"When Dr. Joseph A. Holmes was named the first director of the U.S. Bureau of Mines in 1910, the challenge in front of the mining industry was urgent and profound. Just three years before the Bureau was formed, the United States’ coal industry saw a spike in fatalities, as 3,242 miners lost their lives, including 918 miners killed in 18 major disasters. Holmes, who is often called the Founding Father of Mine Safety, was charged with researching and finding new and safer methods to conduct mining activities as well as more efficient ways to conduct mine rescue to educate miners.The U.S. Bureau of Mines was dissolved in 1996, but the mission of the Holmes Mine Rescue Association remains intact, although the challenges have changed greatly.During the 102 years between its formation and the 2018 Joseph A. Holmes Safety Association & Holmes Mine Rescue Association Annual Meeting and Training Seminar in Denver, CO June 5-7, the mining industry has evolved to become one the safest industries in the United States, with an injury rate on par with school teachers and better than retail workers. The work of Holmes and the Bureau of Mines plotted a course for others to follow, and the fatality rate has plummeted to historic lows. However, those who work in the mining safety and health sector remain diligent to their cause and are slow to celebrate too much the accomplishments of the past century, focusing instead on the work that lies ahead.“The nature of mining accidents has changed and it is very difficult to make changes when you get down to the asymptotic part of the equation,” David Zatezalo, assistant secretary of the U.S. Mine Safety and Health Administration (MSHA) told Mining Engineering. He was the featured speaker at the Joseph A. Holmes Safety Association annual meeting’s banquet dinner and presented a thorough overview of the mission of MSHA and his goals as its top man.Nominated by President Donald Trump, Zatezalo was sworn in as the ninth assistant secretary for MSHA on Nov. 30, 2017 and brings with him a lifetime of experience in mining. During his presentation, he displayed the chart (Fig. 1) showing the dramatic improvement in safety that the industry has achieved during the past century, but he noted that, as the industry gets closer to its end goal of zero fatalities, it has become increasingly more difficult to cross the finish line."

Geochemical studies were under- taken in the Breckenridge mining district to test the effectiveness of soil and rock-chip samples in high- lighting known mineralization in the district, and in locating sites of possible porphyry-type mineralization, if such mineralization is present in the area. Soil sampling can be done more quickly and more cheaply, but it does not provide a direct site- specific sample of mineralized rock. In well-developed soils, supergene effects may mask and/or displace primary hypogene effects; and even in places where the soils are poorly- developed, as in the Breckenridge district, the masking effects may be felt. Soil anomalies, if present, likely will be of lower amplitude and more diffuse (widespread) than corresponding rock-chip anomalies. On the other hand, detailed rock-chip surveys are more time-consuming and expensive, even though they yield direct geologic information in addition to the chemical data. The goals of the combined soil and rock-chip programs were (1) to com- pare the data from the two; and (2) test the effectiveness in identifying significant chemical anomalies with respect to the geology of the district, as it is presently understood.

In Oct. 1981, Ranchers Exploration and Development Corp. began producing silver bullion from the Escalante mine and mill complex, located in southwestern Utah. Current in-place reserves are estimated at 2.42 Mt (2.67 million st) of ore, containing about 936 t (33 million oz) of silver at an average grade of 428.5 g/t (12.5 oz/st). The vein-type deposit is being mined underground through a decline that is currently 864 m (2,835 ft) in length, but will ultimately be at least 1.5 km (5,100 ft) long. The mine is completely mechanized with rubber-tired equipment, and is producing 680 t/d (750 stpd) of ore, five days a week, or 177 kt/a (195,000 stpy). Additional capacity is available. The cyanide leach mill and associated refinery are presently producing 4.35 t/month (140,000 oz/month) of silver bullion-an an¬nual rate of about 52.25 t (1.68 million oz). With minor modifications, the mill can produce up to about 70 t/a (2.25 million oz/year), and expansion is currently underway. History The Escalante property is located 68 km (42 miles) west of Cedar City, UT, on the west side of the Escalante Valley, where the vein forms a gentle ridge in an area of low, rolling hills. For years, the vein has been the object of considerable exploration, but never became more than a sporadic producer, primarily due to low silver prices. Heber Holt of Enterprise, UT, staked the original four claims in 1896; they were patented in 1910. During the next 48 years, limited exploration and mining activity was conducted on the property. Systematic evaluation and development did not, however, begin until March 1958, when Samuel S. Arentz, a prominent mining engineer and consultant, formed several joint ventures with mining and oil companies that conducted drilling, drifting, mining, and water drawdown tests on the property. However, it remained uneconomic, principally because of low silver prices and costs for pumping large flows of underground water. In 1975, Ranchers obtained an option on the property and in 1979 began a drilling program. As of May 1982, the company had drilled 70 core holes, bringing the number of exploratory holes drilled on the property to 138. Test mining began in 1979 as silver prices increased, and full mine development began in 1980. Mill construction began in Sept. 1980, and on Aug 29, 1981, the first ore was fed to the mill.

The first cement plant of TITAN Cement Co was founded in Elefsis in 1902. It was the first cement industrial facility to operate in the Balkans and in the Eastern Mediterranean region. Currently have four cement plants in Greece producing 5.4 million tons per annum, and 25 ready-mixed concrete sites producing 1.4 million m3 annually. It has a total of 20 quarries, 14 of which are producing materials used in the making of cement while the remaining six produce aggregates used in the production of ready-mixed concrete. All plants in Greece are accredited for ISO 14001. The restoration of quarries began in 1976 at the company?s limestone quarry of the Patras plant, and was soon extended to all quarries. At that time, a department responsible for all environmental issues was established. To date, over 700.000 shrubs and trees have been planted and maintained at quarries, plants and ready-mixed concrete facilities, while over 100,000 trees have been donated to municipalities, communities, customers and to the Greek state.

The Manapouri Power Station, commisioned in 1969, is located in Fiordland National Park in the South Island of New Zealand. It is the country’s largest hydro power scheme and can generate 700MW of electricity. The power station utilizes the hydraulic head of Lake Manapouri, using an underground power station and a 10 km-long tailrace tunnel was constructed by drill-and-blast to allow discharge to Doubtful Sound. Power generation fell short of expectations due to higher than expected friction in the tailrace system. To remedy this a second tailrace tunnel was constructed between June 1998and May 2002. It consisted of a 10 m diameter, 10 km long tunnel in predominately gneissic rock and was excavated using an Atlas Copco Robbins TBM. High water inflows, Owner/Contractor production incentives and a major claim settlement were handled during the 5 1/4 year project. This paper discusses TBM advance rates, utilization factors that increased in the second half of the project plus an innovative method for sealing Tunnel 1 to construct the bifurcation.

Belle Ayr Mine was established in 1972 as the first large surface coal mine in the Powder River Basin of Wyoming. During the mid-1980s, it was eventually out-distanced in productivity by newer mines, and management recognized that the mine must recapitalize its major mining equipment and enhance its workforce flexibility in order to be competitive in the deteriorating market situation. Therefore, state-of-the-art haulage and loading equipment was placed into service, and a 12-hour rotating shift and a technician-based workforce was implemented. Belle Ayr Mine is located 29 km (18 miles) southeast of Gillette, Wyoming, in Campbell County (Figure 1). The mine is a part of AMAX Coal West, Inc., and with its sister mine, Eagle Butte, located 8 km (5 miles) north of Gillette, is a major contributor to the overall coal production of AMAX Coal Industries, Inc., headquartered in Houston, Texas. This paper addresses mine planning, recapitalization initiatives, and innovative workforce changes which propelled this large, surface coal mine to the forefront of productivity in the ever tightening low sulphur coal market.

Engineers at the Freeman United Coal Min¬ing Company, located in West Frankfort, Illinois, have been using a computerized production planning and maintenance reporting system, which have been designed exclusively for underground coal mining. This paper will explain a dynamic graphical simulation and computerized time study system that has been used to design and evaluate underground production systems for optimizing productivity. Also with the aid of customized software, detailed records on changeable components of underground equipment, including the vendor's name, date installed, operating and maintenance hours, etc., have helped Management to select the best available components for their equipment. The MPASS System, Mine Performance Analysis Simulation System, developed through a cooperative agreement with Freeman United, Ketron Inc., and the U. S. Department of Energy utilizes new methods to analyze and design production systems in underground coal mines. The MPASS System is a menu-driven user oriented system of interactive computer programs, which form five major subsystems. The subsystems: 1. Simulation 2. Time Study 3. Mine Background Reports 4. INTRODUCTION Tunneling machines were introduced into civil con¬struction in the 1800s, but their practical applications were not realized fully until the 1950s. As of 1977, tunneling machines were being used on most tunneling projects of substantial size. As in civil construction, the mining applications of tunneling machines have included long drives such as tunneling for long ore-body develop¬ment drifts, long haulage tunnels, longwall access drifts, and long crosscut developments. Tunneling machines ranging from 1.75-m (5.7-ft) to more than I1.0-m (36-ft) diam are available from several manufacturers; machines for mining applications generally have ranged between 1.75 and 6.0-m (5.7¬and 20.0-ft) diam. Tunneling machines usually operate at or near horizontal grades and are supported by a rail-haulage backup system. However, declines and inclines have been made using alternative backup equip¬ment. Either circular or shaped tunnels may be bored, depending upon the particular equipment used. Using tunneling machines to bore openings through rock offers several advantages over the use of the drilling and blasting technique. These advantages include a rapid advance, continuous operation, uniformity of the opening size, smooth walls, and safe working conditions. Compared to drilling and blasting, boring reduces the disturbance of adjacent rock, thereby reducing the ground-support requirements and the exposure to water inflows. The smooth walls provide natural arching and present minimum resistance to the flow of air. A tunneling machine should be considered a spe¬cialized mining tool that is well-suited to certain mining applications. Properly applied and sustained, a tunnel¬ing machine contributes to realizing substantial econo¬mies in the development of underground mines. All costs herein are expressed in 1977 US dollars, unless otherwise noted. The mention of trade names and manufacturers is not intended to imply an endorse¬ment of any specific equipment. TYPES OF TUNNELING MACHINES As listed in Table 1, five principal types of tunneling machines were being manufactured in 1977. Each type of machine offers a special solution to a specific mining problem. Although each of these types of machines is described herein, most of this chapter is devoted to the full-face rotary rock-tunneling machine that has demon¬strated the greatest production in both civil construction and mining applications. Full-Face Rotary Rock-Tunneling Machines As shown in Fig. 1, a full-face rotary rock-tunneling machine consists of a rotating head that is fitted with rock-cutting tools and is forced into the tunnel face. This head is supported by a bearing on a structural support member that, in turn, is held in place by a hydraulically positioned wall-gripping mechanism. Both torque and thrust are reacted from the cutterhead, through the structural support and gripper mechanism, and to the tunnel wall. The rock cuttings fall to the invert at the tunnel face, and they are removed by means of cutterhead buckets or scoops that transfer the cuttings to a conveyor belt. After advancing the cutter¬head through a preset boring stroke, the tunneling machine is advanced by hydraulically pulling the gripper mechanism in from the tunnel walls, stroking forward, and resetting the gripper to the new forward position on the wall; the machine then is set for the next advance stroke. Full-face machines cut only circular openings. Ma¬chines with diameters as small as 1.8 m (6 ft) and as large as 6.1 m (20 ft) have been used in mining applica¬tions, excavating rock with unconfined compressive strengths in excess of 276 000 kPa (40,000 psi). Although special machines have been designed to turn on a radius of 30 m (100 ft) in both horizontal and vertical directions, the conveyors and backup sup¬port equipment generally limit tunneling machines to turns with a radius of more than 60 m (200 ft). The full-face rotary rock-tunneling machines have provided the fastest and most reliable production of any mining method or machine. These machines can be equipped to place any of a variety of ground-support systems, and they can be provided as "open" machines, as shown in Fig. 1, or as "shielded" machines, as shown in Fig. 2. The open machines have the advantage of allowing the ground to be supported as near to the tunnel face as possible. The shielded machines have been designed principally to allow placing precast con¬crete segments. The shielded machines completely pro¬tect the equipment and personnel at the heading, but they are economically limited to permanent long-life haulageways where full lining is required. Partial-Face Rotary Rock-Tunneling Machines As shown in Fig. 3, the partial-face rotary tunneling machine is equipped with a cutterhead that rotates slowly at about 0.21 rad/s (2.0 rpm). The single-disk cutterhead produces the tunnel section shown in Fig. 4 by means of an oscillating motion. These machines work on the undercutting principle, and they are avail¬able in spring-line dimensional increments of 0.1 m (0.32 ft) from 1.5 to 2.1 m (5.0 to 7.0 ft); the vertical dimensions are 2.4 to 3.2 m (8.0 to 10.5 ft), respec¬tively. Depending upon the tunnel width, the cutterhead disk may be fitted with from 8 to 16 drag-bit cutters. Penetration is achieved by means of hydraulic rams that force the tungsten-carbide bits into the rock mass. The operation is cyclic, with cutting occurring only on the upstroke. On the downstroke, the rotating cutter¬head draws the fallen muck from the invert into the panzer-type conveyor for rearward transfer. This feature allows time to either reposition ore cars or to exit the tunnel with muck trains while the machine is cutting, as shown in Fig. 5. Industrial minerals deposits present sampling and grade control problems not normally encountered in metal mining operations. No longer are industrial minerals simply crude ground mineral products but are in fact high purity chemical raw materials. Operators of industrial minerals mines and processing plants now must meet increasingly stringent specifications established by the consumer of the final products. Meeting these stringent specifications requires not only sophisticated processing techniques but equally important careful sampling and grade or quality control at the mine. A well-trained geological staff can assure an adequate supply of uniformily high quality feed to the processing plant. In the final analysis this results in significantly higher operating profits.

The wind tunnel has been used extensively for mine ventilation research and continues to be a valuable tool. The advent of the powerful personal computer has reduced the use of wind tunnels because several flow problems car be simulated and investigated quickly. This paper describes the recent design and construction of a wind tunnel which is now being used for mine ventilation aerodynamic research and teaching purposes. The wind tunnel has a square cross section measuring 900 by 900 mm and is 8485 mm long from the entrance to where it joins the fan. The tunnel was designed as an open circuit suction type and is attached to a 7.5 kW variable pitch axial flow fan capable of producing over 6 m3/s of airflow. The entire length of the wind tunnel can be used for flow investigations and all activities are visible since the sides are made of plexiglass. Results are presented from the test program to show that even without honeycombs and screens air flow symmetry is achieved.

There are many types of filters designed to separate a solid from a liquid, including plate and frame presses, pressure leaf filters, gravity filters and vacuum filters. This discussion will be limited to continuous vacuum filtration and, specifically, to continuous rotary vacuum disc filters, drum filters and horizontal belt filters. The use of vacuum filtration can be advantageous in that: 1. The feed slurry can be delivered to the filter either by gravity or by a simple, inexpensive low pressure pump that only has to overcome the resistance of the feed piping system. 2. The filter equipment does not have to withstand high pressures. Consequently, a wide range of materials of construction, including plastic and fiberglas reinforced polyester (FRP), can be used in the fabrication. 3. After the form time, the filter cake is exposed to the atmosphere and the cake is accessible for further operations such as mechanical or physical dewatering, washing, and sampling on a continuous basis. 4. The discharge of the dewatered solids is continuous for directly feeding to the next operation; for example, a dryer. 5. Various methods of discharge can be fitted to the filter to effect optimum discharge of the dewatered cake solids. The principal of operation of a vacuum filter, regardless of type, is basically the same. The filter incorporates a filter medium supported on a drainage system and arranged so that the mixture to be separated (feed slurry) can be introduced to one side of the filter medium on which the solids are trapped and through which the liquid (filtrate) flows due to the applied vacuum.

Introduction Significant increases in extraction ratio on the order of 6% to 8%, equivalent to as much as 142 t/m (48 st per ft) of panel, have occurred at the Big Island trona mine during the last five years with the help of a practical rock mechanics program. This increase in resource recovery also has contributed to improvements in productivity. Both conventional and continuous machine mining have been used to mine two flat-lying trona beds at depths of 250 m and 260 m (820 ft and 853 ft) in the Green River Formation in southwestern Wyoming. Conventional room-and-pillarmining originally was conducted in panels with extractions of 5 8% to 60%. The use of yield pillars has allowed the panel extraction in increase to 66%. Continuous machine mining was introduced, with a 64% extraction ratio using long, narrow pillars and wide rooms. Further improvements in resource recovery seem feasible in light of present mining experience. The rock mechanics program consisted of field instrumentation to determine the pillar and roof response hole locations. to mining and computer modeling to evaluate the stability of various layouts, which were then adopted for mining. Mining background Stauffer Chemical Co.'s Big Island mine and refinery are located 40 km (25 miles) northwest of Green River, WY. Underground mining operations supply trona, a sodium mineral, to the refinery to produce soda ash. The room-and-pillar method is used to mine two 3 to 3.6 m (10 to 12 ft) thick, flat-laying seams at depths of about 250 to 260 m (820 to 850 ft). Mine production in 1986 was 2.3 Mt (2.5 million st). Background Iron ore is the No. 1 metal mining industry in the U.S. with dollar value of $2.3 billion in 1984 (U.S.B.M Mineral Commodity Sunnnaries , 1985). However, during the past decade this nation's iron ore industry has been subjected to a major market depression and a correspondingly downward adjustment in output. The recent trend in the curtailment of iron ore production traces a slow-down of the country's steel industry. Both pig iron and steel production have decreased significantly over the past several years. These trends are shown in Figure 1 from data collected by the federal Bureau of Mines (U.S.B.M. Mineral Commodity Summaries, 1985; U.S.B.M. Mineral Industry Surveys 1986). The industry is presently operating at less than 60% of its annual capacity. The domestic steel industry has been forced by reduced profits or losses to close facilities, curtail operations and restructure the financial status of several corporations. Companies have been sold or are trying to sell selected properties to improve their financial circumstances. Even with such actions, many of the steel companies are in very serious straits, including the seventh largest steel company, LTV, which has filed for bankruptcy. Many of the major steel companies have financial interests in iron ore mining and thus their adverse economic conditions directly reflect those operations. Several iron ore producers have been shut down including Reserve Mining Company in May, 1986 and Butler Taconite in June, 1985. The latter recently filed for bankruptcy under Chapter 11. A1 so in mid-1986, U.S. Steel Corporation, owner of the Minntac mine and iron ore processing plant, underwent corporate restructuring. The effect on their Minnesota plant is not known at this time. An excellent summary of the interrelationship of the iron ore companies and the steel producers has been provided by Skillings (1986), and an analysis of the iron ore situation was given by Robert F. Anderson, CEO of M. A. Hanna Company, in his keynote address at the 1986 University of Minnesota Mining Symposium (Anderson, 1986). Steel imports to the United States decreased slightly in 1985 because of import restrictions, but the long-term import situation remains dim and uncertain. As shown in Figure 2, the imports averaged about 25% in 1985, and the preliminary indications are that this figure could be as high as 30% when the final 1986 information is collected by the U.S. Bureau of Mines. At best, the industry can only hope for imports to stabilize at a constant level in the near future. Although the tonnage is small, the quantity of U.S. export steel has fallen over 50%. With many other materials replacing steel , the projected demand through 1990 is expected to increase only about 1% per year. Consequently, 1986 U.S. iron ore production will probably be 15% lower than in 1985. The 41 mil lion tons of iron ore production expected in 1986 represents only 53% of the industrial capacity, which is about 74.5 mil lion tons. Over 95% of this iron ore is in the form of beneficiated pellets. Today there is not an iron ore producer west of the Mississippi River, nor is there any production in the South. The Birmingham (Alabama) iron ore industry has been shut down since 1971. The western producers ceased operations in the early 1980's. Only the taconite operations in Minnesota and the plants in the Upper Peninsula of Michigan remain as our major domestic iron ore source. The economic situation for both the iron ore producers and the steel industry can be described as confused and in turmoil. Such a condition directly impacts the iron ore processing plants' operations and plans for the future. Plant Practice At present the nation's eight major operating iron ore mines, listed below, are concentrated in northern Minnesota (Mesabi Range) and the Upper Peninsula of Michigan (Marquette Range). In the period 1982-1992, world trade has increased in real terms about 65% (Anon., 1992b). The nominal value of this trade in the early 1990s is approximately $5 trillion. In 1970, mineral commodities constituted approximately 20% of the value of world trade. By 1980, despite rising petroleum prices, mineral commodities made up about 18% of the total value of world trade, with crude oil amounting to 16% and other minerals the remaining 2%. By 1987, the major worldwide economic slowdown and falling oil prices meant that minerals constituted only about 10% of the total value of world trade, or about $500 billion in 1987 (Kimbell and Zajac, 1988). The importance of minerals in world trade since 1987 has remained at about these levels. Approximately 40% of world mineral commodity production is traded internationally. Some economists predict that minerals and other commodities are likely to continue to decline in relative importance in terms of the value of world economic output, as well as world trade, into the foreseeable future (Thurow, 1992). This projected decline in commodity demand may also be related to much slower global economic growth over the next decade (Alperovitz, 1992). Industrial minerals and rocks constituted roughly 2.5% of total mineral commodity exports in the late 1980s, but their relative importance as a portion of mineral commodity exports nearly doubled in the decade 1977-1987 (Kimbell and Zajac, 1988). Industrial minerals constitute the most rapidly growing share of total mineral commodity exports. In addition, price declines among industrial minerals were uncommon in the decade 1977-1987 (Kimbell and Zajac, 1988). While prices of sulfur and most of the fertilizer minerals have seen some erosion and slow growth in markets in the period 1987-1992, other industrial minerals have increased in both value and share of the export market (USBM, 1992). The place of industrial minerals in world trade becomes even more significant when one looks at shipments of chemical fertilizers and basic chemicals that are counted as manufactured products rather than industrial mineral commodities. The United States is the major world producer of industrial mineral commodities and products made from these nonmetallic minerals and rocks. The former Soviet Union, now The Commonwealth of Independent States (CIS), and China have very significant deposits of industrial minerals. Japan exports significant quantities of cement. The US, CIS, and Morocco export large amounts of phosphate rock. Canada and Germany export substantial amounts of potash. Sulfur from Poland, industrial diamonds from South Africa, Zaire, and Brazil, and fluorspar from Mexico are all important exports.

For all the attention given to mining activities in Central and South America, Mexico, Indonesia and Papua New Guinea, it is sometimes easy to forget the activities taking place in one of North America's most important and largely untapped regions -Alaska. The Alaskan minerals industry employed about 3,406 people during 1995. That is a 23% increase from - - 1994. It also contributed about $719.8 million to the state's economy during 1995, a 10% increase. During the past few years, Alaska has seen significant increases in exploration and mining activity. Fairbanks Mining, an arm of Cyprus Amax, began producing gold during 1996 at its Fort Knox Mine, as did Nevada Goldfields' Nixon Fork gold mine. Kennecott's Greens Creek lead-silver-gold mine also reopened and USMX began operations at its Illinois Creek gold mine. In addition, several impressive exploration projects were under way in Alaska. Placer Dome reported good results at Donlin Creek project in southwest Alaska. Likewise, Newmont Exploration is getting serious about its True North gold project. And Nerox is nearing production at its Jonesville coal project in Alaska's interior. In November, about 400 miners gathered in Anchor- age for the Alaska Miners Convention. Throughout the conference and exhibition, attendees listened to updates on these precious- and base-metal and coal exploration and mining operations. They also learned that the state wants more miners to head north and help develop its natural resources. He is the chairman of the Senate Energy and Natural Resources Committee. During his luncheon address, Murkowski said it appears that it is economically feasible to extend the Alaska Railroad to northwest Alaska, taking advantage of the large coal and hard-rock mineral tracts in the Ambler mining district. He also renewed his push for building a major smelter in Alaska to process ore that would come from the Ambler mining district. Murkowski believes that Alaska's economy can be expanded by extending the railroad from Nenana to the Point Lay coal fields to open coal development. Along the way, the railroad would pass through the Ambler district, helping to develop the hard-rock deposits. An outgrowth of this additional mineral development, Murkowski said, could be a new smelter in Alaska. It could use power from the Tyee hydroelectric facility near Wrangell or from the Makushin geothermal project near Unalaska. Either site, he added, offers access to plentiful, relatively low-cost power, at tidewater to facilitate ore deliveries to the Pacific Rim. The Congressional Research Service has determined that there are enough mineral deposits and coal beds to make it possible to finance the extended railroad, he said. The 1,200-km (744- mile) rail extension would cost about $4.2 billion. The North Slope of Alaska contains up to 3.6 Tt (4 trillion st) of coal, Murkowski said, that contains less than 0.25% of sulfur and less than 4% moisture. It aver- ages 12,300 Btu in heat content. "That is a tremendous resource," he said. The Ambler mining district, meanwhile, has at least eight identifiable hard-rock deposits, Murkowski continued. These contain zinc, lead, copper, nickel, chromium, tin, barite, gold and silver. "All eight of these can generate ore concentrates at rates similar to Red Dog's cur- rent 725 ktla (800,000 stpy)" of zinc, he said.

Many organizations are realizing that continual improvement in health and safety relies on addressing the potential for various forms of human error. Of these, rule violations are often the most frequent form associated with accidents. These occur when an individual (or individuals) deliberately contravenes established and known safety rules. They have been associated with up to 90% of the accidents and other incidents in some industries, and yet they rarely receive the systematic attention they deserve in the promotion of safety. This paper presents underlying motives that cause violations. These can vary significantly from mine to mine and from job to job. Managers need some measure of the attitudes of the violators to determine the underlying motives behind the violations if they are to determine effective strategies for promoting safe behavior. Questionnaires form the starting point to address attitudes and the motives behind rule violations. The choice of the most suitable questionnaire, how- ever, will vary from organization to organization. Guidelines on the choice of questionnaire are offered.

1. INTRODUCTION In this section, precious metals are defined as gold, silver, and the platinum group metals comprised of platinum, palladium, iridium, osmium, and rhodium. If the first three years of the 1970's are the criteria, gold, silver, and platinum seem destined for a bright future. Gold advanced in price from the historic figure of $35 established in 1934 to over $100 per oz (1973). For the five years 1966 to 1970, silver consumption exceeded production by about 50%. The silver price appears to be definitely on the uptrend, and a price of over $3 per oz seems probable. On this basis (at 1964 costs) the United States has an estimated 5 billion oz of unmined silver. The fresh burst of interest in platinum with a surge of renewed price strength (end of 1972) is due to the activity in the automobile emission control area, where platinum re¬mains the front runner in the catalyst area. Some international financial experts believe that governments will not rush into reform of the monetary system, and "Special Drawing Rights" (SDR's) will gain increased standing as international mone¬tary reserve assets. Gold will continue to be reviewed as the ultimate store of value and will remain the global monetary base. Devaluation of the U.S. dollar caused by imbalance of import payments over ex¬ports is the basic reason that the price of gold will likely remain above the $90 per oz figure for the balance of the 1970's. Gold and silver usually occur intimately associated, and recovery methods are often closely related; however, they were assigned sepa¬rate chapters, because the major production of each metal is from ore deposits classified as either gold or silver. For example, the gold ore occurrences in South Africa, the world's leading gold producer, have a ratio of gold to silver of approximately 10 to 1. The ores at the Homestake Mine, Lead, S.D., the largest U.S. gold producer, are predominantly gold with a ratio to silver of approximately 5 to 1. Silver in each example is a byproduct. By this same classification, the largest U.S. silver producers are the silver ores of the Sunshine, Galena, and Lucky Friday mines in the Coeur d'Alene district, Idaho. However, the largest single-mine silver producer in the Free World is the Kidd Creek Mine at Timmins, Ont., where the occurrence is with sulfides of copper, lead, and zinc. Thus, the major gold production of the world is derived from those ores classified as gold ores; whereas, less than one-half of the silver production is from silver ores, and only a small percentage of the silver production is from gold ores. 2. GOLD Gold, the noblest of metals, has been mined for thousands of years. Its chemical symbol, Au, is derived from the Latin "aurum," meaning "shining dawn." A precious substance because it is one of the rarest metals, it has unique physical properties: it does not tarnish; it exceeds all other metals or alloys in malleability and ductility; it is a good conductor of electricity and is one of the most nonreactive metals. Its greatest strength is its indestructibility. It is the only metal mined at depths exceeding 10,000 ft. Since the early 1800s, its chief role has been as a medium of exchange, providing the basis for interna¬tional trade. In the past 15 years, however, gold has emerged as a useful industrial commodity. This role has been fortified by the estab¬lishment of a two-tier pricing system which maintains monetary gold prices at an established U.S. Government price, with industrial gold being priced higher by the free-market system. Occurrences and Production Native gold furnishes most of the world's production, since gold forms stable natural compounds with few other elements. Neverthe¬less, the metal is widespread with traces of it found in seawater and in the ashes of plants and animal material. Next to the native metal, the tellurides have been the most important gold-bearing minerals; these include calaverite, krennerite, slyvanite, hessite, and nagyagite. Native gold is invariably alloyed with more or less silver, but it is exceptional to find any other metal in the alloy. It is found in veins associated with quartz, calcite, alunite, other gangue minerals, and with various sulfides. Sulfide minerals in order of frequent associa¬tion are: pyrite, galena, chalcopyrite, sphalerite, arsenopyrite, tetrahe¬drite, and pyrrhotite. Small amounts of metallic gold are commonly disseminated through such sulfides. When considerable amounts of silver occur in gold, the mineral electrum is formed. In a few deposits, gold and mercury form natural amalgam. Native mercury is formed by weathering from cinnabar and other mercury minerals. Among the minerals sometimes mistaken for gold, pyrite, marcasite, and especially chalcopyrite may be men¬tioned. The flakes of biotite and other micas, especially when partially oxidized, also may imitate it closely. World gold reserves never have been estimated carefully. Esti¬mates indicate that present world reserves approximate I billion oz, of which about 60% occurs in South Africa and 20% in the USSR. World gold output is dominated by South Africa which produces two-thirds of the world's production from reserves that are adequate to maintain existing output levels. South African gold output comes from approximately 50 large underground operations that mine and mill 70 to 80 million tons of ore annually to recover 30 to 32 million oz of gold. The USSR is the second largest gold producer, followed by Canada and the United States (see Table 1). Gold production in the United States and Canada rose to high levels in the 1930's, following the increase in price from $20.67 to $35 per oz and with an abundance of good labor due to depression in other industries. Production of over 10 million oz in 1940 was behind only that of South Africa. World War II caused a precipitous drop in gold output due to the imposition of War Production Board Limitation Order L-208 issued in October 1942, which caused over 9,000 gold mines in the United States to shut down, most of which never reopened. The U.S. Treasury, which suspended purchases of newly mined gold in 1968, had been the sole buyer of U.S. produced gold since 1934. This created the two-tier pricing system, and in March 1972 the President signed into law a bill authorizing a $3 increase in the official price of gold to $38 per oz. In February 1973 the President ordered a second devaluation of the dollar, amounting to about 10% which raised the official price to $42.22 per oz. Bentonite is one of the main players in the stability of drilling mud. Its particle size and particle size distribution significantly affect drilling mud’s rheological properties. Although the Specifications for Drilling Fluids Materials report on Oil Companies Material Association (OCMA)-grade bentonite states that the API recommended powder having 2.5 percent mass fraction as the maximum value of residue of diameter greater than 75 μm, there is no other information on the specific size distribution for API bentonite. It was found that API bentonite has a specific size distribution with specific mixing weights. It is important to investigate why that mixture represents the optimum properties of drilling mud and how each individual size fraction has a specific function in the mixture. To clarify the effect of individual size as well as the collective effect of the size distribution, API bentonite was classified into different size fractions, with each tested separately for controlling properties such as filtration loss, plastic viscosity, gel strength, cake thickness and packing density.

INTRODUCTION The Washington Metropolitan Area Transit Authority is responsible for the planned 166 kilometer (103 mile) Metro rapid rail transit system in Washington, D.C. Some 117 kilometers (72.8 miles) of this system is in operation and contains 30.6 route-kilometers (19 miles) of rock tunneling. To complete the planned system an additional 1.6 kilometers (1.0 miles) of single track tunnels in rock are planned and additional contracts will consist of 4 kilometers (2.5 miles) of twin tunnel, resulting in 8 additional linear kilometers (5 miles) of single track tunnel contracts yet to be offered in soft ground. Three recent contracts in soft ground included the New Austrian Tunneling Method (NATM), along with conventional tunneling, as a bid option. Section E5a, Fort Totten Station and Tunnels, was the only one of the three to be constructed using the NATM option. This is the first application of the New Austrian Tunneling Method in soft ground in the United States. This paper will concentrate on the Fort Totten Station chamber NATM design, construction, and the instrumentation readings and evaluations which lead to an engineering back analysis which allowed a refinement of the bid design, resulting in a faster, more efficient construction. Design of the Fort Totten Station and Lines was done by Sheladia Associates. The design team included Greiner Engineering and Jenny Engineering with Mayreder-Consult of Austria, who was responsible for NATM design and NATM construction consultation.

Introduction Borne out of the need for reductions in capital expenditure and increased production, several new and innovative carbon systems have been developed for small precious metal heap leach operations throughout the United States. Perhaps the most important aspect of these systems is the carbon column. Though diverse in design, these columns can be categorized into three basic types. The first is known as the gravity flow type or Cortez type. As the name indicates, solution flows via gravity, throughout 4 to 6 columns which are arranged in a normally stepped con- figuration which is conventionally upflow in design. The second type, known as the vertical column, is an arrangement whereby flow is established through pressurization in an upward manner, throughout 4 to 6 columns in a vertically stacked configuration. This arrangement is typically intended for use with small portable heap leach operations and employs a semi-continuous mode thus preventing gravity from occurring. The third type and newest development in carbon columns is an arrangement whereby the solution flows throughout 4 to 6 columns placed in a side by side configuration and pressurized by a single feed pump (Figure 1). A unique aspect to the third type, is that the adsorb and desorb cycles are accomplished in the same column, due to a fixed carbon charge. An in-depth analysis is warranted due to numerous advantages of this arrangement, and is therefore the subject of this paper. Advantages and Variations One advantage to the third type of carbon column is the fact that initial capital expenditures and operating man- power requirements are reduced. Another advantage is that the system can be made fully portable up to a 0.0315 m3/s (500 gpm) capacity. Systems of this nature are operational at several sites. Above this flow rate, the system's portability is one which can be maintained, but disadvantages (i.e. larger diameter carbon column and greater weights) far outweigh the advantages. There are several variations in each of these systems which allow for unique operation. For example, carbon columns made highly portable, can be moved from unitized heap leach operations to central stripping facilities. Although this method has been used in Colorado, transportation of these units has been known to be quite cumbersome. If distances exceed 5 km (3.2 miles) then special front-end loaders are required. This limits heap locations to within 5 km (3.2 miles) of the central processing facility. Another example transports the carbon charge from the heap leach locations to a centralized processing facility; this method is quite effective. For the past 5 years one mine has employed this technique of transporting carbon in 907 kg (1 ton lots), in DOT containers, towed by company pick-ups. This operation uses a vertical column with a 907 kg (1 ton) carbon charge. A third example uses static carbon columns with fixed carbon charges. Carbon Adsorption System The carbon adsorption system (Figure 1) is fed by a single preg solution feed pump. This feed pump pushes the preg solution through all four to six fixed charged carbon columns, which are on line. The sequencing of the carbon columns is achieved using valves. Expansion of the carbon charge is dependent upon the carbon selected. The carbon typically used is a 6 x 12 mesh size, with a nominal flow rate of 0.00135 - 0.00138 m 3/s (22-25 gpm) per square foot of carbon, based on a 40% bed expansion. With 8 x 16 mesh size,
