"Sub-standard practices and their adverse impact on safety performance remain a challenge in the South African gold mining industry. The purpose of this study was to investigate the root causes of sub-standard practices and the effect on safety performance in South African gold mines. The focus of the study was on the underground production operations at a gold mine in the Free State. The study consisted of three parts: a personal investigation into the causes of sub-standard practices at the mine, a comparison with the results of a cultural study performed at the West Wits mines, and a behavioural survey.The studies described in this article yielded similar results in terms of the key drivers behind sub-standard practices. It was found that substandard practices had a number of causes, and the origin lies within the habits, attitude, and behaviour of employees. Current methods of addressing sub-standard practices and improving safety performance were found to discourage employees from performing work safely and according to procedure. It was concluded that the lack of critical behavioural habits for the given work environment and job title could possibly be the primary reason for the occurrence of sub-standard practices. IntroductionSince the implementation of the Mine Health and Safety Act, 1996 (Act 29 of 1996) and the Mine Health and Safety Regulations, the South African mining industry has been striving to improve health and safety performance and has placed great emphasis on adherence to mine standards.The Act requires employers in the mining industry to report accidents, incidents, and dangerous situations to the Regional Principal Inspector of Mines in their area. The data provided by employers is captured and analysed by the South African Mines Reportable Accident Statistical System (SAMRASS) and made available for public viewing (Department of Mineral Resources, 2011).A review of the national mine-related accident statistics shows clearly that the South African gold mining industry has improved its safety performance markedly over the past decade, but is still facing major challenges (Figure 1). These include meeting the Mine Health and Safety Milestones (Department of Mineral Resources, 2011)."

Titanium can exist in three different oxidation states. Its oxides are numerous and in presence of iron and oxygen, iron-titanium oxide compounds can also form. Some of the oxides are non-stoichiometric, making the determination of the thermodynamic properties of titanium and its oxides in molten steel and in slag difficult. Therefore, for the assessment of titanium activity in molten steel using the electrochemical measurements of oxygen partial pressure, the identification of oxides in equilibrium with titanium is important. This paper reports on an investigation of solid titanium oxide inclusions, which have been identified by combining different methods of analysis on quenched samples. These samples were taken from a Fe-Ti-melt at 1600°C, quenched in cold water then mounted and polished for optical microscopy. The oxide inclusions were analysed using transmission electron microscopy (TEM) with energy dispersive spectroscopy (EDS). Both single crystal and polycrystalline diffraction patterns were used to identify the oxides by comparing the d spacings with those in the ICDD X-Ray Diffraction (XRD) database. XRD was used to analyse the oxide particles on the top of slow cooled samples. It was found in the TEM study that the similarity of the diffraction patterns of the various phases meant that unique identification was difficult to achieve in some of the diffraction patterns. However, TiO, Ti2O3, Ti3O5, Ti4O7were identified in quenched samples, while TiO2, FeO.TiO2and iron oxides were seen in slow cooled samples. The mineral exploration environment has observable economic characteristics, the assessment of which provides guidelines for exploration planning in the mining company. The long-term survival of the mining company is directly dependent on successful exploration. Exploration planning contributes to successful exploration through the development of exploration strategies. The objective of exploration planning is to select exploration environments which will enable the mining company to realize best possible profit and corporate risk conditions. Thus, the formulation of exploration strategies is based on profit and corporate risk criteria. Strategic considerations include the selection of commodities as exploration targets, geographic concentration of exploration effort, the realization of acceptable survival conditions, and the structuring of an exploration programme to optimize the economic benefits of the exploration investment process.

In recent years, the mineral applications dealing with more complex data in distributed environment have grown rapidly. Many mineral and mining organizations have to organize internally and globally distributed manufacturing and experiment sites, which share the roles of databases and other multi-services like software programs. Thus, there are commercial and technical pressures pushing the mineral software vendors to move their current service toward Web service. XML has gained wide acceptance for representing, storing and transferring structured data and documents of many kinds, especially in scientific communities. However, so far there is no such a Markup Language in mineral industry. A XML-based uniform encoding, which is called MineML(Mineral Markup Language), is proposed and developed in this project. The framework of the MineML specification is created. MineML will play the role as shared protocols within mineral domain for archiving, transferring the multi-nature data between the software packages on the desktop over the Internet and sharing the services between users at different sites. Inspired by MLA’s (Mineral Liberation Analyser, JKMRC, University of Queensland, Australia) Mineralogical Data Presentation Program, a proof-of-concept prototype of Web Service is developed here to validate and demonstrate the MineML specification. Certain strategies, specific technologies and tools are adopted to create such an environment. To accomplish that, a set of experimental MineML documents including correlated XML, XSD (schemas) and XSL(stylesheets) documents have been elaborated for archiving, presenting and transferring the mineralogical data information, with the persistence on flexibility and extensibility for generic mineral and mining data handling.

A simplified economic model is derived for the financial return from a new gold mining project. The model is used to analyze the effects of changes in the mining rate, capital costs, working costs, value of the reef and the stoping width. The most effective avenues for improving the return from a new project are indicated. It is shown that those technological innovations which would improve the return significantly, and therefore make it possible to open more mines, are the use of smaller mines, means for improving the mining rate to capital costs ratio for smaller mines, planning and control of mines by computer simulation, an improved mine valuation procedure and means for mining at a decreased stope tramming width.

Mining companies are challenged with increased difficulty in delivering capital projects on schedule and within budget. In 2005, PricewaterhouseCoopers global review of major capital projects in the mining industry only 2.5% of projects could be defined as successful when assessed across the four critical dimensions of scope, cost, schedule and business benefits. Thus 97.5% of major mining capital projects were not successful according to the criteria PricewaterhouseCoopers used. Normally if a project fails on all these criteria it indicates scope creep. Scope creep refers to the change in a project's scope after the project work has started. As a result, the projects drift away from their original design, timeline, and budget, and as a consequence destroy business benefits. The major reasons for scope creep are poor change control and the lack of proper front end loading. Change control and front end loading are based on project processes in the planning and monitoring & control process group of the Project Management Body of Knowledge1 (PMBOK). This paper will focus on key factors to consider during project scheduling/planning with special reference to cost and schedule (using Microsoft Project as a tool although the principal is universal)

Sishen Iron Ore Mine is situated 280 km north-west of Kimberley in the Northern Cape Province of South Africa. The mine produces and sells high quality iron ore of various specifications to various national and international clients. The saleable product is derived from the blending and beneficiation of different types and qualities of hematite iron ore. This paper documents the new developments which were implemented (or are in the process of being implemented) on the mine in the long-term planning, medium-term planning, grade control and material tracking in the beneficiation plant with special attention being paid to the quality of the final, saleable product. The process starts with the long-term planning where the pit shell and pit optimization is determined by Whittle 4X. New developments include the introduction of a linear programming based scheduling system to enhance the long-term schedule generated by Whittle 4X. The LP route was decided on in order to take into account all the constraining factors of the final product that could not be configured in Whittle. To complement this the XPAC Auto Scheduler scheduling package was introduced to do the medium-term mine scheduling. This 18-month rolling plan includes cost parameters to enable the planner to extract a detailed operational budget from the scheduling while ensuring that production standards are maintained and grade constraints are met. The existing grade control system was enhanced with GPS capabilities and a computer system to do material tracking. The high precision GPS system introduced on the shovels allows for the accurate control of the loading procedures at the mining face. Because of the complicated geology and grade composition of blasting blocks it is imperative to accurately differentiate between different materials in the same blasting block and being able to excavate that with the loading equipment. The GPS system on the trucks allows for accurate location monitoring of dumping in order to keep track of specific material types. The state-of-the-art material tracking system developed for Sishen will enable the grade control officer to keep track and record various parameters of the material from its in situ position in the blasting blocks, through the crusher and onto different stockpiles, in various stages of the beneficiation process and onto the blending beds, into the dispatching trains and eventually into the ships for international distribution. These developments elevate the Sishen planning and grade control systems to a new level of service excellence from which management can obtain accurate and timely information on product status. It also enables Sishen to maximize resource utilization while still complying with the tough market constraints.

Beatrix Gold Mine, a deep-level gold mine in the Free State province of South Africa, has the highest methane emission rate of any gold mine in the country. Methane is emitted from underground sources intersected during mining operations and is liberated into the general mine atmosphere. The total methane emission rate for the mine is of the order of 16001/s and the South section of the mine emits approximately 10001/s of methane gas. The mine has a history of gas accumulations which have led to a number of underground explosions. Following the last explosion in 2001 and the subsequent investigation, a number of recommendations were made. Two of these were to consider extracting the underground mine methane to render the mine atmosphere safe, and to declare hazardous locations that require special operating procedures. A number of such workplaces have now been declared at the South section of the mine. Methane gas is a potent, explosive greenhouse gas whose contribution to global warming and climate change is 21 times higher than that of carbon dioxide. To reduce its inherent danger and to mitigate its global warming impact, a carbon credit project under the Clean Development Mechanism (CDM) of the Kyoto Protocol has been developed and implemented, the aim being to capture and destroy the methane at Beatrix mine. The mine has constructed an extraction system to capture and extract 400 (Is of the methane. Percentage methane (per volume) of the gas intersected at source is 85%, with negligible concentrations of other hydrocarbons and water associated. The mine contracted the services of Group Five to design and construct the flare and ancillary equipment on surface, and Promethium Carbon was contracted to assist with the carbon¬-related aspects, approval framework, and administration of the project. A number of design and construction challenges had to be faced to extract and transport the gas effectively to the surface of the mine as the emitters are approximately 3 600 m away from the mine shaft at a depth of 860 m. Further considerations were the requirements for the type of column to be used to transport the methane gas, the pressure loss over the system, the safety systems needed to address the risk factors involved in the transport of the methane gas, and a suitable pumping system to extract the gas to the surface. The system operates under negative pressure provided by two blowers on surface delivering the emissions to a flare capable of burning off 450 8/s of methane gas. In this paper a number of benefits for the mine are discussed. These include, but are not limited to, the removal of approximately 55% of the total volume of methane gas from the general body of the air in the geographical areas of the mine where the methane gas is emitted into the atmosphere, and reducing the risk of methane-related incidents. A further benefit of this project is the mitigation of the global warming impact of the methane gas and the reduction of the mine's carbon footprint by approximately 25%. This project could also assist in alleviating the energy shortage being experienced in South Africa by means of the planned generation of 4 MW of electrical power, utilizing the methane gas, as a second phase of the project.

Frithia humilis is an endangered succulent plant species. Its distribution range overlaps with the coal fields of Mpumalanga and it is therefore threatened by opencast mining activities in particular. One of the 11 known populations of F. humilis was translocated from a licensed coal mining site to three suitable receptor sites within the species? distribution range. A longterm monitoring programme was initiated to track the progress of the newly established populations. Temporal trends in population demography, size classes, and fecundity were recorded. Population numbers of size classes fluctuated annually. However, flower frequency increased over time and seedling recruitment contributed significantly to population growth. Receptor sites with similar geological conditions to the donor site had more persistent cohorts, which suggest that such sites should receive priority during the translocation of endangered edaphic specialists. This study not only confirms that a Frithia humilis population can successfully establish after a translocation, but also serves as an important baseline for future comparative purposes to gauge the long-term success of the translocation effort.

KCM made a major modification to the Nchanga Tailings Leach Plant (TLP) changing the circuit from feeding post leach thickeners in parallel with leach discharge for solid ?liquid separation to a Counter Current Decantation (CCD) configuration utilising most of the existing equipment. The new circuit is aimed at improving copper recovery from approximately 70% to 79% through enhanced wash efficiency. The re-introduction of CCD circuit at elevated throughput of 46 000-tpd design was made possible due to the EIMCO E-Duc technology design which is an auto-dilution system that enhances solids settling rate. Commissioning (manual operation) which accommodated on-going operational requirements was phased and completed in January 2004. The circuit stabilised in subsequent months and in March alone an all time record copper recovery of 81.8% was achieved which contributed an extra 779 tonnes copper above the plant?s projected production plans. 100% of the project cost was paid back by end of April 2004 This paper discusses the work involved and the challenges faced during the re-commission

The management of the Anglo Platinum (AP) mining technical systems (MTS) embarked upon a process of defining the strategy for the MTS department. The newly established department forms part of the greater Group Information Technology (IT). The strategic process took place within an environment that had witnessed a dramatic proliferation of IT implementations across the group within the last three years. The IT department has been structured in terms of the classical plan, build and run functions with the applications and integration streams running perpendicular across these three functions. The application stream is divided into commercial and mining technical systems with business systems integration the third stream. The mining technical systems department supports mine management to effectively gather, analyse and disseminate data relating to the ore reserve by integrating a number of strategically selected IT applications to ensure that the safety, legal and operational requirements are met. This service to business is rendered by management of a strategic outsource partner via tightly controlled service level agreements (SLAs). The framework used for creating a mining technical systems department that is empowered to add value to business was formulated using the AP value chain as both the starting point and guiding process to ensure alignment to the greater internal strategy of the business. Horizontal rotating mills are being used all over the world associated with different mineral and power generation industries. David Brown South Africa, with over 140 years of experience in gear manufacture and design, has been supplying these industries with innovative designs at exceptional quality. The gearing systems associated with these industries are challenged daily with operating conditions with are not always fully understood. Failures are therefore experienced, over time in the industry, which can therefore not always be fully explained. David Brown South Africa has developed a gear monitoring system, the Intelligent Gearing System, in association with Transmission Dynamics (UK), in order to monitor and analyze these operating conditions in real time. The system has been designed on the principles of Six Sigma, incorporating Design for Six Sigma. The purpose with this system design is to monitor the operating conditions on the pinion, record the information and analyze the data in relation to the rate of fatigue damage to the girth gear and pinion. This is done in real time and unattended. The output from the Intelligent Gearing System can be fed back into the operating system of the mill user, in order to display an alarm for operating conditions exceeding the design criteria as well as information on the rate of fatigue damage due to these operating conditions. When the mill operating conditions are better understood, and the effect on the fatigue life of the mill gearing components, mill users can forecast critical spare holding levels with an increased accuracy. The Intelligent Gearing System also allows early actions to remove reasons for system overloads.

Optimization of panels, haulage fleet, and waiting area involves deterministic and low-fidelity methods and experiential knowledge. The process is challenging because coal recovery and operational capabilities must be considered in the solution. The approach in this manuscript comprises the development of an integrated stochastic simulation model of a coal room-and-pillar system that addresses these challenges. The decision variables evaluated are panel width, number of shuttle cars, and change-out time (COT). The results show that the mine should implement the shortest possible COT, decreasing the cycle time and thereby increasing productivity and continuous miner (CM) utilization. The highest productivity and CM utilization for a fleet size of three shuttle cars is found in the 15-entry panel width. For the evaluated fleet sizes, the 19-entry panel width is optimal for the four and five shuttle cars. Among the three variables studied, panel width and fleet size had the most significant effects (5% increase) on the CM productivity, cycle time, and utilization.

Main ore pass systems should last the life of a mine, and too often expensive and hazardous remedial measures are required to repair a damaged ore pass in an attempt to extend its life. In order to maximize the life expectancy of all ore passes at Moab Khotsong, ?wearing blocks? and ?softer support? systems, combined with basic rock engineering principles, have been implemented. Failure of an ore pass can have severe financial implications, both indirectly because the ore pass cannot deliver the broken rock either from level to level or directly in the shaft ore pass system to the required level for tramming to take place, and directly due to the cost of the repair work that is required. Several factors need to betaken into consideration so that production interruptions are eliminated or minimized. Investigations of damaged ore passes at Moab Khotsong and surrounding mines revealed some interesting mechanisms of damage, and some innovative methods of reducing or eliminating the damage have been implemented at Moab Khotsong. The system design was based on 30 years, experience in main shaft rock passes. The design was to counteract scaling, hang-ups and dust problems associated with main rock passes, and the objective was to increase the life of rockpasses. These problems, including the initial design aspects to try and eliminate the repair measures, are discussed in detail.

The limitation of fossil fuel resources and the necessity of reducing CO2emission require an increase of the efficiency of power plants by using combined cycle power systems. Pressurized Pulverized Coal Combustion (PPCC) is a potential concept to realise these objectives. The direct use of hot flue gas for driving a gas turbine requires a hot gas cleanup to achieve corrosion prevention of the turbine blading. One of the main problems is the alkali release during the coal combustion process. Molten coal ash slag has a high potential for alkali retention which depend son the alkali oxide activity in the slag. Knudsen Effusion Mass Spectrometry (KEMS) was used to determine thermodynamic data of alkali gas species over molten coal ash slags and of alkali oxides in molten coal ash slags. The main interest of the investigations focused on the structural dependence of the thermodynamic properties of the alkalis. It was found that there is a strong correlation between the thermodynamic properties and the ratio of nonbridging oxygen to tetrahedral oxygen (NBO/T). An increase of the polymerization of the slag results in a decrease of the alkali oxide activity in the slag and thus a decrease of the alkali partial pressure over the slag. Keywords: Pyrometallurgy, furnace, copper coolers, freeze lining, monitoring, Saveway The use of water-cooled copper components as sidewall elements of submerged arc furnaces and other pyrometallurgical applications has become increasingly common. The high cooling efficiency of copper has made it possible to increase the service life of furnace refractories when compared to conventional refractory linings. However even the self-healing effect of ?freezing? worn areas of the lining does not necessarily protect the copper coolers against wear or corrosion completely. In order to avoid furnace explosions, various methods have been used to monitor the wear of coolers. Unfortunately, these have not been able to consistently identify localized wear. Thos Begbie (South African) and SAVEWAY (German) jointly developed technology to embed a line sensor into the copper cooler during the casting process. This system allows an exhaustive monitoring of the copper cooler. A SAVELINE sensor is placed inside the copper-cooling panel between the water-cooling passage and the hot face of the panel. The sensor follows the meandering shape of the cooling water channels. In the first instance, the sensor provides analogue information as the thickness of the copper reduces. Secondly, this wear signal will be confirmed by a signal of interruption if the sensor is washed away. Because of the structure of the sensor, a self-diagnosis is possible and indications can be confirmed (double proofed). Subsequent to the manufacture of some test pieces, coolers for a large submerged arc furnace were manufactured. The sensorized coolers have been installed at the intermittent surface (slag line) where the largest amount of corrosion has been encountered. The system has been operating since December 2005 in what is considered one of the largest submerged arc furnaces in South Africa.
