The mining sector has been a leader in investing in safety improvement initiatives, yet relatively few mining organisations can accurately track the positive effects of these initiatives or identify areas that may require more attention. The consequences of not taking safety seriously are severe, and unfortunately injuries and fatalities are continuing to occur across the mining sector (Brady, 2019). There is a tendency to turn to lag indicators such as All Injury Frequency Rate (AIFR), Total Recordable Injury Frequency Rate (TRIFR), and Lost Time Injury Frequency Rate (LTIFR) to track these outcomes. As outlined below these approaches are inherently flawed and a renewed focus is needed on indicators within the control of leaders, indicators that have high predictive validity for future events and data that is used in a way to enable learning and change.

Due to the perceived potential to extract the second economic manganese seam at its Black Rock Operations in the Northern Cape Province, South Africa, Assmang commissioned an investigation with respect to mining method and sequencing of the two orebodies in 2007. Based on this initial study into multiseam potential, conditions and impacts, the rock engineering aspect was expanded to determine conceptual rules to govern the extraction of both manganese seams using the normal bord-and-pillar layout implemented at this operation. This work included the theoretical analysis using basic pillar design practices and numerical modelling, including the use of Map3D, UDEC and Phase2 codes.In 2013, Assmang launched a large-scale investigation into the potential to increase mine output from all possible manganese sources at the operation. Included in this investigation was the requirement to confirm or modify the conceptual multiseam extraction rules to implementation. As part of this confirmation process, stress measurements, borehole camera audits and additional numerical modelling were performed at a particular site on the operation where a limited extraction of both seams had been undertaken. To maintain copper recoveries and maximise production at the Sepon Copper Operation it is crucial to control the amount of copper entering the leaching circuit. This requires close control of blending of the multiple ROM stockpiles to achieve the desired head grade. Grade variability in the ROM stockpiles can lead to significant variation in the processing plant copper feed grade.Head grade sampling from the milling circuit at Sepon is complicated due to milling in leach solution to preserve the water balance. Copper is present in both liquid and solid forms and copper in the added milling solution has to be accounted for. This leads to difficulties in manual head sampling which also introduces delays due to sample preparation and assay turnaround. On-stream analysis is complex because of the solution and solid copper and the need to account for recycled copper solution.This paper describes the complexity of feed grade management at Sepon and the application of on-belt analyser technology to assist in the control of copper metal throughput.

The commonly accepted genetic theories of Pilbara iron ores and Woodie Woodie manganese ores have often presented them as quite unique. This apparent uniqueness is not confined to just iron and manganese, but is also seen in gold and copper deposits in Western Australia. The published material on iron and manganese ores would seem to have either a quite complicated genesis or none at all. An alternate theory for the origin of these deposits is presented which involves a protore with simple oxidation and pH controls with hydrothermal fluid sources from the diagenesis and dewatering of the underlying strata. This theory explains the distribution, thinning, cold dolerites and very high concentrations of deleterious minerals observed in these deposits. This genesis system is also related to the systems that deposit hydrothermal gold and copper in sediments in the region.This paper presents the author’s observations, sampling and mapping after several decades studying and exploring for the minerals in Australia and locations overseas. All of these theories were developed with (or in spite of!) the guidance of Mal Kneeshaw, but would never have seen the light of day without him and the others presenting here. Mineral resource development activities impact the health and well-being of many people, from workers, to families and surrounding communities. In the current environment of corporate social responsibility life-of-mine planning considers that mining activities should be conducted in the best environmentally, socially and economically acceptable way for the entire life of the mine (ie from exploration to decommissioning and closure). In this context, the health impacts of mining extend throughout the life of the community from worker injury and chronic exposure to communicable and non-communicable causes of illness, through to family and community health and well-being. At the inaugural Life-of-Mine conference in July 2012, the authors proposed the life-of-mine in the life-of-community (LOMLOC) matrix as a framework for understanding issues at the interface between resource extraction operations and the surrounding environment.This paper reviews and compares health activities extracted from social and environmental reports published by selected major mining companies with American operations in Canada, Colombia and Brazil. These corporate approaches to public health management are mapped to the LOMLOC matrix to identify and compare trends and behaviour in corporate health investments. The mining industry continues to have major disasters (Bingham Canyon, Pike River, Upper Big Branch, Mount Polley and Samarco), which often have a devastating effect on the enterprise, in terms of personnel deaths and injuries, environmental destruction, and financial and reputational loss. These disasters are not imposed from outside the business or organisation, they are almost always a function of loss of control of the management of the organisation’s critical assets and processes that they contain or to which they are exposed.Asset integrity is discussed as an effective response to business risks in order to mitigate, control and recover from loss scenarios that could expose a mining operation to significant property damage and business interruption, or even total loss. Vital to this process is the identification and control of risks to critical assets. Components of a critical asset assurance program are presented for discussion.Formal risk assessment has a longer history in industries other than mining, eg petrochemical, nuclear, military, aviation and space industries. Risk assessment is a structured, proactive approach to understanding and improving risks, as opposed to a reactive ‘fix-it-when-it-breaks’ mentality. Risk assessment techniques were triggered by major public disasters, such as the Flixborough chemical plant disaster (1973), Three Mile Island nuclear plant event (1979) and others.Today, most industries see risk assessment as an inherent part of their business, but do mining sites capture business risks that are related to major loss scenarios, which could have a significant impact on the operation?Does your risk assessment process:- involve a team having the right skill set so that the discussion is both stimulated and technically sound?- use generic assessment when a site-specific assessment is needed?- fail to consider all possible outcomes, including maximum foreseeable loss (MFL)?- fail to identify all hazards and controls associated with a particular activity?- fail to monitor controls effectively?This paper addresses the issues identified by these questions and provides some direction for developing a risk management process where business risk associated with critical asset integrity is integrated with other business and safety and health risk processes to provide a robust method for managing your business. The ventilation circuit at the FrogÆs Leg Mine was originally designed to permit mining to progress down to 440 m below surface at a maximum production rate of 500 kt/a, as per the feasibility study. The FrogÆs Leg resource has now been defined down to 640 m below surface at a maximum production rate of 750 kt/a. In order to allow for this increase in production depth and tonnage, it was necessary to upgrade the ventilation circuit.This paper outlines the upgrades to the ventilation circuit that were undertaken and the underlying rationale behind them.CITATION:Llewellyn, A and Varvari, M, 2013. The foundation of effective leadership is communication. You can have all the leadership theories, models and training you want, but in the end it is all about leaders talking to, listening to and understanding the point of view of those below and above them.The foundation of effective safety is peoples’ behaviours. Every incident, every near miss and every safe act comes down to what one person does and how that affects themselves and others. We tend to focus these behavioural considerations on the people at the ‘coal face’, those most in harm’s way. But it is the individual behaviours of everyone in the organisation, particularly those at the top, which are just as (if not more) important. The foundation of effective safety leadership in the mining industry is that it starts at the top, with the individual behaviours of leaders. The behaviours of each and every leader will be reflected in their teams. And where their teams are made up of leaders, these leaders will pass these behaviours on to their teams and so on. It’s the sum total of what people see their leaders do and what they do as a result, that creates the ‘culture’ of the business – and it’s this culture that makes or breaks how safely people work.

"Characterisation of sulfide minerals (eg pyrite, sphalerite) in historic tailings can assist in determining whether there are economic benefits of reprocessing such materials using modern metallurgical practices. In addition, there are associated environmental advantages if the historic tailings are actively generating acid and metalliferous drainage (AMD). At the Old Tailings Dam (OTD), Savage River, Western Tasmania, 38 Mt of pyritic tailings were deposited from 1967 to 1982; consequently it has been recognised as an AMD source. Long-term management options considered for this site have included the establishment of either a water, hard-engineered or vegetation cover. However, due to geotechnical challenges posed and the lack of success demonstrated by previous pilot studies, these strategies have not been implemented. Instead, resolving the pyrite chemistry can allow for a first-pass assessment of whether reprocessing these tailings is a feasible option as part of a long-term management strategy for this legacy site.Tailings samples (n = 144) were collected across the OTD from up to 2 m depth from five facies (A–E), and four zones (1 to 4; defined by hydrological regime and grain size). These materials were subjected to acid-base accounting (ABA) and mineralogical evaluations to determine their geoenvironmental characteristics. Following this, 26 samples were selected for in-depth laser ablation inductively coupled plasma mass spectrometry (LA-ICP-MS) investigations to evaluate pyrite chemistry across different tailings facies. The results revealed enrichment of Co and Ni in these materials. Specifically, Co content was up to 3.0 wt per cent, particularly within facies C to E, and Zones 3 and 4. Cobalt is recognised as a critical metal, therefore, reprocessing should be considered as a future rehabilitation option for the OTD, pending resource definition using established codes and practices.CITATION:Parbhakar-Fox, A, Fox, N and Jackson, L, 2016. In August, 1975, North Broken Hill Limited announced that an intersection of 37.3 m of high grade lead-silver-zinc mineralisation, assaying 26.2 per cent Pb, 495 g/tonne Ag and 21.2 per cent Zn had been obtained at its Mine at Broken Hill, 520 m north-east of its No. 3 Shaft and slightly below the No. 36 level (1581 m). Subsequent drilling confirmed the presence of the extension of the Broken Hill Orebody on the north-western side of the Globe Vauxhall Retrograde Shear Zone and defined 1.8 million tonnes of high grade mineralisation. The area containing this mineralisation was subsequently named the Fitzpatrick Area in tribute to the late Mr. W.E. Fitzpatrick a former Member of the the Board and Manager of the Ccnpany's Broken Hill operations. This paper describes in brief the exploration programme leading up to the discovery, the general geology of the area and the proposed future exploration progranme for further extensions of the orebody within the mining leases of North Broken Hill Limited.

The latter plant is unique in being the first installation for the successful use of these gases.

Learnings and key considerations between different monitoring approaches are explored, including themes of limitations and advantages that were observed consistently through global supplier feedback.

With a worldwide growing population the demand for higher living standards, food and the development of new technology is rapidly rising. To allow for a continuous growth, larger quantities of minerals and raw materials are needed at a faster pace. Different methods have been developed to access or tap ore more quickly, such as improved drill and blast operations, mechanical mining methods using equipment such as roadheaders (RH), longwall miners and other technical equipment. Another possibility to access and develop mines faster is the use of a tunnel boring machine (TBM).The historical usage of TBMs is surprisingly long, but the opportunity for technology adoption by the majority of worldwide mining companies remains largely untapped. Dozens of full face TBMs have been used by mines around the world, including two that will begin operation soon in Australia. TBMs have been used to safely and rapidly gain access to the orebody, or to expand access to the orebody to increase output by running a TBM along the edge of the orebody for several kilometres. These machines have also been used for ventilation and for conveyor tunnels for ore removal. They have been used to quickly excavate long and winding declines giving access to an orebody at several levels in a short period of time.Tunnel boring technology can be used for mine declines, mine access tunnels and all sorts of different mine designs and set-ups. TBM technology simply needs to be adapted to the mine conditions and, if manufacturers are involved early on in the planning process for mine development, the operation can be very successful. In some cases, historically typical mine designs may need to be re-thought.One mine, the Stillwater Platinum mine in Nye, Montana, is currently employing its fourth TBM in 25 years. Two were used to create tunnels running alongside the orebody, rapidly expanding access to the orebody in many locations for increased output. Two were used to create parallel access tunnels to a mine expansion at the East Boulder orebody; one tunnel is for conveyor haulage and the other for vehicular access. TBMs have been employed successfully many times to rapidly drive vehicular decline tunnels to multiple mine levels, such as at the San Manuel – Magma Copper mine in the USA. While TBM technology may not seem to be optimal for every mine, these success stories show that the method can provide vast benefits for the right projects.Early use of TBMs in extremely hard rock metal mines, in the 1960s and 1970s, had limited success since performance was low. However there have been great improvements made over the years in the strength of rock which can be excavated by TBMs with high performance. Other important improvements involve the provision of a safe work environment, safely and rapidly supporting the rock and predicting geology ahead of the TBM.This paper will briefly note the more than 20 TBM mining projects and highlight some notable successes. It will then go on to give a status report on the two active TBM mining projects in Australia and the difference between a TBM operation versus other excavation methods such as drill and blast (D&B) or RH excavation.CITATION:Rauer, M, 2014. All tunnels have unique characteristics that result in different fire and life safety risk. The fire safety systems for each tunnel also give different benefits at different costs. It is therefore neither pragmatic nor cost-effective to prescribe specific fire standards requirements for all tunnels. Each tunnel needs to be assessed individually but using a common process.The owner/client is normally the risk owner and needs to make the decision on what fire and life safety provisions will be provided within the tunnel. To make this decision they need to be appropriately informed. The low probability and high consequence nature of fire events creates a challenge such that traditional analysis processes, which tend to focus on a major event after it has happened, are not easily applied and do not give the best value for money. Therefore a process is required that provides the relevant information to the owner for decision-making.A quantitative risk assessment (QRA) provides a process and framework for the consideration of fire life safety risk when applied properly. This involves a risk assessment of the ‘do nothing’ option together with assessments of improvement options and their costs. It also involves consideration of benchmark safety levels for other tunnels and the open road.This paper will outline a QRA process for assessing fire and life safety risk in tunnels and how this has been used for three existing tunnels, Homer Tunnel, Lyttelton Tunnel and Mt Victoria Tunnel. Key challenges include determining an acceptable safety level, defining realistic inputs, providing sufficient complexity to get meaningful results but not so complex that transparency is lost and interpretation to enable decision-making by non-technical personnel.CITATION:Weaver, S, Wright, B and Ireland, T, 2014. The Black Star lead and zinc ore-bodies, situated in the western zone of mineralisation at Mount Isa occupy an area 2000 ft. long with varying widths from 100 ft. to 300 ft. The strike is N5°W, and the average dip 60° West.To test at depth the potentialities of this area, an extensive diamond drilling campaign was commenced on 16th October 1927. It was continued, with from one to six machines working, until the end of 1931. During this period, 26 inclined holes, with an average depth of 1224 ft, and a total footage of 31,828 ft. were completed. A vertical hole, 2452 ft. deep, for comparative purposes, has been omitted.These drilling operations came directly under the charge of G J Gray, General Superintendent of Mount Isa Mines, and, in the early stages were supervised by E. D. McDermott, of the Mining Trust, London. The foremen, who were in charge of various sections of the drilling operations, were: H Vale, F Bray, J Winston and A A Sanstedt.

The green production of the steelmaking process has urgent requirements for fluorine-free fluxes. In this article, a new solid waste-based slagging flux was applied in the external dephosphorisation test of low silicon hot metal to replace the traditional fluorite-lime dephosphorisation slag. The results showed that under laboratory conditions, the new flux was utilised for smelting low-silicon molten iron with a phosphorus content of 0.10~0.13 wt per cent, the temperature of the molten iron was 1350°C and the slag amount was controlled at 2 per cent. Within ten minutes of smelting, the dephosphorisation rate of the molten iron reached more than 50 per cent. When making slag with new flux, the slag melting time was about 40 seconds and the P2O5 content in the final slag was higher than 8 wt per cent. In the industrial test of a 150 t molten iron ladle, under the same molten iron conditions, an out-of-furnace dephosphorisation test was conducted in the Kambara Reactor (KR) process. The flux material consumption per ton of steel was 8~10 kg. Within 15 minutes of smelting, the dephosphorisation rate of the molten iron was about 30 per cent. During flux slag making, the slag melting time was about three minutes and the P2O5 content in the final slag was higher than 6 wt per cent. This new solid waste-based flux has significant cost advantages and outof- furnace dephosphorisation effects and is expected to become the benchmark for a new generation of fluorine-free steelmaking fluxes.

Currently all engineering schools face a common problem, an ever expanding technological base which must be presented to students within a fixed time period. Traditionally the programmes of study taken by engineering students destined for employment within the mineral industry include formal instruction in aspects of management to an extent greater than that presented in other engineering disciplines. Providing an adequate coverage of an expanding technological base within a fixed time span can lead to a reduction in the time available for 'non-technical' subjects, such as 'management'. This situation calls for periodic review of course contents and allocations of time. Following consultation with an Advisory Committee representing the mineral industry, mining research establishments, and kindred teaching institutions, the University of Melbourne has revised its mining programme. Prior to mine development, exploration drilling within the Luise Caldera on Niolam Island encountered gold mineralisation to the northwest of the main Minifie and Lienetz orebodies. This mineralisation was referred to as the Kapit orebody, and ten diamond drill holes were subsequently completed over the area to investigate its economic potential. Several significant mineralised intercepts were returned. These were interpreted using the orebody model developed for Lihir at that time which invoked a strong horizontal control to mineralisation. Additional consideration was given to the elevated geothermal temperatures encountered, the proximity of the mineralisation to the ocean, and its perceived depth below sea level, resulting in the Kapit area being given a low priority for follow up investigation. As a result, no further work was completed over Kapit until recently. The Kapit orebody was the target of an intense diamond drilling programme during 2002/2003. Data from this latest round of drilling has significantly improved understanding of the mineralisation and associated geology. The significant orebody now delineated at Kapit substantially differs from past interpretations, and the latest data has upgraded its economic potential. A resource estimate and other critical studies completed in 2003 enabled an updated reserve statement to be issued in February 2004 including the Kapit mineralisation for the first time. Total Probable Reserves at Kapit stand at 25.2 million tonnes (Mt) at 5.28 grams of gold per tonne (g Au/t), for a total of 4.3 million ounces (Moz) of contained gold.

Sampling drawpoints in block cave mines has been a contentious issue for many years. Doubts have been expressed as to the validity of the process used at most mines. This paper outlines the sampling practice commonly in use, discusses the resulting issues associated with reconciliation and drawpoint closure decisions. The paper also looks to the future and highlights issues for some block cave mining operations currently planned to have in the order of one thousand active drawpoints. It is likely that these operations will require a new approach to drawpoint sampling. There are several potential technologies currently available although some appear to be better suited than others to replace the current approaches. CITATION:Ross, I T, 2012. The long-term production schedule of open pit mines that maximises the net present value (NPV) of a given project relies on the proper design of phases (pushbacks) within the ultimate pit limits. There is not a phase design algorithm applied in the mining industry today that gives yearly production schedules that are optimum.The majority of phase design algorithms currently in use by the mining industry aims at grouping the blocks that fall within the ultimate pit limits into phases such that the average value of the blocks within each phase decrease as the size of the phases increase. Current phase design methods are typically based on a pit limit optimisation algorithm, originally introduced by Lerchs and Grossmann (LG) in 1965. To obtain multiple pit shells to be used for the phase design, the LG pit limit algorithm successively applied to economic block models by changing the price of the commodity or some other factor that properly parameterises the size of the pit. These modified economic block models are developed by predetermining the destination for each block in the model based on a breakeven analysis and highest economic outcome between alternative destinations. Phase designs determined by these existing algorithms may be suboptimal if a given project utilises multiple processing facilities for metal recovery and particularly if these facilities have restrictions (constraints) that require blending of attributes in the feed. To overcome these problems in obtaining phases that optimise overall production schedules, a new algorithm is developed at the Colorado School of Mines. The algorithm determines the phases of an open pit mine by taking into account multiple process options for the blocks and period by period blending requirements. This algorithm has been implemented within a software program (the software) and is being prototyped successfully in the design of improved pushbacks at a copper mine in South America and on a gold mine in Nevada. This presentation will demonstrate the shortcomings of the traditional phase design methodologies based on comparison of results coming from the new approach versus the traditional.
