The objectives of this paper are to introduce the International Code of Practice for Risk Management of Tunnel Works and to illustrate to the participants of the 13th Australian Tunnelling Conference 2008 how the insurance industry would like to see the future of tunnelling insurance. Tunnel construction projects have suffered large losses over the years as can be shown from various loss examples and a list of major losses over the last ten to 15 years. Since the consequences of insufficient risk control had to be borne by insurers, these losses have heightened the need for improved and adequate risk management measures so that insurance cover can also in future be provided for this hazardous industry sector. Observations are made as regards to the prevailing trends in the tunnelling industry, which contribute to the increase in risk and have adverse consequences for the insurance industry. To find a solution to this dilemma various options are examined with the suggestion of a Joint Code of Practice for Tunnel Works being agreed by all stakeholders, which would become essential to secure future insurance protection for such project work. The British Tunnelling Society has, in cooperation with the Association of British Insurers, developed such a code, which the International Tunnelling Insurance Group has now adopted for global application in international markets. The content and concept of this code is explained to establish a framework of understanding and applied risk management principles for tunnel works. This initiative also has the support of the International Tunnelling Association and the International Association of Engineering Insurers. The international application of this new code is demonstrated with a few examples, which also includes Australian projects. It is the aim that this code forms in future part of the insurance coverage for tunnelling works in order to put this risk segment on a more sustainable basis. For that reason the insurance industry is seeking the support of all stakeholders of tunnelling construction projects.

Issues between resource utilization and resource preservation often develop in connection with coal mining. One such conflict exists in eastern Utah where PacifiCorp and Energy West Mining Company are working with State and Federal agencies to address the resource recovery/resource preservation dispute. The effects of underground longwall coal mining on golden eagle (Aquila chrysaetos) cliff nesting sites have been studied at the Cottonwood Mine since 1986. Subsidence, escarpment spalling, and the nesting activities of the local eagle population were monitored.

ON mineral lease 38 of the Sulphide Corporation Limited, Broken Hill, a shaft was sunk in the early days Ior prospecting purposes and at the present time it is accessible as far as the 1000-ft. level where a connexion exists with the main workings of the Corporation's Central Mine, mineral lease 9. About the year 1895· a crosscut was driven east from this shaft at the 1330-ft. level cutting the Central Ore-body in mineral lease 9 and the shaft is supposed to extend a further depth of 25 ft; but no records of any of this work are in existence, the whole of the evidence being based on hearsay and memory:Fig. 1 shows the position of the Extended shaft in relation to the main workings. "1.0 INTRODUCTIONThe uranium industry started in Canada with the reopening of the gravity concentrator at Eldorado's Port Radium mine in the North West Territories in 1942. In 1948, uranium exploration and processing was opened to the private sector, and the Federal government offered special pricing incentives to the industry. Eldorado explored the Lake Athabaska deposits in northern Saskatchewan, and in 1953 opened the Beaverlodge mine. Other mines in the area soon followed. In 1953 also, the deposits in the Blind River - Elliot Lake area and in the Bancroft area of Ontario were discovered. Mines and mills were quickly developed in these areas.Canadian uranium production peaked at 15,900 short tons of U3O8 in 1959. In that year, uranium exports at $300,000,000 exceeded those of all other minerals in value. On October 1, 1959, Canadian uranium operations employed 13,900 people.Up to 1956, it was a seller's market. By 1958, world production of uranium was far ahead of requirements. Accordingly, in November of 1959, the USAEC announced it would not renew it's options for continued delivery of uranium from Canadian mines. This led to the ""stretchout"" of existing uranium contracts, and many mines were shut down (see Table 1) .In the early 1970's, a commercial uranium market seemed to be developing. In Ontario, at Elliot Lake and Bancroft, mines reopened. But competition was coming from the Athabaska basin of Saskatchewan. Currently it is these high grade, open pit deposits of northern Saskatchewan which are in the forefront of Canadian uranium developments. Canadian developments since 1942 reflect the political and market forces, as well as metallurgical progress and changes in environmental standards. "Cameco Corporation is operator of the world's richest uranium project, the Key Lake mine/mill complex, located within the Athabasca region of Northern Saskatchewan. Mineable resources from the Key Lake ore bodies with an average U,08 content of 2.5% will be exhausted by the year 2000 and open pit mining at Key Lake will be phased out at that time. The mill will continue to operate at increased capacity, using high grade ore from the McArthur River project, a new underground mine at a distance of about 80 km from the Key Lake site. The McArthur River ore with an average U308 content of about 15 % will be blended with separately stockpiled low grade materials from Key Lake with a U,08 content of about 0.1 %. The average (blended) mill feed grade will be about 4%, similar to peak concentrations found in Key Lake ore.The Key Lake ore tailings are presently being placed in the mined out Deilmann pit under sub-aerial conditions, using spigots and a highly efficient pit/tailings drainage system. The tailings resulting from the processing of the blended McArthur River ore will be deposited on top of the Key Lake ore tailings under sub-aqueous conditions. This staged approach shows distinct technical and environmental advantages over an alternative, fully sub-aerial in-pit deposition scheme.The first part of this paper presents an overview of site specific geotechnical and hydrogeological features found at Key Lake and details some of the extensive test and modelling work that was required for environmental impact predictions during the development phase of the facility. The second part of this paper deals with tailings modifications, innovative placement technologies and drainage methods that have been (and will be) employed at the Deilmann Tailings Management Facility {TMF) to produce a highly consolidated tailings body under non-buoyancy conditions. The application of this technology at Key Lake is expected to produce a tailings ""plug"" in the Deilmann TMF which will have a permeability similar to or lower than the original in-situ Deilmann ore, resulting in a minimization of contaminant mass loadings to the environment both during operation and after decommissioning."

It is a pleasure and a privilege to be with you for the Annual Conference of The Australasian Institute of Mining and Metallurgy.The inaugural meeting of The Institute was held in Adelaide in April, 1893. Through the kindness of your President, who passed on to me several publications concerning The Institutute I have been able to glance back over the eighty years in which The Institute has served both the profession and Australia.In that record, many names of great distinction caught my eyes. When one reads the list of past presidents of the The Institute since 1893 and the list of those to whom The Institute Medal has been awarded, one reads the headings of a biographical dictionary of the Australian mineral industries. Many famous names are there-men from the great mining companies, engineers, scientists, metallurgists and geologists. Simply to read the names is to recall great enterprise and great achievement. Going over those names one hears the echoes of the inspiring story of the establishment of the Australian steel industry, the electrolytic zinc inq.ustry, all the major developments in the mining of coal, gold, copper, silver and lead, the treatment of difficult ores, the geological exploration of a cqntinent, the discovery of uranium, bauxite, petroleum, nickel, the establishment of the alumina industry and finally the mammoth earthmoving operations of today. The list of names is a chronicle of great men who did a mighty work. They stand like milestones in the progress of a nation.When one reads further and runs down the titles of some of the papers published in the Proceedings of The Institute even a layman like myself can appreciate the dramatic developments in mining and metallurgy that have transformed the whole practice of the mineral industries in our land. Australians have made some notable innovations and discoveries both in mining techniques and in the processing of ores. To assist the anthracite industry obtain technical data on pneumatic packing, the Federal Bureau of Mines imported a pneumatic packing machine from Germany and installed it in a mine of a cooperating company in the Northern anthracite field of Pennsylvania. This report covers the second in a series of experiments in applying pneumatic packing for controlling strata movement (subsidence) above mine workings in the Pennsylvania anthracite region. The first experiment2/ was begun in January 1950, in a section of the mine where about 50 percent of the original coal remained in the form of pillars; since the company did not plan to mine the pillars in this particular area, pneumatic packing of the chamber voids was done for test purposes only. During this initial test the machine was operated on 204 shifts, and 652,400 cubic feet of breaker refuse material (22,834 tons) was packed into the voids of 16 chambers. The work was completed in December 1951.

Recently discovered Carboniferous bentonites are variable in charadeI and thickness, but being altered aeolian, ash-fall tuffs, they are probably extensive. Most are the Ca-montmorillonite type without the natural swelling characteristics of the Wyoming deposits, but their situation, thickness, and response to beneficiation through cation exchange may give them some potential as sources of future economic deposits.INTRODUCTIONLoughnan and See (1959) gave an account of bentonites and fuller's earth deposits of New South Wales. The most likely potential industrial sources were those occurring in the uppermost formations of the Newcastle and Singleton Coal Measures (Permian) in the Hunter Valley and deposited during epochs not marked by strong contemporary vulcanicity.One feature of the record of known occurrences at that time was the absence of bentonites from the thick volcanic sequences of the Carboniferous, but since this paper appeared, many bentonite beds have been discovered by the writer in the Carboniferous facies formerly known as the "Kuttung Series".The purpose of this contribution is to report briefly on the occurrence, genesis, mineralogy, and economic potential of these deposits which augment possible sources of bentonite near major industrial centres on the eastern seaboard.The potential world demand for bentonite would increase markedly with the trend to pelletization in the steel industry, if its use in this field became more general. We utilized electronic detonators (EDs) to investigate the effect of high detonator delay accuracy on overbreak, remaining rock damage, and surface smoothness, in comparison with that of long-period delay detonators (0.25 sec interval) PDs. The experiments were conducted in a deep mine, in a test site region composed of very hard granodiorite with a seismic wave velocity of about 6.0 km/see and a uniaxial compressive strength, uniaxial tensile strength, and Young's modulus of 300 MPa, 12 MPa, and 73 GPa, respectively. The blasting design was for a test tunnel excavation 8 m in cross section, with an advance per round of 2.5 m. Five rounds were performed, each with a large-hole cut and perimeter holes in a 0.4-m spacing charged with 20-mm-diameter water gel explosive to obtain low charge concentration. EDs were used in the holes on the perimeter of the right half, and PDs were used in all other holes.

As ligas de alumínio, em particular, apresentam certas restrições quando submetidas ao processo de união por soldagem. Isto se deve, principalmente, por apresentarem elevada condutividade térmica, elevada contração na solidificação e elevada solubilidade de hidrogênio. Levando em conta estes aspectos, o processo de soldagem FSW apresenta grande potencial para esta aplicação, visto que, é autógena, realizada sem fusão do material e sem utilização de escórias ou gases de proteção. A partir deste conceito, o objetivo do presente trabalho é avaliar a união de chapas finas de alumínio da liga 3105 H26, com espessura de 0,7mm. Para realização dos testes, foram analisados fatores como: penetração da ferramenta nas chapas processadas, ângulo de ataque, tipo de ferramenta e modo de controle de penetração (posição ou força). Os resultados indicaram que o modo de controle por força se mostrou o mais indicado para chapas finas, e que o regime transiente no início de soldagem foi a parte mais crítica para controle da qualidade da junta soldada.

Improperly designed support systems have led to unplanned roof falls in longwall tailgates. To prevent such falls requires appropriate support and adequately designed support systems. The National Institute for Occupational Safety and Health (NIOSH) is currently developing a standing support design methodology based on the ground reaction curve. To design the support system with this methodology requires a quantitative assessment of the ground and support interaction, which is developed from instrumented test sites installed in longwall tailgates. To compliment and expand the field data results and to optimize the support design, numerical modeling is also conducted. The models are calibrated from the test site data. In the present study, data was obtained from a longwall tailgate in the Illinois basin. The test site was located in the Herrin No. 6 coal seam at a depth of 500 ft. At the site, the immediate roof was a weak shale overlain by a competent limestone. Above the limestone, sandstone formed the intermediate roof. The floor consisted of a weak underclay. In the study, the performance and interaction with the surrounding rock mass of two different standing supports, an engineered crib and a conventional 4-point wood crib were monitored. The two cribs have similar load-displacement characteristics at least through 6 in of displacement. Beyond 6 in, the engineered crib becomes unstable and begins to strain soften and shed load. The measurements and developed ground reaction curves indicated that the tailgate at the mine was a low convergence environment until well inby the face. Because the convergence was well below their capacity, both supports provide more than adequate levels of support to the tailgate. Significant convergence and support loading did not occur until 90 to 100 ft inby the face. The results of this study were also compared with results from similar studies conducted in the Pittsburgh seam to further advance the development of the ?ground reaction curve? based support system design.

Much has been written of the changes undergone by coal in storage and the deterioration of coal through exposure to the weather. In order to obtain definite information for the benefit of the Government departments and of all who store coal in large quantities, a series of tests was begun in the fall of 1909 under the supervision of J. A. Holmes, then chief technologist of the United States Geological Survey, and was continued by the Bureau of Mines after its establishment in 1910. The tests were confined to determinations of the loss in heating value of the coals and did not include a study of other deterioration; for example, in coking quality or the yield of by-products in coking. The Bureau of Yards and Docks of the Navy Department cooperated in tests of New River (W. Va.) coal, a variety largely used by the Navy. A preliminary report presenting a brief account of the early results of these tests has been published by the Bureau of Mines as Technical Paper 16.a The detailed report is presented in this bulletin, which gives a full account of the tests and the analytical data covering a period of five years' storage. Data of somewhat similar experiments for shorter periods with gas coal from the Pittsburgh bed, with Pocahontas coal on the Isthmus of Panama, and with Sheridan, Wyo., subbituminous coal, which is used for railroad and other purposes in the West, are included.

In bump-prone coal mines, roof and rib support is subject to dynamic as well as static loading. Although less frequently encountered, the dynamic loading component can result in violent coal expulsions into openings seriously threatening the safety of workers. Dynamic support systems that have been increasingly implemented in many deep hard rock mines for dynamic failure containment have not yet found their adaptation to deep coal mines at desired levels. This paper examines the potential use of some of these dynamic support systems in deep coal mining, particularly in the western United States. Fundamental principles of dynamic loading are reviewed in the context of their application to dynamic failure mechanisms in coal mines. Potential benefits and applicability of the suitable hard rock dynamic support systems are discussed in relation to coal bump mitigation. The resulting insight provides a starting point for development of dynamic support elements for bump prone conditions in deep coal mines.

The Belize economy recorded modest growth during 1980; the growth of the gross domestic product (GDP) in real terms was between 1% and 2%. At current prices, the 1980 GDP reached $108 million2. This output was largely attributed to the performance of a few industries, mostly nonminerals. Such performance improved significantly the balance of payments situation by reducing the deficit from $19 million in 1979 to $4 million in 1980. The rate of inflation was an estimated 18% in 1980 and would have been greater if not for a conservative monetary policy and price control on a number of essential items. Expert earnings increased $43 million, or 50%, from the 1979 level. After increasing more than 25% in 1979, imports increased only 6% in 1980 to a level of $141 million owing to the decline in investment and stabilization of petroleum prices. The balance of trade deficit was $9.8 million in 1980, a marked improvement from the 1979 level of $45.1 million. The mineral output for Belize accounted for only a fraction of 1% of the GDP. Such activity was limited to the production of limestone, marl, and sand and gravel, all of which were primarily used in construction of the Northern Highway between Orangewalk and Belize City. Mineral resources are deficient or nonexistent in Belize for the majority of metals and nonmetals, although the Government and others continue to believe that petroleum must exist in Belize. The proximity to Mexican oilfields and the growing finds in the Peters area of Guatemala support this expectation. "The flowsheet for the Voisey’s Bay mill had to meet the challenge of processing an ore containing high pentlandite and chalcopyrite values in a largely pyrrhotite/troilite matrix. In the 1994-97 period, circuit development focused on making a high-grade copper concentrate and a relatively low-grade nickel concentrate. However, in 1998 a circuit with copper concentrate, high-grade nickel concentrate and a Ni/Cu middlings concentrate was specified. Following a three year project delay, work resumed on the flowsheet in 2001, and the target for the overall grade of the nickel concentrate was raised from 14% to 20% nickel. To achieve this target triethylamine tetramine (TETA) and sulfite pyrrhotite depressants were used in an all lime circuit. It later became evident that, with appropriate selection of collector and adjustment of the circuit by the addition of a scavenger on the scavenger cleaner, it would be possible to approach the grade of concentrate required without the use of pyrrhotite depressants. The recycle of water and the contained thiosalts was also proven to be instrumental in raising the grade.Construction and commissioning of the mill were fast-tracked with start-up of the mill in September 2005 some six months ahead of the original schedule. Ramp-up of both throughput and metallurgy was faster than any other recent nickel installations with tonnage being limited mainly by concentrate dewatering due to the higher than initially planned ore grades.INTRODUCTIONNickel was discovered in the vicinity of Voisey’s Bay by Chislett and Verbiski working for Diamond Field Resources in 1993 and the Ovoid deposit was delineated in the winter of 1994/95. By the end of 1995 the Ovoid had been well explored and at that time was thought to have a reserve of at least 30 million tonnes with an average grade of 2.83% Ni and 2.0% Cu. Drilling to the east of the Ovoid had indicated the presence of significant mineralization. In June of 1995, Inco Limited bought a 25% share in the Voisey’s Bay Nickel Company (VBNC) which was a subsidiary holding company set up by Diamond Fields. Inco also bought 8% of Diamond Fields and negotiated the right to sell all the nickel produced from VBNC for twenty years as part of this agreement. In April 1996, Inco bought the 75% of VBNC shares that it did not own."

I had hoped when the time came for me to address this meeting that gold mining would be in a much happier position than we find it to-day.As it is, and because of the demands of national security, we find this great and valuable primary industry relegated to the background and in danger of temporary extinction.These notes refer mainly to gold mining in Western Australia, whence comes the major portion of Australia's production, and their purpose is to record a brief review of the industry immediately prior to the general disorganization that is following the call-up of men from the mines.During this period of reduced production the loss of comparable statistical data will make it impossible to follow the progress of the industry, and, since the period may be long, it occurred to me that information might be derived from an examination of the trend of recent production that would indicate what may be expected of the industry when full production is resumed in the future.Since the revival of gold mining, which started in 1931, most of the old-established mines have reorganized and, in some cases, completely renewed and enlarged their equipment in order to achieve maximum production, and old mines hitherto unpayable have been re-opened and equipped. At the same time large sums have been spent in scientific prospecting and in search for new mines, and it is safe to say that at the end of 1939, when gold production reached its peak, practically every known mine in Western Australia that could be worked profitably was in production.During this period the number of men employed in the industry rose from 5,961 in 1931 to approximately 15,000 in 1939. From the outbreak of war in 1939 to the end of 1941 the output from the mines was maintained at full pressure, maximum tonnage beingrecorded in 1940, although a reduction in the grade of ore treated caused a slight falling off in gold production.During 1941 there was a. fall in both tonnage and grade. The decline in tonnage was due partly to the closing of the Lancefield and Mount Magnet mines, and partly to the shortage of labour brought about by the response of the goldfields to the call for men for the fighting services, the average number of men in the industry in this year being reduced to 12,929.With the entry of the United States into the war in December last and the pooling of the resources of the allied nations, the need for gold as a purchasing medium passed, and with its passing there arose the demand for the transfer of the man-power engaged in gold mining to other fields of industry directly connected with the war effort.There was a ready response by those connected with the industry who...
