Occupational noise-induced hearing loss (NIHL) has been recognized by the National Occupational Research Agenda (NORA) as the most common job-related disease in the United States. Roughly 30 million workers are exposed to excessive noise levels or toxins that are potentially hazardous to their hearing. The National Institute for Occupational Safety and Health (NIOSH) Office of Mine Safety and Health Research (OMSHR)is investigating noise controls to reduce operator overexposures caused by haul trucks used in underground metal/non-metal mines. To identify the most significant contributors to the overall sound level, Source Path Contribution (SPC) analysis was conducted. Noise dosimetry and time-motion analysis were also conducted to quantify operator noise exposure. SPC analysis determined that, under high idle conditions, a source near a hydraulic pump mounted to the rear of the engine and the engine cooling fan were the major noise sources. The time-motion/dosimetry analysis showed that the operators accumulated more than 90% of their noise exposures while they were hauling ore to the dump site and traveling back to the load site unloaded.

The tests reported in this paper were made in the East Texas field in the summer and fall of 1932 as a part of the Bureau of Mines study of the operation of flowing wells and reservoir performance; and to determine, if possible, the minimum reservoir pressure necessary to produce the wells in this field by natural flow through different sizes of flow strings. The minimum reservoir pressure that would be required to maintain natural flow through 2 1/2-inch tubing was determined experimentally for the existing energy conditions in the East Texas field. With the present available data, however, it 'is impossible to predict accurately or to determine by calculation the necessary energy requirements for maintaining the vertical flow of oil and gas mixtures under all conditions. Although some valuable data were obtained which seem to have application to the solution of problems involving vertical flow, this important research of the petroleum industry is so involved that more study of the complicating factors influencing the flow of oil-gas mixtures through vertical pipes is required. Versluys4 has presented a mathematical analysis of the vertical flow of oil and gas mixtures which covers the fundamental theories of the problem-although, as he points cut, there are factors, particularly friction, slippage, and solubility of the gas in the oil that must be mown or determined experimentally before the equations he has developed can be applied to actual

The Cortez gold mine is located in Lander County, NR, about 130 km (80 miles) southwest of Elko. Cortez Gold Mines (CGM) is a joint venture between Placer Dome US Inc. (60% operator) and Kennecott Corp. (40%). The claims that supply oxide and refractory ore feed to the Cortez milling facility are located in both Lander and Eureka counties. These include the Cortez and Horse Canyon deposits, all operated by the joint venture. Other deposits outside the CGM claim area include Fire Creek, Buckhorn, Gold Bar and Tonkin Springs. These deposits all occur along the Battle Mountain-Eureka Trend. History Mining in the Cortez district began in 1862 with the discovery of high-grade silver ores. Grades in the hundreds of ounces per ton occurred in an area 7 km (4.5 miles) southeast of the current CGM mill site. Underground mining through the Garrison and Arctic tunnels provided the bulk of the ore production between 1864 and 1895. To a lesser extent, the Mill Canyon area also supplied ore until 1930.

The Leuna Factory of the I. G. Farbenindustrie A. G., situated near Merseburg, is the biggest heavy-chemical plant in Germany and produces a very wide range of products, mainly based on hydrogen or hydrogen and carbon monoxide. Its principal products are ammonia, synthetic petrol derived from the hydrogenation of brown coal, and synthetic alcohols made from hydrogen and carbon monoxide. The plant was first, visited on April 21, 1945, by a team including E. Cotton, V. Haensel, E. B. Peck, P. K. Kuhne, H. M. Weir, L. L. Hirst, W. W. Odell, G. U. Hopton, H. Hollings, J. F. Ellis, and H. G. Simpson. It was again visited on May 8-24, 1945, by a combined British and American team consisting of R, Holroyd, M. A. G, Banks, D. A. Howes, A. J. V. Underwood, R. A. Taylor, W. F. Faragher, H. Schindler, and J. G. Allen. This team was later joined by J. F. Ellis and R. J. Morley. A third visit with particular reference to catalyst manufacture was made by W. F. Faragher and W. A. Horne, and the notes of these latest investigators have been taken into account in preparing the report. This report was edited by Dr. R. Holroyd.

This paper deals with the various options considered and factors that influenced the final selection of the headframe, shaft layout and hoisting systems required at the Impala No 16 Shaft in order to hoist both the required 3 Mt of ore per annum, and transport men, material and equipment between the surface and the underground workings to a depth of 1640 m. The layout and construction method used for what is at present the world?s tallest concrete headframe is discussed together with the integrated hoist room foundations designed to suit multiple mechanical and electrical hoist supplier?s solutions. The mechanical components of the two tower mounted 6,5m 4 rope Koepe hoists and deflection sheaves and their layout is dealt with in detail. Various features of the bearing type, brake units and brake control system are also discussed. Installation of the mechanical and electrical hoist equipment in the headframe took place during the sinking phase of the project and placed severe restrictions on the use of the heavy lifting facilities. This required a well-managed and systematic approach to delivery of the hoist components and associated equipment, as well as close co-operation between all contractors on site.

"Kahama Mining Corporation Limited, a wholly owned subsidiary of Barrick Gold Corporation, poured the first dore bar and filtered the first concentrate produced at the Bulyanhulu mine on February 28, 2001. This event marked the completion of the initial development of the Bulyanhulu Mine – Barrick’s first mine located outside of the Americas.Bulyanhulu is a 2,500 tpd underground mine located in the Lake Victoria gold belt of northern Tanzania. Gold is recovered as a gravity concentrate and smelted on site to produce dore bars or collected in copper concentrate that is shipped to offshore smelters. The facilities were constructed to meet Canadian standards. Unit operations include single stage crushing, SAG milling, gravity recovery, flotation, thickening and dewatering. Tailings are converted to paste and returned underground as paste backfill or pumped to the surface tailing site. The surface paste tailing disposal system is believed to be first of its kind in the gold industry. Paste disposal has several advantages over slurry, including lower dam construction costs, reduced water losses, and lower reclamation costs. This paper describes the facilities and discusses the challenges faced during development and construction including logistics, lack of infrastructure, recruiting and training, security, and social issues as well as initial operating results.INTRODUCTIONBarrick Gold Corporation completed the Bulyanhulu mine development in 2001. Capital cost of the project was US$280 million. The mine produced 241,575 ounces from start of milling to year end at a total cash cost of $197 per ounce. Numerous challenges were overcome in the mine’s development due to the general lack of infrastructure in Tanzania and the mine’s location some 1200 km inland by road from the port of Dar es Salaam."

This paper is a collection of thoughts and frameworks. It addresses a simple question: æIf we are to believe the large number of people and institutions indicating that humans are destroying the Earth and that the end is nigh, why do we bother trying to be sustainable?Æ The paper lays out evidence for the existence of multiple hazards to global society. The existence of a hazard alone is insufficient to make an assessment of risk. Risk estimates are required to support decision-making. An approach to assessing risk is provided and some limited data are presented. A new ædefinitionÆ or statement of what sustainability (in a broad sense) might have become is then given. A decision framework for guiding decision- making about the risks is developed. This framework includes a definition of leadership with inherent implications for what determines whether a leadership position by title is actually leading. The paper concludes by offering a view of minerals industry sustainability and considerations around maturity in the current economic situation. It is hoped that by placing this collection of frameworks around sustainability, a supporting structure emerges that will facilitate action. It is proposed that action taken in this way will be more effective than reaction to regulation or even social license to operate imperatives because it is driven by a more holistic view of the challenges and the potential roles of individuals û all of whom are potential leaders at some scale of action.

Within these jurisdictions, primary attention is given to requirements that must be met in each country for acquisition of rights to permit development of mineral resources. Coverage includes: First, identitication of the controlling laws or statutes governing minerals acquisition and reference to the administrative authority; second, analysis; of the laws governing mines and quarries and their products; third, analysis of laws applicable to natural gas, petroleum, and related materials; and fourth, mention of laws regulating offshore operations. Where pertinent, sections dealing with historical background or administration have been added in countries where minerals are subject to control of States or Provinces, those areas are considered separately from the central government.

During 1996 and 1997, Oregon experienced flooding on a scale not seen in many years. Some gravel pits located near rivers experienced erosion and, in several cases, breaches between the gravel pit and the river. Initial evaluation of several sites that experienced breaches indicates that impacts to the river system are complex and vary significantly from site to site. The impacts to the adjacent river system may be viewed as both positive and negative. Several floodplain-mining sites in Oregon located in areas of historic river flow have reverted to active riverbed. In many cases, state and federal fisheries biologists and university researchers see the resulting increase in channel complexity and habitat as favorable. The Oregon Department of Geology and Mineral Industries – Mined Land Reclamation Program (DOGAMI-MLR) is performing additional studies of the impacts to the off-channel areas that resulted from the 1996 floods. More data is needed to make sound floodplain decisions regarding mining.

The removal of a cylindrical core from thick-wall sections for creep analysis by the relatively new Weldcore® process represents a very exciting methodology for obtaining more representative creep damage data from large engineering structures. The cylindrical core that is removed, representing about 60% of the wall thickness, leaves a substantial removal site that needs to be repaired. This paper presents data pertaining to taper Friction Hydro Pillar Processing as an alternative repair technique for filling the core removal site. Process parameters were evaluated with special attention being paid to the effect of downward force on process response variables, weld defects, and mechanical properties of 10CrMo910 steel. Process temperature and torque response as well as total process energy input were also considered. This paper also assesses the static and dynamic performance of this repair technique. The influence of a varying downward force and the occurrence and position of discontinuities were quantified. Downward force was shown to have the most notable effect on weld joint dynamic performance and was found to be related to the process temperatures achieved in close proximity to the weld interface.

The need for onsite gas monitoring during, or following, a major incident at an underground coal mine has long been recognised. Recommendations from inquiries following Queensland mine disasters have included ?qualified persons and equipment for rapid analysis of gas samples be available in times of emergency? and that ?this may be accomplished by either mobile laboratories or laboratories established in each mining locality.? Following the Moura No. 2 disaster, the Queensland Government allocated funds to Simtars for a mobile laboratory to respond should such a disaster occur again in the future. This vehicle was commissioned in 1997 and has been used at many mine sites under a wide range of scenarios. Simtars, armed with over ten years of operational experience with this mobile laboratory, has taken an active role in the design and construction of a modernised replacement unit. With a twenty point tube bundle system and ultrafast gas chromatograph the result is, arguably, the best equipped mobile gas analysis laboratory for mine emergency response in the world. This paper outlines the features of the new laboratory, its capabilities and the philosophy behind the design and inclusions.

"Adoption of the International Cyanide Management Code, regulatory requirements and community concerns have increased the need to implement suitable cyanide destruction processes at gold operations to meet ever more stringent discharge limits. As standards set for the allowable concentrations of cyanide, cyanide compounds and other contaminants may vary from country to country, jurisdiction to jurisdiction, and even from site to site, there is no “one size fits all” technological solution. Currently the most common cyanide destruction processes used in industry are sulfur dioxide/oxygen, hydrogen peroxide and Caro’s acid. Regardless of the process used, cyanide destruction plants must be reliable and achieve permit guidelines at all times. The selection of the most appropriate cyanide destruction process for an operation depends on several factors, including permit discharge requirements, tailings chemistry, location, site-specific climatic conditions and size of the operation. Only after considering all these factors together can the most effective approach be selected to achieve the permit guidelines economically and technically feasible. Finally performing the correct test work program is the most important step to ensure that the correct process design criteria can be determined to accurately calculate capital and operating costs.IntroductionThe adoption of the International Cyanide Management Code in 2005, stricter regulatory requirements and community concerns prompted by environmental incidents involving cyanide have increased the need to implement cyanide destruction processes at gold and silver operations.Process solutions associated with the extraction of gold and silver contain several forms of cyanide. The most toxic form of cyanide is free cyanide, which can be present as a dissociated cyanide anion (CN-) or gaseous or aqueous HCN. Weak or moderately stable cyanide complexes such as those of zinc, copper and nickel are classified as weak acid dissociable (CNWAD). Although less toxic than free cyanide, weak acid dissociable cyanide complexes are considered “ecologically sensitive” forms of cyanide and are regulated in most jurisdictions. In practice, reported WAD cyanide values are comprised of both free cyanide and weak acid dissociable cyanide complexes. Cyanide complexes with gold, mercury, cobalt and iron are referred to as strong acid dissociable complexes (CNSAD) and are much more stable even under mildly acidic conditions. Another term commonly used in cyanide chemistry is total cyanide, which represents all cyanide compounds present in solution, with the exception of cobalt complexes. Total cyanide levels in solutions are in most cases higher than WAD cyanide values and the difference between the two usually indicates the presence of iron-cyanide compounds. Typically, in Canada CNTOTAL levels cannot exceed 1 mg/L in solutions."

The results of the study of District VI were published as Serial No. 2726, January, 1926, and those of District VIII as Serial No. 2745, April, 1926, of the Bureau of mines. The counties of Macoupin, Sangamon, and Montgomery, which are three of the principal coal-producing areas in the northern part of District VII, have been chosen for this report because these counties cover an entirely different part of the State than do the two districts already covered. Some leases in, Randolph and Washington counties are included as they are in the southern part of the district, and the data happened to be readily available.

The use of refuge chambers for use in handling underground emergencies is an issue of topical relevance within the US mining industry. Chamber design is of importance to meet some new safety requirements of US federal and some state government mine regulations. A review has been undertaken of a steel mine refuge chamber design to predict what will be internal temperature within the chamber over an extended period with miners present? The design has been examined for the situation when the chamber is sited in still mine air. Spreadsheets have been developed to allow various design assumptions to be tested. The evaluation has been undertaken using thermodynamic heat transfer models with some assumptions specified to produce a model of appropriate simplicity but with adequate degree of accuracy. Under normal still air conditions it can be seen that internal chamber conditions rise quickly to often arduous conditions. There will always be quite a few assumptions necessary in complex heat transfer process evaluations. Some important refuge chamber design parameters are: ? External surface area of the chamber, its wall thickness and construction. ? Metabolic heat generated and number of persons making use of the chamber. ? The temperature of the external mine atmosphere to the chamber over the year and the influence of geothermal gradient on temperature at depth. ? The maximum internal chamber Apparent Temperature considered safe. ? The distance inside the underground mine and adjacent ventilation conditions.

"A kinematically admissible velocity field is applied to study the arching phenomenon in a cohesionless granular backfill placed between two rough vertical inflexible walls. The study is different from the previous works which are almost entirely based on statically admissible stress fields. It is shown that the development of arches is strongly influenced by the previous loading history of the backfill which controls the nature of the constitutive laws that govern the mode of stress deformation of the backfill. This preliminary study employs simple models in order to show the potential use of such an approach in evaluating and understanding mine backfill behaviour in the underground excavation. The pressure exerted on the floor supporting the backfill is also studied in some detail.INTRODUCTIONAlthough the importance of the design of mine backfill has been discussed extensively by a number of authors, i.e. Hassani et al. (1985), such theories have not as yet been incorporated in design criteria for underground mine backfilling operation . One factor that such theories must incorporate is the so-called arching action .The phenomenon of arching has been the subject of study by many researchers in the field of civil engineering and mining engineering. Among these scholars the works of Terzaghi (1943), Krynine (1945), Lusher and Hoeg (1964) and Handy (1985) are noteworthy. All these authors use a statically admissible stress field as a starting point to develop their theories. The approach taken by the authors of the present paper is completely different because they employ a kinematicallyadmissible velocity field. It is their belief that the arching phenomenon is closely linked to the interlocking of grains as manifested by the degree of dilatation of the granular mass when subjected to a shearing process.This paper is written in the following sequence. To define the interlocking quantitatively it is necessary to state the constitutive law governing the stress deformation characateristics of the granular mass. This is done in the next section as the law proposed by Poorooshasb and Pietruszazck (1985, 1986) is stated and simplified to suit the present analysis. Next the mode of formation of an arch in a simple system consisting of a mass of backfill sandwiched between two vertical rough walls of infinite extent is examined. The velocity field obtained from the above example is used to obtain solutions to two more complicated situations. The first situation assumes that the backfill is supported, in addition to the vertical rough walls, by a Winkler foundation at its base. The second situation assumes that the Winkler foundation has been replaced by a horizontal beam of uniform section and simply supported at its extremities. The findings of the study are summarized and their practical implications discussed in the concluding remarks."

Strata behavior and support performance were evaluated in a longwall tailgate test area at the Eagle Nest Mine near Van, WV. The mine operates in the Eagle coalbed which ranges in height from 4 to 6 ft (1.2 to 1.8 m) on the mine property. Rugged topography results in rapid changes in overburden which ranges from 200 to 1200 ft (61 to 366 m). The immediate roof at this mine transitions from sandstone to shale. In some areas the sandstone appears to be massive while in many locations it is highly laminated, fossilized, and interspersed with coal streaks. Horizontal stress levels appear to be sufficient to create instabilities in the roof in some locations, especially where the roof is thinly laminated. Traditionally, hardwood cribs have been used at the mine to provide secondary support for longwall tailgates; Strata Products' Hercules cribs are installed on 8 ft (2.4 m) centers staggered left and right in the tailgate entry. In a 250 ft (75 m) tailgate test area, however, cable bolts were installed in lieu of cribs. The bolts were 12 ft (3.6 m) long tensionable cables anchored with 5 ft (1.5 m) of resin. Four cables were installed across the entry and rows were spaced on 6 ft (1.8 m) centers. Primary roof support was maintained in the vicinity of the test area using 42 in (1.1 m), grade 60, No. 6 (19 mm diameter) resin bolts installed on 4 ft (1.2 m) centers through T-2 channels. The cable test area was located under a stream valley in an area of relatively shallow overburden. Strata instabilities consistently had been associated with the stream valley in both the Eagle and the overlying Powellton coalbeds. In an effort to further expose the cables to the most difficult stress conditions available at the time of the test, the specific site chosen was directly beneath a barrier pillar remaining in the upper Powellton mine (approximately 150 ft (45 m) of interburden separates the two coalbeds).

Researchers at the National Institute for Occupational Safety and Health partnered with the water well industry to develop engi­neering control technologies and training products to address noise exposure at the job site. Noise­induced hearing loss continues to be a problem in the well drilling industry. Research con­ducted by NIOSH found that air rotary drill rigs produce sound levels more than 90 dB(A) over the course of an eight­hour workday. Sounds levels above 90 dB(A), identified during hammer drilling and the hammering of casing, exposes workers to levels that can damage hearing. A typical air rotary drill rig provides no barrier, for noise and dust pro­tection, between the drill’s control panel and the drill stem. The development of a partial cab is the clearest example of NIOSH’s research partnership with the water well industry. NIOSH researchers determined that an engineering noise control to protect the oper­ator at the rig’s control panel would be effective, based on typical work site conditions. Many surface rigs do not have full cabs as a result of the original equipment manufacturer design or the high cost of aftermarket products. Developed as a partial cab, the NIOSH engineer­ing noise control design addressed stakeholders’ needs associated with cost, transport, and worker usage (Figure 1). Field tests found the partial cab effective in reducing sound levels at the rig’s control panel, a typical location for a drill rig operator. Sound levels at the control panel were reduced from 104 to 96 dB(A) (Figure 2). Coupled with continued use of personal protective equipment, such as hear­ing protection devices, the partial cab can help to reduce noise exposure. NIOSH researchers created a CD with informa­tion about noise exposure and the partial cab. Most important to well drilling companies is information on how to construct and attach the partial cab to drill rigs. The CD can be viewed on a standard computer and includes a video of the partial cab in use on a work site. Computer­aided design drawings are included on the CD and provide detail of the cab design as well as information on construction mate­rials and design details. Additionally, the CD provides background infor­mation on noise­induced hearing loss and hearing protection devices to increase knowledge about this occupational health concern. This information can be viewed by all company employees to help promote the partial cab’s importance to workers using the cab. More information on the partial cab noise control technology can be downloaded from www.cdc.gov/ niosh/mining NIOSH researchers also examined the health and safety training needs of workers in the ground water industry. It was quickly realized that most workers learn health and safety on the job and do not attend any sort of formal training. In addition, there is not a lot of time for daily “tailgate talks” or “shop talks” on health and safety issues. As a small business industry, the owner is often operating a drill rig along with his other employees. In general, new employees learn the job in an apprentice style sys­tem. As a result, workers and owners have few opportunities to access uniform health and safety information based upon the job — an outdoor work environment with daily change in work sites. With this information, NIOSH researchers devel­oped two products that deliver health and safety information to fit into the daily work of well drillers. "INTRODUCTION Mine Ventilation has come a long way over the years. The main purpose of mine ventilation is to keep conditions in the mine safe for the worker so that the mine will be productive and profitable. Technological developments to the mine ventilation system itself and the development of mining techniques have helped improve the safety and productivity of the miners and the mine. Most of the developments to mine ventilation and to safety devices came as the result of serious mining disasters. Another effect of those mining disasters, especially in the late 19th century and after, was legislation to set specific safety standards for mines, especially in the underground coal mines. This paper will focus on the connection between mines disasters and the mine safety legislations promulgated because of those disasters. A BRIEF HISTORY OF MINE SAFETY LEGISLATION Mine legislation has a long history in the U.S. dating back well in the 1800’s. However, mine safety legislation is considerably newer. In 1891, the U.S. Congress passed the very first version of mine safety legislations setting up minimum safety standards. By today’s standards, the 1891 legislation was incredibly basic (7). It took a series of horrific mining disasters for Congress to take action a second time. In 1910, Congress established the Bureau of mines. The role of Bureau developed and changed over next 60 years. In 1995, Congress voted to disband it (27). Initially the Bureau was only charged with investigating and coming up with ways to reduce accidents in coal mines. It was not until 1941, when Congress formally allowed Bureau of Mines’ Inspectors into the mines. In 1947, the Congress enacted and adopted a set of federal safety regulations for the inspectors to follow (1). Finally, in 1952 Congress granted the Bureau of Mines the authority to inspect the mines and to issue citations and/or orders of withdrawals. This act was known as the 1952 Coal Act and the penalties were limited to civil penalties. It would take a couple more iterations of mine safety legislation for monetary and criminal penalties to be introduced (10). Up until 1966, mine safety legislation was directed primarily at underground coal mines and surface coal mines. Congress passed the Federal Metal and Nonmetallic Mine Safety Act of 1966, thus providing a set of standards for all mines to follow (1). The 1952 Coal Act was amended again to be the Federal Coal Mine Health and Safety Act of 1969. This amendment was the beginning of modern mine safety legislation (13). In 1973, Congress created what can be considered the first ‘version’ of the modern day MSHA (Mine Safety and Health Administration). The initial version was the Mining Enforcement and Safety Administrations or MESA. Congress created MESA to act as the enforcement agency, relieving the Bureau of Mines of that responsibility so they could focus on their initial purpose, the development of countries natural resources (14). As mining industry grew, mine safety and health became a greater concern and priority. More efforts were put in legislating the industry. The Federal Mine Safety and Health Act of 1977 provided the platform for today’s mine safety and health legislation. Under the Federal Mine Safety and Health Act of 1977, also known as the Mine Act, MSHA came into being (1)."
