The National Institute for Occupational Safety and Health (NIOSH), Pittsburgh Research Laboratory, has undertaken a study to determine how well self-contained self-rescuers (SCSRs), deployed in accordance with Federal regulations (30 CFR 75.1714), hold up in the underground environment with regard to both physical damage and aging. This report presents findings regarding laboratory-tested SCSRs in the sixth phase of testing from mid-1996 to early 1998. The SCSRs were tested on human subjects and on a breathing and metabolic simulator. These results indicate that most of the apparatus, if they pass their inspection criteria, perform satisfactorily. However, the deployed CSE SR-100s exhibited significantly higher inhaled CO2 levels than new units, as in the previous phase. This will cause higher ventilation rates in most users, which will, in turn, result in higher breathing pressures, possibly causing users to prematurely remove the apparatus. CSE Corp. has developed a noise test that can identify apparatus suffering from chemical-bed degradation causing the early CO2 breakthrough. This test was added to the inspection criteria for the SR-100. In addition, several of the MSA Portal-Packs that passed their inspection criteria were found to have KO2 dust in their mouthpieces. Further investigations by NIOSH and the Mine Safety and Health Administration confirmed these findings, which resulted in the decertification of the apparatus and their removal from service.

This final report documents the work done by Arthur D. Little, Inc. (ADL) on behalf of the U.S. Bureau of Mines, Pittsburgh Mining and Safety Research Center (PMSRC), on Contract R0122026 (which began in August of 1971). Under this contract ADI provided technical assistance to the Bureau on a task basis on virtually all aspects of the Bureau's programs related to present and planned emergency and operational communications and miner location systems for underground coal mines. The work consisted of independent investigations, analyses, experiments, breadboard and prototype hardware development, workshops and technology transfer seminars on mine communications, and on-going evaluations and guidance related to the Bureau's contracted programs on electromagnetic noise, mine communications systems, and trapped miner location. This final report documents the work in two volumes, Volume I, "Emergency and Operational Mine Communications," and Volume II, "Seismic Detection and Location of Isolated Miners." The Tables of Contents of both Volumes are included in each Volume. Phase I of the contract was devoted to performing an in-depth assessment of electromagnetic noise measurements taken by several contractors and other investigators, and then defining a new noise measurement program and instrumentation system tailored to obtain the necessary but missing noise data. These data are required for use in the design of new emergency and operational communication systems. This work, and the follow-on coordination and guidance activities of ADL on this noise measurement program in subsequent phases of the contract, are treated in Part One of Volume I.

Field studies were performed during blasting operations for strip coal mining above the room and pillar workings at Jenny mine in eastern Kentucky. The study is part of the Bureau of Mines program to develop criteria for the proximity of surface blasting to underground mines. Measurements and observations were made to identify physical changes in the mine. No change in the frequency of roof falls or any other indication of apparent damage was noted. Recordings of blast induced vibrations were made using sensors on the roof and floor of the underground mine and at the ground surface. A maximum particle velocity of 17.5 in/sec was recorded on the mine roof. The vibration data were analyzed and comparisons were made between roof vibration levels and those recorded on the mine floor and at the ground surface. Results are compared with previous blast vibration studies. Recommendations are made for further study toward developing criteria.

The performances of the three bituminous coal demonstration methane re¬covery boreholes, located in Greene County, Pennsylvania, are reviewed. Development of these vertical holes was largely underwritten by project funds. Reference is also made to an anthracite coal methane project in Luzerne County, Pennsylvania. This involved one vertical hole and was underwritten by a utility company. In addition, an exploratory drilling in the southern anthracite, which provided the project group with coal cores for methane gas analysis, is also cited. The technical and economical data that were gathered from the various methane projects are analyzed. Finally, the advantages and disadvantages of methane recovery by vertical boreholes are assessed.

The value of abrasive materials consumed in the United States increased 37% in value over that of 1983 to $439 million; of which, 58% was manufactured abrasives, 36% was industrial diamond (natural and synthetic), and 6% was natural abrasives. Production value of natural abrasives, which consist of crude tripoli, special silica stone, garnet, and emery, increased 4% Production of crude tripoli, a finely disintegrated chert, increased 12% in quantity and 8% in value. Shipments of processed tripoli increased only 3% in quantity but increased 16% in value, with large increases occurring in both major end-use categories. Production of garnet, an abundant iron-aluminum silicate, decreased slightly in quantity and value after posting continued increases in quantity and value since 1980. The objective of this five year study was to investigate the means of establishing non-polluted ground-water and root zone systems in areas where surface mining intercepts physiochemically unsuitable overburden materials. Overburden may need to be sampled on a 60 m grid in order to delineate materials unsuitable for reclamation and to accurately implement selective handling operations. Unsuitable overburden materials were buried successfully in a dragline operation with a minimum of delays, but costs were increased 12 to 53%. After three years the buried material remains above the reestablished aquifer and below the root zone. Construction of a clay cap over the buried material may not be required in semiarid environments. It was shown that some unsuitable overburden materials do not have to be special handled. When inhibitory material constituted <5% of the total overburden volume, such material was essentially not detectable in the resultant spoil pile due to mixing/dilution processes. When inhibitory material exceeded 15% of the overburden volume it was always found in the resultant spoil pile, generally with a smaller extent, indicating partial dilution. At least a portion of the postmine aquifer had hydraulic conductivity rates lower than the premine system, indicating the reestablished aquifer may, in part, conduct ground water very slowly. Postmine ground-water chemical quality in spoil materials had poorer quality compared to premine ground water.

The Bureau of Mines investigated the mechanics of fuming of iron-carbon melts during top-blowing with oxygen. Melts weighing 45 lb were prepared in an induction furnace and blown with oxygen by means of water-cooled lances. The rate of fuming was measured and correlated with the operating variables. The data showed that the primary cause of fuming was a boil at the bath surface in the impingement zone of the oxygen jet. Nucleation of the carbon monoxide bubbles producing the boil was apparently brought about by the precipitation of an oxide phase. The actual fuming mechanism involved partial oxidation and vaporization of bubble films and fine metal spray generated by the bursting of the bubbles. Bubbles that formed outside the impingement zone produced little fume. The distribution of trace elements between the metal and fume strongly indicated that the fume contained fine spray particles as well as particles that had condensed from vapor. The rate of fuming was found to be proportional to the carbon content of the metal) the concentration of oxygen in the jet) and the bath temperature) provided that a continuous boil occurred. In the absence of a boil) very little fume was produced.

The objective of this handbook is to provide the inspector with guidance for recognizing hazards from inhalation contaminants in above ground work areas of underground and surface coal mining and processing facilities and evaluating these hazards by collecting samples or making field determinations and interpreting the analytical results. The three major objectives of this contract were (1) to survey underground diesel-powered mining machinery to determine noise exposure problems, (2) to recommend noise control techniques, and (3) to develop a technique for predicting underground sound levels from surface sound power measurements. The objectives were achieved. The contractor developed information on the severity of the noise problem caused by diesel engines underground and pinpointed the equipment causing the problems. He recommended a number of corrective techniques that can provide relief until long-term solutions are found. A procedure for predicting underground noise levels from above-ground measurements of the machine noise was found valid for several cases tested.

This publication describes a study by the Federal Bureau of Mines of devices and methods used for testing and splicing 4,160-volt and 440-volt cables and for providing ground-fault protection for 440-volt circuits, as developed and used by the electrical staff of the Tecumseh coal mine, an open-pit operation at Boonville, Ind. The testing and ground-fault devices described should find ready application in other open-pit mines, and, with suitable modifications for voltages involved, in underground mining. The devices and testing methods are based on sound engineering principles and are time-tested and economically feasible, Four devices are used: A high-voltage tester, and a low-voltage high- current tester, both for testing 4,160-volt cables; a tester for 440-volt cables; and, electronic ground-fault protective device for 440-volt cable circuits. All the cable testers and the grounding device were designed and built at the mine from standard parts. They are inexpensive, dependable, and job tested, An attempt has been made here to describe them in enough detail so that interested persons can assemble duplicate units.

"The fifth annual National Safety Competition conducted by the United States Bureau of Mines to determine the best safety records at mines and quarries during the calendar year 1929 has been concluded, and the general results of the contest are presented herewith. To the winner in each of the five groups into which the contestants were divided there has been awarded the bronze trophy 'Sentinels of. Safety,' donated by the Explosives Engineer magazine. The trophy for bituminous coal mines was won by a mine in Alabama; that for metal mines by an iron-ore mine in Michigan; that for nonmetallic mineral mines by a salt mine in New York; that for quarries and open-cut mines went to a limestone quarry in Michigan;. the trophy provided for anthracite mines was, of course, won by a mine in Pennsylvania, as all of the anthracite mines participating in the contest are situated in that State.Winners of the trophies were selected by a committee of sward in accordance with rules governing the contest and on the basis of accident records furnished by the various companies to the Bureau of Mines. These records were summarized by the bureau and the results submitted to the committee. Members of the committee are W. H. Cameron, managing director of the National Safety Council. William Green, president of the American Federation of Labor; James F. Callbreath, secretary of the American Mining Congress; T. T. Read, Vinton professor of mining of Columbia University and editor of 'Mining and Metallurgy'; C. B. Huntress, executive secretary of the National Coal Association; A. J., Curtis, assistant to the general manager of the Portland Cement Association; A. T. Goldbeck, director of the engineering bureau of the National Crushed Stone Association.Thirty-four States were represented in the l29 contest. Among the plants which participated were 68 bituminous coal mines, 29 anthracite mines, 54 metal mines, 19 nonmetallic mineral mines, all of the foregoing being underground mines, and 141 quarries or open-cut mines. A total of 106,533,006 man-hours of work was reported by all of the companies. Accidents during the year numbered 7,008 and represented a period of disability equal to 986,306 man-days. Hence the accident-severity rate for all plants combined represented 9.3 man-days of disability for each thousand man-hours of exposure to hazard. Similarly, the combined accident-frequency rate for all plants showed that 66.3 accidents occurred for every million man-hours of exposure to hazard. Both of these rates indicated an improvement over the combined rates for all plants in the previous year's contest. The reduction in the accident-severity rate centered chiefly in the metal mine group, though all except the quarry and open-cut mine group shared in the reduction; the improvement in the accident-frequency rate was in all croups except anthracite and metal, the rate for the former having increased while that of the latter remained unchanged."

The Bureau of Mines made laboratory investigations to determine the feasibility of utilizing fine tungsten wires to reinforce a number of nonferrous metals and alloys and a superalloy, S-8l6. The composites of tungsten filament and metals were chosen to attain elevated temperature strength and corrosion resistance. Oriented clean or precoated tungsten wires were incorporated with the other metals by extrusion, swaging, and infiltration casting. Although widely varying characteristics were found in the composites, none were considered satisfactory for high-temperature use, chiefly because of reactions between the matrix metal-tungsten surface. Diffusion, solution, and chemical reactions between matrix and tungsten wires were sufficiently slow for some combinations, such as 15 v/o 8-816, to exhibit increased high-temperature strength for a limited time. A few metals failed to wet or bond to the tungsten surfaces, while a few responded after precoating with a third metal.

A laboratory investigation was conducted by the Bureau of Mines to study the magnetic characteristics and to measure the magnetic susceptibility of the mineral siderite. All susceptibility measurements were made by the method of Gouy on powdered samples and at room temperature. Specific susceptibilities of samples from 15 different locations, domestic and foreign, were determined. Values ranged from 66 to 103 (10-6) cgs electromagnetic units. The relation of susceptibility to mineral composition is discussed in this report. The specific susceptibilities of 76 minerals are tabulated in an appendix.

This report presents the results of a program to improve, using existing and proven technology, unstable highwall conditions through better blasting practices in Appalachian strip coal mines. Better borehole blasting agent distribution and optimization of burden, spacing, and delay periods allowing satisfactory breakage with minimal backbreak were the objectives. At a selected field site, geologic conditions were closely examined with regard to joint orientation and spacing, and resulting slope stability considerations. A test series of eight blasts was completed and the resultant highwall monitored by repetitive photography. The study of drilling and blasting economics along with equipment performance led to an economic model of blast parameters and the identification of blast design cost benefits. The work demonstrated that small strip mining companies&apos; are able to achieve a more stable highwall excavation utilizing presently available technology, equipment, and explosive products at improved operational cost and safety.

A ventilated helical boost pump was developed that can handle varying flow rates and/or solids concentrations while operating at a single rotational speed and without computer assisted feedback controls. The boost pump developed and briefly tested during this program was designed to accomodate flows suitable for a 3-in. diameter pipeline. The boost pump&apos;s ventilated design provides the automatic pressure regulation needed to meet the system&apos;s requirements for supporting transient pipeline flow.

This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. The discovery of the Oklahoma City field and its .rapid development into one of the major oil-producing districts is an outstanding epoch in the history of the petroleum industry not only of .Oklahoma but also of the United States. The relatively great depth to which the wells were drilled and the large initial production of high-gravity oil are of special significance. The field has boon under proration almost from the time that its development began, and many new production problems have developed out of the changed conditions of operation. The discovery well was first tested in December 1928 at a depth of 6,402 feet, with an initial production of 4,847 barrels of 38.5° A.P.I. gravity and several months later was completed at a depth of 6,624 feet. The large initial production, the high pressure, and the large gas volume in the different horizons of the discovery well suggested the potential possibilities of gas and oil, production in formations of Pennsylvanian and pre-Pennsylvanian age.
