This NIOSH Toolbox Training program contains 52 different modules designed to stimulate safety discussions among your employees. Each module is intended to be completed in roughly 10 or 15 minutes, so a Toolbox Training session can be conducted once a week for the entire year. Each session counts toward your employee’s 8-hour annual MSHA refresher training, thus saving an entire day of work.. The modules are used to refresh employees on the procedures, policies, and practices of working safely at a specific mine site and can easily be tailored to fit conditions and situations found on your property. Every module follows the same format, so that as the weeks pass, miners will start to approach every safety situation in the same way. A typical Toolbox Training session contains five basic steps that cover all the information in the module. 1. Identification of the hazard and discussions about the types of injuries typically associated with the hazard. 2. Discussions of some of the causes of the hazard. The MSHA example included with each module should be referenced. 3. After the hazard and accident has been explained and understood, discussions about best practices and tips for avoiding a similar accident. 4. Review of similar conditions or near misses from your mine site using the leading questions on the form to start a discussion. Specific examples of equipment or structures on the site that could lead to the same accident should be discussed. The blank space on the module is used to add your own remarks and questions to supplement the given information. 5. Miners fill out the training record on the back of the form with name, initials, and date of training session. The modules are designed to be kept in a standard three-ring binder and easily photocopied. This way, the same set of modules can be used for years to come. A blank form is included that can be used to create more Toolbox Training modules. The key component of Toolbox Training is to start a discussion among employees that will allow them to see the hazards around them every day and respond to these hazards in a safe and consistent manner. To start a discussion, ask questions that need more than a one-word response. Get the miners to talk about similar situations from their own experiences and give suggestions about how to fix the hazards around them. Toolbox Training is a quick and effective way to cover specific mining hazards, but it only works if miners are involved in the discussion.

"IntroductionOn May 27, 1927, a coal-dust explosion occurred at the Palagua No. 5 mine of the Victor American Fuel Company, Delagua, Colorado, resulting in the loss of even lives and in and in considerable damage to the main slope and to several overcasts the explosion did not extend into the mine working where 125 additional men were employed but escaped uninjured. It is believed that the saving of these 125 men and the evidence of large property loss were due in a large measure to coal-dusting and of other sanity measures taken by the coal company management. A number of features connected with the explosion and mine safety practice seem worth the attention of the mining public and grateful acknowledgment is made to the Victor American Fuel Company for its aid in assembling the data herein contained."

Over the past decade, the U.S. Bureau of Mines (USBM) has studied longwall gate road stability at four mines in the Book Cliffs and Wasatch Plateau Coalfields of Utah. These operations are characterized by multiple-seam mining, abruptly varying cover depths to 914 m (3;000 ft), and massive rigid, sandstone units in the main roof or floor. Such conditions encourage severe bumping, roof instability, and occasional floor heave problems.' Various gate road configurations have been employed to alleviate' these problems. Although the mines have comparable basic conditions, the lithologies and qualities of the immediate roof, floor; and seam ate different in each mine and often vary in a single mine. Gate road systems that mitigate hazards in one mine may prove inappropriate in another or for different areas of the same mine. This report relates the geology of the coalfields; describes the location, geologic setting, specific mining conditions, operating history, and the USBM field study and results or conclusions for each mine; summarizes the relative performance of the gate road systems; and emphasizes the need for site-specific geotechnical data to evaluate gate road conditions and performance.

We all know that safety consciousness is the most potent factor in the prevention of accidents. Safety consciousness may be defined as awareness of hazard and alertness to danger, which controls every action of an individual through his desire to remain alive and uninjured.

The coal-bearing strata of Oklahoma occur in the eastern part of the state over an area of 14,500 square miles, 20.7 percent of the state's total area. Commercially valuable deposits of bituminous coal cover about 8,000 square miles in seams ranging from ten inches to five feet thick. Estimated reserves in Oklahoma are 1.6 billion tons, representing 0.3 percent of U.S. coal reserves. The vast majority of coal mined in Oklahoma used in a single power plant within the state. Oklahoma gained primacy when its regulatory program was conditionally approved January 19, 1981. Authority for the state to administer the Abandoned Mine Land program was granted January21,1982. On April 30,1984, the Office of Surface Mining substituted direct federal enforcement of the inspection and enforcement portions of the Oklahoma program. This substituted enforcement was based upon a finding by OSM that Oklahoma had not operated the inspection and enforcement portions of its program effectively. As of January 1, 1986, Oklahoma inspection and enforcement authority was reinstated. Full program authority was granted to Oklahoma on January 14, 1986.

The past full year of war has increased greatly the demand for virtually all kinds of fuel, and the Bureau of Mines research and service facilities have been extended to meet these unprecedented requirements. Liquid fuels, such as aviation and motor gasolines, are fundamental to modern warfare. Bituminous coal and anthracite are needed, not only to servo the expanded industrial program for which coal normally is used but also to replace natural gas, wherever possible, and fuel oil in the Eastern States, New England, and the Pacific Northwest. Railroad transportation with energy derived from coal is replacing much of the motor-vehicle transportation that has been stopped or curtailed to conserve rubber. Pulverized coal hoe been sound to be a satisfactory and cheaper substitute for fuel oil in metallurgical heating furnaces. Celloidal fuel - a mixture of coal dust and oil - may offer a partial substitution for curtain coal-burning equipment. Several industrial experiments have been made with this type of fuel. The expansion of the steel industry in connection with the war program has required a corresponding increase in the production of metallurgical coke and its associated byproducts - light oil, tar, ammonium sulfate, and surplus gas. If the war is of long duration and new reserves of petroleum are not discovered, wartime trends from fuel oil to coal probably will not be reversed.

The Department of the Interior has recently investigated the atmospheric conditions in tunnels of the Union Pacific Railroad in Utah and Wyoming , by observations made from the cabs of freight locomotives . This work was conducted by the Bureau of Mines at the request of , and in cooperation with , the Union Pacific Railroad Co .. and was brought about by several accidents to members of engine crews while passing through tunnels , from pollution of the tunnel air by exhaust gases from freight locomotives . The object was to determine the cause of gassing accidents , by examining into composition of the air in locomotive cabs while passing through railroad turmels ; to learn the effect of these conditions on the engine crew; and to provide a means of protection for the men so exposed . Gas samples and temperature readings taken in the cabs of locomotives were used in studying the atmospheric conditions to which the locomotive crews were exposed . The symptoms and the physiologica effects produced in men exposed to the atmospheres encountered were studied . The pulse rates and body temperatures were taken , and the determinations of the carbon monoxide content of the blood were made . Various methods for the prevention of gassing and for the protection of men therefrom were considered and tested , among which were the use of mechanical devices for deflecting the smoke away from the engine cab, and the use of various types of gas masks and breathing apparatus . While this investigation is a part of the safety work of the Bureau of Mines in connection with hazards from atmospheres containing carbon monoxide ( CO ) , the results are of particular value to railroads operating steam locomotives through tunnels , and are also valuable to other industries where atmospheres having poisonous gases or of a high temperature and humidity may be present . Volatilization in retorts has been, until recently, the only commercial process of producing spelter, hence the zinc mine operators have had to meet the terms of the zinc smelters in regard to the following requirements: (1) Minimum percentage of zinc the ore must contain; (2) the chemical combination of the zinc in the ore; (3) the proportion present of those elements that interfere with the suceessful recovery of the zinc when the ore is retorted. In consequence of these requirements, only a comparatively small proportion of the zinc ores mined can be shipped directly to the smelters.

In order to extend or delimit the known ore reserves adjacent to the Comet Coalition property on the western slope of the Highland Peak Range about 8 airline miles west of Pioche, Nev., the Bureau of Mines undertook an exploration project of diamond drilling. Production figures from this property, formerly known as the Schodde, are not available, but it has been reported that small amounts of rich silver and gold ore were Shipped by wagon to a smelter in the Salt Lake valley during 1900-1910. Small shipments were reported for the period 1915-1927, also. A study of the Pioche District by the Federal Geological Survey in 1942 and 1943 was the basis for the exploration program of the Bureau of Mines in 1947 This program consisted of diamond-drilling three holes. The total footage was 2,967 feet. No. 1 hole was drilled 1,260 feet, a record depth for diamond drilling in the Pioche District. Silver-bearing lead-zinc sulfide ores are found in the Combined Metals limestone, known locally as the C.M. bed, These deposits have been formed by replacement of the limestone that is inclosed in the Pioche shale.

The explosives used in coal mines not only occasion accidents such as occur in the use of explosives elsewhere, but they frequently cause widespread disasters by igniting explosive mixtures of mine gas and air and of coal dust and air, or both. In addition, the firing of ex- plosives so shakes the walls and roofs of the mines as to cause falls, with their attendant casualities. Considering the large number of explosives and their wide range in composition and properties, it is obvious that certain of them should prove more suitable for use in coal mines than others. The problem is to determine which explosives are the most suitable for this purpose and the precise conditions under which they may be used with the greatest safety. Of course this problem could be worked out in actual mining, but experimental investigations when accompanying commercial opera- tions are hazardous and slow. To obtain the information sought, in the speediest and most economical manner, laboratory methods must be resorted to. The adoption of such methods began about 30 years ago with experiments in an iron gallery mounted on wheels, at Zwickau, Germany. This laboratory method of attacking the prob- lem has so commended itself that it has not only been repeated else- where in Germany but has at intervals been officially adopted, with many modifications and additions, by the Governments of Belgium, Austria, France, and Great Britain. Such was the status of coal-mine investigations in Europe when in 1904, an organization in the Geological Survey, which in 1907 be- came the technologic branch, was created to investigate the mineral- fuel resources of this country. At that time no division of the Na- tional Government had official cognizance of conditions existing in our mines, but through its system of collecting fuels for tests the technologic branch was forced to recognize that coal-mine accidents are a factor that must be reckoned with in determining the extent and availability of our mineral-fuel resources. This became so apparent, and it was so obviously a function of this branch to make a systematic research as to the cause and prevention of such acci- dents, that in July, 1907, experts were appointed especially to deal with this problem. As a result, not only was Bulletin 333, on Coal- Mine Accidents: Their Causes and Prevention, prepared and pub- lished, but through personal inspection of foreign testing stations, and a study of their reports and the critiques of their methods and operations, definite plans for the establishment of such a testing station in this country were formulated. In the latter part of 1907, four grave disasters occurred in close succession consequent on explosions in the Monongah mines in West Virginia, where 368 men were killed, the Darr mine in Pennsyl- vania, where 160 were killed, the Naomi mine in Pennsylvania, where 34 were killed, and the Yolande mine in Alabama, where 61 were killed. These frightful disasters so quickened the public con- science, and so plainly demonstrated that both humanitarian and economic needs demanded that steps be taken to prevent the re- currence of such accidents, that in 1908 Congress made a special appropriation for an investigation as to the "causes of mine ex- plosions." A similar appropriation was made in the following year. By act of May 16, 1910, the Bureau of Mines was created "to make diligent investigation of the methods of mining, especially in relation to the safety of miners, the use of explosives, the prevention of accidents," and other matters relating to mining. Therefore, these investigations are at present conducted in obedience to an organic act instead of being dependent solely upon special appropriations. The first appropriation became available July 1, 1908. Through the courtesy of the War Department a portion of the grounds and buildings of the arsenal at Pittsburgh was granted for the use of the technologic branch of the United States Geological Survey. Work was immediately begun in procuring and erecting the necessary instru- ments and appliances for carrying out the desired tests and inves- tigations and in adapting the existing buildings or in erecting new structures to contain the apparatus. The Pittsburgh testing station was officially opened and regular work was commenced on December 3, 1908, the apparatus and appliances having been tested, the force drilled in their use, and the equipment and methods inspected and passed upon by foreign experts.

Computer models for simulating the construction and for calculating the properties of particulate solids in two- and three-dimensional systems (coverings of circles and packings of spheres, respectively) were developed. By simulating the physical process of constructing circle coverings and sphere packings, in which the circles or spheres were randomly selected from many different distributions of sizes, physically realizable loose random assemblies were simulated whose physical and geometrical properties were predictable with the computer. Many of these properties, such as the number of contacts, are extremely-difficult to measure accurately, or considerable ingenuity is required in their determination. All of these properties were easily and accurately calculated by the computer models for many different coverings and packings constructed from equal, discrete, uniform, and log-normal distributions of particles. In addition to the loose random assemblies, dense random covering and packing constructions were simulated whose properties are ideal upper limits for those of the loose random coverings and packings. The properties of the dense random coverings and packings, heretofore only obtainable by theoretical analysis, were also simply obtained. The statistical geometry of loose random coverings and packings was studied, and several new interpretations of the area-covering or space-filling requirements in these systems were made. In two dimensions, it was found that the properties of all loose random coverings are predictable by taking suitably weighted averages of the corresponding properties of just four kinds of polygons whose edges are defined by connecting the centers of mutually tangent circles in the covering. The distributions of these polygons that permit area-covering were determined, and empirical equations for predicting the density, average coordination number, and average pore area in loose and dense random coverings were formulated. The properties of loose packings were determined by performing a simplicial division of the computer-simulated assemblies to produce a unique set of minimum volume, space-filling tetrahedra whose edges connected the sphere centers in each packing. A simple empirical equation was formulated that permits the calculation of the average number of contacts in any sphere packing, random or regular. Some of the statistical geometrical properties of the simplicial tetrahedra in the computer-simulated packings were analyzed. The tetrahedron edge length distributions in all the loose random packings were nearly identical when the edges were presented in terms of sphere diameters, regardless of the sphere population in the packing. Moreover, the gap lengths in the loose packings were to an excellent approximation representable by a uniform distribution of sizes. The distributions of spheres in tetrahedra resulting from the simplicial division of the loose random packings also existed in the tetrahedra in dense random packings. These observations were employed in developing a unique method for predicting the geometrical properties of loose random packings without simulating the assembled packing. Instead, the space-filling simplicial graph could be predicted for any given density of packing of any sphere population. The cumulative packing properties of these tetrahedra were shown to be nearly identical to those calculated by all loose random packing simulations attempted in this investigation. The simplicial tetrahedra completely describe a packing and can therefore be used to predict all properties of interest.

The U.S. Bureau of Mines conducts basic and applied research related to predicting, eliminating, and/or controlling rock bursts in underground hard-rock mines and coal bumps in underground coal mines. An important element in this research is the development of a reliable and efficient microseismic source location technique. This report presents the results of a Bureau study to develop and evaluate a new algorithm to locate microseismic events based on the Simplex method, a powerful linear programming technique. The technique was tested on both field data and simulated data. Microseismic event source locations obtained with this new Simplex-based algorithm were shown to have less error than those locations obtained using a least squares method. It was concluded that the Simplex approach to source location of microseismic events can be used effectively in microseismic monitoring.

This report is the first of a series dealing with the oil-shale reserves of the Green River formation in the Rocky Mountain Region. The known oil-shale reserves of northwestern Colorado at present considered to have economic value apparently are confined to the Piceanco Creek basin between the Colorado and White Rivers. The number of sampled sections is small, but there can be little doubt that the Piceance Creek basin is a potential source of enormous quantities of liquid fuels and the related byproducts obtained from oil-shale processing. The partly blocked and inferred oil-shale reserves of 25-gallon-a-ton shale are estimated to have an oil yield of 125 billion barrels and the reserves of 15-gallon-a-ton shale are estimated to have an oil yield of 494 billion barrels. The number of available sampled sections is inadequate for making firm estimates.

The Bureau of Mines has sponsored a comprehensive research program to enhance the safe, effective, and efficient use of blasting technology by the mining industry. Recent research results of the surface mine blasting program were presented at a seminar on April 25, 1987, in Chicago, IL. Many of the topics discussed at the seminar are presented in this proceedings. The new research described in these papers includes computer monitoring of rock motion, the influence of blast delay times on rock fragmentation, blasting effects on Appalachian water wells, blast vibration measurement near structures, and the reduction of accidents through improved blasting safety.

Background. There is currently little information regarding factors associated with specific modes of motion segment failure using a fatigue failure model. Methods. Thirty-six human lumbar motion segments were fatigue tested using spinal compressive and shear loads that simulated lifting a 9 kg weight in three torso flexion angles (0°, 22.5°, and 45°). Twenty-five segments failed via fatigue prior to the 10,000 cycle maximum. These specimens were visually inspected and dissected so that the mode(s) of failure could be determined. Failure modes included endplate fractures (classified into nine varieties), vertebral body fractures, and/or zygapophysial joint disruption. Logistic regression analyses were performed to determine whether certain morphometric variables, amount of motion segment flexion, disk degeneration scores, and/or loading characteristics were associated with the occurrence of specific failure modes. Findings. Results indicated that stellate endplate fractures were associated with increased posterior shear forces (P < 0.05) and less degenerated discs (P < 0.01). Fractures running laterally across the endplate were associated with motion segments having larger volumes (P < 0.01). Endplate depression was more common in smaller specimens (P < 0.01), as well as those experiencing increased posterior shear force (P < 0.05). Zygapophysial joint damage was more likely to occur in a neutral posture (P < 0.01). Interpretation. These results suggest that prediction of failure modes (e.g., specific endplate fracture patterns) may be possible (at least for older specimens) given knowledge of the spinal loads along with certain characteristics of the lumbar spine. Motion capture involves recording the position and global orientation of joint sensors of a real object, in most cases a real person performing some human activities. This information is usually recorded at uniformly spaced instances of time, or as it is often called frame-by-frame. Then the recorded motion data sets are processed and mapped into a skeleton hierarchy of a virtual, computer simulated human figure to control the motion of the virtual human in the computer simulation. In the first part of the paper we review several new techniques developed to facilitate the manipulation, noise reduction, storage and reuse of captured data, which have a potential to reduce the overall cost of motion simulation and improve its realism. In the second part of the paper we consider the real life problem of reducing a worker?s risk from being hit by underground mining machinery in a confined space. We formulate a set of requirements to motion editing for this particular task and analyze the limitation of existing techniques
