This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. A previous report of the Bureau of Mines dealt with the toxicity of dichlorodifluoromethane5, a new refrigerant that has a remarkably low toxicity. Continued research by the Prigidaire Corporation6 and toxicological experiments performed in the Laboratory of Applied Physiology of the University of Cincinnati7 revealed another organic fluoride, dichlorotetrafluoroethane, whose toxicity also appeared to be of a very low order and whose physical-and chemical properties make it suitable for refrigeration purposes. In addition to low toxicity, safety in its use is further enhanced by its physical properties, which permit the design of equipment that operates at comparatively low pressures and hence mitigates the dangers and problems of leakage. The investigation described in this report was performed by the Bureau of Mines at the request and with the cooperation of the Frigidaire Corporation.

Other Federal agencies have been coming to the Bureau of Mines for help with the purchase and utilization of fuel, largely coal, almost since the day the Bureau was created in 1910. The requests covered a wide range of fuel-economy problems and included boiler-water treatment after it became apparent that water-formed deposits on heating surfaces acted as insulators, decreasing heat transfer and impairing fuel efficiency. Dr. Ralph E. Hall, a Bureau of Mines physical chemist determined the reactions that cause adhering scale to form in boilers (7)2 and went on to introduce scientific principles into water treatment for scale prevention (8). Widespread publicity of these achievements increased the requests for help from Federal steam-generating plants. Since no other Federal agency (even those which were more directly concerned with testing water) wanted to provide boiler-feedwater service, the Bureau decided to continue to give requested help to Federal agencies, Later, the Congress endorsed such agency cooperation as an economy measure,

Asbestos fibers are highly fibrous silicate fibers that are distinguished by having a large aspect (length to diameter) ratio and are crystallized in an asbestiform habit that causes them to separate into very thin fibers or fibrils. These fibers are distinct from nonasbestiform cleavage fragments and may appear as thick, short fibers which break along cleavage planes without the high strength and flexibility of asbestiform fibers. Since cleavage fragments of respirable dimensions have generally proven nonpathogenic in animal studies, little data exists on assessing well-characterized preparations of cleavage fragments in in vitro models. The objective of this work is to quantify the effect of the rock mass surrounding a stope or a tunnel on the seismic signal so that the rockburst damage on the excavation can be derived. It has been shown that the transfer function of rock mass surrounding an excavation can be modelled using a damped oscillator. The method of estimating the transfer function is extended in this paper. The parameters of a damped oscillator are estimated using inversion techniques and the transfer function is modelled as a time-varying system. The technique is tested on data acquired at three different sites in two deep gold mines. Employment of a modal model of the transfer function was suitable in two cases. Application of this technique is essential to track the time variation of the natural frequency during ground motion caused by a seismic event.

A 70% excess of mesothelioma, an asbestos-related cancer, has been reported among men in northeastern Minnesota, where iron mining has been the major industry. The Minnesota Department of Health has studied iron miners who developed mesothelioma to identify possible sources of asbestos exposure. A database of all Minnesota residents diagnosed with mesothelioma between 1988 and 1996 was linked to a database of approximately 72,000 current and former Minnesota iron-mining employees to identify cases who had ever worked in the mining industry. The job histories of the cases were examined to determine if any of their jobs could have involved exposure to commercial asbestos. Seventeen individuals diagnosed with mesothelioma in Minnesota between 1988 and 1996 were found to have worked in the iron mining industry. Of the 15 for whom adequate work histories were available, 14 had identifiable sources of exposure to commercial asbestos in jobs held both inside and outside of the mining industry. The time between employment in these asbestosexposed occupations and the diagnosis of mesothelioma is consistent with the 20 or more year latency period that has been observed in other studies of this cancer. © 2007 Elsevier Inc. Nowadays, most grinding mills driven by a wrap-around ring motor are operating successfully. Some of the earlier problems could be attributed to the lateral stiffness and stability of their bearings. As a result, modifications to the bearings included the rearrangement of the shoes, or the addition of more shoes, closer to the horizontal centerline. This technical paper reports a study of the effects of bearings on the lateral stiffness of grinding mills driven by a wrap-around ring motor. The discussions cover the design of the four shoe trunnion bearings and examine the effects of the angular location of the shoes.

South Africa (SA), a country in which all three commercially important asbestos minerals have been mined and milled, has retained proven cases of mesothelioma linked with environmental exposure to asbestos. This study illustrates the importance of fiber type in the occurrence of environmental mesothelioma. Four studies have reviewed the source of occupational or environmental asbestos exposure in 504 histologically proven cases of mesothelioma in South Africa. One hundred and eighteen cases (23%) were thought to be related to environmental exposure to asbestos. In the vast majority of these cases, exposure was linked to crocidolite mining activities in the Northern Cape Province. Two cases were thought to have occurred in relation to amosite and Transvaal crocidolite exposure in the Limpopo Province. In the balance of cases there was some uncertainty. No cases were reported with exposure to South African chrysotile. Consequently, in the vast majority of cases of mesothelioma, environmental exposure to asbestos occurred in the Northern Cape Province, in proximity to mines, mills and dumps where crocidolite was processed. Crocidolite appears to be far more mesotheliomagenic than amosite, and chrysotile has not been implicated in the disease. The Limpopo Metamorphic Belt is a complex geological terrane with at least three major deformational events. Tectonically juxtaposed lithology, open to isoclinal folding, crosscutting and re-activated shear zones and metamorphic gneisses and schists make pit slope design and maintenance a risky business. Venetia diamond mine?s Cut 3 is planned to reach 360 m by 2011. The difficult rockmass created problems in the original pit and a new more comprehensive slope design was necessary. Nearly 9 km of geotechnical drilling were undertaken to produce a new geological model and provide geotechnical information near the final Cut 3 face. All existing data from the pit were brought together into a database. SRK Consulting were brought into the project to audit the process and provide most of the manpower for the data analysis. Existing failures were back-analysed and the geotechnical domains identified. Analyses of only the southern domains are discussed in this paper. Geotechnical engineers can design slopes at any angle, but it is up to the managers at the mine to decide what risk they are willing to accept in a slope design. Once the issue of risk was discussed and settled in respect of the consequences of failure, each of the main geotechnical domains were analysed to produce a slope design at5% probability of failure with 15% above the first ramp. A risk assessment of the design was then undertaken to determine what risk the design placed on the equipment, personnel and the profitability of the overall mining operation. The risk assessment indicated minimal risk to the operation. One of the main assumptions of the final design is that the pit slopes will be dewatered. A project was immediately put in place to determine a groundwater model for the mine and to thereby determine the best dewatering procedure. However, it should be obvious that it is practically impossible to prevent benches from getting briefly saturated with water during storm conditions. It was therefore necessary to look specifically at supporting the crests of the ramps in the problematic southern domains to guard against the possibility of crest failure during heavy rains. The data available were re-analysed toward this objective. Spiles were recommended as the most cost-effective support system, and a probabilistic approach was used in the design. The final design has a 5% probability of failure, and the analyses indicate a strong sensitivity of the required spile spacing to the strength of the reinforcement material that will eventually be used. The pit slope parameters derived as per the discussion in this paper have just begun to be implemented at Venetia Mine.

1. Paper1?Identification and enumeration of asbestos fibers in the mining environment: mission and modification to the federal asbestos standard Arthur M. Langer Asbestos-related disease is increased with cumulative exposure (the concentration of asbestos in the air and duration of exposure). The maximum duration is assumed to be a working lifetime so federal agencies control cumulative exposure by limiting the airborne concentration of asbestos fibers equal to or greater than 5 lm in length. Air samples are collected in the environment of interest by entraining the airborne particulates including fibers on membrane filters and then counting the number of fibers using phase-contrast light microscopy. In addition to length, the fibers must have a length to width ratio of at least 3 to 1. This ratio is also referred to as the aspect ratio. Some counting strategies placed an upper limit on the fiber width restricting the counting to respirable fibers. Airborne asbestos is controlled by monitoring a sub-population of the airborne asbestos. No asbestos fibers shorter than 5 lm or fibers of any length having widths less than the resolution of the light optical method are counted. In the non-occupational environment, the Environmental Protection Agency requires the use of analytical transmission electron microscopy and fibers and counting fibers with lengths of 0.5 lm or greater and aspect ratios of 5 to 1 or greater are counted. The permissible exposure limit for asbestos has been reduced by more than 120-fold since 1969, when the standard was 12 fiber/ml and the current monitoring methodology was developed. The manufacturing and mining worksites are regulated by two different federal agencies and the asbestos exposure limit was higher in the mining environment until recently (Mine Safety and Health Administration 30 CFR Part 56, 57, and 7, Federal Registry, February 29, 2008). Currently the permissible exposure limit (PEL) is the same in both types of workplace and it is 0.1 fibers per milliliter (fiber/ml) over an 8-h shift as a time-weighted average (TWA). A milliliter (ml) is equivalent to a cubic centimeter (cc) and sometimes this standard is reported as 0.1 f/cc. As the phase-contrast methodology was used to monitor lower exposures and environments, other fibrous mineral particulates were incorrectly characterized as asbestos. These occurred in talc deposits and taconite ores from the Eastern Mesabi Range.

"As mine head grades decline and orebodies become more complex, traditional mineral processing techniques and flowsheets to achieve saleable concentrate become more difficult to design and construct. Mines with lower quality concentrates or concentrates with penalty elements are particular under threat. The economics of these operations are far more susceptible to metal price, concentrate treatment terms and the availability of other, cleaner concentrates. Additional value may be realised for these orebodies through improved recovery by producing a low grade middling concentrate for further processing, in conjunction with a saleable concentrate.The most cost effective way to reduce impurity levels is to do so as early as possible in the mining value chain. Technologies such as fine grinding and fine particle flotation are well established as effective methods for impurity rejection in mineral processing. What is normally overlooked is how a hydrometallurgical process could also be integrated in the overall flowsheet to achieve higher overall recovery at the mill. In the base metals environment, this is mainly because hydrometallurgical processes are associated with production of metal or use of expensive and toxic precipitating agents once the minerals of interest are solubilised. These processes can be very expensive, particularly with rising power costs and poor economies of scale in capital costs associated with low production rates from middling streams.Glencore Technology (GT) has recent experience in the treatment of middling and low grade concentrate streams as well as tailings streams to compliment a concentrator flowsheet in a refractory gold and base metals setting. The value proposition is the isolation of a low grade middlings concentrate from the primary circuit or the tailings stream for upgrading to an intermediate product with an equal or higher grade than the primary concentrate to allow blending for sale. This allows plants to operate on a more favourable part grade-recovery curve while avoiding the expense of metal production. For existing operations this is particularly attractive since it can be added on with no process interruptions.Two case studies are examined showing flowsheets and costings to arrive at the value proposition of the GT low grade treatment flowsheet."

The Bureau of Mines has developed a photoelectric concentrator apparatus which is capable of collecting incremental products from the discharge of wet concentrating tables. The instrument uses photoelectric cells as sensors to differentiate between two products which have differences in color or reflectivity. The photocells provide a signal that controls a mobile splitting device that separates the products. Tests conducted on a wet table demonstrated the ability of the apparatus to separate a product containing 81.5 percent pyrite from a lower-grade product containing only 68.8 percent pyrite. The photoelectric concentrator should have a broad application for collecting concentrates with visible color or reflectivity differences from wet concentrating tables.

This paper summarizes the results from a project aimed at adapting scroll centrifuges for application to the dewatering of flotation concentrates. The main impetus for the project was to reduce the high cost and footprint size of the technology currently used to dewater fine coal. The technical challenge was to enhance solids capture by incorporating a laser-cut screen into the design of the basket. The project has shown that a pilot-scale scroll centrifuge can be used to dewater froth concentrate. A number of different basket designs were tested, the best results being achieved with a basket comprising a laser-cut screen of aperture 70 µm. The best result on a sample of untreated flotation concentrate, which was taken from the open trough flotation cells at a coal preparation plant (CPP) in NSW was a solids recovery of 82 wt% at a product moisture of 24.3wt%. By comparison the best product achieved with a conventional wedge wire basket was a similar moisture content but solids recovery was only 64 wt%. This could only be achieved after the feed had been artificially thickened. Conditioning of the centrifuge feed with polymer flocculants led to an increase in recovery up to 88 wt% with a moisture content of ca 25 wt%. It is thought that optimizing additive selection and conditioning parameters, as well as screen aperture and open area, could improve performance still further. The results were obtained using centrifuging conditions (centrifugal force, feed solids, feed rate) in the pilot-scale machine which should be possible to translate into conventional designs of commercial scale machines, without downgrading solids throughputs. However only with full-scale trials will it be possible to establish this assertion with certainty.

Paper 1?South African experience with asbestos-related environmental mesothelioma: Is fiber type important? Neil White, Gill Nelson, Jill Murray The purpose of this review was to illustrate the importance of the type of asbestos in environmental mesothelioma. South Africa is a logical place to assess fiber type. The three major types of asbestos (crocidolite, amosite and chrysotile) were mined (all asbestos mining stopped in 2002) in South Africa. The association between asbestos and mesothelioma was discovered in South Africa. The occupational literature indicates high mesotheliomagenic potential for crocidolite, less for amosite and less still for chrysotile. Since South Africa has been a major source of crocidolite, relatively high rates of mesothelioma in the mining population is not surprising. High rates of mesothelioma due to environmental exposure is surprising, except perhaps when the authors? point out potential sources for community exposures (e.g., mine tailings used for road surfacing, golf courses, brick and plaster making; insulation in residential ceilings; children playing on fiber-rich waste rock). The authors? reviewed four studies reporting on mesothelioma cases and their sources of exposure and fiber type. Overall 23% of the known cases were attributed to the environmental (not occupational) exposure and about 90% were due to crocidolite asbestos. The proportion of environmental mesothelioma is considerably higher than in Western Australia where crocidolite is also mined. There the overall rate of environmental mesothelioma is about 6% (and less than 1% from residence in a crocidolite mining area).

DURING the happy and peaceful years between the Treaty of Versailles and the third New Deal, metallurgy became one of the most cosmopolitan of the sciences. Any metallurgist can name some twenty or thirty scientists whose work, in this country, in England, in Germany, or in France, Sweden, Italy, or Japan was as well known in other parts of the metal-producing and metal using world as it was locally, despite the fact that many of these men never left their native land. One of these metallurgical cosmopolites is Augustus Braun Kinzel, former Chairman of the Iron and Steel Division, A.I.M.E., and now a Director of the Institute. Gus Kinzel, however, is not only a cosmopolite by reputation; he is also a cosmopolite in fact. For many years he commuted at least twice a year between New York and the rest of the world; he is, therefore, not only an expert on the art and science of metallurgy but is equally expert in world travel and can expound with authority on the comparative quality and especially on the cuisine of the great prewar steamship lines; and being a true cosmopolite, he knows, as well as he knows the iron-chromium diagram, the best vintage years in the Gironde, Cote d'Or, and Saone-et-Loire departments in France and along the banks of the Moselle and Rhine in Germany.

"Traditional comminution tests are a vital component in every mine plant design. Comminution behavior governs both the cost of equipment and its operating efficiency. Most comminution tests are conducted off site on a limited number of samples, owing to the high costs and long lead times involved. Therefore a strong incentive exists to find alternative means for efficiently characterizing comminution behavior for a truly representative suite of samples. The potential for inferring comminution behavior from drill core is investigated in this paper. Our approach involves development of small scale comminution tests on the one hand, and measurement of a wide range of core attributes on the other. Our focus is the relationship between petrophysical properties and comminution behavior.Petrophysical properties of archival diamond drill core from the Ernest Henry Mine (Queensland, Australia) have been measured using a Geotek multi-sensor petrophysical core logger. We have investigated the relationship between the petrophysical properties and the corresponding small scale A*b and estimated Bond mill work index test results for 2m composites of the core. Core samples were petrophysically classified by means of cluster analysis. Comminution behaviour was then modeled for each cluster via multiple regression. The results suggest that prediction of the comminution behavior on the basis of petrophysics is viable at Ernest Henry.INTRODUCTIONMineral processing is the most important downstream operation for mining engineers and metallurgists since most mined materials are subjected to some type of comminution or beneficiation operation (SME, 1992). Comminution or size reduction is typically the first stage of processing. Size reduction steps (crushing and grinding) in any plant consume large amounts of energy, normally more than 50% of total energy (Weiss, 1985). Therefore, it is desirable to obtain laboratory results to design the size reduction equipment appropriately. One of the main factors that control the selection of hard-rock crushing equipment and the design of a crushing circuit is the mechanical character of the ore(s). Work index, density, abrasivity, compressive strength and clay content are essential parameters for ore characterisation (SME, 1992)."

This paper briefly describes the principal known commercial and near commercial deposits of gold-bearing gravel end the principal placer mining operations of Idaho County, Idaho. It is one of a series of similar papers on western mining districts. The data used in writing this paper were gathered during the summer of 1937. Information concerning gold-bearing gravels was obtained from prospectors and operators, while operators supplied most of the data on mining operations. No attempt was made actually to test the gravels. The principal object of the examination was to indicate placer areas worthy of further investigation by prospective operators.
