This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. This chapter summarizes data on crude mineral production for the United States, its island possessions, and the Commonwealth of Puerto Rico. Included also are tables that show the principal mineral commodities exported from and imported into the United States, and that compare world and U.S. mineral production. The detailed data from which these tables were derived are contained in the individual commodity chapters of volume I and in the State chapters of volume II of this edition of the Minerals Yearbook. Although crude mineral production may be measured at 'any of several stages of extraction and processing, the stage of measurement used In this chapter is what is normally termed "mine output." It usually refers to minerals or ores in the form in which they are first extracted from the ground, but customarily includes the product of auxiliary processing at or near the mines.

A general description of the activities and results of bauxite investigation by the Bureau of Mines at its bauxite project in Saline and Pulaski Counties, Ark., has been given in Report of Investigations 4251. In this volume and in a series of Reports of Investigations immediately following the factual data obtained during the project are reported. The data are accompanied by maps showing the location of holes drilled and the deposits developed. Each area upon which work was done is reported by half townships. Each bauxite deposit investigated was assigned a name and a number for ready reference and identification, and its location und name are shown on the appropriate half-township map.-The description of each half township is followed by a description of each deposit developed in that half township. The 14 half-township maps and 77 detail maps of the bauxite deposits in this eeri88 of reports give the locations and numbers of all the holes drilled on the project except the deep holes drilled to test the geophysical anomalies. The locations of those holes show on figure 13 in volume 1 (R. I. 4251). The descriptions of the deposits developed are followed by complete detailed logs of the drill holes. Many of the widely spaced exploratory holes did not discover bauxite of minable thickness and grade. The pertinent information obtained from the widely spaced exploratory drill holes has been summarized and tabulated, and a table tor each half township is given with the discussion covering that area.

One of the lithium minerals most widespread in the earth's surface is spodumene (Li20-A1203-4Si02), a lithium aluminium silicate. For some time, deposits of this mineral were known to exist in the Black Hills of South Dakota, but it was only quite recently that Hess4/ pointed out the great magnitude of the low-grade deposits in forth Carolina. The theoretical lithia (Li20) content of spodumene is 9.04 percent. The only other minerals with such high lithia content are the phosphates, but these occur only in pockets and are not in extensive deposits5/. Actually, however, the lithia content in spodumene is never found to be this high, being nearly always below 7 percent for the pure mineral, and part of the lithia is apparently replaced by other alkalies.

This report presents a literature review conducted by the U.S. Bureau of Mines. The review summarizes what has been learned from the large volume of empirical studies on the effectiveness of eight types of interventions intended to increase organizational effectiveness via in the management of human resources. The interventions concern the following: recruitment, selection, training, objective feedback, goal setting, reward systems, participative management practices and organization development. Although many of these interventions appear to have the potential for improving the safety and efficiency of mining operations, it does not appear that they are being widely used. Examples are given of the successful use of a few of these interventions at mining operations. An analysis of the relationship between coal mine productivity and safety is also presented. This analysis generally supports the view that good safety performance and high productivity are NOT incompatible goals. In fact, safe mines also tend to be more productive and vice versa.

On December 27, 1961, a fire and explosion destroyed the blasting agent mixing plant and an attached storage barn of the Whitaker-Atlas Supply Co., near Norton, Wise County, Va. Five residences, an office building, four trucks, and three automobiles were completely destroyed; a house trailer and several other trucks and automobiles were severely damaged; and other property damage, including much window breakage, occurred up to 3 miles or more airline distance away. Through the quick action of the fire marshal of the Norton Fire Department, Norton, Va., there were no fatalities and only five persons required hospitalization. This incident is of particular interest because of the widespread practice of mixing fuel oil (FO) with ammonium nitrate (AN) to prepare low-cost blasting agents, in many cases under conditions and circumstances not very different from those presented here. A representative of the Bureau of Mines Explosives Research Laboratory, Pittsburgh, Pa., and field representatives of the Bureau's Health and Safety Activities conducted the investigation. Information in this report was obtained from representatives of the Whitaker-Atlas Supply Co., the Norton fire marshal, and others who were on the scene either shortly before or after the incident occurred. Representatives of the Manufacturing Chemists' Association, the Institute of Makers of Explosives, and individual ammonium nitrate producers participated in the inquiries and investigations.

This report describes a program of research to evaluate methanometry in coal mines. The program commenced by conducting an extensive state-of-the-art survey. Details of the survey are presented which includes a rating of various types of instruments that can be used to detect and record methane gas. Laboratory test data, on mine instruments tested are presented together with results from a computerized statistical analysts.

Mex., is one of a sories being prepared by the Mining Branch of the Bureau of Minos. on mining method a and practicos in, various mining districts of the United States. The mine shafts and. surface plant or International Minerals & Chemical Corp. are situated. in. sec.. 12, T. 22 S., R. 29 E., N. M. P. M., about 26-1/2 miles by paved highway east of the city of Carlsbad (figs. 1, 2, and 3). A spur line of the Atchison, Topeka & Santa Fe Railroad connects the mine with the Clovia-Pecos branch of the same road near Loving, a few miles south of Carlsbad. This spur also serves the refinery of United Staics Potash Co. This report gives. the history and describes the geology of the district. Methods of prospecting, exploration, sampling, and estimation of tonnage and values are explained; development and mining methods are discussed in con- siderable detail; transportation, ventilation and drainage are outlined; safety measures are noted; and the wage scale and organization are mentioned.

One phase of Bureau of lines work in health and safety is promotion of the use of safe, satisfactory, and reliable respiratory protective devices. To insure that devices embodying these features will be available the Bureau has established schedules of minimum requirements for oxygen breathing apparatus, gas masks4/, supplied-air respirators (hose mask, air-line respirators, and abrasive blasting helmet hoods, or masks)5/, and dispersoid (dust, fume and mist) respirators6/. Approval test, of oxygen¬-breathing apparatus are conducted by the Safety Division; approval tests of other types of respiratory protective equipment are conducted by the Health Division. These divisions comprise the Health and Safety Branch.

A study of damping capacity was made on a number of magnesium-base alloys, commercial types and experimental compositions. Hardness and tensile properties were determined, and the microstructure of each alloy was examined. Alloys of low yield strength, in general, possessed high damping ability. A correlation was established between damping capacity and composition.

When most people think of explosives transport they think of a hazardous operation. We all know that we aren’t allowed to transport explosives through tunnels and are discouraged from traveling through heavily populated areas. Popular opinion suggests that explosives transport is a very risky operation. The authors decided to look into this a little more thoroughly and found just the opposite to be true. While there have been some disastrous world-wide accidents involving the transport of industrial explosives, this is not the case in the U.S. Over the past 10 years, accidents related to the transport of explosives used in mining and construction have resulted in 5 major injuries, 11 minor injuries, and no fatalities. Explosives and ammonium nitrate (AN) transport outside the U.S. has not had such a good record; there have been 4 major explosives or AN transport accidents resulting in a total of more than 300 deaths. Most of these fatalities could have been prevented if the accident site had been evacuated once the explosive or AN cargo began burning. The safe history of explosives and AN transport in the U.S. is due to diligent efforts by government, labor, and industry. However, accidents will become more common and disastrous if we become complacent.

The Minerals Data Source Directory is a compilation of descriptive information on the availability, content, and location of minerals data in the Federal Government. It is intended to assist Government personnel and the general public in locating and acquiring minerals information. Included in the Directory are descriptions of data bases, publications, information systems, and information offices containing data pertaining to all aspects of mineral production, consumption, trade, and related information. These mineral data sources include the Federal agencies such as the Department of the Interior's Bureau of Mines and Geological Survey whose major mission is the collection and dissemination of mineral information. Also included are agencies such as the Departments of Agriculture, Commerce, Labor, State, and Treasury, and the Central Intelligence Agency, whose primary mission is the collection of data in non mineral subject areas, but in which the mineral industry is a component.

Commodity transportation by barge is possible on about 25,000 miles of navigable inland waterways in the contiguous 48 states. Barge operations are used chiefly for the movement of bulk raw materials ranging from farm products to iron ore, coal, and other bulky semi-finished as well as finished products such as steel, chemicals and petroleum [3,p.7-9]. The barge industry, unlike other transportation industries, is largely unregulated. This is because all liquid bulk commodities most dry bulk commodities transported by for-hire carriers and companies engaged in private transportation of their own commodities are exempted from Interstate Commerce Commission regulations. As a result, about 15 percent of the total ton-miles of barge traffic each year is under Interstate Commerce Commission regulation [13,p.14-25].

"Gas hazards may be divided into two principal classes- namely (1) inflammation and explosions; (2) asphyxiation and poisoning. Inflammation and explosion are prevented first by adequate ventilation so that gas sufficient to propagate flame or explosion can not accumulate, and second, by avoiding sources of ignition such as open lights, flames, sparks, electrical arcs, short circuits, or sparks. Ventilation is also the best preventive measure against asphyxiation and poisoning from toxic gases. When men must enter unventilated or poorly ventilated places containing poisonous or asphyxiating gases, suitable gas masks, hose masks, or oxygen breathing apparatus should be worn. As a rule, loss of life and property from gas hazards is due to ignorance of the properties and hazards of various gases commonly used, and consequently a failure to use proper measures for eliminating and meeting these dangers. It is the purpose of this paper to describe briefly the common gases and vapors with respect to the hazards involved in their use and to discuss the necessary precautions and methods for avoiding these dangersLight, heat, and power are prime essentials of our modern industrial civilization. The combustible gases and vapors used for the production of these necessary elements of our existence comprise the largest and most common group of domestic and industrial gas hazards. Almost every individual in his daily life subject to possible danger from asphyxiation, fire, or explosion due to one or more of these gases. For evidence one need only refer to the daily newspaper where headlines such as the following are frequently seen; ""Family asphyxiated by carbon monoxide from gas heater;"" ""Schoolhouse wrecked by gas explosion in basement;"" ""Man killed by automobile exhaust fumes in his garage;"" ""Fireman overcome by smoke fumes in basement ""Woman"".""Boman severely burned by accidental ignition of gasoline used in cleaning gloves;"" and 'in recent Years we often see ""Building wrecked by explosion of still in residential district.""It will be noted that these accidents are due to inflammation, explosion, or asphyxiation. Any combustible gas or vapor, when mixed, with air or oxygen within certain limiting proportions, will explode on ignition. The violence of the explosion varies with respective proportions of air and gas, and with the degree of confinement of the mixture. For example: Mixtures of natural gas and air containing between 4.5 and 12 per cent natural gas will explode, or propagate flame throughout the mixture without the continual presence of the source of ignition that started the inflammation. The table following gives the explosive limits of the common combustible gases and air:"

The last issue included a discussion of recent indications of a 1983 recovery and a brief analysis of the contribution of the U.S. mining industry to the gross national product (GNP). In this issue we present sales and other data indicating the strength of the 1983 recovery and quantitatively analyze the structural shifts that have taken place in the U.S. economy and mining industry since 1972, and changes in electricity consumption by the mining industry. U.S. Economic Recovery The tentativeness in the February/March issue of Minerals and Materials regarding the strength and sustainability of the U.S. economic recovery can now be replaced with a note of increased confidence. Real growth of GNP on an annual basis in the first quarter of 1983 was 2.5 percent, compared with 1.4 percent predicted in the Economic Report of the President.1/ Preliminary second-quarter 1983 data provide further cause for optimism. Recently released data indicate that consumer spending is playing a key role in the recovery. Retail sales have increased significantly across a wide array of goods, especially consumer durables, where sales increased 4 percent from March to April (table J) and were 12 percent higher in April [ ] than the 1982 monthly average. Consumer durable sales were led by sales of building materials spurred by the growth in residential construction, motor vehicles and parts, and household durables, including appliances and furniture.

Diamond, drilling of some blast holes in stoning operations is practised in at least two iron mines in the Lake Superior district - the Soudan mine of the Oliver Iron Mining Co., on the Vermilion range at Soudan, Minn., and the Anvil-Palms mine, operated by Pickands, Bather & Co., on the Gogebic range at Bessemer, Mich. Diamond drilling, at the Soudan nine has partly replaced percussion drilling because of the extreme hardness of the ore and of the lean formation. According to Wolff,4/ the jasper or lean formation in this mine is the hardest rock regularly drilled in any commercial operation.

The Federal Bureau of Mines has conducted research to delineate factors affecting relative fabricability of electrowon and electrorefined copper. The Bureau's Rolla Metallurgy Research Center coordinated and participated in a statistical round-robin chemical analysis and performed impact tests on lead-doped copper specimens. In a series of round-robin analyses, cathode, bar, or rod from a domestic lot of electrorefined copper and electrowon copper from a domestic and a Peruvian source were analyzed for Pb, Fe, Bi, Se, Ag, O, S, Sb, and seven other elements. The electrowon cathode from both sources surpassed requirements for electrolytic cathode, although lead in the Peruvian lot exceeded specification for oxygen-free electronic grade. The least interlaboratory agreement was for oxygen analysis, Copper specimens, doped with up to ~300 ppm Pb and consisting of as-cast material (fine-grained or coarse-grained) and hot-rolled material, were impact-tested at temperatures to 871° C (1,600° F). Impact energy of fine-grained material dropped 26 pct with increased Pb up to 20 ppm and more gradually thereafter. Coarse-grained specimens fractured at lower energy with little dependence on lead. Hot rolling raised impact energy substantially. Grain structure is evidently as significant as lead in influencing workability. Some discussion is given of possible effects of oxygen, sulfur, and porosity.

Exploration of hidden mineral deposits becomes increasingly important as known ore reserves are depleted. The primary objective in any exploratory program is to obtain enough reliable information to evaluate accurately the potential of the mineral deposit. Constant improvement in equipment and technique has established drilling as one of the most valuable methods of exploration. This paper presents the results of studies of several drilling methods currently used in the search for uranium deposits in sedimentary formations from surface drill sites. Also presented is a description of drilling with wire-line equipment from an underground location in metamorphic rocks. The studies were conducted in the Colorado Plateau area and in northwestern New Mexico and Wyoming. Statements of a general nature pertain to conditions or situations existent in those areas.
