Construction of the Shot Tower Metro Station in Baltimore, Maryland was performed in two phases. In the first phase, a contractor was retained to excavate the station in order to construct twin tunnels from the Shot Tower Station to the John Hopkins Station. The retention system for this 70' deep excavation consisted of a Soldier Pile in Tremie Concrete (SPTC) wall supported by cross-lot bracing. In phase two, The Clark Construction Group, Inc. was retained to construct the station interior structure, two entrances, two station ancillary shaft structures and the track work for the station and tunnel. The excavation for the construction of the two entrances adjacent to the 70' deep station SPTC wall had to proceed prior to completion of the station structure due to the tight construction schedule. A system of overlapping secant piles was specified as a vertical support of excavation element for the West Entrance. The secant piles would have provided a watertight, rigid support to resist the load from the SPTC wall constructed in phase one. A watertight system was required in order to minimize the settlement of adjacent buildings supported on wood piles. Clark Construction was concerned with the constructability, cost and performance of the specified secant piles and looked for other innovative methods of providing a watertight and rigid system. Substitution of a reinforced slurry wall in lieu of the secant piles was cost prohibitive due to the additional cost of concrete placed under tremie. Clark, being familiar with cement-bentonite cut-off walls and slurry wall construction procedures, proposed the construction of a PSHG wall, in lieu of the secant piles. The PSHG wall construction consisted of trenching 30" wide by 20' long by 60' deep panels under a self-hardening slurry. Once the excavation for a panel was completed, a 60' deep prefabricated soldier beam and lagging panel was placed in the slurry. The slurry design mix then self-hardened within the cure time, allowing excavation for the structure to proceed This paper will discuss the design, details and construction methods used on the PSHG wall and will compare the advantages of the PSHG system to conventional support systems.

It is interesting to study the noise standards that have been promulgated in the US over the last decade or so. These regulations are likely to have long-lasting impact. The Occupational Safety and Health Administration (OSHA) Hearing Conservation Amendment (March 1983) continues to have influence not only in the workplace but in the debate over new regulations. Both the Mine Safety and Health Administration (MSHA) and Federal Railroad Administration (FRA) regulatory preamble documents state the desire to be consistent with OSHA. An examination of Table 1, which compares the major components of the three regulations and the 1998 NIOSH “best practices” criteria, depicts the extent to which that intent is met. There has been some regulatory activity in the last decade which may give some hope for evolution and updating based on the wealth of science that has occurred during the last quarter of a century since the OSHA regulation was enacted. However there has also been some “back-sliding” toward more lenient standards. The MSHA noise standard made regulatory progress in September 2000 by emphasizing engineering and administrative controls, followed by personal protective equipment, in the hierarchy of noise intervention. MSHA’s requirement for technician certification (today only available from CAOHC) strengthened the training requirements for audiometric testing in hearing conservation programs and MSHA also added the requirement of dual hearing protection at 105 dB TWA. There were many subtle differences between OSHA and MSHA based on comments and a desire to clarify some of the vague aspects of the OSHA noise standard, and meet the needs of the regulated mining industry. One example pertains to the ceiling for exposures. OSHA says, “no exposures >115 dBA,” which is interpreted to mean no unprotected exposures above that level, giving credit for the assumed effectiveness of hearing conservation programs, hearing protection devices and administrative and engineering controls. MSHA specified that a “P” code1 violation be issued for any protected or unprotected exposures >115 dBA. The Federal Railroad Administration (FRA) noise standard for railroad operating employees, which went The Modern Evolution of Hearing Conservation Regulationsinto effect February 26, 2007, was expressly based on the OSHA standard but also uses MSHA for comparison. The preamble states that the FRA defers to OSHA as the “primary regulator of noise in the workplace,” but also acknowledges the need for some departure from the OSHA regulation (FRA Preamble II.B). As an example, FRA requires testing at 8000 Hz “because it will allow employers to identify hearing loss sooner.” The FRA rejected MSHA’s hierarchy of noise controls in favor of requiring specific engineering interventions and focusing on appropriate hearing protection which would still allow communication and audibility, and identification of excessive noise through employee-filed “excessive noise reports.”2 Where OSHA has no specific mandate requiring employees to take advantage of the employer-paid audiogram, it has been traditionally a condition of employment and is generally accepted that OSHA-covered workers require an annual audiogram. MSHA addressed this issue in its preamble; however, they made no significant change. MSHA employers are required to offer annual audiograms but MSHA stopped short of requiring employees to comply with annual audiometric testing. Many mining companies apply ergonomic principles, but it is done either informally and/or reactively. Examples of an informal approach include replacing worn equipment, such as seats, with models that have ergonomic features, or modifying tasks to eliminate safety hazards that will also reduce risk factor exposures that may result in musculoskeletal disorders (MSDs). A reactive approach is followed when modifying a task/equipment after an MSD occurs. Neither one of these approaches involves a systematic method of proactively applying ergonomic principles to actually prevent injuries or illnesses. This paper will discuss how Vulcan Materials Company applied ergonomic principles and the interventions implemented to reduce risk factor exposures.

The PT. Adaro Coal Mine targeted in this research is located in South Kalimantan, Indonesia. The coal production of this mine is increasing year, with about 35 M tons of coal produced in 2006. This mine faces many ground control problems, especially instability of the footwall. Large-scale landslides occur often in the footwall; this may be because of weak rock and the effect of footwall rock weathering. Hence, prompt measures must be developed and followed in order to preserve the stability of the footwall and make this mine safe. Additionally, the final pit design is now being reconsidered because of the increasing demand for and high price of coal. With these issues in mind, this paper describes a solution for improving the stability of the footwall and discusses the final pit design based on the results of an on-site investigation and numerical analysis.

Problem: Mining in the United States remains one of the most hazardous industries, despite significant reductions in fatal injury rates over the last century. Coal mine fatality rates, for example, have dropped almost a thousand fold since their peak in 1908. While incidence rates are very important indicators, lost worktime measures offer an alternative metric for evaluating job safety and health performance. The first objective of this study examined the distributions and summary statistics of all injuries reported to the Mine Safety and Health Administration from 1983 through 2004. Over the period studied (1983 2004), there were 31,515,368 lost workdays associated with mining injuries, for an equivalent of 5,700 person years lost annually. The second objective addressed the problem of comparing safety program performance in mines for situations where denominator data were lacking. By examining the consequences of injuries, comparisons can be made between disparate operations without the need for denominators. Total risk in the form of lost workday sums can help to distinguish between lower and higher risk operations or time periods. Method: Our method was to use a beta distribution to model the losses and to compare underground coal mining to underground metal/nonmetal mining from 2000 to 2004. Results: Our results showed the probability of an injury having 10 or more lost workdays was 0.52 for coal mine cases versus 0.35 for metal/nonmetal mine cases. In addition, a comparison of injuries involving continuous mining machines over 2001 2002 versus 2003 2004 showed that the ratio of average losses in the later period to those in the earlier period was approximately 1.08, suggesting increasing risks for such operations. Discussion: This denominator free safety measure will help the mining industry more effectively identify higher risk operations and more realistically evaluate their safety improvement programs. Impact on Industry: Attention to a variety of metrics concerning the performance of a job safety and health program will enhance industry's ability to manage these programs and reduce risk.

"INTRODUCTIONThe information compiled in this volume initially formed the response by the Generic Mineral Technology Center for -Respirable Dust (GTCRD) to a review of its research and technology transfer programs performed in 1990 by the Advisory Committee on Mining and Mineral Resources Research. This review for each mineral technology research center is accomplished, as required of the Bureau of Mines by the House/Senate Joint Conference Committee, through site visitation to the research institutions of the GTCRD and an in-depth questionnaire. The information provided by that questionnaire becomes an important factor in their recommendations concerning future directions of the program.The site visitations took place during January and February 1991 at the Milton S. Hershey Medical Center~ The Pennsylvania State University, West Virginia University, University of Minnesota, and Michigan Technological University, in addition to an in-mine visit conducted at the Loveridge Mine in West Virginia. The questionnaire was prepared and made available to the Advisory Committee and the Research Council for their review at the GTCRD annual project review meeting in Hershey, Pennsylvania, in April 1991.The comprehensive nature of the questionnaire and the scope of the information contained therein make it an excellent retrospective of the Center. Having a clear vision of where the Center has been and knowing where it is now helps to guide it into the future. It is believed that such a detailed view of the Center's facilities and research program will be of value to Center personnel and Advisory Committee members. Each aspect of the questionnaire was examined, answered in detail and documented thoroughly. With the completed questionnaire as a guide, this review of the Center's activities provides an update to Respirable Dust Center Research Program Review 1988, published in 1989 as the seventh of the Generic Mineral Technology Center for Respirable Dust volumes. As does that volume, this review presents the complete General Mineral Technology Center questionnaire, with the GTCRD's expanded responses and supporting documentation appended. An examination of the research program, Center personnel currently active on GMTC projects, the number of graduate degrees granted with GMTC supports GMTC publications, the degree of individual interaction, technology transfer activities, the reference center, GMTC equipment and facilities, and envisioned plans for future research directions provides updated information in these areas. In addition, this year, information has been included to demonstrate the interaction between the Center's goals and its research projects, with thorough consideration of actual research needs as specified by the National Academy of Sciences. This volume serves as a review of the years 1989 through 1990, the sixth and seventh years of the Center's operation."

The geology of the area studied is briefly described, together with the prospecting methods used in the search for copper, lead, zinc and uranium ore bodies.Four prospects, Mt. Corella, Yanasinga, Lawlor's and Greenmount, which were diamond-drilled, are described in detail and geochemical results are discussed in relation to the mineralization found. Copper anomalies were found wherever intrusive rocks were mapped, and showed a much closer relation to geology and mineralization than did the predominantly lead and zinc anomalies.Geochemical prospecting aids in the overall mineral search programme because of its speed and economy, as it helps to pinpoint areas worthy of detailed geological examination. Geochemical anomalies are a guide to favourable zones but may not indicate the presence of ore.INTRODUCTIONDuring 1953-55 the National Lead Company of New York carried out a search for minerals in an area south and west of Cloncurry (Fig. 1) in rocks of Precambrian age. After reconnaissance mapping, a number of prospects were chosen for closer examination by detailed geological mapping and geochemical sampling, and from these prospects, five sites were selected for drilling.This paper records the salient features of the exploration programme and discusses the geology and geochemistry of four of the prospects-Mt. Corella, Yanasinga, Lawlor's and Greenmount. Practically all the prospects examined showed high zinc "anomalies" or "dispersion" trains. Greta, Mt. Corella and Lawlor's prospects showed some copper anomalies and, to a lesser extent, lead anomalies. In particular, Greenmount exhibited striking copper anomalies.

As it is my opinion that deep alluvial mining in Victoria and in the Murray Flats in New South Wales is practically in its infancy, and considering that we, the old school of alluvial miners, who have thirty or forty years' experience to guide us, must in the natural order of things hand over the management of our mine, to the younger and less experienced' generation of miners, I thought that as our Association has been formed for the purpose of disseminating information and the exchange of ideas, a few notes on the practical working of alluvial mines might be of some service to aspirants for the position of mining manager.The greatest difficulty the alluvial miner has to contend against is the immense quantity of water contained in the drifts overlying the wash in the gutters or ancient river beds, and to cope with this water successfully and economically is the point to be aimed at by our mining managers; and if they thoroughly understand this part of their business in all its details, they will not only win kudos for themselves, but will be the means of saving large sums of money to the companies they represent, and, in many instances, will make a success of a venture which otherwise might be stopped for want of funds before it is properly proved. At the present time, although we have had over forty years' experience indraining the deep leads of the colony, I regret to say that a large amount of ignorance still prevails amongst both directors and managers.

"High tonnage, low grade gold mines such as Omai’s afford economies of scale that are unto their own insufficient to sustain profitability at today’s depressed gold price. Complimenting profitability objectives is the perpetual challenge to reduce operating costs and optimize unit operating efficiencies while being ever vigilant of issues such as environmental control.Key to achieving these objectives at Omai are throughput and availability gains, gold recovery improvements, cost cutting, a skilled workforce and reliable power generation.This paper attempts a cursory review of startup-to-date plant performance advances and future performance-driven objectives.INTRODUCTIONCommercial gold production at Omai commenced in January 1993 with annual output climbing to 250,000 opy by the end of 1994. A mill expansion completed in August 1996 increased gold production to 340,000 opy by the end of 1997. Current milling capacity is 20,000 in tpd (nom.) with actual capacity exceeding 24,000 mtpd on occasion depending on ore size and competency. Mill feed consists of hardrock (andesite, quartz diorite and rhyolite) and softrock (saprolite, laterite) at a nominal ratio of 70:30, respectively. Present day average weighted grade of the feed is 1.4 g/t gold.PROCESS DESCRIPTIONPrimary crushing, ore handling and the original 12,000 mtpd hard and soft rock SABC mill, hereafter referred to as Mill ‘A’, remains essentially as documented by Konigsmann (1 994) complete with poststartup modifications. The mill expansion, hereafter referred to as Mill ‘B’, features an 8000 mtpd hard rock SABC grinding circuit similar in configuration to Mill ‘A’ and differing only in equipment size. Mill ‘B’ includes a 9000 H.P.,reversible variable speed , single pinion drive, 32 ft. x 15 ft. SAG inill , a 7 ft. short head pebble crusher and two parallel 5400 H.P., 16.5 ft. x 30 ft. ball mills with dedicated cyclone packs consisting of D26 hydrocyclones."

Knowledge of in situ stresses is fundamental to many studies in earth sciences, and coal mine ground control is no exception. During the past 20 years, it has become clear that horizontal stress is a critical factor affecting roof stability in underground coal mines. The theory of plate tectonics and the World Stress Map (WSM) project has been extremely helpful in explaining the sources and the orientations of the horizontal stresses observed underground. Recently, WSM geophysicists studying deep-seated stresses have developed a model of how stress magnitudes vary with depth in the crust. They have devoted relatively little attention to near-surface stresses, however. This paper explores the relationships between deep-seated and shallow in situ stresses in several of the world?s coalfields, using a data base of more than 350 stress measurements from underground coal mines. The analysis indicates that distinct regional trends exist, corresponding roughly to the regional stress fields identified by the WSM. The paper presents equations for estimating stress magnitudes that were developed by treating depth and elastic modulus as independent variables in regression analysis. The magnitude of the horizontal stress increases with depth, at rates that range from 0.8 to 2.0 times the vertical stress, just as the WSM ?critically stressed crust? model predicts. Overall, it seems that the stress regimes encountered in underground coal mines are closely linked to those that exist deep in the earth?s crust.

High-grade ore found in a shaft pillar at the Homestake Mine prompted a request for a Bureau of Mines study to determine if selected areas of the shaft pillar could be mined without jeopardizing the shaft. The design approach was to perform a finite-element study of the shaft pillar. Plan view and vertical sections were developed using two-dimensional, elastic, plane strain assumptions. Input into the model consisted of in situ stress measurements geologic mapping rock mass properties, and previous mining history. Results from the study indicated that ore-beating sections of the shaft pillar could be removed within the displacement tolerances acceptable for the shaft. Homestake engineers, using the results of the finite-element study and considering other factors, have designed and are beginning to mine the shaft pillar.

State-of-the-art seismographs can be employed to assist the blasting engineer in understanding the propagation of vibration waves generated by blasting. Wave traces and frequency plots are used to adjust shot loading and timing, especially around close or near- field structures. Successful close-in blasting vibration monitoring requires careful consideration of the following: Geophone locations Mounting of geophone both on structures and in the ground Differences between ground motion and structural response This presentation will discuss methods used to control and monitor blast-induced ground and structure motions. Case histories are presented to illustrate seismic monitoring used as a tool to control vibrations and guide the blaster in the design process.

In recent years the Augered Cast-In-Place (ACIP) Pile industry has seen a substantial increase in the quality control options available to Contractors, Engineers, and Owners. Quality control methods range from the simple, manual techniques that have been used for years to new computerized installation monitoring equipment and nondestructive testing methods. Our ability to monitor and test ACIP piles has grown to such a degree that the industry is struggling to determine how much quality control is enough. This paper reviews the many options available and describes the pros and cons of each. The various nondestructive testing methods, in particular, have been difficult to apply reliably due to misapplications of methods and misinterpretation of the results. ACIP piles have unique properties that require the various nondestructive testing methods available be considered very carefully in determining the method most appropriate for a particular use. Some nondestructive testing methods, as well as automated monitoring equipment, potentially can be applied effectively to increase the level of quality control for ACIP piles. The experience gained on real projects using these new methods, in conjunction with the old procedures, provides valuable information for future applications.

Anglo American Platinum has adopted a strategy of Best Practice Principles (BPP) for sampling, mass measurement, analysis and metal accounting for more than a decade now. When new plants are designed, the most suitable sampling equipment and sampling protocols are implemented and material flows and plant layouts are designed around sampling strategies. A few years ago, a crushing plant for a high-grade matte stream was commissioned.The design called for the high-grade material to be crushed to 95 per cent passing 2 mm and the design allowed for a single cutter primary Vezin sampler, increments taken from a gravity flowing stream at regular intervals, whilst filling a tanker for the pneumatic offload at our base metal refinery. This was a single point of sampling and analysis that would measure the wanted output from our smelting complex and the input to our refineries. In addition, the design incorporated the functionality to collect increments in an ‘interleaved’ fashion from the Vezin sampler for internal precision checks.The metal accounting strategy for Anglo Platinum is the ongoing measurement of all inputs, outputs and stock levels for all our operations for platinum group metal (PGM) and base metal (BM) content. Included are yearly stocktakes, usually done on 1 February every year, across the smelters and refineries (at least the base metal refinery). A trend was developing in the accounting period immediately following the introduction of the new crushing and sampling facility, whereby the absolute difference between the physical stock and theoretical stock (PGMs predominantly) increased for the two respective sites on an ongoing basis. The smelter (sender) showed an increase in physical stock compared to the theoretical stock and the base metal refinery (receiver) showed that their theoretical stock increased against their physical stock. Because of this anomaly, an additional stocktake was held in September for the period in question and it was confirmed through the physical stock measurement results, that the PGM accounting problem was indeed pointing towards the evaluation of the high-grade matte stream.This paper showcases parts of the process that took place to explain the poor accountabilities for the respective sites. Previous results from the sampler ratification process (of the interleaved sampling design) were analysed again, a physical inspection was carried out on the sampling system again and the sampler performance was monitored (quality control parameters) for the period.From these investigations, it was found that airflows present inside the Vezin introduced the sampling bias and a change in sampling protocol was introduced as well as physical changes to the sampling equipment were made to eliminate the airflow and thus the bias conditions significantly. The accounting period which followed, after a phased upgrading process, showed that the accounting between our smelter and base metal refinery had been restored to within statistical limits (difference between physical and theoretical stock below five per cent relative to theoretical stock).CITATION:Kruger, C J and le Roux, E, 2017. Mining explosions designed to move, bulk or fracture rock are often composed of a number of explosions arranged in a complex spatial and temporal pattern. The effects of the explosions are strongly dependent upon the design and implementation of this blasting pattern. This paper describes the collection, combination and visual display of multiple types of data from these explosions for the purposes of understanding the relationships between the blast design, implementation, and observations. Data sets consist of three-component ground motion, acoustic, video and high speed film, three-dimensional topography, geological and geophysical properties, design blast pattern and timing, and velocity of detonation in the explosive for individual borehole detonation times. The explosions studied include simple single-fired contained explosions, ones designed to bulk and fracture rocks and those that cast material.

The purpose of the present study was to develop and test improved Type V workvests for use on dredge operations. The goal was to design workvests that would be more comfortable to wear, would interfere less with task performance, and would better meet the needs of dredge workers. It was hypothesized that by attaining these goals, the resulting vests would be worn more often and would be kept buckled more of the time than traditional vests. The study involved three major phases. In Phase I, visits were made to dredge operations and a definition of the problem was developed. Problems with current vests were identified and working conditions and tasks surveyed In Phase II, prototype vests were developed and laboratory tests were per-formed to assess comfort and interference. In Phase III, the prototypes were modified and field tested with actual dredge operators and workers.

Stanford University operates the National Research Facility which is located at the Stanford Linear Accelerator Center (SLAC) in Menlo Park, California. In order to improve and allow for enhanced experiments, the current facilities required expansion. A1500 foot long tunnel (20 ft wide by 21 ft high) and a 220 foot long underground cavern (49 ft wide by 32 ft high) were excavated through the Ladera Sandstone formation which is an intermediate geo-material with week rock properties. The overburden between surface and crown of the structures varies from 10 to 80 feet. The underground structures were excavated according to the principles of the NATM (New Austrian Tunnel Method). This paper addresses the challenges that were encountered during the construction phase of the project and how construction issues were resolved mutually by the owner, engineer and contractor. The implementation of geotechnical instrumentation, recorded data and interpretations, soil conditions and behavior encountered during construction are also presented.

This report on the Central mining district of New Mexico is the partial culmination of an intensive U.S. Geological Survey effort dating back some 30 years. Robert M. Hernon went to Silver City in 1948 to finish a detailed restudy of the district that had been started in the thirties by Samuel G. Lasky. He was joined by William R. Jones in 1951, and Jones took over as project chief when Hernon resigned to do consulting work in 1953; it was under Jones' guidance that most of the work was brought to completion or nearly so ( 17, 22-24, 26-30). Hernon returned to the Survey in 1958 to work in .the southern Appalachians. Early in 1965 Jones urged that Hernon be reassigned to the Silver City project to help complete the Professional Paper report on the ore deposits of the Santa Rita quadrangle as well as to take over preparation of the report on the Central district for the Graton-Sales volume. This reassignment was approved, and Hernon began work on the present report. By late June he had completed most of it in draft form, and on June 28 Jones and Hernon left Denver for a short trip to Silver City to fill in a few gaps and to reacquaint Hernon with activities in the district, which he had not visited for 12 years. Late in the afternoon of June 29, 1965, as Bob and Bill were 85 miles short of their destination, their jeep collided head-on with a truck that was attempting to pass another vehicle in a cloud of dust. They both died instantly. As the most recent surviving member of the Silver City project (1958-1962), I was given the honorable though rather sorrowful task of putting the Hernon-Jones report "in shape" for publication. Fortunately the manuscript was already in good shape, so that the task has consisted of little more than minor reorganization and editing, and of preparation of the illustrations, abstract, and bibliography. The sections on wall-rock alteration had been left for Jones to complete in the light of his recently intensified interest in alteration geochemistry (25), and it is in these sections only that. the report may fall short, for they have been filled in from a 1958 manuscript by Jones on the ore deposits of the Santa Rita quadrangle that he intended to revise considerably. The shortcomings, however, are largely in the interpretation, not in the field data. As a brief but accurate summary of what was known about the geology of the Central mining district in 1965, this report stands as a fitting testimonial to the scientific dedication of two exceptional men.

The objectives of this paper are to introduce the International Code of Practice for Risk Management of Tunnel Works and to illustrate to the participants of the 13th Australian Tunnelling Conference 2008 how the insurance industry would like to see the future of tunnelling insurance. Tunnel construction projects have suffered large losses over the years as can be shown from various loss examples and a list of major losses over the last ten to 15 years. Since the consequences of insufficient risk control had to be borne by insurers, these losses have heightened the need for improved and adequate risk management measures so that insurance cover can also in future be provided for this hazardous industry sector. Observations are made as regards to the prevailing trends in the tunnelling industry, which contribute to the increase in risk and have adverse consequences for the insurance industry. To find a solution to this dilemma various options are examined with the suggestion of a Joint Code of Practice for Tunnel Works being agreed by all stakeholders, which would become essential to secure future insurance protection for such project work. The British Tunnelling Society has, in cooperation with the Association of British Insurers, developed such a code, which the International Tunnelling Insurance Group has now adopted for global application in international markets. The content and concept of this code is explained to establish a framework of understanding and applied risk management principles for tunnel works. This initiative also has the support of the International Tunnelling Association and the International Association of Engineering Insurers. The international application of this new code is demonstrated with a few examples, which also includes Australian projects. It is the aim that this code forms in future part of the insurance coverage for tunnelling works in order to put this risk segment on a more sustainable basis. For that reason the insurance industry is seeking the support of all stakeholders of tunnelling construction projects.
