The 500-kt/a (550,000-stpy) refinery in Burnside, LA, owned by Almatis Inc. produced specialty alumina used in the abrasives, ceramics, chemical and refractory industries. The 1.2-Mt/a (1.32 million-stpy) refinery in Gramercy, LA, owned by Noranda Alumina LLC produced alumina mostly for use in aluminum smelters but also produced specialty alumina. An upgrade project at the Gramercy refinery to convert 200 kt/a (220,000 stpy) of capacity to produce more profitable specialty alumina was completed in the first half of 2018, and another project to increase capacity was started but a completion schedule was not available. The amount of new capacity was not disclosed, but it would be used to produce nonmetallurgical grade alumina. The 2.3-Mt/a (2.5 million-stpy) refinery in Point Comfort, TX, which was temporarily shut down in March 2016, remained closed throughout the year."

During the past three years, a new rear-dumping dragline bucket technology has been patented, developed and tested at North Dakota coal mines. Field testing has shown that this new concept in dragline bucket technology can improve strip-mining efficiencies by reducing cycletimes and increasing dumping radius. This article details the process, participants and the results for the development of a rear-dumping dragline bucket. Participants in the process In November 1995, the board of directors of Dakota Coal authorized the execution of a field test and option agreement and a license agreement with the partnership of D.F. Schmidt, electrical/maintenance supervisor, and Bill Lengenfelder, president of Modern Industrial. The board also authorized entering into a dragline bucket field-testing agreement with the Falkirk Mining Co.

There was a time when an occasional failure of a tailings embankment was considered an acceptable risk associated with mining activities. This is not the case anymore. A major tailings failure today could result in the indictment of mine officials, huge fines and a possible site closure due to the loss of operating permits. As a result of this changed operational environment, tailings disposal facilities must be operated with these potential consequences in mind. Physical controls for tailings embankments are prohibitively expensive. However, the nuclear industry has developed a system of administrative controls as an alternative to physical controls. Administrative controls may be defined as control of actions, through organizations and management, using written procedures, record keeping, review and audit to assure operation of a facility in a safe manner (10 CFR 50). Administrative controls can reduce the cost of operating tailings impoundments, as well as increase the level of confidence in these facilities.

One of the biggest challenges for the mining industry is, quite literally, how to the keep the equipment rolling. There is a limited amount of companies that manufacture the 4.2-m- (14-ft-) tall, 5,670-kg (12,500-lb-) tires needed for the haul trucks that are approaching the 362 t (400 st) pay-load class, such as Terex?s MT6300-AC and Caterpillar?s 797F. And, while these might be the most impressive tires, they are not the only ones that are in short supply. The shortage of tires has affected just about every kind of machine, and the demand for those tires has been extremely high for more than five years with no end to the shortage in site. American Tire Corp. (www.americantire.us) is one of the companies working to supply tires to the mining industry. It announced that the demand for its 1.6-m (63-in.) tires that have a zero failure history globally is great and that the company signed contracts worth more than $500 million at MinExpo. This paper presents an algorithm for optimizing the classification of surface mine material subject to excavating constraints. High-resolution expected profit models are input and optimized to classification maps subject to site-specific rectangular excavating constraints. This optimization problem defies traditional closed-form analytical solutions; a practical heuristic algorithm has been developed to quickly determine the optimal final destination for the material subject to realistic constraints. The optimization is fast and generates results that achieve up to 98–99% of the total expected profit achieved with free selection. This algorithm provides a fast viable option for practical application in short-term grade control and in managing multiple realizations in long-term resource estimation.

Because of the lack of a universally accepted definition or standard for RCA, we are left to our own conventions in the marketplace as to what is most ?effective? or a ?Best Practice? for our own facilities or organization. This lack of uniformity in the application of RCA produces an equal lack of uniformity in the measureable outcomes of RCA on the bottom-line from site to site and industry to industry. What is the definition of success for an RCA? Is it regulatory compliance? Is it some quantifiable measure of bottom-line performance? Is compliance and bottom-line performance the same thing? Can we measure the ROI we receive by doing compliant RCA? These questions must be adequately addressed if any definition of RCA is to be effective on the bottom-line. Like our various Corporate Vision and Mission Statements, what would be our Vision and Mission for RCA? When pondering the answer to this question, we should consider: ? What Key Performance Indicators will the Success of RCA be Linked to? This provides a direct correlation to the corporate dashboard and therefore garners support for the RCA effort when implemented at the sharp end. Not until we know what KPI?s will effectively measure our maintenance and reliability objectives can we begin to establish which opportunities will afford the greatest returns1. ? What Bottom-Line Metrics will be Tracked to Measure the Effectiveness of Our RCA Effort? Will it be increased production, profitability, regulatory compliance, etc.? Will it be decreased injuries/fatalities, claims, customer complaints, risks, etc.? We should be specific about these metrics and their correlation to the metrics on our corporate dashboard. ? What Will Trigger an RCA Investigation? Normally companies will establish quantifiable thresholds to be surpassed before an RCA will be conducted. Examples may be an OSHA recordable injury, a failure event in which production losses exceed $250,000, an equipment failure in excess of $50,000 or a regulatory violation. This is the norm. However, we should realize this is using our RCA skills totally on a reactive basis. High Reliability Organizations (HRO) strive for proaction and they also use RCA proactively. Why can?t we use RCA to analyze why unacceptable risks from an FMEA are so high? It is perfectly applicable yet rarely implemented in this fashion.

A pond rehabilitation and environmental education project were proposed by Cleveland-Cliffs Michigan Operations (CCMO) and approved by the Michigan Department of Environmental Quality (MDEQ) and the United States Environmental Protection Agency (USEPA) as partial fulfillment of conditions for a stream mitigation permit. The goals of the project were the rehabilitation of a pond on a tributary of a local trout stream, enhancement of public access and appreciation, and the initiation of a site-based environmental educational curriculum. Cleveland-Cliffs undertook a suite of actions to rehabilitate a pond created by a previous mining operation in 1912 and make accessible to local schools the aquatic and riparian ecosystems. Cleveland-Cliffs partnered with the local governmental unit and two local school districts. The rehabilitated pond and riparian zone provide an accessible natural area that local residents can use for such activities as hiking, biking, walking, fishing and nature study. Vegetative enhancement includes the planting of native shrubs, wildflowers and showy herbaceous plants in the pond, on the riparian fringe and the surrounding area. An interpretive trail was developed around the pond with signs created by a local master watercolor artist. Local schools built and installed picnic tables, benches and a kiosk. Cleveland-Cliffs created pond lesson plans tied to the Michigan Curriculum Framework, developed inexpensive teaching kits and hosted a workshop for teachers from nine local schools. Classes regularly utilize the pond during the school year conducting environmental studies. Nesting boxes, wildflower gardens, bat houses, frog deformity surveys and water quality studies are a few of the activities that classes have conducted at the pond.

A five steps remediation program was commissioned in 1998 for the acid generating Lorraine mine site, located in the Témiscamingue region, Quebec, Canada. It included, as the main expenditure, a cover with capillary barrier effects (CCBE) used to limit oxygen migration. After construction of the cover, instruments were installed on site to monitor the CCBE performance. Modified oxygen consumption tests were also done to measure the oxygen flux through the cover. Results from water content and suction data, measured from 1999 to 2003, show that the capillary barriers effects are effective and that the water retention layer stays near saturation throughout the years. Oxygen fluxes estimated from monthly water content measures are generally below 10 g/m2/year. Results from modified oxygen consumption tests made on the site give almost similar oxygen fluxes. A two dimension numerical model of the Lorraine mine site was also constructed to verify that mathematical models could predict realistically the in situ conditions on such a large scale site. Numerical results obtained for three years show a good concordance between measured and calculated values. The results obtained form these different evaluation methods for the performance of a CCBE concur and confirm the efficiency of this cover in limiting acid mine generation.

"Fiber reinforcement has emerged as an alternative to steel bars in precast concrete segments due to advantages such as saving cost and reducing production time while developing a more robust product with improved handling and long-term durability. ACI recently drafted a new report as the first design guideline on FRC tunnel segments to provide specific guidance for this emerging technology. ACI document offers general information on the history of FRC precast segments from tunneling projects throughout the world; a procedure for structural analysis and design based on governing load cases, and a description of the material parameters, tests and analyses required to complete the design. This paper summarizes the design considerations in this ACI guideline prepared by the authors of this paper as the main contributors. Application of this guideline to design of a tunnel with an internal diameter of 5.5 m (18 ft) results in elimination of steel bars.INTRODUCTIONPrecast concrete segments are installed to support the tunnel excavation behind the Tunnel Boring Machine (TBM) in soft ground and weak rock applications. The TBM advances by thrusting off the completed rings of precast concrete segments that provide both the initial and final ground support as part of a one-pass lining system. These segments are typically designed to resist the permanent loads from the ground and groundwater as well as the temporary loads from production, transportation and construction. Tunnel segments are generally reinforced to resist the tensile stresses at both the Serviceability (SLS) and the Ultimate Limit States (ULS). With traditional rebar, a significant amount of labor is needed to assemble the cages and place the rebar, which results in higher production costs.Fiber Reinforced Concrete (FRC) can be used to enhance the production, handling, and placement of precast concrete segments. This is because of the uniform dispersion of fibers throughout the segment to include the concrete cover, which is very advantageous for resisting the bursting and spalling stresses. These stresses develop as a result of the high loads induced on the segments during the TBM jacking process. The presence of fiber in the concrete matrix helps mitigate against unintentional impact loads during segment handling and tunnel construction operations."

Libby, Montana is the site of a large vermiculite deposit that was mined between 1920 and 1990 to extract vermiculite for commercial applications such as insulation, gardening products, and construction materials. The Libby vermiculite deposit also contains amphibole minerals including tremolite, actinolite, richterite, and winchite. Historically, Libby mine workers experienced high exposures to amphibole structures, and, as a group, have experienced the health consequences of those occupational exposures. It has been suggested that Libby residents also have been and continue to be exposed to amphibole structures released during the vermiculite mining operations and therefore are at increased risk for disease. The Agency for Toxic Substance and Disease Registry (ATSDR) conducted two epidemiological-type studies of residents living in Libby and the surrounding areas to assess these risks. The Environmental Protection Agency (EPA) collected and analyzed exposure data in Libby and used those data to project risks of asbestos-associated disease for Libby residents. The EPA has placed the Libby Asbestos Site, which includes the mine and the town of Libby, on its National Priority List of hazardous waste sites in need of clean up. This article presents a review of the exposure studies conducted in Libby and an analysis of health risks based on the data collected in those studies. Libby mine workers have experienced elevated levels of asbestos-associated disease as a consequence of their occupational exposures to amphibole structures. Libby residents? exposures typically are substantially lower than mine workers? historical exposures, and the health risk projections for residents are, accordingly, substantially lower. © 2007 Published by Elsevier Inc. We look at the empirical evidence of coal block auctions in India to understand how competitive intensity affects the bid amounts in common value auctions. The results suggest that competitive intensity measured through the number of bidders for a coal block is instrumental in determining the bidding outcomes. The presence of higher numbers of bidders in the auction leads to higher bids - every additional bidder causes the bid amount to rise by INR 142.67 per tonne of coal for auctions studied for 127 coal blocks from 2014 to 2023. The paper also examines the moderating and mediating effect of competition on bids and on excess of bids from the comparative market price of coal from alternative source of Coal India Limited. The results indicate that competitive intensity does not have statistically significant impact on the relationship between common value of coal blocks and bids, neither as a moderator nor as a mediator. The strategic behavior of the bidders has also been observed as bidders have been seen to bid in a higher than market price in order to outbid the competitors.

Timber support of mine openings continues to be an effective system for ground support, due to its avail¬ability, flexibility, and ease of installation. Timber support of mine openings as a primary method to support loose rock has been replaced to a large extent by rockbolting and by filling systems. How¬ever, the many designs of square-set timber and varia¬tions of crib and stull support that have been developed to suit many stope configurations have proven reliable over many years of application. Integrated in modern excavation and stoping methods, timber remains an im¬portant ground support material for today's mines. ADVANTAGES AND DISADVANTAGES OF TIMBER SYSTEMS Timber, when used as a part of the total support system, is recognized for its unique support properties. More than a cover to prevent minor falls of ground, timber systems have been carefully designed to provide adequate support with sufficient compressive qualities to achieve a safe working environment in extremely diffi¬cult ground conditions. In some cases, a timber support method is designed to yield while retaining its protec¬tive support qualities. Little or no initial investment in equipment or plant is required for incorporating timber into the mining support system. Low cost cutoff saws and portable chain saws are sufficient equipment for effective use of dimen¬sioned (sawed) or round (stull) timber support. Until recently, small sawmills of various complexity were com¬mon at mine surface plants. Sawmills at the mine sites have generally been replaced by timber vendors' mills. Suppliers have been able to use efficient cutting and lumber handling methods to reduce the cost of mine timbers. Most suppliers will package dimensioned mine timber into bundles that are suitable for handling down mine shafts and on level conveyances. The principal disadvantage of timber as a support method is higher material cost and labor cost associated with its labor-intensive installation. Furthermore, stand¬ing timber reserves of mine quality wood are in short supply in some areas, and heavy competition exists for alternative uses of the timber supplies. Vendors are reluctant to supply large cross section dimension tim¬bers relatively free of defects. Some mine operations require 4.8-m (16-ft) lengths of firm timber over 0.09 m2 (144 sq in.) in section and these are difficult to obtain in large quantities. APPLICATION OF TIMBER FOR MINE SUPPORTS Timber used in mine support has been cut from many species of trees. A firm hardwood with physical properties similar to western Douglas fir has been pre¬ferred in the US. Other harder and softer woods, how¬ever, have been used when readily available or where the specific strength or crushing quality of the wood is a desired specification of the support system design. Hard¬ wood, commonly oak, has been used where maximum crushing resistance is required. Mahogany and fir are widely used for shaft guide timber. Normally, timber is used with its core axis parallel to the long axis of the timber dimension. However, in some cases, it is placed normal to its core in a "butt" log position, so as to provide maximum resistance to abrasion of the ore passage adjacent to the cut end of the block. By selective placement of timber block¬ing, controlled resistance to ground closure can be achieved which will prevent early failure of primary sup¬port members of the timber structure. Timber is widely used in modern mining operations as a construction member for chute-lip support. These designs are tailored to each operation to conform to the dimensions of the haulage equipment being used. As a construction material, wood requires none of the spe¬cial equipment necessary for placement of concrete or steel support. Placement of these alternate support ma¬terials requires carefully dimensioned and engineered excavation for their installation. Wood can be easily sawed to fit on location. The successful use of timber in heavy ground de¬pends on the careful and accurate placement of block¬ing and bracing to achieve the controlled yielding re¬sistance needed to support the mine opening. Similarly, frequency, size, and placement of top and bottom block¬ing stulls in flat-bedded deposits are carefully determined to provide adequate support at the mining face. Con¬trolled caving through failure of the stulls may be required as the scope retreats from the earlier mined-out sections. The support program must achieve both of these dimensionless qualities by specification of stull frequency and size. The utility of the square set-system of ground sup¬port and timber support is described in Chap. 2, re¬printed from US Bureau of Mines Information Circular 6691. Four examples of effective use of timber for support of stoping and other mine openings are pre¬sented in other chapters in this section. The Gilman mine's use of Mitchell slice stoping described in Chap. 3 is an early modification of the square-set mining system. This application proved reli¬able for heavy ground and overcame some of the dis¬advantages of conventional square-set timber. Chap. 4 describes the Burgin mine's use of timber with other reinforcement material. This combination of materials achieved high strength and control with yielding com¬ponent members necessary for heavy, squeezing ground. Chap. 5 describes the Bunker Hill mine's use of square¬set timber in a new configuration that provides an effi¬cient and safe support for stoping using small load¬haul-dump (LHD) mobile equipment. INTRODUCTION The Metropolitan Boston Transit Authority (MIA) Red Line has served greater Boston commuter needs from its Harvard Square terminus since its opening in 1912. Since then, the population of Boston's northwest area has increased substantially, and with this growth have come dramatic changes in the type and distribution of urban land uses, including the number of households, jobs, shopping and recreational opportunities; these changes have created a need for improved transportation. Extension of the Red Line to better serve these communities has been a topic of discussion for nearly 40 years. In November of 1977, the Urban Mass Transportation Administration (UMTA) granted the MBTA approval and funding for the Red Line Extension Project. With funding in place, the MBTA proceeded with final engineering and construction of the section from Harvard Square to Alewife. When completed, the Red Line Extension will add 3.2 miles of new track to the MBTA system. Estimated final cost is about $400 million. On October 3, 1978, the MBTA received bids for the construction of Project 091-105, Harvard Square to Porter Square Tunnels and Shafts, which is part of the overall Red Line Extension. The Joint Venture of Morrison-Knudsen, White, Mergentime was the low bidder at $47,478,600. Official award of the contract came on October 25, 1978, with Notice to Proceed given October 31, 1978. Although production operations at mines, representing the extraction and removal of the mined material, often appear to receive the greatest amount of attention, the emphasis given to auxiliary operations is continually increasing. This is due to the changing economic, social, and environmental climate in which modern mines must operate. Auxiliary operations can best be described as those activities that are not directly related to the extraction and removal of the mined material, but must receive proper attention if recovery is to take place in a safe, efficient, and socially responsible manner. In fact, certain mining auxiliary operations, such as strata control and ventilation, are considered to be of such importance that they warrant separate sections in this Handbook. Judging from a mine-personnel standpoint, it is not uncommon for a mine to hire the same number or more individuals responsible for auxiliary operations as the number assigned to production.

The purpose of this paper is to provide general information about the bioaccessibility of metals and specific information about the use of bioaccessibility in risk assessments (human health risk assessments [HHRA] or ecological risk assessments [ERA]) and a list of sites where regulatory agencies have approved the use of bioaccessibility data in risk assessment. The memorandum also cites current U.S. Environmental Protection Agency (USEPA) guidance as well as state specific guidance where applicable. BRIEF HISTORY OF METAL BIOACCESSIBILITY TESTING AND METHODS The bioaccessible fraction of a constituent is that fraction that dissolves or desorbs from its matrix (e.g., soil) in the gastrointestinal tract and is available for absorption (USEPA 2007a; ITRC 2011; ITRC 2017). This is different from the bioavailable fraction, which is the percentage of the amount of an element that an organism is exposed to that is absorbed by that organism during a given time under defined conditions (USEPA 2007a). The bioaccessibility factor is the percentage of a metal that is potentially available for absorption and, therefore, is a conservative estimate of the bioavailable fraction since all that is available for absorption may not ultimately be absorbed into the organism’s system. Currently, there are two main techniques for estimating bioavailability of metals within a medium: in vivo animal studies (time-consuming and often cost-prohibitive), and in vitro tests that simulate human, or animal digestion in a laboratory setting. In vitro and in vivo test methods and protocols have been extensively evaluated by numerous researchers (Ruby et al. 1996; Rodriguez and Basta 1999; Sips et al. 1998; Sips et al. 2001; Molly et al 1993; Oomen et al 2002; Kelley et al. 2002; Oomen et al. 2003; Furman et al. 2006; Drexler and Brattin 2007; Basta et al. 2007; Wragg et al. 2009; Wragg et al. 2011; Chaney et al. 2011; Bradham et al. 2015; Bradham et al. 2018). The most recent scientific literature demonstrates that protocols used to estimate the bioavailability of metals to mammals also predict bioavailability of metals to birds as well (Beyer et al., 2016).

Within the metal/nonmetal mining sector, fall-related incidents account for a large proportion of fatal and non-fatal injuries. However, the events and contributing factors leading up to these incidents have not been fully investigated. To help provide a clearer picture of these factors, an analysis of imminent danger orders issued by the Mine Safety and Health Administration (MSHA) between 2010 and 2017 at both surface and underground metal/nonmetal mine sites revealed that most orders are associated with fall risks. Of these cases, 84% involved the workers not using fall protection, fall protection not being provided, or the improper use of fall protection. Fall risks for workers most frequently occurred when standing on mobile equipment, performing maintenance and repairs on plant equipment, or working near highwalls. In most cases, a single, basic, corrective action (e.g., using fall protection) would have allowed workers to perform the task safely. Overall, these findings suggest that a systematic approach is needed to identify, eliminate, and prevent imminent danger situations. Furthermore, to protect mineworkers from falls from height, frequently performed tasks requiring fall protection should be redesigned to eliminate the reliance on personal fall protection.
