An examination of the Census of Fatal Occupational Injuries (CFOI) data for the period 1992-1995 shows that 659 fatalities occurred in the mining industry as defined by the SIC. Oil and gas accounted for slightly more than one-half of all fatalities (figure 1-1). The coal industry had the highest fatality rate for the 4-year period, with 35.6 deaths per 100,000 full-time workers, followed by metal (23.1 per 100,000), oil and gas (23.1 per 100,000), and nonmetal (20.1 per 100,000). The fatality rate for all U. S. industries during this same period was 5.1 per 100,000 full-time workers. Figure 1-2 compares the fatal injury rates by year for the four major industry sectors of mining to the fatality rate for all industries combined. Figure 1-3 compares age-specific fatality rates in the mining industry to those seen for U. S. industry as a whole. Fatality rates for all industries combined were lowest for young workers and steadily increased in relation to workers' ages. In the mining industry, fatality rates by age group show a U-shaped curve. Young workers under 20 years of age had the highest injury rate (54.6 per 100,000), followed by workers aged 20-24 (50.8 per 100,000) and workers over age 65 (37.7 per 100,000). Table 1-1 shows the distribution of injuries by type of injury event. The mining industry, in comparison with all industries, had a considerably larger proportion of fatalities classified as caught in or crushed by collapsing materials (9.1% and 1.9%, respectively) and fires and explosions (10.0% and 3.1%, respectively). Transportation events comprised the largest proportion of fatal injuries in both the mining industry and all industries. Within the mining industry only, transportation accounted for the largest proportion of fatalities in all mining sectors except coal. A more detailed examination of the transportation events within the mining industry showed that highway incidents (50.5%) were the most common, followed by nonhighway incidents (22.2%), pedestrian incidents (10.6%), and those incurred during rail, water, and air transport (15.7%). Incidents involving caught in or crushed by collapsing materials comprised a higher percentage of injuries in the coal and metal sectors, while fall-from-height incidents were more frequent in the oil and gas sector. The classification system used in CFOI for type of injury event is not compatible with any variables contained in the MSHA data; therefore, direct comparisons are not attempted. [ ]

In this report, the application and limitation of the various equipment available for surface coal mining are discussed. The correction factors for adjusting equipment performance arising from machine design, material characteristics, and modes of operation are presented. Engineering design, pit layout and equipment selection are discussed with mathematical derivations. Two computer programs (SMCALC and SMRES) have been written to aid in pit design and equipment selection. The Strip Mine Calculation Program (SMCALC) can be used to select the bucket size and reach of shovels and draglines for various pit widths. It will also calculate the number of drills, loading shovels, trucks and bulldozers needed to achieve a production target. The Surface Mine Reclamation Equipment Selector (SMRES) can be used to select the number of bulldozers, scrapers and front-end loaders needed to achieve the desired production.

"Chromates are known to induce tumors (1, 2), mutations (3), and DNA damage (4-6). However, the underlying biochemical mechanism(s) is (are) not fully understood (6). Recent studies have indicated that hydroxyl (·OH) radical may play an important role (7-11). The mechanistic studies of Cr(Vl)-related ·OH radical generation in biochemical systems have shown the reaction of a Cr(V) species with hydrogen peroxide (H20 2 } through a Fenton-like mechanism to be one of the important steps (9-11): Cr(V) + H20 2 - Cr(VI) + ·OH + OH-. (1)In biological systems the Cr(V) species are generated via the reduction of Cr(VI) by a variety of components, such as microsomes (12), mitochondria (13), mitochondrial electron transfer complexes (14), and at least three flavoenzymes (glutathione reductase (GSSG-R),3 lipoyl dehydrogenase, and ferredoxin-NADP+ oxidoreductase) (9, 15, 16). "The University of Minnesota and the U.S. Bureau of Mines collaborated to develop and field test a personal diesel exhaust aerosol sampler (PDEAS). The PDEAS was field tested in five underground coal mines that use continuous miners and a variety of diesel vehicles, including diesel haulage and utility vehicles. One mine was surveyed a second time, with the haulage vehicles fitted with a low-temperature disposable diesel exhaust filter (DDEF). Aerosol samples were collected with a variety of instruments, including the PDEAS and the microorifice, uniform-deposit impactor (MOUDI). This paper presents the diesel exhaust aerosol (DEA) concentration data collected in these mines and assesses the impact of diesel face-haulage equipment, with and without exhaust filters, on underground mine air quality.The average DEA concentration at the haulageway location for five mines, determined by the PDEAS, was 0.89 mg/m3, with a standard deviation of 0.44 mg/m3 • DEA contributed 52 pct, of the respirable aerosol at the haulageway location. Use of the DDEF at one mine reduced DEA by 95 pct, with a standard deviation of 6 pct, and filter life averaged 10 h. DEA contributed a large proportion of the respirable aerosol concentrations in mines with diesel equipment, and a substantial reduction was achieved with use of a DDEF. INTRODUCTIONDiesel equipment is gaining popularity in underground coal mines. There are an estimated 1,951 units of diesel equipment in 152 underground coal mines compared with 1,100 units in 110 mines 5 years ago (J).5 This increased use is due to the recognition that diesel-powered vehicles are more versatile, which can contribute to increased productivity compared with their electrically powered counterparts. Approximately 33 pct of the diesel vehicles haul coal, 45 pct haul personnel and materials, and- 22 pct perform other duties, such as roof bolting, rock dusting, and road maintenance. Most mines using diesel equipment are located in Kentucky, Utah, Virginia, and Colorado."

Objective: Workers with hearing loss face special problems, especially when working in noise. How­ever, conventional hearing conservation practices do not distinguish between workers with normal hearing versus impaired hearing. This study col­lected information from workers with self-reported noise exposure and hearing loss, supervisors of such workers, and hearing conservation program managers through focus groups and in-depth inter­views to evaluate their perspectives on the impact of hearing loss on safety and job performance, the use of hearing protection, and information needed to appropriately manage hearing-impaired workers who work in noisy environments. Results: Concerns about working in noise with a hearing loss could be grouped into the following 10 categories: impact on job performance, impact on job safety, impaired ability to hear warning signals, impaired ability to monitor equipment, interfer­ence with communication, stress and/or fatigue, impaired communication caused by hearing protec­tor use, reduced ability to monitor the environment as the result of hearing protector use, concerns about future quality of life, and concerns about future employability. Mostly, there was an agree­ment between the perceptions of workers, supervi­sors, and hearing conservation program managers regarding difficulties associated with hearing loss and consequent needs. These findings suggest that noise-exposed workers with hearing loss face many of the same problems reported in the literature by noise-exposed workers with normal hearing, with additional concerns primarily about job safety as the result of a reduced ability to hear environmental sounds, warning signals, and so forth. Conclusions: The study outlines potential chal­lenges regarding job safety and hearing conserva­tion practices for noise-exposed, hearing-impaired workers. Awareness of these issues is a necessary first step toward providing appropriate protective measures for noise-exposed, hearing-impaired workers. Of the 19 million adults in the United States with some degree of hearing trouble, nearly half are currently employed in the workforce (NCHS, 1994). More than 30 million US workers are exposed to hazardous noise levels, and noise-induced hearing loss is the most common occupational disease in the United States (NIOSH, 1996). Many workers incur their hearing losses during the first 5 to 10 yrs of employment (Rosler, 1994; Ward et al., 2000) and subsequently work the rest of their careers in an environment where noise and hearing loss interact to impair their ability to hear communication and other important signals. Workers with hearing loss who are exposed to hazardous noise levels present a dilemma to hearing loss preventionists. On the one hand, it is of great importance to ensure that their residual hearing is protected from the damaging effects of the noise to which they are exposed. On the other hand, how­ever, traditional hearing protectors serve to further “deafen” such workers, making communication more difficult and impairing the detection of environmen­tal sounds and warning signals. These workers have special needs that must be addressed. However, conventional hearing conservation practices do not distinguish between workers with normal versus impaired hearing; and, to date, no governmental or professional organization has developed specific guidelines or policies concerning the accommodation of noise-exposed, hearing-impaired workers. Some research regarding noise-exposed, hearing-impaired workers has been completed. These studies have primarily focused on three areas: (a) speech intelligibility in noise versus quiet, with and without various types of hearing protection devices (Abel et al., 1982; Abel et al., 1993; Abel & Spencer, 1997; Rink, 1979; Suter, 1989); (b) signal detection and localization under similar conditions (Abel et al., 1993; Abel et al., 1985; Abel & Hay, 1996); and (c) the use of hearing aids as hearing protectors (Berger, 1986; Dolan & Maurer, 1996; Hétu et al., 1992; Hétu et al., 1993). Research in these three areas has provided an initial framework for development of a management para­digm for these workers. Changes in the types of materials used for the construction of recent automobile models have occasioned a review of the continued applicability of current recycling techniques. Plastics are being used increasingly in late model cars. To evaluate the changes occurring, a 1972 Mercury Montego, furnished by the Ford Motor Co., was completely dismantled and analyzed, and the composition was compared with that of a composite automobile from 15 vehicles ranging in date of manufacture from 1954 to 1965 and designated as a circa 1960 model. The composite vehicle contained about 26 pounds of plastics, while the 1972 model contained over four times that quantity. The increasing use of plastics in automobiles suggests that research to improve the economic potentiality of plastics recycling is warranted.

This report summarizes the results and conclusions reached by various investigators who have attempted to determine the extent to which hydraulic fracturing will affect the secondary recovery of crude oil by waterflooding. It also discusses the most advantageous use of this well-stimulation method in waterflood projects. A thorough search of the literature was made, and pertinent information was abstracted from appropriate reports. A carefully designed and controlled program of formation fracturing is an effective aid in waterflooding because it increases water-injection and oil-production rates. Generally, fracturing has resulted in little or no reduction in ultimate oil recovery, but under certain circumstances recovery has been substantially increased.

Research by the National Institute for Occupational Safety and Health (NIOSH) on multiple-seam mining has resulted in two new ground control products. Information Circular (IC) 9495 In some U.S. coalfields, particularly in central Appalachia and the West, most mines are operating above and/or beneath previously mined seams. The effects of multiple-seam interactions can include roof falls, rib spalling, and floor heave. These can seriously threaten the safety of miners and disrupt mining operations. Information Circular (IC) 9495 is a proceedings document containing a variety of papers that summarize recent knowledge about multiple-seam mining. A NIOSH study that analyzed case histories collected from 44 U.S. coal mines is described in detail. The study confirmed that overmining is much more difficult than undermining, and isolated remnant pillars cause more problems than gob-solid boundaries. The study's results were the basis for the Analysis of Multiple-Seam Stability (AMSS) software package. The proceedings also contain two papers describing the LaModel family of software. The original three-dimensional version of LaModel is essential for detailed analyses of complex multiple-seam scenarios. LaM2D implements a simplified, two-dimensional model suitable for quick approximations of the multiple-seam stresses and strains. Three papers in the proceedings focus on case histories. One describes the extensive multiple-seam experience at the Harris No. 1 Mine in West Virginia, a second contains examples of "extreme" multiple-seam mining from the central Appalachian coalfields, and a third describes interactions with regard to longwall mine experiences in Pennsylvania, West Virginia, and Utah. The final paper reports on a numerical modeling study that provided some insight into the mechanics of multiple-seam mining. [ ] AMSS Software Package NIOSH has developed a simple, user-friendly computer program called Analysis of Multiple-Seam Stability (AMSS). The Derby and Alexander Claim, located within the Tunnel Ridge Wilderness Study Area (WSA), has placer gold resources. No lode deposits are known within the area, but lode gold deposits are found within 1 1/2 mi of the WSA. In additon, rock units which contain lode gold deposits north of the WSA are found within the northern portion of the study area. INTRODUCTION The Tunnel Ridge Wilderness Study Area (WSA), CA-030-402, encompasses 4,623 acres of BLM administered public land in Trinity County, California, about 35 mi west-northwest of Redding (fig. 1). Major access to the Tunnel Ridge vicinity is by U.S. Highway 299. Ac-cess to the WSA boundary is by a secondary paved road from Junction City and several light duty dirt and gravel roads (fig. 2). The WSA is on the southwest slope of Weaver Bally Mountain. The topography is rugged with moderate to steep slopes; elevations range from about 1,600 ft to 5,900 ft.

This paper presents the results of a study using computer human modeling to examine machine appendage speed. The objective was to determine the impact of roof bolter machine appendage speed on the likelihood of the operator coming in contact with. A contact means two or more objects intersecting or touching each other, e.g., appendage makes contact with the operator?s hand, arm, head or leg. Incident investigation reports do not usually contain enough information to aid in studying this problem and laboratory experiments with human subjects are also not feasible because of safety and ethical issues. As an alternative, researchers developed a computer model approach as the primary means to gather data. By simulating an operator?s random behavior and machine?s appendage velocity, researchers can study potential hazards of tasks where it is not possible to perform experiments with human subjects. Analysis information is helpful to the mining industry in terms of making recommendations that reduce the likelihood that roof-bolter operators experience injury due to contact with a moving boom. Data analysis of roof bolter simulations show that the virtual-operator?s response time has little effect on the number of contacts experienced. Based on frequency and cross-tabulation, regardless of other variables, contact incidents were always greater when the boom was moving up, were always greater on the palm, and were always greater for the boom part of the machine. Also, regardless of boom speed, the 25th-percentile-sized operators experienced more contacts than did other operator sizes. Furthermore, regardless of boom speed, the 152-cm mine seam experienced more contacts than did other seam heights tested. Results of a survival analytic approach suggest that controlling the boom speed is the most important factor in determining the risk of an operator making contact. Based on the data collected, boom speed greater than 41 cm/s results in a substantial increase in risk to the roof bolter operator and should probably be avoided. At speeds less than or equal to 25 cm/s are associated with a more modest relative risk, which represents an acceptable level of risk. Also, at speeds between 25 and 41 cm/s, there could be a particular boom speed in this range at which a significant inflection point in the relative risk estimates occurs. Based on the results, this issue needs to be addressed by future research.

The development of a sampling instrument for the assessment of an underground coal miner’s exposure to airborne respirable coal mine dust on a continuous and real-time basis has been long identified as essential for timely initiation of control actions. A respirable dust dosimeter (RDD) which operates on the principle of increasing pressure across a filter media with increasing dust mass on the filter has been developed by the National Institute for Occupational Safety and Health. Comparisons of the performance of the RDD with an U.S. approved gravimetric respirable dust sampler (GRD) have shown that the RDD may meet the requirements for measuring miners’ exposures on a cumulative, near real-time basis such as mid-shift and end of shift exposures. Recently, the performances of the RDD and GRD were compared by conducting side-by-side area sampling experiments in a controlled experimental environment in the laboratory and in underground coal mines under normal operating conditions. In this paper, the results from the in-mine study are presented and discussed. The experimental design for the in-mine study included two mines in different coal seams in different geographic coal regions. To ensure a wide range of concentrations, the experiments were conducted in a continuous miner section in Mine 1, and a longwall face in Mine 2. Airborne dust sampling was conducted for four different shifts at each mine. In each shift, two samples of different exposure durations were collected from two different locations. Each sample consisted of the readings of three dosimeters and the exposed filters from the three paired GRD samplers. As a result, a total of 96 comparative samples were obtained, 48 for each mine. The data were analyzed for the precision of the RDD and GRD measurements and for the relationships between the differential pressure of the RDD and the mass of dust on the paired GRD filter. The analyses indicate that, on the average, the RDD and the GRD track the airborne respirable dust concentrations well, exhibiting both a high correlation between the two measurements and a reliable predictive relationship of one from the other. Details of the experimental design, data analyses procedures, results, conclusions and recommendations of the underground experiments are presented in this paper.

For this service, or rather for the assumption of the risk, the insuring agency charges the mining company a price, or in insurance parlance, ""a premium"".The insuring agency depends upon a volume and variety of business to average the losses incurred by its individual policy holders, and determines its price to cover losses that are suffered and its cost of doing business.It is generally known that all types of insurance are founded on the theory of probability applied to large groups of risks, Workmen's compensation insurance is no exception, and the average price paid therefor must be based on the frequency and severity of accidents in the particular industry or branch of industry under consideration. The frequency and severity rates are determined by statistics, which, of course, unless standardized, kept carefully and consistently, and analyzed properly, are liable to be inaccurate and misleadingThe importance of large groups in arriving at an average price for insurance service can not be overestimated, For example the writer knows of one small group of some twenty metal-mining companies whose losses during the past two years exceeded their premium charge by 54%. Not a single individual company during the two years kept its losses below the premium rate. For several years the group as a whole has persistently exceeded its premium rate, Such risks are not profitable to the insuring agencies and can only be carried where a large volume of good risks will balance the poor risks, but in order to conform to good insurance ethics the premium rate must be based on averages for the industry as a whole, and kept sufficiently low to induce operators to sell their risks."

During the time period from 1990-2007, 1601 reportable fires occurred in the U.S. mining industry (an average of 89 fires per year). The leading causes of U.S. mine fires were flame cutting and welding operations, frictional heating and ignitions, electrical shorts, mobile equipment malfunctions, and spontaneous combustion. The fact that mine fires continue to occur with an alarming regularity reinforces the importance of recognizing and eliminating the potential hazards and the overall need for improved fire control and suppression technology to ensure the best possible outcome during a mine fire. The National Institute for Occupational Safety and Health (NIOSH) is conducting an evolving comprehensive program of research that is addressing mine fire prevention, detection, management and suppression. This paper presents a summary of recent research accomplishments and provides an overview of the next phase of the NIOSH mine fire research program.

Vibration magnitude and frequency of the z-axis vehicle seat are time-variant, which are influenced by not only vehicle vibration characteristics themselves but also road surfaces, speeds and the human body. There is little in the current reporting about evaluating and analyzing automobile seat vibration that focuses on the time-variant. Yaguchi et al.1 has proposed a method to evaluate automobile seat vibration that is based on judgments using a subjective mental state. Their method focuses on the time-variant magnitude of the peak frequency on a power spectrum density. However, their method has no consideration of all the frequency contents of the discomfort, nor comparison between different peak frequency vibrations. Suzuki2 has emphasized that the vehicle vibration should be judged by a series of vibration stimuli to evaluate, because the vehicle vibration is time-variant, which isn’t a matter of the relationship between a single vibration stimulus and a subjective response. He clarified that the human sensation to the vehicle vibration discomfort changes every moment showing the relationship between the frequency-weighted r.m.s. acceleration calculated every 5 seconds and the category judgment to vehicle vibration discomfort every 5 seconds. However, his study doesn’t show what parameter connects to the subjective final judgment to vehicle vibration. Therefore, we applied the method similar to ISO100563 considering the time-variant to the vehicle seat z-axis vibration evaluation. The new method for the vehicle seat vibration considering the time-variant was examined on the hypothesis that the final subjective evaluation must be conducted from the judgment summarizing a series of vibration stimuli. Methods The vibration bench system, which reproduces the movement of a vehicle floor, was used for the experiment with the single-axis (vertical direction) four-post road simulator system, which is usually used for a car, as shown in Fig.1. The experiment was done on the right side of the vibration bench using the floor vibration which was 5.5 minutes, 0.822 m/sec2 (Wk) over the range 0.5-20Hz with 4 male subjects (age ave21.5, SD0.5, weight ave75kg, SD7.91kg, height ave166.8cm, SD5.2cm) and 4 suspension seats. As Fig.2 shows, subjects evaluated the degree of discomfort every 5 seconds to each seat vibration measuring the seat z-axis vibration acceleration. [ ]

Problem: CFOI and SOII data show that 2,287 U.S. workers died and 32,807 workers sustained days away from work due to electrical shock or electrical burn injuries between 1992 and 1998. Method: The narrative, work activity, job title, source of injury, location, and industry for each fatal electrical accident were examined. A primary causal factor was identified for each fatality. Results: Electrical fatalities were categorized into five major groups. Overall, 44% of electrical fatalities occurred in the construction industry. Contact with overhead power lines caused 41 % of all electrical fatalities. Discussion: Electrical shock caused 99% of fatal and 62% of nonfatal electrical accidents. Comprising about 7% of the U.S. workforce, construction workers sustain 44% of electrical fatalities. Power line contact by mobile equipment occurs in many industries and should be the subject of focused research. Other problem areas are identified and opportunities for research are proposed. Impact on Industry: Improvements in electrical safety in one industry often have application in other industries.

In 2004, a total of 741 nonmetal mining operations reported employment to the Mine Safety and Health Administration (MSHA). These mines (e.g., potash, gypsum, salt) comprised 5.1% of all operations.

The Thermo-MIE personal DataRAM dust monitor (pDR) is a light-scattering instrument approved for use in gassy underground mines by the U.S. Mine Safety and Health Administration (MSHA). However, the use of light-scattering monitors has been inhibited by concerns about inaccurate readings resulting from changes in dust size distribution and composition, as well as concerns about the impact of moisture in the mine air. Because of this concern, the National Institute for Occupational Safety and Health (NIOSH) evaluated the pDR using available data from laboratory tests and mine surveys. The data show that accuracy can vary widely from test to test and mine to mine. In most cases, the results can be improved by using a companion gravimetric measurement to adjust the pDR data.
