Through the hearty cooperation of the State coal-mine inspectors, the bureau is able to present in this paper a a complete statement of the coal-mine fatalities occurring throughout the United States during the calendar year 1919. The reports received from the inspectors for the year just closed show a reduction of 10.58 per cent in coal-mine fatalities as compared. with 1918, while in 1918 the reduction was 4.3 per cent from 1917 figures. The total number killed was 2,307 in 1919 and 2,580 in 1918, a reduction of 273. There was a decrease of 128, or 25 per cent, in fa tali ties resulting from mine cars and locomotives, and a decrease of 198, or 15 per cent, in the fa tali ties due to fall of coal or rock. Surface accidents show a decline of about 25 per cent as compared with the previous year. There were, however, increases in accidents due to gas and dust explosions and also explosives. There occurred during the year nine disasters in which five or more men were killed, representing a total of 201 fatalities. The worst disaster of the year was the one occurring at the Baltimore Tunnel No. 2, Wilkes-Barre, Pa., resulting from the explosion or burning of a number of kegs of powder, by which 92 lives were lost. This disaster emphasizes the need for stricter regulations relating to hauling men and explosives into the mine at the same time. The question of whether electricity was responsible or not does not affect the consideration of the various dangers which arise in underground transportation and handling of explosiveB. At best the handling and hauling of explo.sives is hazardous and such being the case, no one, other than the necessary attendants, should be permitted to ride in cars or cages transporting explosives. As a result of this disaster, the report b of the coroner's jury to the governor of Pennsylvania brings out some important dangers to be avoided in the use and transportation of explosives.

The classical approach to control rock fall and ravel in open pit mining has been to use catch benches. The width and volume of benches is most dependent on the bench height, size of loading equipment, blast damage to the face and mining regulations. This was compatible with open pit slopes of less than about 45°. As our understanding of rock mechanics and blasting control has improved over recent years, much steeper rock slopes can be developed at many open pit mines. However, narrower benches result with less rock fall capacity. The transportation industry has been very successful in using catch fences to protect highways throughout North America and Europe for years. The authors recommend that cable and ring net fences have a potential major application in open pit mining to allow steeper rock slopes with significant economic benefit. This paper outlines the application, design and operation of catch fences in surface mining for specific conditions. Typical case examples are presented.

"INTRODUCTION This report embodies the results of a detailed statistical microscopic examination of the ore and the mill products of the Silver King Coalition Mines Co., Park City, Utah.The study was conducted under the joint auspices of the United States Bureau of mines is cooperation with the University of Utah at Salt Lake City, Utah. The work was done in the cooperative Microscopic Service Laboratory, which was equipped by contributions from the following companies: Utah Copper Co., International Smelting & Refining Co., American Smelting & Refining Co., United States Smelting, Refining & Mining Co., Tintic Standard Mining Co., Silver King Coalition Mines Co., and Park Utah Consolidated Mines Co. The salaries and the other costs involved in operation of the laboratory were met by contributions from certain supporting companies. The Loma Prieta earthquake occurred on October 17, 1989 at 5:04 PM, Pacific Daylight Time. It measured 7.1 in magnitude and was felt from Los Angeles north to the Oregon state line and east to western Nevada. The epicenter of the earthquake was located along the San Andreas Fault, in the Southern Santa Cruz Mountains just north of the City of Watsonville, California. It was the largest earthquake to occur in the San Francisco Bay area since the 1906 earthquake. A total of 87 aftershocks of Magnitude 3.0 or larger were recorded within 21 days after the seismic event. As a result of this earthquake thirteen State-owned and five locally-owned bridges were closed to traffic. Three of the most publicized State-owned structures closed to traffic were Struve Slough Bridge, San Francisco-Oakland Bay Bridge and Cypress Street Viaduct. Struve Slough Bridge was twin slab bridges that collapsed and were rebuilt and open to traffic three months after the earthquake. San Francisco-Oakland Bay Bridge experienced collapse of upper and lower spans at Pier 9 and was reopened to traffic one month after the earthquake. The Cypress Street Viaduct experienced collapse of 48 bents that resulted in the upper roadway falling onto the lower roadway.

The early history of the mine is traced from the beginning of shaft sinking in 1886 to the purchase by the Broken Hill Proprietary Co. Ltd. in 1949. A necessary preliminary to mechanisation of the Borehole Seam was development of the Young Wallsend seam, andthis development is described. Surface reorganisation and coal handling arrangements are dealt with in detail. Then follows a description of the Borehole Seam mechanisation, the provision of power supply for this purpose, arid pumping arrangements in themine.INTRODUCTIONStockton Borehole Colliery is in the Parish of Teralba, between the townships of Teralba and Boolaroo, and about 12 miles from Newcastle. The colliery holding, a freehofd area of 2,560 acres, was apparently an original land grant when acquired by Teralba Co-operative Coal Company who decided to mine for coal and named the mine "Teralba Colliery." Shaft sinking was commenced in 1886 but was delayed by financial and water troubles to such an extent that it was not until 1901 that the Borehole Seam was reached.Coal was won until 1904 using a single 16 ft. diameter shaft, ventilation being achieved by means of a wooden boxing down one side of the shaft. This collapsed, and operations ceased until the mine was taken over in 1906 by a company known as "Borehole Colliery Limited," the principal stockholder being the Stockton Coal Company. It renamed the mine "Borehole Colliery." This new company immediately commenced sinking a 13 ft. diameter air shaft. In 1908 the Stocktol1 Colliery at Stockton was abandoned, and during the following year much of its equipment was transferred to Borehole Colliery. PB is providing engineering services to New York City Transit (NYCT) for the rehabilitation of the 59th Street/Columbus Circle Station Complex that services over 150,000 customers daily. The project involves structural repairs, modifications and new construction requiring underpinning and temporary supports, custom framing details, and construction staging procedures to meet stringent limitations within the operating transit station and around the Columbus Circle street intersections.

This study investigates the application of computed order tracking with subsequent rotation domain averaging and statistical analysis, to the drag gearbox of a dragline used in a mining environment. Computed order tracking is a fault detection method developed to deal with the varying speed conditions often found in industry and has been proven effective in laboratory conditions. Its application to real situations, however, requires some adjustment to deal with issues such as inadequate speed data and the fact that the drag gear rotates in two directions. This provides a unique opportunity to observe the performance of the order tracking method in a bi-directional rotating environment, allowing an investigation of the relationships between the results obtained from each operating direction. A monitoring station was set up aboard the dragline and data was captured twice daily for a period spanning approximately one year. The data captured consisted of accelerometer and proximity sensor data. The key on the shaft triggers the proximity sensors, allowing speed and direction to be measured. The measured speed is interpolated by using various speed interpolation techniques. Then the interpolated speed is used to complete the order tracking procedure that resamples the vibration data with reference to the speed. The results indicate that computed order tracking can be successfully employed in real environments. Furthermore it is shown that the rotation direction that opens the gear-tooth crack gives a better indication of incipient failure. It is therefore important not to disregard either direction when monitoring rotating machinery of this kind.

"Until recently, liquefaction-related studies concentrated on clean sands believing that only sands are susceptible to liquefaction. However, a few earthquakes like 1976 Tangshan earthquake, the 1989 Loma Prieta earthquake, the 1999 Kocaeli earthquake, the 2010 Chile earthquake, and the 2011 Christchurch earthquake, etc. showed that sand with fines could also liquefy. The research findings on liquefaction of silty soils are limited, and several structures constructed on such soil deposits are damaged during the past earthquakes. Some previous results of research on earthquake events show an increase of liquefaction resistance in silt soils with increase the fines content, but some report the opposite trend. This study presents the results based on undrained triaxial cyclic tests which were carried out on the fine sand with fines (0%, 10%, 20% and 40% fines). The samples were prepared at the required unit weight (Dr = 50%) and saturated by using back pressure and cell pressure increments. Each consolidated sample is subjected to cyclic loading with sinusoidal wave load form at a frequency of 1 Hz. The liquefaction resistance is evaluated in terms of cyclic stress ratio required to cause 100% pore pressure or 20% failure strain occur at 15 numbers of load cycles. Results are presented in forms of pore pressure build up and axial strain propagation plots. As the fines content increased, an increase in liquefaction potential was observed.INTRODUCTIONLiquefaction and related phenomenon have been responsible for tremendous amounts of damage all around the world. In general, the liquefaction may occur in fully saturated sands, silts and low plastic clays. When the saturated soil mass is subjected to seismic or dynamic loads, there is a sudden buildup of pore water within a short duration, and it could not dissipate which leads to reducing the effective shear strength of soil mass. In this state, the soil mass behaves like a liquid and causes large deformations, settlements, flow failures, etc. This phenomenon is called soil liquefaction. As a result, the ability of soil deposit to support the foundations of buildings, bridges, dams, etc. is reduced. Liquefiable soil also exerts a higher pressure on retaining walls.This lateral movement of soil could cause settlement of the retained soil. A sudden build-up of pore water pressure during quake also triggers landslides. Liquefaction damages of structures are commonly observed in low-lying areas near the rivers, lakes, and oceans."

"This paper investigates the conditions leading to the indefinite termination of production in four critical primary panels at an underground coal mining operation, the observed shortcomings in the mining approach, and the proposed strategy to mine through the affected panels. Initial assessment of the abandoned panel conditions indicated time-dependent strata failure,(i.e. bolted roof failure overrunning intersections), which occurred from mere minutes to up to four weeks post-production, with and without prior warning of failure. This prompted the constant re-supporting of back areas, which raised safety and productivity concerns. Investigation of the initial mining conditions revealed that the failures were due to a critical combination of factors, the chief of which was isolated horizontal stress. Other factors that were initially overlooked by the mine (i.e. influence of hydraulic stress, misinterpretation of borehole data), resulted in the conditions being described as abnormal. Remedial actions were determined, and in so doing, a new strategic approach was formulated to thoroughly address all failure concerns. The four panels were explicitly planned to serve as the main intake and return airways for the recently commissioned secondary ventilation shaft, as well as providing access to millions of tons in proven coal reserves. It is thus imperative to mine the panels. A feasibility study showed that the proposed strategy set for implementation would be financially viable. Construction of foundation piles or diaphragm walls through rocky or boulder containing formations has been long-standing difficulty. It has been widely understood that the down-the-hole hammer is the most capable mechanical means but is restricted in drilling diameter by the unavailability of drillhead sufficiently large diameter and the difficulty of disposal of cuttings. A great number of submersible motordrills have been used for the construction of cast-in-place piles and diaphragm walls and the reverse circulation system employed has demonstrated its advantage of the prompt removal of cuttings. The development of multi-head down-the-hole hammer drill combined with reverse circulation system presents a powerful engineering choice for those "hard-to-drill" ground formations. This paper will introduce the features of the system and some actual operation results in the foundation engineering through those hard rocky ground formation in some countries.

The aim of the present work is to investigate the effects of shooting multiple explosive charges, in the same borehole, on the amplitude and frequency content of the generated seismic wave. The work also investigates the effect of the coupling medium used in the borehole on the quality of the generated seismic wave. The delay period between the shots is on the order of months and up to a year. The investigation utilizes a numerical modeling approach which is capable of modeling the detonation of explosives as well as shock and stress wave propagation in solids.

"The mode of ore occurrence and the methods and costs of mining at one of the zinc-lead mines in the Tri-State zinc and lead district, Oklahoma, are presented in this paper for the information of mine operators in other districts.No. 2 mine property was acquired by lease in 1918 after some 30 holes had been drilled and a shaft had been sunk to the 305-foot level by the first lessees. After acquisition of the lease, the present operating company drilled a rich ore body on the 260-foot level and in 1920 started sinking two shafts. Production was begun late in 1921, and the mine has been operated more or less continuously to the present time.The mill has a capacity of 400 tons in 10 hours, the usual length of the mill shift in the district.The geology of Mine No. 2 is very similar to that of Mine No. 13 except that the Cherokee shale is thicker, ranging from 80 to 160 feet, and all the formations are correspondingly deeper, the Short Creek oolite horizon here being at a depth of about 300 feet. The oolite has not been found on this property, but the lime comes in at about the level at which the oolite should appear.The ore deposits of Mine No. 2 occur in the brecciated and bowlder ground above the Short Creek oolite. The main or 260-foot level is about 40 feet above the oolite horizon, and the lower or 300-foot level is immediately above it.The ore deposition is controlled by a zone of shearing and fracturing, and the mineable areas are divided into two distinct runs which have a northwest trend with a lean or practically barren area dividing them. This characteristic has controlled the mining to a large extent, for it enables the mine superintendent to locate the large pillars in lean ore and leave only small pillars in the richer ore. Figure 1 shows the outlines of the ore body and the pillars.The ore on the main level occurs disseminated throughout the jasperoid breccia as patches or lenses in the jasperoid, or cementing massive bowlders of jasperoid breccia and chert. The ore of the lower level is more of the bowlder type than that on the main level and occurs chiefly cementing the bowlders but also with some disseminated ore in the jasperoid breccia.The gangue minerals are jasperoid breccia, chert, and dolomite. Calcite is found in minor quantities. Sphalerite is the principal ore mineral; galena is found only in minor quantities around the edges of the ore body. When galena is encountered in this ore body, the limit of the ore can be expected.The exploration and estimation of ore reserves at Mine No. 2 are similar in many respects to the methods used at Mine No. 1.4After the mine was opened up and the general character of the ore deposit had been determined, drilling operations were concentrated along the trend of the shear zone. Later drilling has proved what appears to be a parallel shear zone or an offset of the zone now being mined."

WHEN "Chief" Buehler in mid-September set out on his official 10,000 mile swing-around-the-circle visiting Local Sections he decided not to tell his audiences how to organize and operate a state geological survey. As the director for 28 years, of the Missouri Bureau of Geology and Mines (which, he concedes, is the best in existence) lie could have done this to good advantage; but he preferred to discuss affairs of the Institute. In 25 speeches made to member and student groups, he dwelt on sonic or all of the following phases of activities. He stressed the vital importance of the Local Sections in providing the opportunity for members to associate under pleasant auspices with others in their profession. The benefits of Institute membership he classified as tangible and intangible. Of the two lie felt that the latter were more important. A man's progress in his profession depends in large measure upon a broad acquaintance and friendship with others. The Institute is incomparably the best medium for meeting fellow engineers. The primary, function of the new enlarged Membership Committee is to obtain a comprehensive list of engineers eligible to be members, always hearing in mind that the standards of membership must not be lowered in any degree. These men are to be invited by individual letter to apply for membership. Whether they become members or not is of less importance; but it is vital that no qualified man should be outside the fold for the reason that membership had never been suggested to him. Dr. Buehler emphasized repeatedly his feeling that the younger man that had never had Institute membership brought specifically to his attention had been cheated:

Refractories and slags play a vital role in the production of stainless steel. They are in intimate contact with the steel at temperature, and chemical reactions between the three can and do occur. These interactions can be beneficial, in the case of active tundish and ladle slags, and negative in the case of refractory erosion and slag entrapment. This paper presents the results of both plant trials and laboratory investigations of steel/refractory and slag/refractory interactions. The interaction mechanisms observed are described leading to selection criteria for new refractory products. Keywords: slags, steels, refractories, reaction mechanisms.

"Abstract. The paper shows the benefits and accomplishments of this relatively new but established product on the pile foundations market in the United States. The first part of the paper describes general aspects of the design, construction, and geotechnical capacity (designed and in situ) of Tapertube piles. The qualities resulting from the design and manufacturing of the Tapertube pile will be presented first.The second part of the paper describes the installation of Tapertube piles for an addition to Terminal One at JFK International Airport and The Edge development. A carbon print analysis for both projects is developed using the EFFC-DFI carbon print calculator. For the JFK project the benefits of using BIM in redesigning the project for Tapertube piles are explored.General description.The tapertube pile consists of a tapered tubular bottom section attached to a conventional pipe extension. The 50 ksi steel for the taper is mechanically folded into a polygonal cross-section (usually 12-sided) with a longitudinal welded seam. The tapers are fabricated in many sizes with bottom diameters varying from 8 to 14 inches, top diameters from 12 to 20 inches, lengths from 10 to 35 feet, and steel thicknesses from 3/16 to 1/2 inch. A conical steel shoe or a flat plate is welded to the bottom of the taper and the top of the taper is circularized to match the outside diameter of the pipe extension. The inclination of the tapers is generally between 1.5% and 2.5%.The pipe extension is fully butt welded to the taper and has the necessary length for the pile to develop the requisite capacity. Concrete fill for the piles is generally 4,000 p.s.i. or more. Typically the piles are shopassembled and shipped to jobsites in lengths to 80 feet."

The design of tailings containment structures relies on well-understood engineering methods being applied consistently. Unfortunately, not all facilities are designed or managed to the same level of competency. To standardize procedures, the Mining Association of Canada (MAC, 1998) prepared a “Guide to Tailings Management.” It recommended review of designs and an assessment of the risks associated with tailings impoundments be implemented during all phases of amine life. This paper describes how the quantitative risk assessment (QRA) process can be used to define risks associated with the design, construction and operation of a tailings management facility. And it discusses how, once the risk tolerability curves (RTC) are defined for a project, they can assist the risk management (RM) process.

The Embankment Place office development over the platforms of Charing Cross Railway Station in London has presented many interesting and unusual problems to the engineers involved with the project. The main structure is supported on widely-spaced columns which penetrate the station platforms and underlying vaults, calling for high capacity foundations built in severely restricted spaces. Hand-dug underreamed piles were chosen. Whilst a conventional design approach was taken to size the piles, a detailed appraisal of ground movements was made to explore the effects of the new building loads on the existing foundations, not only beyond the site perimeter but also those amongst the new foundations. Before construction of the pile proper could take place, it was necessary to sink a caisson through the upper unstable and water-bearing soils. This being identified as a high-risk item, a full-scale trial was carried out prior to embarking on the main works. The twenty-one hand-dug piles were 2.5m diameter, over 30m deep and belled out to between 4.5m and 6.6m.
