This paper explores the use of a Process Safety Management (PSM) system to manage risk in the mineral processing and metallurgical refining industries. Important elements of a PSM system are described. International regulations and non-regulatory guidelines covering PSM for high hazard industries are reviewed. Case studies of incidents in the mineral processing and metallurgical refining industries are presented to understand the lessons learned from failure to manage risks using a PSM system. Conclusions are drawn to adopt PSM in the minerals and metals processing industry as a tool for loss prevention and risk management.

Due to the high energy consumption of milling, it is desirable to improve the efficiency of this process. One alternative to reduce energy and material consumption while optimising production yield in mill operation is through balanced design and selection of liners. Over-design of liners leads to increased life but at a cost to grinding performance, a relationship which to date has not been studied closely. This paper provides a description of this process of liner life cycle optimization and presents initial results from the first of two life cycles being studied for a 32 foot (ft) SAG mill. The use of commercially proven high resolution laser scanning based liner shape information provided by the MillMapper® software correlated with well controlled site surveys at various points in the liner life was used as the basis for comparison. The survey data was modelled in JKSimMet to provide direct comparisons of mill performance. This potentially provides a methodology for the design of liners that maintain a favourable shape for the majority of liner life, yielding desirable production parameters such as lower specific energy consumption and higher throughput, while at the same time maintaining practical liner life cycles.

The occurrence of airblasts is one of the principal mining hazards in caving operations between the time of cave establishment and breakthrough of the cave to surface. As such, it needs to be managed continuously and risk assessments need to be reviewed on a regular basis for applicability to changing conditions. The likelihood of an airblast occurring is based on a range of geotechnical factors over some of which engineers on-site only have limited control. However, the impact of an airblast to the underground mine, if it does occur, can be managed by following established safety guidelines for the production from the cave.This paper outlines how the Caving Airblast Simulation Tool in Ventsim was used to determine a risk management strategy for a sublevel and a block cave at a mining operation in Australia. The block cave has a footprint of 14 500 m2 with the extraction level located 1250 m below surface while the sublevel cave currently has a footprint of around 18 000 m2 operating between 660 m and 800 m below surface. Both caves are currently in their early operational periods. The risk analysis provided valuable information for the production planning of the caves including confirmation of extraction ratios for the block cave undercut, locations of airblast walls and maximum allowable air gaps for different muck pile heights to ensure safe operation is achieved.

This paper discusses why Argyle Diamonds adopted thickened tailings disposal in 1985 and how the system was developed over the next seven years. A wide ranging review of the thickened tailings disposal system and the tailings deposit it was producing was carried out during 1992 and 1993. The results of that review led to the abandonment of thickened tailings disposal at Argyle because of serious concerns about the stability of the resultant tailings deposit. The exposure to the risk of liquefaction and an assessment of the consequences if this happened to the thickened tailings deposit are shown. The criteria for selecting the new tailings disposal strategy are then described. The final section of the paper discusses the management strategies that have been put in place. These are designed to: ò reduce the risk posed by the thickened tailings deposit to an acceptable level; ò safely store tailings to the end of mine life; ò produce a land form that can be decommissioned to the satisfaction of Government regulatory bodies.

Traditionally, comminution and flotation have been surveyed, analysed and optimised separately. This often results in less than optimal outcomes because of the complex interactions between the two processes. Flotation recovery is a strong function of the valuable mineral particle size distribution with lower recovery of ultra-fines due to poor flotation kinetics, optimum recovery for the intermediate sized particles and lower recovery for the coarser particles due to poorer liberation. Thus, to optimise flotation recovery and grade it is usually best to grind to a fine size. This requirement needs to be balanced by the cost of grinding finer and the revenue that results from an increase in grinding throughput, which often results in a coarsening of the flotation feed.This paper demonstrates a technique which can be used to predict the effect of a change in feed grind size on flotation performance. It can be used to determine the optimum grind size for a particular operation which optimises total circuit profitability. Analysis is relatively easy to perform, as a minimum, requiring only size by assay data of the feed and product streams of the process. Case studies will be presented that demonstrate the usefulness of this technique in feed grind applications. Potential use of the technique for regrind evaluation is discussed. The minerals industry is faced with choices beginning in the exploration phase and extending over the life cycle of the mine. Firms examine the technical feasibility of alternative actions and typically rank them based on economic performance. Increasingly firms also consider alternativesÆ contribution to sustainable development. There are many visions of what a sustainable future should look like and much debate about what should be sustained. Sustainability is a process, a series of incremental actions, and each alternative course of action has the potential to further progress toward one or more sustainability goal. However, because it is not possible to sustain everything everywhere simultaneously, trade-offs and choices inevitably have to be made.Sustainable decision making before investment (ex ante decisions) should incorporate a broad suite of quantitative and qualitative information. One approach is to utilise an integrated sustainability assessment (ISA) framework that supplements traditional technical and financial tools with other tools such as objectives hierarchies (OH) and life cycle assessments (LCA). OH is a problem framing method that captures stakeholder and decision maker interests, objectives and values. LCA addresses material and energy flows, and environmental impacts. The outputs of these tools can be combined with additional qualitative measures and analysed using multi-criteria decision analysis (MCDA). MCDA models facilitate comparison of different alternatives on the basis of a set of criteria and indicators. Some can incorporate information on risk attitudes and preferences ( multi-attitribute value theory models) and others uncertainty about outcomes (multi-attribute utility models). Studies have been published describing and comparing tools for minerals sector decision analysis. Our paper extends this literature, discussing how a combined OH-LCA-MCDA can support ex ante sustainability assessment by incorporating preferences, risk attitudes, and monetised or non-monetised social and economic variables, in a manner that captures their inherent complexity. We also discuss how such a combined model would support trade-off analysis.

Grade Engineering® involves the planning, integration and operation of coarse preconcentration techniques to reject uneconomic material as early as possible in the mining value chain to improve the quality of ore delivered to mineral processing activities. These preconcentration techniques include screening for natural and induced deportment of grade to finer size fractions during blasting and coarse breakage, sensor-based measurements of grade and mechanised sorting at particle or bulk scales and coarse gravity separation. Over the last decade, the Cooperative Research Centre for Optimising Resource Extraction (CRC ORE), with the support of its participants, has developed a framework for the holistic evaluation of economic and production impacts attributable to these preconcentration techniques. The framework combines the characterisation of preconcentration responses, geostatistical modelling, process modelling and simulation of downstream and upstream production impacts and the optimisation of strategic mine plans to quantify changes in net present value due to different preconcentration strategies. This framework has been applied to evaluate preconcentration techniques at numerous metalliferous operations and can also be used to quantify changes in the environmental metrics for energy, water and steel consumption and emission, tailings and waste rock production. This paper collates the results of these Grade Engineering® studies to show the range and magnitude of economic and environmental impacts attributable to preconcentration. The studies have shown that rejection of uneconomic material prior to mineral processing activities can have a pronounced improvement in energy, water and comminution steel productivity, emission intensity and the profitability of metalliferous mining.

During the past three years Lurgi's Circulating Fluid Bed (CFB) technology has been successfully applied to roasting of refractory gold ores and con- centrates. In comparison with the so-called Stationary Fluid Bed roasting systems, a considerable improvement of gold recovery, to around 95 per cent, has been demonstrated and achieved on many occasions in pilot CFB units and in commercial CFB roasting plants as well. The major reason for such improved gold recoveries is the excellent controllability of the roasting process in the CFB reactor system. Here, temperature, gas atmosphere and solids residence time are more precisely controllable at optimal values during roasting operation in order to achieve the highest possible gold recovery in the subsequent leaching process step. Further dominant benefits of CFB roasting systems are their flexibility with regard to feed material, throughput capacity, dry or slurry feed, almost unlimited oxygen enrichment, direct fuel addition and in situ capture of SO2 in the presence of limestone and dolomite. The system will accept 50 per cent turndown ratio, large variations in feed composition and particle size, as well as a variety of different fuel types. The commercial applications of CFB roasting systems for gold con- centrates have been realized so far at Fimiston, Kalgoorlie/Westem Australia and for whole ores at Cortez, Nevada/USA.

Serious exploration for carbonate-hosted lead/zinc deposits in the Pillara district commenced in 1971 when Trend Exploration acquired exploration tenements covering stream sediment anomalies in the Limestone Billy Hills area. Shell Minerals managed the project from 1974 to 1976, and BHP Minerals Limited assumed management during 1976. Carbonate-hosted 'Mississippi-Valley- type' Pb-Zn deposits most commonly occur in Palaeozoic shallow marine carbonate complexes. At Pillara upper Devonian (Givetian/Frasnian) reef complexes are preserved on the flanks of Precambrian basement 'highs'. Within platform and atoll facies limestones sphalerite-galena- marcasite-calcite mineralisation occurs as disseminations, veins and breccia matrix, mostly associated with fractures and fault zones. Early exploration concentrated on detailed surface geochemistry and IP geophysical surveys, with geological mapping and shallow target percussion and diamond drilling. Results were discouraging and did not indicate potential for a shallow base metal resource. During 1976-77 BHP adopted a grid diamond drilling exploration technique and commenced testing an area of approximately 50 sq. km, initially using an 800 m square grid. In 1978 high grade mineralisation was intersected in an area subsequently named Blendevale. This was in the seventh diamond drill hole in the 800 m grid drilling programme, and to date more than 400 holes have been cored in the Pillara area. Detailed drilling at Blendevale has defined a number of mineralised breccia zones within an area of 2.4 km x 0.5 km, at depths of 100 m to 600 m below surface. The breccia bodies are associated with a series of north-south trending normal fault zones, forming a graben structure. The occurrence of fluorine in the copper/gold concentrates produced by Ok Tedi Mining Limited has provided a series of challenges to metallurgists since 1988. Although fluorine is distributed throughout the gangue minerals contained within Ok Tedi ores, itsÆ presence in talc and phlogopite (naturally floating fluorosilicates) has proven to be the predominate sources of fluorine in the final concentrate. Initial treatment for the fluorosilicates consisted of the use of a carboxy-methyl cellulose depressant added to the flotation cleaners. This was superceded in 1998 with the installation of a fluorine reverse flotation process for the removal of naturally floating fluorosilicates from the final concentrate when the fluorine content of the final concentrate exceeds smelter-defined trigger points. The fluorine reverse process is similar to by-product molybdenite concentrate production in that sodium hydrosulfide is used to depress copper/gold flotation whilst allowing the fluorosilicates to float. The process was installed in two parallel modules (to match the existing grinding/flotation modules at Ok Tedi), each consisting of a closed rougher/cleaner circuit configuration. Both rougher and cleaner cells are Jameson Cells from MIM Process Technologies, with the selection based on both processing characteristics and available construction space. This paper discusses both the nature of fluorine in-, and its removal from-, Ok Tedi copper/gold concentrates.

The Water Delivery Alliance (WDA) was established to design, construct and commission the pump station and desalinated water delivery pipeline to link the desalination plant on the Kurnell Peninsula (constructed by others) with the existing distribution network at Erskineville, Sydney, Australia. The WDA consisted of McConnell Dowell Corporation Ltd (Macdow), Bovis Lend Lease Pty Ltd (BLL), Kellogg Brown and Root Pty Ltd (KBR), Worleyparsons Ltd (WOR), Environmental Resources Management Group Inc (ERM) and Sydney Water Corporation (SWC).Sydney’s Desalination Project is one of the initiatives to secure Sydney’s water supply against the effects of climate change, population growth and drought. The desalination plant, 100 per cent powered by renewable energy, can supply up to 250 million litres of water per day (mld), which is up to 15 per cent of Sydney’s water needs. The project involved the construction of an 18 km pipeline with an ultimate capacity of 500 mld to accommodate any future increase in the output of the desalination plant.The Alliance procured three full-face tunnel boring machines (TBM) from Herrenknecht AG to construct the 6.5 km of tunnel that would provide a conduit for the DN1800 welded mild steel cement mortar lined (MSCL) pipeline at a number of locations along the route. Nineteen shafts and 16 tunnels were required to complete this work.Crucially one of the TBM drives involved construction of a tunnel into Botany Bay under protected marine life habitats; enabling the connection of the MSCL water carrier pipeline into the dredged section of pipeline underwater, approximately 800 m off-shore in Botany Bay.This paper details the retrieval of the 17.4 m long TBM from the ocean floor upon completion of the Botany Bay tunnel allowing for subsequent use on the Project. Elements covered in the paper include the 835 m tunnel, design and construction techniques for the tunnel and recovery, and details of the methods used to install the DN1800 water carrier pipeline.

In the year 1926, Edie Creek, in the Morobe District of the Mandated Territory of New Guinea, saw the greatest gold rush of recent years. Situated 35 miles inland from the coast, and at an altitude of over 6000 feet, Edie Creek presented to the original prospectors a wet, jungle-clad uninhabited, and inhospitable region. There were no roads; and the only means of approach from the equally inhospitable coast was by an eight to ten days' tramp through coastal swamps and gorge-scarred mountains. All food supplies and equipment had to be carried on the backs of "kanakas," and in many cases through country occupied by hostile natives.Since the first discovery of payable gold at Edie Creek, mining on the Morobe Goldfield has shown remarkable progress, and in spite of the tremendous natural difficulties, the field today ranks as one of the most important in Australasia.Aeroplanes, more than any other factor, have been responsible for this rapid development. They provide the only means of transport from the coast, and at present serve the needs of over 600 Europeans and 3000 indentured native labourers engaged in the industry.

The Rosedale Outfall Project utilised an earth pressure balance (EPB) tunnel boring machine (TBM) for only the second time in Auckland. Although the tunnel was excavated fully through East Coast Bays Formation (ECBF), the downhill gradient along with some unexpected areas of extreme water posed some significant challenges during construction.The Rosedale Waste Water Treatment Plant Outfall Tunnel project was commissioned in response to the growing needs of North Shore City, New Zealand. The outfall tunnel comprises a 3 km long segmentally lined tunnel and a 2.1 km buried marine pipeline, and discharges 2.7 km offshore. The capacity of the new outfall is 6 m3/s compared to the existing outfall capacity of 1 m3/s.Following previous project experience, and with the tunnel located along its full length in the bedrock weak sandstone/siltstone bedrock of the East Coast Bays Formation, tunnelling conditions were expected to be benign. However during construction six high inflow events were encountered, including a full face (3.4 m diameter) of Albany Conglomerate over a 48 m length of tunnel. This material comprised cobbles in an uncemented sand matrix, and inflows were recorded at over 25 L/s.This paper describes the design challenges on the project including the design of a steel fibre reinforced segmental lining for internal pressures. The specification of the machine is discussed including the modifications required to enhance the risk mitigation measures already in place. The ground conditions encountered are discussed and have improved the knowledge of the range of conditions that can be expected in Auckland.Despite the challenges presented during construction, excellent progress rates were achieved and the tunnelling was successfully completed ahead of program.

The Mary Kathleen tailings storage facility (TSF) contains 7.1 Mt of uranium mill tailings. This waste repository was rehabilitated in the mid-1980s using a multibarrier dry cover. At the time of mine closure, it was predicted that acid mine drainage (AMD) generation would not eventuate and that uranium, heavy metals and radionuclides would be adsorbed within the tailings and not seep from the TSF into the environment. This study reports on the environmental status of the TSF. Gamma-ray measurements indicate an intact dry cover of the waste repository. However, seepage (~0.5 L s-1) of acid (pH 5.71), saline (0.31 per cent), high conductivity (8.8 mS cm-1), slightly oxygenated (DO 2.6 mg L-1), radioactive waters occurs from the toe of the tailings dam into the former evaporation ponds and local drainage system. Seepage waters are calcium-magnesium-sulfate rich with elevated Fe, Mn, Ni, U and Zn values. Oxygenation of tailings seepages causes the precipitation of Fe and coprecipitation and adsorption of other metals (Pb, U, Y), metalloids (As), rare earth elements (Ce, La) and radionuclides (U-235, U-238, Th-227). In contrast, Ba, Ni, Sr, Mn, P, S, Ca, Mg, Na and K as well as Ra nuclides (Ra-226 and Ra-228) remain in solution until pH neutralisation and evaporation lead to their precipitation in efflorescences and sulfate-rich evaporative sediments further downstream. Dry capping of Mary Kathleen uranium mill tailings has resulted in physical containment of the wastes. The dry cover shows no signs of erosion and radiation levels are within acceptable limits. However, the flow rate of surface seepage from the TSF occurs at a much higher rate than predicted. Also, the neutralising potential and adsorption capacity of the tailings and underlying aquifer were overestimated at the time of mine closure. AMD has developed mobilising metals, metalloids, rare earth elements, alkalis, radionuclides and sulfate from the waste repository into surface water seepages.

In 1975 the Commonwealth Government established import parity pricing for all liquid hydrocarbons discovered after the 16th September, 1975. These discoveries were termed "new oil" to differentiate them from existing discoveries and production. Overnight the price of the product being sought trebled, resulting in greatly enhanced economics by increasing the potential returns on existing prospects and by making smaller plays economically viable. After 18 years of drilling, the offshore Gippsland Basin has attained a moderate level of exploration for a basin of its size. Fifty seven prospects have been drilled within the basin but only a few of these were drilled along the southern and northern margins, while the deep water areas along the eastern margin have remained virtually undrilled following the Hapuku-1 oil discovery. The major hydrocarbon accumulations within the basin are trapped beneath the Latrobe Unconformity surface and exploration has largely concentrated on this single play in areas immediately adjacent to the major producing fields.

The eastern third of Australia contains an unrivalled Phanerozoic rock record generated by the regional interaction of Gondwanaland with the Pacific and by the global effect of the 400 m.y. Pangaea - Panthalassa cycle. Between the latest Proterozoic (600 Ma) breakup of Proto-Pangaea and the mid-Carboniferous coalescence of the dispersed continents in Pangaea, the Lachlan and Thomson Foldbelts accreted to the craton by Mariana-type (backarc basin) and Chilean-type (continental margin) subduction from eastward jumps of the Palaeo- Pacific rim. Further jumps of the rim of the Permian and Mesozoic Pacific, now expanded to Panthalassa, accreted the New England Foldbelt to the craton. An Early Permian rift basin was succeeded by a Late Permian to mid-Triassic foreland basin which extended between the craton and the foldbelt behind the 10000 km Panthalassan rim, from the Bowen Basin of Queensland to South America, and was host to the productive coal measures of eastern Australia, as the succeeding Jurassic and Early Cretaceous foreland and epicratonic basins included the coal measures, hydrocarbon reservoirs, and aquifers of the Great Artesian Basin. The magmatic arc on the Australian margin ended after the mid-Cretaceous uplift along the Queensland coast that initiated southwestward drainage from the high Pacific rim. The Chilean-type subduction that had prevailed since the Devonian was superseded shortly afterwards by Mariana-type subduction with the successive generation of the back-arc basins of the Tasman Sea, Coral Sea, and other southwest Pacific basins by the migration of the island arc to its present position 2500 km eastward of Australia.

High temperature roasting studies on chrome-spinel contaminated ilmenite concentrates showed that sulfur, introduced through a sulfidising roast treatment, selectively reacted with the chromium in the spinels. The roast treatments were carried out in a laboratory scale rotating drum kiln using a standard Becher-style heating profile (10 h, 1075ºC), char as the reductant, and variable amounts of added elemental sulfur (0 - 3 wt per cent).After roasting, bronze-coloured grains were visible representing chrome-spinels that had undergone selective sulfidisation. SEM imaging of the products showed that sulfur was preferentially concentrated in ~5 - 15 µm thick rims fully enclosing the chrome-spinel grains. The sulfur-rich rims became a sink for manganese (Mn) from the ilmenite resulting in the rims being rich in Cr-Mn-Fe-sulfides. Quantitative electron probe microanalysis of phases present within the rims identified two sulfide phases. The first sulfur-phase (S-phase) was low in Mn (1 - 3 wt per cent) and high in Fe, Cr and S, while the second S-phase was considerably higher in Mn (30 - 35 wt per cent Mn) and lower in Fe and Cr. The dramatic change in the composition of the chrome spinels after sulfidisation is likely to significantly alter their physical properties (eg hydrophobicity, conductivity, etc). This should make their physical separation from the ilmenite potentially easier than roasting/magnetic separation methods.

There are numerous individual gold deposits in the Archaean greenstone belts of Western Australia, but only 26 have productions and/or quoted reserves in excess of 10 tonnes Au. Most gold production has come from mineralization in shear zones, quartz veins or stockworks in volcanic or intrusive host rocks, particularly tholeiitic basalts or dolerites metamorphosed at sub-amphibolite facies (e.g. Kalgoorlie). Significant prod- uction has also come from BIF-hosted deposits (e.g. Hill 50), and large reserves are quoted for mafic hosts at high metamorphic grade (e.g. Big Bell). Most deposits have marked structural control, great depth extension, and are characterized by zoned alteration haloes of pyritization (with Au), K-metasomatism and carbonation. On the regional scale, most larger volcanic- or intrusive-hosted deposits occur in younger (ca 2.8 b.y.) rift-phase green- stones (Norseman-Wiluna Belt). These belts also have a high prospectivity (36 kg Au/km2 of greenstone belt). Younger (ca 3.0 b.y.) and older (ca 3.5 b.y.) platform-phase green stones of the Murchison and Sodthern Cross Provinces and the east Pilbara, respectively, are progressively less prospective (14 kg Au/km2 and 0.8 kg Au/km2, respectively) and also have progressively smaller deposits. The important regional- and local-scale characteristics of most gold deposits can be explained in terms of a metamorphic- replacement model. In this model, metamorphic H2O-CO2 fluids, focussed by faults and shear zones, infiltrated host rocks and deposit gold at 350 + 50¦C and 1-2 kb, largely in response to fluid-wallrock interaction. In many large deposits, gold precipitation was synchronous with sulphidation of Fe-rich host rocks, but the diversity and abundance of smaller deposits reflects the variety of reactions precipitating gold from ubiquitous, auriferous metamorphic fluids. Regional variations in abundance and size of deposits relate to factors such as tectonic setting, alteration styles and possibly source rocks.
