Investigation of the costs of industrial accidents dates back to the 1920s. The Bureau of Mines was one of the pioneers in accident cost investigation with numerous studies published between 1928 and 1933. The bulk of these early investigations dealt only with costs which appeared as payouts on company books; that is, the direct cost of insurance premiums and the costs of medical expenses which resulted from industrial accidents. These early investigations, especially the Bureau's study of accident costs in Pennsylvania and Indiana coal mines provided the first real evidence that promotion of industrial safety pays back in dollars as well as in human life and wellbeing. In the 30s and 40s, investigations began to accumulate evidence that industrial accidents impacted more than just the direct operating expenses of a company (i.e., higher insurance premiums). The work of Heinrich pioneered these investigations, and produced strong evidence that the un¬insured costs of accidents far outweighed those costs covered by insurance. Heinrich developed and published a ratio of insured to uninsured costs of 1.4. Despite his cautionary statements about blanket application of this ratio it is used even today to estimate total costs of accidents. Simonds, in the late 40s and early 50s, extended the definitions of uninsured or indirect accident costs and developed what is now regarded as the principal method for estimating the total economic impact of industrial accidents. Simonds' method served as the basis for development of the logic of the Accident Cost Indicator Model (ACIM). However, several departures from Simonds' method were made to satisfy certain requirements imposed by the Bureau of Mines, and because information gathered during the course of our investigation justified a departure. If you read any of the ?Because of Human Error? text boxes in this document, situations like these may look familiar in your own workplace. ?The issues of risk, health, safety and the environment are getting more attention now than ever before, at all levels of the organization,? said Calvin Price, vice president of Global Health and Safety at LNC-Lavalin Inc., a company with 21,000 workers worldwide. Most would agree with Price that attention to safety is increasing because responsible companies care about their workers and the workers? colleagues, families and communities. However, cynics might say that the safety spotlight is powered by the corporate bottom line. Direct costs created by workplace injuries total $51.8 billion annually and include workers? compensation payments, medical expenses, civil liability damages and litigation expenses. This sum represents a staggering one quarter of each dollar of pretax corporate profits. Indirect costs may run as much as 20 times the direct costs of on-the-job accidents and illnesses. Examples of indirect costs include training replacement employees, investigating the accident and implementing corrective measures, lost productivity, repairing damaged equipment and property, and costs associated with lower employee morale and absenteeism. What?s interesting is that up to 90 percent of incidents are due to human error, not faulty equipment or other factors.

Sunshine Mining Co.'s Kellogg operation is the primary silver producer in the US. Maintaining a comfortable working environment in this deep underground mine historically has been a complex and costly problem. To improve the ventilation and cooling systems at a time of declining and un-certain silver markets, it was proposed to optimize the performance of existing facilities at minimum cost. This paper discusses a two-year, $400,000 optimization program that resulted in increased total mine airflow, cooler ambient temperatures, reduced compressed air usage, and lower energy consumption by primary fans, air compressors, and refrigeration plants. This saved the company about $650,000 per year.

Supercritical fluid extraction (SFE) has became an attractive alternative technology to be a new process to recover metal, of which can be substitute the existing Leaching/Solvent extraction processes. Solvent extraction of metallic ions is a technically important and also well-established process that is used in the chemical and hydrometallurgical industries.1 Solvent extraction techniques use the chelation of metallic ions dissolved in liquid solution with organic ligands. Conventional solvent-extraction technique needs large amount of organic solvents and often creates environmental problems.2 More environment-friendly technology is needed nowadays, and there have been increasing attentions to extract metal ions by SFE. CO2 becomes one of the most notable alternative organic solvents due to its nonflammable, nontoxic, nonpolluting, and relatively inexpensive properties with its moderate critical point (73.8 bar, 31.06 °C). Many works have been reported in metal extraction under the supercritical CO2 (scCO2) solvent.3-5 In this study, to recover cobalt from wastes especially from spent lithium ion battery scrap, we selected two ligands ? Cyanex 272 and DEA (diethylamine) for the metal extraction in scCO2. Cyanex 272 is a commercially available extractant along with its series, Cyanex 301 and Cyanex 302. Smart and coauthors measured the metal extracting abilities of these three extractants, and found that Cyanex 301 and Cyanex 302 extracted metal ions much better than Cyanex 272 in the scCO2 solvent.10 The pH of water contacted with high pressure CO2 (70-200 bar, 25-70 oC) is about 2.8-2.95 based on the calculation and the measurement.6-7 Cyanex 272 is known to be less effective as a metal extractant in this low pH environment compared with other Cyanex series. However, when DEA is added to metal extraction reaction with Cyanex-272, DEA works as a strong base to Cyanex 272 converting Cyanex 272 to a deprotonated form (Figure 1), which results in an easy formation of metal complex and the enhanced extraction rate of metal ion.8 Therefore, the purpose of this paper is the metal extraction from spent lithium ion battery waste using scCO2 with Cyanex 272 and DEA.

Clay mineralogic studies of ore versus barren rocks in the Grants mineral belt show that some combination of chlorite (rosette form), illite, mixed layer illite-montmorillonite, (- Mg-montmorillonite) are penecontemporaneous with uranium minerals in trend ore; these same clay minerals plus kaolinite are related to the roll type ore near the main redox front of the Grants mineral belt. Clay minerals from barren rocks are characterized by a greater abundance of Na-montmorillonite, kaolinite, and face-to-edge form chlorite. Chlorites from ore zones contain much more vanadium than do chlorites from barren rocks. Trend ore probably formed from southeasterly flowing waters following paleochannels in the Late Jurassic. These deposits are found almost entirely in reduced rocks, and organic carbon may have been an important reductant to remove U-V-Se-Mo from solution as carbonate from ore zones contains some organic carbon based on stable iso¬tope studies. Uplift, remobilization and reprecipitation of some of the trend ore resulted in the formation of redistributed ore, some of which possesses a roll type geometry. Mineralization for the roll type ore was apparently controlled by sulfide-sulfate equilibria at or near the main redox front in the Grants mineral belt. Trend and roll type ore possess different assemblages of clay minerals and different trace element abundances. Laramide faults cut both trend and some roll type ore. Stack ore is found in Laramide fault zones. Limited oxygen isotopic data from clay minerals in two mines at Ambrosia Lake in reduced rocks indicate probable preservation of ancient, formational waters and no evidence for infiltration by young meteoric waters. This information, plus the pre-Laramide faults, suggest, but do not unequivocally prove, that the main redox front has been relatively stable since its formation, probably some time in the Cretaceous. Younger encroachment of the redox front in post-Laramide is proposed by others, however, and the problem is unresolved. Uranium for the deposits was most likely carried in solution as an oxyion, probably as (UO2(CO3)2.2H2O)2- or (UO2(HPO4)2)2-, although organic transport cannot be entirely ruled out. Oxyions of vanadium, selenium, molybdenum and possibly arsenic and antimony, are interpreted from Eh-pH diagrams to have been segregated and transported with U6+ soluble species and precipitated when a chemically reducing environment was reached. The rare earth elements are also enriched in ore zones, but it is not clear if they were transported with the U-V-Se-Mo-(As)-(Sb) suite or derived from a more local source.

This section of the Handbook deals with general principles to be evaluated in the development of a mining operation. It addresses the major issues that must be examined to fully evaluate a mining venture. Of course, one of the main topics to be evaluated is mine planning, which is covered in more detail in other sections (13, 17) of the Handbook and should be referenced in mine development projects. The topics to be discussed in this introduction include equipment and supplies, infrastructure, utilities, and project management. In any of the aspects mentioned, the current existing conditions at or near the mine site must be evaluated. This will include assessment of current use of utilities, infrastructure, housing, equipment suppliers, etc., as well as anticipated growth, both in the supply aspect of these systems as well as projected usage on these systems. After full assessment of both these aspects, an evaluation must be made as to the requirements the project will place on these items. Assessment must be made as to any up-grades required to bring the project on line, what impact the project will have on existing or expanded systems, and anticipated future growth impacts on support systems not only of the project but of other surrounding industry and urban devel-opment

The film you see is a slow motion view of a typical half million ton taconite blast at one of Erie's mines. This blast is made up of 220 drill holes and a total of 325 thousand pounds of blasting agent. The second view of the same shot is taken by a remote control hi speed camera located 1500' directly in front of the blast. Shortly you will see some violent camera movement as the air blast shock wave strikes the camera location. As the blasting sequence progresses the rock fly becomes evident, and if you will look closely amidst the clouds of smoke, rock fragments weighing up to 1/2 ton begin to cascade into view. These missles are the chief concern of blast security. The next camera shot at 4000 frames/second vividly describes the primacord firing lines detonating the blast. Travelling at 22,000 ft./sec., this white snake of fire travels to each hole, detonating the blasting agent. Before we get into the matter of blast security, let me locate our operation for those of you not acquainted with Minnesota's iron ranges and tell you something about our company.

Unfortunately, the term "control" has a negative connotation in the minds of many of our operating plant personnel. Whereas the opposite is true of those associated with research and technical centers in our industry. The magnitude of this difference in opinion and actual experience has often proved to be a major factor influencing the success or failure of process control applications. Through the years, many plant people have endured a number of unsuccessful attempts by "outside" people to "optimize" their plants; and these experiences have proved to have had quite a callousing effect on them. On the other hand, in many plants, successful process control efforts have made firm allies of the operating personnel, and have increased the number of metallurgical people who are well qualified and experienced in process control. The instrumentation industry, computer industry, and the mining industry's ability to quantify what is really happening in our processes have grown and developed rapidly since the first control efforts were made in the 1930's (1). Because of this-,: new control opportunities are becoming increasingly more feasible and older well established ones are becoming even more practical and necessary with regard to the economic s of our industry.

"The Pittsburgh Mining Research Division of the U.S. National Institute for Occupational Safety and Health (NIOSH) recently developed a series of models using computational fluid dynamics (CFD) to study airflows and respirable dust distribution associated with a mediumsized surface blasthole drill shroud with a dry dust collector system. Previously run experiments conducted in NIOSH’s full-scale drill shroud laboratory were used to validate the models. The setup values in the CFD models were calculated from experimental data obtained from the drill shroud laboratory and measurements of test material particle size. Subsequent simulation results were compared with the experimental data for several test scenarios, including 0.14 m3/s (300 cfm) and 0.24 m3/s (500 cfm) bailing airflow with 2:1, 3:1 and 4:1 dust collector-tobailing airflow ratios. For the 2:1 and 3:1 ratios, the calculated dust concentrations from the CFD models were within the 95 percent confidence intervals of the experimental data. This paper describes the methodology used to develop the CFD models, to calculate the model input and to validate the models based on the experimental data. Problem regions were identified and revealed by the study. The simulation results could be used for future development of dust control methods for a surface mine blasthole drill shroud. IntroductionIn surface mines, production drilling is an essential process in blasting to fracture the hard rock overburden for removal. During the process, considerable amounts of respirable dust are produced. Past sampling at the shroud area of blasthole drills had documented time-weighted-average respirable dust concentrations ranging from 8.68 to 95.15 mg/m3 (Organiscak and Page, 1995) and 1.04 to 52.30 mg/m3 (Listak and Reed, 2007). These high dust concentrations, which may contain silica, can lead to respirable dust overexposures for the miners working nearby. These overexposures can lead to silicosis, an occupational lung disease that has no cure and is ultimately fatal.Two basic methods are used to control drilling dust: a wet suppression system or a dry collection system (Cecala et al., 2012). Dry drilling techniques using a dust collection system are preferred in rural mining locations where a constant water supply is not available, and in the case of rotary drilling where water is associated with accelerated bit wear due to bearing degradation and hydrogen embrittlement. In addition, dry drilling eliminates freezing-related issues with water in colder climates. Dry dust collectors are highly effective at removing dust, especially the finer material, as long as they are properly operated and maintained (Listak and Reed, 2007; Organiscak and Page, 2005)."

In the current operational facility, due to the absence of pellet pot facility, a novel technique is developed wherein the pellets with different chemistries prepared in the laboratory are fired in an induration machine by placing them inside the Inconel baskets. In the present work, an attempt is made to optimize the pellet chemistry by developing 2 different flux pellets, viz., low basicity (B2: CaO/ SiO2 = 0.3) pellets and high basicity (B2 = 0.9) pellets, and further the effects of basicity on pellet properties, viz., cold compressive strength, porosity, swelling index, reducibility index, and softening and melting temperature, are studied. It has been found through the multiple operational trials that increasing the basicity of pellets from 0.3 to 0.9 results into improving the pellet strength, reducibility and softening and melting characteristics. Moreover, it is observed that the swelling index of pellets is reduced by adding 2% MgO to both low and high basicity pellets that could be attributed to the formation of high melting point slag in pellets. The operational trial has resulted into producing pellets that exhibit a combination of desired mechanical strength, improved swelling, and reducibility index and possess better softening–melting behavior among all the chemistries studied.

Quartz has been of interest in occupational health for years because it can cause silicosis after prolonged exposure. New OSHA regulations (Air Contaminant; Final Rule) list perlite as a nuisance dust. They conclude that perlite is nontoxic when air-borne total particulate concentrations are 15 mg/m3 or below and when quartz content is less than 1% crystalline silica. OSHA has established an 8-hour PEL of 15 mg/m TWA for total perlite dust containing less than 1% quartz. OSHA has established a 5 mg/m3 TWA PEL for respirable perlite. These limits will protect workers from significant risk of eye, skin, or other physical irritation. This regulation is based on a Group 2A classification as a probable carcinogen in humans, at the 0.1% crystalline silica level, by the International Agency for Research on Cancer (IARC 1987a, b). IARC found sufficient evidence for carcinogenicity of crystalline silica in animals and limited evidence in humans. Perlite is not listed as a carcinogen by OSHA or the National Toxicology Program.

The U.S. Department of Energy (DOE) has entered into a $l.5 million Cooperative Agreement with Alpine Equipment Corporation/Astro International Corporation ("Alpine") of State College, Pennsylvania, for the evaluation of water-jet-assisted (WJA) mechanical cutting of oil shale under actual mining conditions. A commercial-sized, heavy WJA mechanical miner is being assembled based upon the successful "factory testing" of a smaller prototype miner utilizing standard tungsten-carbide and Polycrystalline Diamond Composite (POC) bits in large oil shale boulders. Actual field testing of the machine will be conducted in an oil shale mine located in the Piceance Basin of Western Colorado. In-mine logistical support is being provided by several oil shale companies through the Colorado Mining Association (CMA) and the U.S. Bureau of Mines (USBM).

This study utilized experimental and numerical simulation methods to investigate and analyze the atomization characteristics of three internal air–water mixed atomizers under different operating conditions. By comparing the research results, the effective dust control working parameters for the application of the selected atomizer in the engineering field were determined. Firstly, the atomization characteristics of the three atomizers were studied using a customized experimental system. The results indicated that nozzle 2 had a wide atomization range, a small atomization distance, and small droplet size; nozzle 3 had a narrow atomization range, large droplet size, and fast atomization speed; and nozzle 1 not only had a large atomization range but also could produce droplets of dry mist size with fast atomization speed, meeting the dust reduction requirements of the Majiliang Coal Preparation Plant. Secondly, Fluent software was used to simulate the atomization process and particle size distribution of the three internal mixed atomizers, determining the working conditions of the atomizers, the required number of atomizers, and the application angle of the atomizers in the internal mixed dust removal system. Finally, the atomized dust control system was applied on-site, achieving a total dust removal efficiency of 90.72% and a respirable dust removal efficiency of 89.75%.

Although blasting remains the most inexpensive method of fragmentation in hardrock mining, the significance of the costs of blast-induced rockmass damage in terms of mining efficiency and safety is becoming increasingly recognized. There is no straightforward and systematic method of adequately measuring blast damage in on-going mining operations. This paper considers various techniques available to measure blast damage, making particular reference to recent underground hardrock mine studies. It concludes by proposing a damage audit procedure for integration into routine mine production control.

Comminution represents one of the major costs of mineral recovery from low-grade ores. Efficient mineral recovery in a typical low-grade ore grinding-flo¬tation process depends on grinding the ore small enough to liberate mineralization without producing excessive slimes, which would result in higher power and flotation reagent costs. Size criteria for efficient grinding should be stated in terms of the distribution of mineralization sizes, but as the mineralization-size distribution is difficult to determine, less desirable criteria of absolute particle-size distribution (PSD) are used in practice. Thus, there is considerable incentive to be able to analyze and predict the PSD performance of grinding circuits for control and optimization of the process. Any realistic model of a grinding circuit must account for recycle of oversize material; such oversize recycle adds to the total mill load and limits total production rate. Simulation of such closed-circuit operation to study grind factors affecting total throughput can be done in a straightforward fashion provided recycle calculations can be added to an accurate input-output model of PSD in the grinding mill itself. Such a total simulation of mill performance is of obvious practical value in assessing the practicality and benefits of various over-all mill control schemes and/or modes of operation. For example, considerable interest has been shown in the development of continuous on-line particle-size sensors and such instruments should be an operating reality in a few years. A total closed-circuit PSD model of this type would be invaluable in the utilization of, and in predicting the pay-out from, such an on-line particle-size sensor. The purpose of this paper is to present a new approach to the development of an input-output PSD model of a grinding mill. The model is continuous in the par¬ticle size coordinate, L, and is based on the principle of a particle population balance along the particle size coordinate. "The control of fugitive dust in high-latitude openpit mines is challenging due to unique atmospheric phenomena resulting in complicated flow regimes as well as atmospheric inversion due to the lack of adequate insolation during prolonged winter seasons. In this study, two idealized (one conical and one trapezoidal) three-dimensional openpit mine geometries were simulated for different seasonal conditions using a computational-fluid-dynamics package from Software Cradle. The airflow was solved by Reynolds-averaged Navier-Stokes equations using the standard kappa-epsilon turbulence model. The concept of particle tracking was used to predict the flow patterns of dust particles. For various climatic conditions and two different pit geometries, fugitive dust particles varying in size, from PM0.1 to PM10, and concentration were generated at various locations and dispersed by the airflows inside the openpit mine. The amount and location of dust particles inside the pit were reported at various time intervals. In the summer season, airborne dust particles were quickly transported outside the openpit domain. In the winter season, however, the development of atmospheric inversion significantly affected the amount of dust retention inside the openpit domain. IntroductionThe computational-fluid-dynamics (CFD) modeling of openpit mines is challenging. In a recent paper (2015), we presented different constraints and challenges in developing a three-dimensional CFD model. In order to obtain realistic results from a simulation, however, appropriate values of various inputs are required. The simulation results of a three-dimensional nonhydrostatic model by Shi et al. (2000) showed that both the mechanical and thermal forces are important mechanisms, controlling the development of the atmosphere inside an openpit mine. Alvarez et al. (2007) used a CFD model for predicting dust dispersions in a medium-sized limestone quarry, and presented several iso-surfaces of varying dust concentrations using ANSYS CFX. A similar study by Torno et al. (2010) introduced a more sophisticated parameterization of the dust sources. Flores et al. (2014) used an OpenFOAM CFD simulation package to simulate and predict pollutant dispersion in an idealized and an actual openpit mine in Chile with an intense insolation condition. The model was simulated for three different conditions: (1) air-advection (10 m/s, or 33 ft/ sec)-driven mechanical turbulence, (2) surface-heat-flux (240 W/m2, or 76 Btu/ ft2-h)-driven thermal buoyancy and (3) both air-advection- and surfaceheat- flux-driven conditions. The results of the simulations showed that the buoyant currents contribute to the removal of a large percentage of the dust particles. The buoyant currents modify the purely mechanical-turbulence- induced flow pattern and reduce the particle residence time, commonly observed in a purely mechanical turbulent flow (nonbuoyant case)."

A new analytical approach for characterizing and improving the performance of ball milling is presented. Individual ball mill circuit design and operating variables can be related to overall circuit performance through two distinct parameters for classification and breakage. The ball mill circuit functional performance equation is presented. Applications in the industrial plant and in the research field are discussed.

Assessment of the potential for mining induced subsidence and determination of ultimate design requirements for a mining operation should be based on a number of assessment approaches. Although the rock mechanics engineer has a variety of limit equilibrium and numerical modeling tools available for assessment and prediction of subsidence effects, empirical approaches based on precedent experience provide a powerful tool for calibration/verification of analytical models and prediction of subsidence. Three separate analyses approaches have been applied to the assessment of potential subsidence for a proposed block caving mine in eastern Canada. The investigations have included an assessment of documented historical information from previous block cave mining at the site, deterministic analyses using limit equilibrium approaches to assess kinematically possible failure mechanisms, and three-dimensional numerical modeling techniques. Analyses results have been used and compared with precedent experience and documentation of subsidence behavior from previous mining to optimize the subsidence predictions for the proposed mine.

"The United States coal mining industry is currently experiencing a decline in both demand and commodity price while facing implementation of more stringent air and water pollution regulations. Company profits have decreased significantly, forcing several major companies to declare bankruptcy. The industry was required to adapt to survive. Many of these companies have adjusted by abandoning mines; the last two decades have seen a drastic decrease in the number of mines. To discover the primary causes of this downturn, three major aspects must be studied: properties of abandoned mines, commodity history and regulatory impact. Trends since 1994Each year, U.S. coal mines are required to submit data on a variety of categories to the U.S. Mine Safety and Health Administration (MSHA) and the U.S. Energy Information Administration (EIA). This data include administrative records, production values and information regarding the attributes of the mined material and the mine itself. Each year since 1996, EIA has released a report summarizing the data from two years prior. Additionally, MSHA maintains a database containing a collection of all the most current data. These resources are the primary sources for the figures included in this article. There are several items to note regarding this data:1. The EIA reports from 2016 and 2017 were not released at the time of writing this report.2. The MSHA data set is actively maintained as information is provided. This means the data for 2016 is from June 27, 2016 and data for 2017 is from Aug 24, 2017. All other points represent the end of-year values.3. Each of the figures only includes data pertaining to active mines. EIA defines active mines as both active and temporarily idled mines, and inactive mines as nonproducing and abandoned mines. MSHA separates all four into separate categories, though this paper combines the MSHA values according to the EIA definition for the purpose of comparison."
