Incident investigation is of utmost importance in most high-risk industries and is often used to improve organisational safety. This study was undertaken to examine the content of past incident investigation reports to determine the effectiveness of the incident investigations. The study uses a semi-quantitative method to assess the effectiveness of incident investigations in the Ghanaian mining industry by evaluating the quality of past investigation reports. The assessment tool consists of five elements with several indicators and rating scales for assessing the quality of an investigation report as a measure of the effectiveness of the investigation. The method was applied to 304 investigation reports of three Ghanaian large-scale gold mines, and the results correlated with incidence rates of the mines to determine if any relationship existed. The results showed that the mines differ significantly in the quality of their investigation reports, suggesting differences in the effectiveness of their investigations. In addition, the incidence rates of the mines negatively correlated with some elements of the assessment tool. In general, the method was found useful and revealed areas where improvement is needed.

This paper deals with the various options considered and factors that influenced the final selection of the headframe, shaft layout and hoisting systems required at the Impala No 16 Shaft in order to hoist both the required 3 Mt of ore per annum, and transport men, material and equipment between the surface and the underground workings to a depth of 1640 m. The layout and construction method used for what is at present the world?s tallest concrete headframe is discussed together with the integrated hoist room foundations designed to suit multiple mechanical and electrical hoist supplier?s solutions. The mechanical components of the two tower mounted 6,5m 4 rope Koepe hoists and deflection sheaves and their layout is dealt with in detail. Various features of the bearing type, brake units and brake control system are also discussed. Installation of the mechanical and electrical hoist equipment in the headframe took place during the sinking phase of the project and placed severe restrictions on the use of the heavy lifting facilities. This required a well-managed and systematic approach to delivery of the hoist components and associated equipment, as well as close co-operation between all contractors on site.

Each year, hundreds of mine workers are involved in machinery-related accidents. Many of these accidents involve inadequate or improper use of lockout/tagout (LOTO) procedures. To mitigate the occurrence of these accidents, new safety methods are needed to monitor access to hazardous areas around operating machinery, improve documentation/monitoring of maintenance that requires shutdown of the machinery, and prevent unexpected startup or movement during machine maintenance activities. The National Institute for Occupational Safety and Health (NIOSH) is currently researching the application of Internet of Things (IoT) technologies to provide intelligent machine monitoring as part of a comprehensive LOTO program. This paper introduces NIOSH’s two phase implementation of an IoT-based intelligent machine monitoring system. Phase one is the installation of a proof-of-concept system at a concrete batch plant, while phase two involves scaling up the system to include additional sensors, more detailed safety/performance metrics, proximity detection, and predictive failure analysis.

Recent accidents in the United States underground Coal Mines and the on-going carnage in Underground Chinese Coal Mines has shown the danger associated with the lack of communication infrastructures in an underground environment. The US Mine Safety and Health Administration (MSHA) have introduced legislation that states all Underground Coal Mines must have two-way voice and personnel tracking capabilities by 2009. If Coal Mining companies are being forced to deal with this problem Tunneling Companies will not be far behind. This paper will examine current practices and best case technology to deal with these issues.

Industrial grinding machines used in the mineral processing industries are mostly of the tumbling mill type. These mills exist in various types: ball, rod, pebble, autogenous, and semi-autogenous. Balls and rods are traditionally media shapes used in the tumbling mills. In resent years, as an alternative to balls, grinding charges with various shapes are being used. Cylpebs are slightly tapered cylindrical grinding charges with length equaling diameter, and all the edges being radiused. Cylpebs of equal diameter and length have 50% greater surface area and 45% greater weight than balls of the same material. In addition, they have 9% higher bulk density than steel balls, or 12% higher bulk density than cast balls. An important advantage of cylpebs over balls is that cylpebs produce narrower granulometric distribution due to their shapes. However, some discrepancies were found in the literature regarding the milling performance of cylpebs in comparison with balls (Chi, 2004). In recent studies, Chi (2004) has provided detailed information on cylpebs and has compared these to balls in relation to media mass, size distribution, surface area, and input specific energy. Ipek (2006) has investigated the effects of cylpebs and balls with equal mass and surface area on the breakage rate of quartz, using cylpebs and balls separately. Similarly, Lameck and Moys (2006) have investigated the effects of balls and worn balls on breakage rate of material using worn balls as grinding environment. However, no study has attempted to investigate the effects of grinding environment other than balls on the brekage distribution. As indicated by Prasher (1987), the breakage distribution function is independent of the following variables in ball mills: hold-up (Mika et al., 1967; Kelsall et al. 1969), mill speed and ball load (Herbst and Fuerstenau, 1972), media load and ball density (Kelsall et al., 1973), mill diameter (Malghan and Fuerstenau, 1976), and powder charge (Shoji et al. 1980). The aim of the present paper is to compare the effects of cylpebs and balls on the breakage distribution function of quartz with respect to same mass and same surface area. BACKGROUND Comminution as a rate process is called kinetic approach. Studies on the kinetic approach are reviewed by Oner (2001). Oner states that grinding kinetics are based on studies by Kolmogorov in 1941 and that Robert (1950) is the first to suggest this approach. Later, Melzak, Loeveday, and Reid have improved the grinding kinetics equation; and, Austin and Lynch have conducted two basic studies on the analytical solutions of these equations. Considering the results of studies comparing the size reduction mechanisms, it has been accepted that breakage constitutes of two operations. These two basic operations, first defined by Epstein (1948), are those of selection of material for breakage and the distribution of breakage material to particle sizes smaller than the initial size (Lynch, 1977). Epstein defined the two operations as: ? Pn(y), the probability of breakage of a particle size in the nth step of the breakage process. ? F(x,y), the cumulative distributions by weight of particles of size x<y arising from the breakage of a unit mass of a size y. Probability of breakage is called a selection or a breakage-rate function (Si). Characteristics size distribution after breakage is called a breakage distribution or appearance function (Bi,j). Due to the difficulties in representing particle size distributions mathematically and constantly, the batch equation based on particle size fraction is suggested. Thus, as is the case in breakage distribution and specific rate of breakage, the breakage parameters were defined by batch grinding experiments conducted in laboratories (Oner, 2001). When effective breakage occurs in the mill, the breakage of material with single feed size fraction usually fits first order grinding kinetics and the specific rate of breakage values can be calculated through the slope of semi-log plot. The values of the breakage distributions are deduced from the size distributions at short grinding times where there is mainly size 1 material breaking and only small amount of smaller sizes to rebreak (Austin et. al., 1984). B values can be calculated by using BII. [ ] where j is the top size of the charge, Pi(t) is the cumulative percent undersize of i th size interval at the short grinding time which gives no more than about 30% broken out of the top size interval. Cumulative primary breakage distribution, Bi,j, is also defined in an empirical form by [ ] where xi is the top size and Bi,j is the weight fraction of primary breakage products. The parameters F, ? and ß define the size distribution of the material being ground. On plotting size versus Bi,j on log scale, the slope of the lower portion of the curve gives the value of ?, the slope of the upper portion of the curve gives the value of ß, and F is the intercept as in Fig.1. AT Kellogg, Idaho, J. Fred Johnson is driving the 5000.-ft Hooper Tunnel under contract for the Bunker Hill & Sullivan Mining Co. This was visited by a group of engineers during the recent meeting of the A. I. M. E. at Spokane, who were impressed with several features of the work. An excellent record of advance is being made by the contractor, over 47 ft. being driven in 3 days during October, and 223 ft. advanced in 29 shifts. The size of the tunnel is 8 by 9 ft. The ground is Wallace quartzite and, save for the first hundred feet, stands well and does not require timbering. The grade is held uniformly at 0.5 per cent. Two shifts are employed, with two sets of machine men to the shift. Ingersoll-Rand S-70 drills are used. each machine being set up independently in the heading; 26-30 holes are drilled to the round; using 1 1/4: in. round hollow drill steel. A 7-hole cut is required, and these holes are blasted simultaneously, using 60 per cent gelatine, 12 sticks to the hole. Instantaneous and fire delays of electric detonators are used for the complete round. A feature of the drilling is the use of long carriage drills, necessitating fewer changes of steel, and more time spent on drilling, with less time consumed taking steel back and forth from the face. Of course, this means better blacksmithing so that the drill steel shall stand up for the added footage.

The ~2500 km long Kermadec-Tonga arc northeast ofNew Zealand is the longest continuous intraoceanic arc in the world. At least 94 volcanic centres along the arc are submarine, most (87 per cent) within the Kermadec arc (de Ronde et al, 2003a). Volcanic rocks range from basalt to rhyodacite with trace element and isotopic data indicating significant magma source heterogeneity both along and across the arc. &nbsp; The entire ~1300 km of the Kermadec arc has now been surveyed for hydrothermal emissions (de Ronde et al, 2004). First-stage surveys utilised surface vessels. In February 1996, the first sulfide samples from the southern Kermadec arc were dredged from the Brothers and Rumble II West caldera volcanoes (R/V Tangaroa cruise TAN96/03; Wright et al, 1998; de Ronde et al, 2003b). In September 1998, low-temperature, diffuse venting was discovered at Monowai, Rumble III and Clark volcanoes, and high-temperature, black smoker venting was found within Brothers caldera (R/V Sonne cruise SO-135; Stoffers et al, 1999). The NZAPLUME cruise of March 1999 surveyed 13 volcanic centres (each mainly comprised of single, large volcanoes) of the southern Kermadec arc and documented hydrothermal emissions above seven of them (de Ronde et al, 2001; Baker et al, 2003; Massoth et al, 2003). In July 2001, additional mineralised and hydrothermally altered samples were dredged from Brothers volcano (R/V Tangaroa cruise TAN01/07). The NZAPLUME II cruise of May 2002 confirmed venting was still occurring at all the known hydrothermal sites, and that ten active vent sites occurred along the mid-part of the arc, between Brothers and Raoul Island (de Ronde et al, 2002). In September-October 2004, NZAPLUME III surveyed the northern-most part of the Kermadec arc where another nine volcanic centres are hydrothermally active. &nbsp; Second-stage exploration of the Kermadec arc hydrothermal systems utilised deep submergence vehicles. In October-November 2004, the Japanese submersible Shinkai 6500 made four dives on Brothers. Samples recovered include vent fluids, chimneys, altered rocks and various animals and microbes. In April-May 2005, a more extensive expedition using the American submersible Pisces V made 23 dives on nine different volcanic centres, eight of them for the first time. &nbsp; Extensive plume surveys show Brothers volcano is host to the most hydrothermally active sites of all those known along the Kermadec arc. In total, nine dives have been made on Brothers, six on the NW caldera site and three on the cone site. The NW caldera vent site is a long-term hydrothermal system that is today dominated by evolved seawater but has had episodic injections of magmatic fluid. The cone site is a nascent magmatic-hydrothermal system where crack zones localise upwelling acidic waters (de Ronde et al, 2005). A SE caldera site represents the main upflow of a relatively well-established magmatic-hydrothermal system on the seafloor where sulfide-rich chimneys are extant. Each of these different vent sites represent diverse parts of an evolving hydrothermal system, any one of which may be typical of submarine volcanic arcs.

The mining industry is faced with imperatives to improve worker safety and productivity, adapt to skills shortages, high worker turnover rates, and provide more effective maintenance to new and ever more complicated plant and equipment. While more effective use of data combined with advanced analytics offers opportunities for improvement, providing this information to the worker in the field, in real-time, has proven expensive, cumbersome and ineffective. New capabilities are now emerging which address some of the limitations experienced to date. Augmented reality (AR) provides a means to overlay interactive digital information on top of the physical world. Combining the latest visual display, mobile computing and ‘track and trace’ technologies, AR provides an experience similar to a heads-up display in a fighter jet through small, personnel devices, such as smart phones or wearable glasses. Applying AR would allow a mobile worker to ‘see’ all relevant information for a given task, in the context of their physical environment and at the time they require it. The technologies behind AR are rapidly maturing and becoming available at a price point that is viable for commercial applications. It is anticipated that AR will become ubiquitous at a consumer level within the next five years. The specific challenges faced by the mining industry make it an ideal early adopter of AR technologies. This paper examines a number of use-cases of how AR can be applied to the mining industry and the benefits that can be realised. The paper will identify areas of further work and viable opportunities for immediate adoption within the industry.

In many mineral processing operations, the reported production figures for an accounting period may not make the best use of available data. While most of the mathematical approaches, such as check-in/check-out, mass balancing, used in effective metallurgical accounting strategies are well understood, the sources and magnitude of measurement errors generally are not. As part of the &apos;AMIRA P754-Metal Accounting and Reconciliation&apos; project, a study was undertaken at the Northparkes copper/gold concentrator near Parkes, New South Wales to examine the data used in their concentrator metallurgical accounting system. This paper summarises the results of a detailed examination of the mass measurement systems around the concentrator as well as the primary sampling system used for grade estimation. The goal of this work was to develop practical techniques to quantify the error (ie both accuracy and precision) of the measured metal accounting information. With this information, statistical confidence levels for this data can then be calculated. If the errors are small, the effect of error in each piece of measured data can then be propagated to other calculated production figures such as recovery and feed grade using a statistical technique called `the propagation of error&apos;.

Magnetic surveying and map production is well established in land based mineral exploration. Magnetic data are routinely used to map geology in covered terrains, to estimate the depth to basement in overburden covered areas, and to identify altered zones, mineralization, bedding attitudes, and fault networks both regionally in map view, and through interpretation to depth in 3D. Best practices for magnetic survey methods and magnetic interpretation exist in all areas of land based geophysical survey methodology - mineral exploration, hydrocarbon exploration, geological mapping, archeological mapping, and in military applications. At sea, and particularly under the sea, sophisticated magnetic surveys are being done, but it can be argued that these are primarily in niche applications, for example in naval mine-hunting and UXO applications. Magnetic data is not normally collected on AUVs and ROVs conducting mapping and hydrographic survey operations. OFG has developed physical and mathematical calibration routines so that magnetometers can be mounted directly onto AUV and ROV bodies where significant heading errors and magnetic gradients exist. This simplifies the routine deployment of magnetometers on AUVs and ROVs for mineral exploration and other magnetic survey applications without the need for towed bodies, ungainly mounting frames, or degaussing of the survey equipment. Our experience shows that magnetometers can be operated routinely on AUVs and ROVs and that with careful processing useful maps can be produced routinely at low cost.

Within the metal/nonmetal mining sector, fall-related incidents account for a large proportion of fatal and non-fatal injuries. However, the events and contributing factors leading up to these incidents have not been fully investigated. To help provide a clearer picture of these factors, an analysis of imminent danger orders issued by the Mine Safety and Health Administration (MSHA) between 2010 and 2017 at both surface and underground metal/nonmetal mine sites revealed that most orders are associated with fall risks. Of these cases, 84% involved the workers not using fall protection, fall protection not being provided, or the improper use of fall protection. Fall risks for workers most frequently occurred when standing on mobile equipment, performing maintenance and repairs on plant equipment, or working near highwalls. In most cases, a single, basic, corrective action (e.g., using fall protection) would have allowed workers to perform the task safely. Overall, these findings suggest that a systematic approach is needed to identify, eliminate, and prevent imminent danger situations. Furthermore, to protect mineworkers from falls from height, frequently performed tasks requiring fall protection should be redesigned to eliminate the reliance on personal fall protection.

The Donkin-Morien Mine in Cape Breton, Nova Scotia, has successfully completed the excavation of a 7.6m (25-ft) diameter access decline over a distance of 3.5km out under the Atlantic Ocean. The tunnel, completed in December 1984, was driven through a sedimentary sequence of sandstone, siltstone, mudstone and various gassy coal seams prior to intersecting the Harbour Coal Seam where development work will commence. The tunnel was driven by a Canadian prototype tunnel boring machine (TBM) manufactured by Lovat Tunnel Equipment, Inc., Toronto, Ontario. The 7.6m (25-ft) diameter full-face shielded TBM operated at various grades from 0-20%. This paper outlines the TBM design, excavation, support and backup system utilized in the drivage. Problems encountered and drivage rates will be addressed.

Thirty-five years ago underground coal mining equipment consisted of simple but rugged machines powered by electric motors and hydraulics. The machines were maintained by maintenance personnel armed with a basic knowledge of hydraulics, electricity, and mechanical design. Over the years the the machine has been transformed into powerful complex mining systems. The maintenance burden has also increased. During this period significant advances have been made in maintainability engineering and system design techniques. Many of these innovations and techniques are applicable to the design of underground mining equipment. The objectives of this project were to; (1) determine the extent to which maintainability design concepts and principles have been applied to the design of underground coal mining equipment, (2) try to assess the impact of equipment design for maintenance on productivity and safety, and (3) develop a maintainability design guideline to enhance the design of new or rebuilt equipment. An equipment design review was completed at ten operational coal mines. Interviews were also conducted with mine management, safety, and maintenance personnel to identify machine specific design advantages and problems. Visits were also made to six original equipment manufacturers to discuss the design of their equipment for maintenance. Volume I presents an overview of the protocol used and a summary of the findings. Volume II includes the maintainability design guide for underground mobile mining equipment.

A program ?SlagVis? has been developed to simulate the liquidus temperature, phase compositions and viscosities of ZnO-CaO-SiO2-Al2O3-?FeO? slag, such as Imperial Smelting Furnace (ISF) slag and lead blast furnace slag, combined with F*A*C*T slag database and the mathematical model of slag viscosities based on a modified Urbain formalism. The modeling results have been validated by a number of measurements. The effects of slag compositions on viscosities of ISF slag and lead blast furnace slag have been simulated and discussed.

Such investigations commonly make use of a number of techniques, and the results are combined for a meaningful interpretation, which can lead to a successful solution. Mineralogical applications are described from pyrometallurgical operations. Examples are given of studies and investigations involving mineralogy in chemical-looping combustion, feed monitoring to platinum-smelting furnaces, and losses to slag in base metal and platinum smelting.

In today&apos;s rapid changing construction world, the business pressures on piling and deep foundations reach sky-high levels. Ever increasing complexity of construction projects do severely contribute to these business pressures. Projects are often constructed in densely populated areas and located close to vulnerable buildings and infrastructure. The inherent uncertainty of ground conditions plays a dominating role in these foundation projects. Effective risk management is a key success factor for profitable and sustainable operations of piling and deep foundations. It starts with a fundamental requirement to be aware of risks. Next the different risk perspectives of all stakeholders in any foundation project have to be acknowledged. A simple and structured risk management approach is introduced, to demonstrate that effective risk management is not necessarily that complicated. A major element is the contractual allocation of risks, as a result from differing ground conditions. It is based on the so-called Geotechnical Baseline Report (GBR). The main objective of the GBR is to provide a clear contractual arrangement for the allocation of risks, arising from differing site conditions. The benefits of explicit risk management in general and contractual risk allocation in particular are illustrated with an example and experiences. The presented structured risk management approach, including contractual risk allocation, is still rather new. It proves to be promising for cost-effective risk control of piling and deep foundation projects, anywhere in the world.

To many Australian Mining Engineers, we take it, the singular features of banket deposits are not as familiarly known as they would wish, possibly because in the majority of instances they are not in touch with the South African milling world, and have had little opportunity of studying the development of the banket beds, either from correspondence or literature. The writer hazards this opinion feelingly, because, when in Australia and before he had visited Africa, the utmost knowledge he was able to obtnin was a mass of conflicting theoretic discussion as to the agencies which formed the banket bed, without any practical hints as to their geological horizon, the rocks with which they are associated, the usual appearance and character of the ore, in fine, without that everyday detailed description which the miner requires when seeking information regarding mineral or metal deposit foreign in character to any that he has yet studied, or as yet had any opportunity of studying in his own country.The writer recognised that many more able men than he have from time to time discussed the features and formation of the banket beds of the land, notwithstanding which and feeling that many of his Australian colleagues have found themselves in a similar position regarding accurate detailed information respecting these African banket deposits as he himself was, he hopes that the present paper on the said deposits, as found on the fields under notice, will prove useful to some and of interest to all.
