In September 1855, 55-year-old Noah Spencer Kellogg and his $3 jackass named Bill discovered what was known at the time as a “blow out” of high-grade lead and silver ore. The deposit was located on the side of a North Idaho mountain high above the South Fork of the Coeur d’Alene River. The ore was later assayed at 41,141 g/t (1,200 oz/st) silver. Kellogg named his discovery the Bunker Hill Lode, after the June 17, 1775 Revolutionary War battle of Bunker Hill. A thriving pioneer settlement sprang into existence around the resulting mine’s workings and was eventually— and appropriately — named Kellogg. The peak rising hundreds of feet above the mine’s adit was named “Jack-ass Peak” after Noah Kellogg’s hairy companion.

Manganese ore bodies of Banswara Mn belt were classified into stratiform and stratabound types, based on their mode of occurrence, distribution, and their chemical characters. These ore bodies generally composed of braunite, pyrolusite, cryptomelane/ psilomelane, wad, etc. The stratiform-type Mn ore bodies are conformable with the associated host rocks (Mn phyllite, etc.) and occurred in the form of layers, bands, etc., which are variable in their shape and size. These ore bodies are also strongly folded with the enclosing host rocks. The others, which are stratabound-type Mn ore bodies, also showed an intimate association with the associated host rocks. These types of ore bodies occurred in thin layers, pockets, lenses, stringers, etc. The classified ore bodies are confined with Kalinjara formation of Aravalli Supergroup of Paleoproterozoic age. The ore bodies are structurally controlled and well associated with phyllite, schist, limestone, quartzite, etc. The stratiform-type ore bodies constitute a marginal to low Mn grade and are also characterized by lower Mn content (26.19–39.24%) along with the Mn/Fe ratio of 7.16–12.23, while the stratabound-type Mn ore bodies are rich in Mn (35.92–49.64%) along with the higher Mn/Fe ratio of 14.15–23.97. However, the poor Mn grade along with high siliceous impurities of Banswara Mn deposits is subjected to them for intermittent mining activities in the past and, consequently, did not attract the scientific and industrial community for the evaluation of these resources. Therefore, the authors attempted this study in a scientific and systematic manner (mode of occurrence/distribution and possible methods for evaluation/exploration) to evaluate the potential of existed and newer Mn deposit resources in the Banswara Mn belt. The whole findings would be very much helpful for exploration/ mining geologist along with other concerned, to completely evaluate these Mn deposits in the future.

Business development process. Throughout Cyprus Amax's early months in Russia, the company stayed focused on what was needed from a deposit for economic success. Cyprus Amax's reason for success in Russia is that all involved - its Russian partners, the workforce and expatriates - provided a focused effort that served the project as it worked through the tender process and the Russian regulations. The company developed an understanding of the regulatory process by working with solid, ethical partners. Infused into the Kubaka management structure were Russians with the knowledge to facilitate the process and a professional group of expatriates. Developing credibility and a solid rapport with the Russian mining authority was critical to the success of this venture. Identifying and gaining the trust of key Russian individuals was also critical during the early stages. Gaining their trust and building credibility was part of the process. These key Russians were brought to the United States to visit existing Cyprus Amax mines. These efforts proved valuable in building an understanding with the Russians about the kind of company Cyprus Amax is and the kind of efficient operations that it can run. This was a learning process for all parties. Fortunately, the company was dealing with intelligent and educated people.

Introduction 1300 BC, Egypt. Pharaoh, the god-king, owned all things. He was the only mine operator. As the provider of all things, Pharaoh had great expectations of his officials who gathered the wealth. Pharaoh's official, the mine foreman, was at a gold mine site to see that royal expectations were met. For the official, it could mean a promotion to the good life here and to the godly life hereafter. When he checked the haul for sufficient progress, a lot was at stake. The miner wore a loincloth, perhaps a headband and, if he was a prisoner, ankle manacles. Only an oil lamp helped illuminate the hot, dusty blackness. A fire at the base of the quartz ore face competed for scarce air. The ore so heated crumbled at the prompting of copper wedges. Confined to a crouch, the miner tossed chunks of ore onto a rope-mesh which, when loaded, was drawn up and lugged out. On the surface, the gold was ground to dust. Then it was transported by donkey caravan to the royal depot. There it was weighed, recorded, and distributed to workshops. Many minerals mined Egypt had gold mines to the south in Nubia and to the east in the desert and Sinai. Indeed, gold underwrote Egypt's prosperity. With a constant gold supply, fewer hungry hands robbed burial crypts and tombs. Gold was sacred, "the flesh of the gods." The shiny metal financed the army that policed the desert mining routes and guarded the gold caravans from Bedouin marauders. Gold theft was an offense to the gods. Anyone caught with gold `in his lunchpail,' so to speak, could say goodbye to life, both in this world and the next. In addition to gold, Egypt possessed other mined riches that allowed the Egyptian civilization to flourish. From Sinai and Nubia came copper. So abundant was the red metal that it enabled Egypt to become the supreme power, before the advent of iron. Also mined were amethyst, turquoise, feldspar, jasper, carnelian, and garnet. These were used for the rich inlay work that distinguished Egyptian jewelry and cloisonne. But Egypt's most endurable and awesome material was its stonework - for statues and obelisks and in temples, tombs, and pyramids. Stone quarrying was a vast enterprise. One expedition boasted nearly 10,000 men. These included 5000 laborer soldiers, 130 skilled quarrymen and stonecutters, and - egads! - even 20 scribes. In addition, there were thousands of officials, priests, and officers grooms. There were even fishermen, to provide the multitudes with the catch of the day. Mining methods detailed In 1300 BC, quarrying techniques had changed little since the age of the pyramids some 1300 years before. At that time, in 2600 BC, limestone was locally quarried and fashioned into the blocks of the pyramids. A basic limestone mining method was tunnel quarrying. A ramp was built up to the face of a cliff. A monkey stage was then erected on a ramp. While standing on the stage, quarrymen carved out a rectangular niche in the cliff. The niche was large enough for a quarryman to crawl into. With a wooden mallet, he hammered long copper chisels along the edges of the niche floor to free up the back and sides of the block. The quarryman climbed out of the niche and removed the stage. He then carved out a series of holes in the cliff face for what would be the bottom of the block. The quarryman pounded wooden wedges into the holes. He watered the wedges until they were soaked. The water-logged wedges expanded, splitting the stone along the line of holes. The freed-up block was then levered down from the cliff. On the ground, the blocks were placed on sledges. Men pulled these to nearby water transport. Without block and tackle pulleys, paved roads, and wheels, this was no mean feat. Each block weighed an average of 2.3 t (2.5 st). Whenever possible, the quarrying was done directly from the surface. The grinding mill is probably the most common unit operation in mineral processing. There is, however, much that can still be done in the way of optimization. This paper discusses the results of a well-controlled pilot-scale test in which the performance of a tumbling mill equipped with a conventional rubber lining and lifter bars was compared to a tumbling mill equipped with a smooth rubber lining of the magnetic type. The results indicate that the smooth lining is about 14% more effective with respect to energy consumption. By optimizing the size of the grinding media, the mill capacity can be increased in many grinding operations. In this specific case, grinding with a -20-mm ball charge required 26% less grinding energy than grinding with a -30-mm ball charge. The energy savings can, in most cases, be converted to an increase in capacity. The wear of the grinding media is difficult to assess over a short test period, but the results suggest that the wear is somewhat less when a smooth lining is used. A determination of the wear of the linings was not included in this study, but it is generally accepted that the use of magnetic linings reduces the lining costs dramatically. Computer simulation using the Millsoft code developed by Mishra et al. confirms and explains the increased effectiveness of comminution when a smooth lining and smaller grinding balls are used.

"The reduction behavior of low-grade lump hematite ore (100% 20-50 mm) has been evaluated by performing a series of coal-based direct reduction roasting tests followed by magnetic separation under different process conditions such as roasting time, temperature, reductant type and dosage, and grinding stage. To investigate mineralogical transformation and reduction characteristics of lump hematite ore, roasted ores and magnetic concentrates were analyzed by X-ray diffraction (XRD), scanning electron microscopy (SEM) with energy dispersive spectrometer (EDS) and polarizing microscope. The results have shown that a magnetic concentrate with a total iron (TFe) grade of 90.7% was produced at an iron recovery of 83.9% when lump hematite ore was reduced at 1,200° C for five hours using 35% lignite as a reductant followed by two stages of grinding, for 20 min and 30 min each, and low-intensity magnetic separation. The degree of reduction is closely related to the change in the internal structure of the roasted ore. Lignite with a higher fixed carbon content and lower ash generated roasted ore with a more highly porous structure, which provides better permeability of reducing gas. Magnetite, fayalite and wustite were the dominant intermediates of coal-based direct reduction of lump hematite ore. Parts of wustite reacted with SiO2 or Al2O3 in gangue and generated fayalite or hercynite. FeO in fayalite was replaced by CaO to produce wustite, which was finally reduced to metallic iron. However, Fe in fayalite and hercynite produced with low contents of CaO cannot be reduced into metallic iron. Introduction Low-grade hematite ore, characterized by low Fe content and high SiO2 content, cannot be treated by traditional mineral processing methods due to its nonmagnetic property and fine liberation size. The direct reduction (DR) roasting process has been studied and proven satisfactory in treating difficult-toprocess ores, such as low-grade nickel laterite oxide ore (O’Connor et al., 2006; Cao et al., 2010; Guo et al., 2011), high phosphorus oolitic hematite ore (Li et al., 2012; Yu et al., 2013), low-grade hematite ore (Wei et al., 2013;) and iron ore tailings (Li et al., 2010), which greatly expand the resources of usable iron ores (Srivastava and Kawatra, 2009). Therefore, the use of a DR process to treat low-grade hematite ore to produce direct reduced iron (DRI) is of great interest to many researchers of mineral processing and metallurgy (Weissberger et al., 1986)."

A ventilation research study was conducted by the National Institute for Occupational Safety and Health and a cooperating trona mine in the Green River basin of Wyoming, USA. The mine operation uses the longwall mining method in trona bed 17, a commonly mined unit in the region. The longwall face length is 228 m (750 ft), and caving on the face occurred up to the back of the longwall shields. The mine is ventilated using a main blowing fan and a bleeder shaft. For this study, sulfur hexafluoride (SF6) tracer gas was released in two separate monitoring experiments. For the first experiment, tracer gas was released on the face, this test focused on airflow along the longwall face of the active panel. Face test showed the airflow patterns to be more complex than just head-to-tail flow in the main ventilation air stream on the active panel. For the second experiment, tracer gas was released 2 crosscuts inby the face on the headgate side, this test focused on gas transport in the mined-out portion of the same active panel. Gob test showed a pathway of movement through the front of the active panel gob that moved outby from the tailgate corner. The primary pathway of tracer gas movement in the active panel gob was towards the headgate and tailgate bleeders and out of a bleeder shaft. The rate of movement towards the back of the gob was measured to be 0.19 m/s (37 fpm).

The continuous need for infrastructure expansion forces the mining industry to develop underground structures into more sensitive and violent environments. When underground excavation enters into deep and high stress rock conditions, geotechnical designers find themselves faced with the difficult task of selecting a rock support system that will meet today's stringent requirements. Whatever support system is chosen, it is mandatory that potential rock movement be controlled. All this has intensified the development of safety systems and new, unique solutions which can enable construction companies to go both, further and deeper, without reducing the level of safety. To address these needs, Atlas Copco developed the Roofex® bolt, specifically designed to withstand large static and dynamic deformations under prespecified load conditions. The Roofex Rock Bolt acts as stiff support up to a designable load level before the sliding effect of the mechanical frictional system is triggered and allows highly controlled rock mass movement without loosing the support function.

High horizontal stresses were recognized to impact roof stability more than 60 years ago. Since then, numerous measurements associated high horizontal stresses with difficult ground conditions. This paper presents case histories illustrating the practical usage of roof stress determinations for helping assess stability, not only in the case of high horizontal stresses but also of low stresses. Examples are given of high stresses associated with faults, mine design changes, quantification of stress shadow effect, and anistrophy. The paper concludes with a comparative evaluation on the effect of various stress fields on ground support requirements.

In addition to capital and operating costs, the accurate estimation of ore reserves is a key component to the successful outcome of any feasibility study. In recent years, there have been many project failures caused by the improper estimation of ore reserves. The accurate estimation of an ore reserve relies heavily on an understanding of the geology and the underlying assumptions unique to the estimation techniques being used. These assumptions are reviewed, and the shortcomings of the estimation techniques and their possible impact on a given project's economics are discussed. The latest developments in geostatistics in overcoming existing reserve-estimation problems are presented.

This paper is based upon field studies conducted in a western Canadian open pit coal mine. This work has involved the adaptation of microprocessor-based instrumentation to monitor various operating parameters on-board blasthole drills and mining shovels. A review of instrumentation and monitoring methodology will first be presented. Based upon experience from field-based studies, the paper will subsequently focus on the application of monitoring technology within the framework of an integrated Equipment Monitoring System. Such a system will permit the real-time performance evaluation of key equipment involved in surface mille production.

The milling plant of Bicroft Uranium Mines Limited has been in operation for a period of 2 years. During this time many changes, both physical and chemical, have been made in an effort to improve plant efficiency. Investigational work has been facilitated greatly by the fact that the Bicroft mill consists to a large extent of two parallel circuits; however, the number of inter-related variables is great, and it is difficult to measure, let alone control, all of them simultaneously. The Bicroft mill staff feel that much progress has been made, but there is much yet to be done and many questions still to be answered. The property of Bicroft Uranium Mines is located in Cardiff Township in the Highlands of Haliburton County, southeastern Ontario, some 140 miles northeast of the city of Toronto and 70 miles due north of Lake Ontario. The ore-bearing dikes are classed as mafic, non-segregated, syenite and granite pegmatites. Microline, microline perthite, quartz, uraninite, uranotharite, fluorite, titanite, cyrolite, calcite, pyrite, pyrrhotite and molybdenite as important accessories. The mill was designed for a nominal capacity of 1,000 tpd with all sections except grinding, leaching and clarification capable of 1,500 tons. Feed was entered to one mill unit on October 29, 1956 and to the second parallel line on November 13. The total feed rate was at 1,000 tpd on November 15. The first precipitate was produced on November 12. By August 1958 the average entry had been increased to 1,410 tpd without the installation of any additional major equipment. The overall recovery has been greater than 93,0 pet do several months, but a recovery of 92.5 pct appears to be most economical. Operating time for 8 months in 1958 has been 97.1 pet although all grinding mills were relined during this period.

This paper is in three parts. Part I discusses the various types of cement clinker coolers now in full-scale operation. Part II is a detailed study of the criteria for selection of the type of cement clinker cooler best suited for a given installation. Part III covers a number of cooling problems and their solutions in some of the metals industries. The large and world-wide problem of cement clinker cooling has motivated much study and development and the coverage here of this work is somewhat detailed. In contrast the cooling problems of the metals industries are quite varied between industries and often quite different from plant to plant in the same industry because of basic differences in the requirements of the particular material being processed. Therefore the coverage of metals industries coolers is somewhat more general. It is believed that all of the technology covered here may be of interest to those working with the problems of cooling hot materials.

In considering slurry transportation for Consolidation Coal Company's Loveridge Mine in Northern West Virginia, many things were taken into account. The advantages of slurry transportation in comparison to shuttle car, belt and rail transport have been given in previous publications." These include increased safety and the potential for increased productivity. In addition to these considerations, the slurry transport system installed in an existing mine provides an alternate or additional transportation mode to the one already existing. In the Loveridge Mine, this is rail haulage. Since the rail system is a single track system and the distances involved are several kilometers, it is not always possible to provide for haulage of loaded cars, return of empties, supplies and maintenance in an efficient manner. Slurry transport of the coal from the production faces to the preparation plant allows the rail system to fulfill the remaining transportation needs in an efficient manner. Consol initiated development work in slurry transportation in the early fifties leading to the construction and operation of the 174-km (108-mile) Cadiz-to-Cleveland pipeline which carried 1,200,000 metric t/y (1,300,000 t/y) from 1957 to 1963. Development work on coarse coal slurry transportation for underground mines started in 1970 with feasibility tests on coarse coal at Conoco's Ponca City, Oklahoma, slurry test facility. A series of tests were conducted in several locations as development work proceeded on the various subsystems, mechanical equipment and controls required to produce a working system to follow a continuous miner in a room-and-pillar type mine, then to combine the coal production from several mine faces for mainline transportation to the preparation plant. The face system, including the crusher-pump vehicle, hose hauler vehicle, slurry booster pump units, return water unit and 900 to 1,200 m (3,000 to 4,000 feet) of pipeline was brought together for the first time and tested as a system at Consol's Robinson Run Mine in northern West Virginia in 1975. A pipeline wear test at the Loveridge Mine where 300,000 tonnes (330,000 tons) of minus 127-mm (5-inch) run-of-mine coal were pumped from the raw coal bin into the preparation plant and a large scale test in Ponca City on a multiple feed sump where several slurry flows could be combined to provide surge capacity and a constant reclaim rate for mainline transportation completed the preliminary testing for a complete system. A decision was made to commit the system to a mine, and the south half of the Loveridge Mine was chosen. This site required the transporting of coal from one longwall and two continuous miner development faces. To prevent startup and unexpected problems in the slurry system from shutting down production of coal from the longwall face, the rail car loading point from the main section belt was chosen as the feed point for the slurry system and the design would allow for rapidly switching from one transport mode to the other. Scheduling of required development entry work and the developmental nature of the system led to a decision to install only one continuous miner face slurry transport system rather than two for this first system. The type of continuous miner in use at the mine for the foreseeable future was the Joy 2BT6 twin boring machine. This machine cuts a stable, arch-shaped entry and was therefore allowed to mine about 26 m (85 feet) of entry before roof bolting was re¬quired. Since peak production from this type of machine was not high, a smaller transport system could be used with it.

The Coal Preparation Division of the Pittsburgh Mining Technology Center conducted an investigation to assess and compare the pyritic sulfur reduction potential of coal-pyrite flotation and high-gradient magnetic separation (HGMS). Tests were run on three coals--an Upper Freeport bed coal, a Lower Kittanning bed coal, and a Pittsburgh bed coal--to establish grade- recovery relationships characteristic of both processes. Representative portions of each coal sample were crushed to two top sizes--nominal minus 35 mesh (0.5 mm) and nominal minus 100 mesh (0.149 mm)--and subjected to conventional froth flotation. The froth product was then beneficiated by coal-pyrite flotation or HCMS to provide a direct technical comparison of the two processes. At minus 35 mesh (0.5 mm), the size normally employed for coal flotation, both coal-pyrite flotation and HGMS effectively separated pyrite from coal, and the performance of the two processes was essentially the same. At a size of minus 100 mesh (0.149 mm) , however, the performance of HGMS exceeded that of coal- pyrite flotation. The separations effected by HGMS were more efficient at minus 100 mesh (0.149 mm) than at minus 35 mesh (0.5 mm). On the other hand, it was concluded that either the effectiveness of coal-pyrite flotation decreases with diminishing top size, or the process has not been optimized to treat material finer than 35 mesh (0.5 mm).

Historically, there have been many occurrences ofmine fires and explosions recorded in the United States and other countries that have demonstrated the existence of explosivemethane–airmixtures, herein referred to as explosive gas zones (EGZs). The risk of mine explosions can increase if the EGZs migrate out from the gob into the surrounding mine entries. Fluctuating barometric pressure is the common cause for EGZs outflowing or outgassing from the gob. Numerical analysis using a 3D computational fluid dynamics method was developed to fully understand the outgassing phenomenon. A number of simulations using various magnitudes and periods of barometric pressure changes indicated that the EGZ outgassing potentially occurs due to lags in pressure, which are strongly influenced by mine conditions and ventilation systems. An early warning system with a real-time pressure monitoring and the application of gob ventilation boreholes are recommended to detect and mitigate explosion hazards from gob outgassing.

Performance of mixing mechanisms in flotation cells is paramount, as inefficiencies can lead to financial losses through valuable minerals lost to tailings. Technological advancements have been implemented in flotation cells to optimize performance, but regardless of the efficacy of these developments, room for improvement may persist. Since its launch in 2005, FloatForce® has set a high industry standard for flotation mixing mechanisms. However, current mining trends call for sustainability-driven developments while improving metallurgical performance. Metso has recently introduced FloatForce®+, the latest iteration of FloatForce, developed through extensive laboratory, pilot, and industrial testing, aligning with mining sustainability trends. It has been designed to maximize recovery, reduce power draw and optimize manufacturing materials. This development stems from a redesigned rotor profile paired with a harmonizing stator configuration. This paper presents the development journey of Metso’s latest patented flotation mixing mechanism, an advancement to improve metallurgical performance at a high sustainable standard in flotation cells. The theory, CFD modeling and test work are discussed in detail.

INTRODUCTION The British mining industry comprises 228 nationalised coal mines producing some 113 million tonnes of deep mined coal and employing 242,600 persons of whom 185,200 work below ground. There are in addition 174 small privately-owned coal mines employing 1500 persons underground and producing 1.5 million tones of coal. In the non-coal sector there are 108 privately-owned mines producing some 20 different minerals, including tin, wolfram, fluorspar, potash, rock salt, china clay, ball clay and fireclay. There are currently 4,100 persons employed in these mines of whom 2,346 are employed below ground. One mine producing potash employs 870 persons of whom 465 are underground workers. Since 1975 all matters relating to the health and safety of persons at work and of the general public who are exposed to industrial hazards have come within the purview of the Health and Safety Executive (HSE). The Mines and Quarries Inspectorate (MQI) is a constituent part of that organisation and enforces legislation applicable to all mines and quarries. The National Radiological Protection Board (NRPB) is a statutory and independent body that was established in 1970 to advance knowledge and provide information and advice about radiological protection. The NRPB has an agency agreement with HSE to provide advice and services relating to radiological protection, including protection against exposure to radon decay products in mines. There are as yet no statutory requirements controlling exposure to this source of radiation in Britain. Investigations for the presence of radon in British coal mines were begun in the mid 195Os. The concentrations found were of low order, typically around 2 pCi/l (Duggan et al., 1968). This was not surprising since big coal mines are well ventilated to control methane and airborne dust and it is now generally accepted that radon and its decay products do not constitute a health hazard in British coal mines. At one or two small mines where igneous intrusions were in close proximity to the coal workings, slightly higher readings were recorded. The MQI then commissioned NRPB to make investigations at metalliferous mines and in these much higher readings were detected. It was later agreed that the NRPB should undertake a survey of all non-coal mines in Britain to establish the extent of the problem. This survey was conducted mainly in 1973, (Strong et al.,1975) and follow-up surveys at various mines have since been conducted by the MQI. These investigations revealed that radon readings could be high in mines producing tin, haematite and fluorspar. Some small mines working other minerals in the same localities also showed high readings. More recently owners have been making environmental measurements at mines where there was some cause for concern. LEGISLATION AND RADIATION STANDARDS As a result of the survey work for MQI the Board in 1975 recommended an occupational limit of 4 WLM in a year from radon decay products together with a programme of radiological supervision (Strong et al., 1975). The MQI accepted the recommendation and advised employers and employees that the 4 WLM limit should be adopted. The limit corresponds to a concentration of 0.3 WL, equivalent to 30 pCi/l of radon-222 in equilibrium with the decay products, and was in accordance with an earlier recommendation by the International Commission on Radiological Protection (ICRP, 1959). ICRP recommendations are usually endorsed in Britain. It was recognised that some years would probably be required to achieve general compliance, and this is proving to be the case. Some confusion has been caused in Britain by the conflict between successive Euratom Directives on this and other standards of safety. As relative newcomers to the Community, we were not affected by earlier Directives, but the situation is different now. In 1959, the Commission of the European Communities provisionally recommended a maximum permissible concentration for radon-222 and daughters of 3 x 10-7 µCi/cm3, which corresponds to 300 pCi/l (CEC, 1959). The number was affirmed in 1962 (CEC, 1962) and reaffirmed in 1966 (CEC, 1966) but without any specification of the state of equilibrium. In 1976, the CEC again recommended 300 pCi/l, but on this occasion, the daughters were assumed to be present in the same quantities as in unfiltered air (CEC, 1976): this implies 40 WLM a year in poorly-ventilated mines. The formula is repeated in the latest version of the Directive (CEC, 1980), but the value is now said to be an interim one because it might not ensure compliance with the appropriate dose limit. ICRP (1955) recommended 300 pCi/l in equilibrium with the decay products in 1955 and reduced this value to 30 pCi/l for unfiltered air in 1959 (ICRP, 1959), endorsed it for equilibrium conditions in 1976 (ICRP, 1976), and held to it until 1980: these values imply respectively 40 and 4 WLM a year. The recent recommendation of the ICRP (1980) that the annual limit of intake for the decay products should be 0.02 J in a year corresponds to 5 WLM, but there is a rider that reduction should be made for exposure to gamma rays and long-lived airborne radioactivity. Throughout all segments of the United States mining industry, 163 independent contractor employees were fatally injured between 1990 and 1997. Factors that contributed to these injuries were identified after reviewing descriptive information contained in Mine Safety and Health Administration fatality reports for each of the incidents. Thirty-three unique contributing factors were identified and multiple factors were noted for each fatality. Those most frequently discerned were "failure to follow `standard operating procedures"', (35% of the fatalities), "poor positioning of employee" (26% of the fatalities) and "failure to maintain equipment" (23% of the fatalities). Contributing factors were then associated with the accident and job classifications of the victims. Factors having the highest frequencies in the top five accident classifications where most fatalities occurred were as follows: powered haulage - "failure to maintain equipment"; machinery - "poor positioning of employee"; slip/fall of person - "inadequate fall protection" and "failure to wear safety line"; electrical - "equipment contact with power line"; and falling/rolling/sliding rock - "poor positioning of employee. " Among the job classifications in which most fatalities occurred, leading contributing factors were as follows: truck driver - "failure to follow standard operating procedures" and "poor positioning of employee"; laborer/utility man/pumper - "poor positioning of employee"; mechanic/ repairman and welder - "failure to follow standard operating procedures." Recommendations for utilizing contributing factor information as a basis for safety training are proposed. Training programs may be developed that inform independent contractor employees of factors which, in the past, have contributed to fatal injuries in their job classifications. Then, by combining this information with contributing factors identified in "related" accident classifications, a truly focused training intervention that addresses very specific workplace hazards may he developed.

As the 2009 president of the Society for Mining, Metallurgy, and Exploration, Inc., I am pleased to present the 2009 issue of Transactions of the Society for Mining, Metallurgy, and Exploration, Vol. 326. A significant portion of the contribution that we as a society make is the publication of professional and technical papers that contribute to the advancement of scientific knowledge and information transfer. Communication within the mining community is the key to a successful society. Through the information-transfer mechanisms of books, magazines, meetings and electronic media, the technology exchange that is fundamental to the existence of the Society is accomplished. This technology exchange continues a tradition that dates back to the 1871 meeting of 22 mining engineers in Wilkes-Barre, Pennsylvania, that resulted in the founding of AIME; those individuals envisioned that these transaction volumes “would form a most valuable and greatly needed addition to our professional literature.” This volume certainly satisfies that intent. In keeping with SME’s tradition of promoting technological developments while honoring the past, the 2009 Transactions represents an important milestone. It is the last Transactions to appear as a bound volume. Future volumes will appear as e-books only, with some of the Transactions-only papers appearing online prior to the ebook (exactly what form the new Transactions will take is still in development). The decision to go to an all-digital format for this longstanding publication was a difficult one, but one which we hope will better serve the mining community, especially our international members. This 2009 Transactions of SME, Vol. 326, contains a total of 73 technical papers (510 pages), including all 23 papers (136 pages) that were published in 2009 in the Technical Papers Section of Mining Engineering magazine (Vol. 61) and all 32 papers (232 pages) that were published in 2009 in Minerals & Metallurgical Processing (Vol. 26). In addition, this volume contains 18 papers (142 pages) that are being published for the first time in this issue. The papers presented in this volume were written by some of the most distinguished professionals in the industry worldwide. All 73 technical papers were peer-reviewed and approved. The process of review and publication requires much hard work by authors, reviewers and publications staff. As the 2009 president of SME and on behalf of our members, I would like to extend our gratitude to all whose efforts contributed to the publication of this volume.
