The actions, decisions, thoughts, or perceptions of one or more humans contributed to the accident. These are just a few examples of safety problems researched by the Human Factors Group of the Pittsburgh Research Center, U.S. Department of Energy.

There is outlined below the essential facts brought out in an investigation into the causes for emulsion difficulties as experienced at many water gas plants . The investigation was conducted under a cooperative agreement between the University of Illinois Engineering Experiment Station , the Illinois State Geological Survey , and the U. S. Bureau of Mines . Funds of the Illinois Gas Association were used to defray a part of the costs . Active assistance was rendered by the Peoples Gas Light and Coke Company , of Chicago , who furnished a laboratory at Chicago for conducting the experiments , and loaned the services of Mr. E. A. Thiele to help with the laboratory work . A full report on the results of this work will be published by the Bureau of Mines later but it is believed desirable to bring the principal results now to the attention of the industry . In almost every gas plant where carbureted water gas is made , some difficulty is experienced , at one time or another , in separating the tar fran the water that condenses from the gas . The fact that annoyance from this source is ever present in some plants , and seldom present in others , leads to the belief that certain operating variables have considerable to do with the formation of difficulty separable tarwater emulsions . Accordingly , when samples of tar were collected for laboratory study , the operating conditions and methods were simultaneously observed and recorded.

Lower the respirable dust exposure of longwall shearer operators by use of a modified head to tail cutting sequence. Approach Modify the cutting sequence of double-drum shearer operations cutting unidirectionally head to tail, so that the tailgate drum is used to mine the entire coal face except when cutting out at the head. This approach would place the primary dust generating source, the cutting action of the drum, on the return air side of both shearer operators. How It Works Currently, 75% of the double-drum shearers in operation employ a unidirectional cutting sequence. On half of these operations, the primary cut is taken as the shearer travels from the headgate to tailgate ends of the face, with roof support to be advanced on the intake air side of the shearer. Typically, the leading (tailgate) drum is taking a full cut during the head to tail pass and the trailing (headgate) drum is cutting bottom coal. In this mode of operation, the dust generated by the trailing (headgate) drum can spread out into the walkway, and increase the respirable dust exposure of both head and taildrum operators. As an alternative, if the trailing drum is "free wheeling" or cutting minimal coal during the head to tail cut, and bottom coal taken by the tailgate drum during the tail to head clean-up pass, both operators are able to work on the intake air side of the primary dust generating source (drum) except when cutting out at the head. This modified cutting sequence would save wear on the conveyor and help to eliminate overloads, as well as reduce the respirable dust exposure of both shearer operators. This modifcation is only applicable to long-wail operations currently cutting unidirectional head to tail, and to those operations where this unidirectional cut would not produce operating constraints.

It represents our capacity for vision, for collecting sensory information about our environment and the events unfolding within it.? (source: The Feng Shui Handbook, feng shui Master Lam Kam Chuen.) There are Chinese, Japanese, Russian, Egyptian, and Native American counterparts of the phoenix. All of these birds are identified with the sun and depicted as rising from the ashes ? living again. This is the essence of a community after a natural disaster ? they live again. The Phoenix of Natural Disasters: Community Resilience is edited by Kathryn Gow and Douglas Paton who have done an excellent job of creating a relatively seamless transition between chapters with different authors. The content is organized well with the first chapter providing an excellent overview of what is to come and each chapter including an abstract clarifying the focus of the chapter.

Sections 120 and 121 of Article XI. Fire Control, of a proposed Mine Safety Law in preparation, to be used as desired and when approved by any State, contains these provisions: After the adoption of these regulations, no mine operator shall erect any nonfireproof structure within 50 feet of any mine entrance. or airway. Every operating mine having nonfireproof structures within 50 feet of any mine entrance or airway, except headframes necessary for hoisting, shall have such structures fireproofed or removed and replaced with fireproof structures within six months of the adoption of these regulations. Before reopening, mines idle at the time of the adoption of these regulations shall have all non- fireproof structures fireproofed, removed, or replaced with fire- proof structures as required by the foregoing sentence. Upon receipt of the written statement of a mine operator that the fireproofing of existing structures is impracticable, the chief mining engineer (of the State commission) shall satisfy himself as to the abouracy of the statement. If he concurs that fireproofing is impracticable, and if at the same time he holds the opinion that adequate protection can be provided otherwise by such procedure, he shall have authority to issue a written permit for the substitution of fireproof door at or near portals of mine openings and collars: of shafts. Such doors shall fit as nearly gas-tight as possible. and shall be installed in such a way that the mouths of mine open- ings can be closed readily from outside of structures. The effec- tiveness of such permit may be made contingent upon the operator's continued compliance with any additional precautions which are necessary in the chief mining engineer's opinion to protect the lives of miners.

"The presence of respirable quartz-bearing mineral par-ticles has been well documented in many coal mines, and the Mine Safety and Health Administration regulations address the quartz content of mine dusts. Unfortunately, the correlation between the quantity of respirable quartz-bearing mineral particles and the pathogenicity of the dust is not strong. Work is currently underway by the National Institute for Occupational Safety and Health/U.S. Bureau of Mines and work is being included in the Respirable Dust Center Program by Dr. Seehra to determine the surface availability of quartz in mixed composition mine dusts. However, this paper propose a possible source of surface quartz or a spe¬cial region in the coal that may contribute to the pathogenicity of selected particles in the respirable dust.Assumed in this model is the hypothesis that either the cleat surface is coated with quartz or some cytotoxic sub-stance that in the body is capable of destroying macrophage and thus inducing a fibrotic response (1) that during the frag¬mentation a reasonable proportion of the respirable size coal particles contain some of the original cleat surface and that these particles have the same order of in vivo cytotoxicity as do the quartz particles. It may be that these cleat surfaced respirable size particles are only contributory to the overall pathogenicity of the coal dust, nevertheless, there are enough of these particles to to considered as a potential source of the cytotoxic dust particles.This paper demonstrates that if the cleat surfaces are mineralized with quartz or have a pathogenic coating, then in many mines there will be enough respirable coal dust particles containing some cleat surface to cause a fibrotic reaction. To understand why some of the dust particles contain as part of their surface some of the cleat surface, consider what happens when a beer bottle is broken. While there are a large number of beer bottle fragments, many of the fragments, particularly the larger fragment sizes, will have as a fraction of their surface part of the original beer bottle surface. The model and mathematics of this problem have been worked out by Meloy.(2-4)In the homogeneous IAC model, (2-5) a fragment or par-ticle resulting from comminution is equally likely to occur anywhere in the material being broken. Therefore the probability of a particle of size x having part of the cleat surface in it, p(x), is proportional to x. Thus:"

"The regrettable death of James S. Cunningham, Foreman Miner, attached to Bureau of Mines Rescue Car No. 2, while wearing a half-hour type oxygen breathing apparatus in a gasoline storage tank at Trinidad, Colorado, raised the question as to the possibility of gasoline vapors permeating the rubberized fabric of the breathing bags of such apparatus.It is known that most gases and especially certain organic vapors permeate rubber, by solution or a similar process. The rubber becomes saturated with the gas and if a concentration of gas is maintained on one side of the rubber sheet it slowly gives off gas from the other side. It of course takes some time for the gas to diffuse through the rubber and the length of time before it reaches the other side is proportional to the thickness of the rubber. The amount of gas that passes through per unit of area in unit time increases with the concentration of gas and also varies with the structure and composition of the rubber.It has been suggested that the gasoline vapor could not have penetrated into the interior of an oxygen breathing bag because the pressure is greater on the inside, but this is not true as the process of permeation by solution in the rub¬ber is affected only by the difference in the partial pressures of gasoline vapor on the inside and outside of the bag. The positive oxygen pressure on the inside can only prevent the entrance of gasoline vapor through measurable holes, and has no effect on permeation by diffusion through the rubber.A sample of gasoline was obtained from the tank in which Cunningham met his death. Examination by Dr. E. W. Dean of the Petroleum Division of the Bureau of Mines gave the following results.This shows the gasoline to be heavy and typical of the gasolines sold throughout Colorado for motor fuel A. Salvus half hour oxygen apparatus was arranged with the breathing bag only in a gas tight box in which the space was kept saturated with the vapor of this gasoline. The main part of the apparatus was outside the box. Oxygen from the apparatus was breathed by a man in the usual manner. Gas samples were taken from the box at intervals to determine the concentration of gasoline vapor, also samples of the oxygen breathed were taken from a side tube in the mouthpiece. The room temperature was 75°F. The concentration of gasoline vapor in the box at the beginning of the test was 4.3%. The man breathing from the apparatus thought he detected gasoline in his oxygen in two minutes after starting the test. Analysis of the inspired oxygen at this time showed 0.03% gasoline vapor. The same man remained on the apparatus for 18 1/2 minutes without suffering any ill effects. His place vas then taken by another man who wore the apparatus until the oxygen was exhausted without noticing more than a trace of gasoline at any time. At the end of the test the inspired oxygen contained 0.07% gasoline vapor. The total time of test was 38 minutes."

Operating engineers and mining executives are interested in the science of applied geophysics, but are looking for a simple and understandable exposition of its problems, instead of dissertations which are becoming more and more involved and increasingly difficult to comprehend. It may be well to discuss the goal, the attempted methods of reaching it and the psychological motive, if any, tack of the science. Where does the subject of applied geophysics stand from the viewpoint of (1) the physicist, (2) the mathematician, and (3) the geologist? Fundamentally, the science aims at a differentiation of subterranean geology with the object of finding useful mineral products by the study of gravitational, magnetic, seismic, electric, and other fields which may be associated with the materials naturally or which may be applied to them. The more skillfully these fields are recognized, applied, and analyzed the greater will be the possibility of interpreting results in a way that will be remunerative both economically and scientifically.

The first steps in the process of mine evacuation are the recognition of a problem and an attempt to communicate the problem to miners who may be affected. This chapter will focus on the way that a problem, fire, came to the attention of mine personnel and the messages that were sent to miners in the affected areas. The concept of information uncertainty, which was introduced in chapter 4, will be discussed as it influences problem perception and diagnosis. Sociotechnical and interpersonal communications will be explored and suggestions for improving these systems will be offered. It might seem that the first indication of a dangerous fire would motivate individuals to take self-protective action, to evacuate the affected area or structure, and to provide clear warning to others who are in danger. Research has actually shown that in most situations this does not occur. Instead, time is taken to gather more information, confirm information that is provided, and consider possible alternative explanations that could account for the given circumstances [Canter 1990; Scanlon 1979; Bickman et al. 1977]. This process of confirmation can lead to the loss of critical time. Canter [1990] summarizes the problem in his book, which reports studies of a number of fire events: "As discussed throughout this book, ambiguity and confusion, incoherent instructions and time-wasting actions, lack of appropriate instructions and misunderstanding of the nature of the event that is unfolding, are all hallmarks of fires and emergencies that kill people." In this chapter, the detection of each mine fire and the communication of warning to endangered miners will be reviewed with an emphasis on how those processes were affected by the availability and use of information. Information can become available through a variety of mechanisms during an emergency situation. First, cues may be taken directly from the environment. Smoke is an obvious example. Secondly, mechanical devices, such as smoke detectors, may provide warning messages. A third source of information is interpersonal communications. These can occur face-to-face or through some mechanical device such as a telephone. With all three methods there is a possibility of miscommunication, misunderstanding, and misinterpretation. All of these means of communication were used with varying degrees of success in the three mine fires analyzed here. A fire, like any nonroutine situation, engenders uncertainty about a diagnosis and understanding of the problem [Mead 1938]. This uncertainty leads to delays in realization of the seriousness of the situation and therefore in the proper response to it. Delay in action is an important concern in any fire setting, but is even more at issue in underground coal mine fires. Mine fires are qualitatively different from structural blazes: workers' escapeways may be miles long; the seam height at many operations is so low that it is impossible to walk upright; access to underground workings is always limited to a few (sometimes only two)

"INTRODUCTIONOrganics extractable from respirable diesel exhaust soot particles by organic solvents have been known for some time to be direct acting frameshift mutagens in the Ames Salmonella typhimurium histidine reversion assay (Huisingh, et al). Upon deposition in a pulmonary alveolus or respiratory bronchiole, respirable diesel soot particles will contact first the hypophase which is coated by and laden with surfactants. To model interactions of soot and pulmonary surfactant, we dispersed soots in vitro in the primary phospholipid pulmonary surfactant dipalmitoyl glycerophosphorylcholine (lecithin) (DPL) in physiological saline. We have shown that diesel soots dispersed in lecithin surfactant can express mutagenic activity, in the Ames assay system using S. typhimurium TA98, comparable to that expressed by equal amounts of soot extracted by dichloromethane/ dimethylsulfoxide (DCM/DMSO)(Wallace, et al, 1987). Here we report additional data on the same system using additional exhaust soots and also using two other phospholipids, dipalmitoyl glycerophosphoryl ethanolamine (DPPE), and dipalmitoyl phosphatidic acid (DPPA), with different ionic character hydrophilic moieties. A preliminary study of the surfactant dispersed soot in an eucaryotic cell test system also is reported.MATERIALS AND METHODSSoots D4 and D6 were collected at two different times as scrapings from the exhaust pipe of a diesel tractor-trailer immediately after the engine was stopped. Surfactant dispersions were made by ultrasonic probe sonicating 25 mg of commercially obtained DPL, DPPE, or DPPA into 1O ml of O.85% physiological sterile saline (PSS) for 10 min at 40 watts power. For surfactant dispersions, 2.5 mg soot was mixed into 2.5 ml of the appropriate phospholipid dispersion. Mixtures were sonicated by 40 watts sonicator probe for one min, then incubated in the sonicator water bath for 30 min at room temperature; the mixture was then incubated for one hour at 37C while being rotated continuously. Dilution series were made with saline. For solvent extraction, 2.5 mg soot was mixed into 2.5 ml of DCM and incubated in a sonicator water bath for 30 min at room temperature. The sample was then mixed with 2.7 ml DMSO, placed in a dry bath at 45C, and the DCM evaporated under a stream of nitrogen gas until 2.5 ml of liquid remained. Ames salmonella/microsome assay utilizing TA98 for frameshift mutation was used for mutagenicity testing, with and without S9 activation, as described by Ames et al (1975). Unfiltered sample extractions or dispersions were preincubated with tester cells for 90 min at 37C : 0.1 ml of sample was mixed with 0.5 ml of PSS, or of S9 mixture, and 0.1 ml of an overnight TA98 culture incubated samples were then added to 2 ml of top agar supplemented with biotin and a trace amount of histidine, and overlayed onto Vogel-Bonner minimal media plates. After 48 hours incubation at 37C, revertant colonies were counted. Solvent controls were DMSO, DPL, DPPE, DPPA, and saline. Positive controls were trinitrofluorenone (1 ug/plate), for samples not treated with S9, and 2-aminoanthracene for S9 treated samples (2.5 ug/plate). All samples were assayed in duplicate. Sister chromatid exchange (SCE) assay was performed, following Lett, et al (1981), using V79 cells, of Chinese hamster pulmonary fibroblast origin. Stock diesel sample suspensions were made as 10 mg soot/ml DPL suspension, or 20 mg soot/ml in DMSO. Aliquots of these soot preparations were added to 15 ml of culture medium per sample flask to result in concentrations of 25, 50, and 75 ug soot/ml medium. After 12 hours incubation, 200 ul of 25 uM 5-bromodeoxyuridine (BrdU) was added to each flask. After another 35 hours incubation, 200 ul of 10 ug/ml colcemid were added; and the cells were harvested three hours later. Cells were swollen by hypotonic saline, and fixed twice with methanol/acetic acid, and burst on microscope slides. Cells on the slide were stained with Hoechst 33258, exp"

Each year at least one and in some years several explosions are reported as having resulted from firing of mudcapped shots in coal mines; undoubtedly a number of small or local explosions occur that are not reported; in addition there are numerous accidents, some of them fatal, attributable to this reprehensible blasting practice. It is surprising that so few explosions and other accidents from this cause occur since the practice of firing mudcapped, Even in gassy "adobe," "dobe," "sand blast," or "bulldozed" shots is widespread. mines where general blasting is safeguarded by being done with permissible explo- sives, rocks or bowlders frequently are blasted with mudcapped shots of dynamite; even if permissible explosives are used, this type of shooting is dangerous. The usual method of preparing a mudcapped shot is to place the explosive, either in loose or cartridge form, with the detonator or "cap" on top of a piece of rock or coal and cover it with several handfuls of moist clay, loam, sand, or When the shot is road dust; in many cases there is no covering of any kind. fired a dense cloud of dust, together with an intensively hot fleme, are thrown into suspension, and if the dust is explosive or ignitable an explosion or igni- tion is likely to result practically simultaneously. The presence of gas greatly increases the explosibility of the dust, as well as the probability of an ignition. Every shot, of any known explosive, fired in a coal mine without having the charge properly confined in a drill hole and tamped with incombustible stemming to the collar of the hole, is capable of initiating an explosion that may cause the death of every man in the mine. Attention has recently been directed to this unsafe practice by an explo- sion resulting from a flagrant disregard of safe blasting practices in one of our The mine had been idle for several years most productive coal-mining regions. and was being prepared for reopening on a different system of mining. Six to 10 inches of bottom coal had been left in place during former mining operations, but in the new system it was felt desirable to remove the bottom coal left in certain parts of the mine. Accidents occur when least expected, from sources that we rarely anticipate and with outcomes that can vary greatly. Anyone who has ever been involved in an automobile accident knows, the transition from driving comfortably and safely down a highway to being involved in a potentially life-threatening situation can occur in a heartbeat. Accidents on the job are no different. The mining industry has a relatively small number of electrical injuries, compared to industries such as construction or agriculture, due to the small total employment in mining. Mining does, however, have one of the highest occupational electrocution rates of any U.S. industry, at 2.8 for every 100,000 workers per year. This is approximately four times the average for all industries. Data from the Mine Safety and Health Administration (MSHA) shows that 75 electrical fatalities occurred in the mining industry between 1990 and 1999. Of these, at least 17% involved high-reaching mobile equipment that contacted overhead power lines.

The mining law of the Belgian Congo does not cover the Province of Katanga nor the District of Kivu (in Eastern Province). Regulations of the Comité Spécial du K tanga are in force in Katanga and those of the Comité National du Kivu in Kivu. The three laws differ only in detail, and grants made by the Comité Spécial du Katanga or by the Comité National du Kivu must be approved by the Ministry of Colonies. The following résumé is designed to coordinate the three sets of regulations.

[The meaning of the term "bauxite' has always been somewhat vague and indefinite, and the term has been used in various applications to denote sometimes a rock, sometimes a mineral, but more frequently an ore. It seems logical to think of bauxite as an ore, however, because a very large part (probably at least 85 per cent) of the entire world's production of bauxite is at present being used as a source of aluminum and aluminum chemicals. In the United States there are two districts that, produce bauxite - the Arkansas district and the Alabama-Georgia-Tennessee-Mississippi district. The deposits in Mississippi have been discovered recently and are riot as yet productive but it seems reasonable to consider them as apart of the-district hat includes the deposits of Alabama, Georgia, and Tennessee. The, two districts are often referred to as the Arkansas field and the Appalachian field. Of these two the Arkansas field has been by far the largest producer, and at present over 90 per cent of the bauxite production of they United States comes from the Arkansas field. The bauxite industry in the Appalachian field had its beginning in 1889, and since that time the total production of the states of Alabama, Georgia, and Tennessee has been, nearly a million tons. The production of bauxite in this field has languished in the last five, or six, years, and In 1927 its production was only about 14,000 tons, according to Harder4. The recent decrease in production is due mainly to the fact that many .of the known deposits of commercial-grade ore have become exhausted. There are, however, considerable quantities of low-grade bauxite in the Appalachian field, but these have not been available because there has been no satisfactory way of concentrating low-grade bauxite. The quantity, of low-grade material in the Appalachian field is probably, fully as large as the tonnage of commercial ore which the field has already produced.]

"Information gathered during recent months by field men of the U. S. Bureau of Mines shows that in one State alone there have been 71 fatalities from electrical causes during the last 51/2 years, of which 55 were from contact with mine trolley wires operated at 250 volts or less, direct current. The 1928 law relating to the guarding of trolley wires in the-mines of this particular State reads: ""All trolley wires carrying a voltage of class 2 must be properly shielded except where the same are at least 61/2 feet above top of rail."" Class 2 voltage is defined as being ""between 300 volts and 600 volts direct current, or 240 volts and 480 volts alternating current."" From this it may be inferred that no guards are required, for 250-volt trolley wires, and the natural conclusion to be drawn is that the lack of guards or shields must be held responsible for these fatalities. To prevent fatalities from this source, it has been advocated that every foot of trolley wire in mines should be guarded. While some such drastic step may appear to be amply justified, a closer examination of the records shows that lack of guards is not entirely responsible but rather that carelessness and neglect in the observance of certain well-established simple rules concerning installation and location of trolley wires must also bear a share in the responsibility. In -other words, if trolley wire is installed carelessly and its maintenance neglected, failure may be expected of any guards that have been put up.There are a-number of factors affecting safety as related to trolley wires in mines, among which the following may be enumerated.1. Installation relative to track and passageways.2. Guarding.3. Maintenance.4. Discipline.Acknowledgment is made to D. Harrington and K. L. Marshall of the safety division of the bureau for assistance given in securing data on cost of installing guards for trolley wires.The statement that fatalities should not be charged in every case to lack of guards is supported by the following example: A man operating a locomotive was electrocuted when the trolley wheel jumped the 250-volt trolley wire and allowed it to sag against his shoulder. He was sitting in the regular place provided in the locomotive, but the wire was 11 inches inside of the rail and only 39 inches above it where the wire touched him, or 11 inches from the roof at a distance of 12 feet from the nearest hanger. The accident occurred between two hangers 44 feet apart, and the trolley wire was inside the rail over the entire distance, being 2 inches inside at one hanger and 10 inches inside at the other hanger. A bolt in the roof indicated that there had been a hanger at one time between the two on the 44-foot span.This example is a violation of good practice in the installation and maintenance of trolley wire. In the first place the wire should have been outside instead of inside the rail. In the second place there should have been at least one intermediate hanger so that the span would not be so great and the sag would be reduced. Furthermore, it is possible that the wire was not taut enough even when the intermediate hanger was in place.To provide a degree of safety for men in the mines certain coal-mining States, recognizing the need, have enacted laws to regulate the installation of trolley wires. "Generally, the hazard of cool-dust explosions in tipples and cleaning plants is not given the consideration that is given it underground. This lack of consideration is probably due to failure to appreciate the potential hazard. An explosion in a tipple or cleaning plant is basically little or no different from an explosion underground. If bituminous-coal dust is raised into a cloud in the air and comes in contact with arcs from motor brushes, switches, or open flame, the dust is very likely to ignite, and flame will very probably propagate or extend through it if the quantity of dust per unit of air space is sufficient. The open space in a tipple or cleaning plant may reduce the violence of the explosion, but it is not likely to prevent ignition or propagation of flame.The movement of coal through a tipple or cleaning plant naturally puts into suspension a large amount of dust, which finally settles on exposed surfaces. Any disturbance that will put this dust into suspension in a sense enough cloud can result in the dust being ignited by an arc or open flame if one should be present. An explosion of bituminous-coal dust in a tipple or cleaning plant generally can be prevented either by keeping it thoroughly clean or by removing all sources of ignition, or both.The tests at the U.S. Bureau of Mines Experimental mine have shown that the explosibility of coal dust is proportional (other things being equal) to the ratio of the volatile matter to the volatile matter plus the fixed carbon, indicated thus:The volatile ratio for the Pittsburgh coal bed is calculated as follows:Therefore, as the volatile matter decreases the liklihood of ignition decreases; however, coal-dust explosibility depends on other factors also, such as fineness, dryness, and cloud density; and under some conditions other factors also intervene.Proof of the explosibility of coal dust in the absence of explosive gas has been shown in numerous cases in mines and mine plants and also by a series of tests. In these tests a dense cloud of dust is blown onto an open flame or an electric arc, almost invariably with resultant exploding of the dust. The Safety Division of the United States Bureau of Mines has made a number of demonstrations of the explosibility of Pittsburgh coal-bed dust, using a small gallery or box in which a cloud of coal dust is blown onto a lighted match; larger-scale tests have been made with an electric arc as the igniting agent. Bituminous-coal dust is readily ignited with a medium arc, but with low-volatile coals a larger arc is required. For test purposes, a 100-ampere arc has been used. Ignition has occurred readily under these conditions with Pocahontas No. 5 coal dust with ratios of volatile to total combustible of 0.232 to 0.253, and with Pocahontas No. 3 coal dust with a combustible ratio of 0.242. Lower Kittanning coal dust, with a ratio of 0.187, did not give definite ignitions, and Pocahontas No. 4 dust did not explode, although part of the coal-dust cloud ignited. It is probable that even the low-volatile coal dust will be found to be explosive with a higher-amperage electric arc as the initiating agent."

"In accident prevention, a study of statistical data is necessary to determine primary causes of accidents to devise means to eliminate causes at the original sources. Explosions in coal mines of the United States during the 10 years ending December 31, 1931, ranked third in the causes of fatal accidents; they constituted 16.2 per cent of all fatal accidents in bituminous mines and 8.4 per cent of all fatal accidents in anthracite mines. A study of coal-mine explosions in Pennsylvania, with approximately 160,000 men employed in anthracite mines and about 145,000 in bituminous mines or about 205,000 out of -the approximately 525,000 men employed in all coal mines of the United States, should give information of value to all persons endeavoring to reduce or eliminate coal-mine explosions. This circular reviews explosions of gas or dust or of both in the coal mines of Pennsylvania to show the hazard of gas and dust, to point out the influence of certain factors in causing explosions, and to emphasize the efficacy of explosion-preventive measures.The data published annually by the State Department of Mines and of the United States Bureau of Mines were used in compiling the tables of explosions. Through the cooperation of W. H. Glasgow, secretary, Department of Mines of Pennsylvania, and his assistants, many doubtful cases were clarified, so that as far as recorded the history is comparatively complete. Figures used in compiling some of the tables were furnished by W. W. Adams, chief statistician, demographical division, United States Bureau of Mines.Anthracite in the Wyoming Basin of Pennsylvania was used locally by gunsmiths in 1755, by blacksmiths in 1769, and as domestic fuel in 1808.Mining and shipments developed slowly between 1806 and 1812, but more rapidly after that.Early mining along the outcrops gave essentially no trouble from gas, but by 1870, when State inspection was established, explosions were frequent; ventilation was crude and accumulations of gas were generally ""brushed"" or burned out.Table 1 lists 1,232 ignitions and explosions of gas in anthracite mines from 1870 to 1932, inclusive, every one of which was accompanied by loss of life. Some details are given of 110 representative cases in which five or more lives were lost or in which it appeared that some violence was present.During the period 1870 to 1932, inclusive, gas explosions caused 2,213 fatalities, or an average of about two deaths for each ignition or explosion. Records are not available as to the number injured in these and in other explosions where no deaths occurred, but from a study of explosion reports it appears probable that the number injured exceeds the number killed. In the 110 cases described, 599 were killed and 294 were injured. As shown in Table 2, fatalities from gas explosions averaged 8.3 percent of the total fatalities in Pennsylvania anthracite mines from 1870 to 1931, varying from 6.0 percent in 1931 to 13.1 percent in 1927."

GLOSSARY Acid- and toxic-forming materials. Rock or coal layers containing significant amounts of pyrite or other minerals which, if exposed by coal mining, will cause acid or toxic drainage to occur when acted upon by air and water. Add mine drainage. Any water with a pH less s than 6.0 draining from a coal mine. Water is often orange-colored because of the presence of oxidized iron. Adit. An underground mine entry at sites where coal seams are exposed at ground level. Apple core. A complete hilltop (360°) left exposed by not backfilling the highwall. Approximate original contour. The surface configuration achieved by backfilling and grading the mined area so that be reclaimed land closely resembles the general pre-mining surface configuration and blends into and complements the drainage pattern of the surrounding terrain. Area mining. A surface mining method that is carried on in level to gently rolling topography on relatively large tracts of land. Active area mine pits may be several miles long. Arid and semiarid regions. Lands, generally west t of We 100% meridian, where water use by native vegetation equals or exceeds that supplied by natural precipitation, and rainfall is insufficient to support agriculture. Annual precipitation is normally 20 inches or less. Auger mining. A mining technique often used by surface mine operators when the overburden becomes too for the coal to be mined economically using traditional surface mining methods. Large-diameter (usually 2'-4') horizontal holes are drilled as much as 300 feet into We vertical Me of the coal bed by an auger. Like a bit used for boring holes in wood, coal augers consist of a cutting head with a screw-like extension. As the auger turns, the head breaks up the coal and the screw carries it back into a conveyor that loads it directly into a truck. Backfilling. The operation of refilling an excavation using material removed during the mining process. Bench. The level or flat working area at the base of the highwall that is excavated between the exposed coal seam and the original surface of the ground. Bucket wheel excavator. A continuous-digging machine that uses a rotating vertical wheel with buckets for large-scale stripping and excavating. Cessation Order. Order to cease mining issued to a coal operator by the regulatory authority due to severity of violations, Contemporaneous reclamation. Restoration of mined land that occurs as soon as practical after coal removal. Contour mining. A mining method commonly used in eastern mountainous topography where coal is removed in a narrow strip around the hillside. The extent of the cut into the hillside is determined by the depth of overburden at the highwall compared with the thickness of the coal seam. Daylighting. A term used to describe a surface mining procedure that exposes abandoned underground coal mine workings to enable removal of the remaining coal. Daylighting is followed by land reclamation. Dragline. An excavating machine, usually used in large, flat areas, that drags a bucket (which holds up to 220 cubic yards of material) toward the machine by cables, loads it with spoil, and then hoists it wit a boom up to 350 feet long, allowing the machine to excavate wide benches by depositing the spoil hundreds of feet away from the highwall. Grading. The process of smoothing the disturbed areas of the mine site, after the coal is removed, to closely approximate the pre¬mining terrain. Haul road. Any road constructed, improved, maintained, or used by mine operators and located within the permit area. Head-of-hollow fill. See "valley rill." Highwall. The cliff-like excavated face of exposed overburden and coal in a surface mine. Hydroseeding. Planting of seed mixed in a water medium. Water, mulch, lime, fertilizer, and seed are blended and sprayed onto regarded soil.

ORE CONCENTRATION PROCESS-PATENT OIL FLOTATION PROCESS. The invention of the patent issued to Sullman, Picard & Ballot November 6, 1906 (United States Letters Patent No. 835120), is based upon the discovery of an wholly unexplained phenomenon arising from the agitation of ore pulp containing oil and air in certain proportions. This was a discovery in an art in which it was well known that the elements of oil, air, and agitation possessed certain characteristics and produced certain results. As early as 1860 a British patent first suggested the use of oil in water concentration of ores by pointing out the affinity which oils have for metallic sub- stances in preference to gangue. Subsequently many patentees employed the known affinity of oil for metal in ore concentration proc- esses by using oil in proportions varying from 2 per cent to 300 per cent of the ore and recovering the oil-coated metal particles by causing them to rise to the top or sink to the bottom of the pulp. In these processes the agitation was either gentle or thorough but never great, and though always employed to produce thorough oiling of the metal it was used in different degrees for the sole pur- pose of causing the metal particles to rise or sink. In the prior art the oil was used for its known selective affinity for metal, agitation was em- ployed to mix the oil with the metal, and air was introduced to supple- ment the buoyancy of oil in raising oil-coated metal particles to the surface. To this extent had the art of oil flotation advanced when the patentee entered it, having reached the commercial stage in only two processes, representing respectively metal-flotation and metal-sinking processes, and having reached the stage of success in none. In the line of experiments by the patentee when the proportion of oil to ore was reduced to 1.5 per cent a "float" appeared; at 1.04 per cent still more "float" appeared; at 0.32 per cent the "float" again vastly in- creased; at 0.1 per cent the "float" again vastly increased. It thus developed that in using oil at 0.1 per cent or even at 0.05 per cent of the ore and after violently agitating the pulp from 24 to 10 minutes, there arose to the surface when the pulp was brought to rest, a thick froth or foam of oil-coated air bubbles carrying oil-coated metal particles to the extent of about 90 per cent of the metal con- tent, the foam being sufficiently stable to permit removal and metal recoveries. This was a new result based upon phenomenon then unknown and still unexplained and constituted discovery and promised a change in the art of oil flotation from laboratory experi- ments and mill failures to commercial success. The patent referred to covering the points mentioned has been sustained by the District and the Circuit Courts of Appeals and by the Supreme Court. Miami Copper Co. v. Mineral Separation, 244 Federal, 752, pp. 754-757. ORE CONCENTRATION PROCESS-OIL FLOTATION-INFRINGEMENT. The Miami Copper Co. in its process of concentrating ores by the oil flotation process used three distinct processes that were either the processes included and covered by the Sullman, Picard & Ballot patent or their fair equivalent and which were clearly an infringe- ment of the patent, and the company can not escape liability for such infringement by taking an additional step, although such additional step if taken alone would not constitute an infringement of the Sullman, Picard & Ballot patent. Miami Copper Co. v. Mineral Separation, 244 Federal, 752, p. 768. ORE CONCENTRATION PROCESS-VALIDITY OF GREENWAY PATENT. The patent issued to H. Greenway, June 9, 1914 (Letters Patent No. 1999699), is valid. The process disclosed in this patent was a substantial departure from processes of the prior art. The heating of water is a matter of large moment and the use of acids is a matter of constant and considerable expense. By wholly dispensing with both by the use of a minute quantity of hydroxy compounds, the patentee has disclosed an original and novel plan which has broad- ened and made more simple the agitation process of air flotation. The novelty and inventive character of Greenway's discovery is patentable.
