There is strong evidence that many major oil and gas (O&G) projects are plagued by large cost and schedule overruns, or significant underperformance in productivity. Similar evidence exists for large mining projects. This paper claims that uncertainty is the underlying reason why projects fail to deliver the metrics used to justify their investment, or to deliver the economic value possible if alternative decisions had been made.Most technical and economic analyses are performed to reduce uncertainty to help make decisions. Decision-making is about ranking alternatives and choosing the best; rarely are precise predictions required, if this is even possible. Better uncertainty assessment is one of the keys to better decisions, thus better business outcomes. To get better decisions it is suggested that the focus or reward/penalty incentives be shifted away from the quality of outcomes to the quality of decisions. High-quality decisions are those that are generated from the skilled use of the tools, methodologies and processes from the discipline of decision analysis.Uncertainty is quantified by probability, which is a degree of belief in the truth/falsity of some proposition about a quantity relevant to a decision, such as volume of in-place resource, future price or action of a competitor. The probability a person ascribes to a proposition depends upon their information set; hence probability is subjective – it is not an inherent attribute of the quantity of interest or the physical system.The above simple observations have profound implications for modelling approaches, how they deal with uncertainty, the use of data to help assess uncertainty and the reliability of information provided by subject-matter experts. The limited circumstances under which frequency data (from spatial or temporal variables) can be used to assign probabilities is also reviewed.People, in general, are subject to a range of biases and cognitive errors. Research with O&G subject-matter experts shows that their expertise offers little or no protection from making errors in probability assessments and other judgements.CITATION:Begg, S H, 2016. FIGURES for the first 11 months of 1935 indicate that the total coal production of the United States for 1935 will be approximately 416,000,000 tons, or almost identical with the production figures for 1934. Bituminous production will show a gain of about six million tons, and anthracite production a loss of about the same tonnage, compared with the previous year. This does not take into account so-called "bootleg" anthracite coal amounting to approximately 5,000,000 tons. Twenty-nine years ago, in 1906, the production of coal was almost identical with this year's production. At that time, 626,000 men were employed in producing this tonnage, which took a toll of 2138 lives from accidents. In 1935, approximately 540,000 men were required to produce the same tonnage with an estimated loss of 1155 lives. This shows definite safety advancement in the industry, but even so accidents are still costing an average of 3c. a ton more than they should.

Seals are explosion-resistant structures built in underground coal mines to isolate abandoned mining areas from the active workings. An equivalent single degree of freedom (SDOF) method such as the single-degree-of-freedom blast effects design spreadsheet (SBEDS) developed by the U.S. Army Corps of Engineers can be used to design seals to resist mine explosions. The critical input for the SDOF code is the seal resistance function. The objective for this study is to develop engineering data for design of plain concrete (PC) and reinforced concrete (RC) seals using finite element (FE) models. The FE models can then be used to generate resistance functions for different PC and RC seal designs, which can be used in the SDOF codes. The U.S. National Institute for Occupational Safety and Health (NIOSH) researchers designed and built a test fixture for use in conjunction with NIOSH’s Mine Roof Simulator to apply a four-point bending load on the face of the tested seal. Eight large-scale experiments for each PC and RC seal were conducted. The experimental design considered the effects of the foundation strength, hitching and roof-to-floor convergence. Calibrated FE models for PC and RC seals were developed using ANSYS Ver. 13. With a very limited number of engineering-based assumptions and a single set of data—including Young’s modulus, compressive strength, stress-strain relationships, etc.—the PC and RC seal models successfully calculated the recorded resistance functions and failure patterns for a wide range of boundary conditions. Plug and RC seals were assumed for a 1.83-m- (6-ft-) high entry to demonstrate the application of the calibrated finite element models for calculating the resistance functions of these seals. The resistance function of a particular plug seal case was found to be a bilinear relationship. The resistance function of a particular RC seal without shear reinforcement was found to be a nonlinear relationship.

The founder of social psychology is sometimes identified as Kurt Lewin. In a 1947 article, Kurt Lewin coined the term ‘group dynamics’. He described this notion as the way that groups and individuals act and react to changing circumstances. Lewin theorised that when a group is established it becomes a unified system with unique dynamics that cannot be understood by evaluating members individually. The discipline of social psychology really emerged out of World War II and tackled questions associated with human judgement and decision-making by the Nazis. For a comprehensive look at the development of social psychology look at Abelson, Frey and Gregg (2004).Social psychology is concerned with the way social arrangements affect human judgement and decision-making. What the author has done is apply the knowledge of this new discipline to an understanding of risk, safety and leadership. This paper will explain some of the developments in social psychology, such as the discovery of the bystander effect, Groupthink, bounded rationality, heuristics, risk homeostasis and the Milgram effect.The social psychology of risk tackles the way decisions are made about risk in their social context. Unless organisations understand that decision-making is socially conditioned, they are unlikely to be able to tackle the complexities of culture change or the development of risk maturity.The paper will offer a new understanding of why people do what they do, how risk makes sense and why a ‘dumb down’ approach to safety is dangerous. Researchers examined datasets from two separate studies that measured common demographic variables but different safety performance outcome variables. Of specific interest was how types of experience might relate to workers’ preparedness to effectively mitigate or respond to both routine and nonroutine risk in the mining industry. The results suggest that individual factors such as length of job, industry, and mine experience are predictive of routine safety performance — as measured by self-reported proactive and compliant safety behaviors — and/or nonroutine safety performance — as measured by perceived competence in self-escape knowledge, skills and abilities (KSAs) — in significant and sometimes unexpected ways.

The first data compiled by the Bureau of Mines relating to coal- mine accidents in the United States were published in Bulletin 69," in which the total fatalities by years and States were tabulated from 1896 to 1912. Since the publication of Bulletin 69, further research has been conducted, with the result that the bureau is now able to publish for the first time all of the fatal accidents described in the State inspectors' reports by States, causes, and calendar years, from the beginning of inspection service to date. For the anthracite mines of Pennsylvania, there is a continuous record since 1870; the records of the Pennsylvania bituminous mines are continuous from 1877, and, with the exception of one year, the Ohio records are complete from 1874 to date. In fact, the records of all of the States, with few exceptions, are complete; and incomplete data for any year may generally be attributed to the fact that no report was published during that year. The magnitude of the coal-mining industry in the United States is such that too much stress can not be placed on the need of safeguard- ing the three-quarters of a million men employed. Although the figures given herein show an appalling record for past years, they are not published for the purpose of proclaiming the high fatality rate of the past. They are a record of experience covering a period of 45 years, showing causes and results of many accidents in the coal mines of the United States. They represent a diagnosis of the hazard of the mining industry, pointing out the principal physical causes of accidents, and make available a body of uniform facts that will serve as a basis of preventive measures for use in future operations. It is with these great purposes in view that the Bureau of Mines presents the classified data in this report. Classification and study of mine accidents on the basis of number of persons killed is common and very old, but fails to throw enough light upon the problem involved. Many serious accidents in mines have no fatal results, hence in a tabulation of fatalities they are overlooked and the real hazard of the industry is not made clear. An accident does not necessarily include an injury that may result therefrom. Such accidents as the breaking of a hoisting cable, an explosion of powder, gas, or dust, a runaway car or motor are all serious. Although no loss of life results, they should be recorded, investigated, and studied to prevent their repetition. No accurate data for the United States are available showing how far the personal element as related to the miner and his co-worker, the mine foreman, or to the superintendent and others in authority, contributes to accidents. Many accidents are due to inexperience on the part of the miner, his failure to heed orders, a misunderstanding of instructions and last, but not least, carelessness of himself or his fellow worker. A foreman or superintendent may fail to give proper warning regarding the conditions of certain parts of the mine; he may not have inspected certain rooms or entries on the day of a mine accident; the mine may not be properly equipped, or the operator may neglect to comply with the inspector's recommendations. There is, therefore, a personal element on the part of both the operator and the miner that must be considered. Legislation and the enactment of compensation laws will make the operator realize the seriousness of the mine-accident situation, when fatalities and injuries are to be paid for in legal tender of the realm. The miner must be educated and made to realize the dangers he encounters; he must learn that self-preservation and the safety of his fellow workmen should receive his first attention; he should cooperate with his employer, his asso- ciates, and the State mine inspector to the end that the mine hazard may be reduced to a minimum. "Cooperation for safety" should be the watchwords of the operator, miner, State inspectors, and all others in any way interested in the mining industry.

The corrosion resistance of stainless steels can be strongly affected by surface treatments. Changes in corrosion resistance can relate to surface composition, integrity and stability of the passive film, and to surface profile (roughness). Operations producing oxidation at the surface, such as annealing, heat treating, and welding, can result in a chromium-rich scale and a chromium-depleted metal surface. In order to restore full corrosion resistance, chemical or mechanical clean-up techniques must remove not only the oxide, but also a very thin layer of metal. Laboratory and field test studies show that these factors are particularly significant in less highly alloyed steels. Annealing stainless in a reducing atmosphere (bright annealing) avoids chromium depletion, but as-bright-annealed material does not display optimum corrosion resistance in certain environments. Electro passiva-tion can be applied to improve pitting resistance without significantly changing bright surface appearance. Potentiometric studies show that electropassivated material also displays significantly greater resistance to breakdown of passivity in acid solutions. The application of special surface finishes for appearance purposes can affect corrosion resistance. Roughness is a key factor. Samples of related appearance but with different surface profiles can show significant differences in corrosion resistance. Thus nonreflec-tive stainless with a relatively smooth surface produced by a special rolling process is much superior to material chemically etched to a similar low reflectivity. THE corrosion resistance of a stainless steel is affected by other factors in addition to its bulk composition and metallurgical structure. These other factors include those relating to the specific chemical and physical condition of the steel's surface. This paper will consider certain aspects of surface condition in relation to corrosion resistance. In particular, studies will be described on the effects of oxidation encountered in heat treating or welding operations, of passivation after bright annealing, and of different surface finishes. EFFECTS DUE TO OXIDATION When stainless steels are exposed to high temperature in an oxidizing environment, as in annealing or welding, the oxides formed are relatively rich in chromium. As a consequence of oxidation, the metallic layer directly beneath the scale may be low in chromium to some very thin but finite depth.1"8 Such a change in chromium content could lower the corrosion resistance at the surface, whether or not the oxide layer is removed. Studies were performed relating to the effect of oxidation and chromium depletion produced during annealing and welding on the corrosion resistance of stainless steels. The nominal compositions of steels used in this study and others are given in Table I. A) Annealing Studies on 17 pet Cr Steels. 1) Effects of Pickling Treatments. Studies were performed to investigate the corrosion resistance of air annealed type 430 stainless as a function of the amount of electrolytic pickling after annealing. The material used in this study had been air annealed and electrolytically pickled in the mill. All scale had been removed, and yet the material showed unusually poor corrosion resistance. Laboratory electrolytic pickling studies were performed to determine whether additional metal removal from the surface would improve corrosion resistance. Electrolytic pickling was performed in a sulfuric acid bath using various conditions of bath acidity, temperature, time, and current density. The samples were then rated for corrosion resistance in a ferric chloride spot test. For this spot test, a solution was prepared by dissolving 10 g of FeCl3 . 6H2O, 5 g of NaCl, and 2.5 ml of concentrated HC1 in 200 ml of water. An area on a specimen was cleaned and a drop of test solution was placed on the prepared area. After 5 min, the drop was rinsed off and the area wiped dry. The spot was then rated between 1 and 5 by visual comparison with standard spots. The numerical rating of the standard spots can be described as follows: 1—No clouding of surface; 2—very slight clouding of surface; 3—slight clouding of surface; 4—moderate clouding of surface; 5—strong clouding of surface. A rating of 1 or 2 was considered to have passed the test, a rating of 4 or 5 represented failure, and a rating of 3 was considered borderline. The severity of etching in this test has been related to the actual chromium content at the surface.4 Etching (clouding) occurs on type 430 stainless (rating of 4 to 5) when the surface chromium content is 14 pct or less. The effects of current density and retention time when electrolytically repickling this material in 6 pct sulfuric acid are shown in Fig. 1. Increasing the cur-rent density or retention time improved corrosion resistance as measured by the spot test. Similarly, increasing acid concentration of the bath or bath tem-perature was found to improve corrosion resistance. The conditions making for improved spot ratings resulted in increased metal removal, regardless of specific pickling conditions used, Fig. 2.

This report deals with information regarding a new Medium Frequency (MF) Wireless Communication System for underground mines. This new telecommunication system works on low loss electromagnetic wave propagation modes which allow it to provide quality radio communications almost anywhere in a mine. Tests show the MF system can provide improved safety and managerial control, increased productivity, better organizational team-work and reduced maintenance cost; furthermore, MF's unique design allows for easy installation at a minimum cost. This paper covers the following: 1) MF design plan, 2) Overview of mine radio communications systems, 3) Preparations needed for designing an MF system, 4) MF equipment, 5) MF installation, 6) MF design considerations, 7) MF design specifications, and 8) Recommendations dealing with MF system. Experiments in four mines are illustrated to demonstrate the success and viability of the MF system. The mines range from small (500 tons/day) to large (67,000 tons/day) capacity and represent several different mining methods. The MF Wireless Communication System represents an advancement in mining communication and the groundwork for other advanced mine systems.

"INTRODUCTION The tests described in this publication were planned to provide information on the explosibility of metal-powder dust clouds. This subject has become increasingly important because of the large quantity and variety of metallic powders now used for military and industrial purposes. To provide the increased amounts of metallic powders needed in the manufacture of munitions and to supply the demand created by the rapid growth of powder metallurgy, existing metal-powder plants have been enlarged, production schedules accelerated, and new plants erected. In some cases new methods of producing metal powders have been developed. This increased production and the development of new manufacturing methods have caused an increase in the number of metal-powder fires and explosions, with accompanying heavy loss of life and property. These have emphasized the need for information on methods of providing protection against hazards inherent in the production and handling of powdered metals. This paper gives results of laboratory inflammability and explosibility tests made to April 1, 1943, and is a progress report. Many descriptive details have been omitted purposely; they will supplied in a later publication, expected to be a Bureau of Mines bulletin."

The burning of coal without smoke is a problem that concerns the Government directly because of the advantages of smokeless combustion both in public buildings and on naval vessels. In addition, smoke abatement is a factor in conserving the fuel resources of the United States. Hence, as part of its general investigation of the best methods of utilizing the coals of this country, the Bureau of Mines, which is in charge of the fuel-testing work formerly carried on by the United States Geological Survey, has made extended tests to determine the conditions necessary for the smokeless combustion of bituminous coal in boiler furnaces. Bulletin 39,¢ a preliminary report, treats the smoke problem from a theoretical point of view, detailing the steps that are being taken by municipalities, manufacturers, and citizens to abate smoke, also showing the possibility of a clean city. The present bulletin not only shows that bituminous coals high in volatile matter can be burned without smoke, but also that large plants carrying loads that fluctuate widely, where boilers over banked fires must be put into service quickly and fires forced to the capacity of their units, can be operated without producing smoke that is objectionable. Proper equipment, efficient labor, and intelligent supervision are the necessary factors.

This publication presents abstracts of articles and investigations pertaining to the utilization of fly ash. The terms "fly ash" and "flue. ash," as' used in most of the papers, refer to the ash from pulverized-coal firing collected by electrostatic precipitators or mechanical collectors. Several investigators do not define the material on which they report beyond calling it fly ash, but they probably refer to the fine material resulting from burning pulverized coal. A few of the papers report on investigations involving not only the use of fly ash but boiler slag as well. These can be identified by the title of the articles. Bibliographical information and the abstracts are arranged according to the date the papers were published, with the most recent listed first. With the continually increasing use of pulverized-coal firing, profitable disposal of the fly ash is becoming a problem of greater importance to many utility and industrial companies. Also, in recent years, the public demand for cleaner air, and the more highly efficient fly-ash collectors available have resulted in the collection of a greater percentage of the fly ash produced. The solution to better utilization of fly ash has, therefore, been the object of research for some years.

In two decades Turkey has emerged from the debacle of the Ottoman Empire to become one of the most modern nations in the Middle East. The numerous achievements of this period were remarkable in that they affected human and social, as well as economic, fields. Most of this progress was achieved by Turkey's unaided efforts and the utilization of domestic resources; however, since 1948 various United States aid programs3/ have made significant contributions to increased agricultural and industrial output. Turkey's singleness of purpose in favor of a strong, independent, and prosperous republic has evoked universal admiration and encouraged the hope of stability in the Middle East. Much of the development of the Turkish economy to date has been fostered by the State; however, with growing strength the leaders in 1950 considered the time appro¬priate to relax the nationalistic economic policy and permit private enterprise to play a larger role in increasing domestic output and raising the tempo of industrialization. In pursuance! Of this policy, laws encouraging investment of foreign capital in Turkey and others stimulating development of mining and petroleum enterprises were enacted in 1954. This new legislation, plus the cooperative attitude of the Turkish Government, have encouraged wide interest in the mineral resources of the country.

TRADITION tells us that the earliest use of Alabama iron was to make shoes for the horses of General Andrew Jackson and his men during the first part of the nineteenth century. The first recorded incident in the making of iron was the request of U. S. Indian Agent Benjamin Hawkins "that a crew of blacksmiths be sent to the Creek Nation in the lower Mississippi territory." These frontiersmen made up the first body of pioneer iron makers in the territory. The first known blast furnace was built by Joseph Heslip in 1818 at Russellville, named after Major William Russell, who was General Jackson's chief of scouts. The Tannehill furnace was built in 1830 in what is now Tuscaloosa County and its ruins, as shown above, can be seen today.

In this report considerable space has been devoted to general and theoretical principles on the ground that under modern conditions the best results in any field of activity can be attained only with the a. id of an adequate knowledge of the best theories underlying the practice. The diffusion of such knowledge in the field of sampling metallic metallurgical materials appears to have been quite insufficient, and it is felt that when the principles involved are once thoroughly comprehended their universal application will be appreciated, so that it will make no difference whether pig iron or gold bullion is offered for test. It may further be noted that a discussion of the theories underlying practical methods is placed under the caption "Practical Sampling," with the idea that no one can be a practical sampler unless he is capable of analyzing his methods. No attempt has been made to advocate any one method, but it is hoped that the analyses of all of the methods in use will show where each may be correctly and safely applied. The fact that it is possible to demonstrate the theoretical correctness of several methods indicates the invalidity of the claim that any single one of them is the standard.

This paper gives the results of a reconnaissance of the mining districts in Humboldt County, Nev., made during the month of June, 19373/. During the field work, virtually all the mining districts in the county were visited and many data were obtained on a number of active and inactive properties. No attempt has boon made to include in this report all the properties in the various mining districts, and it should be emphasized that whether or not a property is mentioned herein has no being on its merit. The report covers the location of the various mining; districts, types of deposit, information on past operations and current activity, and general information likely to be useful to operators, investors, and others interested in mining. The geology of the deposits is discussed only briefly. The names of the various mining districts mentioned in this report are those generally used when they were organized for purposes of record and regulation. As organized, the districts embraced largo areas with no definite boundaries, and the names have little significance.

The proposed Caldecott fourth bore will consist of a two lane highway tunnel along California State Route 24 near the City of Oakland. The proposed design and construction sequence for the 15-m-diameter tunnel are based on the New Austrian Tunneling Method (NATM). The initial support system incorporates combinations of shotcrete, rock dowels, lattice girders, spiles, and grouted steel pipe canopies. The final lining is cast-in-place reinforced concrete. A waterproofing membrane and drainage system are placed between the initial and final linings. State Route 24 is a lifeline route, required to be open to emergency vehicles within 72 hours after a major earthquake, defined as having a return period of 1,500 years and a peak ground acceleration of 1.2 g. Although the seismic design criteria are stringent, the design of the tunnel lining system is ultimately controlled by static ground loads in the weak rock along the alignment.

The objective of this handbook is to provide the inspector with guidance for recognizing hazards from inhalation contaminants in above ground work areas of underground and surface metal and nonmetal mining and processing facilities and evaluating these hazards by collecting samples or making field determinations and interpreting the analytical results. Portions of the handbook that should prove most useful to the inspectors include a survey check list, rules for determining if sampling is required, list of most frequently encountered contaminants, list of candidate contaminants and their sampling conditions, interpretation of results, and a procedure for a health and safety inspection.

The U.S. economy made significant progress in 1984. The current-dollar gross national product (GNP), the market value of the Nation's output of goods and services, increased almost 11 percent over that of 1983 to slightly more than $3.66 trillion (table 1), and the real gross national product increased almost 7 percent.2/ Such rapid growth in real GNP has not occurred since the early 1950's. Real final sales increased only 5 percent during 1984, and real business inventories increased $24.8 billion ($57.9 billion in current dollars). Total employment of 106.7 million was an all-time record, and the unemployment rate declined from 9.5 percent in 1983 to 7.4 percent in 1984. During 1984 the U.S. dollar showed great strength on international markets, contributing to large current-dollar negative net exports of goods and services (-$62.5 billion) and even larger negative net exports of merchandise (-$107.6 billion). The strong dollar and the substantial increase in net imports of goods and services helped reduce U.S. inflationary pressures and provided a major source of loanable funds for financing domestic private investment.
