The relative infrequency of fatalities and other serious events can give them an appearance of being random, of being beyond any reasonable degree of anticipation and prevention. In fact, the vast majority of these events result from exposures that are identifiable, measurable, and manageable. The lessons of prominent incidents, such as the Space Shuttle Columbia, OxyÆs Piper Alpha, Esso Longford, BP Texas City, Massey Mine, as well as lessons from single fatality events, is that alongside the proximate causes of each incident, there also coexisted an underlying fabric of systems, mechanisms, and culture that allowed risk in the workplace to persist and often to become normalised. All of these ingredients of the organisationÆs safety fabric lend themselves to the influence and intervention of senior leaders. The difficulty is that traditional injury prevention paradigms are not sufficient for addressing the precursors that lead up to catastrophic and fatal events. A recent study of such events, led by Dr Thomas Krause concluded that the descriptive aspect of HeinrichÆs safety triangle is useful, but that the predictive aspect is not. The study surfaced reasons why a reduction in less serious injuries does not necessarily correspond to a proportionate reduction in catastrophic and fatal risks. This presentation expands on the findings to include the concept of exposure to the event potential linked to precursor events and the framework for leading an organisation to be both cognisant and effective in eliminating such events.CITATION:Russell, R, 2012. The paper describes an efficient method for analysing the different attachment kinematics of excavators and other linkages by taking into account digging forces, the workspace and the stability of the machines. The different attachment kinematic systems of hydraulic excavators and a æclassical analysing methodÆ will be presented. The method was integrated as a plug-in for the multibody simulation (MBS) software MSC.ADAMS to create an easy to use solution for design engineers. A comparison of the new MBS-solution with the classical static analysis is a part of this paper and shows the advantages of the new method. This takes into account the manifold and new dynamic analysing opportunities, as well as more detailed models leading to more accurate results.

Health and safety are the first priority for Australian minerals sector. Australia has a global reputation in the mining industry. However, accidents occur every year. Legislation and regulation are important to prevent the accident from occurring, so do the new technologies. It is sad to witness the collision incident persisting and the long called collision avoidance system (CAS) is still not widely used in underground mines. The main reason is that the existing systems are still not advanced enough. The current CASs utilise GPS, infrared, ultrasonic, RFID, Radar, electromagnetic etc have their limitations and disadvantages. For instance, GPS is widely used in a surface mine, but it cannot be applied in underground mine; Infrared and electromagnetic have the false positive problem.

The Argyle diamond deposits in the East Kimberley were found as a result of a long term exploration programme, begun in 1972 by the Kalumburu Joint Venture (KJV), to search for diamonds on the Kimberley craton. When CRA Exploration Pty. Limited joined the KJV in 1976, it was re-named the Ashton Joint Venture (AJV). The Kimberley craton was considered prospective by analogy with structural settings of diamondiferous kimberlites in Africa. Diamonds were found during regional drainage sampling in Smoke Creek in late 1979, which led directly to the discovery of the rich Argyle lamproite diatreme (AK1) and associated alluvial deposits of Smoke and Limestone Creeks. The diatreme intruded into quartzites, sandstones and shales of Adelaidean age, and consists predominantly of lapilli ash tuff, with minor coarse and fine ash tuffs, 'non-sandy tuff' and olivine lamproite dykes. Erosion of the diatreme has produced the alluvial deposits of Smoke Creek and Limestone Creek to the north and east. Argyle diamonds are of predominately industrial quality, and the typical diamond is a brown, irregularly shaped, frosted, heavily resorbed form with abundant graphite inclusions, well developed etch channels and prominent hexagonal surface depressions. The pre-feasibility evaluation of AK1 comprised diamond drilling, surface and underground sampling, metallurgical sampling, marketing studies and ore reserve estimation. The proven AK1 ore reserve is 61Mtonne at 6.8ct/tonne. Evaluation of the alluvial deposits gave results of Upper Smoke Creek, 580 000 tonne at 4.6ct/tonne and Limestone Creek, 1.7Mtonne at 3.5ct/tonne. Commercial production from the two alluvial deposits began in 1983 with production for the year reaching 6.2 Mct. Mining of the alluvial deposits at three to five Mct/year is proposed to continue until the start of mining of the AK1 deposit in late 1985 or early 1986 at which time a new processing plant and associated infra- structure will have been constructed adjacent to the AKl deposit in the Limestone Creek area. Production is planned at three Mtonne/year, producing 25Mct/year, which will make Western Australia the largest producer of natural diamond (by weight) in the world.

Water has played a key role in the Pilbara’s 60-year history of iron ore operations. In contrast to many of the current-day mining environments, iron ore mining was initially hosted by geological formations that were typically above the water table. Such mining operations required no dewatering or dewatering at rates that were less than mine and mining township requirements. The challenge for these operations was not pit dewatering but water supply for mining operations and municipal supply for nascent mining towns.The current industry sees new deposit styles being increasingly targeted, including channel iron and Marra Mamba Formation deposits. These deposits are often lower in the landscape and can require extensive dewatering. Furthermore, increased water demand for ore beneficiation has resulted in major programs of water-supply borefield development. Added to this is the emergence of magnetite projects and their significant levels of water use in ore beneficiation.The large volumes of water that are being extracted to support Pilbara iron ore mining is attracting attention. The Western Australian Department of Water has recently commissioned a study to assess the potential use of surplus mine water as a medium- to long-term supply for irrigated agriculture.This paper outlines the changing landscape of water management in Pilbara iron ore operations and explores strategies for maximising mining productivity. This paper will address this question from the point of view of a drill and blast engineer or researcher working in open cut and underground mining operations. The paper will establish a common set of reference standards for high-speed imaging and data capture, which engineers and researchers can apply when deciding which type of imaging technology is appropriate. It will present an overview of current image analysis technologies and practices in use across a range of appropriate scientific fields and draw lines of commonality of approach between these fields and the field of mining and blasting applications analysis and research. In addition, the paper will examine the different technologies from an image quality and data stream reference point, ease of operation, cost, and reliability parameters. Finally, this paper will present an argument for the hybridisation of established and new technologies which could provide a more beneficial approach to the use of imaging as a tool of quality control, research and analysis in the mining based industries. This will include a novel approach to digital high-speed imaging and data analysis in the field, which should deliver high-quality images and data analysis in a time frame that is more appropriate to the need for immediate action, analysis and problem solving.

It is well understood that designers and constructors must protect the existing surroundings of tunnel construction, particularly in the selection of excavation methods, ground support and waterproofing systems. However, while this can readily address the existing conditions, the tunnel owner and the designers need to consider the changes that are likely to occur during the life of the tunnel. It is typical for transport infrastructure to have a design life of at least a century. While the effects of constructing the tunnel on its surroundings are a short-term effect, the converse case, the protection of the tunnel, and the need to maintain its integrity, will affect developments over and adjacent to the tunnels beyond the foreseeable future. Decisions made during the design phase have the potential either to facilitate development or to hinder it for the long term.Examples principally from Australia, New Zealand, Singapore and Hong Kong show that the provision for future development is addressed in different ways. These have ranged from making no specific provision, putting the onus onto developers to ensure that the tunnels are not affected adversely, to including a loading allowance for future development in the performance specification or contract documents for a tunnel project.These two contrasting approaches have strong points and risks. A highly prescriptive definition of permissible loadings around the tunnel can provide unambiguous guidance for the approving authority and the developer, but runs the risk of being overly conservative to allow for all the envisaged works, while still missing a detrimental combination of effects that theoretically complies with the specified requirements. In contrast, a general requirement to not cause detriment is open to interpretation and potentially difficult negotiations to reach agreement between the tunnel owner/operator and developer and finally the appointed contractor (probably with no contractual relationship with the tunnel owner).In practice, a combination of prescriptive and performance requirements is likely to provide the optimum combination of clarity and flexibility. Nautilus Minerals (Nautilus) is a sea floor resource exploration and development company focused on developing the Solwara 1 Project, a copper-gold sea floor massive sulfide deposit in the territorial waters of Papua New Guinea, for which it holds a mining lease and associated environment permit.During the environmental impact assessment and project development phase, Nautilus completed a number of hydrodynamic modelling exercises in order to predict the nature, volume and extent of the plumes likely to be generated by the sea floor mining tools and return water discharge at the sea floor.The modelling was conducted using the suspended sediment fate model (SSFATE), developed by the United States Army Corps of Engineers and RPS APASA engineering consultants. The SSFATE model was originally developed in order to predict and manage dredging plumes, and has since been adapted for deep ocean sea floor mining. The model uses a Lagrangian approach to remove the limitations of grid resolution, and simulates far-field effects of sedimentation plumes by assuming that the average transport and turbulence associated with currents are dominant over the initial turbulence at the release point. The random walk procedure utilised in the model tracks particle size classes as they disperse and settle, and also takes into consideration any re-suspension that may occur following settlement (RPS APASA, 2014).CITATION:Grogan, R, 2016. The Shamrock Gold Mine has been brought from exploration to production within three years. Mining of the openpit is scheduled for June 1990 with commissioning in April of the 180 000 TPA carbon-in-pulp mill on old tailings and rock dumps. The development of a small openpit containing 344 000 tonnes at 3.6 g/t Au in a complex sulphide ore has required extensive metallurgical testwork to optimise recovery, and ensure profitability. The project has been undertaken in a changing economic climate, with increasing restrictive legislation. The decision to mine was further complicated by the escalating cost of installing a heap leach operation in South East Queensland. Decreasing costs of consumables, in particular sodium cyanide, and the opportunity to purchase a suitable transportable mill at a reasonable price made the project robustly viable as a CIP operation.

Anglo Coal Australia (ACA) takes a proactive position with regards to attracting and retaining employees at a graduate level. Currently, ACA has graduates in its program across all disciplines: Mining Engineering, Mechanical Engineering, Electrical Engineering, Commercial, Occupational Health and Safety, Environmental Engineering, Human Resources, Safety Engineering and Processing Engineering. The current program is three years in duration and involves two (generally) equal periods at different ACA mine sites or Brisbane Corporate Office. ACA takes an active role in promoting its status at the university level by maintaining a scholarship program. Six undergraduate scholarships are offered each year across all disciplines. Associated Vacation Employment is offered to each scholarship holder during the period of his or her university study. This commitment to education is regarded highly by future graduates. ACAÆs active role in sponsoring student bodies is also known and appreciated amongst the university population. ACAÆs graduate program has been a successful training program for future leaders of the coal mining industry. The company is committed to making the program more suited to the needs of the graduates across all disciplines and as a result the program is not stagnant but is constantly changing and evolving. Recently the æGraduate PassportÆ has been drafted with an aim to keep each graduateÆs needs in relation to experience and training firmly in focus for the duration of the three-year program. Appropriate training for each graduateÆs position is supported by the company; for instance, support through funding and time for a commercial graduateÆs CPA qualifications is provided.

The ore deposits investigated are composed primarily of hcmatite and can be grouped into three categories depending on the amount of leaching of silica and carbonate which has occurred from the original iron formation, and on the amount of recrystallization of the hematite. Minor constituents are magnetite. goethite, and quartz.Texturally. there are five different types of hematite, formed at different stages during the genesis of the ores.These are1. anhedral hematite grains formed by recrystallization of hematite already present in the original iron formation.2. hematite formed by martitization of euhedral magnetite, 3. massive aggregates of fine- to medium-grain hematite formed by recrystallization of (l) and (2).4. tabular or prismatic hematite grains occurring around the boundaries of the massive aggregates or martite euhedra. and in fractures and5. specular hematite. which is present in fractures in ore types (l) to (4).Minor and trace-element analyses, determined using an electron-probe microanalyser. indicated that only silica (less than 0·01 per cent) and cobalt (less than 0·002 per cent) were present in the magnetite and hematite from unaltered iron formation and in the hematite from the ores. Low concentrations of trace elements are typical of magnetite in ores formed as marine precipitates. On the basis of the analyses alone. it proved impossible to distinguish between the five hematite types. A determination of the variation in rare-earth trace element contents between iron oxides in the iron formation and iron oxides in the ores indicates that the rare earths were depleted during ore formation. A possible explanation is that they were removed in solution with silica as it was leached from the iron formation.

The need for formalised geotechnical reporting has not been fully explored at many operations. With the continuing need to provide safer and more efficient workplaces, the requirement for good geotechnical reporting systems (as with other reporting systems) has become of greater importance.Historically, the risk from rockfalls has been the largest cause of fatal injuries in the underground mining industry. The response of the ground to mining is generally poorly documented at many mines. At Olympic Dam, it was found that the existing corporate hazard and incident reporting (HIR) procedure did not record enough specifics about geotechnical issues. A separate and more specific geotechnical reporting process was set up to address this problem.This paper examines the role that geotechnical reporting plays at the WMC Resources Olympic Dam Mine; from reporting and rectifying day-to-day geotechnical issues that affect the operation to documenting and analysing longer term trends.

The Knelson Concentrator is a centrifugal gold concentrator developed in Canada, which utilises the combination of high gravitational ("G") forces and a unique fluidising action to efficiently recover free gold particles from minus 6 mm to fine micron gold in both alluvial and hardrock applications.Operating parameters including water requirements, fluidising pressures and feed rates are given, as well as an indication of their various effects on gold recovery.Results include a Colorado School of Mines Research Institute (CSMRI) report comparing the Knelson concentrator with various other gravity concentrators.

Acid and metalliferous drainage (AMD) is routinely derived from waste rock, tailings, pit wall rock, underground mine voids, heap leach pads, ore stockpiles, concentrate stockpiles and even slag piles. In many jurisdictions, there is an assumption that tailings materials represent the highest AMD risk. This is likely due to their highly sulfidic and ultra-fine grained nature, and the fact that they occur in large tonnage, partially unsaturated deposits. These characteristics are not unreasonably associated with reactive and therefore highly polluting mine waste materials. Interestingly, however, the high sulfide content and highly reactive nature of tailings achieves the rapid consumption of oxygen in tailings pore spaces. This oxygen depletion can limit or halt sulfide oxidation throughout much of the tailings pile. Oxygen can only be resupplied to the tailings via atmospheric exchange at the air-tailings interface. The grain size of most tailings materials limits this resupply to a diffusion related process that only affects the uppermost 100–2000 mm. Hence sulfide oxidation in tailings can be inherently self-limiting. Unfortunately, the same cannot be concluded for waste rock materials.Using a unique site assessment strategy that focuses on quantifying acidity fluxes from various domains at a mine site, it has become increasingly evident that sulfidic waste rock produces the majority (typically 60–80 per cent) of the soluble acid and metalliferous pollution at most active and decommissioned sulfide-bearing mine sites. Whether sulfidic waste rock is disposed in engineered above ground facilities, backfilled into pits or even underground voids, it remains the primary source of pollutants for decades to centuries.Conventional end-dumping waste rock practices result in angle-of-repose pathways for air and water, and these internal fabrics exacerbate sulfide oxidation (ie pollution generation) and discharge from waste rock materials. The primary aim of many cover systems is to retard the access of air and water to these conduits. Recent thinking is that more emphasis needs to be directed at strategic waste rock pile construction practices, and less reliance placed on thin soil cover systems to prevent AMD.Waste rock pile construction approaches that minimise porosity and permeability but raise the retained moisture content and enhance water residence times have the potential to lower sulfide oxidation rates and thereby improve water quality outcomes. Accurate measurements of the moisture content of iron ore have increasing importance in modern mining operations. Knowledge of the moisture content allows process control actions to be carried out to ensure that the moisture is within prescribed limits, whether this is ensuring sufficient moisture to be above the dust extinction limit or that the moisture level is below the transportable moisture limit for sea shipments. Additionally, controlling moisture within desired limits can allow for greater throughput of iron ore by limiting the amount of water transported. There are a range of options available for operators wishing to obtain moisture data, though there are few demonstrated and proven devices that are able to provide real-time analysis of the full bed-depth of material. Scantech’s through-belt moisture systems are able to accurately analyse the full bed-depth of conveyed iron ore in real time using its unique microwave transmission technology, providing valuable data for operators. Recent advances in these systems have overcome errors associated with the highly attenuating nature of iron ore, particularly for large conveyor belts and deep ore. Furthermore, analysis of different ore mineralogy and its effect on the accuracy of moisture analysis has been examined and quantified. This paper discusses some of these recent results, demonstrating achievable accuracies in spite of changing ore mineralogy. Anglo American Metallurgical Coal (AAMC) manages five open cut coal mines in Queensland and New South Wales, producing approximately 30 million tonnes per annum. The size and complexity of these mines require efforts by a large and diverse number of personnel to minimise adverse geotechnical events from occurring. To achieve this, the AAMC coal mines in Australia are developing a concept of managing the geotechnical risks associated with open cut coal mines. The overall system utilises a combination of internal training, a management system (operational management systems) and standards, numerical modelling, monitoring, inspections and audits. The development of the standards as well as training and mentoring of exploration personnel, geotechnical engineers, engineering geologists and mining engineers with an interest in the geotechnical aspects of mining, training of the production workforce, mentoring and training of the technical staff create the basis for achieving a pro-active rather than a reactive system. Audits are used to assure that personnel are maintaining the standards and to facilitate cross-mine leanings so that best practices are developed across all sites.

MOUNT STEWART is situated half a mile south of Leadville railway station, which is distant 240 miles by rail from Sydney via Mudgee. It forms a low ridge from which rock waste and alluvium slope gradually to the black-soil flats of the Coolaburragundy River. The Mt. Stewart mine comprises:(a) The Mt. Stewart workings in M.L. 44, parish Talbragar, Co. Bligh.(b) The Grosvenor workings in M.Ls. 44 and 45, parish Talbragar, Co. Bligh.(c) The Extended workings in M.L. 44, parish Talbragar, Co. Bligh.The workings contain 1375 ft.. of vertical shafts, 1105 ft. of underlay shafts, 800 ft. of rises and winzes, 8520 ft. of drives and crosscuts, and 18,000 ft. of stapes.The mine was first opened in 1888 as a silver-lead mine, and a waterjacketed furnace was erected during 18B2. The furnace was installed with two internally-fired tubular boilers of 35 h.p. and 25 h.p. respectively, and two Baker's rotary pressure blowers. The results of the smelting campaign indicate that, during 14 months, a total of 15,000 tons of ore were treated, yielding silver and lead.
