The City of KalgoorlieBoulder is situated approximately 600 kilometres to the east of Perth, and as the major regional centre of the Eastern Goldfields, supporting a population of approximately 25 000. The formation of Kalgoorlie Consolidated Gold Mines in 1989 brought the existing operating entities adjacent to the city under common ownership and management. The Joint Venturers forming the management company are Homestake Gold of Australia and Gold Mines of Kalgoorlie (a Normandy/Poseidon company).This traditional underground mining centre has been worked since 1893, yielding 40.5 million ounces of gold ore with an average head grade of TEN grams per tonne. Historically the heart of the operation has been the famous Golden We located on the Eastern perimeter of the city, in the area known as Fimiston. The other significant operations that form KCGM are the Mt Percy Open Pit and the Mt Charlotte Underground Operation.

"The Jaguar project is relatively unique in that it was evaluated, financed, built and operated to completion within the last ten years, providing the opportunity to holistically review and reconcile expectations against the actual outcomes. It is a case study that allows wider conclusions to be drawn with respect to the risks and opportunities involved with project evaluation and execution.Jaguar is located in the Leonora Mining District of Western Australia and was commissioned in mid-2007 as an underground accessed, single high-grade volcanic-hosted massive sulfide (VHMS) deposit with an on-site concentrator, producing copper and zinc concentrates exported via the Port of Geraldton to South-East Asian markets.The feasibility study (FS) had several iterations (JML, 2005a); the last dated July 2005 was specifically targeted to secure project funding, and summarised the reserves-only portion of the resource and its planned exploitation. The commissioning of the mine was the transition for the owner Jabiru Metals NL from junior explorer to producer and the project was the dominant asset of the company.Jaguar operated through several significant deviations from the linear project plan set out in the FS, including the Global Financial Crisis (GFC), a corporate takeover and disruption to the mine plan from the onset of material geotechnical issues. Whilst the GFC or mining disruption were not specifically predicted in the FS or peer reviews, the generic effects of the events were assessed in the project risk assessment and they influenced subsequent management decisions. In parallel to the internal project-specific risks, the short planned life of Jaguar was a risk to the continuation of the corporate entity which was being addressed by significant investment into brownfields exploration to extend or replicate the project, and ultimately added value such that attracted a corporate takeover.Holistically, the project delivered the returns predicted in the FS, but in a much different guise than was expected. The Northern Beaches Storage Project (NBSP) is part of Sydney Water’s SewerFix™ Program to improve the wastewater system and protect public health and the environment. It was built by the SewerFix Wet Weather Alliance (the Alliance), a partnership between Sydney Water, RPS, MWH, Parsons Brinckerhoff and United Group Infrastructure.The project temporarily stores excess wastewater diverted from the Narrabeen Submain during wet weather. The project considered a below-ground storage facility, which involved constructing a deep tunnel from Brookvale to Fairlight, connecting into the Northside Storage Tunnel and a storage tank, which was subsequently adopted and constructed.The storage tank scheme included a pumping station and inlet and outlet pipes installed by microtunnelling. The storage tank site is in the Brookvale industrial area situated in an area of deep marine sediments overlying Hawkesbury Sandstone at about 30?m below ground. Microtunnelling for the inlet pipe was conducted from a 12?m deep jacking shaft. The jacking shaft and 14?m deep pumping station excavations were carried out from within secant pile earth retaining structures adjacent to existing buildings.Construction activities had the potential to cause settlement of the adjacent buildings. During excavation and microtunnelling the water ingress, adjacent groundwater level and settlement of the adjacent buildings were monitored. The monitoring data was assessed against an action plan, which modified construction activities as required to minimise the risk of damage to buildings from settlements caused by lowering of the groundwater table outside the excavations. The secant pile construction, excavation, microtunnelling and pumping station construction proceeded without incident or damage to adjacent buildings.This paper highlights that tunnel/underground options should be considered during options development, describes the original storage tunnel scheme and presents details of the successful microtunnelling and deep shaft construction adopted on the NBSP.CITATION:Taylor, N and Hewitt, P, 2014. The Pongkor Gold Mine employs 678 staff. The mine is owned by the Indonesian state company PT Antam Tbk and is located 100 kilometres south west of Jakarta, in Java, Indonesia. Mine and infrastructure development started in 1992 following intensive geological exploration over 15 years. First gold production was in June 1994. The mine comprises three main orebodies: Ciguha, which is nearly mined out, dips at 75¦ from surface to 150 m. Kubang Cicau, dipping at 80¦ from surface to 230 m, and averages 3.5 metres thick at a grade of 10 g/t Au and 128 g/t Ag; and Ciurug, dipping at 60¦ from surface to 300 m and averages eight metres thick at a grade of 14 g/t Au and 172 g/t Ag. Present global ore reserves are 4.6 million tons at a grade of 12.3 g/t Au and 133.6 g/t Ag. Exploration is continuing for new veins. Initial mining was by manual cut and hydraulic fill at Ciguha, followed by Kubang Cicau. In 1996, it was proposed to convert Ciurug to a mechanised cut and fill operation. Two single boom jumbos and three load-haul-dumps (LHDs) were purchased with the assistance of a gold loan from NM Rothschild and Sons. Implementation of this upgrade was delayed by both a national economic crisis and an invasion of the concession by illegal miners. It was not until May 1999 that raise bore vent rises and access drift infrastructure commenced. Major underground development started in 2000 and is continuing. Production cut and waste fill stoping started in July 2001. The change from manual to mechanised methods has required a total change in work practices and attitudes. However, the process has resulted in a production increase from 600 tpd to 1200 tpd. A three-month compliance test (a condition of the financing loan) was successfully completed, from July to September 2001, with an average production of 700 tpd at Ciurug and 500 tpd from Kubang Cicau.

Environmental and amenity factors have introduced a new dimension to the technical problems of the mining industry in finding, extracting and processing the mineral and energy products on which we all depend. Intervention by outside regulatory bodies is restricting the production of minerals and denying the industry access to large areas in which the orebodies of the future may occur. The solution lies to a large extent within the industry itself and appropriate management techniques are necessary to ensure that exploration and mining are seen as an acceptable as well as a necessary land use. Close liaison between operators and the responsible authority is necessary to achieve these objectives and to make the best use of our land resources.

The authors often see problems on mine sites that are due to insufficient feedback between technical discipline silos as the realities on the ground fail to inform the models and plans used to drive resource exploitation. Resource modelling, reserve definition, life-of-mine planning, short-term mine planning and scheduling, mine operations and beneficiation operations are all processes that inform each other, but in many instances feedback between these processes occurs in an ad hoc way and only after problems have come to light.Universal reconciliation uses a multidisciplinary approach across the entire mining value chain to identify both opportunity and loss of value and to maximise operational performance. This paper will look at the operational effectiveness of universal reconciliation and will outline the important elements that make it successful, including how to map out the detail underpinning modelling, planning and execution processes. We do this by documenting the physical processes, logical work practices, organisational roles and responsibilities, data origin and quality, working assumptions and information management practices at an operation. The results quickly show how misinformation and assumption causes suboptimal decisions to be taken, where inefficiencies exist and what the opportunity is to remedy these issues.A universal reconciliation study opens up an opportunity for stakeholders to reassess how the operation should be run and how operational effectiveness should be measured. Clear envelopes for reconciliation become obvious and questions about the practicality and data quality can be addressed.A reconciliation code of practice is a key element of a successful universal reconciliation project and the authors outline the important aspects that will provide an operating model, showing the effectiveness of the resource and reserve modelling, the practicality of the mine planning and scheduling practices, how well the mine operations team are executing these schedules and how effective the processing area is given what it is fed. Most importantly, a system that allows operations time to react before the losses become systemic and that is based on identifying early warnings from hard data when these processes start to wobble is outlined. St Helena tunnel is the centrepiece of the Pacific Highway upgrade from Tintenbar to Ewingsdale, abbreviated and commonly known as T2E. At the deepest point the twin tunnels run 45 m below the ridge line of St Helena Hill, located about 5 km west of Byron Bay. The tunnel breakthrough was achieved early in 2014 and the works for permanent lining will be completed in early 2015.Once the project will be handed over to the public in 2015, T2E will deliver a safer highway and uninterrupted traffic flow between New South Wales and Queensland. The design speed in the project area has been set to 110 km/h by the client Roads and Maritime Services (RMS), which will reduce the travel times between Brisbane and Ballina to just over two hours.The two tubes of the tunnel have been designed to accommodate three lanes in each direction with shoulders and walkways on either side of the carriageway. These requirements result in a huge cross-section of about 20 m width and an excavated area of about 210 m2.With the tunnel alignment being situated in basaltic rock, St Helena tunnel is conventionally driven. The selected excavation method was predominantly drill-and-blast with temporary support being installed immediately after the excavation. Despite the wide span of the tunnel, the geology allowed the tunnel perimeter to be supported by only rock bolts and a layer of shotcrete over the majority of its length.Due to environmental reasons one of the approval criteria of the project was that the tunnel must not have a detrimental long-term impact on the groundwater system within St Helena Hill. Consequently the tunnel has been designed as a fully tanked structure with a waterproofing sheet membrane installed on the temporary shotcrete.The permanent structure of the tunnel has to meet a 100-year design life and it is designed as a cast in situ concrete lining. Considering the water pressure on the lining the concrete is reinforced with a significant amount of bar reinforcement.The presented paper provides a more detailed explanation of the geology and the consequential temporary and permanent support measures together with the waterproofing system.CITATION:Mueller, A M, 2014. INTRODUCTIONDuring the past few years there has been considerable interest in developing the so-called direct reduction processes for treating iron oxides. The various methods proposed were recently reviewed by Scortecci and Palazzi (1960).In particular the application of fluidized bed techniques received considerable attention and, on first examination, this seems to be the ideal method 'of dealing with fine iron oxides. Several difficulties (e.g. product agglomeration) require the method to be investigated further before its potentialities can be fully assessed.PROPOSED FLUIDIZED BED REDUCTION PROCESSESBrassert and De Jahn (1951) were among the earliest to suggest the use of fluidized beds for the reduction of iron ore to metallic iron. Their patent was followed by many others proposing similar concepts, mainly from Lewis (1955), Ogorzaly (19.56), Shipley (1956), Agarwal (1960'), Osborn (1958), Texaco Development Corporation (1956) and Stelling (1957).The H-iron process (Squires and J ohnson, 1957; Lubker and Bruland, 1960) is the only scheme that has been an economic success. It is a common misconception that the grant of mining tenure, whether it be an Exploration Permit, Mineral Development Licence or Mining Lease, will entitle the holder to access all land within it in order to explore or mine. The ability for miners to access their tenure in order to undertake activities is critical to the industry. While significant time and energy is quite rightly dedicated to resolving native title and cultural heritage issues before the commencement of mining operations, the same cannot always be said for addressing potential impediments that arise as a result of the overlap between mining and other tenures. Miners should be aware of the overlaps that occur with other tenure and the impact of these overlaps on the minerÆs ability to explore or produce from the area. The existence of an overlap with petroleum tenure, nature refuges, national parks, wild river areas or reserves such as aboriginal land leases are all likely to have an impact on the minerÆs ability to access the area and conduct activities. In some instances, such as an overlap with petroleum tenure, Queensland legislation encourages the miner to reach agreement with the holders of the overlapping tenure. However, in other instances, such as an overlap with a national park, mining tenure is unlikely to be granted at all. This paper summarises the impacts and impediments that arise as a result of overlaps of tenure that are likely to affect mining operations. It also provides examples of how these impediments can, if possible, be overcome under Queensland legislation.

The provision of a water supply is a major consideration in planning mining developments particularly in remote areas. The evaluation and development of adequate water supplies in and areas of Australia is often as important as the discovery of the orebody. The copper orebody at Warrego near Tennant Creek could not be mined economically if cheap treatment water was not available near the mine. This paper describes the difficulties in obtaining water in the Tennant Creek area and the water supply investigations carried out near the Warrego Mine which resulted in the successful discovery and evaluation of a reliable large scale water supply. It is contended that the establishment of a water supply must be approached on a systematic and scientific basis similar to the approach with the exploration and establishment of actual ore reserves. Mining companies with similar water supply problems may benefit by approaching their problem on this basis. The approach adopted by Peko Mines is outlined in this report.

During the 1990s, wider and more productive longwall panels were being introduced into the Newcastle Coalfield. The strata of the Newcastle Coalfield incorporate numerous massive sandstones and conglomerates, some of which formed the immediate roof of the coal seams being mined. The unexpected implications of introducing wider panels into this geological environment included more variable and increased subsidence, severe periodic weighting (with associated mid-face falls) as well as windblast.Subsidence prediction at the time was based on empirically derived data essentially relating subsidence to the width/depth ratio of the panel without accounting for actual widths, nor the impacts of variable geology. This approach required modification as locally derived data was gathered and reinterpreted.The interaction of massive strata located close to the roof of the mined seam resulted in dramatic implications for some of the operations. When associated with longwall goaf geomechanics severe periodic weighting events and crippling mid-face roof falls were the consequence. When associated with narrower panels and shortwall goaf geomechanics, mines in the region experienced aggressive windblast events. The relationships between the overlying massive strata, longwall panel geometry and the various risks encountered during this period is in the author’s opinion, the most concise still documented.As the extensive coalfields of Queensland turn to underground mining operations the relevance of these experiences cannot be underestimated. This paper will discuss recent developments as well as incorporating diverse subjects such as microseismic monitoring, low frequency radio emissions and ‘electrophonics’ as they relate also to the topic.Like Moriarty, massive strata can be disguised and can offend when you least expect. This paper documents a trail of crimes committed by conglomerate in the Newcastle Coalfield that were inflicted on the general public, miners and mining equipment. Although this paper concentrates on the criminal behaviour of conglomerate, this story is also relevant to other massive units such as coarse sandstones that may be more difficult to identify and apprehend. The employment of a tunnel boring machine (TBM) is the default delivery method for the realisation of traffic, utility and primary resource underground space worldwide, with hundreds of kilometres completed safely each year. This is not yet the case for the mining sector in Australia, specifically underground coal mining. The safety advantages of TBM delivery when compared to New Austrian Tunnelling Method (NATM) delivery, or that of drill and blast, remains unrealised.With coal access drifts ranging between 1000?m and 3000?m in length, the construction of these facilities using TBMs could be done in a fraction of the time when compared to current practice. TBM world records currently stand at 435?m/week or 1600?m/month for the 7.0?m to 10.0?m range. Resource owners and developers should embrace the opportunity to deliver access to their orebodies in only a few months, excluding machine assembly and disassembly time.There are challenges in utilising TBMs for construction of Coal access drifts, such as; TBMs generally being bespoke design, maximising capital efficiency for drift development, and geotechnical variances between projects. This paper explores safer and more cost-effective options. This paper also provides detail of how the configuration within a completed drift could be optimised to ensure all site specific needs are met with regard to future use of the drift. The paper also includes descriptions of the engineering solutions that are available for physical and atmospheric challenges specific to coal drift applications and how the TBM manufactures are adapting to the regulatory requirements are also covered.Due to downward pressure on high cost bases and ever increasing safety requirements the popularity of TBMs in the Australian mining sector can be expected to increase and eventually become the default delivery method for coal drift construction, building them cheaper, faster and?safer.CITATION:Sibthorpe, D, 2014. Sustainability is one of those fuzzy words like innovation: everybody has an idea what it means, but if one starts asking for concise definitions then it becomes clear that everybody puts their own nuance on the meaning of these words. It’s not because these are difficult words per se, but rather because our understanding of them is highly dependent on the environment we work, live and play in. Upon subduction, water is liberated from the sediments and the porous struc- ture of the oceanic crust and from de- hydration of minerals for redistribution and chemical reaction. A survey of pro- cesses which are likely to occur along the subducting plate reveals that water may well be basic in subduction induced gas generation, magma formation and ore deposition. In analogy to the industrial water gas process, reaction of water with car- bonaceous matter, in subducted sediments is likely to yield substantial volumes of hydrogen and carbon dioxide. While water and carbon dioxide appear to play a determining role in the development of differentiated magmas, hydrogen will react with metal sulfides and sulfur to form hydrogen sulfide which is essential in sulfide ore depo- sition. A survey of possible metal extrac- tion chemistry along subduction suggests a stepwise solubilization of metals from sediments and oceanic crust, and a poten tial accumulation of dissolved metals in zones of characteristic composition. The metal spectrum in ore deposits at near surface appears to reflect the com- position of the solution in the respec- tive subduction zone. A tendency for preferential solubi- lization of copper at subductidn level, from among the transition metals, an in- creasing generation of hydrogen sulfide in the sane direction and partition effects as well as sulfide precipitation characteristics lead to enrichment in copper and to its selective deposition in the direction towards the trench as observed in S. Latin America. A postscript deals with the formation of native copper.

Increasing attention is being focused by regulators and industry on the residual environmental liabilities associated with mining projects following site relinquishment (termed surrender in Queensland). Even sites that have been rehabilitated to a high standard and to the satisfaction of the regulator are not completely risk free and may require some degree of ongoing management. As well as ongoing monitoring and maintenance costs, considerations should include how costs associated with potential risk event occurrence can be quantified and, if considered material, how provisioning should be undertaken to minimise the risk to the state and to taxpayers. In Queensland for example, once a resource site is fully rehabilitated and meets its environmental authority conditions or rehabilitation completion criteria, the tenure and environmental authority can be surrendered. During the surrender process any ‘residual risks’ resulting from the use of the site for the resource activity must be considered, and, if requested, a payment given to the government to cover any ongoing management and risk costs. Accent Environmental, in conjunction with Lane Associates, was engaged by the Queensland Government as part of the government’s financial assurance framework reforms to develop an innovative, ‘one-size fits most’ Residual Risk Calculator for the Queensland resources industry. The calculator is an Excel-based tool that uses detailed user inputs to estimate post-surrender management requirements and liability at resource sites. If residual environmental risks associated with rehabilitated mine sites can be identified, quantified and managed through pragmatic regulatory tools and effective provisioning, site relinquishment/surrender can be achieved without compromising environmental and social outcomes or exposing the state to unreasonable financial risk. The effective management of residual risk can also help facilitate operations as well as relinquishment/surrender of sites where a ‘walkaway’ solution may not be otherwise technically feasible.

The Rio Tinto (Mount Bruce Mining Pty Ltd) Koodaideri Iron Ore Project is located approximately 110 km north-west of Newman in the Pilbara region of Western Australia. During detailed ecological studies of the project area, a significant maternity roost (containing around 430 individuals) of the Orange Leaf-nosed Bat (OLNB) (Rhinonicteris aurantius) was discovered within the boundaries of the proposed mining area. The OLNB is listed as Vulnerable under the Commonwealth Environment Protection and Biodiversity Conservation Act 1999 and is listed as a Schedule 1 species under the Western Australian Wildlife Conservation Act 1950.With a good understanding of blast vibration attenuation in the Pilbara’s different geologies and in the absence of other known studies, it was considered that appropriate buffer distances between mining and bat colonies were not scientifically determined. Therefore, to inform the environmental approvals process that was underway, a series of seismic blasts was planned to assess and measure the sensitivity of the OLNB to trial-sized seismic blasting activities to develop a more scientifically based buffer zone for the protection of the OLNB. This was to be achieved by testing ground attenuation of the site and measuring any response from the OLNB to the firing of seismic charges. Rio Tinto sought the independent expertise of ecological consultants to evaluate and participate in the work to quantify the level of sensitivity of the bat colony and to outline the proposal to government authorities.After detailed risk assessment and project mapping, the seismic blasts were implemented, with the aim of firing individual charged and measured vector peak particle velocity (VPPV) at the OLNB roost cave, combined with monitoring OLNB disturbance activity within the roost that was invoked by the blasts. A series of six charges were fired with the VPPV ranging from 0.66 mms-1 to 18.70 mms-1. The OLNB activity was monitored using high frequency microphones, which detect bat calls emitted during flight. Under daytime conditions, the bats would normally be dormant, so the null hypothesis was for no calls to be detected if the bats displayed no disturbance behaviour. This work has improved understanding of mining activities (blasting) on the OLNB species, assisting in establishing mining exclusion zones that are based on scientific data, contributing to a sustainable future for both the environment and the mining industry.CITATION:Martin, D, 2015. The original and most basic relationship between mining company and explosive supplier was a simple one of consumer and product supplier. The divide between the two was quite clear-cut with neither taking much interest in the activities of the other. The explosive supplier had little understanding or share in the outcome, efficient rock breakage and therefore impact on mining and milling. Likewise, the mining company did not encourage the improvement or development of products. The transactions were limited to product and funds. This environment does not encourage any form of knowledge sharing, trust or optimisation of the mining process.Conversely, there are now a variety of relationships to the point that the mining company may simply be an asset manager whereby all æoperationalÆ aspects are contracted out. For the drill and blast aspect this takes the form of a æRock-On-GroundÆ contract whereby the explosives company takes on design, drill, load and fire responsibilities and is paid a rate per volume of broken rock. This removes the majority of risk to the mining company derived from having to produce broken rock and guarantees the cost of a portion of the overall unit cost of mining. The onus is now on the explosives supplier to break rock to a specification as efficiently as possible.There exist a number of variations between the æbasic supplyÆ and æRock-On-GroundÆ. These contracts may also include value sharing to encourage the explosive supplier to optimise the mining operation to maximise profit.This paper looks at some of the drivers for the change in explosive supplierûconsumer relationships and examines the æprosÆ and æconsÆ of a few of the options.

Visual computing opens the design process to a wider audience and allows rapid communication of mine plans to all stakeholders. The integration of software and hardware technologies that take advantage of a graphical environment will continue to improve both long-term and short-term mine design (Figure 1). A good design must be adaptive to respond to decision changes as the mine develops and more information becomes available. The constraints on mine design predicate careful analysis in order to operate the mine as efficiently and safely as possible.Continuous improvement in mining is assured by developing innovative technologies that target mine survey, drill and blast, modelling, short-term design and pit and stope optimisers. The design life of a mine has to be sufficiently long to ensure the mine is commercially successful, justifying the high start-up capital costs. Being able to execute a plan so that it honours the design realises the rewards of computerised mine planning and the long-term investment in data acquisition.

Poorly judged or under-appreciated change plays a role in many major accidents. In particular, operational changes related to continuous improvement, maintenance and introduction of new technology can lead to unintended safety (and other) consequences. This paper provides an overview of the role of management of change in preventing unwanted impacts on safety, community relationships and the environment. It draws on a range of incidents covering mining, minerals processing, refining and smelting where inadequately assessed or ineffectively implemented change led to significant safety, environmental or community consequences. The paper will also highlight cases of well-managed complex change and the underlying model of decision-making in place to make those decisions. All these examples are drawn from Atturra’s experience in response to accidents or working with sites to improve their management systems. The paper will report on the challenges of applying conventional risk assessment approaches to complex or technically significant change. These typically result in either onerous and lengthy change procedures that are prone to workarounds or poor implementation. Conversely, ‘light touch’ or efficiency driven change processes present the risk of intended consequences by failing to bring appropriate expertise to bear in decision-making. Such balances can be resolved by considering the designation of ‘technical authority’ with appropriate expertise to consider the change and deconflict any production, cost or schedule trade-offs. Finally, the paper will reflect on the key components of management of change systems in the context of major accidents, highlighting good practice in how they are implemented. This will consider the integration of existing corporate processes, risk management and organisational structures into management of change systems, not just the content of change forms and procedures.
