This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. An evaluation of blasting procedures in surface mining operations was undertaken to determine means for improving safety. The objectives of the study were to: 1. Develop guidelines for safer blasting practices 2. Specify changes or additions in regulations concerning the use of explosives 3. Identify areas in which additional knowledge needs to be developed. As a basis for the evaluation, an idealized blasting cycle was prepared. Developed in the form of a scenario of surface mining operations, this synthesis of good mining practices served as the standard against which observed activities were compared. Blasting hazards were divided into two categories: those known to exist and those which are potential. Known hazards were identified in terms of situations which have actually resulted in explosives-related accidents. The evidence for known hazards was obtained primarily from MESA records of mine accidents. Potential hazards were identified by observation of mining activities and consultations with mining, explosive industry and government regulatory agency personnel.

The Bureau of Mines designed a down-fired refractory-lined combustor that efficiently burned pulverized coal to produce a gas suitable as working fluid for an open-cycle gas turbine. The gas contained relatively little carbon, so the load on the ash-separation equipment was significantly reduced. Moreover, reduction in carbon content of the gas increased the average density of the solid particles making them more amenable to removal by centrifugation. Centrifugal-type separators failed to remove sufficient ash; however, to limit blade wear enough to achieve operating periods generally deemed necessary for commercial practicability.

Over the period 2000 to 2003, roof falls have accounted for 4 to 14% of the fatalities in underground mining operations. The National Institute for Occupational Safety and Health (NIOSH) is conducting research to reduce the frequency, exposure, and risk of these events through an ongoing program of field and laboratory studies. One area of research involves the evaluation of polyurethane grouting technology that is commonly used to stabilize fractured mine roof strata. Concurrently, NIOSH is conducting work to evaluate the application of ground penetrating radar for advanced delineation of problematic mining areas. In this study, NIOSH partnered with Sub-Technical, Inc. who injected a polyurethane grout (also known as glue) into a roof area of NIOSH’s Safety Research Coal Mine. The mine roof area was scanned using ground penetrating radar technology before and after grout injection in an attempt to determine the extent of grout infiltration. A comparison of the pre-and post-grouting radar records showed a significant change at a mine roof depth of about four to five feet. The interpreted radar records were then compared with drill core information, borescope evaluation of roof-bolt holes in the study area, and underground observations. At this site, the interpretations of the radar records correlated with data obtained from the core holes, borescope evaluations and underground observations. This study showed that ground penetrating radar technology can be a useful tool for detecting changes in mine roof due to the injection of the grout.

The data in this bulletin were obtained during an investigation of screening practice by the University of Illinois engineering experiment station and the United States Bureau of Mines under a cooperative agreement between them and the State of Illinois Geological Survey Division. Earlier work on this subject includes a study of the general practice of coal preparation in Illinois, made in 1914 and published in 1916,1 and an investigation of Illinois coal-washing methods, results of which were published in 1912 in a bulletin 2 that contained complete information on the methods then in use for sizing washed coal. OBJECTS OF THE INVESTIGATION The present investigation was limited to a study of screening as applied in preparing mineral products with the expectation of obtaining more comprehensive information on screening machinery.

Man has been engaged in the construction of tunnels in. rock for several thousand years. During that time, methods of tunneling have changed drastically. Hordes of men chipping away at the tunnel face have been replaced by mechanical drills and explosives and by tunnel boring machines. Spoil is no longer carried on the backs of humans, but is now removed by trains, trucks or conveyor systems. These and other changes have had the effect of greatly increasing the rate of tunneling, while at the same time, greatly decreasing the potential dangers to workmen. Although the techniques, equipment and material used for tunneling are being continually improved, rock tunneling is still a slow and expensive process. Significant improvements and innovations are needed to reduce the cost and time required for tunneling if we are to fully utilize the subsurface to aid in the solution of urban problems and in the conservation of resources. This report is intended to facilitate the orderly development of tunneling technology by focusing attention on the inadequacies of rock tunneling and the improvements that are needed. The data presented were derived from the results of a comprehensive questionnaire that was completed during the summer of 1969 by more than 600 individuals and organizations in 17 countries. Table 1 lists the countries and the number of replies from each. Appendix A contains a summary profile of the national responses, while the text presents the results and a brief analysis of the conclusions that might be derived from the data in the questionnaire. The data shown in Appendix A represent the consensus opinion of each of the 17 nations without regard to the number of individual replies or the amount of tunneling activity that is represented. The national consensus was determined for each country by a plurality of the individual responses, in some cases modified by the opinion of the rapporteur. Tire above procedures can lead to situations which are misleading. For example, experience and activity arc a rational basis for analysis; but, if national replies are weighted on this basis (i.e., number of responses), informed and valuable opinion of less active countries would probably be overwhelmed and lost. In analysing the data on the basis of national responses without weighting, it becomes apparent that many of the same problems exist under a given set of conditions regardless of the number of tunnels that have been driven.

Electrical arcing injuries are the most common form of electrical injury in mining. MSHA data show that 381 "non-contact electric arc burn" injuries occurred in the mining industry from 1996 - 2005. A more detailed study of data from 1990-2001 showed that five types of apparatus - circuit breakers, conductors, nonpowered hand tools, electrical meters and test leads, and connectors and plugs were involved in two-thirds of the lost workday injuries. Many of the protections from electrical arcing injury available to workers in other industries stem from the application of National Fire Protection Association Standard 70E - Standard for Electrical Safety in the Workplace (NFPA 70E). NFPA 70E is a generally accepted industry guideline document, but excludes underground mining from its scope. Many of its practices and procedures, however, can be applied in the mining workplace to supplement the existing electrical safety requirements of Title 30 Code of Federal Regulations. This paper describes how NFPA 70E can be applied to mining workplaces after an analysis of the mine power system electrical arcing hazards has been completed (and Hazard/Risk Categories determined) using the tables from NFPA 70E or the calculation method as described in IEEE 1584. NFPA 70E can then be used to determine the levels and types of personal protective equipment (PPE) needed to protect miners against burn injuries. Appropriate lockout and tagout (LOTO) procedures for use in the mining industry are also described. Suggestions for better selection of electrically-rated hand tools are included. Recommendations for electrical meter selection and use based on transient withstand capability are also discussed.

This summary of State laws or explosives was compiled primarily to ascertain what subjects relating to their control have been acted upon by each State legislature and, in general, how they have been treated. It does not include any legislation passed after 1942, or any proclamations, regulations, or administrative orders. It does not purport or attempt to be authoritative. If any problem arises involving interpretation of the statutes of any particular State, the original source must be consulted for the exact words used by the legislature. This summary, however, will be useful In determining whether relevant statutes exist, and In locating them quickly,-with a minimum of effort. It will be of value also in comparing statutes in one State with similar statutes in other States to determine what construction has been placed upon them by the courts of those jurisdictions or to contrast the varying provisions of many States upon a similar subject. To make these comparisons easier, the statutory provisions for each State have been grouped under several major topics in outline form. Although this sometimes has resulted in disruption of the continuity of individual acts, the increased general clarity and availability of comparable sections have more than justified the procedure followed.

Several growth chamber and laboratory experiments were conducted to study the germination behavior of plant species on mine spoils and plant growth responses to nutrients on spoils and topsoils. Experiments were conducted to study the germination potential of some native and introduced species under various conditions. The seeds of green needlegrass, side oats grama, thickspike wheatgrass, crested wheatgrass, birdsfoot trefoil and emerald crownvetch were less tolerant, whereas those of alkali sacaton. durar hard fescue, slender wheatgrass, yellow sweetclover and alfalfa were most tolerant to the sodic mine spoil conditions. Addition of leonardite improved the germination potential of some of the poor genninators. Slender wheatgrass and yellow sweetclover were used as test plants to study the fertility status and productivity potential of stockpiled topsoils and spoils collected from Center. Glenharold and South Beulah of North Dakota. The topsoils were neither saline nor sodic but low in N and P. The spoils were neutral to alkaline with SAIL of 12-32, EC of 1.8-5.5 mmhos/cm, Olsen&apos;s P< µ4g/g, and total N <0.07%. Application of N and P together to the topsoils increased the yield of wheatgrass and sweetclover by 88% and 70% of their respective maximum yields, whereas near maximum yields were obtained for both the species by the same treatments to the spoils. The average yields on topsoils were higher than the yields on spoils. The concentrations of major nutrients were higher in plants grown on topsoil, whereas the opposite was true with the concentrations of Mn, Zn, and Cu. Growth chamber experiments were also conducted to study the growth responses of alfalfa, oats and slender wheat grass by the application of Mn and Zn to a sodic coal mine spoil which was previously used for pot culture experiments. Significant growth responses to Mn and Zn were observed in alfalfa but not in oats or slender wheatgrass. Fertilizing the spoil with Mn along with NPK increased the yield of alfalfa to 85% and with Mn + Zn to 99%. Growth responses and plant composition results of this study confirm that the application of major nutrients to spoils can accentuate Zn and Mn deficiencies in some plants.

A series of experiments were undertaken by the Bureau of Mines to determine the emission products of several types of conveyor belting and other combustible materials found in mines. These experiments were conducted under intermediate-scale, simulated mine conditions to determine smoke characteristics and gas concentrations. From these determinations, heat-release rates, particle sizes, obscuration rates, combustion yields, and production constants were calculated. Three types of belts were investigated: chloroprene, also known as neoprene (NP); polyvinyl chloride (PVC); and styrene-butadiene rubber (SBR). The belts were designated as ignitable or self-extinguishing depending on the length of the burning time and the subsequent combustion products. Under these experimental conditions, the SBR belts were the easiest to ignite. The PVC and NP belts tended to self- extinguish within a few minutes after ignition, but were still capable of maintaining a brief flaming period. These conveyor belt combustion results are compared with previous analyses of wood, transformer fluid, and coal fires. Together they form a data base by which findings from future experiments with other mine combustibles can be compared.

"Packaged fuel" is the trade name applied by the industry to a product consisting of coal screenings and a binder compressed into 3 -or 4-inch cubes and wrapped (six or eight in a package) in sturdy paper and sealed with gummed tape. Parry describes the product in greater detail and distinguishes clearly between packaged fuel and briquettes. The industry developed from the desire of retail coal dealers to improve utilization of the relatively large quantities of slack sizes that result from the degradation, during shipping and handling in the yard, of friable coal, particularly Pocahontas coal. During the war an attempt was made to apply this technique to anthracite fines, and a recent publication of the U. S. Bureau of Mines reported the results of physical and combustion tests on packaged fuels containing anthracite fines. References made in this report to the previous investigation on packaged fuels containing anthracite represent data or conclusions presented in Report of Investigations 3882. This investigation was conducted as a continuation of the previously reported studies and consisted of physical and combustion tests of packaged fuels made from high-volatile bituminous coal, as well as the low-volatile bituminous coal ordinarily used in the industry, to determine their relative suitability for domestic firing. Particular attention was given to the smoke-producing properties of the packaged fuels because of the increasing importance of the prevention or reduction of smoke from hand-fired domestic heating plants resulting from the widespread enactment of smoke-prevention ordinances.

This publication describes mining and milling methods and costs at the Madison mine of National Lead Co., St. Louis Smelting & Refining Division, in the Fredericktown district of southeast Missouri and is one of a series prepared by the Federal Bureau of Mines on methods, practices, and costs of mining and milling throughout the United States. The are bodies of the Madison mine are known for the complexity and diversity of their are minerals. Sulfides of copper, nickel, and cobalt occur in commercial quantities with lead sulfide, which is the most valuable metallic mineral in are bodies of the Bonne Terre formation in southeast Missouri. Several attempts were made by various operators to mine the complex ores and separate the sulfide minerals before 1927 when National Lead Co. became interested in the Fredericktown district. Operating then as the St. Louis Smelting & Refining Co., the company began mining and milling ores from the Madison mine in 1944 and has operated continuously since then. A recently built refinery produces copper, nickel, and cobalt metal from a concentrate containing iron sulphides and small quantities of copper, nickel, and cobalt sulphides.

Introduction This Instructor’s Copy contains most of the information needed to use the Wearing Hearing Protection Properly 3-D training reel. It offers practical suggestions on how to use the reel as a training aid, performance objectives for deciding if the training is appropriate for your needs, and discussion notes that provide additional information to the user. The 3-D reel was designed as a self-teaching aid that drillers and others who work at a drill site can use wherever the opportunity for such training presents itself. The latter could be a company preshift requirement or a tailgate session where workers would have an opportunity to view and discuss the reel. It could also be an individual looking at the reel in a pickup truck prior to beginning work at a drill site. The 3-D training reel is versatile in that it can be used for a small group of persons or for a single worker. For any of these situations, information is transferred to the individual by viewing each of the seven colorful, in-depth 3-D scenes and relating the words that appear in each scene to what is depicted in the visual. Real gross national produce (GNP) grew at a seasonally adjusted annual rate of 2.0% in the fourth quarter (table 1). The restrained quarterly growth rate was the slowest in 2 years, and was substantially slower than the 2.5 % rate of the previous quarter. According to the U.S. Department of Commerce, the summer drought reduced the fourth-quarter growth rate by 1.1 percentage points. A growing trade deficit and a reduction in business investment were the key economic factors curtailing growth. The trade deficit widened by $6.8 billion, reflecting both a surge of imports and a slowing of exports. Business investment declined 3.7%, mainly as a result of a slowdown in computer sales. On a year-to-year basis, the economy grew at an inflation-adjusted rate of 3.8% for 1988, the fastest in 4 years, despite the drought. However, on a fourth-quarter-to-fourth-quarter basis, real growth measured 2.7%. Increased exports and business spending for plant and equipment accounted for much of the 1988 GNP growth. Employment continued to grow as the manufacturing sector added 414,000 new workers (table 2). Of these, the primary metals industry added 26,000 workers, and the clay, glass, and stone industry added 10,000. The civilian unemployment rate remained low throughout 1988, fluctuating between 5.3% and 5.4% during the last quarter.

Fires that occur in abandoned coal mines, waste banks, and in coal outcrops constitute a serious health, safety, and environmental hazard. Toxic fumes, the deterioration of air quality, and subsidence constitute the greatest hazards from these fires. Although tires on abandoned mined land (AML) occur in every coal-producing state, the severity of the problem varies. Methods to extinguish or control AML fires, including excavation, fire barriers, and sealing, are generally expensive and have a relatively low probability of success This U.S. Bureau of Mines report includes information from a variety of sources, i.e., agencies of the Federal Government, State agencies, research reports, conference proceedings, product information, and technical literature. This information has been collated into a comprehensive discussion of AML fire problems. Data on past fire control projects and on the estimated extent of the current problem have been compiled. Factors affecting the occurrence, propagation, and extinguishment of AML fires are discussed. Conventional fire control methods are described, and their probable effectiveness is evaluated. Information on the hazards of AML fires and safety considerations is included. The status of current technology, recent improvements in fire control methods, and areas of current research are discussed.

Watershed Jll in Jefferson County, Ohio we instrumented in 1977 as one of three watersheds established for the purpose of investigating surface mining and reclamation effects upon local hydrology, sediment discharge, and sediment and water quality. Each watershed was less than 50-acres in size, and was in a different geologic setting within southeastern Ohio. Surface runoff and ground¬water levels were monitored. Samples were taken for sediment determination and chemical analysis prior to mining. Monitoring and sampling continued through the period of watershed disturbance caused by mining and reclamation activities, which began in May 1980 at Watershed Jll. A psuedo-postreclamation period followed the initial cessation of man-machine activity on the watershed. A second reclamation effort occurred in the fall of 1981 and was to be followed by a third in 1982 after observations ceased. A true final postreclamation period was not monitored at this site, because the erosion control practice (terrace diversions) failed after each reclamation. Watershed Jll was a small pasture and wooded watershed with sandstone and shale above the mined coal seam prior to mining. A clay layer under the coal supported a perched saturated zone, termed the top aquifer for convenience. A lower underclay supported a second saturated zone called the middle aquifer. A lower perched aquifer had been dewatered by previous deep mining. After about three years of premine data and about two and one-half years of mining reclamation and psuedo-postreclamation data, the following general observations may be made: 1. The mining-reclamation phase was very short, and the postmining period was characterized by erosion failures and remedial disturbances that kept the watershed condition and size in a state of change. It was impossible to collect sufficient data to confidently characterize the hydrologic-sediment-chemical aspects of the mining period or of a postreclamation period and, most importantly, of the future watershed after its various properties and characteristics have ceased adjusting to its severe disturbance and reconstitution. 2. A seeding of wheat followed by grasses and legumes was not a particularly successful reclamation practice in terms of stabilizing the new soil. Numerous rills and small gullies developed in spite of installed diversions. 3. Storm runoff rates and yields at the watershed outlet increased from the premine, (phase 1) to the mining-reclamation (Phase 2) periods, and remained at or near the Phase 2 level through the postreclamation (Phase 3a) period. 4. The top aquifer changed in physical and chemical character as a result of mining and was becoming reestablished as the project drew to a close; but it is impossible to predict its final configuration or character. 5. Dewatering of the top aquifer resulted in a complete cessation of baseflow because the middle aquifer water table lay below the elevation of the watershed outlet. 6. The top and middle aquifers were merged in one area of the watershed where the upper coal underclay was disturbed. 7. Failure of the diversion terraces and their outlet obscured the effects of the reclamation practice upon sediment concentration and yields. There was little difference in sediment concentrations between Phases 2 and 3a. 8. Drinking water standards were met by surface waters throughout the period of observation except for two samples that exceeded the nitrate-N allowable level of 10 mg/l. 9. In all Phase 3 surface-water samples, iron concentrations were within the limits established by OSM, and manganese concentrations exceeded the regulation level only once. pH fell below the lower regulation level of 6.0 between 10% and 20% of the time. 10. The chemical quality of sediments was generally about the same after reclamation as before mining, with the exception of organic carbon, which decreased.

The U.S. Bureau of Mines subjected the CSE SR-lOO self-contained self-rescuer (SCSR) approved by the National Institute for Occupational Safety and Health (NIOSH) and the Mine Safety and Health Administration (MSHA) to a series of laboratory treatments designed to simulate various environmental conditions in underground coal mines. The tests were designed to predict the ability of the self-rescuers to withstand those environmental stresses without causing a decrease in wearer protection. The apparatus were heated to 710 C for 48 h, cooled to -450 C for 16 h, vibrated for 9 h, and dropped 1 m on each axis. A critical concern was internal damage to an apparatus, without any obvious external signs, that would cause it to malfunction or seriously degrade its performance. Carbon dioxide (C02) levels in apparatus with combined treatments were higher than in apparatus with individual treatments and untreated apparatus. The higher levels remained within safe limits, however. None of the treatments caused venting of the small oxygen cylinder that provides starter oxygen. Some bottles were found to be empty because of manufacturing defects in the burst disks. This problem was corrected in later models. The heat treatments did make the case ends more difficult to remove and the breathing bag sticky and subsequently more difficult to unfold. An apparatus that was dropped and vibrated had a broken desiccant bag that released some of its contents which compromised the seal of the relief valve. The case was dented from the drops. As with the first-generation self-rescuers, if there is visible damage to the case of the apparatus, the apparatus should be considered to be internally damaged and must be removed from service.

Attention of mine operators and engineers in the Picher zinc-lead field has been drawn increasingly in recent years to marginal ore deposits lying at greater than normal depth below surface in the marked structural depressions known as the Miami and Bendelari troughs. Not only mining but prospect and development drilling is relatively costly in the bottoms of the structural depressions known as the. Miami and Bendelari troughs which cross the center of the Picher field. Nevertheless, operations by various mining companies in recent years have shown that no insuperable problems exist, and with the gradual lowering of grade cut-offs throughout the field, certain potential deep-ore areas, previously only partly explored seemed worthy of further investigation. Some such areas were called to the attention of Bureau of Mines engineers by lease holders and operators, while others were noted in the course of Bureau&apos;s systematic investigation of Tri-State ore reserves. The writer was assigned to make a study of. those possible "trough extensions," in the course of which all available nine maps, drill logs, and prior reports were assembled. Subsequently 41 churn drill holes were bored by the Bureau in the periods October 1944 to May 1945 and October 1946 to April 1947. The general area under investigation this project is indicated on figure 1, which likewise shows its relation to nearby towns, roads, and railroads. The location of individual deposits included in the report is indicated more clearly on figure 3. Minable ore was found in only two holes, and the structural information procured by drilling was less encouraging than anticipated. This report, however, presents all factual data obtained, in the belief that they may be useful in subsequent prospecting and development projects in the areas investigated. There are also set forth some of the stratigraphic data relative to the Picher field ns a whole and the Miami and Bendelari troughs which were assembled in the preliminary investigation of this project.
