"The belief is now fairly general among the coal-mining people of the United States that one of the most effective methods of preventing wide-spread coalmine .explosions is that of introducing rock-dust into coal mines to reduce the inflammability or explosibility of dusts found on mine surfaces. Notwithstanding this belief it is improbable that even a small fraction of 1 per cent of the coal mines of the United States are at all adequately protected by rock-dust; in a survey made in the summer, and fall of 1927 it was found that but 463 out of more than 7,000 operating bituminous and lignitic coal mines were using rock-dust at all, and that few if any of the 463 were fully or adequately protected by rock-dusting.Inspections of a large number of mines through the United States by Bureau of Mines safety division field men have failed to reveal a single adequately rock-dusted mine, Yet there is no doubt that many of the owners and operators believed their properties to be fully protected by rock-dust against the much-feared hazard of widespread explosion. Two of the most disastrous explosions of recent years occurred at properties where the owners were definitely of the belief that the rock-dusting done had made the mines immune to widespread explosion involving coal-dust. In both instances it was found that although some rock-dusting had been done, the mines were more nearly 1 per cent than 100 per cent rock-dusted.This condition as to inefficiency or insufficiency of rock-dusting in the coal mines of the United States is almost universal, and unless our mining men awaken to its, dangers, the disaster record of the coal-mining industry is much more likely to become worse than better. During the fiscal year ended June 30, 1928, the attention of the safety division of the United States Bureau of Mines was called to. 22 explosions With 320 fatalities in bituminous coal mines of the United States; and of the 22 explosions there were 10 with 258 fatalities in mines that were listed as having used rock-dust; upon investigation it was found that as usual the rock-dusting had been anything but comprehensive or adequate. It is probable that in a very few instances last year the relatively small amount of rock-dusting stopped the .extension of the explosion and thus prevented further expansion of the death list. In general, however, the explosions occurred in a part of the mine that had not been rock-dusted - and right here is the crux of the failure or probable failure of present-day rock-dusting in the United States, There should be no part of any bituminous or coal mine in which all of the exposed surfaces (roof, ribs, and floor) have not been thoroughly rock-dusted and kept thoroughly rock-dusted. In other words, all exposed surfaces of all accessible places in bituminous and lignitic coal mines should be kept covered with rock-dust so that the combined rock-dust coal-dust mixture at all times and in all places has less than 35 or 40 per cent combustible matter, The only exception to this rule is that surfaces which are wet (and this does not mean merely damp) need not be rock-dusted. However, a mine opening may have several inches of water on the floor and yet have much explosive dust on the ribs and roof above the water; in such places the ribs and roof should be kept rock-dusted, irrespective of the wetness of the floor."

"The carnotite ores of Colorado and Utah were the chief source of radium for many years. In the fall of 1922; news of the very rich deposits of radium ore discovered in the Belgian Congo caused almost complete cessation of domestic production. The Congo ore contains pitchblend and a number of alteration products, including the new minerals bequerelite, curite, kasolite, stasite, and dewindite. Reports indicate that a considerable amount of ore containing over 50 per cent U308 has been produced. It seemed certain that radium could be extracted from the Congo ore at a much lower cost than from the relatively low-grade carnotite; and in order to preserve their elaborate marketing organizations, the larger American producers made agreements with the Belgium syndicate to market the foreign product in this country. The price of radium has been held at $70 per milligram. Extraction of radium from carnotite by the usual methods does not appear to be profitable at that price. With one exception, all American firms ceased production after their accumulated stocks of ore had been treated. The United States Radium Co., which continued to treat carnotite for several years, has recently shut down its plant.Although there is probably a large profit derivable from the treatment of Congo ore, it is not likely that the Belgium Syndicate will meet sufficient competition to cause any reduction in the present quotations. It is not known how long the high-grade ore will last, but when exhausted the price of radium may be expected to advance. According to K. L. Kithil3 it is the opinion of interested parties that, so far as can be forseen, radium will in future years never again be obtained at the present low price.Carnotite contains the valuable element, vanadium, which is not present in the Congo ore. The recovery of-vanadium is an important factor in the cost of extracting radium from carnotite, and under favorable conditions carnotite can be profitably treated for the vanadium alone. Vanadifecous sandstones are found ranging all the way from roscoelite (vanadium mica), containing little or no uranium; to a high-uranium and low-vanadium carnotite. Ores containing 2 per cent or more U308 are classed as radium ore and are sold on the basis of the uranium content, which is proportional to the radium content. Only in exceptional cases have ores been evaluated on the basis of both the radium and vanadium contents, although the presence of vanadium has undoubtedly influenced the price of radium ore."

This report (Safety File 3.0) is the fourth in a nine-part series of recommendations addressing the functional safety of processor-controlled mining equipment. It is part of a risk-based system safety process encompassing hardware, software, humans, and the operating environment for the equipment’s life cycle. Figure 1 shows a safety framework containing these recommendations. The reports in this series address the various life cycle stages of inception, design, approval and certification, commissioning, operation, maintenance, and decommissioning. These recommendations were developed as a joint project between the National Institute for Occupational Safety and Health and the Mine Safety and Health Administration. They are intended for use by mining companies, original equipment manufacturers, and aftermarket suppliers to these mining companies. Users of these reports are expected to consider the set in total during the design cycle. • 1.0 Safety Introduction.—This is an introductory report for the general mining industry. It provides basic system/software safety concepts, discusses the need for mining to address the functional safety of programmable electronics (PE), and includes the benefits of implementing a system/software safety program. • 2.1 System Safety and 2.2 Software Safety.—These reports draw heavily from International Electrotechnical Commission (IEC) standard IEC 61508 [IEC 1998a,b,c,d,e,f,g] and other standards. The scope is “surface and underground safety-related mining systems employing embedded, networked, and nonnetworked programmable electronics.” System safety seeks to design safety into all phases of the entire system. Software is a subsystem; thus, software safety is a part of the system’s safety. • 3.0 Safety File.—This report contains the documentation that demonstrates the level of safety built into the system and identifies limitations for the system’s use and operation. In essence, it is a “proof of safety” that the system and its operation meet the appropriate level of safety for the intended application. It starts from the beginning of the design, is maintained during the full lifecycle of the system, and provides administrative support for the safety program of the full system. • 4.0 Safety Assessment.—The independent assessment of the safety file is addressed. It establishes consistent methods to determine the completeness and suitability of safety evidence and justifications. This assessment could be conducted by an independent third party. • 5.0 Safety Framework Guidance.—It is intended to supplement the safety framework reports with guidance providing users with additional information. The purpose is to assist users in applying the concepts presented. In other words, the safety framework is what needs to be done and the guidance is the how it can be done. The guidance information reinforces the concepts, describes various methodologies that can be used, and gives examples and references. It also gives information on the benefits and drawbacks of various methodologies. The guidance reports are not intended to promote a single methodology or to be an exhaustive treatment of the subject material. They provide information and references so that the user can more intelligently choose and implement the appropriate methodologies given the user’s application and capabilities. [ ]

GLOSSARY Acid- and toxic-forming materials. Rock or coal layers containing significant amounts of pyrite or other minerals which, if exposed by coal mining, will cause acid or toxic drainage to occur when acted upon by air and water. Add mine drainage. Any water with a pH less s than 6.0 draining from a coal mine. Water is often orange-colored because of the presence of oxidized iron. Adit. An underground mine entry at sites where coal seams are exposed at ground level. Apple core. A complete hilltop (360°) left exposed by not backfilling the highwall. Approximate original contour. The surface configuration achieved by backfilling and grading the mined area so that be reclaimed land closely resembles the general pre-mining surface configuration and blends into and complements the drainage pattern of the surrounding terrain. Area mining. A surface mining method that is carried on in level to gently rolling topography on relatively large tracts of land. Active area mine pits may be several miles long. Arid and semiarid regions. Lands, generally west t of We 100% meridian, where water use by native vegetation equals or exceeds that supplied by natural precipitation, and rainfall is insufficient to support agriculture. Annual precipitation is normally 20 inches or less. Auger mining. A mining technique often used by surface mine operators when the overburden becomes too for the coal to be mined economically using traditional surface mining methods. Large-diameter (usually 2&apos;-4&apos;) horizontal holes are drilled as much as 300 feet into We vertical Me of the coal bed by an auger. Like a bit used for boring holes in wood, coal augers consist of a cutting head with a screw-like extension. As the auger turns, the head breaks up the coal and the screw carries it back into a conveyor that loads it directly into a truck. Backfilling. The operation of refilling an excavation using material removed during the mining process. Bench. The level or flat working area at the base of the highwall that is excavated between the exposed coal seam and the original surface of the ground. Bucket wheel excavator. A continuous-digging machine that uses a rotating vertical wheel with buckets for large-scale stripping and excavating. Cessation Order. Order to cease mining issued to a coal operator by the regulatory authority due to severity of violations, Contemporaneous reclamation. Restoration of mined land that occurs as soon as practical after coal removal. Contour mining. A mining method commonly used in eastern mountainous topography where coal is removed in a narrow strip around the hillside. The extent of the cut into the hillside is determined by the depth of overburden at the highwall compared with the thickness of the coal seam. Daylighting. A term used to describe a surface mining procedure that exposes abandoned underground coal mine workings to enable removal of the remaining coal. Daylighting is followed by land reclamation. Dragline. An excavating machine, usually used in large, flat areas, that drags a bucket (which holds up to 220 cubic yards of material) toward the machine by cables, loads it with spoil, and then hoists it wit a boom up to 350 feet long, allowing the machine to excavate wide benches by depositing the spoil hundreds of feet away from the highwall. Grading. The process of smoothing the disturbed areas of the mine site, after the coal is removed, to closely approximate the pre¬mining terrain. Haul road. Any road constructed, improved, maintained, or used by mine operators and located within the permit area. Head-of-hollow fill. See "valley rill." Highwall. The cliff-like excavated face of exposed overburden and coal in a surface mine. Hydroseeding. Planting of seed mixed in a water medium. Water, mulch, lime, fertilizer, and seed are blended and sprayed onto regarded soil.

Materials from phosphate rock mining and processing were analyzed for fluorine and uranium content as part of the Bureau of Mines program to more effectively recover resources from mining and mineral processing reject materials. The analysis of both feed and waste materials generated in the beneficiation and acidulation of phosphate rock was performed to obtain information on the disposition and concentration of these elements in phosphate rock processing streams. Fluorine was determined by a distillation-titration method and by ion chromatography. Uranium was determined by the dibenzoylmethane (DBM) method, the reliability being confirmed by roundrobin analysis of samples within Bureau of Mines research centers. National Bureau of Standards (NBS) Standard Reference Material (SRM) 120b-Phosphate Rock was also analyzed for fluorine and uranium to verify procedures and results. Fluorine values ranged from 0.3 wt-pct in gypsum filter cake to 3.7 wt-pct in phosphate rock concentrate. Uranium ranged from less than 5 µg/g in gypsum filter cake to 200 µg/g in a dried clay slime.

"The economic features of coal mining must take into account the drain on the man power. Study over a recent 5-year period reveals an average yearly loss of 2,125 underground men, of whom 1,182 are killed by falls of roof and coal, and according to recent data compiled by the Bureau of Mines it is estimated that 54,000 men sustain lost—time disabling injuries from the same cause.Under normal activity of the mines approximately 50 lives are taken by falls of roof during every 10 days the mines are operated, or 5 persons are killed per day, which, at a compensation charge of $5,000 each, amounts to $25,000 per day. Add to this the similar number killed by causes other than by falls of roof and we have $50,000 as an approximate daily fatality charge for each day the mines operate. On the basis of 200 working days per year, this amounts to the alarming figure of $10,000,000 annually. An investment at 5 per cent to yield this amount would require $200,000,000, which is equivalent to a capital investment of $0.40 per ton of coal, produced annually, on the basis of 500 million tons. These figures apply only to fatalities for which a fixed or stated amount is paid for compensation and do not include the compensation and hospitalization paid for the approximately 103,000 upon whom idleness is enforced by a lost-time disability injury. In one State having an annual production equal to approximately 30 per cent of the total bituminous production of the United States, the most recent data shows that for each fatality there are eight serious injuries causing 60 days or more lost time. The frequency rate of accidents for that State is less than the average for the United States, and so a conservative comparison may be made by using the figures of that State in estimating the total serious injuries, which gives 16,000 as losing 60 days or more. The average number of underground employees in the coal mines of the United States for the past 10 years has been approximately 570,000; the annual underground fatalities amount to approximately 2,000 and non-fatalities to 108,000, or a total of 110,000, which means that one out of every 5.17 employees ,or 19.3 per cent, receives an injury which results in either a fatality or a lost-time disability; this figure in a little over five years will reach a number equivalent to 100 per cent of all underground workmen. Against this is the record of many miners who have worked 25 years or more without a lost—time disability injury, indicating that coal mining is not necessarily productive of injury or death to workers.In the matter of falls of roof, it is found that 85 to 87 per cent of all fatalities occur at the working face, face of a room or entry, or in pillar work. It is at the working face that the greatest roof hazards are encountered, and when about 75 per cent of the underground employees spend approximately 90 per cent of their working hours at the face, it is logical that there is where injury is most likely to occur.Although the roof hazards may vary in different beds of coal, experience has proved that roof hazards can be successfully controlled in all coal mines, essentially regardless of the character of the roof if constant care and attention are given by the management and employees in applying the principle of safety. In almost every mining district some companies maintain an extremely low frequency rate of injury, whereas in the same district with essentially the same roof conditions the opposite is true.A study has recently been made of a number of reports upon fatalities in one district of one of the principal coal—producing States and it showed that 50 lives had been lost at 23 mines as the result of falls of roof or sides. These reports were made by the mine officials, safety engineers, and safety committees and an attempt was made to give both the cause and the person or factor responsible for the fatality. The mine pager phone to public telephone interconnect system permits mine paging telephones to be interconnected with the public phone system, thereby providing emergency and off-hours communications. The system, a push-to-talk, push-to-listen system, is compatible with any mine paging system utilizing dc voltage on the line to enable paging. A tone generator of the proper frequency enables an outside caller to page over mine pager system loudspeakers. A caller inside the mine can make an outgoing call to a preselected, preprogramed number with the use of a proper frequency tone generator. The system includes an automatic hangup timer that restricts the time off-line due to a wrong number or incorrect hangup to 20 seconds and 6 minutes, respectively.

In studies of mine and tunnel ventilation conducted by engineers of the Bureau of Mines , various smokes are used to determine flow of air , direction of the currents , and the rate of mixing . The instrument for generating streams of sulphur trioxide (SO ) smoke , shown in the drawing herewith, has been found helpful , convenient and practicable for such purposes ; it is also especially useful in some places where fire for generating smokes is not permissible . The apparatus consists simply of a glass tube containing fuming sulphuric acid on granular pumice stone , attached to a rubber syringe bulb . When a squeeze of the bulb blows air through the tube , a stream of dense white smoke evolves, which follows and makes visible the air currents . The tubes after preparation are hermetically sealed to prevent deterioration . When ready for use , they are easily broken open at a file scratch for the purpose on both tips , and inserted into a rubber nipple at the bulb . Sometimes it is of advantage to attach them to a longer rubber tube leading to the bulb . Tubes of the size shown (about 1/2 inch outside diameter by 5 inches long) are usually sufficient for the needs of a day . Thereafter they cease to fume because the acid absorbs moisture from the air and looses its effectiveness . Smaller sizes of tubes and larger have been made , but the size shown has proved most practicable . Immediately after a person has finished using a tube it should be buried or disposed of in a place where it will not be touched by shoes or clothing, or come in contact with wooden floors , furniture , or metals , because the acid , which will eventually leak from the tubes , is highly corrosive . -- Reports of Investigations , Department of the Interior , Bureau of Mines .

Approach The present design of the dust helmet prevents use of the miner&apos;s cap lamp when the front shield is raised, When the shield is raised I the cap lamp is directed at the mine roof or back, making the lamp useless in the event the miner needs to wear his self-rescuer. A flip bracket was designed by the Bureau of Mines to correct this situation. How It Works The safety hat is modified by the addition of lamp clips, a bracket and a hinge on the cap lamp and face shield. Two clips hold the lamp cord firmly to the helmet. When the safety hat shield is raised, compressive force from the lamp cord causes the cap lamp hinge to swing open about 90%, allowing the lamp

To help mine safety and health professionals manage and reduce hazardous noise exposure with a software tool that simplifies the record-keeping and analysis process. Background Hearing loss resulting from overexposure to noise continues to be a problem throughout the U.S. mining industry. Studies indicate that 70%?90% of all miners have a noise-induced hearing loss severe enough to be classified as a hearing disability by the time they reach retirement age. To address this problem, the National Institute for Occupational Safety and Health (NIOSH) developed the Determination of Sound Exposures (DOSES) software program. DOSES is designed specifically for use by mine management and safety personnel to simplify the record-keeping and analysis associated with time-motion studies, making it easier to identify and solve noise exposure problems.

Approach Two types of noise control packages have been developed, that can be readily fabricated and installed into bulldozers by mine personnel. How It Works Both of the noise control packages reduce the noise exposure of the operator by two techniques, blocking the noise paths or absorbing the sound before it reaches the operator. Develop quieted, hand-held pneumatic percussion drills that can replace standard hand-held drills in many underground mining applications. The Problem Percussion drills are the noisiest machines used in underground mines; noise levels of 115 to 120 dBA are common, requiring the use of hearing protectors and limiting the time a miner is permit-ted to operate the drill during an 8-hour shift. Hand-held pneumatic percussion drills pose a particularly difficult problem because the operator cannot be isolated from the noise source (e.g., an acoustical cab), and alternative technologies for drilling roof bolt holes and blastholes in tight quarters are not readily available.
