The platinum mining industry has experienced a significant increase in fatal accidents. Mine accidents are in principle preventable, and there is enormous pressure on employers to reverse this trend. This paper reviews safety management systems (SMS) as well as behaviour-based safety (BBS). Based on the review, SWOT analyses are performed on the SMS and BBS processes and a model of an integrated safety management system (ISMS) is introduced. A number of fatal accidents is analysed to identify factors that contributed to accidents in the mining industry. Thereafter the potential weaknesses of an SMS of a platinum mine are exposed and the potential benefits of implementing the ISMS for this case study are demonstrated.

'Increased underground extraction of coal' was the theme selected for the 1981 mining vacation school, which was held at the University of the Witwatersrand in January and February. The response of the industry to the invitation to register was so good that three separate schools of one week each were arranged between 26th January and 13th February. In terms of the total number of delegates, some 225 actually attending, the school can be described as the most successful ever organized by the Institute.

This paper briefly describes the excavation procedure of a powerhouse, comprising of its incremental excavation, reinforcement measures, and the installed monitoring system. During the first phase of excavation, the data obtained from monitoring of the behavior of Mudstone layers were used to recommend extra support measures. These data are also used to design the support of the powerhouse during its extension phase. Historically, mine ventilation practices as it was referred to then and which was enforced by legislation of those days, focused primarily on the conditions in the working place and little attention given to actual exposures of workers to contaminants in the working environments. With the advent of the Leon Commission of Inquiry into Safety and Health in the South African Mining industry in 1994, findings in the report indicated that more attention should be given to occupational health of workers. The upshot of this report resulted in the promulgation of the new Mine Health and Safety Act (MHSA) with certain sections of it devoted to all the aspects of occupational medicine and occupational hygiene. This paper describes the present situation with regards to statistics on occupational hygiene performance and compensation paid out to persons who have contracted occupational diseases, as well as the way forward in terms of legislation implemented in September 2002 on the chronic and acute issues affecting the mining industry in SA.

Following study of historical South African practice and a review of recent experience gained during the design, construction and testing of the high pressure plugs at the South Deep Water Barrier Project, this paper makes recommendations to improve practice. Initially, site investigation requirements are described with reference to geological, hydrogeological and seismic considerations, including site preparation and pre-grouting of the surrounding rock mass, if permeable. Structural and hydraulic aspects of plug design address topics such as residual permeability, aggressive mine water, plug materials, mortar strength and bleed. In situ thermocouples and piezometers are recommended to optimize the timing of plug tightening and monitor plug performance during service. Plug construction covers aggregate preparation, mortar quality control, plug tightening grouts, injection pressures, records and the overriding need for a method statement specification to control all aspects of plug construction. On completion of the plug, tests to establish surrounding rock stress regime, plug strength, and residual permeability are described to enable a comparison with original design assumptions. Monitoring and maintenance procedures are proposed to ensure satisfactory long-term performance of the plugs.

Abosso Goldfields Limited (AGL) is in the process of developing an expansion of their Damang Open Pit gold mine in Ghana. AGL acquired the Damang mine in January 2002, with SRK participating in the geotechnical investigation and design for the project since 2001. A deep resource drilling program, completed during 2004, intersected an extension of the orebody below the then planned final pit floor. A feasibility study has been completed to determine the economic extraction of this deep ore with an expanded pit shell. The Damang mine is in a stockwork sulphide deposit at the culmination of the Damang anticline. The deposit occurs as hydrothermal mineralisation associated with dominantly east dipping thrusts and sub-horizontal quartz veins. Primary gold mineralisation also occurs in the conglomerates of the Tarkwaian Formation. The geological units consist of steep easterly dipping dolerite, sandstone, siltstone and phyllite beds, with the weathering profile comprising saprolites overlying a relatively thin joint oxidized horizon. It is planned to deepen the current open pit to a final depth of up to 270 m, primarily by the development of a push-back of the eastern and western walls. The edge of the Eastern Tailings Storage Facility (ETSF) is currently located within 80 to 100 m of the crest of the existing pit. The geotechnical design focused on the optimization of the bench face angle and finally recommended a ?Steep Slope Strategy?, involving 24m high near vertical benches. Limiting bench stack heights of 96m at a maximum of 66° were derived from the influence of major structures and the variation in rock mass conditions. The overall slope angles in each pit sector were developed around the requirement for haulage ramps. One of the main design challenges was to optimise the standoff distance between the final pit crest and the ETSF. This process had to consider maximum access to the resource without undue influence on or from the nearby tailings dam. A planned increase to the height of the dam wall was an additional factor in the design. Dewatering of the pit walls was also a major consideration in this tropical environment.

ABSTRACT: Hydraulic experiments have been conducted since 1997 within a small fractured calcareous rock mass in southeastern France, at a site called Coaraze. On this reduced-scale site closing a floodgate located on a downstream spring produces repeated hydraulic loadings. Simultaneous measurements of normal deformations and pressure in several joints have provided a better understanding of the hydromechanical behaviour of such a fractured rock mass. The present paper first describes both the site and conceptual interpretation of the measurements performed. We showed that accurate measurement reproduction requires incorporating certain features: a variable normal joint stiffness, the joint vicinity (through introducing a Z coefficient), and the initial stress state. The paper then develops an original analytical interpretation of the coupled pressure-displacement measurements carried out, which reveal the high level of measurement suitability to the non-linear behaviours examined. 2D modelling, using the discontinuous code (UDEC) was used to proceed with the calibration step. In diesem kleinen Standort, werden wiederholte Wasserdruck Erhöhungen durch das Schliessen eines Hahnes produziert. Gleichzeitiger Druck und normale Verformung Meßungen in mehreren Fugen haben erlaubt, das hydromechanische Verhalten einer solche gebrochene Felsenmasse besser zu verstehen. Der Artikel beschreibt zuerst den Standort und stellt die begrifflichen Interpretationen der Meßungen vor. Um die Meßungen gut zu reproduzieren ist es wesentlich, bestimmte Faktoren wie die veränderliche normale Steifheit der Fugen, die Nachbarschaft der Fugen (über einen Z-Koeffizient) und der Spannungstand zu berücksichtigen. Der Artikel erklärt die originale Methode für die Druck- und Verformungs-Meßungen Interpretation und zeigt die gute Korrelation der Meßungen mit dem nicht-linearen angenommen Verhaltens der Fugen. Die numerische 2D-Modellierung mit dem UDEC Software ist für die Verkeilung der Faktoren gebraucht worden. Dans ce site de taille réduite des mises en charge répétées sont produites par la fermeture d'une vanne située à l'aval du site. Des mesures simultanées de déformation normale et de pression dans plusieurs joints ont permis de mieux appréhender le comportement d'un tel massif fracturé. L'article décrit dans un premier temps le site et présente les interprétations conceptuelles des mesures. Pour bien reproduire les mesures, Il est essentiel de prendre en compte certains facteurs tels que la raideur normale variable du joint, le voisinage du joint (par l'intermédiaire d'un coefficient Z), l'état de contrainte initial. L'article précise la méthode originale d'interprétation des mesures pression-déplacements qui montre la bonne adéquation aux lois de comportement non linéaire proposées. Namakwa Sands aims to conserve environmental resources and prevent or minimize environmental impacts from its operations. The challenge faced by Namakwa Sands is the rehabilitation of the arid winter rainfall Strandveld and lowland Succulent Karoo vegetation zones affected by its mining activities. Extensive experimentation with different rehabilitation methods resulted in the development of an effective rehabilitation programme. The use of topsoil as a seed bank and shade-cloth as windbreaks have significantly aided the rehabilitation process. The aim of the programme is to stabilize the soil environment so as to allow the germination and development of the local plant species. The ability to control wind erosion is critical to the success of the rehabilitation effort. A programme of biodiversity audits has been started to measure the recovery of plant species in the rehabilitated area. The principle of continuous improvement, coupled to site-specific rehabilitation studies, is applied to enhance the rehabilitation programme.

Strategic and tactical planning are both required in the mining industry but have completely different objectives and require different environments. If these are not fully understood, then the best value for a mining operation may not be realized. This paper examines the differences in these approaches and how they interface in the mining environment. The objectives, content, skills required and the implied management issues for these two components are examined with the hope that this will assist management and the planning teams in defining their respective roles and expectations. The opinions are those of the author based on his experience on several mining operations and projects.

Ongoing rockpass problems on mines within the South Africa region, prompted AngloGold Ashanti to embark on a risk assessment of major rockpasses. This commenced with the collection of relevant data to identify gaps in the available knowledge and identify existing problems. This was followed by an assessment of rockpass layouts to determine existing and potential interactions with other excavations. An initial risk assessment has been completed and the rockpasses have been ranked in terms of their risk profile. The lack of geological data and an easy and reliable method of surveying rockpasses have been identified as problem areas. Shortcoming is terms of rockpass management procedures have also been identified. Further work is required to quantify the risk in terms of probability of occurrence and consequences.

The paper describes the construction of 5 plugs to resist high water heads at depth within the South Deep Gold Mine, Randfontein, South Africa. A total of 20 segments were involved to create these plugs, which were 7.5 m long at the 58 Level and 6.25 m long at the50 Level. Emphasis is placed on the QA/QC and verification methods, which were developed and governed all phases of the work, from initial rock preparation and pregrouting through to the final grout tightening of the rock/plug interface and the surrounding rock. This paper is one in a series of papers detailing other aspects of the project, which was constructed largely between May 2001 and August 2003.

During mining, the stress in the coal face varies, and, at the same time, the gas flows from the coal body into the excavation, which will cause the gas pressure to change continuously. Assuming the coal gas content of the coal to be constant, and applying the law of energy conservation, the influence of stress concentration on the gas pressure was analysed. It was found that the stress is proportional to the gas pressure. Finally, as a result of analysis and the law of gas flow, the gas pressure dynamic distribution ahead of tunnelling was modelled, and the mechanism by which blasting induces outburst was explained by making use of the model. The inclination of any pit wall composed of stacks of benches is limited by the overall quality and structural complexity of the rock mass in which the slope is situated. For a bench stack angle that satisfies the stability criteria, various combinations of bench height, bench face angle and berm width can be used in the slope design. The bench face angle and berm width geometries must be chosen to fit the bench stack angle but must provide a berm that will be capable of catching the volume of failed wedge material from the bench faces above. Other considerations for bench height and berm width are equipment size, ore selectivity and blasting design. This paper studies the effects of different slope configurations on slope performance as it relates to wedge stability, volume of rock safely contained on berms, and rockfall. Optimisation of the geometry is carried out based on the need to minimise the volume of failed material which limits the effectiveness of the berm. This will reduce the number of rocks that can continue to fall further, reaching geotechnical safety berms or pit ramps, that put safety of personnel and equipment at risk. For general study it is most effective to investigate and compare the performances of each of a wide range of geometrical combinations of bench height, bench face angles and berm widths that fit a variety of different bench stack angles. These analyses were conducted for a predetermined bench stack height and three different bench stack angles.

Mud rushes are hazardous occurrences that have occurred frequently in mines in South Africa, and have been the cause of numerous fatalities in the past. Mud rush incidents not associated with accidents are believed to be common occurrences that are not generally reported. Many such incidents are probably associated with the operation of chutes and box fronts. This paper describes the conditions necessary for mud rush occurrences, postulated mud rush mechanisms, and methods of combating the mud rush hazard.

The demand for rare earth elements (REE) is increasing rapidly owing to the emergence of new cleanenergy and defense-related technologies. China’s dominates the world production of REE. In this paper, we review the ore characteristics of four typical REE deposits in China and discuss the conventional beneficiation techniques, including gravity and magnetic separation, flotation, and hydrometallurgical processing of these ores. Some of the latest laboratory findings and industrial applications, as well as the development of novel reagents for REE processing in China are presented. The REE beneficiation technologies developed and employed in China could constitute a good case study for many other emerging projects being explored around the world.

This paper addresses the methodology of introducing risk into the development of mine plans for open pit mines. Recommendations are put forward for achieving a risk balance between mineral resource estimates and slope design. A design process identifying criteria for different stages of planning is presented. Business risk is separated from geological/geotechnical risk and procedures to address these within the development of mine plans are suggested. It is concluded that a formal design process improves communication between key decision makers and adds value to shareholders, investment.

ABSRACT: In this paper numerical simulation of jointed block mass behaviour obtained in an experimental study has been presented. Singh et al. (2002) conducted extensive tests on the jointed model materials with several joint configurations. The results of this experimental study were used to validate the strength behaviour of numerical block mass model developed using professionally available software. This software basically contains different kinds of material models. From the results of numerical simulation the existence of strength anisotropy was observed. The strength anisotropy so obtained has been compared with the experimental values reported by Singh et al. (2002). The anisotropic curves in both cases are U-shaped with flat base. They show good agreement in sliding mode of failure zone in which inclination ß (= 90° - ?) of joint set-I lie between 20° to 40° for all cases of interlocking. ZUSAMMENFASSUNG: In dieser numerischen Papiersimulation des verbundenen Blockmasse Verhaltens, das in einer experimentellen Studie erhalten wird, ist dargestellt worden. Singh et al. (2002) leiteten umfangreiche Tests auf den verbundenen vorbildlichen Materialien mit einigen gemeinsamen Konfigurationen. Die Resultate dieser experimentellen Studie wurden verwendet, um das Stärke Verhalten des numerischen Blockmasse Modells zu validieren, das mit professionell vorhandener Software entwickelt wurde.
