Incident investigation is of utmost importance in most high-risk industries and is often used to improve organisational safety. This study was undertaken to examine the content of past incident investigation reports to determine the effectiveness of the incident investigations. The study uses a semi-quantitative method to assess the effectiveness of incident investigations in the Ghanaian mining industry by evaluating the quality of past investigation reports. The assessment tool consists of five elements with several indicators and rating scales for assessing the quality of an investigation report as a measure of the effectiveness of the investigation. The method was applied to 304 investigation reports of three Ghanaian large-scale gold mines, and the results correlated with incidence rates of the mines to determine if any relationship existed. The results showed that the mines differ significantly in the quality of their investigation reports, suggesting differences in the effectiveness of their investigations. In addition, the incidence rates of the mines negatively correlated with some elements of the assessment tool. In general, the method was found useful and revealed areas where improvement is needed.

The Piedras Verdes copper mine is a new run-of-mine (ROM), heap leach, solvent-extraction/electrowinning (SX-EW) operation located 21 km (13 miles) north-northwest of the town of Alamos in the state of Sonora, Mexico. The Piedras Verdes copper mine is wholly owned by Frontera Copper Corp. (FCC), a Canadian corporation, through its 100 percent beneficial interest in Cobre del Mayo S.A. de C.V. (CDM), a Mexican registered company. The January 2008, minable reserves were estimated to be 193.6 Mt (213 million st) at a grade of 0.36 percent copper. The overall recovery of total copper was estimated to be 61.9 percent. The property has an additional 17-year life and is designed to produce 31.7 kt/a (70 million lbs/year) of cathode copper. Construction commenced in early October 2005. The first copper cathode was produced 12 months later at a total capital cost of $100 million. Elevations at the site range from 150 to 300 m (490 to 985 ft) above sea level. The area has excellent infra-structure in the form of towns, power, water, roads, rail, airfield and access to the deepwater port of Guaymas (Fig. 1). The surrounding area is also famous for its numerous polymetallic (lead, zinc, copper, silver and gold) epithermal veins. Some of these were mined for more than 200 years and provided the wealth on which the town of Alamos was built. The original village of Piedras Verdes was located in the area of the proposed openpit mine and, consequently, it was necessary to construct a new townsite 6 km (3.7 miles) to the south before project development could commence. The new town site (Fig. 2), complete with power and water supplies, roads, sewer system, schools, clinic and recreational facilities, was completed and occupied by 49 families in September 2005. CDM received its remaining permits from the Mexican environmental authorities on Sept. 20, 2005. This enabled the company to proceed with full-scale development. CDM owns or controls 27 mining concessions covering the resource. Surface rights to the Piedras Verdes project were controlled by four Ejidos (communal agrarian and/or ranching societies) and five individual landowners. CDM has acquired all surface rights necessary for its mining operations and associated facilities either through outright purchase or long term lease agreements.

They are gone now. Gone like that long ago generation that gave them birth. The Homestake Mine, nestled in the northern Black Hills, closed in 2001 after 125 years, the longest run of any major American gold mine. The Homestake Mining Co., which guided its destiny for those many years, is gone too. It merged with Barrick Gold. The Hearst family, which guided the early destiny of both, has long since gone beyond its mining heritage and into the national and even international spotlight. The mine and the company passed from the scene with as little fanfare as heralded their birth, amid the Black Hills gold rush of the 1870s. They both grew into American mining legends, as did the town they sired and nurtured, Lead. In a real sense, the mine and the company symbolized one of the last links to a vanished world and a mining era.

During the last several years, implementation of trusses, in lieu of wood cribs, has been successful for supplemental tailgate support in the Pittsburgh seam. This paper reviews, the truss support system provided by the new high strength 0.6" tensioned cable truss, which is now also being used in cribless tailgates. Cable loading and roof deformation were monitored to evaluate roof and floor performance. The test results indicate that the cable truss system successfully controlled the roof and floor during the first and second panel extraction. This paper will describe the advantages of cable trusses and their load transfer mechanism as well as mine evaluation.

G.F. Learning The paper by R.V. Ramani and R.J. Sweigard is a wonderful description of the labyrinthine web that has been spun about the mining industry by energetic bureaucrats and politicians over the past 50 years. The remedy for the problem, however, is not more of the same, but less. That may be difficult for the industry to achieve, for it is not a technical solution but a political one. And the current fervor for more detailed planning at all levels of government and private enterprise has become deeply ingrained. The authors recommend the provision of more information about mining and mineral resources to "macro" (i.e., government) land use planners. They apparently overlook, however, the already strong tendency on the part of most government land use planners to consider themselves omniscient. Thus, giving them more information about the technical problems of mining will only make them want to get more and more involved in the "micro" (private, site specific) mine development and production plans of the individual mining firm. In fact, this has already happened at all levels of jurisdiction from municipal to federal government. Examples are legion. The most effective way to ameliorate the adverse impacts of government land use planning on existing and potential mining operations is to: (1) introduce greater flexibility in the definition of land use zones by local and state governments; (2) adopt realistic and relevant ambient environmental performance standards in governing relationships between mineral land uses and concurrent or subsequent nonmining land uses; (3) allow greater leeway for economic considerations in land use decisions in contrast to the explicit legalistic approach now in vogue; (4) recognize that all minerals are not the same and that sand and gravel mining should not be treated the same as underground metal mining, coal stripping, oil field production, or in situ leaching; and (5) eliminate the notion that mining operators should be responsible for determining in detail the use of land by subsequent owners of mined land. This last bit of conventional ethic really makes no more sense than requiring the builders of every shopping center or government office complex to provide detailed plans for the use of that land when its use for shopping or government is ended. Did the builder of Ebbetts Field plan for Brooklyn after the Dodgers went to Los Angeles? Should the developer of the Bingham Pit plan for suburban Salt Lake City after the copper mining goes to Chile? The nation's mining industry must address these questions before further bankrupting itself to provide more data to planners and spending thousands of dollars per acre to create land that when reclaimed is worth only a few hundred dollars per acre. Reply by R.V. Ramani and R.J. Sweigard We thank Mr. Leaming for his valuable contribution. His views on the problems of land use planning and mineral resources are most welcome additions to our paper. As the title indicates, our paper was more concerned with the impacts of land use planning on mineral resource conservation than with the details of the planning process. On the whole, his five recommendations would be helpful for mineral resource conservation. However, we would suggest that the argument he presents for his final recommendation does not address the differences between mining as a land use and commercial or institutional uses. We believe that this difference is the crux of the issue. We share Mr. Leaming's desire to ameliorate the adverse impacts of land use planning. Possibly the most detrimental impact is the loss of mineral resources. Any development, whether mineral or community, that does not give proper consideration to other resources can result in permanent loss or sterilization of resources. With proper planning, some of these losses can be avoided. As our paper indicated, one factor that limits the consideration of mineral resources, and ultimately leads to their sterilization, is the generally inadequate levels of resource characterization and understanding of the unique nature of mineral resources and mining operations. The last point raised by Mr. Leaming is also important. In terms of reclamation and land use planning in mining districts, we certainly do not advocate spending more than what the results are worth. The main thrust of the paper was to explore the avenues for conserving the mineral resources so that, at some appropriate time, the issue of mining and reclamation can still be addressed.

[ ] In November 2005, Essroc Cement, part of the Italcementi Group, commissioned a long overland conveyor at their Nazareth plant in Pennsylvania. This conveyor, tagged CV-105, was designed to transport 1000 tph of limestone from the quarry to the kiln, and 400 tph of hot clinker (at 300°F) at back from the kiln to a handling facility about halfway back. Conveyor CV-105 is 8969 ft (2733 m) long. It gains 152 ft (46 m) of elevation in the first 1570 ft (480 m), then is mostly flat for the remaining 7400 ft (2250 m) (Figure 1). After the limestone is discharged at the head pulley, hot clinker is loaded onto the return side of the belt. This clinker can be discharged at one of two locations.

Heat and humidity buildup within refuge alternatives (RAs) may expose occupants to physiological hazards such as heat stress. The Mine Safety and Health Administration (MSHA) regulations require RAs in underground coal mines to provide a life-sustaining environment for miners trapped underground when escape is impossible. RAs are required to sustain life for 96 h while maintaining an apparent temperature (AT) below 95 °F (35 °C). The National Institute for Occupational Safety and Health (NIOSH) tested a 10-person tent-type RA, a 23-person tent-type RA, and a 6-person metal-type RA in its underground coal mine facilities to investigate the thermal environment over a 96-h period. The test results showed that mine air and mine strata temperatures surrounding an RA occupied by simulated miners (SMs) increased over the 96-h test period. The test results suggest that RA manufacturers should consider this increase in temperatures when calculating and evaluating RA components during surface and laboratory tests. The findings can equip stakeholders with additional considerations for calculating the interior heat and humidity temperature profiles for occupied RAs not tested in situ

The use of Tunnel Boring Machines (TBMs) for tunnel construction of long tunnels in the Andes of South America has been of mixed success in the past due to a variety of reasons and key lessons can be recognized. A review of past TBM tunnel projects in the Andes has unveiled some interesting findings for different TBM tunnel approaches with both world records of progress as well as nearly in-completed projects with machines being abandoned. Key considerations for the use of TBMs for tunnel construction in the Andes include geological issues; (faults, rock type, rock strength, rock abrasivity, durability, groundwater inflows),depth of cover and the potential for overstressing/rockbursts, site access and terrain, portal locations, intermediate access possibilities, minimum tunnel size, support requirements, contractor and labour experience, and project schedule demands. TBMs offer highly recognized schedule benefits(high progress, no concrete lining) for a project if intermediate access adits are limited as well as hydraulic benefits for water conveyance projects. Minimum specifications for TBMs can be easily developed to suit the unique requirements of any project so as long as all factors are carefully considered. The overall conditions and factors associated with some projects may suggest that it is not appropriate to use TBMs due to a perceived high risk. Finally, access for geotechnical investigations by drilling in the commonly steep terrain of the Andes is typically difficult and often not impossible. Alternative approaches for geotechnical investigations for TBM tunnels including comprehensive mapping, high-resolution airborne electric-magnetics for delineation of possible fault zones, and rock block sampling is presented and discussed.

FCX is also a member of the Wildlife Habitat Council (WHC), a nonprofit coalition dedicated to protecting and enhancing wildl ife habitat. In alignment with ICMM SD Principle 7 and our Environmental Policy, FCX has set a performance goal to prepare a biodiversity inventory in 2010 and land management plan by end of 2011 for all active mining sites. In 2010 FCX established a Biodiversity Task Force to assist sites in biodiversity management; optimize use of resources around the globe; implement FCX goals and targets; and align biodiversity programs with ICMM principles and guidance. The task force consists of Environment and SD Department management, scientists and engineers from FCX sites, and WHC staff. The Task Force recently issued a biodiversity inventory guidance document to set expectations and provide consistency across the sites when developing a biodiversity inventory and land management plan.

Since Sept. 11, 2001, the author has been asked by several mining companies and mining publications to share some of his insights on how the terrorist attacks on the World Trade Center (WTC) and Pentagon might affect the mining industry globally. Until now, the author has been reluctant to express his thoughts in a publication that could possibly fall into the hands of terrorists and give them some ideas that they may not have been previously considering. However, the author discussed the contents of this article with several friends and acquaintances in the international intelligence community. They said that nothing commented on here is new to the terrorists. Potential terrorist targets Sources within the worldwide intelligence gathering community have told the author that current intelligence data indicates that it is likely that the Islamic fundamentalist groups responsible for the attacks on Sept. 11, 2001will likely be directing their effort at foreign business entities. Any overseas or any domestic businesses of highly visible companies that are part of the anti-terrorism alliance should consider themselves a potential target.

This paper summarizes the main achievements of our previous project entitled "Development of a technological system producing quality guaranteed fly-ash based binder". Furthermore, this study deals with the laboratory experience of lignite fly ash utilization for two different goals. On the one hand the fly ash was tested as a binder; on the other hand it was investigated as an underground mine backfill material. Considering the first goal, the utilization of a mixture of mechanically activated fly ash and lime for producing an independent binding composition has been assessed involving determination of relationship between the degree of activation of fly ash and the strength of the obtained concrete. The results confirmed that this hydraulic binder is suitable for construction of stabilization and capping layers or base layer. Regarding the backfilling application, this paper presents laboratory experiments, their results including settling tests, hydraulic permeability and strength measurements and dense slurry technology related results.

In response to recent mining disasters and new mine rescue team legislation, NIOSH researchers conducted meetings across the United States with mine emergency response experts to investigate current needs and issues. Some of the issues include emergency response preparedness, mine rescue contests, real-life training capabilities, and training facilities. Many new teams are being formed that must be trained for mine rescue team competitions as well as be ready to respond to a variety of mine emergencies including a fire or explosion, a massive roof collapse, mine inundations, or vertical shaft rescue situations. Therefore, it is important that these teams are adequately prepared and trained. This paper presents a summary of domestic and international coal mine rescue training facilities and identifies those that provide unique, real-life, and/or state-of-the-art training. Research findings from this report will be used in further NIOSH investigations to improve coal mine rescue training. "The Ninetyeast Ridge (NER) extends roughly N-S as a topographic feature for 5000 km, from 31° south to 10° north (Fig. 1) where it becomes buried beneath the Bengal Fan at its northern end. These two geographic characteristics make the NER an ideal natural laboratory for determining the influence of latitude and terrestrial input to ferromanganese (Fe-Mn) crust formation and composition. In addition to study of geographic controls on crust composition, the NER is the largest feature in the Indian Ocean that may host crusts of economic interest.Crusts were collected from 9 of 33 dredge sites from 1.4° north to 26° south latitudes during the 2007 R.V. Roger Revelle Knox06 cruise. Nineteen samples were analyzed for 69 elements each. Nine bulk crusts and 10 layers from four crusts were analyzed. Three of the crusts sub-sampled for layers did not have the bulk crust analyzed and the weighted means for those layers were calculated to obtain bulk compositions. One sample was left out of the statistical analyses because it is phosphatized and chemically quite different from the other samples (Table 1).In order to determine changes in composition with latitude, Pearson product-moment correlation coefficients were calculated for the chemical dataset and latitude of dredge sites, as well as water depth. At the 95-99% confidence level, the following elements increase in crusts moving north along the NER: Mg, Ti, S, Cd, Ni, Tl, Mn, Ba, Co, Cu, In, Li, and Zn; and the following elements decrease northward: Fe, Fe/Mn, P, As, Sc, the rare earth elements plus yttrium (REY), Bi, Ge, and Pb. These correlations indicate that the iron oxyhydroxide component and its associated elements are more prevalent to the south, and the manganese-oxide phase and associated elements increase toward the north end of the NER. This change occurs between dredges D20 and D14 (Fig. 1), with crusts from D20 and south having Fe/Mn !1.0 and those north of D20 ""1.2, with the exception of the phosphatized crust. Crust composition also changes with water depth: Th, Ge, Pb, and the middle rare earth elements (REEs) increase with increasing water depth. Thorium commonly increases in deeper water crusts, which explains the higher Th concentrations in Atlantic and Indian ocean crusts in general compared with Pacific crusts (Fig. 2 and Hein et al., 2012), including the samples analyzed here with a mean water depth of 3584 m. However, water depth does not explain the latitudinal changes because the mean water depths of samples along the northern and southern segments of the NER are roughly comparable. Surprisingly, Si and Al, representing the aluminosilicate/detrital phase, do not correlate with latitude; that is as the ridge approaches and is buried by the Bengal Fan the Si and Al contents do not increase. However, overall, Si and Al have higher concentrations in NER crusts than in central Pacific crusts. Ba can be used as a proxy for primary productivity and statistically does increase to the north along the ridge in the direction of the equatorial zone of high biological productivity. Correlations of Mg, Li, and Zn with latitude may also reflect increased primary productivity at lower latitudes."

This paper presents and discusses some of the computer models developed and currently used at the Earth Mechanics Institute (EMI) of the Colorado School of Mines (CSM). These include the performance models for full and partial face machines. Examples of input and output of these models will be illustrated for typical mining operations by using machines such as Tunnel Boring Machines (TBM), Continuous Miners (CM), Marietta borer miners and longwall drum shearers. The general approach to modeling and the available models are discussed and some examples of these models are presented. The comparison between the estimated and achieved performance, as well as the degree of improvement in the performance due to balancing and modification in cutterhead design are also discussed.

The Biwabik Iron Formation is a ~1.9 billion year-old sequence of iron-rich sedimentary rocks that was metamorphosed at its eastern- most extent by ~1.1 billion year-old intrusions of the Duluth Complex. The metamorphic recrystallization of iron-formation locally produced iron-rich amphiboles and other fibrous iron-silicate minerals. The presence of these minerals in iron-formation along the eastern part of what is known as the Mesabi Iron Range, and their potential liberation by iron mining has raised environmental health concerns. We describe here the geologic setting and mineralogic composition of the Biwabik Iron Formation in and adjacent to the contact metamorphic aureole of the Duluth Complex. The effects of metamorphism are most pronounced within a few kilometers of the contact, and decrease progressively away from it. The contact aureole has been divided into four metamorphic zones?each characterized by the composition and crystal structure of the metamorphic minerals it contains. The recrystallization of iron-formation to iron-rich amphibole minerals (grunerite and cummingtonite) and iron-pyroxene minerals (hedenbergite and ferrohypersthene) is best developed in zones that are most proximal to the Duluth Complex contact. ©2007 Elsevier Inc. Bauxite residue (red mud) is an alkaline waste derived from the industrial process for extraction of alumina from bauxite ore. Some bauxite residues contain considerable concentrations of critical metals, for example, rare earth elements. Lithium in red mud was also reported as a potential resource. In the current study, a Li-rich red mud was leached using acetic acid to investigate the leaching behavior of lithium followed by neutralizing with hydrochloric or oxalic acid. By neutralizing red mud using diluted hydrochloric acid (0.01 mol/L), 58.04–60.27% of lithium was leached using 25% acetic acid at 95 °C for 60 min. In the meantime, more than 95% of sodium and 85% of calcium were also dissolved in the leachate solution. While oxalic acid (0.5 mol/L) was used to neutralize red mud prior to acetic acid leaching, lithium performed lower leaching efficiencies (42.41–46.88%) in comparison of using of hydrochloric acid–neutralized red mud. Nevertheless, these leaching efficiencies of lithium were close to that of calcium and much higher than those of sodium under the condition. In this study, 25% (v/v) of acetic acid, at 85 °C for 60 min, was demonstrated as optimum conditions for lithium extraction from oxalic acid–neutralized red mud using acetic acid. The results would be useful and of interest for lithium recovery and purification from red mud.

Face and pillar bursts, bumps and bounces are violent failures that occur in underground coal mines in response to a complicated interplay of face and pillar geometry, seam depth, coal properties and interactions between roof, seam and floor strata. Additional complications arise from the presence of gas, mainly methane, and associated pressure and flow that vary with time and are further influenced by the rate of face advance. Where gas plays a dominant role, the term outburst is often used; where stress concentration is primary, terms such as burst, bump or bounce may be used. Hargraves [1983] presents an often cited review of outburst phenomena with emphasis on Australian conditions. Beamish and Crosdale [1998] indicate outbursts have occurred for over a hundred years and present tabulations of outburst numbers in major coal basins throughout the world. A central consideration is the rate of depressurization that occurs in the course of mining. Outbursts are not limited to coal seams and adjacent strata; they also occur in salt and potash mines [Molinda et al 1988]. Stress concentration and gas pressure act in consort, so the combined action is important to understanding the mechanics of bursts or outbursts and to the development of measures to reduce the associated hazard. Gas flow is a complicated phenomenon associated with diffusion of gas from micro-pores and the flow of gas along cleats. Laboratory measurements indicate both are dependent on stress and pressure [Harpalani and Ouyang 1998]. Moreover, gas permeability in porous media is often many times water permeability [Klinkenberg 1941]. However, there is also evidence that gas and water permeability are similar at elevated stress and follow Darcy?s law [Dabous et al]. Gas flow is also of importance to mine ventilation. Price et al [1973] in cooperation with personnel of the former U.S. Bureau of Mines developed finite difference models to estimate methane emission in mines in the Pocahontas No. 3 and Pittsburg seams using a premining pressure of 4.69 MPa (680 psi) and Darcy's law implying laminar flow. However, the effects of stress were not taken into account. In this regard, mine measurements of permeability are usually based on Darcy?s law [Peide 1990]. Early models of gas flow with stress effects were one-dimensional in space and time (x,t) and were developed with the goal of estimating the gas pressure profile from the face into the seam as time passed [Schlanger and Paterson 1987, Zou and Yu, 1999]. A zone of high permeability adjacent to the mining face was recognized in these early models. Later models based on the popular finite element method allowed two spatial dimensions (x,y) and variation in time (t) and for coal seam heterogeneity [Tang et al, 2002]. Zhu et al [2007] formulated a set of nonlinear equations involving a complicated stress-pressure-permeability relationship for application to coal seam gas flow and concluded the coupling was important but failure of coal should also be considered. Wold et al [2008] used a hybrid finite difference - finite element model interleaved with a reservoir simulation model to study outbursts based on extensive mine and laboratory measurements. An important feature of the modeling was the use of spatially variable material properties and Monte Carlo simulation to obtain quantitative estimates of outburst conditions and probabilities. Connell [2009] used the same reservoir simulation model as Wold but used the popular finite difference code FLAC3D for hydro-mechanical responses to evaluate two permeability relationships, one depending on porosity and the other on stress, and concluded that permeability changes with gas production were more complex than computed by either permeability model. Gadde [2010] in an integrated laboratory and field study of coal pillar strength explored the effect of spatial variability and concluded that design based on average strength was acceptable in some circumstances, although gas pressure was not an issue. Liu et al [2011] also applied a FLAC code to analyze two-dimensional gas flow and deformation of layered but homogeneous strata in a Chinese coal mine using a strain-softening model and concluded that damage and gas pressure were important to strata safety relative to the case without gas.
