A 24,300 tonnes per year state of the art Silicon smelter is being constructed for Barrack Silicon Pty Ltd at Kemerton, 20 kilometres north of the City of Bunbury in Western Australia. The plant is designed for the reduction of high purity quartzite lump to metallic silicon in two 27MVa Demag submerged arc electric furnaces. The primary reductant, a high purity charcoal, will be manufactured at the plant from locally sourced jarrah wood waste (Eucalyptus Marginata). A glance at the following chart reveals that because of the continued poor economy, the slump in industrial mineral production continued into 1980. Gypsum was down 17% from 1979; cement down 9%, sand and gravel, 17%, and crushed stone, 11%. All these downward trends reflect the high interest rates and high costs that slowed construction starts. Domestic fluorspar was down 21% and imports off 14% due to the steel iudustry slump. However, potash production was up 8% and phosphate, 5%. Increased drilling activity spurred au increase in barite production of 25%. Most other industrial minerals posted losses with only a few insignificant increases. As expected, prices were higher, reflecting increases due to high costs for labor, supplies, and above all, energy. Increasing energy and transportation costs also had their effect on industrial mineral production. Railroad freight rates increased 16-22% including fuel surcharge; truck rates increased 11-14%a; and ocean freight rates increased 20% for finished goods. The 1979 shortage of boa and hopper cars did not continue into 1980-most likely due to low production of many of the hulk industrial mineral commodities. All in all, 1980 was not a good Year for industrial minerals.

This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. Uranium exploration over a wide area of the Southern Frame Embayment, South Australia, has defined a number of Lower Tertiary fluvial palaeochannels incised in older rocks. The buried channels contain similar stratigraphicsequences of interbedded sand, silt, and clay, probably derived from the adjacent uranium-rich Olarv Provinee. Uranium mineralization is pervasive within two major palaeochannels, and four small uranium deposits have been found in the basal sands of these channel sequences, at the margins of extensive tongues of limonitic sand.One of the channels is examined in detail with respect to channel morphology, stratigraphy, and rock alteration and the geology and ore controls of the contained uranium deposits are described.A geochemical zonation reflected by variation in limonite, pyrite, carbon, and humic malleI' is defined across one of the deposits. A genetic model is proposed suggesting formation by a uraniferous geochemical cell which migrated down the stream gradient and concentrated uranium on its lateral margins adjacent to the channel bank.

The challenge; to design production facilities and in particular separation equipment which have the ability to deal effectively with the ever changing composition of well fluids, over the life time of the field in which it is operating, ensuring maximum uptime of the asset coupled with consistently effective separation. Design engineers are, however, continually confronted with a dilemma when selecting materials of construction for Oil/Gas separation equipment, since they are generally obliged to select an acceptable material of construction, based on ?predicted? operating conditions, at the lowest acceptable cost. Consequently many designs are somewhat basic and do not take into account the constantly varying dynamics of the well fluids being released into the separator. As oil fields mature, oil companies endeavour to maximize revenue by maintaining oil and gas throughputs. This often results in an associated increase in water and sand volumes and, in order to maintain efficient separation, an increase in operational temperature and pressure; subjecting the equipment to operational conditions for which it was never designed. In reality, therefore, the limitations and constraints placed upon new construction design often result in Maintenance Engineers having to solve the ensuing problems in difficult ?in-field? environments. Should refurbishment or upgrade be required then the status of the platform and that of the vessel during shutdown will dictate how this is achieved. Hotwork on the vessel is generally not preferred due to the requirement for post weld heat treatment and re-classification of the pressure vessel, not to mention the added risk in terms of safety. Unfortunately, until recently, there have been few alternatives to conventional ?hotwork? repair and protection systems, especially since the performance of cold curing polymer based repair and coating systems, for use in oil and gas process equipment, has, until recently, been limited by their resistance to temperature and pressure in immersed conditions. In line with the continued development of high temperature / high pressure oil and gas fields however, recent breakthroughs in polymer technology can now offer both repair and protection solutions which are able to function effectively at temperatures up to 180°C in immersed conditions combined with resistance to the destructive forces of explosive decompression from pressures in excess of 90 bar. This paper will look at recent work that has been undertaken to determine the suitability of modern polymer based repair and protection systems as an alternative to hotwork; thus enabling newly designed and upgraded internals to be cold bonded into position and further protected with temperature resistant coatings, in order to minimize the downtime of the asset and further to protect the vessel internals from the Erosive Corrosive environment.

A new method of constructing retaining walls, in-situ, prior to excavation has been developed. This "Vertical Earth Reinforcement Technique" VERT, has been demonstrated at a full scale test wall at the National Geotechnical Experimentation Site at Texas A&M University. This 10 meter high wall, was constructed of elements of soil mixing in an array that forced composite behavior between the soil mixed elements and the in-situ soils. Inclinometers and extensometers were used to verify the performance of the wall. The construction and results of the testing are presented.

The cyclone, or centrifugal separator, ,is firmly established in industry as a low cost separator of solids from gases.While it will also separate fluids of different densities, and solids from liquids, its main use is to remove relatively fine dusts from gases whose pr~erties are generally not greatly different from those of air. This latter phase of operation is the one that will be dealt with here.The duty of such a cyclone separating system is to take in a supply of dusty gas and separate it into its components of dust and clean gas as perfectly as possible for the least consumption of pressure head. Any separation at all causes pressure loss and, in general, the greater the separation, the greater the pressure drop needed to effect it. The percentage of dust in suspension which is separated from the gas-dust mixture is commonly called the collection or separation efficiency. Overall or "true" efficiency must take pressure drop into account and may be considered as proportional to the ratio of the percentage of dust collected to the resultant pressure drop. For any cyclone It will vary with different dusts.Many commercial adaptions of the simple cyclone are made solely on the basis of pressure drop improvement, without taking into account the effect on separation efficiency, probably because the latter is so difficult to assess correctly. It appears unfortunate but true that the majority of such adaptions and patent gadgets fitted to cyclones which decrease pressure drop, do have an adverse effect on separation. In other' words, the true efficiency is seldom increased but often diminished.Without a knowledge of what actually goes on inside a cyclone, it is impossible fully to appreciate the real effect of a modification. The backfilling of underground stopes is a common practice throughout the mining industry. The stresses imposed by the backfill on the surrounding rock mass need to be assessed because of their influence on ground stability and on the hydro-geochemical response of the fill material. There is, however, a complex interaction between the relatively soft backfill and the much stiffer adjacent rock mass. In recent years, the authors have presented results from investigations conducted on stress distributions (obtained numerically and analytically) in and around backfilled stopes with (sub)vertical walls. In this paper, the influence of stope inclination and of backfill properties is evaluated. Results indicate that, in general, the vertical and horizontal stresses in inclined stopes are close to, but somewhat smaller than the stresses in vertical stopes. Nonetheless, significant differences can be observed in some cases, especially for the vertical stress along the inclined central line and along the hanging wall. It is further shown that neglecting deformability in limit equilibrium analyses is an acceptable assumption. However, Poisson?s ratio and the dilatancy angle, usually neglected in limit equilibrium analyses, may have significant effect on the stress distribution. Results also confirm that the backfill strength parameters (cohesion and friction angle) are critical factors required to properly evaluate the stress distribution, at least within a certain range of parameter values.

Steel and concrete piles and power-actuated hammers have developed almost entirely within the past 100 years. Prior to that time, piles were mostly timber, driven with a drop hammer or a maul. The Engineering News Formula ? for timber piles driven with a drop hammer at a factor of safety of six ? was promulgated by Wellington in 1888. Steel com­panies supplied fabricated and rolled shapes for piles in the early 1900's. The thin shell and then uncased concrete piles originated about that time. Now leads have become sophisticated to accurately spot a pile at any required batter. Hammers have grown to phenomenal size and vibratories are being accepted at full value.

This paper presents a case history of design, prediction, probe and production pile driving monitoring, loading tests and installation of over 3,300 prestressed concrete piles for a building complex consisting of 12, 5- and 6-story over parking buildings. The project was recently completed in three-phases over a span of 4 years. The subsoil conditions were explored by performing 25, Standard Penetration Test (SPT) borings throughout the site. Various types of driven pile foundations were considered and following an evaluation of high structural loads and site-specific constraints such as variable depth of bearing layer of Florida limestone and quality of clay and limestone layers, a prestressed concrete pile support system was selected as the most economical and effective foundation approach. The building foundations were designed for a pile capacity of 353 kN (40 tons) in compression, 45 kN (5 tons) in lateral capacity for a deflection of 0.64 cm (0.25 in.), and 133 kN (15 tons) in uplift capacity. The axial and lateral allowable pile capacity and lengths were predicted using SPT91 and LPILE computer programs, which are generally used in Florida. The validation of these capacities was derived from 144 probe piles and performance of 12 pile loading tests on selected probe piles. Prior to installation of probe or production piles, a termination criterion was established using 1-dimensional wave equation analyses in addition to the design pile embedment requirement. The predicted pile capacities compared very well with the capacities from the loading tests on probe piles. This information was effectively utilized as part of the overall quality control program to install a total of 4,186 prestressed concrete piles (including 144 probe piles) in different building areas across the project site to depths ranging from 8 to 21 m (25 to 90 ft) below existing ground elevation as part of foundation support system. This engineered, monitored, and tested foundation solution provided an effective and satisfactory basis for completion and approval of the foundation work for this 12, 5- and 6-story over parking building complex. A recent reconnaissance has indicated that all the building foundations are performing satisfactorily.

A systematic study of groundwater conditions at eight underground mine sites exhibiting a wide range of groundwater qualities throughout Australia has been completed at the WA School of Mines. This has resulted in a new corrosivity classification for groundwater driven corrosion processes for cement grouted cable strand used in the Australian underground mining industry. The new corrosivity classification is simple to use and corrosion rates may be predicted from readily obtained in situ measurements of groundwater dissolved oxygen.

A bulk mineral analyser (BMA) suitable for the determination of Cu or Ni in bulk ore samples has been developed at the Lucas Heights Research Laboratories. This analyser, which uses the nuclear resonance scattering technique, is highly specific to the wanted element. The technique has previously been extensively field tested for copper at Mount Isa, Queensland.The present paper discusses the field trial in 1982 of a commercial prototype BMA for nickel. The trial was undertaken at the lateritic nickel mine at Greenvale, Queensland. The BMA and its shielding, together with a laboratory spectrum analyser, were sited in the sample shed adjacent to the workings. Two hundred and thirty-eight ore samples taken directly from the mine blastholes were assayed by the BMA. The average mass of sample was 12 kg, metal contents varied from 0.2 to 3.5 wt/per cent Ni and 4 to 43 wt/per cent Fe and free moisture varied from 11 to 55 wt/per cent. A measurement took 10 min to complete, although this time could have been reduced at the higher nickel grades. The r.m.s. deviation between BMA and chemical assays was 0.09 wt/per cent nickel. Iron and bulk density were also determined by the BMA, r.m.s. deviations being 1.7 wt/per cent and 0.03 g/cmJ respectively. Similar results could be expected from a BMA in routine industrial use at other nickel mines.

Mining coal by employing a method called longwall mining is gaining more and more popularity in the United states since it offers many advantages over the conventional deep mining methods. Those advantages are a higher productivity, an almost total extraction of the mineral since no pillars must be left behind for support, and an increase in safety since the personnel are protected by the shield supports and rarely have to work near unsupported, exposed root. However, on some longwalls, the dust levels liberated by various equipment such as belt conveyors, chain conveyors, crushers, longwall shearers, and moving shield supports can exceed the maximum permissible limits and therefore have an adverse effect on productivity. Many attempts have been made over the past years to reduce the dust liberation by changing designs, installing various types of sprays or scrubbers, treating the mine water with wetting agents, or even using foam to catch dust before it becomes airborne. In the year 1984, the Bureau of Mines awarded Eickhoff Corporation a contract to demonstrate a water-jet-assisted cutting system for the multiple purposes of reducing dust levels, improving bit consumption, and increasing material size. During the previous phases of this contract, various trials were conducted aimed to design, build, and test d shearer loader to meet the objective of the contract. In the early stages of the research program, it was expected to see a reduction in the overall power consumption of the machine due to the fact that the high-pressure water jet would pre-cut the mineral in front of the bit. This, in turn, would then reduce the bit wear. Since there is less cutting action performed by the bit, a reduction of the dust liberation and a reduction of product fines was sought.

Measurement and sampling of diesel particulate matter in mine air presents a challenge due to the complexity of the diesel aerosol. The results of a series of tests carried out by the National Institute for Occupation Safety and Health, Pittsburgh Research Laboratory, in an underground mine were used to evaluate several measurement methods that are currently used to characterize particulate matter emitted by diesel-powered equipment. This paper presents an overview of these techniques and examples of the results along with a discussion of the advantages and information gained by each technique. The measurement methods discussed include particle size measurements using a scanning mobility particle sizer, total particulate matter measurements using a tapered elemental oscillating microbalance, a size selective sampling method for workplace and personal exposure assessment and a size-selective high volume sampling method for carbon analysis.

Control of respirable dust produced by auger type continuous mining machines is normally attempted by water sprays from fixed nozzles mounted on the frame of the machine. In tests conducted under this contract, a 'wet auger' system was used, where the spray nozzles were mounted on the rotating auger itself. The tests were conducted at the Hunter mine of Smith and Stover Coal Company using a Jeffrey 1.00L auger miner. Operating the mining machine in the wet auger mode, it was found to produce 47 pct less respirable dust than when the fixed conventional sprays were used. On section dust levels were reduced too 3.5 mg/m3 which is equivalent to approximately 2.0 mg/m3 for a full shift portal to portal sample. The major difficulty that had to be overcome was the development of a simple reliable water transfer mechanism to get the water into the rotating augers. This was successfully achieved by the development of a water-through-the-shaft water system. This system can be retrofitted to existing machines and is available in kit form.

The modern Golden Cross mine entered the production phase in January 1992. The project is designed to produce over 100 000 oz/yr of gold and over 300 000 oz/yr of silver during a mine life of eight years. An open pit and an underground mine are operated producing around 850 000 t/yr of ore, to feed a conventional carbon-in-leach processing plant. Golden Cross is owned in joint venture by Coeur Gold New Zealand Limited (80%) and Viking Mining Company Limited (20 %).   Since the start of production several major aspects of the design have altered. The underground mine began as a mechanised cut and fill operation. The cut and fill stoping progresses in 3.3 m flatback lifts with maximum stoping width of 6 m. Where the ore zone is wider than 6 m a post pillar cut and fill method is used. In 1993 a new bench mining plan was introduced between the 270 and 260 levels. Flatbacking continued in the footwall drives in anticipation of variability of the ore width and orientation. The development of levels below the main service 200 level has indicated a possibility of bulk mining the ore block rather than flatbacks. This could lower mining costs but also introduce higher dilution rates. Development is in the advanced stages for trialing this method.

In order to predict the composition of inclusions and to ascertain a method to prevent inclusions from forming in liquid Fe-16 mass Cr alloy, a thermodynamic database was developed through a quantitative determination of thermodynamic relationships of Cr-Al-Si-Mn systems in the liquid Fe alloys. Applications of the databases to the prediction of Mn/Si complex deoxidation with Alin Fe-16 mass Cr alloy were demonstrated in succession. The electromagnetic levitation technique using a cold crucible was employed to separate inclusions from liquid metallic alloys completely and to prevent any contamination from surroundings. Various interaction parameters in the liquid Fe-16 mass per cent Cr alloy system were determined; these are: [eAl(Fe)Cr= 0.19/T, rAl(Fe)O,Cr= -0.01, rAl(Fe)O,Cr= -0.00013, eS1Cr(Fe)= -0.38/T,rSi(Fe)O,Cr= 0.007, rO(Fe)Si,Cr= 0.0044, rA1(Fe)Si,Cr= -0.0075, rSi(Fe)A1,Cr= -0.0078, rSi(Fe)Mn,Cr= 0.005, and rMn(Fe)Si,Cr=0.01.] An improved database, utilizing interaction parameters for molten metal determined in this study, and the Fact Sage slag database of the MnO-SiO2-Al2O3-CrOx-MgO system were used for thermodynamic calculations. These were carried out under various thermodynamic conditions, and results were compared with experimental data of Si/Al, Mn/Si, and Mn/Si/Al deoxidation equilibria. With the abovementioned results, together with experimentally obtained data, thermodynamic conditions were determined, in particular Mn and Si contents, under which liquid inclusions form in liquid Fe-16 mass per cent Cr alloy. The critical Al content in liquid Fe-16 per cent Cr alloy, above which solid inclusion of corundum phase ((Al,Cr)2O3) formed, was as low as 10 ppm. This indicates that extreme care must be taken in controlling the Al content in the metal to avoid formation of solid corundum phase ((Al,Cr)2O3) inclusion. The compositional change prediction of inclusions, which originate from slag entrapment and by Ca addition, was demonstrated by use of the alloy database optimized through this study, and the Fact Sage oxide database. For Si-deoxidized Fe-16 per cent Cr alloy, high Si and low basicity of slag are effective to obtain liquid inclusions. For Al-deoxidized Fe-16 per cent Cr alloy, it is important to keep the Si content lower than 0.1 per cent and it is required to add Ca. KIeeman (1968) described practice in the Mount Hebbard dump concentrator, Broken Hill, N.S.W. The purpose of this note is to give data on flotation pulp chemistry in the plant for comparison with results of Woodcock and Jones (1970a, 1970b).Ten chemical parameters were measured at ten points in the circuit using the techniques of Woodcock and Jones (1970a, 1970b). Fig. 1 shows a condensed time plot of some continuous measurements in conditioner overflow and rougher feed. Fig. 2 shows average values of selected parameters at the start and end of each of the three rougher flotation stages.DISCUSSIONpH and redox potentiaIsFig. 1 shows that pH and Eh were relatively constant at the start of sulphide lead flotation. There was a fairly steady increase in pH throughout the circuit (Fig. 2), but flotation was always conducted in a slightly acid pulp in spite of the large amount of alkaline reagents added. Eh values became more positive as the pulp progressed through the circuit (Fig. 2) in accord with results elsewhere.

The Loma Prieta earthquake occurred on October 17, 1989 at 5:04 PM, Pacific Daylight Time. It measured 7.1 in magnitude and was felt from Los Angeles north to the Oregon state line and east to western Nevada. The epicenter of the earthquake was located along the San Andreas Fault, in the Southern Santa Cruz Mountains just north of the City of Watsonville, California. It was the largest earthquake to occur in the San Francisco Bay area since the 1906 earthquake. A total of 87 aftershocks of Magnitude 3.0 or larger were recorded within 21 days after the seismic event. As a result of this earthquake thirteen State-owned and five locally-owned bridges were closed to traffic. Three of the most publicized State-owned structures closed to traffic were Struve Slough Bridge, San Francisco-Oakland Bay Bridge and Cypress Street Viaduct. Struve Slough Bridge was twin slab bridges that collapsed and were rebuilt and open to traffic three months after the earthquake. San Francisco-Oakland Bay Bridge experienced collapse of upper and lower spans at Pier 9 and was reopened to traffic one month after the earthquake. The Cypress Street Viaduct experienced collapse of 48 bents that resulted in the upper roadway falling onto the lower roadway.
