"Tunnel safety is evolving as science, engineering and technology make advances. Applicable safety codes and standards simply do not cover all the potentials of design, construction and operation. This paper reviews a series of tunnel projects code gaps in Seattle USA that were uncovered during the design and construction of a new highway tunnel, conversion of bus tunnels to combined road and rail tunnels, new rail tunnels using TBM and NATM, conversion of a road tunnel to light rail, and retrofit of an existing road tunnel to add lanes and improve safety systems. The perspectives of the ‘fire code official’ (Authority Having Jurisdiction) on methods to identify and resolve gaps in safety code and standard regulations, while garnering the consensus of the owner, designer, and contractors are presented. Additionally, the emergency responder command perspective on safe tunnel design, tunnel rescue during construction and operational safety is explained TUNNEL SAFETY EVOLUTION The underground has become nearly the only option to provide essential transportation corridors to continue our economic growth in key global areas. Tunnels are not built on a whim. Often significant portion of a regions wealth are invested to meet current, and hopefully future transportation demands. There are more transportation tunnels being built now than at any time in the planets history. The location of these tunnels is often selected for us based upon regional demand. Most commonly under densely populated urban centers and between populations’ centers to bypass geologic conditions like mountains and water bodies. Tunnels are being built in unforgiving soils, between tectonic plates, in extremes environmental conditions and often, under the very heart of the highest density population centers on the planet. All of these locations can bring with it conditions that challenge our common assumptions about safety. These extraordinary locations and soils create risks for both those constructing the tunnel, and in design engineering to meet the ‘minimum’ requirements at the lowest costs. There is lengthy history of above ground safety. Most countries and major local jurisdictions have adopted, or created safety requirements for the construction of above ground buildings. Through decades of use, these safety requirements have evolved, changed and been expanded to cover a wide gamut of actual and potential safety risks. However, the use of the underground for tunnel transportation corridors has grown exponentially with limited attention to safety requirements."

Energy Developments Ltd. (EDL) has developed power plants at two Australian coal mines that featuring 94 gas-fueled Caterpillar G3516 engine/generator sets. They will convert to electricity 145 kt/a (160,000 stpy) of coal-seam methane gas for sale to a local utility. The $100-million project will cut the sites' methane emissions in half, provide a low-cost energy source and offset mine operating costs. The project began in May 1995. It is located at the Appin and Tower longwall underground mines, 80 km (50 miles) south of Sydney in New South Wales. The installation is one of the first to capture coal-seam gas from mine ventilation air and gas drainage systems. Coal- seam gas contains 40% to 60% methane, which fluctuates constantly. The system is expected to convert 578,000 m3/d (20.4 million cu ft/d) of coal-seam gas to 94 MW. It will also reduce greenhouse gas emissions by 3.5 Mt/a (3.8 million stpy) of CO, equivalent emissions. The Appin and Tower mines each produce 2 Mtla (2.2 million stpy) of clean coking coal. They operate around the clock with about 400 workers per site. The mines are owned by BHP Steel, Collieries Division, which will share the revenues from energy sales. The Appin Power Partnership consists of EDL and Lend Lease Development Corp.

We in the coal industry, especially in the State of Illinois, put tremendous emphasis, including time, money and effort, into on-the-job training. The results of such training and safety have been tremendous. For example, in 1981 Illinois was the first major coal producing state in the history of coal mining to have zero mine-related fatalities. And in 1985, I am very proud to say, we had the second best record in the history of coal mining, with only two fatalities. Obviously, one fatality is one too many. Our goal is, has been and will continue to be to strive for excellence. It is sad, however, that in 1981 while were were celebrating zero fatalities in the workplace, there were 21 fatalities that occurred off the job. Thus, it has become a classic American story. Success with safety has been found in the workplace, but training and preparation to prevent accidents away from the job site have almost been ignored. Accidents are the leading causes of death in the United States for all persons age 1 thru 38, and are the fourth leading cause of death for people of all ages. Every single day, on the average, more than 290 Americans are killed in accidents - a rate of one every five minutes. The leading cause of fatal accidents in the U.S. is motor vehicles. And the number two cause is home accidents. A staggering statistic is that people are being killed off the job at a rate of about 9 to 1 over those on the job.

The Davis magnetic tube concentrator is a laboratory machine designed to separate a small sample of pulverized magnetic iron ore into magnetic and non-magnetic fractions. It was developed in 1921 by mechanization of a test previously performed manually. (1)(2)(3)(1) No significant changes have been made in the design of the apparatus since that time. The Davis magnetic tube test continues to serve as a practical basis for judging the amenability of an ore to magnetic separation and for controlling magnetic separation plant operations. The apparatus consists of an inclined cylindrical glass tube supported adjacent to closely spaced pointed poles of a powerful C-magnet. (Figure 1). To conduct a tube test, a sample of a suitably prepared ore is poured into the water-filled tube. With wash water flowing through, the tube is oscillated to gradually reject the weakly magnetic ore particles until the desired degree of separation is attained. Both the magnetic concentrate and the tailings may be recovered for examination.

A gold and silver open-pit mine located in northwestern Chile contracted ACZ in 1991-92 to assess the stability of their spent ore pile and to determine the cause and magnitude of cyanide contamination associated with leaching operations. The mine is a conventional open-pit located at an elevation of approximately 4,800 meters (m.) (15,744 feet). A spent ore pile is located approximately 2,000 m. southwest of the mine and approximately 1,000 m. northeast of the active pad and plant facilities. The pile was constructed on a gentle northwest slope dipping 12 to 13 degrees. The pile is bordered by a small river to the northwest and by a larger river to the northeast. Both rivers eventually flow into Bolivia. Heap leaching and processing commenced at the mine in 1988. Since 1988, nearly 6.0 million tons of ore at an average grade of 1.96 grams/ton (0.07 ounces/ton) of gold and 52 grams/ton (1.83 ounces/ton) of silver have been treated by cyanide heap leach methods with zinc (Merrill Crowe) precipitation. In approximately three years of operation, the mine has yielded more than 225,000 ounces of gold and over 1.5 million ounces of silver. Currently ore is agglomerated and hauled to the leach pad by truck and is top loaded to produce a 5-meter lift height. Four contiguous reusable (on/off) leaching pads were constructed with an asphalt layer above a carefully compacted sub-base of High Density Polyethylene (HDPE) liner and screened fines. Leach solution of pH 10-11 containing 1.2 to 1.5 gram/liter of cyanide is applied to the heap at a rate of 10 liter/hour/square meter for a total active application period of 30 days. Leached (spent) ore is transferred to the spent ore pile located approximately 1,000 meters to the northeast. The pile currently contains approximately 6 million tons of spent ore. The pile liner, which has been constructed over a prepared sub-grade consists of three geosynthetic layers (geotextile/flexible membrane liner/geotextile). Long term objectives include washing the pile to recover an additional 3 to 4 percent gold and silver.

Many underground instabilities, such as excessive shield loading and roof falls, occur at longwall faces causing safety problems and production losses. Such instability problems are directly related to roof conditions which can be categorized using geological information. . However, most geologic information describing anomalies and variations of immediate and main roofs are not available before mining resulting in unexpected problems in a longwall face. It is apparent that the evaluation of these geologic anomalies provides a great benefit for longwall stability. The purpose of this study is to evaluate roof categories associated with geological anomalies employing fuzzy set theory. The evaluation is based on real-time monitoring information, such as shield leg pressure, developing load on shields, intervals of periodic weighting, and the ratio of developing loads during periodic to normal times. This paper introduces the application of fuzzy set theory for characterizing roof conditions and evaluating of roof similarities.

Effective mine operation demands that the right people have the right information at the right time. Caterpillar's Mining and Earthmoving Technology System (METS) meets this demand by providing "real-time" information transfer between the mine offices and machines equipped with the Caterpillar Computer Aided Earthmoving System (CAES). Transfers of mine plans from the mine planning software to CAES-equipped machines give operators access to current mine plans within minutes of their creation. Additionally, office personnel have access to a current digital terrain model (DTM) of the mine site as it is updated in "real-time" by CAES-equipped machines. METS provides mine personnel in operations, planning, and management the information they need when they need it.

The Rowland Coal Preparation Plant is located on Workman's Creek in Raleigh County, West Virginia, approximately 20 miles Northwest of Beckley and 10 miles Southeast of Ameagle. It is owned and operated by the Rowland Division of Consolidation Coal Company. This paper describes the plant flowsheet, process considerations that were evaluated in its development, and observations made during and since the plant was constructed. The final flowsheet was developed specifically to wash the coal from the Rowland Mine and contains several equipment combinations and applications heretofore never placed in a single, commercial operation. The major process equipment is presented in a functional description of the flowsheet. The coal reserves are leased from the Rowland Land Company and consist primarily of #2 Gas and Middle Eagle coals and various major splits of the Middle Eagle seam. The Rowland Coal Preparation Plant was designed to process 560 TPH of raw coal consisting of 50 percent #2 Gas coal intimately mixed with 50 percent Middle Eagle coal and containing lumps no larger than a 12-inch cube.

The Freeport Sulphur Company operates four sulphur mines in the coastal marsh area-03 South Louisiana. Employing the Frasch process of mining, approximately 13-million gallons of water per day is heated to a mining temperature of 325°F and is injected into the sulphur bearing formations of the four mines. A volume of cold waste water, roughly equal to the Input, is removed from the formations and presents a disposal problem. Two additional mines are currently being developed and will increase the disposal volume by an additional 5-million gallons per day. Although these properties are in a relatively small area, geographically speaking, as shown in Figure 1, they each represent a separate disposal problem with the treatment methods designed for the individual mine.

Since 1980, more than 40 miners have lost their lives by being trapped and suffocating in stock-piles, bins, silos, and similar facilities. Mine Safety and Health Administration (MSHA) is concerned about these accidents, most of which could be prevented if management and miners appreciated the hazard. In February 1986, the coal industry had a suffocation accident that drew national attention. A group of engineers were standing on a coal stockpile. It gave way under their feet. Five of the engineers were buried in the coal and suffocated. As with all fatal accidents, MSHA investigated the cause of this tragedy. I also asked my staff to look into the history of similar accidents. I found out it is rare to have multiple fatalities due to suffocation in material. But, over the years, there has been a long series of individual fatalities from this type of accident. Here are some examples. In Oct. 1985, a coal mine employee in Virginia entered a hopper to collect a coal sample. The feeders were started while he was standing in the hopper. He sank into the coal and suffocated. In Sept. 1985, an employee at a Texas sand company climbed onto a surge pile, to check a problem with sand flow. When other employees found him, the sand had swallowed him. Only his face and one hand were showing. Though the victim's face was clear of the sand, he suffocated before rescuers could free him. Many people do not realize you can suffocate in this kind of situation, though your head is free. Every time you exhale, the material has room to shift and press harder against your chest. Then you have less room to inhale. If this goes on long enough, it gets to the point where you cannot inhale at all. That is what happened to the victim in this accident. In May 1985, a plant operator at a Wisconsin sand and gravel operation purposely slid into the drawhole of a surge pile. He tried to break up large rocks that were clogging the drawpoint. Material piled on the side of the surge pile slid down and buried him. The operator was wearing a safety belt and line. But he was covered by 3 to 3.7 m (10 to 12 ft) of material and could not be pulled free. This incident illustrates another point. A safety line can help. But it is not enough. Workers should never put themselves in a position where loose material can fall on them from above. In Jan. 1985, a young Utah coal miner was waiting for a ride home from a co-worker whose job was at the tipple. A jam developed in the coal bin. The men could not clear the jam from the outside. So the young miner jumped into the bin. He started digging with a shovel at the toe of the coal pile. It caved in on him and he suffocated. The young miner had only three months of mining experience. He had not been trained for work around the coal bin. Since 1980, 16 coal miners and 26 nonmetal miners have lost their lives in loose-material suffocation accidents. Suffocation accidents occur in many areas around mines - bins, hoppers, stockpiles, surge piles, and silos, among others. One recent accident occurred at a surface coal mine in Illinois. The victim was walking over an area that had been drilled and blasted the week before. He fell into a void that had been crusted over with earth. He too suffocated. And many materials have been involved in suffocation accidentscoal, crushed stone, sand, salt, different kinds of ore, ammonium nitrate pellets used in blasting, and a powdery byproduct of iron ore milling called calcine. In addition to fatal accidents, there are near misses. We do not know how many. Most of them are not reported. A person who gets trapped in material, but is rescued, is usually unhurt. MSHA sometimes hears about these accidents by chance, or because they are reported in the local media. We do know that near misses are happening far more often

There are bound to be changes with age, so I should not have been surprised or disappointed when what I had considered as high ad- ventures were much fewer and more widely scattered in the last 20 years of my long and active business life. But such adventures came until I was past 80 years of age and no one could expect more than this. Frequent visits to mines in this country and Canada and directorships in Bralorne and its subsidiaries, Homestake Mining Co., Bradley Mining Co., and the Bunker Hill Co.; and Copper Range Co. and its great new White Pine mine in Michigan, lasted far beyond the usual retirement age. The least happy part of my last two decades of active life was the death or sickness of many of my old friends. Hubert d'Autremont, whose friendship no one could replace, died suddenly in 1946. Henry Carlisle and Temple Bridgeman, for many years office associates, had heart attacks that cut down their activities far too soon. Many other less intimate friends died or became ill, but I knew that with advancing years such losses were inevitable. These memoirs would be endless if they covered all of the unusual work and associations I found in the last 20 years. So I must confine the rest of my recollections to expeditions that would have thrilled any engineer at any time. A characteristic common to most tailings dam failures caused by earthquakes or excessive rain storm is that the tailings tend to liquefy and flow for substantial distances as a viscous fluid, with the potential for extensive damage to property and life. Failures of El Cobre, Chile (1965), Mochikoshi, Japan (1978), Aberfan, Wales (19661, Buffalo Creek, West Virginia (1972) are examples of such catastrophic incidents. In these four incidents more than 400 lives were lost, and the loss of property was approximately $200 million. In order to be able to assess the potential for damage in case of such a failure, it is necessary to predict the possible extent of flow slide movement. This paper discusses simple procedures for performing such analyses. Also, in order to illustrate the use of these analysis procedures in tailings dam design, two case histories are analyzed.

Eruption of rhyolitic ash-flow tuffs in the area of the McDermitt (Cordero) caldera began about 17.4 m.y. ago and continued for about 1.5 m.y. During this period of silicic eruptions, a circular area about 30 km in diameter collapsed to form a caldera that subsequently filled with sedimentary and volcanic rocks. 3 A total of more than 200 km of silicic rocks was erupted from the volcano. The final phase of igneous activity, about 15.5 m.y. ago, included resurgent doming and intrusion in the central part of the caldera. Three to 4 m.y. later +I2 m.y. ago), hydrothermal solutions using the northern part of the caldera ring fractures as conduits deposited cinnabar, pyrite, alunite, and other minerals in tuffaceous lake beds and volcanic rocks accumulated within the caldera. The ore-forming solutions may have been a residuum from the silicic magma or connate water or ground water heated by the still-hot resurgent magma in the central part of the caldera. The source of the mercury has not been identified. Later oxidation of the sulfide minerals and redistribution of the supergene minerals corderoite, several mercury nitro-oxychlorides, marcasite, iron, and titanium oxides in the sedimentary rocks account for most of the newly discovered ore body at Cordero.

The Henderson molybdenum mine, near Empire, CO, has operated since 1976 using a panel caving process and a rail haulage transportation system. After two decades of service, the rail haulage system required a substantial capital expenditure to improve operations. The timing for this worked well because Henderson was able to combine this capital need into a mine plan that addressed process bottlenecks and current mining practices as a new production level was designed and developed. This process was a logical step, following an equipment upgrade program, and became known as the Henderson 2000 project. One of the key components of Henderson 2000 is a new ore transportation system. Throughout Henderson's life, the bottleneck has been the rail haulage system. Henderson staff evaluated several transportation alternatives and decided on a truck haulage configuration feeding an underground crusher and conveyor system. This configuration will eliminate the haulage bottleneck and will allow the mine to increase capacity. The installation of this system will be completed by the year 2000. The second key component of Henderson 2000 is the development of a new 7175 production level to extract the lower reserves of the ore body. Henderson staff benchmarked other caving operations and compared their mining practices to Henderson's basic mining guidelines. It became apparent that Henderson's practices could become more efficient, particularly in the areas of drawpoint spacing and column heights. These concepts were incorporated into the layouts

Dry magnetic separation of feebly magnetic minerals and wet magnetic separators suitable for highly magnetic minerals have been commercially utilized for a number of years. The beneficiation of magnetic taconite ores by conventional, low intensity magnetic separators in combination with flotation is well established technology (1). However, over the past decade much effort has been expended in developing techniques for the upgrading of feebly magnetic oxidized iron ores (2,5). Such research and development have been stimulated by the limitations of the above-mentioned techniques in application to ores containing fine material; for example, ores requiring fine grinding for liberation or slime-forming ores such as the oxidized taconites or semi-taconites. Wet separation has allowed the reduction of the minim particle size that can be effectively treated from that allowable for high intensity dry magnetic separation. Consequently, the development of high intensity wet magnetic separation methods has received much attention in the search for an effective method to treat the weakly magnetic minerals. Recently Lawver and Hopstock (5) have provided an excellent review of the state of know- ledge of this technology. In addition, a valuable compendium of available laboratory and commercial devices is included. Nonetheless, the feasibility of the use of high intensity magnetic separation in a particular application is largly empirical in nature and requires experimentation to chazacterize the response of a particular ore. The purpose of this paper is to assess the incorporation of high intensity magnetic separation for the beneficiation of a non- sliming specular Michigan hematite. Particular attention is focused on the existence of a lower limit of particle size that can be treated effectively and is inherent in the use of these techniques. Additionally, a rationalization of various models presented in the literature is attempted.

Artificial neural networks (ANN) are of growing interest in a wide variety of applications. Pattern recognition, speech processing, financial analysis, signal processing and process control are at present the most successful ones. This paper is an attempt to illustrate the potentiality of ANN applied to mineral processing. An introduction to the technology of ANN is first proposed to describe the various components and to explain the various phases encountered in setting up a network. Then examples are given to illustrate the use of ANN dynamic modelling of mineral processing units or plants and in process control.

The Piedmont province of the southern Appalachians is becoming the focus of interest for many exploration geologists. In the past, only those deposits with significant surface exposure have been exploited. Thus, few deposits have been found and relatively little exploration has been conducted in recent years. Modern geochemical and geophysical techniques can alleviate this problem of concentrating on exposed ore bodies by allowing us to "look through" the soil and saprolite to detect the presence of significant mineralization at depth. This paper will attempt to evaluate the feasibility and effectiveness of several geo¬chemical exploration techniques in the Piedmont, of Cabarrus and Stanly Counties, North Carolina. The area chosen for the study is shown in Figure 1, lying almost wholly within the Mt. Pleasant quadrangle, North Carolina. The area is on the eastern boundary between the Charlotte belt, a zone of plutonic and intermediate grade metamorphic rocks, and the Carolina Slate belt, a series of complexly interbedded volcanic and sedimentary rocks of predominantly low grade metamorphism. The area is now considered to be a 500 million year old active, continental margin.1 As is typical of most of the piedmont, outcrop is sparse. Figure 2 is a sketch map of the general geology of the area under study, showing the Charlotte belt rocks in the northwestern quadrant and slate belt rocks, to the east. Within the area are a number of old mines and prospects, the most famous of which are the Furniss and the Phoenix mines in Cabarrus County. All are reported to have contained Au and Cu. Ag and Pb were reported from some. A summary of the deposits is given in Table 1. Data used in this study are from three Master's theses conducted at the University of North Carolina at Chapel Hill. 3,3,6 In separate studies, well waters5, B-zone soils3, and vein and float quartz6 from the area described were sampled and analyzed. Each study concluded separately that geochemical anomalies in the vicinity of the ores justify use of that specific sample type in a geo-chemical exploration program. "INTRODUCTION Conventional tailing storage and management practices typically utilize engineered facilities, such as ring dikes and valley impoundments. To reduce the environmental costs associated with disposing of mine wastes in tailing storage facilities (TSFs), several beneficial re-use strategies have been developed over the years. For example, thickened tailings have been used for paste backfill. It is common for tails to comprise up to 60% of the material’s total weight and it has been estimated that TSF volumes can be reduced by 60% when using this strategy, (Benzaazoua, et al. 2004). Other beneficial re-use strategies have produced unpaved temporary access roads, hot and cold asphalt, chip seals, self-compacting concrete, and golf course sand traps. This project sampled mine tailings from two different TSFs in Colorado and produced structural concrete that could potentially be used for commercial foundations, sidewalks, and paved roads. The tailings were substituted for the fine aggregates typically found in conventional concrete. The two main objectives of this study were to investigate the structural integrity of the produced concrete samples and the leaching of heavy metals from the resulting concrete. The ratio of tailings to fine aggregate (T/FA) was carefully studied to find if there was a correlation between the substitution rate and the structural integrity. The potential maximum substitution rate was also investigated for the produced concrete specimens. The potential of leaching mining impacted water (MIW) was evaluated by comparing the leachability of the raw mine tailings to the encapsulated tailings by using standard shake extraction prediction tests. MATERIAL AND METHODS Materials The four components that make up the concrete mix design for this project are: cement, coarse aggregate (gravel), fine aggregate (sand and mine tailings), and water. The type I/II ordinary Portland cement (OPC), gravel, and sand came from local cement and concrete companies in Denver, Colorado. Mine tailings were collected from two different TSFs in the Rocky Mountain Region, where Pride of the West TSF in Silverton, CO will be denoted as Site A. The primary metals of interest at the mill site are gold, silver, lead, copper, and zinc. The location of Site B will not be mentioned in this report due to confidentiality agreements with the mining company"

This paper presents a case study of respirable dust characterization in a thin-seam coal mine in southern West Virginia. Samples were collected in various locations within the mine and analyzed using scanning electron microscopy with energy-dispersive X-ray (SEM-EDX), thermogravimetric analysis (TGA), and Fourier transform-infrared (FT-IR) spectroscopy. The study found high non-carbonate mineral content, particularly aluminosilicates and silica, in the dust. The results indicate that fine aluminosilicate particles may interfere with SEM-EDX analysis, leading to an underestimation of coal content. The findings highlight the importance of using multiple analytical methods to accurately characterize respirable coal mine dust.

A SIMAN and CINEMA PC-based simulation model was developed to simulate and animate the truck-shovel operations of surface mines. The model is capable of simulating one mathematical dispatching strategy using linear and dynamic programming and six heuristic dispatching strategies. Data were collected from an actual operating mine through time studies and analysis of historical records. The statistical analyses were performed and appropriate theoretical distributions were fitted for various time events in the truck-shovel work cycle. The model was then applied to study overall impact of these dispatching strategies on constrained and unconstrained mine productivity. The simulation/animation model developed in this study seems to be an efficient planning tool for choosing optimal fleets and finding an 'optimal" dispatching strategy to produce an equilibrium between "the quality and quantity of production.
