologies that can be efficiently applied on a large scale. At the same time, we are committed to using the best available engineering to reduce the risks associated with our existing and new conventional tailings facilities. The best engineering can be achieved by people with the appropriate education, experience, attention to detail, discipline and authority. We have implemented a system of governance to make sure, we have the best engineering applied to all our facilities, considering the specific conditions at each site.This paper presents the current tailings governance process at BHP, and the initiatives that were undertook towards achieving a zero failure goal. Every person in an organisation has what could be called ‘a different take’ on what it means to be a safety leader. The directors will tell you they are ultimately responsible for everyone’s safety and that they can be imprisoned as a consequence of a workplace fatality. Managers, take more of a focused approach and would explain that they are totally responsible for the safety of their teams. And then there are front-line leaders. Those that not only have the responsibility (like directors and managers) but they also have the accountability, and the ability, to influence how safely their teams work. But it is not just physical safety that leaders should be concerned with.

Chance find of an economically important mineral on the sur- face and its exploitation without proper exploration work, has become the habit of humanity since the Bronze Age. The explora- tion and exploitation techniques have, however, been substan- tially improved in the technologically developed western world and system controlled major communist countries. Nevertheless, the greatest need is felt in the Third World countries where large mineral potential exists, but with inadequate investments in exploration and exploitation techniques, a handicap that has been beneficial to the more developed countries, since long. The great number of the Third World countries which are striv- ing hard to develop, need to formulate mineral exploration and exploitation strategies which could enable them to develop their resources. This requires planning and creating new concepts of ore deposit types and genetic models, rheology, economics and their innovation and integration. The following approach could help them to be economically "more independent" by the end of this century. 1. Bridging of pure and applied geology gap, and creating con- sciousness of research and development. 2. Revision of mineral resources concepts adopted to modern technological world. 3. In the stability assessment of open pit walls, geotechnical engineers have to deal with uncertainty related to structural geology, rock mechanical parameters, groundwater and so on. This uncertainty will immediately and significantly impact on wall stability assessment and therefore. influence mineral recovery and ultimately the financial return of open pit mining. However, the conventional factor of safety applied to measure the stability of slopes and the relevant analytical computer packages cannot quantitatively consider the variability or uncertainty of the input data. As a supplement, therefore, geotechnical engineers have begun to apply probability to the stability assessment of open pit walls over the last two decades. To-date unfortunately, no powerful computer package is available to geotechnical engineers to conduct the probability analysis of slope stability. This paper presents a computer package of probability analysis for rock and soil slope stability. In this computer package, the strength parameters of rock and/or soil material such as cohesion, friction angle and unit weight of the different rock and/or soil type domain in a complex structural geology environment can be considered as the basic input random variables. Groundwater level can also be taken into account as a random variable in this package. Two `rigorous' limit equilibrium solutions, the Morgenstern/Price method and the Sarma method, can be used to calculate the factor of safety during Monte Carlo Simulation applied in this program. Five types of probability distribution are included in this package for the user to simulate basic input random variables. Two realistic cases from BHP Iron Ore are used to illustrate this package.

The Mount Morgan minesite, located approximately 40 km SW of Rockhampton in Central Queensland, comprises 270 ha of unrehabilitated land which is the currently the major source of acid mine drainage run-off and seepage to the adjacent Dee River. Additionally, the minesite hosts an Open Cut Pit which contains 9000 ML of acidic and metal-rich water. In the absence of active management of water in the pit, there is a 50 per cent probability of an uncontrolled spill occurring in future years. The development of a contingency plan for management of overflow from the pit in the short term, has incorporated findings from numerous studies relevant to the Open Cut Pit including, but not limited to, water balance, contaminant source identification, toxicity testing, water quality profiling and limnological modelling, and estimates of probability of overflow. Extensive stakeholder consultation and peer review were also undertaken during the formulation of the contingency plan. The medium-term goal of lowering the water level in the pit to reduce the current high probability (50 per cent) of uncontrolled overflow to acceptable levels of risk is proposed to be achieved by a campaign of chemical water treatment. The treated water will be discharged to the Dee River, and sludge returned to the pit. The methodologies used for the selection and development of the water treatment process, that is proposed to involve treatment with lime, are described. Water treatment is the short- to medium-term component of a long range rehabilitation plan for achieving a negative pit water balance, and sustainable management of acidic water across the whole site. Achieving this ultimate goal will involve: reducing the inputs of water to the Open Cut Pit such that the annual water balance is zero or net negative, thus removing the need for water treatment in perpetuity; maximising, by surface rehabilitation works, the quality of run-off reporting to the Dee River; and minimising the volume of acidic seepage reporting to the seepage interception system. Possible strategies for achieving this long range goal consistent with overall site rehabilitation objectives are currently being developed and costed.

The ultimate goal of such programmes should be directed toward eventual operation and management of the established industry by the local population.

Standards in the public reporting of resources and reserves in Australia have dramatically improved since the implementation of the Australasian Code for Reporting of Mineral Resources and Ore Reserves (The JORC Code) in 1989. Whilst the JORC Code states clearly that Mineral Resources and Ore Reserves are imprecise calculations and should be treated as estimates, there is strong evidence that many people regard them as fact, including some accountants, directors, auditors and investors. Mineral Resources and Ore Reserves are key inputs in the valuation of a mining company and the determination of financial performance. Geological knowledge is the foundation upon which Mineral Resources, Ore Reserves and investment decisions are made, however this information carries with it substantial uncertainty. Case studies and company reports have established that errors derived from inadequate geological inputs have led to reduced profitability in operating mines and in extreme cases the premature closure of mine operations. Whilst there are many causes of mine closure and reduced profitability, including commodity prices and exchange rates, the cumulative effect of all geological uncertainty contributes significantly to the erosion of the credibility of mining within the investment community. There are a number of methods used to characterise and model uncertainty in Mineral Resource and Ore Reserve estimation. Empirical mining indexes, sensitivity analyses, Monte Carlo simulations and Conditional simulations are used to identify and estimate geological uncertainty, notably the quantification of uncertainty in grade and tonnage estimates. These techniques have not generally been applied to estimating geological uncertainty associated with key Ore Reserve inputs such as appropriate mining technique, equipment selection, production rate, mining dilution, stability requirements, materials handling, metallurgical processing, product quality and waste storage. New software tools are needed to model the entire mining process with all its geological and other interdependencies, enabling all uncertainties to be separately identified and estimated. By its very nature, mining involves the assumption of significant technical, financial, political, environmental and social risk. Success comes by understanding and managing these risks and consequently the mining industry will never choose to reduce the level of geological uncertainty to nil. The ultimate aim of improved identification and estimation of geological uncertainty is to quantify inputs into the Ore Reserve process, allowing a company to assess the risks and manage these accordingly. This approach is relevant to feasibility studies for new projects as well as life-of-mine planning through to the weekly schedule at existing mines.

A sample of pyrrhotite flotation concentrate pulp assaying 15.3 g/t An, which had been prepared from an old tailing dump, was treated to extract the gold. Examination of the concentrate with an SEM showed that exposed or free native gold, 5-40 ¦m in diameter, was present but no locked gold was observed. Direct cyanidation of the concentrate to extract the gold showed that the flotation reagents present inhibited gold extraction. A range of pre-treatments was used to overcome these effects.Direct cyanidation for 48 h extracted only 26% of the gold, indicating possible contamination of the gold in the concentrate with flotation reagents. Pre-treatment of the concentrate pulp by aeration for 24 h, or by 72 h aeration with an addition of 3.65 kg/t of sodium sulphite, gave little improvement in gold extraction by subsequent cyanidation. Pre-treatment by filtration, or by aeration with 12.5 L/t 100 volume hydrogen peroxide, followed by cyanidation for 48 h gave gold extractions in the range 67-77%. Combining filtration and an aeration period with hydrogen peroxide addition raised the subsequent cyanidation extraction of gold to 88%, giving a tailing assaying 1.5 g/t Au.

As the resources sector becomes tougher with rising costs and cyclical metal prices, mining companies need to continue to reduce their costs and do more with less. Many new resource projects fail to come in under or on budget based on Feasibility Studies. Particularly, capital expenditure (CAPEX) proves difficult to achieve for resource projects. A number of resource projects have failed because of aggressive plant CAPEX cost cutting resulting in projects with no surge capacity and are unable to achieve design throughput. In addition, the plants are not operable because of the omissions. In recent times many businesses have also experienced the "costs" of simply implementing aggressive operating "cost cutting” measures as a strategy for solving business performance problems. This paper looks at technology driven, employment related, luxury cuts, department cuts and restructuring cuts. In the past companies used an incremental approach based on the use of past budget information as an integral part of the budget construction process going forward. The use of performance reports and Management Information Systems (MIS) is examined along with the role of continuous improvement in achieving sensible cost cutting.

Fire suppression systems are but one of the many technologies currently available to manage operational tunnel risks. However, the actual reasons for their use are often shrouded in mischievous discussions of the marketing variety.The international tunnelling community still stands divided on the merits (or otherwise) of fire suppression systems. For some the debate focuses on their cost compared to the theoretical benefits they bring to human safety.Long standing concerns about their potential adverse effects in an emergency are often at odds with the experience from tunnels which use them. High installation and operational costs are cited as reasons not to use them, yet toll road operators and insurers increasingly use them as a risk mitigation strategy for toll revenue protection.Fact or fiction, myth or reality, should such systems be seriously entertained or are they just another expensive gadget in the escalating array of expensive toys for modern tunnels? Ultimately this presentation argues that they have their place as a risk mitigation measure to achieve a range of objectives. The exact circumstances for their use ultimately fashioning their design and operational characteristics. This is an abstract only. No paper was prepared for this abstract.

OxianaÆs Sepon Copper Operation (part of the combined Sepon Gold and Copper Operation) is located in Savannaket Province, in southern central Lao PDR. The site is relatively remote, being some five hours inland from the Laos/Thailand border crossing on the Mekong River. The copper processing plant was designed to treat 1.3 Mtpa ROM ore to produce 60 000 tpa of copper cathode. Commissioning of the processing plant was completed in mid March 2005 with ramp up completed in January 2006 with 5260 tonnes of cathode harvested in this month. The focus of this paper is on SeponÆs unique grinding circuit. The entire grinding circuit is operated at highly acidic conditions, adding a number of different operational issues over and above those normally associated with grinding circuits. It became evident early in the ramp up, that reaching design mill throughput of 170 dry tonnes per hour with the original design would be challenging, coupled with the realisation that ceramic grinding media consumption had been underestimated. In order to lift the overall throughput of the processing plant, a significant continuous improvement process has been implemented within the grinding circuit to lift this key unitÆs production capacity. This presentation will focus on the key areas that have been addressed in the 12 month ramp up period and highlight areas for future work. Results to date have shown this improvement process to be very successful, with throughput rates in excess of 200 tph regularly achieved.

A study has been undertaken into mine ventilation systems currently in use within Australian modern coal longwall (LW) extraction mines. It reviews systems and discusses evolving changes being adopted to address the more complex challenges. There is a strong move to longer panels, wider faces, greater extraction heights, increased production rates, more efficient ventilation and decreased personnel. Each of these changes is motivated by improvement in productivity and generally leads to cost reduction. In addition, mine workings are moving deeper, which results in increased ventilation control issues such as higher total and respirable dust levels, greater seam gas contents in parallel with lower in situ permeabilities, spontaneous combustion and heat management issues. A comprehensive database of Australian LW ventilation practice is analysed. Currently there are a variety of LW panel ventilation circuits used in Australian underground coal mines due to various combinations of seam characteristics, gas emission rates, spontaneous combustion, geological features and surface constraints. The main issues usually addressed in the designing and planning of ventilation circuits for LW panels are airway velocity, gas concentrations, LW cutting methods (eg Bi-di, Uni-di or half web), ventilation of control devices, pressure differentials and leakage paths and understanding gas concentrations across the length and width of the goaf. If the ventilation circuit can manage the applied contaminant load (gases, heat and dust) at an acceptable cost and circuit duty, then supplementary controls, such as gas drainage, refrigeration and dust sprays and scrubbers, may not be required. Over the last six years Mandalay Resources has increased production rates and improved safety outcomes at its Costerfield mine in Victoria, Australia. This mine is an underground operation utilising narrow mining techniques to extract vertical veins, and produces up to 80 000 equivalent ounces of gold per annum, in both gold and antimony value.The operational challenges of narrow vein mining include controlling overbreak when veins being mined are only 300 mm wide. The mine has developed methods and modified equipment to suit extraction of these veins. Productivity improvements over the period have involved development of a stoping method and equipment to suit 1.8 m wide development drives. Over the last five years and during a skills shortage, recruitment, training and retention of personnel has been key to keeping corporate knowledge and skills to execute a careful efficient mining technique to minimise overbreak.Managing an underground operation close to nearby residents has unique challenges. Historically Victorian residents had family members work as underground miners; however, in the current day with few underground operations in this jurisdiction, community knowledge of mining has decreased. The Costerfield Operations are only 100 m from the nearest residence and the company values community engagement, partnership and investment with local Heathcote organisations.The mine has invested substantially in executive coaching of its leadership team early, which has paid dividends in problem solving potential of that team and ensuring the operation delivers on its promises to all stakeholders. The operation has become safer and employees are now more aware of their individual contribution in producing safe, efficient gold and antimony that benefit the whole operation, community and the region.CITATION:McCarthy, M, 2016. The Queensland Department of Natural Resources and Mines (NR&M) accepted the environmental liability for the Mount Morgan minesite in 1991. This gold and copper mine located 38 km southwest of Rockhampton had been in operation for more than 100 years. Mining leases are still active while the feasibility of a reprocessing operation is carried out. From 1993, NR&M have operated a seepage interception pumpback system which has reduced the chronic load of acid rock drainage (ARD) to the adjacent Dee River, a tributary of the Fitzroy River system. In early-2000, it was recognised that a ten-year plan for rehabilitation of the mine, along with significant funding would be required if a long-term sustainable rehabilitation strategy was to be implemented at this minesite, ultimately making the seepage interception system redundant. A rehabilitation planning workshop coordinated by NR&M was held in May 2000. This workshop brought to the mine a range of technical expertise and key stakeholder groups. This workshop provided a technical forum for review of studies undertaken and to identify priority projects, which needed to be undertaken in the future to more clearly define contaminant sources on the mine site and to prioritise the rehabilitation program. Stakeholder consultation enabled the project team to be clear on community and other government (eg Environmental Protection Agency) expectations. It is clear that the future of the economically depressed community of Mount Morgan will be reliant on the careful management of the significant heritage values at the minesite for tourism, education and research. In the first year (2000), priority studies were initiated, which included whole of site studies on water balance, contaminant sources and a review of occupational health and safety and environment of the minesite and conservation and heritage scoping. Baseline vegetation and soil materials surveys were also initiated. The outcomes of these studies helped to shape the work program for the following year, while providing a basis for an additional funding application, which was granted. The second year of studies focused on building upon the existing baseline studies completed. In addition, studies focussed on addressing the priority issue of contingency planning for overflow from the open cut pit in accordance with best practice management of release water and river monitoring strategies were completed. Completion of these projects over the next six months will enable the optimal rehabilitation strategy to be defined in 2003. This optimal strategy may require the use of a water treatment plant for part of the ten-year program to provide space in the open cut pit for the deposition of ARD contaminant sources (eg waste dumps, tailings). It is envisaged that some sources will be rehabilitated insitu while others will be relocated to the open cut pit. Implementation of this strategy will be dependent on subsequent funding.
