A common theme running through recently introduced mining health and safety legislation in many parts of the world is risk assessment. Most recently, risk assessment legislation has been introduced across the mining industry of South Africa and the view that the introduction of risk assessment based legislation provides an ideal opportunity to improve on past health and safety performance is presented. Risk assessment techniques range from highly complex safety system techniques that are used in high technology process industries, to subjective techniques based on simple judgements that are finding use in many other industries. The objective of this paper is to shed some light on risk assessment, not by detailing the risk assessment process but by identifying the crucial elements that need to be considered if risk assessments are to be successful in changing health and safety performance. A case study is presented illustrating the introduction of risk assessment into the Ingwe Coal Corporation, one of South Africa?s largest mining houses, to indicate how the system was developed and how it is being used.

The Bafokeng Rasimone Platinum Mine (BRPM) is a joint venture between Rustenburg Platinum Mines Limited and Royal Bafokeng Resources (Pty) Ltd (Bafokeng). The mining lease has been granted over Boschkoppie 104JQ and is located approximately 10 km south of the Pilanesberg Alkaline Complex. The Merensky Reef, which constitutes the principal orebody, is geologically complex. A relationship between its geological variability and the associated mineralization has been established by defining a number of facies types. The Merensky Reef has been subdivided into seven distinct facies types: the pegmatoidal facies, contact facies, FW1 potholefacies, FW3 pothole facies, FW6 pothole facies, and FW7 pothole facies. The Merensky Reef, which does not form part of the potholed reef, has a large component of footwall mineralization. The pothole facies, in contrast, has mineralization located in the hangingwall. Statistical analysis of the facies types indicates that they are unique populations and hence need to be evaluated independently. The mining lease has distinct structural domains that have added additional complications to evaluation of the Merensky Reef. The Merensky facies on BRPM exhibit unique geological and statistical characteristics within well-defined structural blocks. Due to the extreme variability of the Merensky Reef on BRPM a unique approach has been developed, within Anglo Platinum, to evaluate the deposit. Mining cuts are defined and composited for the individual facies types, and then modelled individually by applying known geological controls. The resultant grade models are weighted based on statistical and local geological knowledge. The object of weighting the facies is to replicate the variability observed in the orebody. The introduction of stringent geological controls to the evaluation process has resulted in increased confidence in the in situ grades estimated as an input to the mine planning process.

In surface mining operations, ultra-heavy trucks operate on haul road networks which are typically from 10?40 km in length. These road networks are often not optimally designed and maintained and as a result, identifying and remediating road functional defects is problematic. Most surface mine operators agree good roads are desirable, but find it difficult to translate this into an effective road maintenance system. This paper presents a haul road maintenance management system which utilizes an assessment of rolling resistance based on a functional performance evaluation of a haul road, together with mine haul truck operating cost models to derive the optimal frequency of road maintenance. A typical application is presented which illustrates the potential of the technique to manage maintenance as and where needed with resultant reduction in total road user costs and an improvement in service provided for the road user. Mintek was contracted by Oriel Resources plc in 2004 to conduct a preliminary assessment study on smelting ferronickel from the large but relatively low grade Shevchenko nickel ore deposit in Kazakhstan. A detailed feasibility study (?DFS?) was awarded to Bateman BV following promising results from the preliminary assessment study (?PAS?) and small-scale pilot plant test work. As part of the DFS, a total of 450metric tons of ore was collected from three different locations in the deposit and shipped to Mintek. After sampling, the bulk sample was prepared for smelting. This comprised crushing, screening and calcining with each step done separately for the three different lots. The chemical composition of the ore from the deposit is relatively high in silica and therefore falls out of the MgO to SiO2 range that is normally processed by pyrometallurgical means to produce ferronickel. The main objective of the testwork was to demonstrate that a method could be found and successfully applied to smelt these siliceous ores and produce an acceptable nickel-containing alloy. A 2-m ID DC arc furnace was installed with a twin-cathode (electrode) configuration to replicate the proposed commercial-scale furnace and also incorporated an advanced design of water-cooled copper panels in the lower furnace sidewalls to withstand the aggressive nature of the high SiO2-containing slags. The reductant used was a high-volatile coal from Karaganda in Kazakhstan. The reductant addition was adjusted to achieve a 90% recovery of nickel. In the assessment of the process metallurgy, some deviation from equilibrium conditions was observed. This deviation is ascribed to the intermittent existence of a smelting bath partially covered with feed during the testwork. Optimization of smelting conditions should result in the metallurgy (slag metal equilibrium) being closer to the theoretical predictions and make the production of a 20% nickel alloy possible at 90% nickel recovery. The solution for the successful processing of these siliceous ores was found in the attainment of the necessary operational stability through the control of arc length, operating temperature, open-bath smelting conditions and flux additions of up to 10% dolime for the most siliceous of the three ore types. The test work carried out by Mintek provided the basis for the process flow sheet and engineering design of the DC arc smelting furnaces for the Shevchenko Nickel project.

Computerized thermodynamic databases for solid and liquid metal, slag and solid oxide phases in the Al2O3-CaO-CoO-CrO-Cr2O3-FeO-Fe2O3-MgO-MnO-NiO-SiO2-S system have been developed by critical evaluation/optimization of all available phase equilibrium and thermodynamic data. The databases contain parameters of models specifically developed for molten slags, liquid steel and solid oxide solutions such as spinels, pyroxenes, olivine, monoxide (wustite, periclase, lime), corundum, etc. By means of the optimization process, model parameters are found which reproduce all thermodynamic and phase equilibrium data within experimental error limits. Furthermore, the models permit extrapolation into regions of temperature and composition where data are not available. The databases are automatically accessed by user-friendly software that calculates complex equilibria involving simultaneously slag, metal, refractories and gases, for systems with many components, over wide ranges of temperature, oxygen potential and pressure. A short review of the available databases is presented. The critical evaluation/optimization procedure is outlined using the Al2O3-CaO-FeO-Fe2O3-SiO2and Al2O3-CaO-MnO-SiO2systemsas examples. Several applications of the databases to deoxidation of iron and to inclusion control in steel are discussed. Pipe pigging is a technique for the in situ (field) coating application, to create a seamless internal anti-corrosive lining of long pipelines. The technology originated in the cleaning of pipes, and these proven cleaning methods continue to be used today. Firstly mechanical cleaning of the pipeline is performed, using wire brush, razor back, course stud and fine wire pigs, then foam pigs, or criss cross pigs, and urethane blade scrapers to remove loose debris, wax, residual hydrocarbons and corrosion products from the pipeline. Part of the proprietary techniques and expertise is to ensure that these cleaning pigs do not get stuck Next chemical cleaning of the pipeline is performed, using repeated pig runs with detergents, solvents, acid etching, corrosion inhibitors, consecutively batched between two pigs, to ensure a clean, dry metal finish to a visual cleanliness standard of ISO SA2½. These in situ techniques were developed, 40 years ago to apply internal corrosion protection to pipes using a thin layer of epoxy based paint (50 to 100 micron). Until recently these old techniques were never improved upon. Although many thousands of kilometres of pipes were successfully coated with these thin film epoxy based paints and old application techniques, these thin paint films had problems in weld coverage, and were also unable to fill pits caused by pitting corrosion, and unable to replace metal loss caused by channel erosion. New coating material developments, revised application methods and modified pigging equipment have now made it possible to apply thick film coatings as corrosion barriers to the internals of pipes in the field, using these pigging techniques. The thick coating films can be up to 1 mm (40 mils) thick in a single coating run over long distances. The new thick film coating materials and application techniques are similar to continuous screeding, creating a far more robust seamless internal pipe lining when compared to the old technology of thin film paints. With thick film polymers the option now exists to create a structural GRP pipe within a pipe, be it steel or cementicious. It has been preferable, even necessary, to move away from the traditional epoxy paints and coatings. It was found difficult or impossible to use epoxy materials for these pipe coating applications due to the requirement for achieving a suitable viscosity with adequate hold up. In order to reduce epoxy viscosity a solvent or reactive diluent is required. This inevitably lead to solvent entrapment and poor cure characteristics in cold climates. Further mud cracking in over thick areas, the inability to extend or reduce pot life, poor control of cure time and limited resistance to aggressive or chemical environments made solvent based epoxy resins a poor choice.

In South African metalliferous mines, stoping operations are largely confined to narrow, tabular orebodies making mechanization extremely difficult. Limited flexibility in terms of stope width has necessitated drilling by means of handheld, pneumatic rock drills at most of our operations. As the largest producer of platinum in the world, we drill in excess of forty million stope blast holes per annum. Cleaning of the broken ore is also done by conventional means?utilizing scrapers and scraper winches. Conventional methods such as these carry high costs in terms of risk exposure as well as labour intensity. The purpose of this paper is to describe the development of a long hole stoping system suitable for the narrow platinum reefs. The new system had to be substantially safer and more cost effective than conventional mining. We selected a site that was conducive to mechanized mining and set out the parameters that we considered would meet our requirements. We then conducted a feasibility study to evaluate the conceptualized long hole stoping system in terms of specific goals. An analysis of the proposed system identified the Key Performance Indicators (KPIs). One of the most critical goals was to be able to create a man-free stope. This would mitigate the safety risk associated with normal stoping operations, as well as the inefficiency cost previously referred to.

This paper will highlight the system components of a successful underground mining automation project recently launched at DeBeers Finsch Mine. It will also show that integration of short-term mine planning with other upstream and downstream mining processes in the extraction of kimberlite ore from a block cave is not just critical to the future sustainability of this underground mining operation but also for profitability and cost-effectiveness. This paper will discuss the information data flow required from the mine-planning process through to production for underground block cave mining. The benefits associated with this type of integrated mine planning and automation system will be highlighted.

The objective of ergonomics is to enhance the effectiveness and efficiency with which human tasks are carried out. Furthermore, the objective is to improve desirable human values. This can be achieved by enhancing safety, reducing fatigue and stress, increasing comfort, increasing job satisfaction, having greater user acceptance and thereby improving the quality of life. Mechanization has influenced the prevalence of injury, mainly due to the application of human physical effort as the power source. With mechanization, the operator?s function is now essentially one of control rather than being an energy source, however, the risk of injury has not diminished. Mechanization in itself has introduced inefficiencies, health risks and physiological stress. The major objectives of ergonomics are discussed through examples of acceptable and less successful implementation of ergonomics within mechanized mining. The influence of the machine system on the human is addressed and practical solutions to improve and optimize the interaction of the human and the machine are outlined. Furthermore, practical examples of control and display integration, anthropometric and biomechanic influences on design, orientation, posture and stereotypes are used to accentuate the advantages of the application of sound ergonomics principles to the specialized area of mechanized mining.

Scaling of rockpasses was problematic at Tau Lekoa Mine prior to production commencing in 1991. High horizontal stress levels relative to the host rock mass strength have been instrumental in causing the observed scaling. Scaling has been more pronounced in the deeper rockpasses and these are monitored regularly. Rehabilitation of one of the reef passes and one of the waste passes was required in the period 1999 to 2001. This paper describes the extent of the problems experienced to date as well as the rehabilitation completed.

Safe and good operation of Søderberg electrodes is dependent upon high quality electrode paste coupled with correct electrode operating procedures. Good electrode performance depends further on proper melting and densification of the electrode paste. The melted paste should form a dense and viscous layer that prevents gasses from the baking process to move upwards. Both paste cylinders/blocks and briquettes can be used, but the procedures may differ between the various types of paste products. For briquette columns the formation of the important viscous layer is obtained through melting and densification of the briquettes. By basic rheological studies of electrode paste and by laboratory tests of briquette beds at isothermal conditions we have achieved a better understanding of the melting process. The effects of temperature gradients, pressure and friction between the briquettes and the electrode casing have also been evaluated qualitatively in pilot scale experiment and in industrial scale tests. The paper shows that melting and densification starts at a considerable lower temperature than the pitch softening temperature. It is concluded that the formation of a briquette electrode column depends on parameters as temperature, briquette column pressure and the friction. Operating conditions influencing the above parameters should be defined and established for each specific furnace operation. Such factors are e.g., charging and slipping procedures, column heating and electrical conditions.

Records over the past ten years indicate that there is potential for serious multiple accidents due to engulfing associated with rockpasses. Significant attention is not usually given to the design of rockpasses, with the implication that mistakes leading to poor performance of the passes are often repeated. This paper outlines general guidelines for the design, support, operation and rehabilitation of rockpasses based on information obtained from the limited amount of literature on the subject.

Mine accidents/injuries are complex and generally characterized by several factors, from technical to social characteristics. In this paper, a sociotechnical model for work injury in mines was developed and tested through structural equation modelling with application to case study mines in India. In total 14 variables were considered in this study. Most of the variables are not directly quantifiable. Instruments were developed to quantify them through a questionnaire survey. Underground mineworkers were randomly selected for the survey. Responses of 300 participants were used for the analysis. The analysis was conducted in three parts. Firstly, high-low plots were constructed to explore the variations in responses to the questionnaires by the accident involved (AG) and non-involved (NAG) workers. Secondly, a t-test was conducted to see whether the responses were significantly different for the two groups or not. The chi-square results showed significant differences of responses among the two groups. Finally, how these difference scan cause accidents/injuries in mines was hypothesized and evaluated using structural equation modelling. The measurement model of the Linear Structural Relations (LISREL) program was used to identify the latent constructs of the sociotechnical model. The structural model of LISREL was used to estimate the interrelationships among the constructs. The case study results showed that there is a sequential interaction among the sociotechnical factors leading to accidents/injuries in mines. Work hazards induce more job stress in the workers while social support mitigates the same. Job stress and safety environment predict the workers? job involvement. A worker who is more job-involved exhibits better safety performance, which in turn reduces work injury. The safety environment is shown to have a direct mitigating effect on work-related injury. The findings of this study clearly revealed that job stress and safety environment are the two key factors influencing work-related injuries in mines that need to be addressed properly through effective safety programmes.

Uranium is back on the radar. Uranium has always been a controversial metal, from its role in the arms race to its claim to be the only viable alternative to fossil fuel. This paper will cover its properties and use, its occurrence and production in South Africa, the current market drivers and its future role as a primary energy fuel. It will highlight the difference between uranium used to make a bomb and uranium used in a power station. The recent significant uranium price rise will be put into context of the market drivers including geopolitical issues. Planned expansions in nuclear power station capacity will be reviewed, as will new mine production. Future demand will also be speculated on.

The concentration of cassiterite at Iscor's Uis Mine is described, and information is given on aspects of the concentration of iron ore at Thabazimbi and Sishen Mines. At Uis, a very Iow-grade tin ore is concentrated by three-stage jigging, spiral concentration, and tabling to give a high-grade concentrate with a recovery of 80 per cent of the tin. A Iow-grade tantalite-columbite concentrate is also produced. The possibility of recovering mica, the use of sluice boxes instead of spirals, and the recovery of cassiterite from minus 45 µm material are being investigated. At the two iron-ore mines, heavy-medium separation is applied to size fractions larger than 0,5 mm. The types of ferrosilicon and the circuits used for the recovery of ferrosilicon from the various dilute-medium circuits are described. The spiral-concentration plant being installed at Thabazimbi for the treatment of the minus 0,5 mm portion of the ore is described.

The role of mould fluxes in continuous casting is reviewed. The main functions of the flux are described and the importance of the flux in providing lubrication and the right level of heat transfer between the shell and the mould is described. The major problems of longitudinal, transverse and star cracking, gas and slag entrapment and deep oscillation marks in the steel product and ?sticker breakouts? are analysed. The important physical properties of the flux in combating these various problems are identified. Physical property data for the fluxes are given.

In anticipation of the flooding of an adjacent dolomitic groundwater compartment, five rough parallel sided boundary plugs at 58 and 50levels have been designed to resist safely hydrostatic heads of 1500and 1250 metres, respectively, over a service life of 100 years. Following a description of the engineering properties of the surrounding rocks, the paper details the design considerations relating to structural length and strength, hydraulic gradient, rock mass watertightness, constituent materials of mortar intruded concrete, pregrouting of permeable discontinuities within the surrounding rock mass, a bentonite barrier to resist aggressive mine water, plug tightening and design performance requirements. Routine quality control tests include permeability of the rock mass, fluidity, bleed and strength development of the intruded mortar, and temperature of the intruded concrete during curing. A special test programme includes determination of strength and stiffness of cored concrete and rock samples, shear strength of the rock/plug interface and associated in situ contact stress in order toe stimate the load safety factor against plug failure by shear.

The South African coal mining industry is currently disposing of about 10 million tons of ultra-fine coal (<150 µm) per year. Once discarded, these sulphur-containing ultra-fines contribute to several environmental problems. As part of a project initiated by the Water Research Commission to investigate the use of Cleaner Production (CP) in the mining industry, a study was carried out to determine whether a CP approach could be used to identify opportunities to reduce this coal waste, and to determine which of these opportunities would be most feasible. In order to do this, a CP assessment was conducted at three case study collieries in the South African Witbank coalfield. Mass-balancing and sampling, followed by laboratory characterisation tests and site surveys, were used to determine the quantity, quality and sources of the ultra-fine coal at the three collieries. Literature reviews, brainstorming sessions and interviews then followed to generate the CP options. An environ-mental, economic and technical feasibility assessment was then prepared for each option, to determine the most viable interventions for implementation. A number of opportunities were identified through the assessment. By preventing coarser coal from being discarded with the ultra-fine coal, the quantity of coal disposed of could be decreased at all three collieries, and by up to 24% in one case. Increasing the crusher top size would reduce the amount of coal that is milled to less than 150 µm, so that less is wasted. The ultra-fines that have already been disposed of on slurry dams can be completely reclaimed and converted into a valuable product, which can be sold as power station feedstock. The newly processed ultra-fines could be beneficiated using flotation and exported together with the coarser coal. The results of the assessments thus suggested that workable CP opportunities to reduce ultra-fine coal wastage exist at the sites investigated, and that their feasibility is colliery-specific. The associated financial benefits of the proposed options suggested that CP is a realistic approach to addressing environmental problems. The thermodynamic activity of MnO was measured in MnO-CaO-MgO-SiO2 slags that are typically encountered in the production of manganese-containing stainless steels. A gas equilibration-quenching technique was employed in measuring the activities. Activities of Mn in Pt-Mn alloys were redetermined at 1600ºC, which reconfirmed earlier results of strong negative deviations. MnO activities were then measured by equilibrating slags of various compositions based on a statistical experimental design procedure with Pt-Mn alloys under controlled partial pressure of oxygen in MnO saturated platinum crucibles at 1600ºC. Activities of MnO were found to increase with increasing concentration of MnO, basicity and CaO to MgO ratio. Two multi-coefficient regression models were developed to predict the activities and activity coefficients of MnO in slags at 1600ºC. Keywords: activity, MnO, slag
