This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. This Bureau of Mines publication provides information to assist in selecting a lixiviant (leach solution) for in situ uranium leaching. The cost, advantages, and disadvantages of lixiviants currently used and proposed are presented. Laboratory and field tests are described, and applications of geochemical models are discussed. Environmental, economic, and technical factors should all be considered. Satisfying environmental regulations on restoring groundwater quality is becoming an overriding factor, favoring sodium bicarbonate or dissolved carbon dioxide over ammonium carbonate. The cheapest lixiviant is dissolved carbon dioxide, but it is not effective in all deposits. Technical factors include clay swelling by sodium, acid consumption by calcite, and the low solubility of oxygen in shallow deposits. Laboratory leaching tests can provide useful data. However, they can be misleading if, for example, the ore is allowed to oxidize before testing or if distilled water instead of formation water is used for making solutions for permeability tests. Geochemical models presently are more useful for indicating trends in solubility than in reliably predicting concentrations.

Analyses of statistical data indicate that haulage accidents are one of the largest sources of injuries and fatalities in underground coal mines, and that the rate of accidents in transportation has been increasing during the past few years. These previous analyses were performed by correlating accident or fatality data at a rather high level of miner activity. In this present investigation, underground surveys were made by observing conditions and activities in some detail at a rather low level of activity in an attempt to understand factors contributing to hazards in the specific situations observed. The aim of the current study was to derive preventive measures that might obviate the hazardous conditions and unsafe acts observed; and, following this, to provide recommendations for additional research and development to reduce accident potential in underground transportation. The major project tasks are diagrammed together with the task outputs in figure 1-1. Initially, a preliminary study of ell underground haulage operations was performed in order to establish the kinds of data needed and to provide the rationale leading to the data requirement. At the same time, the various types of haulage equipment used for each operation were defined. A preliminary analysis of each type of haulage operation was performed in order to establish the sequence of tasks required to operate haulage equipment. These tasks were transferred directly onto observation forms, and were used in developing interview forms. In parallel with the task analysis, a review was made of accidents and hazards that might occur during each type of operation.

Gaseous and particulate exhaust emissions are characterized for a 1200 Bhp two-stroke cycle diesel, a 1000 Bhp four-stroke cycle pre-combustion chamber diesel, and a 900 Blip four-stroke cycle, direct injection diesel using steady-state modal testing and a versatile exhaust dilution-collection system. In addition, a 1.50 Bhp four-stroke cycle, pre-combustion chamber, turbocharged diesel was characterized for use in experimental water injection evaluations. Water injection downstream of the turbocharger compressor and upstream of the turbocharger exhaust turbine are evaluated for their particulate removal effectiveness on the small turbocharged diesel engine. The water droplets are inertially separated using cyclone separators designed for minimum pressure drop and outsize, lip to 50 percent of the total particulate mass generated was removed using a combination of both inlet and exhaust water injection techniques.

This study was conducted using field interview methods to determine the data and information requirements of the underground coal mining industry for improving mine ground control procedures in existing and future mines. The establishment of a central geologic and engineering information resource center with computer searchable abstract files that can locate and supply descriptive and case-study literature addressing specific mining problems is recommended. "Manganese ore from Metals Reserve stock piles at Deming, N. Mex., Cushman, Ark., and Philipsburg, Mont., were processed in the Bureau of Mines pilot plant at Boulder City, Nev., to determine their suitability for the production of electrolytic manganese. Manganese was produced successfully from all of the ores.The ores, crushed and ground to minus 33-mesh, were roasted in a Skinner furnace to reduce the manganese and were then leached with spent electrolyte, from the cells, containing sulfuric acid. Acid-leach extractions of 96 percent on Deming, 89 percent on Cushman, and 97.5 percent on Philipsburg ores were obtained. Neutralization of leaches with ammonia results in the highest manganese extraction; but Cushman and Deming leaches could not be neutralized with ammonia, because the amount required was more than enough to maintain the proper ammonium sulfate concentration. A combination of ammonia and calcine was used. However, ammonia was satisfactory for neutralization of Philipsburg leaches. Sulfuric acid needed for make-up was as follows: Deming, 0.380 pound per pound of manganese extracted when neutralized with ammonia and calcine; Cashman, 0.402 pound per pound of manganese extracted when neutralized with ammonia and calcine; Philipsburg, 0.594 round per pound of manganese extracted when neutralized with ammonia alone.All of the leach slurries settled well enough to be separated and washed in countercurrent thickeners.Impurities were precipitated with hydrogen sulfide and removed by filtration. Cushman ore solution contained only- small amounts of impurities and required 0.055 gram of hydrogen sulfide per liter of solution for purification. Deming and Philipsburg solutions contained large amounts of zinc and required 0.55 and 0.65 gram of hydrogen sulfide per liter of solution, respectively.Electrodeposition was satisfactory on all three ores and current efficiencies of 62 to 66 were obtained.A process for the production of electrolytic manganese has been developed by the Bureau of Mines, first on a laboratory scale4/ and then on a semicommercial pilot-plant scale.5/ At first the- pilot plant operated on ore from the Three Kids mine near Las Vegas, Nev., to determine the practical features of the process on ore of this type. Since completing the work on this ore, the pilot plant has been used for processing other domestic ores to obtain engineering data and other economic information regarding the amenability of such ores to the process."

This report summarizes the results of a one-year agreement for research on the electromagnetic environment in coal mines, which was conceived as the beginning of a long term cooperative program between ITS and the Bureau of Mines. The concepts on which it was originally based are a result of the report on mine rescue and survival by the Committee on Mine Rescue and Survival Techniques of the National Academy of Engineering, dated March 1970. This report drew particular attention to the need for improved communications during and following mine disasters. In particular, the outstanding problems identified were the location of, and communication with, trapped miners. Some tentative solutions, generally involving long-wave electromagnetic signals, were offered. Consideration of the problems of disaster location and communication soon leads to consideration of the communications techniques (or lack of them) for use under normal mining conditions. In particular, if a disaster communication and/or location system is to be readily available and operable under disaster conditions, it should also be usable during mining operations. In this way operators can become familiar enough with the system to operate it during disaster conditions, as well as provide a mechanism for regular testing.

To determine whether reflectors could substitute for the trip lights referred to in the Federal Mine Safety Code, the Bureau of Mines studied trip safety devices under simulated haulageway conditions. The noticeability of reflectors compared with trip lights and fluorescent panels was evaluated from distances of up to 850 feet with light source intensities of up to 1-6 foot-candles (ft c). Reflectors tested consisted of a reflective lens in a holder secured to a frame that can be attached to a trip. Reflectors were found to be suitable for use as a trip safety device. In fact, for the conditions in many underground haulageways some reflectors would be more effective than a permissible trip light.

A method for measuring leakage of vapors into a chemical-cartridge half-mask facepiece during simulated use is described. The respirator, fitted with high efficiency cartridges, is worn in a dichlorodifluoromethane (CC12F2)--air test mixture while a series of timed activities (facial movements, varied breathing, exercise, etc.) is performed. Complementing other tightness tests, this method is useful particularly for studying face fit and the effect of facial movements on leakage. The concentration of CC12F2 within the facepiece is monitored continuously to observe the immediate effect of physical activity on leakage. A composite sample is collected simultaneously in a plastic bag as a measure of average leakage during the 18-minute test period. Leakage, reported as percent, is calculated from the concentrations of CC12F2 within the facepiece and in the test atmosphere.

To further investigate the potential of precast concrete sets as a support medium for underground mines, the Bureau of Mines designed an eight-piece precast reinforced concrete drift set for a small opening. Twenty-two sets of three different member sizes were tested to destruction under two loading conditions. A discussion of the test results and typical failure patterns is presented.

The plan to develop a standard system of names and definitions of regions and districts in Alaska was borne of the need for a tool to use in statistical and economic studies of the mineral industry of the Territory. The purpose of this report is to present to the mineral industry, and to those individuals interested in the mineral or allied industries in Alaska, the system used by the Bureau of Mines in connection with its legal function of collecting and disseminating mineral statistics. It is hoped that the specific descriptions given will result in a more complete understanding of what is meant by the regional and district names that appear in publications of the Bureau of Mines relative to the mineral industry of the Territory. Problem of Standardization of Names and Definitions The question might be asked, does the naming and defining of regions and districts in Alaska present a problem relative to the establishment of a standard? Is it necessary to establish a standard?

Rock bolting has become a major method of coal mine roof support because of its effectiveness, economy, and convenience. The polymeric-resin-bonded bolt has generally proved to be more effective than the mechanically anchored bolt, but the higher cost of the resin bolt limits it to applications where the increased support is essential. The low-cost high-early-strength cement grout developed during this project will encourage more widespread use of fully grouted roof bolts provided practical installation techniques can be developed. Laboratory tests of four types were performed on various rapid-set and accelerated cement products to find a material with properties suitable for replacing resins. A proprietary gypsum cement was ultimately used with good success. A field test of the cement grout was made in an operating coal mine. An intersection was bolted in the standard pattern with cement-grouted bolts and instrumented. A nearby resin-grouted intersection was also instrumented and observations were carried out in both intersections for 2 months. No substantial difference was found between the support provided by the cement and the resin.

The rare-earth group are among the least familiar of the 92 known elements. Moreover, the relatively few people who have even heard of the rare earths ordinarily regard them merely as very rare elements having unusual names but doubtful commercial application. This typical attitude can be attributed partly to the fact that no commercial uses have been developed for most of the members of the group and partly to the fact that those which do enter into useful articles do so anonymously. Most people speak with a degree of familiarity of the "chromium" plating on their automobiles or household appliances; "antimony" ash trays and cigarette boxes are on sale in many department stores; and "tungsten" lamps and even "molybdenum" steels are advertised in popular magazines. However, the average user of a pocket cigarette lighter is not conscious that the important sparking element is a "flint" made from ferrocerium, and if he does chap on to inquire about the composition he is likely to be told that it is "sparking alloy", "misch metal", or 'pyrophoric alloy." Similarly, the wearer of spectacles made of "Crookes" or other tinted glass probably is not aware that the light softening effects are due to the presence of rare-earth oxides, or that the arc light on his street may owe its brilliance to cerium fluoride in the carbon electrode. Despite a variety of practical applications, however, most of the rare-earth metals still must be listed as scientific curiosities. In fact, little is known as to the properties which may make them useful products at some future date. Meanwhile it is important to note that Nature has been relatively generous in her distribution of the rare earths and that adequate supplies of most of them will be forthcoming when these elements have proved their worth as commercial products.

Two coal seams were selected and the conceptual designs were based on the geological and physical conditions typical of these seams. As a result of the screening process presented below, the two coal seams selected were the Lower Kittanning Seam in central Cambria County, Pennsylvania and the Pittsburgh Seam in northwestern West Virginia and southwestern Pennsylvania. SELECTION PROCESS FIRST SCREENING OF LONGWALL OPERATIONS The list of longwall operations prepared in Phase I and included in the Phase I report, was used as the basis for a first screening. From this list it was determined which longwall operations were not in the Appalachian coal fields and not in a coal seam of the thickness range of 42 to 72 inches. The first screening eliminated 30 of the listed operations, leaving 48.

"INTRODUCTION Although the presence of sulfur causes many petroleum’s to be classified as ""low-grade"" because of the difficulties encountered in manufacturing products to meet rigid specifications, the petroleum industry is finding it necessary to use such crude oils to satisfy present needs. As the demand for petroleum products increases, and available reserves diminish, this utilization of high-sulfur crude oils will be accentuated. Programs designed to supplement the supplies of fuel available from petroleum by the manufacture of synthetic fuel from natural gas, shale oil, and coal are in progress. Such fuels, especially shale oil and, less so, oil from the hydrogenation of coal, also contain sulfur compounds which must be effectively removed. Thus, the need for fundamental research on the sulfur compounds that may be expected in petroleum and allied products is becoming greater as the industry is forced to call upon all available resources to meet the demand for petroleum products. In recognition of this need, the Bureau of Mines has inaugurated a long-range program assigned to furnish fundamental data concerning the presence, characteristics, and effects of sulfur compounds in petroleum and its products."

Human beings are natural storytellers and story listeners. We learn very early that stories entertain us, but also give us the ?rules? about what is expected of us and how to make meaning of our experiences. Stories are integral to our lives, and have great power to change or influence how we think or how we react. They are an innate part of how we learn, and have obvious benefits to trainers or educators who are trying to impact what and how we learn. This paper and presentation discuss the social and cultural power of stories, and how they have been used to develop training for skilled blue-collar workers. Included will be examples as well as suggestions for trainers on where to find stories, what type of stories to pay attention to, and how to use them to improve occupational safety training.

Develop a user-friendly, personal computer-based numerical modeling program that performs structural analyses of underground tabular deposits, making these readily available to the mining industry. Background When ore is extracted from an underground mine, the overburden weight is redistributed to unmined areas, recompacted gob zones, and artificial structures such as packwalls and/or cribs. This redistribution of stresses continually changes throughout the mining cycle and can be especially complicated when multiple-seam mining is involved. Numerical mine structural analysis methods provide a way of calculating the redistribution of overburden stresses and the resulting loading for any given area of an underground mine, thus enabling engineers to design safer and more productive mines. In 1973 Crouch and Fairhurst developed a mine structural analysis computer program called MINSIM. This computer program was based on the three-dimensional displacement-discontinuity approach, a subvariation of the boundary-element method. In 1979 Sinha expanded MINSIM's capability to provide an analysis of parallel multiple seams. Sinha called his program MULSIM. During 1984 and 1985, the Bureau of Mines upgraded and completely restructured MULSIM into what is now called MULSIM/BM.
