The relative infrequency of fatalities and other serious events can give them an appearance of being random, of being beyond any reasonable degree of anticipation and prevention. In fact, the vast majority of these events result from exposures that are identifiable, measurable, and manageable. The lessons of prominent incidents, such as the Space Shuttle Columbia, OxyÆs Piper Alpha, Esso Longford, BP Texas City, Massey Mine, as well as lessons from single fatality events, is that alongside the proximate causes of each incident, there also coexisted an underlying fabric of systems, mechanisms, and culture that allowed risk in the workplace to persist and often to become normalised. All of these ingredients of the organisationÆs safety fabric lend themselves to the influence and intervention of senior leaders. The difficulty is that traditional injury prevention paradigms are not sufficient for addressing the precursors that lead up to catastrophic and fatal events. A recent study of such events, led by Dr Thomas Krause concluded that the descriptive aspect of HeinrichÆs safety triangle is useful, but that the predictive aspect is not. The study surfaced reasons why a reduction in less serious injuries does not necessarily correspond to a proportionate reduction in catastrophic and fatal risks. This presentation expands on the findings to include the concept of exposure to the event potential linked to precursor events and the framework for leading an organisation to be both cognisant and effective in eliminating such events.CITATION:Russell, R, 2012. Spiral concentrator is one of the most commonly used gravity concentration devices in mineral processing. The particles are classified based on their specific gravity and size. The present work analyses the experimental and numerical investigations of the water flow field, such as fluid depth and free surface primary velocity of two high gravity spiral concentrators at different flow rates. The measurement techniques use a flow depth gauge and flow visualisation technique adapting lycopodium powder as the tracer. Flow features in terms of liquid depth and primary velocity at the free surface of the flow are captured across the trough in two different high gravity spirals. A highspeed motion camera with an imaging frequency of 5400 frames per second and 768 × 768 pixel resolution is utilised to track the tracer particles in the liquid flow on the dark surface background. Further, the two-phase flow is studied numerically on high gravity spirals based on the volume of fluid (VOF) approach utilising the RNG k-ε model and Reynolds Stress Model (RSM) for predicting the fluid flow on trough surfaces such as water depth and free surface primary velocity. The CFD predictions of the flow field for the two spirals are attempted to validate against experimental data. The high gravity spiral concentrators show lower flow depths and free surface primary velocities but steep velocities on the outer trough zone compared to that of low gravity spiral concentrators (conventional coal spirals). Among the computationally tested high gravity spirals, higher pitch and higher secondary slope design shows greater flow characteristics.

In order to clarify the gas flow and motion of coke and reaction in the raceway, the mathematical raceway model was developed. This model consists of the equations of momentum and continuity of gas and coke. The simulated results were compared with the results of the cold and hot experimental model. The relationship between the blast volume and the raceway size, the pressure distribution and the velocity distribution of coke are in good agreement with the experimental results. Moreover the equations of reaction and heat conservation were simultaneously calculated with the dynamic equations. Distributions of gas composi- itions and temperature were simulated and found to be in good agreement with the distributions of gas compositions of CO, CO2 on the tuyere level measur- ed in the hot experimental model.

The mining industry has aggressively sought competitiveness through increased productivity and return on investment. A primary strategy has been the pursuit of economies of scale. This has also been dependent upon the implementation of new technology and systems integration. System integration has been accomplished through the implementation of mine dispatch, global positioning and mine wide communication technologies. These technologies have tended to advance at different rates of development and implementation. The result has been that across the industry, and even within companies, mines are at various stages of operational system re-engineering in these areas, driven by improvements in productivity and operational efficiency. Although this strategy has been successful, little effort has been made to analyse change in operational performance in order to understand and model the effects of scale in a quantitative manner. This paper considers the particular impact of equipment scale, together with potential future trends. It then presents and demonstrates a preliminary method to quantify through simulation the anticipated benefits of economies of scale.

AngloGold operates various gold mines in South Africa, the largest of which are situated in the North West province. The impact of AngloGold Vaal River mining operations on the environment and, specifically, on aquifers and groundwater is assessed. The location of such aquifers and plumes is determined by remote-sensing and ground-sensing operations, followed up with the correct placement of monitoring boreholes from which information such as water rest levels, chemistry and geological data is gathered. This study utilises an integrated approach consisting of the following mutually reinforcing phases: Phase 1: Airborne multi spectral scanning; Phase 2: Airborne thermal remote sensing; Phase 3: Ground resistivity survey; Phase 4: Borehole placement; and Phase 5: Sampling and monitoring. The integration of the investigation tools and phases in use at AngloGold Vaal River has been found to be very useful in determining the exact sources of contamination, the direction of flow of such contamination, the positioning of interceptive measures and the analysis of such contamination. This adopted approach will enable the development of flow transport models that will further enhance our knowledge of contaminants in aquifers at the mine and enable management to conduct educated interventions.

Located 350 km west of Sydney and 20 km outside Orange, NSW, lies the Ridgeway Gold Mine û NewcrestÆs flagship Underground Gold Mine. Key to success at Ridgeway has been the fast tracked development of the 3.8 km, 1 in 5.3 conveyor decline which houses the 850 t/hr conveying system feeding the dedicated Ridgeway Concentrator, built adjacent to the existing Cadia Hill Concentrator. Eroc was contracted to develop the steep conveyor decline, drawing on its personnel and experience from past metalliferous, tunnelling and coal mining projects to complete the ambitious schedule within 12 months. This paper deals with the complexities and experiences in safely delivering what is understood to be one of AustraliaÆs steepest declines of any substantial length in a metalliferous mine.

About 74% of global seismic energy release occurs in the Circum-Pacific zone. Worldwide, about 71% of seismic energy release is in the form of shallow earthquakes, 27% as intermediate, and 2% as deep earthquakes. Seismic energy release is variable in time and has been shown to correlate with the amplitude of the Chandler wobble. It was high during the period 1949 to 1967, and is now at a rather low level, with the latest earthquake over M = 8.0 having taken place in Indonesia in 1971. The average worldwide recurrence rate for the expected largest magnitude earthquake (M "9.5) has been estimated as several tens of years, in marked contrast to the several tens of thousands to hundreds of thousands of years separating the largest volcanic eruptions. Geological studies are needed to establish reliable mean intervals between large earthquakes, and to detect the e;:istence of long-term fluctuations in the ste of occurrence. Seismic gaps have proved seful in static forecasting of large earthquakes, by identifying places where closer study of the variation with time of seismicity, deformation, and other parameters, may lead to the dynamic p_ediction of time, place and magnitude of earthquakes. The model of stress concentration at asperities explains the main features of seismicity in the Circum-Pacific. Slow earthquakes have been detected instrumentally, notably an event immediately preceding the 22 May 1960 Chile earthquake, the largest this century. It seems likely that this type of deformation may explain both instrumentally detected and macroscopically observed pre-seismic deformation, as well as unusual animal behaviour reported before some earthquakes. Seismic and volcanic events are interdependent, with large earthquakes commonly triggering eruptions. Melting of rock and intrusion of magma is likely to accompany many earthquakes.

A review of all major TBM projects from 1974 to 1990 has been undertaken and the results entered into a database. A similar database was prepared from information obtained from a principal TBM manufacturer covering the period 1952 to 1990. The database includes information such as machine specifications, machine performance data, tunnel specifications and ground characteristics. The database can be used as a guide to gauge potential machine performance in a range of ground conditions and tunnel geometries. The review also indicates that current TBM technology is limited to certain ground strength conditions.

The economic significance of a fundamental understanding of the mineral pyrrhotite is dis- cussed. Particular emphasis is placed on the relationship of other sulphide minerals to the pyrrhotite when it is the major sulphide mineral present in an ore body. Studies of the Kambalda nickel deposit are used to give a better under- standing of (a) the genesis and other character- istics of a particular ore body (b) the textural relationships between pyrrhotite and other sul- phide minerals (c) magnetic properties of natural pyrrhotite, and (e) the oxidation and supergene processes that take place in a sulphide ore body.

Desulfurisation of steel during ladle refining is governed by both thermodynamics and kinetics. For effective desulfurisation, thermodynamically, a low partial pressure of oxygen at the steel-slag interface and highly basic slag which possess a large sulfide capacity are desired. Kinetically, the mass transfer coefficient of sulfur in the liquid steel and in the slag determines the rate of desulfurisation. A fast desulfurisation rate is achieved by maintaining sufficiently strong argon stirring of the melt. In the present work, sulfur mass balance for steel and slag was performed after the desulfurisation treatment for several industrial heats. The decrease in the sulfur quantity of the liquid steel was more than the increase in the sulfur quantity of the slag. The discrepancy in the sulfur mass balance suggests that the sulfur could be transferred to: (a) the gas (via slag) phase; or to (b) the refractory (crucible) phase. The preliminary investigation of the refractory samples taken at slag-refractory and steel-refractory interface showed no interaction of sulfur with the refractory. Therefore, the possibility of sulfur transfer to the gas phase was verified by carrying out melting experiments of sulfur containing slag in an argon/air atmosphere. Sulfur containing slag was initially melted in an MgO-C crucible under an argon atmosphere followed by creating the air atmosphere just above the melt. Careful sampling and analysis from the cold spots of the crucible, much above the melt surface, revealed formation of a fibrous structure. Scanning electron microscopy energy dispersive X-ray spectroscopy (SEM-EDS) analysis of these fibrous deposits confirmed the presence of oxides and sulfides. Oxidation of sulfur containing slag under air atmosphere was also performed using a thermogravimetric analysis (TGA) set-up. It showed a constant sulfur loss in the range of 1300– 1500°C. Thermodynamic calculations carried out using FactSage™ ver 8.3 for these experimental conditions are in good agreement with the observed results.

The legacy of underground coal mining may be a land surface which, in parts, may be unstable in the long term. A case study analysis has been undertaken to explain some of the hazards and how the risk may be assessed and managed.The Ipswich Coal Measures were extensively mined with shallow underground workings, which now underlie a number of suburbs in the eastern side of the city of Ipswich in south eastern Queensland. The area was subject to underground mining from the 1840s to 1997. In general, the mines were small, family-owned operations that were restricted by complex geology, interactions with neighbouring operations and ownership of surface facilities. The oldest available mine plans date back to the late 1890s.The residents in these mining areas have a long history of unexpected mining subsidence events. A level of risk management for property owners has been implemented by the Ipswich City Council, which requires a mining subsidence risk assessment to be submitted as part of building applications for property development in the areas suspected of having mining subsidence risk. The originally estimated mine life at Kidston (starting 1984) was 12 years. Through a continuing process of re-evaluation and improvement in the operation and by expanding the geological knowledge in the region, the mine life has been considerably extended. With falling grades and harder ore, the additional reserves have come with increasing risk - not only in an economic sense associated with diminishing profit margins but also operationally, requiring innovative milling technology and geotechnical designs to provide a lower cost environment for continued profitable mine production.

The amount of material extracted from each blasted ring in sublevel caving mines is vital when deciding the overall mine production target. The orientation of the production drifts is normally perpendicular to the strike of the orebody, and rings are blasted from the hanging wall side to the footwall side in a retreating manner. For an inclined orebody, a given ring will contain different amounts of material based on its relative position along the production drift. This layout creates a distinctive problem with respect to the extraction ratio optimisation along the axis of the production drift. This paper describes the current drawpoint control strategy practiced at LKAB and presents the variation of drawpoint performance parameters along the production drifts. It uses automatically generated data from a bucket weighing system that measures tonnage drawn per bucket, and it classifies the blasted rings into four zones based on their relative position along the production drift. It considers the loading procedure for blasted rings at Kiirunavaara mine and analyses the variations in drawpoint performance along the production drift. Indian iron ores with less than 62 per cent Fe are currently unexploited, except for a few sectors which export them to foreign countries. It is anticipated that by 2012 the countryÆs domestic steel production is likely to rise to the tune of 60 million tonnes per annum. To meet this requirement, it is necessary to utilise not only the existing high-grade deposits but it is also necessary to look into exploiting the leftover iron ores of lower grades. Hence, reserves with Fe content 62 per cent and below (particularly the ores with In the present paper, an attempt has been made to beneficiate three different types of low-grade iron ores selected from three different parts (states) of the country. In the paper, alternative routes for beneficiation and effective utilisation have been tested using different techniques in the laboratory in an attempt to match their (Al2O3/SiO2) ratio after thorough characterisation and liberation studies. The paper also encompasses the geological formations, occurrences and distribution of Indian iron ores as well as highlighting the problems associated with the fines, possible metallurgical routes, and their market segregation from the perspective of end users.

"Unconventional gas resources, including coal seam gas (CSG) reservoirs, can be located above and below aquifers that provide groundwater for agricultural, industrial, urban and other uses. Development of these resources may, therefore, lead to potential impacts on groundwater supply.Management of reservoir/aquifer interconnections necessitates a program of focused data acquisition. For example, groundwater transfer from shallow aquifers to underlying CSG reservoirs may occur where CSG-containing units subcrop beneath shallow aquifers, possibly at significant distances from operating wells.Arrow Energy is undertaking studies of connectivity between aquifers including an investigation in collaboration with the Queensland Office of Groundwater Impact Assessment to evaluate connectivity between the Condamine Alluvium (CA) and CSG-containing Walloon Coal Measures (WCM).The Condamine Valley hosts highly productive agricultural land with groundwater resources heavily utilised for irrigation. Water demand outstrips available supply and groundwater has been subject to restrictions on further development since the early 1970s. Developments that could have an additional impact on groundwater resources in the CA are of major concern to local landholders. This paper describes: planned field investigations at two sites: 1. Stratheden, where pumping from the WCM will be undertaken 2. The carbon reduction of tantalum pentoxide was investigated in a plasma-arc furnace. The sample was a mixture of tantalum pentoxide and graphite, vacuum pressed into a briquette and was reduced at about 3100¦C. Argon was used as the are gas. The optimum mixing ratio of carbon and tantalum pentoxide, C/Ta205 is 5.10--5.15. The sample, weighing 7 g is fused within 60^ 70 seconds by plasma-arc heating and the reduction of molten tantalum oxide with solid carbon pro- ceeds rapidly to approximately 99.5 % Ta during this period. The reduction that follows in the molten state is slow. Within about 10 minutes, the maximum tantalum content of the massive metal product reaches 99.9 % and the oxygen and carbon remaining in the product are 50 ppm and 400. 500 ppm respectively.

The Burdekin Basin in the Townsville hinterland is an intracratonic extensional basin which contains up to 4500 m of marine and continental siliciclastics and carbonates. The basin sequence is recognised as having accumulated during five major episodes of deposition. Facies analyses of these show that each consists of one or more transgressive or regressive phases. Each episode led to the accumulation of thick sequences in several half-grabens in a transtensional back-arc setting. The depositional episodes can be recognised in comparable sequences in the Broken River Province to the west and Drummond Basin to the south.An Early Devonian episode resulted in the deposi-tion of dominantly marine facies, currently mapped as Fanning River Group, in the northern part of the Burdekin Basin. Two transgressive phases are recorded there, firstly in the Pragian, then later in the late Emsian to early Eifelian. A Middle Devonian episode is represented by dominantly marine facies of the Fanning River Group, representing the peak of a widespread late Eifelian to Givetian transgressive phase. A Late Devonian episode is characterised by continental facies of the Dotswood Group. Deposition occurred during a major regressive phase in response to regional uplift of surrounding areas, and the onset of acid-intermediate volcanism to the south. A Late Devonian to Early Carboniferous episode followed, which was a period of alternating marine transgressions and non-marine regressions that resulted in the deposition of the Keelbottom Group. A final Early Carboniferous episode, marked by the deposition of calc-alkaline volcanics and non-marine faciesòof the Glenrock Group, was a direct outcome of Early Carboniferous subduction further to the east. Compression of the basin followed in the Late Carboniferous, resulting in folding and reactivation of basement faults.

Preconcentration, through gravity separation (Wallace et al, 2015), magnetic separation and particle size (Clout, 2013), has been used in many historical projects to convert low grade material into ore. More recently, pre-concentration technologies such as bulk sorting (Kurth, 2017), screening (Bearman, 2014) and coarse particle flotation (Vollert et al, 2019) have also demonstrated their ability to progressively reject waste through the value chain. These technologies can not only deliver stepchange improvements to the economics of mining operations, but can transform the environmental and social aspects by minimising water and energy consumption and significantly reduce the quantity of fine tailings produced. However, implementing progressive waste rejection requires technologies and approaches to be combined and applied in the right way to capture the value. From a technical perspective, progressive waste rejection introduces challenges related to the orebody, equipment performance, engineering and understanding the value chain from the resource through to final product. From a management perspective, the key issues are related to people and processes, in particular; motivating the business, challenging the paradigms, and introducing new ways of operating within the constraint of existing management structures to manage risks and minimise lost opportunity costs. This combination of technical and management issues means that progressive waste rejection is a complex undertaking for mining companies, yet the reward for the business and other stakeholders is potentially high and should not be ignored. This paper presents how progressive waste rejection can offer step-change improvements to the business case for high and low grade greenfield and brownfield projects based on case studies completed by Ausenco. The paper then outlines different implementation approaches from concept studies to execution to address the various technical and management challenges outlined above.

A positive displacement pump is being dev eloped for pipeline transportation of coking coal to a -50 mm export specification, at a con- centration and velocity which will allow trans- portation in the plug flow region, minimising degradation of the particle structure, and pipe= line friction losses which will be lower than with turbulent or laminar flow. Pump operation is by hydraulic powered free pistons in a rota- ting cylinder block, with cylinders overlapped by stationary inlet and discharge ports in a water lubricated and sealed rotary valve. Before this development coking coal was not considered suitable for transportation in slurry form, for the valves in the double act- ing duplex piston pumps used for slurry pipe- lines, cannot be divorced from the slurry and require that the solid material must be reduced to a fines'consistency before introduction into the pump suction. The hydraulic transportation of 10 million tonnes of -50 mm coking coal, 50 km from the Bargo area west of the escarpment to an offshore shiploader at Port Kembla is feasible. Study shows that economic factors alone would dictate that the development of a pumping system for long distance transportation of coking coal is necessary.
