The traditional measure of Lost Time Injury Frequency Rate (LTIFR) as a measure of improvement in safety performance is recognised as inadequate and misleading, lulling managers into believing that safety is improving. It is a reactive measure, and steadily declining LTIFRs around the world have failed to be accompanied by a similar reduction in the fatality rate. This paper confronts the industry with the attitudes of some mining managers, such as one that allowed fatalities to occur in the case of a gold mining company at a rate of one death per 150 000 ounces of gold produced while claiming their safety record has improved, as shown by a falling LTIFR, and viewing fatality reductions as a long-term goal instead of an immediate priority. A quantum shift in management attitudes is required to focus on proactive measurements to detect the indicators of an impending disaster, and to predict conditions conducive to fatalities. Corporate CEOs are challenged to attend the funerals of all workers killed in the workplace. CEOs who fail to attend such funerals send a message to employees that the CEOÆs priorities lie elsewhere, and that profits take priority over workersÆ lives. The dichotomy of whether mining companies are run for the benefit of shareholders or whether companies owe social responsibility to the community is examined. The role of individual responsibility for personal safety is examined, together with the psychology of employees in the mining industry. A number of proactive measurable indicators are suggested to stimulate discussion on developing robust measuring tools to predict and therefore prevent fatalities.

This paper will discuss the issues associated with blasting in an urban mining environment that have been addressed by Winstone Aggregates at the Mt Wellington Quarry. These issues include - control of vibration and air overpressure, repeatability of shot performance, fragmentation on an extremely low maximum instantaneous charge, compliance with Territorial Authority regulations and perception. Over the last two years, in order to address these issues, Winstone Aggregates have undertaken a programme of continuous improvement, which has included; ò implementation of new technology; ò revising site procedures; ò conducting an investigation into the structural response of local buildings; ò investigating safe extrapolation; ò extensive public consultation; ò establishing credibility during the consent process; ò identification of perception issues; and ò building relationships and trust with neighbours. The issues addressed at the Mt Wellington Quarry have increasing significance for traditionally rural mining operations, as the input of the local community being urban or rural, increases. New and existing operations have the potential to adopt best practice now, building trust with the community and raising the profile of the industry.

PACMANUS is an actively-forming seafloor analogue of volcanic-hosted massive sulfide orebodies, situated on the crest of an andesite-dacite-rhyodacite ridge overlying rifted older arc crust in a back-arc basin. Hydrothermal deposits range from Mn-oxide crusts to Cu-Zn-rich massive sulfide mounds and chimneys containing significant Au and Ag. Preliminary data suggest a substantial magmatic component to fluids and metal sources, and we speculate that PACMANUS is the cap to a complete hydrothermal system including subvolcanic and intrusive-related mineralisation.

Pilot-Plant Testing of Ammoniacal-Cyanidation for Gold Recovery from Copper Gold-Bearing Ore The results of pilot-plant tested ammoniacal-cyanidation of gold recovery from copper gold-bearing ore are presented in this paper. This ore is from a deposit of Russian Federation with gold and copper grade 9.5 g/t and 0.4 per cent, respectively. Copper is largely present as malachite and azurite. The presence of chalcopyrite and bornite is also observed. In order to recover gold, gravity is recommended. Two options of ammoniacal-cyanidation using leaching of gravity tails, CIP and filtration, were tested. It was found that ammoniacal-cyanidation using filtration of leaching tailings and recycled solutions is the most optimal option. The proposed flow sheet provides for 89.7 per cent gold recovery from gravity tailings at the consumption of NaCN 0.9 kg/t and (NH4)2CO3 7.5 - 9 kg/t. Compared to conventional cyanidation, this technology decreases the consumption of sodium cyanide from 4 up to 0.9 kg/t at the same gold recovery. It can be recommended for the treatment of copper gold-bearing ores with the similar composition.

The degree of regional denudation is one of the most important factors for judging the preservation potential of ore deposits as it can be used in a predictive manner to help guide exploration. There are many methods available to judge the denudation degrees of mineral deposits. These include regional geochemical assessment methods, the characterisation of primary lithogeochemical halos, the thermoelectricity of pyrite and apatite fission-track methods. Each method has its specific principles, application scopes, limitations and successful cases. This paper presents an overview of a program PC-BC developed for application to the design and evaluation of block cave operations. Conventional mine planning software tools are not well suited to the evaluation of this type of deposit. The paper looks at the modelling approach as applied to a block cave mining project. The following steps are described:Initial preparation of data including block models, draw point footprint geometry and draw cone geometry. Computation of individual draw point reserves. Procedure for modelling of vertical mixing of material within each draw point and subsequent determination of the best height of draw for each draw point based upon an economic cut-off value. Smoothing of draw heights amongst neighbouring draw points to produce a realistic draw envelope. Mineable reserves within the envelope are then computed. Tools for preparation and evaluation of various production schedules. This includes definition of draw rates, and rates for commissioning new draw points. In an operating mine, the capture and analysis of daily draw tonnages is important. How PC-BC accommodates this is described as well as the processes for depleting the draw column reserves. In the 12 years since the program was first developed, it has been used on a number of projects for feasibility studies and operating mines. Some practical examples of the use of the program in these projects are presented. Finally, some commentary on the sensitivity of various parameters is presented. Particular emphasis is placed on the overall footprint design.

The New Zealand Steel Ironsand Slurry Pipeline was successfully commissioned in February 1986. The project includes 18 km of high pressure, polyurethane lined buried pipe. McConnell Dowell Constructors Limited has developed a coupling which has allowed this pipeline to be fully welded. The design and development of the coupling and its application on this project is discussed.

Sensor-based ore sorting is being increasingly used to reduce the amount of low-grade and waste material processed in mineral concentrators. This type of preconcentration provides bottom-line benefits to users by reducing the amount of energy, water and consumables, as well as reducing capital cost. Existing operations can increase metal production, while previously uneconomic deposits and low-grade stockpiles can also be exploited. The technology can also be used to separate ore types for selective processing. The path to implementing sensor-based sorting may include:• geometallurgical evaluation• first inspection testing to investigate sensor response• bench-scale testing where sensor selection is not obvious or for difficult applications• performance testing in full scale sensor-based sorting machines• larger scale site-based piloting with a temporary semi-mobile plant installation. Sorting requires material to be suitably prepared and presented to the machines and typically this consists of crushing and screening to limit top size and optimise liberation. However, where material streams are suitably sized and prepared, additional equipment may not be required (e.g. the sorting of semi-autogenous grinding (SAG) Mill pebble streams).This paper presents a case study of economic upgrading of gold ore, by preconcentrating with sensor-based ore sorting. The case study examines sorting amenability, test work, the feasibility study through to implementation, with associated flow sheet development. The development process is analysed and evaluated with a view to rationalising the process for development of future projects. Mining schools around the world, many unfortunately now closed, were often established to teach the techniques of narrow vein mining and associated processing to students studying part-time while working in the industry. This arrangement ensured graduates (or æassociatesÆ) had a mix of skills and knowledge invaluable to the industry.It is argued that there is still a need to educate students in these narrow vein techniques and that there will be a growing demand for such graduates with a high level appreciation of this type of mining. There is also a need for innovation in narrow vein mining and processing to develop specific technologies and allow the use of techniques applied in æbulk miningÆ to achieve improvements in productivity, safety and economic performance.This paper aims to review the situation with regard to education and innovation in narrow vein mining.CITATION:Hall, S, 2012. With a price slowly rising above US$35 per ounce gold became a mineral worthy of prospecting and mining endeavour in the early to mid 1970s. Fortune therefore appeared to smile onNew Zealand with its favourable geological environment for gold and past record of major gold production from both hard rock and placer deposits. This was a particularly welcome development as the post war years, including both the base metal exploration boom of 1967/1971 (which produced major nickel, iron and bauxite mines in Australia) and the earlier period of uranium exploration failed to produce any mines in New Zealand with the exception of the small Tui Mine and iron sand mining. Whilst hard rock mining now appears imminent it is 15 years further along the track since the 1967/1971 exploration boom and eight years since the upsurge in prospecting for gold in 1978. New Zealand still has no hard rock base or precious metal mining. This is not for want of proven or potential gold mining situations for these clearly exist at Waihi, Monowai Reef, Golden Cross, MacRaes Flat and elsewhere.

"Gold ores often show significant inter-domain variation with respect to the gravity recoverable gold (GRG) content and the leach behaviour of the non-GRG component. Liberation assessment is extremely difficult for gold due to its malleability and the high sampling errors and poor test reproducibility resulting from gold’s high specific gravity and the low metal concentrations that occur in most gold ores. The use of microscopic techniques for the liberation assessment of gold is also impractical due to the high number of sampling errors and high cost of analysis.Curtin University’s Gold Technology Group has developed an online carbon-in-pulp/carbon-in-leach (CIP/CIL) simulation model called SIMCIL, which has been used since 1999 to model and simulate industrial leach-adsorption circuits. Recently the model has been upgraded to include an integrated liberation-leach modelling (ILLM) approach, which can be used to evaluate the impact of various process parameters (such as grind size, bleed to gravity etc) and circuit configurations on leach-adsorption circuit behaviour and overall plant gold recovery. A new time-efficient ore characterisation methodology has been researched and developed to provide the required liberation-leach data used in the ILLM approach, which is implemented online for the modelling of gold operations. Cannington mine is a silver lead zinc operation located in north west Queensland. The operation has been in production since 1997. Ore is primarily moved through an underground material handling system comprising of a crusher, conveyor and hoist. The underground crusher and conveyor system were sited in lower grade ore when the mine was first commissioned. As Cannington is now a mature operation, this ore is considered run of mine and stoping progressed into the crusher exclusion zone. As the stoping fronts advanced there was increased damage on the crusher chamber and infrastructure. In response to monitoring data, the crusher monitoring system was significantly upgraded in 2016. The monitoring system included rock mass and infrastructure instrumentation. It improved the understanding of stress impacts and mechanisms driving the damage within the chamber. It allowed the site geotechnical team to respond to changes utilising ground support, mining sequences and increased levels of monitoring. As stoping and damage to the rock mass continued, so did the process of responding to monitoring data. Ultimately, the monitoring system enabled a safe and controlled closure of the chamber. Working in Australia’s harsh climate can have a significant impact on the health, safety and productivity of our teams, but is our working environment expected to be more prone to heat stress in the future? Climate modelling suggests that we are likely to encounter more frequent heatwaves and higher apparent temperature. Does this mean that our teams will be at greater risk of heat stress, and are we prepared to deal with the changes in our future climate and the potential impacts on our working population? This paper will review the recent research on climate and projected changes to all meteorological parameters that are important in understanding the likelihood of heat stress. Blasting is an essentially violent process aimed to disrupt and loosen rock for productive excavation and downstream processing. Dilution of the orebody to some extent is the inevitable result, particularly in narrow vein gold mines. A small change in gold recovery is usually a strong lever on revenue generation. As a result blast designs are carefully engineered to both minimise and control the direction of rock movement. Traditionally blast designs in gold mines have used the minimum powder factor that will still permit rock excavation. This now conflicts with the recent industry trend to optimise the total process flowsheet from the mine to the mill. Numerous mines have achieved success in reducing total mining cost by increasing powder factors. Not only is explosive energy cheaper than mechanical comminution energy requirements, the increased blasting effect may deliver benefits across all downstream processes of digging, hauling, crushing and milling. Kalgoorlie Consolidated Gold Mines (KCGM) Fimiston operation is the largest gold mine in Australia, located in the heart of the Kalgoorlie Goldfields. Blast dilution has been reduced at KCGM by managing location of blast boundaries and by firing along strike to choked faces. Significant economies of scale have resulted from application of a bulk mining methods. The bench height of 10 m is the largest in the Western Australian Goldfields, and is typically mined in a single pass with large face shovels. Recent increases in production rates in both the mine and mill has been partly attributed to a 30 per cent increase in powder factor. KCGM then examined the benefit of a further 18 per cent increase in powder factor to 1.0 kg/BCM. A concern though was the impact of very high powder factors resulting in excessive blast movement and subsequently ore dilution. This was the subject of a project conducted with Orica Explosives. OricaÆsDMC(Distinct Motion Code) blast model was applied to understand the effect of not just further increases in powder factor but also blast geometry and explosive type. The modeling work demonstrated that high powder factors may be applied with minimal increase in ore dilution provided the combination of blast geometry and explosive type is optimised. These results are currently being implemented to achieve further reductions in total mining costs without a significant impact on recovery.

Mining deep orebodies is increasingly becoming the norm as the ‘easy’ orebodies are depleted. Deep ore mining requires significant ventilation and cooling infrastructure to not only meet regulatory requirements for air quality but also to achieve acceptable workplace temperatures with the higher strata rock temperatures and increased auto-compression. At the same time, advances in battery equipment technology are turning the possibility of a battery powered fleet into reality. The switch from a diesel fleet to a battery powered fleet offers several positive opportunities for the design and operation of a deep mine.The potential to reduce ventilation and cooling requirements due to battery powered equipment results in significant cost savings for mine development and infrastructure. Battery powered fleets are not subject to the same dilution requirements associated with diesel fleets thus allowing lower ventilation rates. At the low ventilation volumes possible with a battery powered fleet, surface refrigeration plants are limited with respect to the amount of cooling that can be delivered to the workface. Considerations for calculating the optimum air flow and cooling requirements are presented. Considerations for locating the refrigeration plant are also discussed. The 115-year history of the Hall-Heroult process for production of aluminium has been characterised by a sustained technology-driven improvement in costs and productivity. In the past decade the industry has effectively transferred best practice into older technology with improvements through computer control of the process, and redesign of cell linings using computer modelling . Incremental development is now at the point where diminishing returns will apply for further technical improvement effort however, until the gap between technical limits and current best practice is addressed through major process redesign. The process remains a long way short of its technical limits in energy consumption, carbon anode consumption, cell life and product purity. Investment cost in new capacity is very high, supporting the retention of older and less efficient technology and hence limiting the application of best practice in environmental performance and occupational health exposure. The industry is a major emitter of greenhouse gases yet the process has the potential to release oxygen as the primary waste. All of the above issues will be addressed in the new century, but only through commitment to the necessary long-term research and development that is required to achieve a redesign of the process.

Large-scale infrastructure construction needs to comprehensively manage risks that can impact the health and well-being of many people, from workers (temporary, contracted or full time), to families and surrounding communities. This paper starts with an analysis of incidents (injury, fatality, other damage) in tunnel construction between 2000 and 2014 and assesses the distribution of these events by continent, year, and severity of outcomes. These incidents are then analysed and mapped to a framework of hazards derived from RISKGATE (2014). A comparative register of current tunnel construction risk management guidance is derived from current Australian codes of practice and this register is also mapped to the RISKGATE framework of hazards to identify where coal industry knowledge may offer utility to the tunnelling industry.An initiative of the Australian Coal Association Research Program (ACARP), RISKGATE is an interactive online risk management system developed to assist the mining industry in implementing continual improvement in the management of major unwanted events. To date, a comprehensive knowledge base has been captured for risk management of tyres, collisions, fires, isolation, strata underground, ground control open cut, explosions, explosives, outbursts, coal bumps and bursts, human/machine interface, manual tasks, slips/trips/falls, inrush and occupational hygiene. By capturing operational knowledge from industry experts, RISKGATE provides a cumulative corporate memory at a time of high personnel turnover in the coal industry.This paper proposes and outlines how knowledge within the RISKGATE platform could be applied to managing the significant health and safety risks associated with tunnelling. The authors suggest that to varying degrees the knowledge already captured in RISKGATE is likely to be transferable to tunnelling construction operations. Questionnaires were sent to 12 underground metalliferous mines in Queensland to enquire how these mines control diesel emissions from mobile diesel engines used underground. Where responses or controls were absent, discussion has been provided. A significant finding concerns secondary ventilation design rates. Some mines were designing ventilation rates to meet the superseded Mines Regulation Act of 1989 which required 0.04 m3/s/kW. The current Queensland Mining and Quarrying Safety and Health Regulation (2001) is risk based. In line with other jurisdictions, including New South Wales (NSW) and most Provinces in Canada, a minimum ventilation rate of 0.06 m3/s/kW is acceptable. However, this design ventilation rate will vary in practice depending on the diesel engines and controls. Dedicated ventilation engineers and technicians should be appointed to evaluate the effectiveness of the ventilation system. Personal exposure monitoring is required to confirm that exposure standards are not being exceeded. The primary control practice should be to reduce the emission at source. This can be ensured by purchasing low emission (Tier 2, Tier 3) equipment. In addition, the use of ultra low sulfur fuel (et al, 2002). Diesel fuel with a low aromatic content such as Eromanga diesel fuel will also mean employees are exposed to lower levels of poly-cyclic aromatic hydrocarbons. These controls should be carried out in parallel with good maintenance practices. Preventative maintenance should be programmed and emissions be routinely tested, including both gaseous and particulate pollutants and compared against defined criteria. According to McGinn, 2002, there are a number of fundamental tools necessary to carry out preventative maintenance. These are outlined in maintenance guidelines and best practices for diesel engines in underground mining produced for the Diesel Emissions Evaluation Program (DEEP). An effective diesel emission management system needs to be fully integrated within the Safety and Health Management System. These requirements are further discussed in this paper.
