There has been an unprecedented rise in the fatality rate in the Australian mining industry over the last few years. The factors behind this rise are not clear; however, it is noted that the increase in fatalities has occurred during a fall in commodity prices.This paper examines prevention methods and systems to deal with hazards that have the potential to result in either single or multiple fatalities. Managing the risks associated with mine ventilation is used to illustrate the principals behind the approach.The paper also touches on the broad science of human error and proposes a multifaceted approach to controlling and preventing fatal risks.A summary of the steps involved in developing an effective principal hazard management plan is provided.CITATION:Clough, A J, 2015. The main asset of a mining company is the mineral resource; without resources there is no mining business and the infrastructure is useless. In this context the role of the mining geologist is to always keep the resource inventory to an optimum level for the mining business and to maximise the transformation of the resources into reserves in a multi- disciplinary work environment. In short, a ægood mining geologist has to always provide the best ore to the millÆ.To do his/her work properly, several technical skills are needed: to know how to observe and describe nature in order to optimise the data acquisition process, particularly when mapping; to have a good understanding of natural variability in order to develop proper sampling strategies and sample preparation protocols to minimise all the errors generated by the sampling process; to have a good understanding of assaying practices: their limitations as well as their assaying errors; to carefully and properly store the geological data and information and to preserve the geological patrimony; to fully understand ore forming processes to generate the best possible geoscientific models, which are fundamental for the mining business; to fully understand the consequences of the geoscientific models along the value chain from rock to cathodes; and to continuously validate and improve geological practices and models by continuous data collection and observation of geo- mining-metallurgy processes. Some very important behavioural competencies are also needed:long-term vision, passion for excellence and value generation, strategic influence, systemic and innovative thinking, and knowledge and experience transfer. Last but not least, it is important to point out that a deep knowledge of the mining business is essential to add value by using geology best practices.

Sophisticated pit optimisation software has allowed mine planning engineers to generate mining outlines that consider the numerous factors that have a major effect on the success or otherwise of a mine plan. The outlines are generally generated using a range of values for which one can assume a fixed mining cost and a variable revenue. During analysis, the expected costs and revenues are used to investigate the cashflow and sensitivities. Ultimately, the final pit and incremental outlines, or shells, are selected for a given expected price. This paper proposes an alternative method for incremental pit shell selection that considers the risk the mine be prepared to accept. The life of mine is divided into a number of Business Risk Periods (BRPs). The BRP represents the period of time over which the owner is prepared to invest capital. During each BRP, the mine will then operate at a selected investment cost which is generally equal to or greater than the minimum required to meet production targets. In this way, a robust mine plan can be produced which allows the mine to react to change without having to alter the incremental shell limits while maintaining a strategic risk profile. This paper will summarise factors to be considered in selection of the BRP and the incremental pit shells within the BRP.

For the last decade Australia has been the leading exporter of both steam and coking coal. In the ten-year period 1989 - 1998 steam coal exports increased by 93 per cent and metallurgical coal exports increased by 50 per cent. However, profitability of the steam coal sector has been disappointing. The coking coal sector, especially the hard coking coal segment, has fared better but its profitability was more because of its ability to maintain relatively high prices rather than its inherent productivity. Until quite recently the Australian coking coal sector was widely expected to be able to maintain this comfortable state without undue effort for the three major æpartnersÆ; the companies, the State Government and the unions. But coking coal could now be heading down the path followed by steam coal that ends with long-term over-supply and, despite steady improvements in productivity, poor profitability. Steam coal entered the ænewÆ era of long-term over-supply some years ago. Over-supply was due to the rapid increase in production in Indonesia, often from new mines and Australia. AustraliaÆs production rise was underlain by the apparent belief that the best way to lower costs was to increase production. In the first couple of years costs didnÆt fall much but the additional tonnage was sold at lower and lower prices into the spot market. By mid-1998 the difference between the contract price set with Japanese utilities and spot sales was often in excess of $US12 per tonne. Japanese contract prices and tonnages have since fallen significantly. Steam coal has moved from being a product to being a commodity. Any profits are due more to the low Australian dollar rather than inherent mine productivity. Seaborne coking coal trade has been dominated by Australia, Canada and the USA, with the USA becoming less competitive. Unfortunately, when the Queensland capacity expansion was well down the pipeline, the Asian financial crisis occurred and the Japanese economy entered what appears to be a long-term recession. Contract prices fell 18 per cent, tonnages decreased and some mines have been reclassified as supplying lower priced coking coal. The paper analyses past and expected future trends in labour and mine productivity, rail freight and FOB export port cash and total costs of the Australian steam coal and coking coal sectors and discusses the market outlook for Australia. The future economic viability of the industry is analysed and discussed in the light of the rapidly changing global environment.

Since its formation in Papua New Guinea and raising of A$2 million in 1980, Niugini Mining Limited (NML) has grown to a company capitalised on the Australian Stock Exchange at about A$350 million. In 1995 the company produced some 237 000 ounces of gold, 700 000 ounces of silver and nearly 5000 tonnes of copper from its operations at Red Dome, in North Queensland and San Cristobal in The Atacama Desert of Northern Chile. This production contrasts with Niugini Mining's first mining venture at Mt Victor in the Eastern Highlands of PNG which yielded 20 000 ounces of gold and 11 600 of silver between 1987 and 1989. Following the formation of an exploration joint venture in 1982 with Kennecott Explorations (Australia) Ltd, now a subsidiary of RTZ, Niugini Mining's early recognition of the gold potential of volcanic rocks at plate margins led to discovery of the Lihir deposit in July 1982. This deposit containing about 42 million ounces of gold in resources, is one of the largest undeveloped gold deposits in the world. Following raising of equity funds through the float of Lihir Gold Ltd and debt finance in 1995, construction of the mine at a cost of US$673 million is now underway with first full-scale production at approximately 600 000 ounces of gold per year expected in early- 1998.

Current attitudes by exploration companies towards working on another party's exploration permit or equivalent vary widely. That this is so is due to the virtual absence, on the one hand, of anything in the mining legislation in any jurisdiction that establishes what are the exclusive rights to explore, as opposed to apply for a mining tenement, that the grant of such an authority conveys on the holder, and on the other hand, of any code of ethics or set of guidelines to proper practise. By contrast, in most states and territories, if not all, it is an offence to enter onto a mining tenement, other than to cross it, without the consent of the holder, although until recently most field personnel would have been unaware of this and most explorers would have transgressed at one time or another. A similar vacuum exists where aerial surveys are concerned. There are in fact no guidelines. It has always been assumed that a party wishing to carry out such a survey was free to do so, without even an obligation to notify the holders of any tenements or permits in the area to be surveyed, let alone make available to them any data from the survey. Concepts of 'geophysical trespass' now being tested in the US also raise the possibility that data won from adjoining ground using geophysical techniques may be privileged. Attitudes now appear to be changing. In the absence of legislation or any code of practise, what does the industry regard as ethical behaviour? Do we need to define the rights and obligations of various parties more clearly, and if so, what should they be and to what extent will it be necessary to confirm them by legislation as opposed to adopting a code of practise? Will the existence of such a code in fact improve corporate practise?

Investigations of the Mineragraphic Section; Commonwealth Scientific and Industrial Research Organisation, have included most of the known stannite-bearing ores in Australia, and have revealed several new occurrences of this relatively uncommon mineral. They provide information about its paragenesis, and its possible effect on tin recovery, that may be of general interest.Several of the investigations used in the preparation of this paper, were made in the first instance by pr. F. L. Stillwell or in collaboration with him.CASSITERITE-SULPHIDE ORES IN NORTH-WEST- TASMANIA Stannite is a minor component of most of the cassiterite sulphide ores of north-western Tasmania. These include the orebodies of Mount Bischoff, the Cleveland mine, Moina, the Stanley River district, Renison Bell, and the Heemskirk-Zeehan district, and form a well defined metallogenetic provence which is related to the Devonian granites of this part of Tasmania.In these ores mineralisation began. with deposition of cassiterite, a little wolfram, and quartz, topaz and tourmaline. With falling temperature, deposition of cassiterite ceased and arsenopy-rite, pyrite, and especially pyrrhotite, formed in abundance, tending to give massive sulphide ore. A little chalcopyrite; stannite, sphalerite and native bismuth accompanied the pyrrhotite. Deposition of quartz ceased after the pyrrhotite, was formed, and the later-formed nonsulphide gangue consists chiefly of carbonates and fluorite. With, the advent of the carbonates the pyrrhotite was altered to fine-gained intergrowths of marcasite and pyrite. Excess iron from the pyrrhotite entered the intergrown carbonate, or was precipitated as magnetite. Some solution of the cassiterite occurred at this stage, the tin being redeposited chiefly as stimnite replacement rims around the residual cassitellrite grains. The Permian rocks at Gympie were first studied as far back as 1868 in conjunction with the discovery of gold. The relationship of the Gympie Group to other sequences in the Gympie Composite Terrane is an important subject of investigations in a new mapping project of the Geological Survey Program of the Department of Resource Industries.In two areas the Gympie Group has been divided into four defined formations. They are the Highbury Volcanics (base), Ram mutt Formation, South Curra Limestone and Tamaree Formation (top) in the Gympie area, and the Mant basalt (base), Kolbar Formation, Gigoomgan Limestone and Teebar Formation (top) in the Gigoomgan area. Geochemical and lithological evidence from the basal volcanics of the Gympie Group (Highbury Volcanics, Mant basalt) indicate that they developed in an intraoceanic island arc setting. The overlying volcaniclastic sediments (Rammutt Formation, Kolbar Formation), which contain some continentally-derived material, are mainly of local derivation and shallow-water marine. A hiatus is present at the top of the Early Permian Rammutt Formation and Kolbar Formation. Following the hiatus, limestone sedimentation characteristic of a basin of restricted circulation developed in the Late Permian. There is some evidence of a hiatus between the limestone deposition of the Gigoomgan and South Curra Limestones and the overlying probable turbidite deposits of the Teebar and Tamaree Pnrmatinnc

The Red Dome Gold/Copper Mine is located approximately 15 km west of the township of Chillagoe which itself is approximately 220 km by road west of Cairns, Far North Queensland. The deposit, with other mines in the Mungana region, was worked from about 1901 to 1927. Exploration by Amoco Minerals Australia in the 1970s and early 1980s outlined a geological resource of 15 million tonnes. A mining reserve of 9.3 million tonnes was identified and originally developed by Elders Resources Limited. The mine commenced operations in March 1986 and was the first large-scale gold project in Australia to be developed solely on the heap leach process. The mine operated successfully as a gold heap leach operation until 1989 when new grinding, flotation and carbon-in-leach plant facilities were installed to process the non heap-leachable component of the orebody including the copper rich -ore. In March 1991, Red Dome Pty Ltd became a wholly owned subsidiary of Niugini Mining Limited. After six years of successful operation and having excavated a 200 in deep pit, it became evident in 1992 that unless more open cut reserves could be identified the operation was limited to six to nine months of high grade ore and a further 15 months of low grade stockpile material. A preliminary feasibility study was carried out to assess the viability of deepening the open cut. This indicated that from a technical and financial point of view it was possible to expand and deepen the open cut. This presented the daunting challenge of developing a 300 in deep, single access, open cut between a high chert ridge, a government railway line and a National Park. The meeting of this challenge is the subject of this paper.

The Bunga Beds of the Late Devonian - Boyd Volcanic' Complex is a well exposed succession of rhyolite, sedimentary rocks and minor basalt outcropping along the south coast of New South Wales. The sedimentary rocks vary from shallow water basin margin facies, to deep-water basin centre facies, and the majority of .the volcanics are associated with the deep water facies association. The volcanics are dominated by rhyolite domes, lavas and cryptodomes, whose margins are extensively autobrecciated and quench fragmented. In situ pyroclastics represent a very minor volcanic component and are restricted to the remnants of two small doors- top tuff cone successions. Pyroclastic debris is common in the sedimentary succession enclosing the volcanics and was clearly derived from shallow to emergent basin margin areas through mass-flow redeposition. The succession has bearing on the origin of the fragmental component of, the volcanics that host Kuroko- type massive sulphide deposits. The characteristics of these fragmental volcanics are not well documented. We suggest that based on the Bunga Beds,'Kuroko-type volcanic successions are dominated by coherent rhyolite/ felsic lavas and intrusive domes, autobreccias and hyaloclastites, that in situ pyroclastics are rare or lacking, and that redeposited volcanic debris is abundant. Pyroclastic debris is most likely to have been derived from shallow areas marginal to the deep-water volcanic centre that produce the Kuroko-type volcanics and sulphides.

The nickel exploration boom of the late-1960s and early-1970s led to the discovery of numerous komatiite-hosted nickel sulphide deposits, associated with variably serpentinised bodies of olivine-rich cumulates. Subsequent research has shown the importance of volcanological processes to understanding the genesis of these deposits. Crucial developments have come through consideration of the fluid dynamics of magmas and magma-crystal mixtures, through observation of the dynamics and geometry of modern day basaltic lava flows, through application of theory and experimentation on the kinetics of crystal growth from magmas, and most importantly from extensive detailed field mapping of komatiitic sequences in the Yilgarn Block and elsewhere. From these studies has emerged a new integrated view of the volcanological setting of komatiite-hosted deposits, and a recognition that a complete continuum of deposit types exists. Nevertheless, komatiite associated magmatic nickel deposits generally fall into two overall types based on the nature of the sulphide ore. In the first type the bulk of the ore is either massive Fe-Ni-Cu sulphide or a variable mixture of massive sulphide and an ore characterised by olivine crystals in a continuous matrix of sulphide occupying between 30 and 75 per cent by volume of the rock (called Matrix Ore). Nickel grades of massive sulphide ore range from 2 - 20 wt% and those of the matrix ore generally fall in the range 1 - 5 wt% (average 2.5 wt%) Ni/Cu ratios of the sulphide fraction are generally >10.

The Bong Mieu Project lies within the Pre-Palaeozoic Kontum Craton near the southern margin of the regional Phuoc Son û Tam Ky Suture. The Suture and accompanying Truongson Fold Belt of Indosinian age host significant gold and base-metal bearing districts including OlympusÆ other project at Phuoc Son to the west of Bong Mieu and OxianaÆs Sepon project in Laos. The Bong Mieu Project is located some 90 km south of Da Nang and some 30 km south-west of Tam Ky, the provincial capital of Quang Nam province in central Vietnam. The project area is covered by an Investment Licence of some 30 km2 which was granted in 1991 and a mining permit of some 365 ha granted in 1992. Several mesothermal, gold-silver deposits with assigned resources are present on the property. Deposits with assigned resources contain some 615 000 ounces of gold in situ. These are at Ho Gan and Ho Ray which are potentially exploitable by open-pit mining and Nui Kem which is potentially exploitable by underground mining. Newly discovered extensions to the known deposits at Ho Gan, Ho Ray and Nui Kem, and several other new prospects, are currently being investigated by drilling. The Ho Gan mineralisation is of mesothermal, shear and shear breccia quartz vein, as well as intrusive breccia hosted style. Nui Kem mineralisation is of mesothermal, shear and shear breccia quartz vein style. The Ho Ray mineralisation is of skarn (gold-bismuth-tungsten) and lesser quartz vein style. The deposits straddle and flank the east-west trending Bong Mieu Anticline which trends through the central portion of the Investment Licence. In this paper the geology of the deposits, details of resources, results of recent exploration on the Investment Licence and recent engineering and mining studies at Ho Gan are briefly reviewed.

A narrow (200-300m) zone of amphibolite facies metamorphic rocks underlies the Papuan ophiolite near the Musa-Kumusi divide, SW of Popondetta, Oro Province. The amphibolite rocks are bounded up-section, to the NE, by peridotite and gabbro of the ophiolite; and down-section, to the SW, by mafic greenschist, which in turn overlies blueschist facies mafic schists of the Emo Metamorphics.Amphibole compositions within the narrow contact zone show a strong chemical zonation which reflects a steep thermal gradient, with maximum temperatures >800¦C at the peridotite contact. Lake Kopiago is located some 71km northwest of Tari in the Southern Highlands Province and lies on the northern edge of the Papuan Fold Belt. The area is underlain by middle Miocene carbonate and fine grained clastic sediments intruded by late Miocene hornblende gabbros.Two mineral deposits occur to the south of Lake Kopiago. These are the Bisamu gold prospect and the Wunaki zinc-lead gossan.The Bisamu deposit consists of a calcic skarn assemblage which is geochemically anomalous in gold, silver, copper, zinc, cobalt, bismuth and tellurium. The prograde mineral assemblage includes garnet, clino-pyroxene and epidote whereas the retrograde assemblage includes epidote, quartz, actinolite and chlorite. Pyrrhotite is the dominant sulphide with minor pyrite and marcasite and trace chalcopyrite and sphalerite. Minor magnetite is also present. The Bisamu skarn shows many of the features typical of gold-skarn type mineral-isation.

The large, low grade, Kidston Gold Mine in North Queensland has achieved outstanding economies of scale and profitability. The total mineable ore reserve including mined to-date (as at December 1993) was 70.75 million tonnes at 1.54 grams gold per tonne. After nine years of mining, grade control mine production routinely reconciles against mill production and ore reserves to within five per cent of contained gold. The grade control system developed at Kidston is examined in detail, with emphasis on the geological thinking behind the strategies, as well as the development and application of a computerised, graphical production control and reporting system. Reconciliation procedures and results are explained, illustrating the success of geostatistical methods, as a risk management tool, in both ore reserves and production grade control.

The Telfer Gold Mine, owned and operated by Newcrest Mining and located in Western Australia, is currently being expanded. A new underground sublevel cave development is proposed along with an appreciable expansion of the open pit. The Telfer area is generally subject to low rainfall, however, during the wet season cyclonic events can result in sizable rain events, which have the potential to cause substantial inflow. In addition to this, during the first five years (nominal) of the mineÆs life, there is the potential for drainage paths between the pit and the underground mine via decommissioned underground workings which lie directly under the pit. After this initial five year period the pit floor will be mined out below the old workings and will therefore no longer connect to the underground, however cave breakthrough to the surface may start to occur thus providing another water path into the underground mine. To arrive at an optimum, safe solution for dewatering the mine that successfully addressed the above items, a rigorous risk analysis was included as an integral part of the design process. This involved the determination of the probability of events occurring and the consequences of these occurrences. This was compared against the cost of mitigation for the various events. This paper outlines the process by which a safe concept addressing all the issues relating to dewatering the proposed mine has been arrived at. This process included: Establishing the pit catchment areas. These were based on historical run-off data and took into account practical mitigation works. Establishing an Average Recurrence Interval (ARI) based on reference material and historical rainfall data. Utilising a standard approach based on Australian Rainfall and Run-off (ARR) to calculate pumping and storage requirements for the various catchments. Analysing the 20 years of recorded rainfall in the Telfer area to establish the duration, magnitude and sequence of events. This analysis assisted in determining the surface run-off coefficient. Establishing potential water storage areas and determining the risks associated with using particular storage options. Establishing the percolation rate through the cave zone. A degree of uncertainty regarding the percolation rate in the cave zone required that the consequences of using various percolation rates were considered to aid in establishing the rate which was adopted. Establishing preliminary concepts and determining the most economically viable concept. Conducting a risk assessment based on the principles outlined by Australian Rainfall and Run-off (ARR) to confirm that the appropriate Average Recurrence Interval (ARI) had been selected. A risk assessment was also conducted in accordance with client requirements. Establishing the optimum pump station sizes and storage capacities. Establishing the appropriate numbers of duty and standby equipment. By using the foregoing process, which included a number of stages of risk analyses, an optimum dewatering system concept was produced.

The Gordonstone Project with a capital cost in excess of $500 million will become the largest underground coal mine in Australia. The project development decision was taken on 31 July, 1990 and at 31 May, 1991 construction was approximately 35 per cent complete. The first train of processed product coal will be railed to Gladstone during December 1992. The general theme of this paper therefore will address the planning and development processes at Gordonstone particularly the interrelatedness of technical, production, construction and environmental disciplines to ensure success during operations.

Permian oroclines in eastern Australia must have formed by sliding on a detachment surface. It is identified as a westward- dipping Devonian and Carboniferous sub- duction zone. Major strike-slip faults probably sole out on the same surface. One major fault with an associated serpentinite belt was pulled away from the craton abd bent into loops. Basins formed in the loops.

The large, high grade 3000 and 3500 copper orebodies at Mount Isa lie greater than 1200 m below the surface, and partly beneath the silver-lead-zinc mining operations at the Isa Mine.Since discovery over 30 years ago, the primary data acquisition programs have been integrated with long-term copper production strategies. These potentially sizeable and distinctly separate accumulations of copper mineralisation are the only sources capable of supplementing a decline in output from the 1100 orebody in the 1990s. Within a decade these deeper copper orebodies will become the major source of copper ore.

Facies associations in the Okiduse Volcanics, woodlark island, suggest that two major volcanic centres erupted synchronously during the Middle Miocene. These centres are referred to as the Watou Mountain Eruptive Centre and the Uvarakoi Caldera. The Watou Mountain Eruptive Centre, centred on Boganuse Mountain in the Okiduse Range, was a typical, moderate-sized composite volcano (or stratovolcano). The seq-uence is readily divisible into lava (Boganuse lava unit), breccia (Nikuben breccia unit) and epiclastic (Talpas sandstone unit) dominant facies, typical of composite volcanoes. These facies girdle Boganuse Mountain and they are interpreted to represent vent, cone forming and ring plain associat-ions, respectively. These facies indicate a reconstructed volcano of approximately 1,700m height with a basal diameter of 7km. The presence of limited exposures of intrusive associated with the vent facies (Kuike'beimmicrodiorite) suggests that the volcano is only partially dissected. The Uvarakoi Caldera is a felsic volcanic system contrasting with the mafic Watou Mountain centre.
