The prevention of fatal risks is a key objective for the global resources industry, especially considering poor performance with regard to fatality rate reduction and the continued occurrence of mine disasters globally. However, the industry has a credible history, over several decades, of the development and deployment management systems to control potential Serious Injury, Fatal and Catastrophic (P-SIFC) risks. While these systems have had a significant impact on the thinking and approaches to the prevention of fatal risks, fatal accident rates have not shown the desired reduction. Recent fatal and serious injury events in Australia have led to the establishment of several enquiries to further identify improvements that could be considered. Globally, fatal accident rates remain stubbornly high or plateaued in many jurisdictions, and several catastrophic events have occurred, such as tailing dams disasters in Brazil and South Africa, and the recent coalmine disaster in Turkey. Apart from the apparent lack of progress, it is evident that the deployment of critical risk systems has introduced more bureaucracy, slower response systems and additional burdens on already overloaded middle and supervisory levels of management. Organisations are rarely able to effectively tap into the risk source of risk knowledge of the frontline worker and supervisor, for various reasons discussed in this paper. To establish a valid and valuable channel of critical risk information flow from the frontline employee, Debmarine Namibia instigated the development and deployment of a system called SafeSENTRY, where the focus is on the ‘dynamic identification of dynamic risks’. This paper outlines the system, the principles of the design, the range of risk metrics that can be derived from the observations of the frontline operators, and demonstrates how a range of posited shortcomings in critical risk management is addressed how features of the High Reliability Organization (HRO) have been developed.

Isotopes and hydrochemistry were used to define groundwater flow systems and better understand the hydrogeological setting of the Karijini National Park, which is in the Central Pilbara region adjacent to the Marandoo iron ore mine. Based on the stable isotope composition of the water samples, the data can be divided into two main groups. Groundwater is characterised by depleted dD and d18O, suggesting no significant evaporation effect. Surface water on the other hand is more enriched in dD and d18O due to evaporation. The relatively high concentration of Cl- compared to rainfall and depleted dD and d18O values of groundwater indicate that recharge of the aquifers is occurring during intense rainfall events when rapid infiltration occurs. Evapotranspiration then acts to concentrate ionic species prior to recharge. Relationships between the major ion concentration and catchment area, surficial Tertiary cover and distance between recharge and discharge were identified. The results show that these catchment variables combine to determine unique water chemistry for each sub catchment in Karijini National Park. The TDS concentration of the groundwater in the Marra Mamba Iron Formation that hosts the Marandoo ore body is higher than most of the water bodies surrounding the mining area suggesting significant modification or different recharge mechanisms to that of the Karijini groundwater.

The experience of Grey River Gold MiningLimited in pursuing these statutory approvals has identified weaknesses and strengths in the present system and problems which need to be resolved. The Company has learnt lessons, which if heeded, will assist future large scale alluvial mining projects.

The successful development of industrial minerals projects requires an integrated, holistic understanding of the metallurgical and product performance test work in the context of the geological setting of the deposit and applied to mining, processing and marketing of saleable products. The product specifications provide the essential starting point for geometallurgical studies. A sound understanding of the desired product is needed to design and interpret a program of test work. Examples are provided from recent studies on bentonite and graphite deposits.For bentonite projects, the naïve testing of only the cation exchange capacity (CEC) as a measure of purity can lead to unexpected outcomes when the deposit is mined. The CEC attribute may remain the same, while other important properties such as pH and swelling index change markedly with depth, due to weathering and oxidation, leading to disappointing production outcomes.For graphite projects it is not enough to report only total graphitic carbon content (TGC). Knowledge is required of the in situ flake size and textures in addition to purity and flake size distribution of extracted graphite, which may all vary from one domain to another across the deposit. Such geometallurgical attributes of the ore will impact on the ‘basket price’ used to estimate the value of marketable parcels of expected product.Disclosure of such information must be considered part of fully informing the market. As with all new project developments, consideration of the JORC (2012) and VALMIN (2015) Codes requires disclosure of all material information. The Old Man Range, south of Alexandra, in central Otago consists of a package of highly folded greenschist facies schist of the Wanaka lithologic association. The range is crosscut by several east to ENE-striking high angle gold-bearing quartz veins, (eg Whites Reef and Grays Reef), which were extensively worked in the late 1800s. The range is bounded on its east side by the Old Man Fault, which separates these rocks from generally flat lying and planar Caples Terrane schist. To the west of the Old Man Fault, about halfway up the range, a large subparallel, 100 m wide fault zone strikes NNW and appears to cut off the historic veins. The fault zone consists of highly sheared and folded rocks and numerous high angle cataclasites and centimetre-scale clay gouge zones. Hydrothermal alteration is not apparent and the individual faults and shears are clay altered, possibly due to weathering, without visible quartz veins. Gold, limonite pseudomorphs after pyrite and rutile are concentrated in some of the narrow cataclasites and gouge zones. These mineralised structures are texturally similar to the gold-bearing reefs and may be structurally related. It is postulated that the NNW-striking shear zone and ENE-striking veins may be a conjugate array formed during a period of Cretaceous normal faulting.

Copper ores are widespread through the Lachlan Geosyncline in New South Wales, but the size of the orebodies is generally small. The largest deposits occur in the Cobar district where an iron-rich copper ore type has been developed in Silurian sediments. Similar de- . posits to the south in the Nymagee-Mt. Hope-Melrose districts are related to quartz feldspar porphyries of probable Upper Silurian age. Deposits to the east in the Tottenham-Girilambone districts are less complex mineralogically and are possibly related to an ultra-basic source.Copper mineralization in the Highlands further to the east is often developed in Ordovician andesite or MiddleUpper Silurian acid volcanic sequences. The deposits in andesite contain considerably less galena than those in the acid extrusives; a small proportion of the mineralization in the latter has texture, mineralogy, and geologic setting analogous to the Captain's Flat copper-Iead-zinc deposit, for which both syngenetic and epigenetic origins have been proposed. Very little copper mineralization occurs within granitic material, even though rocks of this type ranging from Silurian to Carboniferous in age crop out through much of the geosyncline, and the extent of the metal contribution from plutons to the deposits in volcanics is open to question. Numerous deposits are known in the contact zone around Mid-Devonian and Mid-Carboniferous granites, however, and the formation of these orebodies has undoubtedly been influenced by plutonic activity. Small occurrences of copper with associated minor nickel sulphides are developed in a narrow serpentinite belt of possible Lower-Middle Devonian age in the southern section of the Highlands.

Gold Fields Australia is constructing two major ventilation shafts at the Wallaby gold mine in Western Australia. The new ventilation shafts are required to facilitate the long term ventilation strategy for the mine, thereby allowing continued extraction of ore beyond 1000 m below the surface. The construction work has involved all of the challenges that shaft construction can throw up, including challenging geotechnical conditions, unexpected high water inflows and major circulation loss during pilot hole drilling for raise boring. An extensive geotechnical and hydrological investigation program was undertaken prior to awarding the shaft construction contracts. While most of the risks were identified, several proved to be much more challenging than expected. The shaft construction methods included conventional sinking into the fresh rock profile in each shaft, followed by raise bore pilot hole drilling and reaming. A headframe was constructed for the first shaft, which was sunk to 92 m with a stage suspended in the shaft to facilitate the concrete lining and provide rope guides for the kibble winding. Drawing from experience when sinking the first shaft, additional ground pre-conditioning was implemented for the second shaft, which was pre-supported with continuous piles to 28 m followed by sinking to 50 m using a crane instead of winders and a headframe. Following the conventional sinking through the weathered zone in each shaft, the shafts were completed using a raise bore. The first shaft utilised a riser pipe and rotary vertical drilling system (RVDS) with the second shaft having utilised a dual rotary rig for pilot hole drilling. THOSE who dabble in mining are no exception to the general rule that persons who know a little of a subject are apt to think that they know all about it. Some colour may be lent to the belief that anyone may succeed in mining when we hear stories ofsuccessful diggers and others who have accidentally dropped on to something good through no foresight on their part; but we seldom hear the contra account of the far larger number who fail. Those who go more thoroughly into the subject, and take up mining as a profession, know very well that it is not one that can be carried out successfully by rule of thumb, but when they point this out to the public they are credited with trying to make it a closed profession; and outsiders are for ever trying to find out the "trade secrets," at the same time spurning the main one, which is hard study, experience, and the ability to apply their knowledge. The average mining man is not rich. When he amasses a fortune it is generally through speculation rather than professional work, thought no doubt his technical knowledge may have assisted...

The history of the industry which recovers rutile and zircon from the heavy mineral beach sand deposits of Eastern Australia began with the initial discovery of zircon in 1895. Whereas a large number of small companies operated during the rutile boom of 1955-57, the industry is now controlled by a few large groups.Mining is mainly by suction dredging. Metallurgical practice is a two-stage process in which a bulk concentrate of heavy minerals, recovered by gravity methods, is separated into its constituent minerals by processes based mainly on surface, electrical, and magnetic properties.Various separating devices using these principles have been developed in the industry and combined in various ways in the operating plants.There is scope for future metallurgical developments.INTRODUCTIONThe existence of deposits of heavy minerals ("black sands") on the northern coast of New South Wales has been known for at least 90 years. The earliest metallurgical operations aimed at the recovery of gold, platinum, tin and monazite. Mainly because of the low grade of these deposits with respect to these minerals, but also due to metallurgical difficulties, such operations were abandoned by about 1895 (Whitworth, 1959). The presence of zircon was recorded quite early (Carne, 1895) but Raggatt (1925) appears to have been the first to report the presence of rutile.The economic exploitation of the deposits was initiated in 1928 when D. H. Newland, State Geologist of New York, made an examination on behalf of The Titanium Alloy Manufacturing Company of America. A study of the crystalline materials on or near the surface of freshly made or "green" superphosphate granules and mature, "set" superphosphate granules was undertaken using X-ray diffraction, X-ray mapping, scanning electron microscopy and electron probe micro analysis techniques. Calcium sulphate (CaSO4), calcium sulphate hemihydrate (CaSO4 1/2H20), and mono calcium phosphate monohydrate (Ca(H2PO4)2H2O), were the only crystalline species present in the "green" superphosphate.In the set superphosphate however, additional compounds were present, namely complex iron and aluminium phosphates Ca(Fe,AI) H(HPO4hF2)2H2O, (Fe)3(H3O)H8 (PO4)6H2O and calcium sulphate dihydrate (CaSO4.2H2O).The complex phosphates tied up the major part of the Fe and Al in the system and were concentrated on or near the surface of the granule. An intimate mixture of CaSO4.2H2O, and CaSO4, along with the above mentioned complex Fe-AI compounds, crystallize close to the surface of the granule and cause the setting of superphosphate that takes place during storage.

OZ Minerals’ Carrapateena mine is a Sublevel cave mine located in South Australia, approximately 160 km north of the regional centre of Port Augusta. Carrapateena commenced underground development in 2016, with production commencing in 2019 and ramping up to 4.25 Mtpa by December 2020. In pursuit of continual improvement, the Carrapateena mining team sought to identify how it could better interrogate its current productivity levels and its drivers (root causes). During this process the team were confronted by a disconcerting number of different opinions (which often conflicted) on our actual productivity performance which led to a need for review. Although reliable data and a well-designed time utilisation model (TUM) existed, it was still unclear what actions needed to be put in place to improve productivity. This was for the following reasons: • Data sources were disparate unless significant effort was applied to compile them. • Data sources were not available in a format that could be easily viewed or analysed. • General understanding of the data which was collected and how it could be used was poor. • Process of regularly analysing performance and forming tangible actions to improve was lacking rigour. A project was then started with the goal to fix the areas stated above. An initial requirements gathering exercise was undertaken to identify key data sources for site which returned a staggering 42 known systems which were then shortlisted to the key systems that contained operational data and could generate tangible improvements when analysed. This paper discusses the approach to selecting the OZ Minerals Insights Hub as the chosen platform, and the effort and collaboration between cross-functional departments that was involved in transforming the data so it could be displayed in a useful way. The data sources identified included a fleet management system, two planning databases, a control system historian, and a mobile fleet machine data system. New processes were also introduced to digitally capture daily commentary; this was achieved utilising SharePoint Forms. Data pipelines were then developed in the Insights Hub platform to automate the ingestion and transformation of the raw mining data into curated data models for end user consumption. Tableau dashboard packages have been created to support facilitation of short interval control, daily review meetings, and weekly review meetings as well as data analysis of key equipment and locations. These dashboards visualise the mining data models in a form that provides the user a clear picture of what has occurred, why it has occurred, and where to focus improvement efforts. This has resulted in a team which is more accountable, more equipped to assess the root cause of performance, and more regularly able to make data driven decisions. The next step? To build this out from Mine to Mill and create a data indexing tool which will drive self-service reporting, a data agnostic platform and predictive tools that help our people continue to be one step in front.

The Phoenix mine on the Mararoa Reef at Norseman, Western Australia, provides an outstanding: AUstralian instance of the recurrence of payable ore beneath the bottom of an abandoned mine, and an interesting and instructive example of the development ofideas with regard to ore recurrence in a particular district. The Mararoa Reef is the most productive of several hypothermal goldquartz vains in pre-Cambrian basic volc81olics which strike northsouth and dip on the average, 45 degrees east. The Phoenix minewas abandoned at 500 ft. vertical depth after producing 342,000 tons of 9.9 dwt. recovery grade (equivalent to nearly half of the total production of the Norseman field) from a 2,500 ft.. long, horizontally disposed oreshoot, which is reported to have had an ore outcrop only 30 ft. in length.Interest in the field was revived in 1933, and by 1941 new ore mined and developed on the Mararoa Reef by Central Norseman Gold Corporation N.L. amounted to approximately 700,000 tons of 6.7 dwt. recovery grade. In the same period another oreshoot of similar size and grade was developed by Norseman Gold Mines N.L., south of the old Mararoa company's site. As shown by the plans, the ore occurs in discontinuous shoots. The intervening area of shear is poor in quartz as well as in gold. Geological work on behalf of Western Mining Corporation Ltd., and Central Norseman Gold Corporation N.L. has been in progress in the mine and in the field more or less continuously from 1913 to 1941 and again since 1946. This paper describes the successive advances in the understanding of the exploratory problem up to late 1941, to which several investigators have contributed. In stopes of the 2000- and 2150-ft. levels in the North mine section of North Broken Hill Limited, whence a substantial proportion of the company's production is being drawn, and especially in stopes which involve the portion known as the Northern orebody, occurs it light-coloured friable porous material which represents average grade ore from which the lead and zinc minerals, and certain gangue constituents, have been largely or wholly removed by leaching. The 2000- and 2150-ft. levels lie, respectively, 1550 and 1700 ft. below extreme base of the pre-mine oxidized zone. Composition of the cellular product shows it to be supergene. The evaluation of historical goldfields has been common over the past 15 years, reflecting changing economic conditions, improvements in knowledge and technology and lack of greenfield sites. Many goldfields comprise narrow gold-quartz veins, which are structurally complex with an irregular distribution of gold, have variable geometry and architecture and display late deformational effects. The study of historical mine records can be an important tool during early and advanced project evaluation, since it contributes to understanding the orebody, exploration target size and setting expectations. Mine records include documents such as plans, sections, reports, news cuttings, production tabulations, etc. A major advantage for the modern explorer is the capability of computer modelling to display this data in 3D. Historical records can give the explorer substantial information on various deposit characteristics, not least grade and geological continuity, geometry and architecture, mineralogy, metallurgical properties, bulk density and ground conditions. From an evaluation perspective, historical records can be used to support a Mineral Resource estimate, though its application will depend upon the quality and quantity of the data and judgement of the Competent Person. Effectively used, historical records can reduce geological uncertainty and focus evaluation efforts û thus reducing project risk.

Two types of Mt Tom Price hematite iron ore from Hamersley Iron P/L were tested using a Humboldt Wedag Jones P40 wet high intensity magnetic separator (WHIMS). The parameters tested were magnetic intensity (7000, 9000 and 11 000 Gauss), groove plate type (8R and 12R) and gap width (2.5 and 1.8 mm). It was found that at 9000 gauss magnetic intensity, reasonably high-grade and recovery in the concentrate were obtained. At 2.5 mm plate gap width, the 12R groove plate performed slightly better than the 8R groove plate. Further testwork is recommended to optimise the magnetic intensity, feed flowrate and pulp density, wash water pressure and flowrate with 12R and 8R groove plates at 2.5 mm gap width.

One of the chief drawbacks to Bendigo mining always has been the difficulty of draining the mines in an equitable manner, so that each company or mine owner bears the properly share. The trouble seems to have been apparent on this field since the diggings were discovered. One mine would push on more expeditiously than its neighbour in looking for the precious metal and sink its shaft, whilst those in the vicinity would simply potter about, and discontinue sinking until gold was struck in the deeper ground, and: then they would go ahead, start sinking, while the pioneer would have to pump all or most of their water. This sort of thing continued here for years until the leases were worked down to the very deep ground and the water became such a b.urden on those companies that had deep shafts, that first one stopped hauiing and then others; with the result that over 1000 feet of water was allowed to accumulate on the New Chum line. lt appeared that if something were not done the whole of the main line of reef in the Bendigo district would be at a standstill, when a scheme was formulated and handed to Mr. Lansell, who, with other gentlemen, took the matter up, and at the present time all the mines on that particular portion of reef are in full work and the water all bailed out.lt is this scheme that I am requested to place before you and also to give you the details of the work done since its adoption.

Anglo American Platinum has adopted a strategy of Best Practice Principles (BPP) for sampling, mass measurement, analysis and metal accounting for more than a decade now. When new plants are designed, the most suitable sampling equipment and sampling protocols are implemented and material flows and plant layouts are designed around sampling strategies. A few years ago, a crushing plant for a high-grade matte stream was commissioned.The design called for the high-grade material to be crushed to 95 per cent passing 2 mm and the design allowed for a single cutter primary Vezin sampler, increments taken from a gravity flowing stream at regular intervals, whilst filling a tanker for the pneumatic offload at our base metal refinery. This was a single point of sampling and analysis that would measure the wanted output from our smelting complex and the input to our refineries. In addition, the design incorporated the functionality to collect increments in an ‘interleaved’ fashion from the Vezin sampler for internal precision checks.The metal accounting strategy for Anglo Platinum is the ongoing measurement of all inputs, outputs and stock levels for all our operations for platinum group metal (PGM) and base metal (BM) content. Included are yearly stocktakes, usually done on 1 February every year, across the smelters and refineries (at least the base metal refinery). A trend was developing in the accounting period immediately following the introduction of the new crushing and sampling facility, whereby the absolute difference between the physical stock and theoretical stock (PGMs predominantly) increased for the two respective sites on an ongoing basis. The smelter (sender) showed an increase in physical stock compared to the theoretical stock and the base metal refinery (receiver) showed that their theoretical stock increased against their physical stock. Because of this anomaly, an additional stocktake was held in September for the period in question and it was confirmed through the physical stock measurement results, that the PGM accounting problem was indeed pointing towards the evaluation of the high-grade matte stream.This paper showcases parts of the process that took place to explain the poor accountabilities for the respective sites. Previous results from the sampler ratification process (of the interleaved sampling design) were analysed again, a physical inspection was carried out on the sampling system again and the sampler performance was monitored (quality control parameters) for the period.From these investigations, it was found that airflows present inside the Vezin introduced the sampling bias and a change in sampling protocol was introduced as well as physical changes to the sampling equipment were made to eliminate the airflow and thus the bias conditions significantly. The accounting period which followed, after a phased upgrading process, showed that the accounting between our smelter and base metal refinery had been restored to within statistical limits (difference between physical and theoretical stock below five per cent relative to theoretical stock).CITATION:Kruger, C J and le Roux, E, 2017.
